Abstract

一种新型复方莪术油制剂,主治外阴炎、阴道炎、宫颈糜烂、宫颈炎等妇科疾病。采用中西药结合的方式,以莪术油、硝酸益康唑、冰片等为主要成分,配合以其他辅料,可以制成栓剂、胶囊栓或者凝胶剂。本发明与传统制剂相比,制剂稳定,疗效高,无气味,对上述妇科炎症的治愈率达到90%以上。本发明还公开了该新型复方莪术油制剂的制备方法。

复方莪术油制剂及其制备方法

本发明属于一种用于治疗阴道炎、宫颈糜烂等症的药物,具体地说是由中药提取物加入化学药物组成的。本发明还公开了该药物的制备方法。
背景技术:
妇女阴道炎、宫颈糜烂等症是常见病、多发病,给患者带来诸多痛苦,严重影响广大妇女的身体健康,目前较常用的治疗方法和使用的药物之一,是用莪术油制成的复方制剂,如栓剂、泡沫剂等。中国发明专利《保妇康栓》(专利申请号94103219.1)公开了一种莪术油复方制剂,这种复方制剂对此类疾病虽收到一定效果,但由于其有效成分单一,疗效受到限制。另外,莪术油本身具有易挥发、有臭味的性质,使其在存放过程中由于挥发或氧化分解而使疗效减失,同时其难闻的臭味也给患者带来不便。
发明内容:
本发明的目的在于克服现有技术的不足之处,提供一种疗效广泛,见效快,既治标又治本,长期存放药效不减失且无难闻气味的莪术油复方制剂。
本发明的另一目的是提供该复方莪术油制剂的制备方法。
本发明提供的莪术油复方制剂该的药物组成为:
莪术油                                    1.05~6.30L
硝酸益康唑                                0.25~1.50KG
冰片                                      0.015~0.09KG
制成10000个应用单位(下同)。
本发明提供的莪术油复方制剂可以制成以下各种不同的剂型,这些不同的剂型需添加不同的辅料。
一、栓剂,除上述药物成分外还包括以下辅料:
吐温-80                                   0.21~0.82KG
二甲基亚砜                                0.2~0.8KG
硬脂酸聚烃氧(40)酯                        6.65~19.94KG
聚乙二醇-1500                             5.32~15.95KG
聚乙二醇-6000                             1.33~3.99KG
本莪术油复方制剂(栓剂)的制备方法包括以下步骤:
(1)、将冰片加入到莪术油及吐温-80中,搅拌使其混合均匀;
(2)、将硝酸益康唑加入到二甲基亚砜中,搅拌使混合均匀;
(3)、将硬脂酸聚烃氧(40)酯、聚乙二醇-1500、聚乙二醇-6000置化料罐中,水浴75-85℃使其融化;
(4)、将(1)步骤所得物和(2)步骤所得物加入到(3)中,混合均匀过筛;
(5)、将上述药液控制温度在45-55℃左右,制成需要的剂型。
二、胶囊栓剂,由前述药物成分和辅料组成药液,外面包覆胶囊壳。
其中辅料包括:

本莪术油复方制剂(胶囊栓剂)的制备方法为:
1、药液的制备
(1)称取大豆油及药用凡士林,水浴加热至35-45℃,加入尼泊金乙酯,搅拌使其溶解;
(2)称取冰片加入莪术油中,搅拌使其溶解;
(3)称取硝酸益康唑、二甲亚砜及溶有冰片的莪术油加入(1)制备的基质中,搅拌均匀。
2、囊壳胶液的配制
称取明胶,加水,55-65℃水浴使其溶解,加入甘油、柠檬黄、药用钛白粉,搅拌使其混合均匀,并经真空脱气使胶液透明无气泡。将内容物药液和囊壳胶液分别装入机器压丸成型,干燥后包装。
该新型栓剂药物分散于基质中以胶囊外壳包裹。
三、凝胶剂,其中莪术油为β-环糊精包合物。其辅料组成为:
卡波姆                              0.75~2.25KG
丙二醇                              1.5~4.5KG
蒸馏水                              8.62~25.5KG
本莪术油复方制剂(凝胶剂)的制备方法为:
1、称取卡波姆、丙二醇于容器内混合调成,加入适量的蒸馏水浸泡,搅拌使卡波姆完全溶解,以氨试液调PH值7-9备用。
2、取莪术油β-环糊精包合物、硝酸益康唑、冰片,加蒸馏水溶解,加入到1所得的基质中,搅拌均匀灌装即可。
3、莪术油β-环糊精包合物的制备:
称取一定量的β-环糊精,加适量的水,加热溶解,冷却至室温,加入一定量的莪术油,搅拌数小时后,置于冰箱中冷却1-2天,分离,得固体物在常温下干燥,即为稳定的莪术油β-环糊精包合物。其中莪术油β-环糊精包合物组成的重量比为:β-环糊精∶莪术油=2.5~50∶1~5。
本发明选用硝酸益康唑、莪术油与冰片制成复方制剂,是中西药物结合的优秀一例。其中硝酸益康唑具有抗菌谱广的特点,对皮肤丝状菌、念珠菌、酵母菌、黑色丝状菌、曲霉属、青霉属、放线菌等有抑制和杀灭作用,对革兰氏球菌和杆菌有较强的活性,其机制是作用于致病菌的细胞膜,改变其通透性,阻断营养摄取导致死亡。莪术油具有行气活血、消积止痛、活血化瘀、去腐生肌,增强肌体免疫力之功效,且可预防子宫颈癌的发生。对细菌、霉菌、滴虫、病毒等病源微生物具有较好的杀灭作用,并能修复病变组织,促进创面愈合。冰片具有消肿、止痒、止痛、并具有协同消炎的作用,使其疗效更强。
本发明是在多年研究和临床实践的基础上,证明中药成分的毒副作用小,远期疗效明显;而西药成分起效快,但以治标为主,因此将其配伍应用,既可减轻毒副作用更能发挥远近期疗效,在此思想指导之下,经筛选、优化、完善组方及其有效剂量范围而成。
本发明针对莪术油各复方制剂分别采用囊壳包覆、以β-环糊精包合等先进工艺,克服了莪术油易挥发、有臭味的缺陷,避免在贮存过程中由于莪术油含量减少造成的药效降低,使患者不再有臭味难耐的烦恼。使制剂更加稳定,有利于疗效更好的发挥。
体外药学研究证明了硝酸益康唑和莪术油复方制剂对妇女常见阴道感染真菌具有良好的抑制作用,益康唑和莪术油也具有协同作用。
下面是本发明提供的复方莪术油制剂的体外药物学研究结果:

试验菌株:
选用近期(2001.12-2002.10)临床分离妇女常见阴道感染真菌423株,试验前所有菌株处于对数生长期,以备接种。全部菌株均经中国医科大学第二临床学院微生物室采用法国梅里埃API20C系统鉴定,其中包括白色念株菌205株菌,热带念株菌55株,光滑念珠菌25株,季也蒙念珠菌22株,近平滑念珠菌15株,皮状丝胞酵母14株,克柔念珠菌8株,无名丝胞酵母4株,乳酒假丝酵母4株,头状丝胞酵母3株,葡萄牙假丝酵母3株,红色毛癣菌28株,石膏样癣菌14株,羊毛样小孢子菌9株,黑曲霉15株,同时以白色念珠菌ATCC90028国际标准菌株为质控菌。
实验方法:
采用微量肉汤稀释法测定。

从上表可以看出,益康唑和莪术油对白色念珠菌的抗菌活性强于制霉菌素、酮康唑和氟康唑,而益康唑对白色念珠菌抗菌活性与克霉唑和咪康唑基本相当。
益康唑对152株非白色念珠菌也有较强的体外抗菌作用。其对热带念株菌、光滑念珠菌、季也蒙念珠菌、近平滑念珠菌、皮状丝胞酵母、克柔念珠菌等非白色念珠菌的体外抗菌活性均高于制霉菌素和高于或近于克霉唑。
二.硝酸益康唑与莪术油的协同效应

本栓剂制备方法是:
取冰片加入到莪术油及吐温-80中,搅拌均匀得药液(1);取硝酸益康唑加入到二甲基亚砜中,搅拌均匀得药液(2);将硬脂酸聚烃氧(40)酯、聚乙二醇-1500、聚乙二醇-6000置化料罐中,水浴80℃使其融化搅拌均匀得基质(3);将药液(1)和(2)加入到(3)中,混合均匀过100目筛。药液冷却,控制温度在50℃左右,注模制成栓剂10000枚,用量为一日两枚,早晚各一枚。

本新型栓剂的制备方法是:
1、药液的制备:
(1)称取大豆油及药用凡士林,水浴加热至40℃,加入尼泊金乙酯,搅拌使其溶解。
(2)称取冰片加入莪术油中,搅拌使其溶解。
(3)称取硝酸益康唑、二甲亚砜及溶有冰片的莪术油加入基质中,搅拌均匀。
2、囊壳胶液的配制:
称取明胶,加水,60℃水浴使其溶解,加入甘油、柠檬黄、药用钛白粉,搅拌使其混合均匀,并经真空脱气使胶液透明无气泡。
3、成型:
将内容物药液和囊壳胶液分别装入机器压丸成型,制成10000枚,干燥后包装。用量为一日两枚,早晚各一枚。

本外用凝胶剂的制备方法是:
1、称取卡波姆、丙二醇于容器内混合调成,加入适量的蒸馏水浸泡,搅拌使卡波姆完全溶解,以氨试液调PH值8备用。
2、取莪术油β-环糊精包合物、硝酸益康唑、冰片加蒸馏水溶解,加入到1所得的基质中,搅拌均匀灌装即可。
其中莪术油β-环糊精包合物组成的重量比为:β-环糊精∶莪术油=10∶4。
上述莪术油β-环糊精包合物的制备方法为:按比例称取β-环糊精,加适量的水,加热溶解,冷却至室温,按比例加入莪术油,搅拌3小时后,置于冰箱中冷却2天,分离,得固体物在常温下干燥,即为稳定的莪术油β-环糊精包合物。

(3)将硬脂酸聚烃氧(40)酯、聚乙二醇-1500、聚乙二醇-6000置化料罐中,水浴75~85℃使其融化;
(4)将(1)步骤所得物和(2)步骤所得物加入到(3)中,混合均匀过筛;
(5)将上述药液温度控制在45~55℃左右,制成需要的剂型。
7、一种权利要求3所述的复方莪术油制剂的制备方法,其特征在于该方法包括以下步骤:
(1)药液的制备:

(2)制备莪术油β-环糊精包合物:按比例称取β-环糊精,加适量的水,加热溶解,冷却至室温,按比例加入莪术油,搅拌数小时后,置于冰箱中冷却1~2天,分离,得固体物在常温下干燥,即得稳定的莪术油β-环糊精包合物;
(3)取莪术油β-环糊精包合物、硝酸异康唑和冰片加蒸馏水溶解,加入到(1)所得的基质中,搅拌均匀灌装即可。
