Abstract

Description

分子透入口腔粘膜的能力可认为是与分子大小,脂质溶解性和离子化作用有关。小分子例如小于约300道尔顿的似可迅速地穿过粘膜。然而当分子大小增加时,渗透性迅速下降。而且,脂溶性化合物比非脂溶性分子更易透过粘膜。关于这一点,分子的相对渗透性似乎是与它们的分配系数有关。分子的电离程度看分子在膜表面的pKa和pH而定,它对分子渗透性的影响也较大。当分子未电离或电荷中和时出现最大吸收,吸收随电离程度增加而降低。因此,填充了大分子的药物对通过口腔粘膜吸收存在着巨大挑战。

这些以及其他目的是通过提供一个用于将药物经粘膜给药到口腔的系统来完成的,该系统包含一个由亲水性聚合物组成的粘附剂层,它有一个适于与口腔的第一层组织接触且当其湿润时能附着在其上的表面,和一个与相邻的含药层接触并附着其上的相对表面,该含药层含有有效量的药物和任意的有效量的渗透增强剂,当粘附剂层接触并附着在第一层组织上时,含药层适于与口腔的粘膜组织接触并与其有药物传递关系。优选的渗透增强剂包括干扰细胞外膜的化合物,溶剂,甾族洗涤剂,胆汁盐,螯合剂,表面活性剂,非表面活性剂,脂肪酸,及其混合物,其中胆汁盐增强剂更为优选。药物优选肽类药物,如促胰岛素肽,降钙素,胰岛素,去氨加压素,甲状旁腺激素,和糊精,以及它们的前体,类似物,和片段,其中这些前体、类似物和片段具有药物学活性。亲水性聚合物优选包含至少下列成员中一种:羟丙基纤维素,羟丙基甲基纤维素,羟乙基纤维素,乙基纤维素,羧甲基纤维素,葡聚糖,瓜耳胶,聚乙烯吡咯烷酮,果胶,淀粉,明胶,酪蛋白,丙烯酸聚合物,丙烯酸酯的聚合物,丙烯酸共聚物,乙烯聚合物,乙烯共聚物,乙烯醇的聚合物,烷氧基聚合物,聚环氧乙烷聚合物,聚醚,以及它们的混合物。聚环氧乙烷和聚丙烯酸的混合物是尤为优选的。另外粘附剂层可含有选自下列成员中的一种或多种物质:填料,片剂赋形剂,润滑剂,调味剂,染料等,含药层还可含有选自下列成员中的一种或多种物质:片剂赋形剂,填料,调味剂,掩味剂,染料,稳定剂,酶抑制剂,润滑剂等。优选该系统为双层片剂,其中粘附剂层与龈接触并附着其上,而含药层与颊粘膜有药物转运关系。
将药物经粘膜给药到口腔的方法包含在口腔的第一层组织上应用一种系统,该系统包含一个由亲水性聚合物组成的粘附剂层,它有一个适于与口腔的第一层组织接触且当其湿润时附着在其上的表面,和一个与相邻的含药层接触并附着其上的相对表面,该含药层含有有效量的药物和任意的有效量的渗透增强剂,当粘附剂层接触并附着在第一层组织上时,含药层适于与口腔的粘膜组织接触并与其有药物传递关系。

本文中所用的“化学增强剂”,“渗透增强剂”等包括所有能增加渗透物、药物或其他分子穿过粘膜的流量的增强剂且仅受官能度的限制。换句话说,所有干扰细胞膜的化合物,溶剂,甾族洗涤剂,胆汁盐,螯合剂,表面活性剂,非表面活性剂,脂肪酸,以及任何其他化学增强剂都包括在内。
药物或分析物穿过粘膜的流量可通过改变抵抗性(扩散系数)或改变驱动力(扩散梯度)而增加。流量也可通过使用所谓的渗透或化学增强剂而增加。
渗透增强剂由两种基本类型的成分组成,即:干扰细胞膜的化合物和溶剂或含有干扰细胞膜化合物和溶剂的二元系统。如上所述,其他类型的渗透增强剂也是已知的,如甾族洗涤剂,胆汁盐,螯合剂,表面活性剂,非表面活性剂和脂肪酸。
干扰细胞膜化合物在局部药物制剂中的用途和在通过皮肤或粘膜的药物释放中的功能是本领域技术人员熟知的。这些化合物被认为是通过干扰角质层细胞膜的脂质结构而帮助皮肤渗透的。欧洲专利申请43,738,1982年6月13日公开,其中列出了这样的化合物,该文结合在此参考。相信任何干扰细胞膜化合物都可用于本发明目的。典型的干扰细胞膜化合物可用下式表示:

合适的溶剂包括水;二醇如丙二醇和甘油;一元醇如乙醇,丙醇,和高级醇;DMSO;二甲基甲酰胺;N,N-二甲基乙酰胺;2-吡咯烷酮;N-(2-羟乙基)吡咯烷酮;N-甲基吡咯烷酮;1-十二烷基氮杂环庚-2-酮和其他n-取代烷基-氮杂环烷基-2-酮(阿综)等。
美国专利4,537,776,Cooper,1985年8月27日授权,其中就某些二元系统增强渗透的用途的现有技术和背景资料作了详细总结描述。因为其已公开得很完全,因此该文中所用的资料和术语作为参考结合在此。
与此相似,上文提到的欧洲专利申请43,738指出:利用挑选出的二醇作为溶剂与宽范畴的干扰细胞膜化合物一起来释放亲脂的药理学活性化合物。因为欧洲专利申请43,738已详细公开了干扰细胞膜化合物和二醇,因此其也作为参考结合在此。
用于增强metaclopramide渗透的二元系统公开于英国专利申请GB2,153,223A,1985年8月21日公开,其由C8-32脂族一元羧酸(如果是C18-32则是不饱和的和/或分支的)的一价醇酯或C6-24脂族一元醇(如果是C14-24则是不饱和的和/或分支的)和N-环化合物如2-吡咯烷酮,N-甲基吡咯烷酮等组成。
由二甘醇单乙基醚或单甲基醚与丙二醇一月桂酸酯和月桂酸甲酯组成的增强剂的结合物公开于美国专利4,973,468中,用于增强类固醇如孕激素类和雌激素类的经皮释放。美国专利4,820,720公开了由单月桂酸甘油酯和乙醇组成的、用于经皮释放药物的二元增强剂。美国专利5,006,342列出了许多用于经皮给药的、由脂肪酸酯或C2-C4链烷二醇的脂肪醇醚组成的增强剂,其中酯/醚的脂肪酸/醇部分为约8-22个碳原子。美国专利4,863,970显示了局部应用的渗透增强组合物,其含有包含在渗透增强赋形剂中的活性渗透物,其中含有特定量的一种或多种干扰细胞膜化合物如油酸,油醇,和油酸的甘油酯;C2或C3链烷醇以及惰性稀释剂如水。
其他不一定与二元系统相联系的渗透增强剂包括DMSO或DMSO的水溶液,如Herschler,美国专利3,551,554,Herschler,美国专利3,711,602,和Herschler,美国专利3,711,606中指出的,和如Cooper,美国专利4,557,943中提到的阿综(n-取代烷基-氮杂环烷基-2-酮)。
本文中所用的“胆汁盐”是指甾族洗涤剂,它们是胆烷酸的天然或合成盐,例如胆酸和脱氧胆酸的盐或这种盐的结合物,也包括未电离的酸形式。胆汁酸与甘氨酸或牛磺酸的轭合物的盐是优选的,尤为优选的是与牛磺酸的盐。具有相同物理性能且也起渗透增强剂作用的胆汁盐类似物也包括在这一定义内。“NaTC”是胆汁盐,牛磺胆酸钠。“CHAPS”是胆汁盐类似物,3-[3-胆酰氨基丙基)二甲氨基]-1-硫酸丙酯,内盐。
本文中所用的“经粘膜”或类似术语是指渗透物进入并通过粘膜而达到药物的有效治疗血液浓度或深组织浓度的通道。
本文中所用的术语“渗透剂”,“药物”或“药理学活性剂”或其他任何类似的术语是指任何化学或生物学物质或化合物,包括适于利用本领域中已知的方法和/或本发明提出的方法而经粘膜给药的肽,其能产生预期的生物学或药理学作用,可以包括但不限于:(1)对生物体有预防作用和预防不期望的生物学作用如预防感染,(2)减轻病情,例如减轻由疾病引起的疼痛或炎症,和/或(3)减轻,减少或完全消除生物体的疾病。作用可以是局部的,如提供局部麻醉作用,或者可以是全身的。本发明并没有提出新的渗透物或新类型的活性剂,反而对现有技术中存在的或者可能后来被确定为活性剂的并适于利用本发明进行释放的活性剂或渗透物的释放方式进行了限定。这样的物质包括正常释放到体内的众多类别的化合物,包括通过身体表面和膜,包括经皮肤释放的。概括地说,包括但不限于:抗感染药如抗生素和抗病毒剂;止痛药和止痛结合物;减食欲药;抗蠕虫药;抗关节炎药;止喘剂;抗惊厥药;抗抑郁药;抗糖尿病药;止泻药;抗组胺药;抗炎药;抗偏头痛制剂;止恶心药;抗肿瘤药;帕金森神经机能障碍;止痒药;精神抑制药;退热药;镇痉药;抗胆碱能药;拟交感神经药;黄嘌呤衍生物;心血管制剂包括钾和钙通道阻滞剂,β受体阻滞剂,α受体阻滞剂,和抗心率失常药;抗高血压药;利尿药和制尿药;血管舒张药包括全身性冠状、外周和大脑的;中枢神经系统兴奋剂;血管收缩药;咳嗽和感冒制剂包括减充血剂;激素如雌二醇和其他甾类化合物,包括肾上腺皮质类固醇;安眠药;免疫抑制剂;肌肉松弛剂;抗副交感神经药;精神兴奋剂;镇静剂和安定药。利用本发明的方法,电离和未电离的药物均可释放,也可释放高或低分子量的药物。
本文中所用的“有效量”是指无毒但足以提供预期的局部或系统作用并在任何医学治疗时能以适当的优点/危险比实现这一点的药物或药理学活性剂的量。本文中所用的渗透增强剂的有效量是指为了提供预期的粘膜渗透性的提高和,相应地,预期的渗透深度,给药速率和药物释放量而挑选出的渗透增强剂的用量。
本文中所用的“粘附剂”,“粘附聚合物”,“粘膜粘附剂”或这种类似的术语是指亲水性聚合物,天然或合成的,通过亲水性设计使其即可以是水溶性的也可以是可膨胀的,并且是与增强剂和药物相适应的。这样的粘附剂的功能是将双层片剂剂型附着到口腔的粘膜组织如龈上。这些粘附剂对药物的活性没有副作用。这样的粘附剂包括羟丙基纤维素,羟丙基甲基纤维素,羟乙基纤维素,乙基纤维素,羧甲基纤维素,葡聚糖,瓜尔胶,聚乙烯吡咯烷酮,果胶,淀粉,明胶,酪蛋白,丙烯酸聚合物,丙烯酸酯的聚合物,丙烯酸共聚物,乙烯聚合物,乙烯共聚物,乙烯醇的聚合物,烷氧基聚合物,聚环氧乙烷聚合物,聚醚,以及它们的混合物等等。
根据图1,它显示了按照本发明的通过口腔粘膜组织给药优选肽类药物的剂型,它使得抑制药物通过粘膜组织的有效经膜流量的药物/粘附剂的相互作用被大大减小或消除了。该剂型为双层片剂10,包含一个粘附剂层12和一个活性或含药层14。粘附剂层12配制成可附着到口腔的粘膜表面,以使活性层14与粘膜组织有药物转运关系,由此药物通过粘膜渗透并吸收到个体的血流中。在图1说明的实施方案中,片剂10置于口腔中以使粘附剂层12附着在龈表面16上,而活性层14与颊粘膜18有药物传递关系。
双层片剂是用合适的压片机通过传统的双层片剂压制技术制成的。根据图1,双层片剂10由粘附剂层12和活性或含药层14组成,为了便于应用,它们可以是不同的颜色以将各层区别开。将含药的非粘附剂层14标记出来有助于患者用药并防止其他口腔组织附带粘附到片剂上。粘附剂层12可以通过将成分干燥混合后压制成片而制备,或者将该成分混合物进行湿法制粒然后按照可接受的制药技术进行压片。一般说来,合适的是:将粘附剂聚合物和任何助剂如填充剂,片剂赋形剂,润滑剂,调味剂,染料等混合,然后压制该混合物。
首先将药物与渗透增强剂以及任何其他助剂如片剂赋形剂,染料,调味剂,掩味剂,稳定剂,酶抑制剂,润滑剂等均匀混合制备成含药或活性层14。可以用干法混合或用常规的湿法制粒及筛制技术后再进行干燥。无论怎样,都再将混合后的含药层成分置于已部分压缩的粘附剂上,再将这两层压在一起。本领域普通技术人员将意识到:首先制备含药层,然后再制备粘附剂层,这样也可制成速溶片剂。
本发明组合物的含药层和粘膜接触的表面积优选制成大小约为0.05cm2-10cm2,优选约0.07cm2-约5cm2,约0.18cm2-5cm2是最佳的。一般含药或活性层的厚度为约0.1-3mm,优选约0.5-2mm。
下述实施例阐述了按照本发明的双层片剂的制备方法。实施例1
用下述方法制备双层片剂。用70重量份的聚环氧乙烷(Polyox301N;Union Carbide),20重量份聚丙烯酸(Carbopol 934P;B.F.Goodrich),和10重量份可压缩的木糖醇/羧甲基纤维素填充剂(Xylitab 200;Xyrofin)制备粘附层。将这些成分在缸中滚动混合3分钟。然后将该混合物转移到蒸发皿中并用无水乙醇快速湿法制粒,制成半面团样稠度的软材。立即并迅速地将该软材压过14目(1.4mm筛孔)不锈钢筛,所得湿颗粒附着其上。用多孔铝箔将筛子盖上,在30℃下将该湿颗粒干燥过夜。将干燥后的颗粒从筛子上除下来,然后通过20目(0.85mm筛孔)筛以进一步减小颗粒的尺寸。用乳钵和研杵将未通过20目筛的颗粒简单研磨使成微细粉末,然后再通过20目筛。然后将所得颗粒置于混合缸中,加入0.25重量份的硬脂酸和0.06重量份的薄荷调味剂(Universal Flavors)并混合成颗粒。各成分的最终重量百分数是69.78%的聚环氧乙烷,9.97%可压缩的木糖醇/羧甲基纤维素填充剂,19.94%聚丙烯酸,0.25%硬脂酸,和0.06%薄荷调味剂。将50mg该混合物置于0.375英寸直径的塑模上并用0.25公吨压力在Carver Press Model C上预压3秒钟形成粘附层。
用49.39重量份的甘露糖醇,34.33重量份的羟丙基纤维素(Klucel LF;Aqualon,Wilmington,Delware)和1.00重量份的牛磺胆酸钠(Aldrich,Milwaukee,Wisconsin)制备活性层,将它们在缸中滚动混合3分钟。然后将该混合物转移到蒸发皿中并用无水乙醇快速湿法制粒,制成半面团样稠度的软材。立即并迅速地将该软材压过14目不锈钢筛,所得湿颗粒附着其上。用多孔铝箔将筛子盖上,在30℃下干燥该湿颗粒。然后将干燥后的颗粒顺序通过20目和60目(0.25mm筛孔)筛以进一步减小颗粒的尺寸。用乳钵和研杵将未通过筛子的颗粒简单研磨使成微细粉末,然后再通过筛子。过筛后的颗粒称重,然后通过几何稀释法将0.91重量份的GLP-1(7-36)酰胺和0.06重量份的FD&C黄色#6HT铝色淀染料顺序与该干燥颗粒混合。然后将该染色后的颗粒置于混合缸中并与0.25重量份的硬脂酸镁(润滑剂)和0.06重量份的薄荷调味剂一起滚动混合3分钟。将50mg这种物质的样品置于部分压缩的粘附层上,然后将这两层用1.0吨压力压3秒钟,得到适于经颊释放的双层片剂。
通过此过程制得了齿龈片剂,其中活性层含有0.91%重量的GLP-1(7-36)酰胺,15%重量的NaTC,和84.09%重量的填充剂,润滑剂,着色剂,助剂或调味剂。实施例2
采用实施例1的方法,只是改变了活性层成分的用量,活性层含有65.30%重量的甘露糖醇,34.33%重量的羟丙基纤维素,0.25%硬脂酸镁,0.06%FD&C黄色#6HT铝色淀染料,和0.06%薄荷调味剂。该方法制得了用于人志愿者体内双盲研究的安慰剂片剂。实施例3
用实施例1的方法制备颊部片剂,其中活性层所含原料量相同,只是通过干法混合而不是用湿法制粒来制备。实施例4
该实施例描述了在随机分组条件下的双盲的、安慰剂对照的、交叉比较的治疗结果。挑选8位健康的志愿者进行体内研究,分别接受按照实施例1和2制备的的含有400μg GLP-1(7-36)酰胺的含药双层片剂或安慰剂后,测定他们体内的血糖,胰岛素,胰高血糖素和GLP-1(7-36)酰胺浓度。志愿者的选定标准是有正常葡萄糖耐量,体重在22<BMI<26,知道并同意参与该研究,年龄为20-60岁的。排除的标准是有葡萄糖耐量损伤(2小时葡萄糖耐量试验;OGTT),胃肠症状,正在进行治疗或正患病,急性感染,异常试验室变量(血红蛋白,血细胞比容,白细胞,肌酸酐,胆红素,钙,钾,钠,碱性磷酸酶,γ-GT,SGOT,SGPT,胆甾醇,和甘油三酯),以及血压大于185mmHg收缩压和/或90mmHg舒张压的。
按照受试者在研究中的登记顺序给他们分配受试号。分配给每个受试者的号确定其接受的治疗顺序。接受治疗顺序1者先用药物,然后用安慰剂进行治疗,接受治疗顺序2者先用安慰剂,再用药物进行治疗。
受试者在治疗前一晚通知其禁食。在临床上给予这些受试者含药的双层片剂或安慰剂。时间为0时,将双层片剂应用于受试者的齿龈,活性层与唇的组织或面颊的内皮接触,受试者处于睡眠姿势。4.5小时后除去该双层片剂。这一期间不允许进食直到4.5小时后给予标准餐,任何时候都不许吃东西。标准餐含有550千卡热量,来自蛋白质、脂肪和碳水化合物的能量分别为28%、22%和50%。试验持续到双层片剂应用后8小时,9小时后受试者离开医院。在临床中受试者由护士给药并始终处于观测下。在试验期间干扰或破坏受试者良好状态的任何症状或感觉都仔细记录。每次试验前测定禁食状态下的标准安全变量。时常监测血糖,低血糖时(<2.5mmol/1)给予葡萄糖输液。如果有任何受试者出现低血糖症状,为了安全原因将测定血液外葡萄糖。每次试验间有1-4天的清除期。该研究在首批志愿者登记后6周内完成。
在用药前10分钟,以及用药后5、10、15、20、25、30、40、50、60、75、90、120、150、180、210、240和270分钟,6小时和8小时时采集血样进行药物动力学分析。将样品冷冻直到利用双倍抗体放射免疫测定法(RIA)测定其GLP-1(7-36)酰胺含量。利用HPLC测定双层片剂样品的肽含量。利用梯形规则通过曲线下面积(AUC)计算平均血糖、胰岛素、胰高血糖素和GLP-1(7-36)酰胺浓度。以肽的血浆浓度为基准,计算GLP-1(7-36)酰胺的最大浓度(Cmax),半衰期(T),和最大血液浓度对应的时间(Tmax)。检验结果是否为正态分布。对正态分布的样品进行双尾t检验,非正态分布的数据使用Wilcoxon等级和测验。所有统计检验使用0.05显著水平。
图2显示了给予安慰剂(虚线)或药物(实线)的受试者的血糖测定结果。安慰剂组的血糖水平从双层片剂应用前10分钟到应用后270分钟都保持相对恒定。在相同的时间期限内,接受GLP-1(7-36)酰胺滴剂的受试者的血糖水平在用药20分钟后低于安慰剂组,约50分钟达到最低水平,约为3mmol/L,约90分钟恢复到正常水平。进食后两组的血糖水平是难区分开的。这些数据显示:用按照本发明的双层片剂将GLP-1(7-36)酰胺经颊给药使得与安慰剂对照组相比血糖水平显著降低。
图3显示了给予安慰剂(○)或药物(●)的受试者的血浆胰岛素测定结果。在双层片剂给药前10分钟到给药后270分钟的期间内,安慰剂组的血浆胰岛素水平保持相对恒定或轻微下降。给药后10分钟接受GLP-1组的血浆胰岛素水平迅速从正常水平升高,到给药后15分钟升高到正常水平的约3倍。GLP-1(7-36)酰胺给药后约20分钟达到血浆胰岛素水平峰值,约50分钟又迅速下降到正常水平。其他时间,药物组的胰岛素水平与安慰剂组的胰岛素水平一致。因此,GLP-1(7-36)酰胺经颊给药使得血浆胰岛素浓度迅速升高,接着迅速下降,这都发生在药物给药后的1小时内。
图4显示了给予安慰剂(○)或药物(●)的受试者的血浆胰高血糖素测定结果。从双层片剂给药前10分钟直到其给药后270分钟,安慰剂组的血浆胰高血糖素水平缓慢而平稳地下降。药物组的血浆胰高血糖素水平从0时开始急剧下降,直到约30分钟后显著低于安慰剂组,然后该浓度又急剧升高到显著高于安慰剂组,给药后约60-75分钟达到峰值。从此峰值开始,血浆胰高血糖素水平不断下降直到药物给药后约150分钟开始与安慰剂组的水平一致。这些数据表明:GLP-1(7-36)酰胺经颊给药后使血浆胰高血糖素水平迅速下降到低于安慰剂组,然后又升高到高于正常水平,在给药后约150分钟又再次恢复到正常水平。
图5显示了给予安慰剂(虚线)或药物(实线)的受试者的血浆GLP-1(7-36)酰胺的测定结果。对于安慰剂组,从药物给药前10分钟开始直到给药后270分钟,GLP-1(7-36)酰胺在血浆中的量相当恒定地保持在非常低的水平。进食后,血浆GLP-1(7-36)酰胺浓度升高,然后又不断下降。对于药物组,在给药后5分钟内开始血浆GLP-1(7-36)酰胺浓度急剧升高,给药后约30分钟达到峰值。然后该GLP-1(7-36)酰胺浓度迅速下降直到给药后约90分钟,之后更缓慢地下降,从约150分钟开始下降到与安慰剂组相一致。进食后,GLP-1(7-36)酰胺浓度与安慰剂组大致相同。这些结果表明:GLP-1(7-36)酰胺经颊给药使得其迅速通过颊粘膜吸收到血流中,升高的GLP-1(7-36)酰胺浓度在血液中保持到给药后约150分钟。
综上所述,这些数据表明:用本发明的双层片剂将GLP-1(7-36)酰胺经颊给药使得其迅速吸收到血流中,从而导致血浆胰岛素量急剧升高并相应降低了血液中葡萄糖的量。另外,血糖水平没有导致低血糖症。实施例5
在该实施例中,GLP-1(7-36)酰胺经颊给药后的相对生物利用度与GLP-1皮下给药后的相对生物利用度进行对比。这是与公开数据进行对比的。能提供静脉内输液数据的对空腹个体的两项研究是可得到的:参见D.M.Nathan等人,胰高血糖素样肽-1(7-37)对糖尿病和非糖尿病者的促胰岛素作用,15糖尿病护理270(1992);C.φrskov等人,胰高血糖素样肽-17-36酰胺和胰高血糖素样肽-17-37对健康者的生物作用和代谢率是不能区分的,42糖尿病658(1993)。这两种研究提供了约15ml/分钟/kg的消除率。
一项通过M.A.Nauck的将GLP-1皮下给药于空腹者的研究说明了在药物动力学中与剂量的变化(图6)。这可能是由于生物利用度与剂量的变化,清除率与剂量的变化,或二者作用的结合。以任何比率对所有结果的回归分析表明清除率为约40ml/分钟/kg,这与皮下给药相对于静脉给药来说38%的生物利用度相符合。
总结于图5中的数据利用AUC分析以提供对经颊给药与皮下给药对比的相对生物利用度的估计。这些数据见表1。

基本上按照实施例1制备双层片剂,只是活性层是通过将206mg羟丙基纤维素(Klucel EF;Aqualon),241.4mg甘露糖醇,90mgNaTC,和60mg人胰岛素进行干法混和而制备的。向该混和物中加入800μl无水乙醇,将该混和物湿法混和得到面团样稠度的湿颗粒。将该湿颗粒通过14目筛并于25℃下在通风炉中干燥过夜。干燥后的预制颗粒通过20目筛,然后与1.5mg硬脂酸镁,0.36mg薄荷调味剂和0.72mgColorcon FD&C黄色#6HT铝色淀染料一起置于玻璃管形瓶中,再次干燥混和。将50mg这样的物质置于部分压缩的粘附剂层上,然后用1吨的压力将这两层压缩3秒钟得到适于经颊释放的双层片剂。该双层片剂的活性层表面积为约0.71cm2,其中活性层含有140U(5mg)人胰岛素(10%重量),7.5mg NaTC(15%重量),17.7mg羟丙基纤维素(34.33%重量),20.1mg甘露糖醇(40.24%重量),0.25%重量的硬脂酸镁,0.06%薄荷调味剂,和0.12%重量的FD&C黄色#6HT铝色淀染料。实施例7

利用狗的颊粘膜模型来论证肽X的经颊转运。选择狗作为动物模型是因为狗的颊组织的结构在组织学上与人组织类似。C.Ebert等人,双氯灭痛在狗动物模型上的经颊吸收,控释技术310-21(P.Lee,W.Good编,美国化学会专题论丛,No.348,美国化学学会,Washington,D.C.,1987)。啮齿动物的颊组织往往会角化,但狗却与人一样显示出没有角化层的较好形成血管的颊组织。
用按照实施例13-15制备的双层片剂进行体内试验以测定他们在将肽X系统释放到狗体内时的有效性。使用前1个月将重30-35kg的杂种狗调整到所需状态。用乙酰丙嗪使这些狗轻度镇静,但在每次试验期间仍是醒着的。将片剂贴在齿龈上,活性层接触上唇的颊粘膜并与其有药物传递关系。使用尼龙喷嘴来防止动物舔舐该片剂。在该模型中,仍维持正常的分泌功能且不需要冲洗。将导管插入隐静脉以进行静脉血采样。将血样采集到柠檬酸“VACUTAINER”管中并立即在冰上冷却15分钟,然后以3400rpm离心10分钟。收集所得血浆上清液并储存于-70℃下加盖的聚丙烯管中直到用于分析肽X的含量。
肽X的这些经粘膜释放的试验结果概括于表2中。

这些结果表明:用本发明的双层片剂剂型将肽X经粘膜释放到了血流中。肽X的平均表观生物利用度与皮下注射相比约为8.74%。最大血液浓度对应的时间在约30-60分钟范围内,应用1片药平均为约40分钟,应用两片药平均为约50分钟。用含有甘草酸铵的制剂进行的试验显示达到最大浓度的时间较长,约60-100分钟,平均约85分钟。对于装填成两片的1%药物来说,平均最大血液浓度(Cmax)为约1.5ng/ml,对于装填成1片的2%药物来说,约为1.3ng/ml。图7显示:肽X经颊释放(○;1.64mg肽)和皮下注射(●;0.1mg肽)后的血浆肽浓度曲线非常相似。实施例17
与实施例16的方法相同,只是用鲑鱼降钙素取代肽X,一些试验中改变了片剂的大小和数目。结果概括于表3中。

Claims (44)
1.一种用于将药物经粘膜给药到口腔的系统,它包含一个由亲水性聚合物组成的粘附层,该粘附层有一个适于与口腔的第一层组织接触并当其湿润时附着其上的表面,和一个与相邻的含药层接触并附着其上的相反表面,该含药层包含有效量的药物和任意的有效量的渗透增强剂,当所述粘附层接触并附着在所述第一层组织上时,所述的含药层适于与口腔的粘膜组织接触并与其有药物传递关系。
2.按照权利要求1的系统,其中所述渗透增强剂选自下列成员:干扰细胞膜化合物,溶剂,甾族洗涤剂,胆汁盐,螯合剂,表面活性剂,非表面活性剂,脂肪酸,及其混合物。
3.按照权利要求2的系统,其中所述渗透增强剂是甾族洗涤剂。
4.按照权利要求3的系统,其中所述渗透增强剂是胆汁盐。
5.按照权利要求4的系统,其中所述药物是肽。
6.按照权利要求5的系统,其中所述亲水性聚合物包含至少一种选自下列成员的物质:羟丙基纤维素,羟丙甲基纤维素,羟乙基纤维素,乙基纤维素,羧甲基纤维素,葡聚糖,瓜尔胶,聚乙烯吡咯烷酮,果胶,淀粉,明胶,酪蛋白,丙烯酸聚合物,丙烯酸酯的聚合物,丙烯酸共聚物,乙烯聚合物,乙烯共聚物,乙烯醇的聚合物,烷氧基聚合物,聚环氧乙烷聚合物,聚醚,及其混合物。
7.按照权利要求6的系统,采用双层片剂形式,其中所述粘附层还含有一种或多种选自下列成员的物质:填充剂,片剂赋形剂,润滑剂,调味剂,和染料,其中所述含药层还含有一种或多种选自下列成员的物质:片剂赋形剂,填充剂,调味剂,掩味剂,染料,稳定剂,酶抑制剂,和润滑剂。
8.按照权利要求7的系统,其中所述胆汁盐增强剂是胆汁酸与牛磺酸的轭合物的盐。
9.按照权利要求8的系统,其中所述亲水性聚合物包含聚环氧乙烷和聚丙烯酸的混合物。
10.按照权利要求9的系统,其中所述肽是选自下列成员中的一种:促胰岛素肽及其前体,类似物,和片段。
