本发明属生物制剂技术领域。本发明公开了一种肝癌诊断、监护及肝硬化监护试剂盒。该试剂盒是一种体外诊断试剂,用来测定人血清中α-L-岩藻糖苷酶(AFU)活性,对原发性肝癌的诊断、监护以及肝硬化的监护具有广泛的临床意义。本发明提供了制备方法。

Description

肝癌诊断、监护及肝硬化监护试剂盒
技术领域
本发明属生物制剂技术领域,具体涉及一种肝癌诊断、监护及肝硬化监护试剂盒。
背景技术
血清中α-L-岩藻糖苷酶(AFU)测定用于肝癌诊断,已有近二十年历史(早期用于岩藻糖苷贮积症诊断,八十年代起用于肝癌诊断),但测定试剂中的关键试剂——底物,一直采用对硝基苯-α-L-岩藻吡喃糖苷(PNPF)。PNPF制成测定溶液后,化学性质十分稳定,保存和使用十分方便,但其最大缺陷在于反应产物PNP的解离常数pKa值(7.0)远高于AFU酶反应的最适pH值(约5),在pH5时不能显色。因此,市场上用PNPF作底物制成的试剂盒,存在着以下无法克服的缺点:
1.由于产物PNP在反应过程中不显色,不能采用酶分析中最合理的连续监测法,只能采用两点法,即在反应一定时间后加入终止液,终止酶反应并使反应产物显色,进行比色测定;
2.由于终止液加入前后,反应液的成份、各成份的浓度和溶液pH等,都会发生显著改变,因此每例测定必须加做样品空白,既增加操作程序,而且必然引入新的误差;
3.反应时间长。经典的以PNP为底物的两点法,反应时间长达一小时,发展到今天的自动化方法,尚需300秒。对酶分析来说,有失测定反应初速度的理想条件。
发明内容
本发明所要解决的技术问题在于:
1.改变PNPF两点法不能采用连续监测法的状况,研制以2-氯对硝基苯-α-L-岩藻吡喃糖苷(CNPF)为底物的连续监测法试剂盒;
2.重点解决CNPF在液体状态下稳定性差的问题,研制稳定的液体型AFU(CNPF)连续监测法试剂盒,显著提高方法的实用性和技术水准,广泛适应各大小实验室的需要;
3.通过优选,使酶反应条件更臻合理。
本发明提供了一种肝癌诊断、监护及肝硬化监护试剂盒。本发明的试剂盒是由试剂1和试剂2组成的,其中试剂1为(浓度以毫摩尔浓度(mmol/L)或重量/体积百分比(W/V)表示):
柠檬酸三钠             55.5-300mmol/L
或Na2HPO4          74.8-300mmol/L
CAPSO                  0-300mmol/L
NaAc                  200-300mmol/L
NaN3                  0-0.1%(W/V)
NaH2PO4              0-300mmol/L
试剂2为(浓度以毫摩尔浓度(mmol/L)或重量/体积百分比(W/V)表示):
柠檬酸                 10.6-300mmol/L
HAc                   12.6-300mmol/L
HCl                   0-300mmol/L
苯甲酸                 0-12mmol/L
NaN3                  0-0.1%(W/V)
CNPF                   0.1-2%(W/V)
本发明的另一所要解决的技术问题是提供了上述肝癌诊断、监护及肝硬化监护试剂盒的研制方法,该方法包括下列步骤:
1.用不同种类、不同浓度、不同pH的缓冲系统,测定同份样品中AFU活性,初选出相对满意的缓冲系统;实验研究中各物质浓度范围如下:
柠檬酸三钠  0-300mmol/L        柠檬酸  0-300mmol/L
Na2HPO4  0-300mmol/L        HAc   0-300mmol/L
NaAc       0-300mmol/L        HCl   0-300mmol/L
3-环己氨2-羟-1-丙烷磺酸钠(CAPSO)0-300mmol/L
苯甲酸      0-12mmol/L         NaH2PO4 0-300mmol/L
NaN3       0-0.1%(W/V)        CNPF      0-2%(W/V)
2.对初选出的缓冲系统,逐一完成缓冲液浓度和pH对酶活性的影响试验,以及各系统中底物CNPF的稳定性实验,进一步缩小缓冲系统的可选范围;
3.择优选用能提高CNPF稳定性的缓冲系统,实验求出该系统中酶反应产物CNP的摩尔吸光度(ε),并求出该系统中CNPF浓度对AFU活性的影响曲线,在此基础上确定下一步实验的CNPF的浓度范围;制备在此范围内不同CNPF浓度的缓冲系统,经过对血清AFU活性测定,用Linewear-Burk双倒数作图法,求出在该系统中AFU对CNPF的米氏常数Km值;结合CNPF浓度对AFU活性的影响曲线,确定试剂中CNPF的配制浓度;
4.经过上述选择,配制成选定的缓冲系统和底物浓度的试剂,经37℃加热试验,并通过与其他糖苷类试剂的比较,进一步确定CNPF在该系统中的稳定性;
5.作重复性试验和线性试验;
本发明的试剂盒既可作稳定的双试剂(试剂1和试剂2)直接使用,也可将试剂1与试剂2按4∶1的比例混合,作稳定的单试剂直接使用。通常,本发明的试剂盒采用连续监测法,必要时也可加用终止液,改为两点比色法。检测参数为:

如改用单试剂操作,只需将试剂1与试剂2按规定比例混合均匀,即得稳定的液体单试剂。
如所用仪器不含有405nm波长,可将主/次波长改为410nm/600nm,计算因数改为3337,其余测定参数不变。
本发明的试剂盒是一种体外诊断试剂,用来测定人血清(体液)中α-L-岩藻糖苷酶(AFU)活性,对原发性肝癌的诊断和监护,以及监护肝硬化病人是否并发肝癌,独具价值。本发明的试剂盒既适用于全自动生化分析仪,也适用于半自动生化分析仪,需要时还可改为手工操作使用,操作十分简便,适用于各种实验室,市场前景广阔。

1、一种肝癌诊断、监护及肝硬化监护试剂盒,其特征在于该试剂盒是由试剂1和试剂2组成的液体型双试剂,其中试剂1为(浓度以毫摩尔浓度(mmol/L)或重量/体积百分比(W/V)表示):
柠檬酸三钠               55.5-300mmol/L
或Na2HPO4            74.8-300mmol/L
CAPSO                    0-300mmol/L
NaAc                     200-300mmol/L
NaN3                   0-0.1%(W/V)
NaH2PO4               0-300mmol/L
试剂2为(浓度以毫摩尔浓度(mmol/L)或重量/体积百分比(W/V)表示):
