Abstract

Description
技术领域
本公开涉及外科缝合装置,并且更具体地涉及用于将缝钉线支撑物组合件以可拆卸的方式固定或保持到外科缝合装置的组合件和方法。
背景技术
外科医生采用外科缝合装置来顺序地或同时地将一排或多排紧固件(例如,缝钉或两件式紧固件)施加到身体组织,以便将身体组织的区段联结在一起。当缝合相对较薄或易碎的组织时,重要的是有效地密封缝钉线以防止空气或流体泄漏。另外,常常需要加固缝钉线以抵抗组织,从而抑制组织中的撕裂或者拉动缝钉穿过组织。一种抑制撕裂或拉穿的方法涉及在缝钉与下面的组织之间放置生物相容性加固材料或“支撑物”材料。在此方法中,将一层支撑物组合件放置在组织上,并且以常规方式缝合组织。
因此,能够容易且有效地附接和移除外科缝合装置的支撑物组合件的新系统和方法将是期望的。
发明内容
本公开描述了包括支撑物保持组合件的外科缝合装置,该外科缝合装置证明了满足性能要求并且克服与将支撑物组合件以可拆卸的方式固定到外科缝合装置相关联的可用性挑战的实用途径。
根据本公开,一种外科缝合装置包括工具组合件和支撑物组合件。工具组合件包括可在闭合构型与打开构型之间转换的第一钳口构件和第二钳口构件。第一钳口构件或第二钳口构件中的至少一者包括保持组合件,该保持组合件包括限定凹部的支撑件。支撑物组合件包括支撑物材料和脊部。支撑物材料包括第一部分和第二部分。脊部包括沿支撑物材料的第一部分和第二部分的长度延伸的细长部分以及以正交关系附接到细长部分的接合部分。接合部分以可拆卸的方式接纳在第一钳口构件或第二钳口构件中的至少一者的保持组合件的支撑件的凹部中。

继续参考图1,外科缝合装置200包括可移动地安装在手柄202上的触发器236。触发器236的致动使工具组合件107从打开构型转变到闭合构型并且随后致动外科缝合装置200以将缝钉线施加到组织。外科缝合装置200进一步包括回缩机构217,该回缩机构可以被手动抓握并且向近侧拉动以使外科缝合装置200的击发机构回缩。为了将工具组合件107相对于要缝合的组织正确地定向,外科缝合装置200另外设置有安装在手柄202上的旋钮234。旋钮234围绕外科缝合装置200的纵向轴线“A-A”的旋转使工具组合件107围绕纵向轴线“A-A”旋转。关于外科缝合装置200的构造和操作的详细讨论,可以参考美国专利申请公布号2014/0263550,该美国专利申请的全部内容以引用方式并入本文。
图1和图2展示了与外科缝合装置200一起使用的支撑物组合件300。支撑物组合件300以可拆卸的方式固定到外科缝合装置200的工具组合件107,以与第一钳口构件108的砧座111和第二钳口构件110的缝钉仓112配准。支撑物组合件300包括支撑物材料310和脊部330。支撑物材料310被配置成加固和密封由外科缝合装置200施加到组织的缝钉线。支撑物材料310包括第一部分312和第二部分316,在该第一部分与该第二部分之间限定有活动铰链318。活动铰链318将第一部分312和第二部分316偏置背离彼此。在一方面,支撑物材料310具有V形。在一方面,支撑物材料310整体地形成为单个构造。在另一方面,支撑物材料310可以由弹性材料形成。脊部330横向间隔开并且沿着支撑物材料310的相对的周边部分延伸。在一些方面,脊部330被设置成将支撑物组合件300以可拆卸的方式固定到第一钳口构件108和第二钳口构件110并且维持支撑物材料310的第一部分312和第二部分316的大致平坦表面。每个脊部330包括沿着支撑物材料310的长度延伸的细长部分332和设置在细长部分332的相对的两个端部处的接合部分336。每个接合部分336从细长部分332横向向外延伸,使得接合部分336例如正交于细长部分332。接合部分336以可拆卸的方式与外科缝合装置200的工具组合件107(图1)接合,如将描述的。特别地,细长部分332从支撑物材料310的第一部分312的远侧端部312a和第二部分316的远侧端部316a向远侧延伸,使得接合部分336与支撑物材料310的第一部分312的远侧端部312a和第二部分316的远侧端部316a间隔开。在一方面,每个脊部330整体地形成为单个构造。在另一方面,脊部330可以由聚合物形成。在另一方面,脊部330可以是生物可吸收的。

在此类构型下,支撑物组合件300围绕延伸部650缠绕,并且支撑物材料310的远侧端部312a、316a以可拆卸的方式固定到相应的锚固部分620、630。当支撑物组合件300被支撑在装载组合件600上时,支撑物组合件300的第一部分312和第二部分316限定角度α(图6),该角度与由处于打开构型的第一钳口构件108和第二钳口构件110限定的角度β(图4)大致相同。
如图8所示,在使用之前,支撑物组合件300以可拆卸的方式支撑在装载组合件600上。在使用中,装载组合件600定位在第一钳口部件108与第二钳口部件110之间,钳口部件108、110处于打开构型。此后,第一钳口部件108的第一保持组合件180的支撑件182和第二钳口构件110的第二保持组合件190的支撑件192与装载组合件600的支撑件622、632对准。工具组合件107和装载组合件600进一步朝向彼此移动,直到支撑物组合件300的接合部分336(图2)被接纳在第一保持组合件180的凹部184(图3)和第二保持组合件190的凹部194(图5)中。以这种方式,支撑物组合件300以可释放的方式安装在外科缝合装置200的工具组合件107上。在缝钉已被施加到组织之后,支撑物组合件300被附接到组织,并且支撑物组合件300可通过将工具组合件107拉离组织而与第一保持组合件180和第二保持组合件190脱离。
还可以设想,支撑物组合件300可以由任何生物相容性天然或合成材料制成。形成支撑物组合件300的材料可以是可生物吸收的或不可生物吸收的。应当理解,可以使用天然的、合成的、生物可吸收的和/或非生物可吸收的材料的任何组合来形成支撑物组合件300。
可用于制造支撑物组合件300的材料的非限制性示例包括但不限于:聚(乳酸)、聚(乙醇酸)、聚(羟基丁酸酯)、聚(磷嗪)、聚酯、聚乙二醇、聚氧化乙烯、聚丙烯酰胺、聚羟乙基甲基丙烯酸酯、聚乙烯吡咯烷酮、聚乙烯醇、聚丙烯酸、聚乙酸酯、聚己内酯、聚丙烯、脂肪族聚酯、甘油、聚(氨基酸)、共聚(醚-酯)、聚草酸亚烷基酯、聚酰胺、聚(亚氨基碳酸酯)、聚草酸亚烷基酯、聚氧杂酯、聚原酸酯、聚磷腈和其共聚物、嵌段共聚物、均聚物、共混物和其组合。
在一些方面,天然生物聚合物用于形成支撑物组合件300。适合的天然生物聚合物包括但不限于胶原、明胶、纤维蛋白、纤维蛋白原、弹性蛋白、角蛋白、白蛋白、羟乙基纤维素、纤维素、羟丙基纤维素、甲壳质、壳聚糖和其组合。此外,天然生物聚合物可以与本文所描述的任何其他聚合物材料组合,以产生支撑物组合件300。
支撑物组合件300可以是多孔的或无孔的,或者多孔层和无孔层的组合。在支撑物组合件300是无孔的情况下,支撑物组合件300可以延迟或抑制组织从周围组织向内生长,从而充当粘附屏障并抑制不需要的疤痕组织的形成。因此,在各方面中,支撑物组合件300具有抗粘附特性。由此类材料形成无孔层的技术在本领域技术人员的能力范围内,并且包括例如铸造、模制等。
在一些方面,支撑物组合件300是多孔的并且具有止血特性。在支撑物组合件300是多孔的情况下,支撑物组合件在其表面的至少一部分上具有开口或孔。用于形成多孔层的适合的材料包括但不限于泡沫(例如,开孔泡沫或闭孔泡沫)。在一些方面,孔可以具有足够的数量和大小,以便在多孔层的整个厚度上相互连接。在其他方面中,孔可以不在多孔层的整个厚度上相互连接。在其他方面中,孔不延伸遍及多孔层的整个厚度,而是存在于多孔层的表面的一部分处。在一些方面,开口或孔位于多孔层的表面的一部分上,且多孔层的其他部分具有无孔纹理。阅读本公开内容的本领域技术人员将预见多孔层的其他孔分布模式和构型。
在支撑物组合件300是多孔的情况下,孔可以使用适于形成泡沫或海绵的任何方法(包括但不限于组合物的冻干或冷冻干燥)形成。用于制备泡沫的适合的技术在本领域技术人员的能力范围内。多孔支撑物组合件300可以为至少0.2cm厚,在一些方面为从约0.3cm至约1.5cm厚。多孔支撑物组合件300可以具有不超过约75mg/cm2的密度,并且在一些方面低于约20mg/cm2。多孔支撑物组合件300中的孔的尺寸可以为从约20μm至约300μm,并且在某些方面中为从约100μm至约200μm。
支撑物组合件300还可以包括加固构件。加固构件可以与多孔或无孔层相关联,或者可以定位在支撑物组合件300的无孔层与多孔层之间。替代性地,加固构件可以完全定位在支撑物组合件300的一个或多个单独的层内(例如,嵌入多孔层、无孔层或这两者内)。还可以设想,加固构件可以定位在构成支撑物组合件300的层之一的表面处,并且在一些方面,可以定位在支撑物组合件300的外表面处。
加固构件的一些适合的非限制性示例包括织物、网、单丝、复丝编织物、短切纤维(在本领域中有时称为短纤维)和其组合。在加固构件是网状物的情况下,其可以使用本领域技术人员已知的任何技术来制备,诸如针织、编织、起绒、刀切等。在单丝或复丝编织物用作加固构件的情况下,单丝或复丝编织物可以以任何期望的方式定向。例如,单丝或复丝编织物可以在支撑物组合件300内相对于彼此随机地定位。作为另一个示例,单丝或复丝编织物可以在支撑物组合件300内沿共同的方向定向。当短切纤维用作加固构件时,短切纤维可以以任何期望的方式定向。例如,短切纤维可以是随机定向的或可以以共同方向定向。短切纤维因此可以形成非织造材料,比如垫或毡。短切纤维可以(例如,通过热熔合)联结在一起或者它们可以彼此不附接。短切纤维可以具有任何适当的长度。例如,短切纤维的长度可以为0.1mm至100mm,并且在一些方面可以为0.4mm至50mm。在一方面,支撑物组合件300具有随机定向的短切纤维,这些短切纤维先前没有熔合在一起并且嵌入支撑物组合件300中。
可以设想的是,加固构件可以由本文先前描述的任何可生物吸收的、不可生物吸收的、天然的、或合成的材料和其组合形成。在使用单丝或复丝编织物作为加固构件的情况下,可以有利地使用任何市售缝合材料作为加固构件。
在一些方面,至少一种生物活性剂可以与支撑物组合件300和/或用于构造支撑物组合件300的单独部件(多孔层、无孔层和/或加固构件)中的任一个部件组合。在一些方面,支撑物组合件300还可以用作用于输送生物活性剂的载体。如本文所用,术语“生物活性剂”以其最广义使用,并且包括具有临床用途的任何物质或物质的混合物。因此,生物活性剂本身可以具有或不具有药理学活性,例如染料或香料。替代性地,生物活性剂可以是提供治疗或预防效果的任何试剂,诸如影响或参与组织生长、细胞生长或细胞分化的化合物。
可以根据本公开利用的生物活性剂种类的示例包括抗粘连剂、抗微生物剂、镇痛剂、退热剂、麻醉剂、抗癫痫剂、抗组胺剂、消炎剂、心血管药、诊断剂、拟交感神经剂、拟胆碱剂、抗毒蕈碱剂、镇痉剂、激素、生长因子、肌肉松弛剂、肾上腺素能神经元阻断剂、抗肿瘤剂、免疫原性剂、免疫抑制剂、胃肠药、利尿剂、类固醇、脂质、脂多糖、多糖和酶。还预期可以使用生物活性剂的组合。
抗粘合剂或抗粘附剂可以用于抑制在支撑物组合件300和与目标组织相对的周围组织之间形成粘附。这些试剂的一些示例包括但不限于聚(乙烯基吡咯烷酮)、羧甲基纤维素、透明质酸、聚环氧乙烷、聚乙烯醇和其组合。
可以作为生物活性剂包括在本公开的支撑物组合件300中的适合的抗微生物剂包括三氯生(triclosan),也称为2,4,4'-三氯-2'-羟基二苯基醚;氯己定(chlorhexidine)和其盐,包括醋酸氯己定(chlorhexidine acetate)、葡萄糖酸氯己定(chlorhexidinegluconate)、盐酸氯己定(chlorhexidine hydrochloride)和硫酸氯己定(chlorhexidinesulfate);银和其盐,包括乙酸银、苯甲酸银、碳酸银、柠檬酸银、碘酸银、碘化银、乳酸银、月桂酸银、硝酸银、氧化银、棕榈酸银、银蛋白和磺胺嘧啶银(silver sulfadiazine);多粘菌素(polymyxin);四环素(tetracycline);氨基糖苷类,例如妥布霉素(tobramycin)和庆大霉素(gentamicin);利福平(rifampicin);杆菌肽(bacitracin);新霉素(neomycin);氯霉素(chloramphenicol);咪康唑(miconazole);喹诺酮类,例如恶喹酸(oxolinic acid)、诺氟沙星(norfloxacin)、萘啶酸(nalidixic acid)、培氟沙星(pefloxacin)、依诺沙星(enoxacin)和环丙沙星(ciprofloxacin);青霉素类,例如苯唑西林(oxacillin)和哌拉西林(pipracil);壬苯醇醚9(nonoxynol 9);夫西地酸(fusidic acid);头孢菌素(cephalosporin);和其组合。此外,抗微生物蛋白质和诸如牛乳铁蛋白和乳铁蛋白肽B等肽可以作为生物活性剂包括在本公开的生物活性涂层中。
可以作为生物活性剂包括在根据本公开的支撑物组合件300中的其他生物活性剂包括:局部麻醉剂;非类固醇抗生育剂;拟副交感神经剂;精神治疗剂;镇静剂;解充血剂;镇静催眠剂;类固醇;磺酰胺;拟交感神经剂;疫苗;维生素;抗疟剂;抗偏头痛剂;抗帕金森剂,诸如L-多巴;镇痉药;抗胆碱能剂(例如,奥昔布宁);止咳药;支气管扩张剂;冠状动脉血管扩张剂、硝酸甘油等心血管药物;生物碱;镇痛剂;麻醉剂,诸如可待因、二氢可待因酮、度冷丁、吗啡等;非麻醉剂,诸如水杨酸盐、阿司匹林、对乙酰氨基酚、右旋丙氧芬等;阿片样物质受体拮抗剂,诸如纳曲酮和纳洛酮;抗癌剂;抗惊厥药;止吐药;抗组胺剂;抗炎剂,诸如激素剂、氢化可的松、泼尼松龙、泼尼松、非激素剂、别嘌呤醇、消炎痛、保泰松等;前列腺素和细胞毒性药物;雌激素;抗菌剂;抗生素;抗真菌剂;抗病毒剂;抗凝血剂;抗惊厥药;抗抑郁药;抗组胺剂;和免疫剂。

Claims (20)
1.一种外科缝合装置,包括:
工具组合件,所述工具组合件包括能够在闭合构型与打开构型之间转换的第一钳口构件和第二钳口构件,所述第一钳口构件或所述第二钳口构件中的至少一者包括保持组合件,所述保持组合件包括限定凹部的支撑件;和
支撑物组合件,所述支撑物组合件包括:
支撑物材料,所述支撑物材料包括第一部分和第二部分;和
脊部,所述脊部包括细长部分和接合部分,所述细长部分沿着所述支撑物材料的所述第一部分和所述第二部分的长度延伸,所述接合部分以正交关系附接到所述细长部分,所述接合部分以可拆卸的方式接纳在所述第一钳口构件或所述第二钳口构件中的至少一者的所述保持组合件的所述支撑件的凹部中。
2.根据权利要求1所述的外科缝合装置,其中所述支撑物材料包括活动铰链,所述活动铰链将所述支撑物材料的所述第一部分和所述第二部分彼此连接,所述活动铰链使所述第一部分和所述第二部分偏置背离彼此。
3.根据权利要求1所述的外科缝合装置,其中所述支撑物材料的所述第一部分和所述第二部分限定V形轮廓。
4.根据权利要求1所述的外科缝合装置,其中所述支撑物材料整体地形成。
5.根据权利要求1所述的外科缝合装置,其中所述脊部的所述接合部分从所述脊部的所述细长部分的端部部分径向向外延伸。
6.根据权利要求1所述的外科缝合装置,其中所述第一钳口构件或所述第二钳口构件中的至少一者具有渐缩端部,并且所述保持组合件的所述支撑件设置在所述渐缩端部上。
7.根据权利要求6所述的外科缝合装置,其中所述保持组合件的所述支撑件具有三角形形状以减小所述支撑物材料和所述脊部的弯曲。
8.根据权利要求1所述的外科缝合装置,其中所述脊部的所述细长部分沿着所述支撑物材料的外周部分延伸。
9.根据权利要求1所述的外科缝合装置,其中所述支撑物组合件的所述接合部分通过卡扣配合或摩擦配合以可释放的方式固定至所述保持组合件的所述支撑件。
10.根据权利要求1所述的外科缝合装置,其中所述支撑物材料的所述脊部是生物可吸收的。
11.根据权利要求1所述的外科缝合装置,其中所述脊部的所述细长部分的至少一部分介于所述支撑物材料与所述第一钳口构件或第二钳口构件之间。
12.一种外科套件,包括:
支撑物组合件,所述支撑物组合件包括:
支撑物材料,所述支撑物材料包括第一部分和第二部分;和
第一脊部,所述第一脊部包括细长部分和接合部分,所述细长部分沿所述支撑物材料的所述第一部分和所述第二部分的长度延伸,所述接合部分从所述细长部分的相应的远侧端部部分横向向外延伸;
装载组合件,所述装载组合件包括基座部分和从所述基座部分延伸的延伸部,所述基座部分包括在所述基座部分的相对的两个端部上的第一锚固组合件和第二锚固组合件,所述支撑物组合件包括围绕所述装载组合件的所述延伸部缠绕的部分,所述支撑物组合件的所述第一脊部的所述接合部分以可拆卸的方式固定到所述装载组合件的所述基座部分的相应的第一锚固组合件和第二锚固组合件;和
外科缝合装置,所述外科缝合装置包括工具组合件,所述工具组合件具有能够在闭合构型与打开构型之间转换的第一钳口构件和第二钳口构件,所述第一钳口构件或所述第二钳口构件包括相应的保持组合件,每个保持组合件包括限定凹部的支撑件,所述凹部被构造成以可释放的方式接纳所述第一脊部的对应的接合部分。
13.根据权利要求12所述的外科套件,其中所述支撑物材料还包括将所述支撑物材料的所述第一部分和所述第二部分互连的活动铰链,所述活动铰链将所述第一部分和所述第二部分偏置背离彼此。
14.根据权利要求12所述的外科套件,其中所述支撑物材料整体地形成为单个构造。
15.根据权利要求12所述的外科套件,其中所述第一脊部的所述接合部分以正交关系从所述细长部分的相应的远侧端部延伸。
16.根据权利要求12所述的外科套件,其中支撑在所述装载组合件上的所述支撑物组合件限定第一角度,所述第一钳口构件和所述钳口构件在间隔开的构型中限定第二角度,所述第一角度和所述第二角度是相等的。
17.根据权利要求12所述的外科套件,其中所述支撑物组合件还包括第二脊部,所述第一脊部和所述第二脊部侧向间隔开并且设置在所述支撑物材料的相对的侧向侧上。
18.根据权利要求17所述的外科套件,其中所述第一钳口构件或所述第二钳口构件的所述保持组合件包括限定凹部的一对支撑件。
19.根据权利要求17所述的外科套件,其中所述第一脊部或所述第二脊部由生物可吸收的材料形成。
20.根据权利要求12所述的外科套件,其中所述外科缝合装置的所述保持组合件的所述支撑件通过卡扣配合或过盈配合将所述脊部固定到所述保持组合件。
