Abstract
本发明涉及医药贴膏领域,具体是一种适用于亚急性甲状腺炎的止痛降温凝胶贴及其制备方法和应用。本发明的适合亚急性甲状腺炎急性期甲状腺疼痛时的局部止痛降温贴,有局部降温、清热解毒、消肿止痛的功效,用于缓解甲状腺局部肿痛和发热,且有安全、不良反应少的优点。

技术领域
本发明涉及医药贴膏领域,具体地说,是一种适用于亚急性甲状腺炎的止痛降温凝胶贴及其制备方法和应用。
背景技术
亚急性甲状腺炎(亚甲炎)又称肉芽肿性甲状腺炎,是最常见的甲状腺痛性疾病,本病与病毒感染有关,多发生于30-50岁的女性,属自限性疾病,病程在6-8周,预后良好。急性期的主要症状是发热和甲状腺局部疼痛及肿大,甲状腺剧烈疼痛是很多患者的门诊主诉,也是亚急性甲状腺炎最典型的症状。目前的主要治疗方法是非甾体抗炎药和激素治疗,但有病情反复、胃部不良反应和骨质疏松的副作用。
目前公认中医药治疗亚急性甲状腺炎效果显著,中医理论认为亚急性甲状腺炎的主要病机是肝火炽盛,造成甲状腺区域这一肝经循行所过之处火郁化毒,治疗的方法主要以清热解毒、凉血散结为主。除了内服汤药,局部贴敷甲状腺可以使药物直接作用在局部,从而更好发挥阵痛、解毒和消肿的作用。
中国专利文献CN110179917A公开了一种用于治疗桥本甲状腺炎甲状腺肿的穴位敷贴的中药组合物,其药效成分按重量份计,包括如下组分:蒲公英1-5份;山慈菇1-4份;黄药子1-4份;当归1-5份。该发明进一步提供中药组合物或药物制剂在制备用于治疗桥本甲状腺炎甲状腺肿的药物中的用途。该发明提供的一种用于治疗桥本甲状腺炎甲状腺肿的穴位敷贴的中药组合物,通过改变中药作用途径,减少药物不良反应,增强了药物疗效,减少了患者长期口服中药的痛苦,提高了患者依从性。
但是关于一种适用于亚急性甲状腺炎的止痛降温凝胶贴目前还未见报道。
发明内容
本发明的目的在于提供一种适用于亚急性甲状腺炎的止痛降温凝胶贴及其制备方法和应用。
本发明的第一方面,提供一种适用于亚急性甲状腺炎的外用中药组合物,由以下重量份的原料药组成:延胡索20-30份、大黄10-20份、黄芩10-20份、山慈姑10-20份、冰片3-5份、薄荷5-10份。
进一步地,所述的外用中药组合物由以下重量份的原料药组成:延胡索25-30份、大黄15-20份、黄芩15-20份、山慈姑15-20份、冰片3-4份、薄荷8-10份。
更进一步地,所述的外用中药组合物由以下重量份的原料药组成:延胡索25份、大黄15份、黄芩15份、山慈姑15份、冰片3份、薄荷8份。
本发明的第二方面,提供一种如上所述的外用中药组合物作为活性成份的止痛降温凝胶贴。
进一步地,所述的止痛降温凝胶贴的制备方法,包括以下步骤:
(A)按照组成和配比配置好中药剂量后,将药材洗净,40℃条件下烘干,粉碎成细颗粒后混合;采用超临界二氧化碳萃取法提取混合物的有效成分,得到混合萃取物;
(B)将丙烯酸(酯)类共聚物钠、卵磷脂分散在甘油中,得到甘油分散膏,将黄原胶分散于1,2-戊二醇中,得到1,2-戊二醇分散液,将超纯水、甘油分散液、1,2-戊二醇分散液、赤藓醇依次投入到主锅中,并加热至80℃-85℃,保温搅拌30分钟,得到水相物料;将聚二甲基硅氧烷加入到主锅中,待主锅温度升至80℃-85℃时保温均质10分钟后,保温搅拌3-5分钟,然后降温;当主锅内的温度降至60℃-65℃时,将对羟基苯乙酮加入到主锅中;当主锅内的温度降至45℃时,依次加入透明质酸钠、丁二醇、步骤(A)得到的混合萃取物,在均质器上震动3-5分钟混匀,搅拌30分钟出料,得到所述的止痛降温凝胶贴(参考专利文献CN114377053A)。
进一步地,步骤(A)所述的超临界二氧化碳萃取法提取混合物的有效成分的方法为:设定分离釜Ⅰ压力为6Mpa、温度为40℃,分离釜Ⅱ压力为4Mpa、温度为30℃,CO2流量基本稳定在25kg/h左右,萃取釜设定压力为25Mpa;从夹带剂泵处注入95%的乙醇,稳定萃取2.5h后,依次关闭装置阀门,停止萃取;期间间隔一段时间用洁净大烧杯于分离釜Ⅰ和分离釜Ⅱ处收集萃取物;混合萃取物,除菌过滤后密封阴凉保存。
本发明的第三方面,提供一种如上所述的外用中药组合物在制备治疗亚急性甲状腺炎的药物中的应用。
本发明的第四方面,提供一种如上所述的止痛降温凝胶贴在制备治疗亚急性甲状腺炎的药物中的应用。
进一步地,所述的止痛降温凝胶贴用于甲状腺局部敷贴,每天1-2次,每次1个小时。
本发明的中药组分的主要功效是清热止痛、解毒散结。其中延胡索为君药,行气止痛、调达肝经,大黄、黄芩为臣,清热泻火解毒,山慈姑为佐,清热解毒。散结消肿,冰片和薄荷为佐,不仅清热解毒,更利用其清凉透达的性质加强局部的清凉感受、并且增加外敷中药的透皮效率。
本发明中各原料药的药性如下:
延胡索:为罂粟科植物延胡索Corydalis yanhusuo W.T.Wang的干燥块茎。夏初茎叶枯萎时采挖,除去须根,洗净,置沸水中煮至恰无白心时,取出,晒干。归肝、脾经。性味辛、苦,温。活血,利气,止痛。用于胸胁、脘腹疼痛,经闭痛经,产后瘀阻,跌扑肿痛。其止痛作用尤其突出,内服外敷都有明显的止痛作用。
大黄:本品为蓼科植物掌叶大黄Rheum palmatum L、唐古特大黄Rheumtanguticum Maxim ex Balf。或药用大黄Rheum officinale Baill的干燥根及根茎。秋末茎叶枯萎或次春发芽前采挖,除去细根,刮去外皮,切瓣或段,绳穿成串干燥或直接干燥。归脾、胃、大肠、肝、心包经。性味苦,寒。泻热通肠,凉血解毒,逐瘀通经。用于实热便秘,积滞腹痛,泻痢不爽,湿热黄疸,血热吐衄,目赤,咽肿,肠痈腹痛,痈肿疔疮,瘀血经闭,跌打损伤,外治水火烫伤;用于火毒疮疡。
黄芩:本品呈圆锥形,扭曲,长8~25cm,直径1~3cm。表面棕黄色或深黄色,有稀疏的疣状细根痕,上部较粗糙,有扭曲的纵皱或不规则的网纹,下部有顺纹和细皱。质硬而脆,易折断,断面黄色,中间红棕色;老根中心枯朽状或中空,呈暗棕色或棕黑色。气微,味苦。归肺、胆、脾、大肠、小肠经。性味苦,寒。清热燥湿,泻火解毒,止血,安胎。用于湿温、暑温胸闷呕恶,湿热痞满,泻痢,黄疸,肺热咳嗽,高热烦渴,血热吐衄,痈肿疮毒,胎动不安。
山慈姑:为兰科植物杜鹃兰、独蒜兰等的假球茎。5~6月挖职假球茎;除去茎叶、须根,洗净,晒干。归肝、脾经。性味甘微辛,寒。消肿,散结,化痰,解毒。治痈疽疔肿,瘰疬,喉痹肿痛,蛇、虫、狂犬伤。
冰片:分机制冰片与艾片两类。机制冰片以松节油、樟脑等为原料经化学方法合成的龙脑;艾片为菊科艾纳香属植物大风艾的鲜叶经蒸气蒸馏、冷却所得的结晶,又称“艾粉”或“结片”。本组分中使用前者。开窍醒神,清热止痛。用于热病神昏、痉厥,中风痰厥,气郁暴厥,中恶昏迷,目赤,口疮,咽喉肿痛,耳道流脓。
薄荷:本品为唇形科薄荷属植物薄荷Mentha haplocalyx Briq.的干燥地上部分。夏、秋二季茎叶茂盛或花开至三轮时,选晴天,分次采割,晒干或阴干。归肺、肝经。性味辛,凉。宣散风热。清头目,透疹。用于风热感冒,风温初起,头痛,目赤,喉痹,口疮,风疹,麻疹,胸胁胀闷。
本发明优点在于:
1、本发明的适合亚急性甲状腺炎急性期甲状腺疼痛时的局部止痛降温贴,有局部降温、清热解毒、消肿止痛的功效,用于缓解甲状腺局部肿痛和发热,且有安全、不良反应少的优点。
2、相比与目前市售的降温凝胶贴和敷贴,本发明的特点在于专用于亚急性甲状腺炎急性期甲状腺贴敷,比降温凝胶贴更加了中药成分,缓解甲状腺疼痛肿大的作用更强。相比于敷贴,凝胶成分本身通过蒸发水分产生凉感,体感更舒适,且凝胶有粘性可以固定在甲状腺部位,减少因固定方法本身带来的不适。
具体实施方式
下面结合实施例对本发明提供的具体实施方式作详细说明。
下列实施例中未注明具体条件的实验方法,通常按照常规条件,或按照制造厂商所建议的条件。本发明实施例所用药材除另有注明,均可以从中药材销售公司获得。
实施例1:本发明的中药组成
延胡索20份、大黄10份、黄芩10份、山慈姑10份、冰片3份、薄荷5份。
实施例2:本发明的中药组成
延胡索22份、大黄12份、黄芩12份、山慈姑12份、冰片4份、薄荷6份。
实施例3:本发明的中药组成
延胡索24份、大黄14份、黄芩14份、山慈姑14份、冰片5份、薄荷7份。
实施例4:本发明的中药组成
延胡索25份、大黄15份、黄芩15份、山慈姑15份、冰片3份、薄荷8份。
实施例5:本发明的中药组成
延胡索28份、大黄18份、黄芩18份、山慈姑18份、冰片4份、薄荷9份。
实施例6:本发明的中药组成
延胡索30份、大黄20份、黄芩20份、山慈姑20份、冰片5份、薄荷10份。
实施例7:本发明的止痛降温凝胶贴的制备方法和使用
按照组成和配比配置好中药剂量后,将药材洗净,40℃条件下烘干,粉碎成细颗粒后混合。采用超临界二氧化碳萃取法提取混合物的有效成分。设定分离釜Ⅰ压力为6Mpa、温度为40℃,分离釜Ⅱ压力为4Mpa、温度为30℃,CO2流量基本稳定在25kg/h左右,萃取釜设定压力为25Mpa。从夹带剂泵处注入95%的乙醇,稳定萃取2.5h后,依次关闭装置阀门,停止萃取。期间间隔一段时间用洁净大烧杯于分离釜Ⅰ和分离釜Ⅱ处收集萃取物。混合萃取物,除菌过滤后密封阴凉保存。
将丙烯酸(酯)类共聚物钠、卵磷脂分散在甘油中,得到甘油分散膏,将黄原胶分散于1,2-戊二醇中,得到1,2-戊二醇分散液,将超纯水、甘油分散液、1,2-戊二醇分散液、赤藓醇依次投入到主锅中,并加热至80℃-85℃,保温搅拌30分钟,得到水相物料;将聚二甲基硅氧烷加入到主锅中,待主锅温度升至80℃-85℃时保温均质10分钟后,保温搅拌3-5分钟,然后降温。当主锅内的温度降至60℃-65℃时,将对羟基苯乙酮加入到主锅中;当主锅内的温度降至45℃时,依次加入透明质酸钠、丁二醇、上述混合萃取物,在均质器上震动3-5分钟混匀,搅拌30分钟出料,得到所述的止痛降温凝胶贴。
实施例8:本发明的临床实验
一项纳入48人的临床观察研究,根据是否使用降温凝胶贴分为对照组(未使用)和干预组(使用)。用评分评估甲状腺疼痛及缓解情况,并记录过敏等不良反应和疼痛视觉模拟量表(VAS)评分:使用长为10cm的直线,两端是0分和10分,分值越低代表疼痛越轻,0分为无痛,10分为完全无法忍受的剧烈疼痛,请患者划出与自己疼痛情况最为接近的点,医师根据患者标出的点评算出分数,各组均于治疗后第1天、第3天、第5天、第7天、第14天、第28天记录VAS评分,计算并比较组间均数差值。

甲状腺局部疼痛是亚甲炎的主要症状,因为疼痛引起的全身乏力、纳差寐差等也进一步延长病程,给患者带来困扰,正因如此,亚甲炎虽然是一种自限性疾病,但缓解疼痛依然是迫切的临床需求。此甲状腺敷贴止痛降温,研究结果显示,对照组和治疗组的亚甲炎患者均在发病后的第3天就诊,干预组从第3天起明显降低VAS评分,相较于对照组的优势直至观察的第28天,全程明显降低VAS评分,说明此凝胶贴在缓解疼痛方面优势显著,有很高的临床使用价值。
以上已对本发明创造的较佳实施例进行了具体说明,但本发明创造并不限于所述实施例,熟悉本领域的技术人员在不违背本发明创造精神的前提下还可做出种种的等同的变型或替换,这些等同的变型或替换均包含在本申请权利要求所限定的范围内。
Claims (8)
1.一种适用于亚急性甲状腺炎的外用中药组合物,其特征在于,由以下重量份的原料药组成:延胡索20-30份、大黄10-20份、黄芩10-20份、山慈姑10-20份、冰片3-5份、薄荷5-10份。
2.根据权利要求1所述的适用于亚急性甲状腺炎的外用中药组合物,其特征在于,由以下重量份的原料药组成:延胡索25-30份、大黄15-20份、黄芩15-20份、山慈姑15-20份、冰片3-4份、薄荷8-10份。
3.根据权利要求1所述的适用于亚急性甲状腺炎的外用中药组合物,其特征在于,由以下重量份的原料药组成:延胡索25份、大黄15份、黄芩15份、山慈姑15份、冰片3份、薄荷8份。
4.一种如权利要求1-3任一所述的外用中药组合物作为活性成份的止痛降温凝胶贴。
5.根据权利要求4所述的止痛降温凝胶贴,其特征在于,所述的止痛降温凝胶贴的制备方法,包括以下步骤:
(A)按照组成和配比配置好中药剂量后,将药材洗净,40℃条件下烘干,粉碎成细颗粒后混合;采用超临界二氧化碳萃取法提取混合物的有效成分,得到混合萃取物;
