背景技术
结核病(Tuberculosis,TB)是由结核分枝杆菌(Mycobacterium tuberculosis,Mtb)感染引起的慢性传染性及肉芽肿性疾病。据WHO发布的《2020年全球结核病报告》显示中国仍是TB高负担国家之一,其发病数位居全球第二。2019年全球TB患者约有1000万,而中国TB患者就有约100万,这对我国经济社会发展造成了极大的影响。近年来,随着多重耐药Mtb的出现、艾滋病继发结核感染的病例增多,导致TB的发病率呈上升趋势,结核病现已成为全球公共卫生问题。研究发现多重耐药Mtb能通过调节自身苯丙氨酸代谢、能量代谢及脂肪酸代谢,增加其对抗痨药物卡普霉素和对氨基水杨酸的耐药性。Mtb还可通过上调巨噬细胞miR-196b-5p表达抑制巨噬细胞抗炎功能,并启动异常T细胞包括γδT细胞、MAIT细胞反应促使T细胞IL-22分泌和BTLA表达进行性丧失,以及上调T细胞miR-16表达促使抗原记忆性T细胞耗竭和功能弱化。因此,早期有效的治疗对TB病情的控制至关重要。这就要求提供快速,可靠和即时的诊断方法,以进行有效的病例管理。目前,筛查和诊断结核病的常用方法有影像学检查、内镜检查、活体组织检查和细菌培养,但这些方法仍不能准确诊断结核病。此外,细菌培养被认为是结核病诊断和耐药性测试的“金标准”,但此方法仍需要4到12周的时间才能获得最终的确认报告。因此,开发新型特异性TB诊断标志至关重要。
结核性肉芽肿(Tuberculous granuloma,TG)是TB的基本病理特征,也是宿主免疫与Mtb的“决战场”。TG的形成是一个动态的病理过程,需要多种组分参与,包括多核巨细胞(也称Langhans巨细胞)、类上皮细胞、免疫细胞、成纤维细胞、细胞因子和趋化因子等。然而,目前科学家仍在不断的研究与发现参与TG形成的组分。新近越来越多研究证实TB从潜伏感染者到活动性TB患者的进展主要取决于感染的局部,即TG。因此,全面了解TG结构对TB的诊断标志的开发具有较大意义。
间充质干细胞(Mesenchymal stem cells,MSCs)是一种几乎存在于机体所有组织中的多能干细胞,它具有干细胞的共性,即自我更新和多向分化潜能。在某些生理和实验条件下,MSCs可分化成特定细胞,如成骨细胞、脂肪细胞和神经细胞等,还可在激活后募集到组织损伤部位,再生新的组织并修复缺损。MSCs亦可分泌大量细胞因子。近年来有研究发现骨髓来源MSCs可通过分泌IL-6激活鼻咽癌上皮细胞CD73/腺苷受体信号通路,进而促进细胞生长和耐药。MSCs也可迁移至TG,参与局部环境的免疫调节。
MSCs与Mtb感染存在关联性。新近研究发现MSCs可促进脾肉芽肿中Mtb生长,然而MSCs经poly(A:U)处理后可抑制脾肉芽肿中Mtb生长,提示MSCs对Mtb生长具有调节性。MSCs可通过清道夫受体(Scavenger receptor,SR)和甘露糖受体(Mannose receptor,MR)内化或吞噬Mtb,并通过自噬限制Mtb生长。同时,MSCs还可作为Mtb在TG中休眠的“壁龛”,并调节局部免疫和Mtb耐药过程。另外,Mtb还可募集MSCs到感染部位抑制T淋巴细胞反应,从而逃逸宿主免疫监视。
目前,TB研究受限于缺乏代表人类结核性肉芽肿(TG)的模型。当前使用最广泛的实验动物(小鼠和斑马鱼)在Mtb感染后不会形成真正的TG。非人类灵长类动物TG与人类TG最相似,但由于高昂的运营成本和道德问题,不能被广泛使用。人类TG是一个复杂的免疫过程,单纯或共培养的细胞缺乏3D环境和组织响应,无法模拟TG。因此,开发一种创新性体外TG模型极为重要。
发明内容
鉴于现有技术存在的缺点,本申请提供了一种结核病诊断标志物及应用,以解决现有技术存在问题,具体采用如下的技术方案:
一种用于结核病诊断的标志物,标志物为CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞、CD161+CD4+CD3+T淋巴细胞、细胞因子IL-10和细胞因子TNF-α中的一种。
一种检测前述的标志物的物质在制备结核病诊断产品中的应用。
进一步地,产品基于ELISA检测法检测细胞因子IL-10和细胞因子TNF-α的含量。
进一步地,产品基于流式细胞术检测法检测CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞和CD161+CD4+CD3+T淋巴细胞的含量

本申请的有益效果是:
本申请研究发现,基于构建出的基于MSCs体外新型TG模型,CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞、CD161+CD4+CD3+T淋巴细胞、细胞因子IL-10和细胞因子TNF-α的含量在结核病患者和健康人群之间存在差异表达,因此,可以将基于MSCs体外新型TG模型中的CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞、CD161+CD4+CD3+T淋巴细胞、细胞因子IL-10和细胞因子TNF-α作为结核病患者的诊断指标,并且应用到相关诊断产品的制备中,具有较好的应用前景。

具体实施方式
下面详细描述本申请的实施例,实施例的示例在附图中示出,其中自始至终相同或类似的标号表示相同或类似的元件或具有相同或类似功能的元件。下面通过参考附图描述的实施例是示例性的,旨在用于解释本申请,而不能理解为对本申请的限制。
可以理解的是,本申请未详细说明的试剂仪器均可从市面购买获得,未详细说明的操作方法均按本领域常规操作或厂商说明书进行。
在本申请中,20例来自佛山市第四人民医院的TB患者作为结核实验组。HV(Healthy volunteers,健康志愿者)为健康对照组。
搭建基于MSCs体外新型TG模型的构建方法如下:
1)培养结核分枝杆菌:
将BCG(Bacillus calmette guerin,卡介苗)菌株接种于7H9培养基中,在恒温振荡培养箱中培养一定时间。具体地,在37℃恒温振荡培养箱中培养4周~6周。
其中,7H9液体培养基配置方法为:Middlebrook 7H9粉末4.7g,2mL甘油,0.5mLTween-80,加水补至900mL,121℃高温灭菌10min。待培养基温度冷却50℃后加入100mLOADC增菌液混匀,4℃保存。
2)培养MSCs:
在本申请中,采用的MSCs为LR-MSCs(Lung resident mesenchymal stem cells,人肺间充质干细胞)。
具体而言,将LR-MSCs接种于含10%胎牛血清,100U/mL青霉素和100μg/mL链霉素的α-MEM完全培养基,并置于37℃、5%CO2的细胞培养箱内。
其中,α-MEM完全培养基配置方法为:将50mL胎牛血清(FBS)和5mL青霉素-链霉素(双抗)加入到445mL的α-MEM基础培养基中,颠倒混匀即为500mL α-MEM完全培养基。配好之后,贴上封口膜封闭,放在4℃冰箱中低温保存备用。
3)分离PBMCs(Peripheral blood mononuclear cells,外周血单个核细胞):
在本申请中,采用密度梯度离心法分离人PBMCs。
具体地,分离过程如下:
(1)取一支15mL无菌离心管,加入与血液样本体积相同的淋巴细胞分离液;
(2)用巴式管小心吸取血液样本缓慢倾斜加于分离液的液面之上,将离心机升降速度调至0,3400rpm/min,室温水平离心30min;
(3)用巴氏管小心吸取第二层白色淋巴细胞层(第一层为血浆层;第二层为环状乳白色淋巴细胞层;第三层为透明分离液层;第四层为红细胞层)于另一无菌15mL离心管中,向离心管中加入10mL无菌PBS缓冲液,吹打混匀细胞悬液,1200rpm/min,室温水平离心10min,弃去上清;
(4)用适量红细胞裂解液重悬细胞,置于冰上裂解10min,1200rpm/min,室温水平离心10min,弃去上清;
(5)向离心管中加入10mL无菌PBS缓冲液,吹打混匀细胞悬液,1200rpm/min,室温水平离心10min,弃去上清;
(6)向离心管中加入10mL无菌PBS缓冲液,重复上一步;
(7)用1mL细胞冻存液重悬得PBMCs悬液,转移到冻存管。将细胞冻存管放置到程序降温盒,于-80℃冰箱过夜后,从程序降温盒中取出细胞冻存管,并转移至液氮罐中进行保存。
通过上述过程,分别分离出TB患者的PBMCs和HV对照组的PBMCs。
4)构建TG模型:
将1.5×105个PBMCs与1.5×105个LR-MSCs接种于低黏附6孔板中,在BCG感染复数为0.1的条件下进行3D体外共培养,从而建立基于MSCs体外新型TG模型。将建立好的基于MSCs体外新型TG模型在37℃、5%CO2饱和湿度培养箱共培养7天。如图1A-C所示,随着培养时间的增加,细胞聚集越紧密越多,体积也越大。本研究将体外新型TG模型放置于37℃、5%CO2饱和湿度培养箱培养7天,然后通过HE染色分析其病理形态,结果显示培养体系出现了上皮样细胞放射状排列,并呈结节样结构,表明本申请的构建的基于MSCs体外新型TG模型具有结核性肉芽肿特征,该模型可以用于后续实验,见图1D。
在步骤4)中,采用的MSCs分别来自结核病患者和健康人群,从而分别建立基于结核病患者MSCs体外新型TG模型和基于健康人群MSCs体外新型TG模型。通过研究两个模型之间的区别,从而判断结核病患者和健康人群的差异。通过对模型的的研究,对于结核病的诊断、结核病早期预警、相关诊断产品、靶向药物的研制具有极大的指导意义。
1.TB患者与HV来源PBMCs建立的体外新型TG模型中MSCs表型的变化
分别分离20例TB患者和20例HV来源的PBMCs,将1×105个PBMCs与1×105个LR-MSCs在BCG感染复数为0.1条件下进行3D体外共培养,建立基于MSCs体外新型TG模型。体外TG模型在37℃、5%CO2饱和湿度培养箱培养7天后,采用流式细胞术检测LR-MSCs细胞CD73和CD90表达(图2A和图2B)。结果显示,在TB患者来源的PBMCs建立的基于MSCs体外新型TG模型中CD73+LR-MSCs细胞含量(图2C)及CD73+LR-MSCs细胞中CD73表达的平均荧光强度(图2D)均显著高于HV来源的PBMCs建立的TG模型,差异有统计学意义(P<0.05)。另外,CD90+LR-MSCs细胞含量(图2E)及CD90+LR-MSCs细胞中CD90表达的平均荧光强度(图2F)均显著降低,差异有统计学意义(P<0.05)。以上结果显示TB患者与HV来源PBMCs可影响体外新型TG模型中MSCs的表型,尤其是TB患者来源的PBMCs会影响新型TG模型中MSCs的功能和分化。
2.TB患者与HV来源PBMCs建立的体外新型TG模型中单核巨噬细胞和T淋巴细胞表型变化
分别分离20例TB患者和20例HV来源的PBMCs,将1×105个PBMCs与1×105个LR-MSCs在BCG感染复数为0.1条件下进行3D体外共培养,建立基于MSCs体外新型TG模型。体外TG模型在37℃、5%CO2饱和湿度培养箱培养7天后,采用流式细胞术检测PBMC中CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞和CD161+CD4+CD3+T淋巴细胞的表达(图3A、图4A和图4D)。结果显示,在TB患者来源的PBMCs建立的基于MSCs体外新型TG模型中CD11c+CD11b+单核巨噬细胞含量(图3B)及CD11c+CD11b+单核巨噬细胞中CD11c表达的平均荧光强度(图3C)均显著高于HV来源的PBMCs建立的TG模型,差异有统计学意义(P<0.05)。另外,在TB患者来源的PBMCs建立的基于MSCs体外新型TG模型中CD161+CD8+CD3+T淋巴细胞(图4B)和CD161+CD4+CD3+T淋巴细胞含量(图4E)也均显著升高,差异有统计学意义(P<0.05);而CD161+CD8+CD3+T淋巴细胞(图4C)和CD161+CD4+CD3+T淋巴细胞中CD161表达的平均荧光强度(图4F)则均无显著差异。以上结果显示TB患者与HV来源PBMCs可影响体外新型TG模型中单核巨噬细胞和T淋巴细胞的表型,尤其是TB患者来源的PBMCs会影响新型TG模型中CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞和CD161+CD4+CD3+T淋巴细胞的功能和分化。
3.TB患者与HV来源的PBMCs建立的体外新型TG模型分泌的细胞因子的变化
分别分离20例TB患者和20例HV来源的PBMCs,将1.5×105个PBMCs与1.5×105个LR-MSCs在BCG感染复数为0.1条件下进行3D体外共培养,建立基于MSCs体外新型TG模型。体外TG模型在37℃、5%CO2饱和湿度培养箱培养7天后,采用ELISA检测体外新型TG模型培养上清IL-10、TNF-α和IL-6含量。结果显示,在TB患者来源的PBMCs建立的基于MSCs体外新型TG模型中IL-10含量显著低于HV来源的PBMCs建立的TG模型(图5A),差异有统计学意义(P<0.001)。而TNF-α含量则显著升高(图5B),差异有统计学意义(P<0.05)。另外,IL-6含量在HV和TB患者来源的PBMCs建立的体外新型TG模型中无显著差异(图5C)。以上结果显示TB患者与HV来源的PBMCs可影响体外新型TG模型中培养上清IL-10和TNF-α的表达,尤其是TB患者来源的PBMCs可促进新型TG模型中TNF-α表达,而抑制IL-10表达。说明TB患者来源的PBMCs建立的TG模型具有较强的促炎作用。
本实验首次构建了基于MSCs体外新型TG模型,并通过加入TB患者与HV来源的PBMCs,发现TG模型的细胞表型和其产生的细胞因子可以成为新的TB诊断标志。具体地的,基于MSCs体外新型TG模型中的CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞和CD161+CD4+CD3+T淋巴细胞,以及其分泌的细胞因子IL-10和TNF-α均可以成为新的TB诊断标志。
基于上述研究结果,本申请还提供:一种检测CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞、CD161+CD4+CD3+T淋巴细胞、细胞因子IL-10和细胞因子TNF-α中的至少一种的物质在制备结核病诊断产品中的应用。前述的结核病包括肺结核、肾结核、肠结核、骨结核、胃结核和肝结核。

1.一种用于结核病诊断的标志物,其特征在于,标志物为CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞、CD161+CD4+CD3+T淋巴细胞、细胞因子IL-10和细胞因子TNF-α中的一种。
2.一种检测权利要求1的标志物的物质在制备结核病诊断产品中的应用。

所述产品基于ELISA检测法检测细胞因子IL-10和细胞因子TNF-α的含量。
4.根据权利要求2的应用,其特征在于,
所述产品基于流式细胞术检测法检测CD73+MSCs细胞、CD90+MSCs细胞、CD11c+CD11b+单核巨噬细胞、CD161+CD8+CD3+T淋巴细胞和CD161+CD4+CD3+T淋巴细胞的含量。
5.根据权利要求2的应用,其特征在于,
