Description

                      本发明领域
本发明涉及采用某些生物活性的玻璃组合物使牙齿变白、变亮、漂白或从牙齿中去除色斑的各种方法。
                      本发明背景
牙齿由内牙质层和外硬釉质层(牙齿的保护层)所组成。牙齿的釉质层呈天然不透明白色或略显灰白色。正是此釉质层可以被玷污或变色。牙齿的釉质层由形成稍微多孔表面的羟基磷灰石矿物晶体所组成。我们相信釉质层的这种多孔性质使得玷污剂和变色物质渗入釉质内并使牙齿变色。此外,牙齿天然得到由唾液糖蛋白组成的称为表膜的膜。该表膜也可被玷污。此处所用的术语“牙齿”指的是磷灰石和具有表膜的物质。
一个人每天所面临的或与其接触的许多物质均能“玷污”牙齿或减少牙齿的“白度”。具体而言,人们消耗的食物、烟草制品和流体如茶和咖啡都可以玷污每个人的牙齿。然后这些污染物和变色物可以渗入到釉质层内。这个过程逐渐地进行许多年,最终使人牙齿的釉质发生显著的变色。
WO 96/10985公开了含有玻璃的粒状生物活性二氧化硅可以减少牙齿的髓质刺激和/或增强牙齿的结构,因而可用于过敏性牙齿的治疗。据建议这种玻璃可以仅由二氧化硅或氢氧化硅所组成,也可以含有一种或一种以上选自Ca、P、Na、K、Al、B、N、Mg、Ti或F的其他元素。同时还建议使用包括钙和磷酸盐的可以协助诱导牙质补充矿质的生物活性玻璃组合物,或者将不同来源的钙和磷酸盐以及不含以上物质的生物活性玻璃联合使用。
WO 97/27148公开了可以使牙齿补充矿质的粒状生物活性及生物适合的(biocompatible)玻璃,特别可用于降低牙质的过敏性。
人们采用过各种形式的处理以使牙齿变白,包括in-office漂白、夜用护板(night guard)漂白和增白牙膏。然而现有的各种方法和组合物有多种理由不能令人满意。如降低色斑(shade)的效果就不是很理想的。
                      本发明概要
我们现发现以上提及的生物活性玻璃可用来使牙齿变白。因此本发明提供一种使牙齿变白的方法,它包括使牙齿结构与牙齿变白量的粒状生物活性和生物适合的玻璃进行接触。
                      本发明详述
以下所用词语的意义与根据其目前用于口腔和牙科护理领域中的意义相同。此处本发明更具体的用法见下述。
此处所用的术语“变白”在所有情况下均包括漂白、变亮或从牙齿中清除污染物。
本发明提供一种用于使牙齿变白、变亮或漂白的方法。此方法也适用于将污染物从牙齿中去除。如WO 96/10985和WO 97/27148中所公开的那样,该方法包括使牙齿与有效牙齿变白量的生物活性玻璃接触。优选进行多次使用。每一次使用可包括约0.02-0.3克的生物活性玻璃。使牙齿变白的本方法适宜地包括每天使牙齿与所述牙齿变白有效量的生物活性玻璃接触两次,持续时间不少于两周。此处所用的术语“牙齿变白量”意指在每天使用两次、每次使用不少于2分钟,4周后使处理前(pre-treatment)色调深于A3.5的病人一种或一种以上的色调得到Vita色标(shade guide)变亮的任何量。
因此,本发明提供一种使牙齿变白的方法,它包括使牙齿与牙齿变白有效量的粒状生物活性和生物可配伍玻璃(包括二氧化硅或氢氧化硅以及任选一种或一种以上选自Na、K、Ca、Mg、B、Ti、Al、P、N或F的元素)接触。
优选生物活性玻璃至少包括Na、Ca和P,虽然也可以联合使用简单的钠硅石玻璃以及外部来源的钙和磷酸盐。本领域技术人员将知道外部来源的钙与磷酸盐可来自唾液本身,也可以配制成口腔卫生组合物。
根据本发明的生物活性玻璃是一种玻璃组合物,当放置于体外模拟体液时将形成一层羟基碳酸盐磷灰石。根据本发明的生物活性玻璃可通过各种方法如熔体衍生法(melt-derived)或溶胶-凝胶法进行制备并可具有各种组成元素及范围。例如,以下组成(按重量计)将得到一种可为人接受及优选的生物活性玻璃:
          SiO2             40-60
          CaO               10-30
          Na2O             10-35
          P2O5            2-8
          CaF2             0-25
          B2O3            0-10
          K2O              0-8
          MgO               0-5
具有这些性能的生物活性玻璃提供了一种与牙齿结构相互作用的更为有效的材料。在玻璃组成中加入氟化物将提高及增强牙齿的结构。其他实例包括具有例如约40-约86%(重量)SiO2、基本上没有钠、约6-36%(重量)Ca和约2-12%(重量)P2O5的溶胶-凝胶玻璃。根据本发明的生物适合的玻璃是一种不激发严重的不利免疫反应的玻璃。
在本发明的一些实施方案中,使用极细小的颗粒。例如,2mm-亚微细粒范围的颗粒是有利的。通过扫描电子显微镜检术或激光散射技术(如Coulter LS100)可确定根据本发明的颗粒大小。令人惊奇的是,较小的生物活性粒状玻璃不会引起明显的免疫反应。此外,它通常不会被巨噬细胞所吞噬并在该应用中表现出钝性。
可将本发明的组合物配制成各种口腔卫生组合物,如洁齿剂、牙膏、凝胶、粉末、漱口液、冲洗溶液和抽吸或咀嚼的形式如橡皮糖(gum)、锭剂、片剂和糖锭剂。
这种口腔卫生组合物适宜包含0.1-50%(重量)、优选为1-25%(重量)、更优选为5-10%(重量)的生物活性玻璃。
除了活性成分外,适用于本发明中的口腔卫生组合物还可含有各种常见载体、粘合剂、表面活性剂、湿润剂、着色剂、颜料、防牙菌斑剂、抗菌剂、生物胶粘类试剂、研磨剂、防龋剂、调味剂、甜味剂、松散剂等。
在牙膏配方的情况中,研磨剂通常包括无定形的、胶凝的、沉淀的或热解法二氧化硅、塑料颗粒、氧化铝、碳酸钙和正磷酸锌、不溶性偏磷酸盐以及焦磷酸钙。二氧化硅是特别优选用于此处的研磨剂。专利和科学文献列举了大量的这类研磨剂的实例。美国专利4,822,599列举了一系列洁齿剂研磨剂,同时也参考了其各种商业来源及制备方法。生物活性玻璃颗粒可全部、部分或不替代现今用于牙膏中的研磨剂。
无机增稠剂可包括在本发明的洁齿剂中,包括各种热解法二氧化硅如可购自Cabot Corporation的Cabosil,以及各种增稠二氧化硅包括可购自J.M.Huber的商标为Zeodent 165的那些。有机增稠剂如天然和合成树胶,并且胶体也可结合到本发明的洁齿剂组合物中。这种增稠剂的实例包括角叉菜聚糖(爱尔兰藓)、黄原胶和羧甲基纤维素钠、淀粉、聚乙烯基吡咯烷酮、羟基乙基丙基纤维素、羟基丁基甲基纤维素、羟基丙基甲基纤维素和羟基乙基纤维素(微晶纤维素/纤维素树胶的共干掺合物)。无机或有机增稠剂可以约0.05-约2%(重量)、优选为约0.1-约1.5%(重量)的浓度结合到本发明的组合物中。
具有防龋效果的能提供氟化物的盐也可以结合到本发明的口用组合物中,其特征在于其在水中释放氟化物离子的能力。优选使用能提供约10-5,000ppm氟化物离子、优选约1000-1500ppm氟化物离子的水溶性氟化物盐。在这些物质当中有各种水溶性碱金属盐如氟化钠、氟化钾、一氟磷酸钠和氟硅酸钠。一氟磷酸钠是优选的能提供氟化物的盐。
用于本发明实践中的颜料包括各种无毒、水不溶性的无机颜料,如二氧化钛、氧化铬绿、群青色和群青粉红色,和各种氧化铁以及各种通过将FD&C染料的钙盐或铝盐渗入到二氧化铝上所制备的水不溶性染料色淀如FD&C绿#1色淀、FD&C蓝#2色淀、FD&C R&D#30色淀和FD&C黄#15色淀。颜料的颗粒大小为5-1000微米、优选为250-500微米,浓度为0.5-3%(重量)。染料通常对过氧化物成分的存在很敏感并且不包括在洁齿剂中,虽然人们发现当洁齿剂中存在CaO2时FD&C绿#3具有抗褪色的能力。
可以使用任何适宜的调味或增甜材料。适宜调味成分的实例为调味油,如薄荷油、薄荷、冬青、黄樟、丁子香、一串红、桉树、甘牛至草、肉桂、柠檬和橙以及水杨酸甲酯。适宜的甜味剂包括蔗糖、乳糖、麦芽糖、山梨醇、环磺酸钠、紫苏亭和糖精钠。调味剂及增甜剂总量适宜不低于制品的0.01-5%。
各种其他物质也可以结合到本发明的口用组合物组分中。其各种非限制性实例包括防腐剂、硅氧烷和叶绿素化合物,各种维生素如维生素B6、B12、C、E和K,各种抗菌剂如洗必泰,各种卤代二苯醚如三氯苯氧氯酚,各种脱敏剂如硝酸钾和柠檬酸钾及其各种混合物。这些辅助剂以基本上不对所需的性能及特性造成不利影响的量结合到洁齿剂中,并且根据所涉及组分的具体类型加以选择和以适当的量使用。
粒状生物活性玻璃的制备对于本领域技术人员而言是众所周知的,无需在此做进一步的描述。类似地,各种凝胶、牙膏、清洗剂、漱口液、橡皮糖、口香糖的制备在本领域中也是众所周知的。通过各种方法包括简单混合可将各种生物活性玻璃结合到这些产品当中。本领域技术人员将会意识到,在某些情况中,有必要避免使生物活性玻璃与含水或输送媒介物的其他组分接触,以防生物活性玻璃发生预反应。这可通过本领域技术人员公知的方法来做到,包括例如一个具有两个注射器的混合室的形式。
生物活性玻璃除了直接施用到牙齿之外,本发明的生物活性玻璃组合物也可施用于盐水或蒸馏水基的介质中。
本发明也提供如此前所定义的生物活性和生物适合的玻璃并任选与一种钙和/或磷酸盐一起用于使牙齿变白的组合物的生产中。
                        实施例
筛选10名健康成年志愿者进行4周的临床实验。在基线处从中三(middle third)到切割边缘采用按如下顺序排列的Vita色标进行色调测定:B1、A1、B2、D2、A2、C1、C2、D4、A3、D3、B3、A3.5、B4、C3、A4、C4(Lumin Vacuum-Farbskala,Vita)。本研究只有色调亮度不超过A3.5的病人才符合入选要求。在进行每一次研究约会时和之前记录每一个受实验者硬组织及软组织的状况。病人们按指示每天仅使用7.5%的洁齿剂两次,保持2分钟的刷洗时间并避免使用其他的口腔呵护产品。两周后和四周后召集所有受实验者。
两周后色调记录变化范围为0-8,平均为4.2。四周后色调记录变化范围为0-8,平均为5.8。没有记录到在硬组织及软组织上有有害的影响。统计分析表明,在两周及四周与底线之间存在着显著的区别(p<0.05,ANOVA)。总而言之,7.5%的洁齿剂表现出使牙齿变白的效果(平均色调变化为5.8)而没有任何不利影响。本研究部分由US Biomaterials Corporation资助。
如实施例中所述包括7.5%(重量)生物活性玻璃的7.5%洁齿剂具有如下组成:

Claims (9)

1.一种用于使牙齿变白的方法,它包括每天使需要变白的牙齿与牙齿变白有效量的粒状生物活性和生物相容的玻璃进行接触,直到所述牙齿Vita色标的至少一个色调变亮,所述粒状生物活性和生物相容的玻璃包括二氧化硅或氢氧化硅以及任选一种或多种选自Na、K、Ca、Mg、B、Ti、Al、P、N或F的元素,其中所述生物活性和生物相容的玻璃按重量百分比计包括:
          SiO2           40-60
          CaO             10-30
          Na2O           10-35

6.根据权利要求1-3中任一项的方法,其中所述粒状生物活性和生物相容的玻璃包括小于5mm的颗粒。
7.根据权利要求1-3中任一项的方法,其中所述粒状生物活性和生物相容的玻璃包括小于2mm的颗粒。
8.根据权利要求1-3中任一项的方法,其中所述有效量为0.02-0.3克。
