背景技术

WO 2013/041042公开了吡唑甲酰胺作为Janus激酶抑制剂,其可用于治疗类风湿性关节炎、哮喘、慢性阻塞性肺疾病(COPD)和癌症。该公开的化合物具有下式:

发明内容
本发明的一个实施方案是药物组合物,其包含药学上有效量的1-[(3R,4S)-4-氰基四氢吡喃-3-基]-3-[(2-氟-6-甲氧基-4-吡啶基)氨基]吡唑-4-甲酰胺和生育酚、聚乙二醇(PEG)和二羧酸的酯的表面活性剂。
本发明的另一个实施方案是治疗特应性皮炎的方法,其包括向有需要的动物口服施用有效量的上述药物组合物。

具体实施方式
申请人已经开发了1-[(3R,4S)-4-氰基四氢吡喃-3-基]-3-[(2-氟-6-甲氧基-4-吡啶基)氨基]吡唑-4-甲酰胺的制剂,式(I)化合物:
式(I)化合物是选择性Janus激酶(JAK)抑制剂。但是,式(I)化合物是水溶性差的化合物。通常,药物活性剂的水溶性差导致狗中的口服生物利用度差,并因此导致药物的生物学功效差。申请人现在已经发现,与不含表面活性剂的制剂相比,式I化合物的固体药物组合物中包含生育酚、聚乙二醇(PEG)和二羧酸的酯的表面活性剂产生的制剂在口服施用于动物后提供了化合物的改善的生物利用度。
本发明的一个实施方案是固体药物组合物,其包含:
a)药学上有效量的式(I)化合物
或其盐或溶剂化物;以及
b)药学上可接受的载体,其中所述载体包含表面活性剂,所述表面活性剂是生育酚、聚乙二醇(PEG)和二羧酸的酯。
在一个实施方案中,式(I)化合物以结晶形式存在。
在一个实施方案中,组合物中式(I)化合物的量为约0.5至约10%或约1.0至约5.0%或约2%(w/w)。
生育酚是一类有机化学化合物,其中许多具有维生素E活性。有四种生育酚形式(α(alpha)、β(beta)、γ(gamma)和δ(delta)(参见下文)。它们的特征在于具有羟基和疏水侧链的苯并二氢吡喃环,并且苯并二氢吡喃环上甲基的数目和位置不同。
在一个实施方案中,生育酚是α生育酚,特别是D-α-生育酚。
聚乙二醇是具有H-(O-CH2-CH2)n-OH结构的聚醚化合物。
PEG的分子量优选在100至10,000Da的范围内。在一个实施方案中,PEG的分子量为1000Da。
二羧酸是含有两个羧基官能团(-COOH)的有机化合物。二羧酸的分子通式可以写为HO2C-R-CO2H,其中R可以是脂肪族或芳香族的。二羧酸优选地是C2-C20烷基,在一个实施方案中,二羧酸是琥珀酸,一种C4烷基二羧酸。
在一个实施方案中,这样的表面活性剂是D-α-生育酚聚乙二醇1000琥珀酸酯(TPGS或维生素E TPGS)。TPGS是由聚乙二醇1000和维生素E琥珀酸的酯化形成的。因此,在TPGS中,PEG的分子量是1000Da,生育酚是α生育酚,并且二羧酸是琥珀酸。
在一个实施方案中,组合物中TPGS的量为约0.5至约10%或约1.0至约5.0%或约1.0%至约2.5%(w/w)。

药物组合物可进一步包含一种或多种润滑剂。润滑剂减少了形成的片剂和用于形成片剂的模具壁之间的摩擦,由此使片剂更容易从模具中取出。润滑剂的示例是硬脂酸镁、滑石、氧化硅和硬脂富马酸钠。在一个实施方案中,润滑剂是硬脂酸镁。
药物组合物可进一步包含一种或多种助流剂。助流剂用于改善流动性。在一个实施方案中,助流剂是氧化硅、滑石或其混合物。
药物组合物可进一步包含一种或多种填充剂/压缩助剂。填充剂/压缩助剂用于增加具有低剂量活性成分的药物剂型的尺寸或体积并增加剂型(例如片剂)的机械强度。填充剂的示例是微晶纤维素(MCC)(Avicel PH102)、无水乳糖、乳糖一水合物(Fast Flo 316)、淀粉、多元醇(例如山梨糖醇、甘露糖醇、麦芽糖醇)、麦芽糖糊精、右旋糖、磷酸钙和硫酸钙。在一个实施方案中,填充剂是微晶纤维素、乳糖一水合物纤维素或其混合物。
药物组合物可进一步包含一种或多种崩解剂。一旦与液体接触,崩解剂有助于使片剂破碎成更小的片。崩解剂的示例是羟基乙酸淀粉钠、交联羧甲基纤维素钠和交聚维酮。在一个实施方案中,崩解剂是羟基乙酸淀粉钠(A型)。
药物组合物可进一步包含一种或多种粘合剂。粘合剂用于增加剂型(例如片剂)的机械强度。粘合剂也用于促进(湿或干)制粒过程中的颗粒形成。颗粒形成增加了最终混合物的(药物)含量均匀性和流动性。粘合剂的示例是PVP、羟丙基甲基纤维素(HPMC)和羟丙基纤维素(HPC)。在一个实施方案中,粘合剂是羟丙基纤维素(HPC)。

在实施方案中,药物组合物是固体,优选胶囊或片剂。
制备TPGS制剂的方法
TPGS是一种蜡质材料,并且难以添加到粉末混合物中。实际上,TPGS可以在高剪切制粒机或热熔挤出机中作为融化的液体或水溶液被添加至粉末混合物中。
在湿法制粒方法中,使用制粒流体(例如水或粘合剂溶液)将较小的颗粒结合在一起以形成颗粒。湿法制粒方法的示例是流化床制粒和高剪切湿法制粒。
在高剪切制粒机中,将TPGS水溶液或TPGS-粘合剂溶液喷雾到制剂中除润滑剂之外的固体组分上。随后将干燥湿物质去除水以获得干燥的颗粒。研磨颗粒并与润滑剂混合。颗粒外赋形剂(例如填充剂和崩解剂)也可以在润滑之前加入。将所得润滑混合物压制成片剂。
或者,可将TPGS加热至熔融状态并使用高剪切制粒机或热熔挤出机将其掺入制剂中。将由活性成分、填充剂、崩解剂和TPGS组成的颗粒冷却至室温,但不需要去除水。类似于湿法制粒,然后可研磨颗粒并在压制前与润滑剂共混。颗粒外赋形剂(例如填充剂和崩解剂)也可以在润滑之前加入。
颗粒可以在流化床或托盘干燥中干燥,与规模无关。流化床干燥比托盘干燥更有效。在制备本发明药物组合物的背景下考虑使用任一种方法。
基于最终药物组合物中期望的TPGS浓度确定TPGS溶液浓度。最终制剂中TPGS的期望浓度越低,TPGS溶液可以越稀。基于制剂中期望的TPGS含量和产生具有用于下游加工的期望属性的颗粒所需的制粒液体的量来确定TPGS溶液的浓度。
本发明的一个实施方案是制备药物组合物的方法,所述方法包括:
i)形成式(I)化合物与填充剂和崩解剂的粉末混合物;
ii)用TPGS水溶液喷洒所述干粉并混合以结合所述溶液与所述粉末;
iii)干燥步骤ii)的产物以产生颗粒;
iv)减小所述颗粒的尺寸;
v)润滑步骤iv)的产物;以及
vi)将步骤v)的产物压制成片剂以制备药物组合物。
在一个实施方案中,步骤i)的粉末混合物的形成或步骤ii)的混合是使用高剪切制粒机完成的。
本发明的另一个实施方案是制备药物组合物的方法,所述方法包括:
i)形成式(I)化合物与填充剂和崩解剂的粉末混合物并加热该粉末混合物;
ii)融化TPGS并将其与步骤i)的经加热的粉末混合物结合;
iii)冷却步骤ii)的产物以产生颗粒;以及
iv)减小步骤iii)的所述颗粒的尺寸;
v)润滑步骤iv)的产物;以及
vi)将步骤v)的产物压制成片剂以制备药物组合物。
在一个实施方案中,式(I)化合物在步骤i)中进一步与粘合剂结合。
在一个实施方案中,填充剂是微晶纤维素、乳糖或其混合物。
在一个实施方案中,崩解剂是羟基乙酸淀粉钠。
在一个实施方案中,润滑剂是硬脂酸镁。
在一个实施方案中,粘合剂是羟丙基纤维素。
治疗方法
本发明的一个实施方案是治疗特应性皮炎的方法,其包括向有此需要的动物施用有效量的式(I)化合物的药物组合物
或其盐或溶剂化物;和药学上可接受的载体,其中所述载体包含表面活性剂,所述表面活性剂是生育酚、聚乙二醇(PEG)和二羧酸的酯。
一种治疗特应性皮炎的方法,其包括向有此需要的动物施用式(I)化合物的药物组合物

所述药物组合物是口服施用的。
在一个实施方案中,待治疗的动物是伴侣动物哺乳动物。在另一个实施方案中,伴侣动物是狗、猫或马。在另一个实施方案中,伴侣动物是狗。
在本发明的一个实施方案中,施用于动物的活性成分的剂量为约0.1mg/kg至约2.0mg/kg、约0.2至约0.8mg/kg、约0.3至约0.7mg/kg、约0.5mg/kg、约1.0mg/kg。

式(I)化合物和TPGS的药物组合物可以与抗组胺药、抗生素、止痒剂和神经酰胺组合施用。这些组合可以同时或依次施用。
实施例
实施例1-表面活性剂选择和对比实施例
为了增加化合物的溶解度并促进生物利用度的提高,筛选了许多表面活性剂。下表列出了1-[(3R,4S)-4-氰基四氢吡喃-3-基]-3-[(2-氟-6-甲氧基-4-吡啶基)氨基]吡唑-4-甲酰胺在所测试的表面活性剂溶液中的溶解度。
由于SLS对1-[(3R,4S)-4-氰基四氢吡喃-3-基]-3-[(2-氟-6-甲氧基-4-吡啶基)氨基]吡唑-4-甲酰胺的强增溶作用及其易于掺入粉末混合物中的能力,通过将3%w/w SLS添加至制剂中进行药代动力学(PK)研究。含有1%活性成分的测试制剂的组成如下。还制备了含有0.2%活性成分的测试制剂。

实施例2
通过湿法制粒制备具有4%活性成分(API)和5%TPGS的制剂。将34g的15%w/wTPGS水溶液喷洒至大约90g含有4g API的粉末混合物上。通过在40℃烘箱中托盘干燥过夜去除水。研磨颗粒并将其与约1g硬脂酸镁混合。调节硬脂酸镁的重量以保持最终混合物的1%w/w。

实施例3(2%活性成分,2.5%TPGS融化制粒):
TPGS在烘箱中预熔化。预先加热制剂中除硬脂酸镁之外的所有剩余固体。将2.5g熔化的TPGS添加至大约95g含有2g API的粉末混合物中。在环境条件下,在托盘上空气冷却至室温。研磨颗粒,然后与大约1g硬脂酸镁混合。调节硬脂酸镁的重量以保持最终混合物的1%w/w。
实施例4-含有TPGS的制剂在狗中的生物利用度
TPGS被评估为表面活性剂,其可促进活性成分1-[(3R,4S)-4-氰基四氢吡喃-3-基]-3-[(2-氟-6-甲氧基-4-吡啶基)氨基]吡唑-4-甲酰胺在狗中的生物利用度的提高。如上所述制备制剂。该制剂的组成如下:
略微调节微晶纤维素和乳糖一水合物的百分比以适应活性成分浓度的变化。
针对1%至10%TPGS w/w的TPGS制剂范围评估式(I)化合物在狗中的生物利用度。所有制剂的剂量为体重的1mg/kg。图2显示用10%w/w TPGS,5%w/w TPGS和无TPGS配制化合物时的比较药代动力学(PK)曲线的数据。不包含TPGS的制剂的Cmax被测量为约0.3μM。5%TPGS制剂和10%制剂的Cmax分别为约0.6μM和0.4μM。参见图2。
这些结果表明,虽然包含TPGS对于促进生物利用度是重要的,但是降低TPGS的浓度可能是有益的。因此,进行了比较5%w/w至2.5%w/w和1%w/w TPGS的后续研究。结果如图3所示。在该研究中,不包含TPGS的制剂的Cmax被测量为约0.2μM。1%、2.5%和5%TPGS制剂的Cmax分别为约0.3μM、0.3μM和0.25μM。参见图3。再次,这里的数据证实较低浓度的TPGS改善了生物利用度。
由于研究地点之间的差异,不可能直接将图2和3的数据相互比较或与图1的数据进行比较。但是,在两个TPGS研究中,含有TPGS的制剂比不含TPGS的制剂具有更大的生物利用度。考虑到使用SLS的生物利用度研究的结果,这是出乎意料的,其中尽管SLS对于式(I)的化合物具有大得多的溶解度,但相对于不含SLS的制剂,SLS制剂不改善式(I)的化合物的生物利用度。在1%浓度下,SLS具有0.1952mg/mL的式(I)化合物的溶解度,并且在1%浓度下,TPGS具有0.099mg/mL的式(I)化合物的溶解度。
实施例6(3%活性,3.5%TPGS)
通过高剪切湿法制粒制备具有3%活性成分(API)和3.75%TPGS的制剂。将含有11%w/w TPGS和15%聚乙烯吡咯烷酮的溶液喷洒至大约2kg含有3%w/w API的粉末混合物上。在流化床干燥器中干燥湿颗粒,直至干燥损失为≤2.5%。研磨干燥的颗粒并将其与大约另外的A型羟基乙酸淀粉钠和硬脂酸镁混合。调节颗粒外添加的A型羟基乙酸淀粉钠·重量以保持最终混合物的3%w/w。调节硬脂酸镁的重量以保持最终混合物的0.5%w/w。
实施例7
通过高剪切湿法制粒制备具有3%活性成分(API)和3.75%TPGS的制剂。将含有约10%w/w TPGS的溶液喷洒至大约2kg含有3%w/w API的粉末混合物上。在流化床干燥器中干燥湿颗粒,直至干燥损失为≤2.5%。研磨干燥的颗粒并将其与大约另外的A型羟基乙酸淀粉钠和硬脂酸镁混合。调节颗粒外添加的A型羟基乙酸淀粉钠重量以保持最终混合物的3%w/w。调节硬脂酸镁的重量以保持最终混合物的0.5%w/w。

实施例8-功效研究
在被诊断为特应性皮炎的狗中,在一项隐蔽且随机化的概念验证研究中评价化合物。该研究的目的是评价化合物对客户拥有的狗的特应性皮炎的功效和耐受性。在两个剂量下评价化合物,并与安慰剂对照进行比较。狗将每天口服施用两次,持续长达14天,随后每天施用一次,持续长达28天,或每天施用一次,持续28天,并且将使用瘙痒视觉模拟量表(PVAS)和犬特应性皮炎程度和严重程度指数(CADESI-4)评分工具分别评价瘙痒和皮肤病变。

8.根据权利要求1-7中任一项所述的药物组合物,其中所述填充剂是微晶纤维素、乳糖或其混合物。
9.根据权利要求1-8中任一项所述的药物组合物,其中所述润滑剂是硬脂酸镁。
10.根据权利要求1-9中任一项所述的药物组合物,其中所述崩解剂是羟基乙酸淀粉钠。
11.根据权利要求1-10中任一项所述的药物组合物,其中所述粘合剂是羟丙基纤维素。
12.一种治疗特应性皮炎的方法,其包括向有此需要的动物施用有效量的根据权利要求1-11中任一项所述的药物组合物。
13.一种治疗特应性皮炎的方法,其包括向有此需要的动物施用根据权利要求1-11中任一项所述的药物组合物,其中所述式(I)化合物的有效量为约0.1至约2.0mg/kg体重。
14.根据权利要求12-13中任一项所述的方法,其中所述施用是口服。
15.根据权利要求12-14中任一项所述的方法,其中所述动物是伴侣动物,优选狗。
16.根据权利要求12-15中任一项所述的方法,其中所述组合物每天施用一次或每天施用两次,优选每天施用一次。
17.根据权利要求12-16中任一项所述的方法,其中所述组合物与食物一起施用。
18.根据权利要求12-17中任一项所述的方法,其中所述组合物每天施用两次,持续14天,然后每天施用一次,持续14天。
19.根据权利要求12-18中任一项所述的方法,其中所述组合物每天施用一次,持续28天。

21.一种制备根据权利要求1-11中任一项所述的药物组合物的方法,所述方法包括:
i)形成式(I)化合物与填充剂和崩解剂的粉末混合物;
ii)用TPGS水溶液喷洒所述干粉并混合以结合所述溶液与所述粉末;
iii)干燥步骤ii)的产物以产生颗粒;
iv)减小所述颗粒的尺寸;
v)润滑步骤iv)所述的产物;以及
vi)将步骤v)所述的产物压制成片剂以制备所述药物组合物。
22.根据权利要求21所述的方法,其中形成步骤i)所述的粉末混合物或步骤ii)所述的混合是使用高剪切制粒机完成的。
23.一种制备根据权利要求1-11中任一项所述的药物组合物的方法,所述方法包括:
i)形成式(I)化合物与填充剂和崩解剂的粉末混合物并加热该粉末混合物;
ii)融化TPGS并将其与步骤i)所述的经加热的粉末混合物结合;
iii)冷却步骤ii)所述的产物以产生颗粒;以及
iv)减小步骤iii)的所述颗粒的尺寸;
v)润滑步骤iv)所述的产物;以及
vi)将步骤v)所述的产物压制成片剂以制备所述药物组合物。
24.根据权利要求21-23中任一项所述的方法,其中在步骤i)中将所述式(I)化合物进一步与粘合剂结合。
25.根据权利要求21-24中任一项所述的方法,其中所述填充剂是微晶纤维素、乳糖或其混合物。
26.根据权利要求21-25中任一项所述的方法,其中所述崩解剂为羟基乙酸淀粉钠。
27.根据权利要求21-26中任一项的方法,其中所述润滑剂是硬脂酸镁。
28.根据权利要求24-27中任一项所述的方法,其中所述粘合剂是羟丙基纤维素。
