很久以前人们就知道把一种涂油脂的界面直接和伤口相接触可以促进伤口愈合,而同时必须将该伤口和吸湿性的敷布隔开。常用于复盖皮肤伤口的产品之一是Solvay药品公司出售的“Lumiere”薄纱油脂敷布。但是这种由胶粘纤维制成的大孔网眼并用凡士林和秘鲁香脂制成的润滑脂涂敷的产品其缺点是经常粘附在伤口上,或当揭去该敷布后在处理的器具上或伤口上就失去了油脂。还有其它出售或文献叙述的产品可以起到上述薄纱油脂敷布的相同作用,如Smith and Nephew公司出售的商标名称为JELONET的敷布是一种用石蜡浸渍过的棉纱网,商标名称为ADAPTIC(Johnson and Johnson)的敷布是一种用水包油型乳液浸渍过的粘胶编织品。这些不同产品的性能和上述“Lumiere”薄纱油脂敷布很类似。还有一种已知的产品是根据EP 261,167Molnlycke公司生产的商标名称为MEPITEL的产品,它是一种用疏水性硅氧烷凝胶涂敷的可延伸的弹性网,能够粘着在干燥的皮肤上;但这种产品使用很少,因为硅氧烷凝胶价昂而粘附性又强。在相同领域已公开的文献中,如EP 497,607推荐采用一种亲水的粘性树脂涂敷的网状敷布,但是和前面的产品相同,它对伤口周围的皮肤有强粘附性,因而除去敷布时经常很疼。EP 521,761所叙述的一种医疗敷布是由高度增塑的三元嵌段弹性体连续层所组成的,其中间一个嵌段是饱和的。此连续层形成了一种吸收性的不粘附并且高度疏水的敷布。EP 567,704叙述了类似的产品,它是在吸湿层中浸渍水凝胶物料而形成的一种抗菌敷布,该吸湿层遇水会膨胀;其目的是要用于非渗出性和严重感染的伤口。在相同应用领域中,EP 420,841所叙述的一种敷布是设计成能释放出活性成分的由一种能与微粒结合的粘合剂带做成的。这些微粒是由疏水基体和分散其中的含活性成分的亲水颗粒所组成。EP 752,840申请了专利的敷布是一种在纤维基质中浸渍一种形成水凝胶的热塑性聚合物所制成的。这种能生成水凝胶的聚合物是由亲水基团和疏水基团共聚而制得。共聚产物浸渍在例如由棉纱网组成基质的纱中。所得产品可以认为既有疏水性又有很强的亲水性,因此该水凝胶有高度的吸湿性。EP 617,938报导了一种由吸收层和一个含水解胶体的不连续聚合物层组成的敷布,目的是获得更大的吸收能力且不会渗漏。
但是,以上这些不同的产品不完全令人满意,因为价格高昂,或是因为难于把敷布放在应放的位置或难以拿掉,还因为很难达到预期效果-伤口全面而快速地愈合。
至于最后一点,那无疑是最重要的,人们知道只有当敷布不吸附新再生的组织并能在除去渗出液时同时保持伤口潮湿,才能顺利地治愈伤口。
在使用过程中,已知的薄纱油脂型敷布经常强烈粘附在伤口上,这通常意味除去敷布时很痛因而明显地降低治愈的速度,这是由于除去敷布时又造成了损伤。此外,已注意到如果敷布太具疏水性,伤口就干燥,而如果敷布是完全亲水性的则与伤口接触的一层就溶胀而闭塞接触层中的任何通道,使伤口浸润。
因此,在医务界所使用的与伤口接触的一种敷布是要求其完全不粘附在再生的组织上并能保持有利于愈合的最佳湿度条件而同时又要避免浸润的敷布。
发明内容
本发明的目的是在技术上解决上述问题,解决的方法是采用含一种开孔的网状柔性织物类型的无菌无粘性的敷布,该织物的纱是用粘着的但不粘附的凝胶涂复的,因而留下基本上未堵塞的孔,其特征在于该凝胶是由含少量水解胶体的亲水颗粒分散体的高度增塑的疏水性弹性基体所形成的。
具体的,本发明提供了一种要用于直接与伤口接触的无菌无粘性的敷布,包含一种开式网眼的柔性织物,该织物包含用粘合的但无粘着力的凝胶涂覆的纱,因而使网眼基本上未被堵塞,其特征在于,该纱是疏水性的连续的低延伸度的单纤丝,而该凝胶是由高度增塑的疏水弹性基体所形成的,该基体中按重量计包含100份三元嵌段弹性体每1000至2000份油状增塑剂和0至400份药用凡士林含100份三元嵌段弹性体,并含3至20%按凝胶重量计的水解胶体的亲水颗粒的分散体。
根据本发明的一个优选实施方案,生成织物的原料是一种有长的单纤丝或连续单纤丝的合成纤维,更合适的是聚酯纱。
根据本发明的另一优选实施方案,该疏水性基体是一种S-EB-S型的高分子量三元嵌段弹性体。
根据本发明的另一优选实施方案,该弹性体是用凡士林和石蜡油的混合物进行增塑的,其比例至少占凝胶重量的65%。
根据本发明又一优选实施方案,分散在凝胶中的水解胶体是羧甲基纤维素的钠盐。
具体实施方式
人们已注意到加入并分散在粘性的弹性疏水基体中的少量水解胶体会使所得到的凝胶表面具有亲水性,同时用油高度增塑的基体仍为疏水性,而不导致吸水。与直接接触伤口的凝胶表面的两亲性相比,这种疏水和亲水性能非常有利于伤口在愈合过程中其表面保持最佳的湿度,而存在的油脂使伤口和敷布隔离的结构加快了伤口愈合过程,同时又完全避免了敷布和伤口的粘合。
采用在潮湿环境下稳定的粘性和弹性的凝胶时,会使织物的纱恰当地互相分离,在敷布位置不变时,纱与伤口完全隔离,因此任何时间都不存在纱和再生的组织之间直接接触的危险。但如果纱进入伤疤,则当除去敷布时就会使组织破裂而疼痛。加入大量的油性增塑剂会使疏水性基体有良好的弹性和柔韧性,因而可以得到很柔顺的敷布。它很适用于所保护的表面,并且由于其强粘性和基体的弹性大于涂复后的纱的弹性,因此敷布任何时候都不会损坏。此外,油性增塑剂,优选矿物油和药用凡士林的混合物,呈油脂状并使敷布的表面不具有粘附性;这使弹性体和伤口之间仅有非常有限的直接接触,基本上所有的接触都是通过油状化合物发生的,这种接触比伤口活组织和弹性聚合物之间的接触较能被接受。
在凝胶中分散的少量水解胶体能降低凝胶的亲水性,从而能足够维持潮湿的环境而有利于治疗,但又不够使凝胶能吸收大量的水。实际上,也并不希望吸收大量的水,因为这会导致凝胶溶胀逐渐堵塞敷布结构中的网眼。敷布就逐渐成为封闭的,渗出液无法排出而易被浸软。
根据本发明的一个特征,医用敷布有一个载体是用柔软材料制成的纱的编织物。制纱的材料的延伸性很小而且没有弹性。该载体是一种具有宽的开式网眼的织物。可以用编织或针织的方法制成方形或多边形均匀大小的开式网眼。在编织时,可以用旋转纱的方法使网眼固定,得到较好的尺寸稳定性。网眼的大小可以是在单位面积为约0.5至10毫米2,优选0.5至3毫米2的编织物中的开式网眼比例(网眼面积对总面积的比例)为约50至90%。用于制备织物的纱优选无弹性并且伸长性不高,即伸长率或延伸率小于35%的连续复丝纱。“连续复丝纱”一词应理解为一种由一根或多根长的加捻单纤维(长丝)制成的纱;选择长的单纤维能够避免短纤维易从载体上脱离分散至与伤口接触面积很近的地方。因为相同的原因,制纱的原料优选疏水型的人造或合成的材料,其成分为聚酯、聚酰胺和乙酸纤维素,可以制得的长丝和纱其原纤维比得到的该纤维少得多。选择某种合成材料,如聚酯,可以用热法定型载体的大网眼结构。大网眼织物优选用同样性质的纱制作,但也可以选用性质不同的经纱和纬纱制作的织物。最后,很低延伸率的无弹性物料(如聚酯)的另一优点是在用凝胶涂复织物的纱时比较容易加工。
根据本发明的另一特征,用油脂状不粘附的凝胶涂复的织物载体,使大多数载体的孔不被堵塞。该凝胶包含用不溶于水的油或脂高度增塑的疏水三元嵌段热塑弹性体基体并含有分散的水解胶体颗粒。这种疏水弹性柔性的基体和分散的亲水水解胶体颗粒的组合物使敷布的性能特别有利于治疗,即疏水性基体使敷布具有优良的物理稳定性,可以在伤口上保持数天不动,或不会分散,也不会粘附在新生的组织上,而亲水性组分使它能维持在有利于治疗的湿度,同时防止伤口干燥,因而既能阻止结硬皮又能防止敷布粘附伤口。
在实践中,选用的是一种S-EB-S或S-EP-S三元嵌段型疏水的合成热塑性弹性体,即聚苯乙烯型嵌段和聚乙烯-丁烯或聚乙烯-丙烯型的聚烯烃嵌段的共聚产物。根据本发明,为了得到一种疏水的物料,选用了具有中等或高分子量,布鲁克菲尔德(Brookfield)粘度至少300厘泊(在25℃下测定,10%的甲苯溶液)的S-EB-S或S-EP-S型三元嵌段弹性体。
这种类型弹性体的选定,结合油型增塑剂,能得到一种高粘着性的弹性凝胶油脂状外观,但实际上不显示粘附性(在玻璃片上测得的粘性强度小于8g/5cm)。
疏水热塑性弹性体必需增塑而加入一种疏水性油的组分。为了做到这一点,应选择一种矿物油或植物油其与上述弹性体有良好相容性并且对皮肤组织有公认的耐受性。优选使用低粘度石蜡和环烷基化合物的石蜡油或其和药用凡士林的混合物。
在很适用于增塑弹性体的产品中可举出的例子有壳牌公司出售的牌号名称为ONDINA的石蜡油,特别是标准ONDINA 15油,再加上根据法国药典的凡士林,可用做本发明的一种优选配方。三元嵌段型弹性体必需加入一种增塑油,其用量比例应使可伸展的弹性疏水凝胶的断裂伸长率至少200%,回缩率至少为50%。通常,用1000至2000份低粘度油和0至400份凡士林增塑100份弹性体就可以得到足够粘结性、很适用的弹性凝胶。一种优选实施方案的配方是100份高分子量的S-EB-S弹性体,如壳牌公司的KratonG1651,1600份由95%不含芳烃衍生物的低粘度石蜡油和5%符合法国药典的凡士林所组成的疏水油性增塑剂。这个实例是指出一种途径,当然也可以用与此不同的物料和不同的比例制成本发明的凝胶。
前面已谈到的是加入一种水解胶体的细小亲水颗粒使其分散在疏水基体中。“水解胶体”一词是指本领域的技术人员已知的能吸收水的化合物;它们有果胶,藻酸酯和羧甲基纤维素,后者是以钠盐的形式优选用于本发明。水解胶体必需是固体细粉状,例如它的平均直径小于100微米,更好的是小于50微米。水解胶体在凝胶配方中的用量取决于所用的水解胶体的类型。但是,值得指出的是少量这种化合物可以使凝胶具亲水性而足以维持一种有利于治疗的潮湿环境,同时又防止伤口干燥而和敷布粘附在一起。如果采用羧甲基纤维素钠(SCMC),只加入2至3%的SCMC就能得到一种使伤口表面轻微潮湿并滑溜的凝胶。超过20%的用量会妨碍凝胶的粘着性,显著地增强了它的亲水性,并未改善其预想的结果,反而增加了基体溶胀性,因而敷布小孔被堵塞的危险增加了。在实践中,细粉状的水解胶体的添加量为3至20%(相对于疏水基体重量的重量),在高温搅拌状态下加至疏水基体中,因而均匀地分散在凝胶中。充分混合疏水增塑的弹性体就生成水解胶体分散于其中的基体,因而得到两亲性凝胶的特性。这种两亲的凝胶能维持足够潮湿的环境而有高含量水解胶体的化合物不具有吸湿性。根据另一种不同的方法,可以不加水解胶体而配制成疏水性凝胶,涂覆在纱上也就涂覆在载体上了,能使网孔不被堵塞。当凝胶的温度尚热时,把细微颗粒的水解胶体喷在凝胶的表面上。根据这种方法,约0.2至0.5%重量的水解胶体已足够得到一种在表面上有两亲性质的无粘性敷布,而且敷在湿伤口上使用情况良好。这种方法还能制成一种非对称的敷布,只把水解胶体的微粒喷在准备直接与伤口接触的一面,这样就能得到一面亲水一面疏水的敷布。
在实践中,该组合物通常还包含一种或更多的抗氧剂或稳定剂,如Ciba-Geigy公司出售的商标名称为IRGANOX的苯酚化合物。
如果需要增强敷布对伤口的愈合性能以达到特定的治疗效果,也可以向凝胶配方中加入一些活性成分。为此,可以把具有抗菌性质的化合物分散到凝胶中,这类化合物有磺胺嘧啶银、抗生素如新霉素或多粘菌素,和非甾族或甾族的抗炎药如丙炎松化合物。
根据本发明优选实施方案之一,用凝胶涂复大网眼织物使织物中的纱得到涂复,而使大部分的网眼未被堵塞。
前面已说明,可以采用一种编织或针织的载体,其具有大的矩形、方形或多边形的网眼,开孔度相当于每厘米有4至20个网眼。该织物具有开孔比例(开孔面积和总面积的比例)为50至90%。优选一种连续的复丝纱,用于制造载体,并且为了制造本发明的优选载体,可以选择具有疏水性的延伸度低于35%的人工或合成的原料来制纱。该纱可以是聚对苯二甲酸乙二酯类型的聚酯、聚酰胺或乙酸纤维素;优选采用的织物具有热定形的大孔,由连续聚酯纱(聚酯或聚对苯二甲酸乙二酯纤丝)制成,如市售名称为薄纱罗,每平方米的克数约30至80g/m2的织物。这些织物实际上在经纬方向上均不可伸长,优点是比弹性织物更易加工,能获得更均匀涂复的纱。
优选用无溶剂的热混合方法(所谓的热融法)制造无粘性的凝胶,用油性增塑剂和抗氧剂掺入该弹性体,再加入细粉状的水解胶体。如果要用活性成分,则可将其加入。
用凝胶涂敷织物的工艺必需使纱适当地包封在凝胶中,同时织物大部分的网眼又不被凝胶封闭。根据所用载体的结构,凝胶的用量可在约50至300g/m2,优选60至160g/m2中变化。从凝胶的成分考虑,用加热涂复法而不用溶剂,织物网连续地经过第一个涂布辊,涂复一层熔融的凝胶,厚度用刮刀预先确定,再经过第二个辊除去网孔内的凝胶。然后将该仅在纱上涂复了凝胶的织物网在某一区域内用向上流动的空气冷却,使凝胶不能流动而均匀地分布在纱的周围。如果必要,可以采用能产生层流空气流的系统,用来调整在纱周围凝胶的分布,并且打通上一个步骤中尚未打开的网眼。
根据另一不同方法,织物的网状物经过一个140-150℃熔融的凝胶浴;将已涂复熔融凝胶的网状物再经过两个预先确定间隙的固定辊互相挤压,除掉多余的凝胶。在纱上残留下凝胶的量基本上取决于两个固定辊的间隙。已涂复好的网状物经冷却后,按上述相同的方法处理。
然后,不粘附敷布的冷却的网状物用两层保护膜如聚酯薄膜覆盖。由于敷布网状物的不粘附性,这两层膜不需要进行不粘附性处理,它们的作用只是从主包装中更容易取出,并易于用手把敷布放在伤口上。然后,网状物被切割成单独的大小适用的敷布,装入密封的袋中并消毒。
本发明的无粘性敷布与目前已知的界面如“Lumiere”薄纱油脂敷布的用法相似。通常,敷布的放置要直接和伤口相接触,可以用单层也可以多层;载体和凝胶的顺从性可以使敷布延伸到健康皮肤的周边而合适地敷在整个伤口上。如果伤口渗出严重,可以用一吸湿性衬垫覆盖在无菌敷布上,并用粘胶带或条将这种叠合物贴在伤口旁的健康皮肤周围将其固定。这样制成的敷布可以保持很长一段时间,因为高度粘着性的凝胶不会分离,而少量水解胶体会在伤口表面保持一定湿度,该湿度足够避免伤口干燥。此外,由于所用凝胶无粘性,能够无危险的除去与凝胶不粘的吸收衬垫,不用移去无菌敷布来监控伤口的变化。敷布是半透明的,但仍可以查看伤口,有时还必需拿掉敷布以便肉眼仔细观察,或在该治疗过程中要直接用药物处理此一部位;除掉敷布可以无任何疼痛,也可不伤及新生的组织,因为凝胶既不粘附伤口的表面也不粘附伤口周围的皮肤。另外,由于凝胶的高度粘着性,使织物的纱夹在凝胶中,SCMC的存在会维持轻度潮湿的环境,敷布可整体地移去而不会留下颗粒或油脂。现在某些市售的产品就是这样。因此,清洁伤口也就容易多了。所有这些优点,即优良的粘着性以及对湿表面和干燥皮肤的不粘性,结合起来就得到有利于伤口愈合的最佳条件。通过以下的解释性实施例可充分地理解本发明的范围,但它们不能看做是对本发明范围的限制。
实施例1
在150℃下,混合搅拌8kg链烷烃油(ONDINA15,壳牌公司出售),1kg(约凝胶总重量的5%)高分子量S-EB-S弹性体(KRATONG 1651壳牌公司出售)和25克抗氧剂(IRGANOX 1010)制备凝胶。当混合物均匀后,加入1kg(约凝胶总重量的5%)凡士林(其品位符合法国药典或Codex)和7.2kg(约凝胶总重量的75%)ONDINA 15油。搅拌30分钟后,温度降至130-135℃,加入3.04kg(约凝胶重量的15%)羧甲基纤维素钠(Aqualon公司出售,牌号为7H4XF)。搅拌40分钟后,凝胶可用于涂复织物的纱了。所用的织物是一种细度为33分特的经纬纱所织成的热定型薄纱罗聚酯(聚对苯二甲酸乙二酯),网眼孔为方形约0.8至1mm2;织物每平方米的克数为45g/m2(此织物由Texinov公司生产)。将该织物通过温度为135-145℃熔融的凝胶浴而涂复一层凝胶,再通过根据要求而预先确定间隙的两个固定的辊除去多余的凝胶。然后,网状的敷布用向上流动的冷空气冷却。沉积在织物的纱上的凝胶数量为约130g/m2。冷却的网状物在其两面与两层23微米厚的聚酯保护膜复合,然后切成所需形状的敷布片,每片都装入小袋,密封,用β辐射杀菌。
实施例2
这个实施例采用和实例1相同的织物和凝胶,但涂布辊的间隙调节到使得到的网状物每平方米含60克凝胶。
实施例3
按实施例1的方法生产出一种无菌敷布,但所用的水解胶体不同,代替羧甲基纤维素钠的是藻酸丙二酯,由Pronova Biopolymer公司的出售,名称为PROTONAL酯SDLB。
实施例4
按实施例1的方法生产敷布,但所用增塑剂的数量不同。此例用13.2kgONDINA 15石蜡油(即约凝胶总重量的65%)和3.04kg CodexA凡士林(即约凝胶重量的15%)。
实施例5
按实施例1的方法生产敷布,但所用的弹性体不同。此例中所用的是600g KratonG1651和400g KratonG1652的混合物。
实施例6
按实施例1中相同凝胶配方生产敷布,但所用的织物是Molypharm公司出售,牌号为548的矩形网眼胶粘纤维纱网。沉积在织物纱上凝胶的数量为约180g/m2
实施例7
按实施例1的方法生产敷布,但所用的弹性体不同,此实例中所用的是Kuraray公司生产,牌号为SEPTONR4055的S-EP-S型高分子量弹性体。用量则和实施例1相同。
实施例8
按实施例5的方法生产敷布,但所用的弹性体的凝胶不同,在搅拌结束时,加入200克细粉状的磺胺嘧啶银。这种有效成分使敷布具有抗菌性能。
对本发明的敷布的治疗功能和使用的容易性已进行了评定并与现在常用于伤口保护和护理的产品进行了比较。研究是在豚鼠皮肤表皮的伤口上进行的。
该实验是在每只豚鼠(每组5只动物)的肋部外侧做一个9平方厘米的方形皮肤表皮的伤口,使颈阔肌肉在原位不动。把进行研究的无菌敷布贴在伤口上,用一块无菌纱布网复盖并粘上胶带。在治疗的初始阶段,每两天换一次敷布,以后三天换一次。监测22天的愈合情况。评定每换一次敷布的效果,根据预先确定的判断标准测定敷布的水分含量、它对伤口的粘附性、发炎或出血状况以及伤口的治愈程度。
对本发明三个实施例生产的产品和两种市售的产品进行了比较研究。结果如下:
□对如上所述的实施例1的敷布,22天后一个伤口已完全治愈,三个伤口几乎治愈,最后一个伤口有一个很小的未愈合的面积(0.1cm2)。在整个治疗过程中未发现敷布粘附,无包涵物,敷布有些干燥;
□对实施例2的敷布,一个伤口19天后完全治愈,三个伤口22天后治愈,最后一个有一个小的未愈合面积(0.5cm2)。整个治疗过程中未发现敷布粘附及包涵物。敷布在治疗开始时很潮湿,治疗结束时有些干燥;
□对实施例6的敷布,22天后没有伤口治愈,其中一个接近愈合。其它的尚有小面积未愈合(0.5至0.6cm2)。很少的情况下敷布对伤口有轻度粘附,但无明显包涵物。敷布在所有愈合阶段都有些干燥了。
□第一个对比产品是胶粘纤维织物涂复凡士林并含约1%秘鲁香脂的市售薄纱油脂的敷布(根据1998 Vidal词典的说明),对此两个伤口22天治疗后已愈合了,其它三个伤口还有中等大小面积(0.6至1cm2)未愈合。对这种产品,可以看到一直粘在伤口上,特别是在前15天,在前6天还有包涵物。治疗过程中敷布基本上干燥了。
□第二种对比产品是一种在水包油乳液中浸渍过的胶粘纤维编织品制成的商品敷布(根据1998 Vidal词典中的附页),对这种敷布,22天后没有伤口愈合,两个伤口晚期愈合(剩下的伤口面积为0.1至0.3cm2),另两口伤口还大(约1cm2)。前15天经常发生敷布的粘附,但包涵物很少。在整个治疗过程中敷布的外表是干燥的。
从这些实验看来,本发明的无菌敷布,特别是当它们和疏水性合成纤维的织物结合使用时,伤口治疗得更快。此外,由于敷布既不粘附伤口又不粘附伤口周围的皮肤,因而在所有情况下都可以使患者无疼地、更舒适地更换敷布。

Claims (11)

1.一种要用于直接与伤口接触的无菌无粘性的敷布,包含一种开式网眼的柔性织物,该织物包含用粘合的但无粘着力的凝胶涂覆的纱,因而使网眼基本上未被堵塞,其特征在于,该纱是疏水性的连续的低延伸度的单丝,而该凝胶是由疏水弹性基体所形成的,该基体中按重量计包含100份三元嵌段弹性体每1000至2000份油状增塑剂和0至400份药用凡士林,并含3至20%按凝胶重量计的水解胶体的亲水颗粒的分散体。
