Description
本专利申请案涉及一种例如可以应用在宫腔镜中的用于冲洗体腔的装置,其在使用流量传感器的情况下确定导入和导出的冲洗液体量。基于这个流量测量,能够判断相关的液体量是否残留在患者体内。
例示性的宫腔镜是借助内窥镜对子宫内侧和输卵管管口进行的经阴道的内窥镜检查。在妇科中将宫腔镜用于诊断,但也将其用于手术干预。借助宫腔镜例如可以寻找出血性疾患的原因,肌瘤或者息肉,但也可以诊断癌瘤(例如子宫内膜癌或者宫颈癌)。例如也能够将肌瘤和息肉移除。此外也可以为了检查目的而将子宫内膜移除。可以使用高频电流来实现必要的止血。
在这些宫腔镜检查或手术期间,在使用冲洗溶液的情况下对子宫进行扩张和冲洗。这类冲洗溶液例如可以由氯化钠(食盐溶液)或者山梨糖醇/甘露醇(糖溶液)构成。
重要之处在于,在宫腔镜检查和干预的范围内,将使用的冲洗溶液尽可能完全地重新从身体移除。残留在身体中的冲洗溶液可能会引发中毒综合症(TUR综合症)。
根据现有技术中的宫腔镜冲洗装置,通过称重来确定导入的冲洗溶液与导出的冲洗溶液之间的差。例如,在治疗前、治疗期间和治疗后对包含冲洗溶液的储袋进行称重。将导出的冲洗溶液接收在接收容器中,同样地,在手术前、手术期间和手术后对该接收容器进行称重。根据导入的溶液与导出的溶液之间的差推断出可能残留在患者体内的冲洗溶液量。
此确定方法具有一系列缺点。在实践中会出现因操作人员而造成的称重误差,特别是在使用数个包含冲洗液的袋子或需要在治疗期间切换收集容器的情况下。由于物体在称上的放置,以及由于称重系统在手术期间的倚靠或者运动,还可能出现其他误差。此外,必要的设备和称重装置无法整合在其他医院设备中。为此需要单独的保持及存放位置。相关的现有技术参阅公开案DE 4417189 A1、EP0755282B1和EP 1175238 B1。其中,DE4417189 A1描述过一种类似的具有机械流量测量装置的用于宫腔镜的系统。该机械流量测量装置基于:导引流入或流出的液体经过具有锥形内孔的玻璃体。锥体内的悬浮体显示流速,根据该流速计算出总流量。但测量精度有待提高,特别是当液体包含血液、气泡和/或组织残余时。
有鉴于此,本发明的目的在于避免上述现有技术的缺点,并且提供一种装置,其更加易于使用,能够整合至常见的医院设备,且同时实现至少相当的、甚至更佳的结果。
本发明意外发现,通过使用超声波流量传感器,能够在宫腔镜的范围内精确地测量导入和导出的冲洗液体量。如下文所述,通过使用这类超声波流量传感器,与传统的称重技术相比能够提升测量精度。这一点特别出人意料,因为根据认知,流量传感器会灵敏地对溶液的变化,特别是组织残余、血液、气泡等的混入作出响应。然而事实表明,即使是在实际条件下,所述流量传感器也已足够精确,足以以足够的精度测定差值。
因此,本发明涉及一种用于冲洗体腔的医疗技术装置,
包含
至少一个包含冲洗液的储存容器,
通向医用泵的第一软管连接,
从所述医用泵通向体腔的第二软管连接,从所述体腔通向第二医用泵的第三软管连接,
从所述第二医用泵通向收集容器的第四软管连接,
其特征在于,
在所述第一或者第二软管连接上从外部插接有第一超声波流量传感器,
在所述第三或者第四软管连接上从外部插接有第二超声波流量传感器,
以及,所述超声波流量传感器是与一个评价单元连接,所述评价单元测量这两个超声波流量传感器的流量,并显示所述流量的差。
本发明的装置的基本结构如下:
储存容器包含必要的冲洗液。从所述储存容器将液体通过第一软管连接导引至一个医用泵,例如滚轮泵。所述医用泵的输出通过第二软管连接通向体腔(例如患者的子宫)。所述软管连接中的一个配设有超声波流量传感器。在使用滚轮泵的优选情况下,所述第一和第二软管连接可通过单独一个软管形成。其中,所述超声波流量传感器可以沿流动方向布置在所述滚轮泵前或者布置在所述滚轮泵后。
第三软管连接从体腔(例如子宫)通向第二医用泵,例如另一滚轮泵。作为替代方案,如图2所示,可以将一个真空泵连接至所述收集容器。所述真空泵可以整合在滚轮泵中,或者为外部的泵。在任何情形下,所述第二医用泵均将液体从子宫吸出。所述第二医用泵的输出与接收容器连通。所述第三或者第四软管连接也配设有超声波流量传感器。在使用滚轮泵的优选情况下,所述第三和第四软管连接可通过单独一个软管形成。在此,所述超声波流量传感器同样可以沿流动方向布置在所述滚轮泵前或者布置在所述滚轮泵后。
所述超声波流量传感器是通过线缆或者无线地(例如通过或者Wi-Fi)与一个评价单元连接。这个评价单元测定导入的冲洗溶液量和导出的冲洗溶液量,并且显示这些量的差。可选地,当然也可以显示导入的冲洗液以及导出的冲洗液的绝对量。如果求得的差大于在所述系统上设置或者预配置的阈值,则在本发明的一个优选实施方式中触发警告信号。对于食盐溶液而言,所述阈值例如可以是2500ml,或者对于含山梨糖醇/甘露醇的溶液而言,所述阈值例如可以是1000ml。在本发明的实施方式中涉及的是仅从外部在任意位置上插接至述及的软管的超声波流量传感器。所述传感器可以多次使用,例如应用于每日的数个检查,在此期间无需消毒。在此情形下不存在任何污染危险。所述软管是在医疗技术中、特别是在宫腔镜中常用的软管材料。使用的软管通常是针对一次性使用设计,并在使用后作废弃处理,故不存在污染危险。
当然,这两个滚轮泵/真空泵以及所述评价单元可以安装在单独一个壳体中。
可选地,在供液系统的范围内也可以设有冲洗液的预热装置,其例如通过对应的连续式加热器实现。在WO 2018/019317 A1中描述过这类通流调温装置的示例。
其他技术领域已揭示过适用于本发明的目的、并且可购得的超声波流量传感器(例如Sonotec GmbH的Sonoflow夹紧式传感器,Em-tec GmbH的SonoTT夹紧式变送器,Em-tec GmbH的BioPro TT或者CeramTec GmbH的Sonoc),故在此不再赘述。
实验表明,即使是在使用数个包含冲洗液的袋子的情况下,测量精度也远高于使用本文开篇所述系统时的精度。就宫腔镜手术而言,手术期间的冲洗液总量达到20L或以上的情况并不少见(亦即,例如4个袋子,每个5000mL)。
本发明的装置可以用在各种临床应用中:除例示性揭示的宫腔镜以外,特别是也可应用在泌尿外科应用中(例如经尿道的膀胱或者前列腺切除术(TUR)或者输尿管剜除术)。

1.一种用于冲洗体腔的医疗技术装置,包含
至少一个包含冲洗液的储存容器,
通向医用泵的第一软管连接,
从所述医用泵通向体腔的第二软管连接,从所述体腔通向第二医用泵的第三软管连接,
从所述第二医用泵通向收集容器的第四软管连接,
其特征在于,
在所述第一或者第二软管连接上从外部插接有第一超声波流量传感器,
在所述第三或者第四软管连接上从外部插接有第二超声波流量传感器,以及,所述超声波流量传感器是与一个评价单元连接,所述评价单元测量所述两个超声波流量传感器的流量并且显示所述流量的差。
2.根据权利要求1所述的医疗技术装置,
其特征在于,
所述医用泵为滚轮泵。
3.根据权利要求1所述的医疗技术装置,
其特征在于,
在测得的流量差超出预设的阈值的情况下,所述评价单元显示一个报警信号。
