Abstract

一种织物或非织物筛孔圆盘,沿其圆周相间距各处灌装有一定剂量的药粉。该圆盘可选择性地转位,从而按顺序把所灌装的药剂置于吸入器中上下压板的一对孔之间。压入的气流穿过压板上的两孔以及灌有药剂的筛孔而夹带一次剂量的药粉,然后被病人用嘴承吸入。由于药粉灌装在筛孔中,冲击筛孔和药粉的气流使药粉受压并流过筛孔底部而把药粉冲个粉碎,从而使药粉成为悬浮微粒,因此药粉被吸入时其颗粒大小能起到最佳药效。

Description

干粉吸入器的药粉载盘
本发明涉及药粉载盘,特别涉及包含可装入在哮喘病等患者使用的吸入器中的干药粉的载盘。病人用一嘴承吸气,气流所携带的一定剂量的药粉经过嘴承被病人吸入而均布到病人肺中。
人们长久以来通过吸入合适药物并使其均布到肺的支气管中从而减轻哮喘和增加肺活量来治疗哮喘等呼吸道疾病。多年以来,两种使用最广、最便利的治疗方法是使用喷雾式加压吸入器吸入一定剂量的雾状药液或用干粉吸入器吸入一般与赋形剂混合的药粉。随着人们对地球大气的臭氧层的衰减与氯氟碳化合物的释放之间存在着很强相关而日益表示关切,这些物质在加压吸入器中的使用受到质疑,从而对干粉吸入装置的兴趣日益浓厚。
少量的细粉粉末、最好是微粒粉末主要使用来治疗吸呼道疾病。数量一般少于50微克(μg)的Salmeterol hydronapthoate之类的这种粉末被病人用呼吸吸入。人们发现,有效药物的颗粒大小在厚度上应小于5微米(μ),以确保它们深入到肺中。因此,一定剂量的药物必须微粒化、烟雾化、或粉碎、使病人吸入一定剂量药物后能起到应有的药效。
当前,在治疗呼吸道疾病中主要使用四种不同的方法提供细粒粉末而无需使用挥发剂。
第一种方法依靠使用内装一定剂量有效药物和潜在辅药的硬胶药丸。哮喘病人使用的吸入器包括一种戳穿或打开药丸的装置,需要时,它可插入吸入器。病人在嘴承上吸气造成真空从而形成气流而吸走被戳穿药丸中的药粉。然后从吸入器上取下空药丸,而后装上另一个药丸。美国专利3,906,950、4,013,075、3,807,400和3,991,761示出了使用这种药丸戳孔技术或药丸打开技术的吸入器。在这些吸入器中,戳孔后的药丸的两端在吸药时保持不动。因吸气而通过药丸的气流把药粉吸走并应从打开的药丸内部把所有药粉吸尽,但人们发现,病人造成的气流在吸气时一般不足以从用作壳体的药丸中吸尽药粉,事实上,药丸阻碍药粉的吸尽。
另一种吸入器不使用一个个药丸,而是装有一插件,插件周边近旁互相等距地排列有一系列药泡。每一药泡内装一定时的药粉。如EPO专利申请211595和455463以及467172A1所示,当每一药泡移动到预定位置时,用适当打开装置打破该药泡而释放出药粉,然后被病人吸入。人们发现,药泡中凹痕内的潮气凝成的微小水珠会使备好的药粉发生凝结。因此,在被气流带动并由病人吸入时,颗粒大小并不符合起到最佳药效的要求。
另一种吸入器使用一内装足够服用若干次的药粉的容器,该容器通常称为Draco Turbuhaler,美国专利4,668,218、4,667,668和4,805,811对它作了详细说明。该容器包括一种从该容器取出药粉并备好一次吸入剂量的装置。药粉的取出和剂量的制备使用的是一块具有一定厚度和若干杯状或截头圆锥状穿孔的板。该板可用机械装置从一个位置移动到另一个位置,在前一个位置上,一些孔以取自容器的药粉充满,在后一个位置上,充满药粉的孔位于一管道内。当病人在与管路相通的嘴承上吸气时,管路中的气流就把药粉从孔中吸走。一刮平装置用来刮平板上穿孔中的药粉,以确保穿孔装满药粉而获得不变剂量。但人们发现,在吸气以便从板上一个或多个孔吸起药粉时,并不能吸尽药粉,这是由于病人的吸气力不足并由于穿孔不是圆柱形,一些药粉会落回孔中或根本就未离开穿孔。此外还有上述凝结问题。因此即使假定合适剂量的药粉都已离开转板或转盘上的穿孔,也必须设置涡动装置使凝结成团的被吸药粉微粒化、烟雾化。
最后,德国专利NO.4020571A1公开了一种在干粉吸入器中供应药粉的方法,其中,为了形成悬浮微粒,装有药粉的丝绒或丝绒状材料被引入一喷射气流中。该喷射气流因伯努里效应而上举丝绒状材料中的药粉,把它带起,然后被病人吸入。这种装置的问题是纤维本身会与药粉混合在一起;
本发明避免了现有技术中的许多问题,使精确剂量的药粉通过吸入器提供给病人,而其颗粒大小可获得最佳药效。
按照本发明,把用来治疗哮喘病的Salmeterd hydronapthoate之类的药粉以一定剂量注入到一织物或非织物筛孔圆周上相间距的各位置上。圆盘可选择性地转位,从而可按顺序把注入的一定剂量的药粉置于吸入器中上、下压板的一对孔之间。气流穿过压板上的孔以及灌注有药粉的筛孔而扬起药粉,然后由病人经一嘴承吸入。
由于药粉注入在筛孔中,这些筛孔可以是编织成的,例如丝网,或用人造纤维构成,例如聚酰胺、聚烯烃、聚酯,甚或纤维素衍生物之类的天然改性纤维;甚或可用金属或比方说玻璃、瓷器之类的陶瓷冲压或蚀刻而成,因此冲击筛孔和药粉的气流会使药粉冲压并流过筛孔底部而把药粉冲个粉碎,从而使药粉成为悬浮微粒,因此药粉颗粒大小在吸入时能起到最佳药效。而且,由于筛网的多孔性以及药粉沉积在孔隙中,因此涡流能完全围住每剂药粉并把它带起,从而确保药粉从筛孔完全扩散入气流中。使气流流过一喷嘴和底下压板而形成压力变动,即可在气流流过筛孔时形成涡流,从而有助于粉碎受压缩的药剂。
从下述说明、权利要求以及附图中所可更清楚看出本发明的其它目的和优点,附图中:

请仔细参见各附图,其中,各附图中的相同部件用相同标号表示,图1示出构成本发明主题的作为药粉载体的筛孔圆盘10。
药粉载盘10的大小适合于插入在图1中以标号20简示的一以呼吸吸药的干粉吸入器中。
筛孔圆盘10可用编织或非编织材料制成,或者用金属毛坯冲压而成,甚或用不锈钢或陶瓷照光酸洗而成,从而形成许多微小孔隙12。药粉的一次剂量16被压入筛孔圆盘10圆周14上相间距各处的孔隙12中。剂量16的大小决定于所使用的药物。例如,用来治疗哮喘的普通药物为salmeterol hydronapthoate,它的一次发放剂量约为50微克。这种药物的每一药剂14以约0.030-0.250英寸的直径和约0.002-0.1英寸的厚度即可达到预定药量。
筛孔圆盘10上可制出约0.004-0.125平方英寸、并位于一对压板22、24之间的孔隙12,每一压板上有扩大的孔26,在用合适机构装置把筛孔圆盘转动到刻度所示各位置上时,孔26与药剂16之一对齐。压板22、24使压力分布在筛孔圆盘10的周边处,从而把药剂16保持在筛孔圆盘10邻近周边14的药剂位置上。然后如图2B箭头30所示,使压入的气流通过压板上的孔26和灌装有药粉的筛孔圆盘10而扬起粉状药剂16,然后病人通过嘴承32吸入该气流及所夹带的药剂7。
由于药粉灌装压入在筛孔圆盘10中,而筛孔圆盘10可以是丝网之类的编织物,或用合成或天然纤维构成,甚或用金属或陶瓷受压或蚀刻而成,因此冲击筛孔和药粉的气流使药粉冲压并流过筛孔底部或筛孔纤维之间的孔隙12而把药物冲个粉碎,从而使药粉成为悬浮微粒,因此药粉被吸入时的颗粒大小能起到最佳药效。而且,由于筛网的多孔性以及药粉沉积在孔隙中,因此气流能如图2B所示整个围住并扬起每一剂药粉,从而确保药剂从筛孔完全扩散入气流中。
这可与图3A到图5所示各现有技术中所使用的方法进行比较。
如图5所示,一干粉吸入器中使用了一插件34,邻近该插件周边处相互等距地排列有一系列药泡36。每一药泡装有定量的药粉38。当每一药泡36移到一定位置上时,用合适打开装置破碎该药泡36而释放出药粉38,然后被病人吸入。人们发现,药泡36中凹痕40所含潮气凝成的小水珠使所备药粉38发生凝结。因此在被气流夹带着被病人吸入时,颗粒大小并不能起到最佳药效。
另一种吸入器使用内装足以分几次服用的药量的一容器。该容器包括一用来从该容器每次取出一次吸入剂量药粉的装置。该取出装置包括图4A所示一块具有一定厚度和若干杯状或截头锥状穿孔44的板42。该板可用机械装置从一位置转动到另一位置,在前一位置上,用从容器中取出的药粉46充满若干穿孔44,在后一位置上时,充满药粉的穿孔被置于一管道中。当病人在一与该管道相通的嘴承上吸气时,所形成的气流如图4B中箭头48所示流进管道中,从而把药粉46从穿孔44中吸走。一刮平装置用来刮平板上穿孔44中的药粉以确保穿孔44充满药粉而获得不变的剂量。但人们发现,在吸气而从板42上的一个或多个穿孔44吸入药粉时,并不能吸尽药粉,而由于病人的吸力不足以及穿孔不是圆柱形,如图4B清楚所示,某些药粉会回落入穿孔44或从未离开穿孔44。此外,还存在上述凝结问题。因此,即使假定转板42上有合适剂量的药物离开了穿孔44,也必须设置涡动装置来使由气流所夹带的凝结成团的药粉悬浮成微粒。
最后,如图3A所示,一装有药粉52的丝绒或丝绒状材料50被引入图3B中箭头54所示的喷射气流中。该喷射气流54因伯努里效应而把药粉52从丝绒状材料50上扬起并夹带着药粉被病人吸入。这种装置的问题是丝绒材料50的纤维会与药粉混合在一起。
使用时,如图1所示,筛孔圆盘10夹紧在压板22与24之间,药剂16被转位到吸入器20中压板上的两孔26之间的位置上。用来夹带药剂16并流过孔隙12从筛孔圆盘10吸走药剂16的气流可通过一入口孔60供应到一内装活塞64的加压室或加压筒62中,该活塞64通常与压缩弹簧66相互抵住。一盖帽68可从吸入器20的嘴承32上揭去,喷嘴73中吸入器通道71中所形成的真空使得阀70被打开而与通道71中的低压相通,这转而使压缩弹簧66把活塞64推入加压室62,从而从加压室20驱动气流穿过阀70而进入通道71和压板22、24上的孔26,从而完全扬起药剂16而使药剂16通过嘴承32被病人吸入。
然后用手转位以便重整吸入器而把下一个药剂16置于两孔26之间,同时阀70关闭,流入加压室62的气流克服压缩弹簧66的弹力而把活塞64推回使弹簧压缩的位置。
或者,可使从入口孔60流入的气流冲击一膜片74而启动一解扣装置76使气流经嘴承72从通道71流出。该解扣装置76能启动一起动装置78而驱动转位装置79,从而比方说通过转位装置79使筛孔圆盘10转位,即该转位装置使一棘轮80用凸轮带动筛孔圆盘10,从而使盘10在药剂16之间转动一定距离。计数显示器82可显示筛孔圆盘10上余下的药剂16的剂数。来自补充气流入口84的气流可使膜片74回复到其非启动位置。

Claims (14)

1、一种用于呼吸吸动的干粉吸入装置的药粉载体,至少包括一部分,该部分包括许多孔隙,至少一定剂量的药粉灌装在所述载体的所述一部分的所述孔隙中并横跨所述载体的所述一部分中许多所述孔隙之间的空间,以便被导入所述部分的一气流夹带。
2、按权利要求1所述的药粉载体,其特征在于,所述载体用丝网制成。
3、按权利要求1所述的药粉载体,其特征在于,所述载体用金属制成。
4、按权利要求1所述的药粉载体,其特征在于,所述载体用陶瓷材料制成。
5、按权利要求1所述的药粉载体,其特征在于,所述载体用编织纤维制成。
6、按权利要求1所述的药粉载体,其特征在于,所述载体用非编织纤维制成。
