Abstract

Description

糖尿病是一种慢性病症,其特征在于由于胰岛素分泌、胰岛素作用或者两者的缺陷而导致的高血糖。1型糖尿病的特征在于极少或没有胰岛素分泌能力,并且患有1型糖尿病的患者需要胰岛素来生存。在2型糖尿病中,胰岛素分泌受损和胰岛素抵抗的组合作用导致血糖水平升高。在至少三分之一的2型糖尿病患者中,疾病进展至绝对需要胰岛素疗法。
胰岛素的时间-作用概况对于控制餐后血糖水平是重要的。在健康个体中,胰腺响应于吸收的食物而分泌大量胰岛素,这导致血液胰岛素水平在几分钟内升高。在患有1型糖尿病的个体和患有2型糖尿病的某些个体中,必须施用胰岛素。但是,施用的胰岛素比内源分泌的胰岛素更慢地进入血液,并且缓慢的起效可能导致在餐后早期的高血糖。作用持续时间过长会导致两餐之间胰岛素过多,这导致晚期餐后低血糖和/或体重增加。
此前曾努力加快胰岛素产品的时间作用。在1990年代和2000年代早期,“速效”胰岛素类似物变得可用。即使使用所谓的“速效”胰岛素类似物,如赖脯胰岛素(HUMALOG®)、门冬胰岛素(NOVOLOG®)和赖谷胰岛素(APIDRA®),也是直到注射后50-90分钟才达到最大循环胰岛素浓度注射。这没有快到足以匹配碳水化合物吸收概况。

仍需要旨在用餐时间使用的胰岛素组合物,与现有的市售胰岛素产品相比,所述胰岛素组合物从注射部位的胰岛素吸收更快且起效更快。
本发明尝试通过提供药学上可接受的胰岛素制剂来满足这些需求,与现有的市售胰岛素产品相比,所述胰岛素制剂具有更快的胰岛素至血液中的吸收和更快速的起效。
根据本发明的第一方面,提供了包含胰岛素和多磷酸盐化合物的药物组合物,所述多磷酸盐化合物选自焦磷酸盐、三偏磷酸盐、三磷酸盐和四磷酸盐,条件是所述组合物不包含糖多聚体或EDTA。

本发明的组合物具有在0.24和3mM之间的胰岛素浓度(40-500 IU/mL;1.4mg/mL -17.5 mg/mL)。本发明的组合物可能具有40、100、200、300、400和500 IU/mL (1.4、3.5、7、10.5、14和17.5 mg/mL)的具体浓度。优选的浓度是100和200 IU/mL。

市售胰岛素组合物每六个胰岛素分子具有约2.4个锌原子(HUMLIN®R U-500),并且一些每六个胰岛素分子具有约3.0个锌原子(HUMLIN®,NOVOLOG®)。本发明的某些实施方案包括锌,其浓度足以提供每六个胰岛素分子约2-4个锌原子。其他实施方案包括浓度高达约5 mM的锌。在某些实施方案中,锌的浓度范围从约0.2至约5 mM。在某些实施方案中,锌的浓度范围从约0.5至约2 mM。在某些实施方案中,锌的浓度选自0.2、0.3、0.4、0.5、0.6、0.7、0.8、0.9、1、1.3和2 mM。

Antimicrobial effectiveness testing; pp. 2499–2500。优选的防腐剂是芳基酸和酚类化合物,或此类化合物的混合物。胰岛素产品中最常用的防腐剂是苯酚、间甲酚和苯甲醇。可以使用上文引用的方法容易确定有效浓度。例如,目前的市售组合物包含3.15 mg/mL间甲酚(HUMULIN®和APIDRA®),1.72 mg/mL间甲酚和1.50 mg/mL苯酚(NOVOLOG®)和2.5 mg/mL间甲酚(HUMULIN®R U-500)。
本发明的胰岛素组合物的pH通常为7.0至7.8,且使用生理学上适当的酸和碱(通常为盐酸10%和氢氧化钠10%)来调节。市售胰岛素制剂的pH通常在7.2至7.6的范围内,其中以7.4±0.1为常见目标pH。
希望在施用组合物时,尽可能接近地大致匹配注射部位的体液的张度(即重量摩尔渗透压浓度),因为不与体液大致等张的溶液在施用时可产生令人疼痛的刺痛感。因此,希望组合物与注射部位的体液大致等张。如果不存在张度剂的情况下的组合物的重量摩尔渗透压浓度足够小于组织的重量摩尔渗透压浓度(对血液,约300 mOsmol/kg;欧洲药典要求渗透压为> 240 mOsm/kg),通常应该添加张度剂以将组合物的张度提高到约300mOsmol/kg。典型的张度剂是甘油(甘油)和氯化钠。使用标准技术容易地确定要添加的张度剂的量。Remington: The Science and Practice of Pharmacy, David B. 药代动力学(PK)和药效动力学(PD)研究
研究1. 25 mM焦磷酸盐或25 mM三磷酸盐
使用15只具有预先安装血管通路口的患糖尿病(四氧嘧啶诱导的)、阉割的雄性Yucatan小型猪(平均17个月龄且平均体重42 kg)。将患糖尿病动物单独圈养,且始终可随意获取淡水。每天对其饲喂两餐日常饮食(house diet) S-9,且每天两次接受适当的维持基础和进餐胰岛素以管理其糖尿病病况。
配制测试物品(制剂A和B),并用冷袋过夜运输到研究地点。将其冷藏储存,直至给药时间,且然后在所有动物给药完成后放回冰箱。在给药期间,当不取出测试物品时,将其保留在隔热盒中。HUMALOG®胰岛素对照来自市售的药瓶。

将研究设计为三向交叉设计。这种设计允许通过在每个研究日期(各自隔开7天的3个日期)给药一种测试物品,使每个个体动物接受三种测试物品中的每一种。在研究前一天,动物饲喂其日粮的一半,并在它们的早晨维持施用时接受0.2 U/kg Humalog Mix 75/25胰岛素。所有研究动物禁食整夜,并且在研究日的药物施用前没有接受它们的夜间胰岛素或膳食。
在研究当天早晨,将所有动物置于束缚用的吊索中,并使它们的血管通路口可用(配备用于血液取样)并检查通畅度。将动物随机分入治疗组(3个组,每组n = 5,得到每个治疗n = 15)。
在收集两种基线血液样品(-30min和-20min)后,将动物返回至其围栏并饲喂~300g S-9饮食。在展示膳食完全食用后20分钟,用Terumo胰岛素注射器(0.5 ml 1/2''针),在动物的侧腹中,皮下注射(0分钟)测试物品。给药涉及单次注射0.2 U/kg的胰岛素活性。在整个剩余的采血期间,所有研究动物均可随意获取清洁的淡水。
在以下时间点从每只动物采集系列血样(每个2.0 mL):SC给药后-30、-20 (然后立即饲喂)、0 (临给药前)、5、10、15、30、45、60、75、90、105、120、150、180、240和360分钟。血液样品(抗凝血剂:无[血清])在环境温度下保持至少30分钟,但不超过2小时以允许凝结。然后,通过离心分离血清并将其分成两个等分试样并在大约-70℃下冷冻储存。
使用自动化Cobas c311临床化学分析仪(Roche Diagnostics, Indianapolis)测定血清葡萄糖浓度。从Humalog治疗组中排除两只动物,使得对于该测试物品为n = 13。一只动物由于不符合基线葡萄糖> 200 mg/dL的标准而被排除,且另一只动物由于通路口通畅度问题而没有参加。下表3中提供了使用含赖脯胰岛素的制剂治疗(在时间0,0.2U/kg)后的血清葡萄糖浓度(mg/dL)。

使用竞争性放射免疫测定法(RIA)测定血清胰岛素浓度。在测量内源猪胰岛素和外源胰岛素二者的RIA中,血清胰岛素替代125I-胰岛素用于与豚鼠抗大鼠胰岛素抗体结合。使用山羊抗豚鼠IgG血清试剂沉淀抗体复合物。在经过热处理的木炭剥离的血清中,RIA的定量上限和下限分别为5000和20 pM。使用Phoenix WinNonLin 6.3进行非区室药代动力学分析。低于定量下限的值被分配为20 pM的值以进行计算。将高于定量上限的样品稀释并重新分析,或忽略。如上所述,由于通路口通畅度问题,来自HUMALOG治疗的一只动物未参加研究,使得对于该测试物品为n = 14。

PK/PD数据证实,与HUMALOG相比,焦磷酸盐或三磷酸盐加快了时间作用并降低了Tmax。具有焦磷酸盐的制剂A具有比HUMALOG快~47%的Tmax (中值Tmax快~33%),并且具有与HUMALOG相当的平均Cmax。具有三磷酸盐的制剂B具有比HUMALOG快~77%的平均Tmax (中值Tmax快~67%),和比Humalog高~125%的Cmax。与HUMALOG相比,含有25 mM焦磷酸盐或25mM三磷酸盐的制剂导致更快的Tmax和更高的Cmax。
研究2. 三磷酸盐浓度对PK/PD的影响

与HUMALOG®相比,与HUMALOG相似的制剂中5、10或20 mM的三磷酸盐加快时间作用,并降低Tmax和增加Cmax,且以剂量依赖方式进行。
研究3. 对不同市售胰岛素的影响
使用15只具有预先安装血管通路口的患糖尿病(四氧嘧啶诱导的)、阉割的雄性Yucatan小型猪,来研究三磷酸盐对血糖和不同市售胰岛素的血清胰岛素时间作用概况的影响。动物的圈养和营养以及测试和对照物品的运输和存储如上文研究1和2所述。
通过向HUMULIN-R®、NOVOLOG®和APIDRA®的市售小瓶添加足够的三磷酸盐,达到20 mM三磷酸盐的浓度,来配制测试物品(下表中的制剂F、G和H)。请注意,下表中列出的其他成分的浓度反映了那些成分在那些产品的市售小瓶中的浓度;没有调整浓度以解释由于加入三磷酸盐而产生的轻微稀释。

研究为四向交叉设计,其允许通过在每个研究日期(各4个日期,间隔7天)给药一种测试物品,使每个个体动物接受三种测试物品和对照中的每一种。如上文关于研究1和2所述,准备动物用于研究。
将动物随机分入治疗组(4个组,每组n = 3-4,得到每个治疗n = 15)。由于通路口故障,从HUMULIN + 20 mM三磷酸盐和NOVOLOG + 20 mM三磷酸盐组排除一只动物,使得对那些治疗组得到n = 14。由于通路口故障,从HUMALOG组排除一只动物,且由于不符合纳入标准,从HUMALOG组排除一只动物,对该组得到n = 13。
如上文关于研究1和研究2所述,进行基线血液样品的收集、测试物品注射、血样的收集,以及血液和血清样品的制备、运输和测量。
使用自动AU480临床化学分析仪(Beckman Coulter)测定血清葡萄糖浓度。血清葡萄糖结果(mg/dL)在下表9中给出。

PK/PD数据证实,与没有三磷酸盐的HUMALOG相比,在各种市售胰岛素的制剂中使用三磷酸盐加快了时间作用并降低Tmax。与单独的HUMALOG相比,所有包含20 mM三磷酸盐的制剂均导致更快的Tmax。与单独的HUMALOG相比,含有APIDRA和HUMULIN以及20 mM三磷酸盐的制剂引起更高的Cmax。
临床研究
进行临床研究以研究本发明的组合物的药代动力学和药效动力学作用。所述研究被设计为5-期的交叉研究,以比较4种含有不同浓度三磷酸盐和赖脯胰岛素的制剂与不含三磷酸盐的赖脯胰岛素制剂相比,在皮下(SC)给药后的作用。通过向U200市售赖脯胰岛素制剂添加足量的三磷酸盐和氯化镁,以达到下表11中所示的浓度,来配制测试物品:
表11:测试物品和对照物品的组成。除了赖脯胰岛素外,向其添加三磷酸盐和MgCl2以产生测试物品的U-200 HUMALOG制剂还含有5 mg/mL氨丁三醇、16 mg/mL甘油、3.15mg/mL间甲酚和0.046 mg/mL Zn2+。
招募健康主体,并且将每个主体随机分配至治疗序列,其包括单一15个单位SC剂量的单独的赖脯胰岛素,和单一15个胰岛素单位SC剂量的每种测试制剂。单个主体在给药时机之间需要至少3天。
在多个时间点采集血样,以确定随时间推移赖脯胰岛素的血清浓度。使用经过验证的赖脯胰岛素特异性的酶联免疫吸附测定法测量赖脯胰岛素的血清浓度。使用Phoenix®7.0版(或更高版本)和S-PLUS®软件(8.2版),使用标准的非区室分析方法进行药代动力学分析。将游离血清赖脯胰岛素浓度用于计算药代动力学参数。结果提供在下表12中。

结果显示,与不含三磷酸盐的对照相比,含三磷酸盐的制剂具有加快的药代动力学参数。
此外,在每期进行5小时的血糖正常的葡萄糖钳夹,以评估对每种治疗的糖动力学应答。在这个评估中,使用随时间的葡萄糖输注率(GIR)作为胰岛素作用的量度。使用S-PLUS软件8.2版,将局部加权散点图平滑(LOESS)函数应用于每个治疗组和/或治疗期中的所有个体GIR相对于时间概况。每个主体的拟合数据用于计算糖动力学参数。
数据分析显示,与不含三磷酸盐的对照相比,含三磷酸盐的制剂具有改善的药效动力学参数。上述研究证实,向胰岛素制剂中添加少量某些多磷酸盐(例如焦磷酸盐或三磷酸盐)可以导致胰岛素药代动力学概况中较早的Tmax和较高的Cmax。
