Abstract

金属蛋白酶抑制剂2(人前体Swiss-Prot P16035,也称为“金属蛋白酶组织抑制剂2”和“TIMP2”)是一种分泌蛋白,其与金属蛋白酶复合并通过与其催化锌辅因子结合而不可逆地使它们失活。已知TIMP2作用于MMP-1、MMP-2、MMP-3、MMP-7、MMP-8、MMP-9、MMP-10、MMP-13、MMP-14、MMP-15、MMP-16和MMP-19。据报道,TIMP2抑制内皮细胞的增殖。因此,已经提出编码的蛋白质通过抑制响应于血管生成因子的静息组织的增殖,以及通过抑制经历重塑的组织中的蛋白酶活性而在维持组织稳态中起作用。

急性肾损伤(AKI)是由于各种病况引起的并且具有严重后果。AKI定义为以下任何一种:
在48小时内SCr增加≥0.3mg/dl(≥26.5μmol/l);或
将SCr增加至≥1.5倍基线,已知或假定在过去7天内发生;或
尿量<0.5ml/kg/h,持续6小时。

重要的是,通过更广泛地定义肾功能急性变化的综合征,RIFLE标准超越了ARF。根据RIFLE标准定义的,AKI的概念包括其他不太严重的病况。Bellomo等人(Crit Care.8(4):R204-12,2004)描述了RIFLE标准:

概述

可替代地或另外地,基于受试者正在经历或已经经历大血管手术、冠状动脉搭桥或其他心脏手术,和/或已经接受以下的一种或多种:NSAID、环孢菌素、他克莫司、氨基糖苷类、膦甲酸、乙二醇、血红蛋白、肌红蛋白、异环磷酰胺、重金属、甲氨蝶呤、不透射线的造影剂或链脲霉素,可以将这样的受试者鉴定为具有增加的风险。
在某些实施方案中,基于生物标记物结果,可以将这样的受试者鉴定为具有增加的风险。优选地,生物标记物结果包括测量的尿TIMP-2浓度和测量的尿胰岛素样生长因子结合蛋白7(IGFBP7)浓度中的一种或多种,并且最优选地从测量的尿TIMP-2浓度和测量的尿IGFBP7浓度计算的组合结果,例如[TIMP-2]×[IGFBP7]结果。

术语“特异性结合”并不意味着抗体唯一地与其预期靶标结合,因为如上所述,抗体与展示抗体所结合的表位的任何多肽结合。相反,如果抗体对其预期靶标的亲和力与其对不显示适当表位的非靶分子的亲和力相比大约高5倍,则抗体“特异性结合”。优选地,对于靶分子,抗体的亲和力比其对非靶分子的亲和力高至少约5倍,优选10倍,更优选25倍,甚至更优选50倍,和最优选100倍或更多。在优选的实施方案中,优选的抗体以至少约107M-1和优选约108M-1至约109M-1、约109M-1至约1010M-1、或约1010M-1至约1012M-1的亲和力结合。
亲和力计算为Kd=koff/kon(koff是解离速率常数,kon是结合速率常数,Kd是平衡常数)。通过测量不同浓度(c)的标记配体的结合部分(r),能够确定平衡时的亲和力。使用Scatchard方程绘制数据:r/c=K(n-r):其中r=平衡时结合配体的摩尔数/受体的摩尔数;c=平衡时游离配体浓度;K=平衡结合常数;和n=每个受体分子的配体结合位点数。通过图形分析,在Y轴上绘制r/c,相对地,在X轴上绘制r,从而产生Scatchard图。通过Scatchard分析进行的抗体亲和力测量是本领域众所周知的。参见,例如,van Erp等人,J.Immunoassay 12:425-43,1991;Nelson and Griswold,Comput.Methods ProgramsBiomed.27:65-8,1988。

抗体的产生

然后可以通过首先用纯化的目标多肽筛选亲和力和特异性并且如果需要将结果与抗体与需要被排除结合的多肽的亲和力和特异性进行比较来选择通过这些方法产生的抗体。筛选程序可包括将纯化的多肽固定在微量滴定板的不同孔中。然后将含有潜在抗体或抗体组的溶液置于各自的微量滴定孔中并孵育约30分钟至2小时。然后洗涤微量滴定孔,并向孔中加入标记的第二抗体(例如,如果产生的抗体是小鼠抗体则标记的第二抗体是与碱性磷酸酶缀合的抗小鼠抗体)并孵育约30分钟然后洗涤。将底物加入孔中,并且在存在针对固定化多肽的抗体时出现颜色反应。
然后可以在所选择的测定设计中进一步分析如此鉴定的抗体的亲和力和特异性。在开发针对靶蛋白的免疫测定中,纯化的靶蛋白作为标准品,用于使用所选抗体判断免疫测定的灵敏度和特异性。因为各种抗体的结合亲和力可能不同;某些抗体对(例如,在夹心测定中)可以在空间上相互干扰等,抗体的测定性能可能是比抗体的绝对亲和力和特异性更重要的量度。

抗体的重组表达
一旦获得了编码本发明抗体的核酸序列,就可以使用本领域众所周知的技术通过重组DNA技术产生用于产生抗体的载体。可以使用本领域技术人员众所周知的方法构建含有抗体编码序列和适当的转录和翻译控制信号的表达载体。这些方法包括,例如,体外重组DNA技术、合成技术和体内遗传重组。(参见,例如,Sambrook等人,1990,MOLECULARCLONING,A LABORATORY MANUAL,2d Ed.,Cold Spring Harbor Laboratory,Cold SpringHarbor,N.Y.和Ausubel等人eds.,1998,CURRENT PROTOCOLS IN MOLECULAR BIOLOGY,JohnWiley&Sons,NY中描述的技术)。

对于重组蛋白的长期高产量生产,优选稳定表达。例如,可以改造稳定表达本发明的抗体的细胞系。不是使用含有病毒复制起点的表达载体,而是可以用由适当的表达控制元件(例如,启动子、增强子、序列、转录终止子、多腺苷酸化位点等)控制的DNA和选择性标记转化宿主细胞。在引入外源DNA后,可使工程化细胞在富集培养基中生长1-2天,然后转换到选择性培养基中。重组质粒中的选择性标记赋予对选择的抗性,并允许细胞将质粒稳定整合到它们的染色体中并生长形成病灶,其进而可以被克隆并扩增到细胞系中。该方法可有利地用于改造表达本发明的抗体的细胞系。此类改造细胞系可特别用于筛选和评估与本发明的抗体直接或间接相互作用的化合物。

急性肾功能衰竭的诊断
本发明部分涉及向诊断患有AKI的患者施用抗TIMP-2抗体。如上所述,本文所用的术语“急性肾(或肾脏)损伤”和“急性肾(或肾脏)衰竭”部分地根据血清肌酐从基线值的变化来定义。ARF的大多数定义都有常见的要素,包括使用血清肌酐和常用的尿排出量。患者可能出现肾功能不全而没有可用于该比较的基线肾功能测量值。在这种情况下,可以通过假设患者最初具有正常GFR来估计基线血清肌酐值。肾小球滤过率(GFR)是每单位时间从肾(肾脏)的肾小球毛细血管过滤到鲍氏囊中的流体体积。肾小球滤过率(GFR)可以通过测量血液中具有稳定水平并且可以自由过滤但不会被肾脏再吸收或分泌的任何化学物质来计算。GFR通常以ml/min为单位表示:
通过将GFR归一化至体表面积,可以假设GFR为约75-100ml/min/1.73m2。因此测得的速率是尿液中源自可计算的血液量的物质的量。
有几种不同的技术用于计算或估计肾小球滤过率(GFR或eGFR)。然而,在临床实践中,肌酐清除率用于测量GFR。肌酐是由身体自然产生的(肌酐是肌酸的代谢产物,肌肉中存在肌酸)。它由肾小球自由过滤,但也由肾小管以非常少的量主动分泌,使得肌酐清除率将实际的GFR高估10-20%。考虑到测量肌酐清除率的容易程度,这个误差范围是可以接受的。
如果肌酐尿浓度(UCr)、尿流速(V)和肌酐血浆浓度(PCr)的值是已知的,则能够计算肌酐清除率(CCr)。由于尿浓度和尿流速的乘积得到肌酐的排泄率,肌酐清除率也被认为是其排泄率(UCr×V)除以其血浆浓度。这在数学上通常表示为:

肌酐清除率测量的准确性(即使在收集完成时)也是有限的,因为随着肾小球滤过率(GFR)降低,肌酐分泌增加,因此血清肌酐的升高更少。因此,肌酐排泄远大于过滤负荷,导致GFR的潜在大幅高估(多达两倍的差异)。然而,出于临床目的,重要的是确定肾功能是稳定的还是变得更糟或更好。这通常通过单独监测血清肌酐来确定。与肌酐清除率一样,血清肌酐在ARF的非稳态条件下不能准确反映GFR。尽管如此,血清肌酐从基线变化的程度将反映GFR的变化。血清肌酐易于测量,并且其对于肾功能而言是特异的。
为了就mL/kg/hr基础的尿排出量确定尿排出量,每小时的尿液收集和测量是足够的。例如,在仅有累积24小时的排出量且没有提供患者体重的情况下,已经描述了RIFLE尿排出量标准的微小修改。例如,Bagshaw等人,Nephrol.Dial.Transplant.23:1203–1210,2008,假设平均患者体重为70kg,并且患者根据以下标准分配RIFLE分类:<35mL/h(风险)、<21mL/h(损伤)或<4mL/h(衰竭)。
风险评估
因为AKI与显著的发病率和死亡率相关,并且因为没有可用于逆转AKI的特定治疗,所以早期识别和管理是最重要的。实际上,识别具有AKI风险的患者,或具有可能的AKI但在临床表现之前的患者,可能比仅治疗已建立的AKI产生更好的结果。因此,本发明部分涉及向处于即将发生(在72小时内,更优选在48、24、18或12小时内)AKI的高风险的患者施用抗TIMP-2抗体。
高风险患者包括具有急性肾损伤(AKI)的风险因素但具有正常GFR的患者。例如,他们可能包括患有糖尿病和高血压或正在服用药物例如非甾体类抗炎药或血管紧张素转换酶抑制剂的患者。这些个体代表需要被鉴定以在可能的情况下修改风险因素并在指示时(例如,对比研究)启动预防策略的群体。患有肾前性AKI的患者是具有肾前尿指数和自动调节机制失效(其导致GFR降低(例如,脱水))的患者。这些个体有可能迅速逆转他们的肾前状况。在此期间,可能存在对近端小管细胞刷状缘的损伤但不可检测。然而,使用新型生物标记物,可以识别早期损伤和治疗。AKI患者可能代表从严重肾前性AKI的延伸。或者,在某些情况下,AKI可能不会在肾前状态之前(例如,败血症、暴露于肾毒素)。血清肌酐是晚期标记物,并且在存在实质性损伤后将检测AKI。此时,干预可能为时已晚。
KDIGO急性肾损伤临床实践指南(Kidney Intl.2(Suppl 1),2012,其全部内容包括所有附录通过引用并入)提供了对AKI的风险评估的描述,特别是在第2.2章和附录D中。风险因素包括水合状态,低蛋白血症,高龄,女性,黑人种族,CKD、糖尿病、心脏病、败血症或肺病的存在,暴露于某些药物以及肾毒性的造影剂,心脏手术等。
生物标记物的使用
血清肌酐测量(SCr)是肾功能的金标准标记物。然而,SCr浓度在检测肾功能的突然下降时可能是不准确的,因为剩余健康肾单位的功能储备阻止了SCr的显著升高,直到50%的肾单位丢失。因此,因为SCr是肾损伤的“晚期”标记物,即使基于SCr的肾功能估计是“正常的”,肾损伤也可能已经开始。已用于评估AKI的其他生物标记物包括血清和尿胱抑素C,血清和尿中性粒细胞明胶酶相关脂质运载蛋白,尿肾损伤分子1,白细胞介素-18,肝型脂肪酸结合蛋白和N-乙酰-β-D-氨基葡萄糖苷酶。
2014年,美国食品和药物管理局批准了基于金属蛋白酶组织抑制剂2和胰岛素样生长因子结合蛋白7的尿浓度的组合([TIMP-2]×[IGFBP7])的测试的上市以鉴定患者处于发展未来AKI的风险。在一项研究中,ICU入院后不久单次测量尿TIMP2·IGFBP7,发现12小时内发生中度至重度AKI的AUC-ROC为0.84。使用0.3的截止值,测试显示灵敏度和特异性分别为89%(77-97%)和49%(43-54%);在截止值为2.0时,测试显示灵敏度和特异性为40%(23-57%)和94%(92-96%)。Gunnerson等人,J.Trauma Acute Care Surg.80:243–49,2016。这些测试提供了用于根据本发明的方法鉴定高风险患者进行治疗的理想方法。
药物组合物和施用

其他治疗方式

当严重时,高钾血症是医学紧急情况并且需要立即干预。治疗方法分为三种主要方法:1)导致钾从细胞外空间转移到细胞内空间的干预,2)稳定钾的膜作用的干预,和3)增强钾消除的干预。这些干预措施通常同时提供。
患有AKI的患者通常发生酸碱异常,最显著的是代谢性酸中毒,其在临床上显著时需要干预。
在AKI和其他严重疾病中经常遇到由于应激性高血糖引起的升高的葡萄糖,并且关于与血糖控制相关的结果益处、可能受益的患者和群体以及期望的葡萄糖靶标和实现(相对)血糖正常的特定治疗方法的证据仍然存在争议,共识和目前的护理标准建议如下:使用胰岛素治疗以达到110至149mg/dL(6.1-8.3mmol/L)及以上的血糖水平;并经常监测血糖。
除了保守治疗之外,医生可以确定肾替代疗法的开始或维持是适当的。间歇性RRT包括血液透析和持续的低效透析,而连续RRT是指超滤(UF),连续静脉血液滤过(CVVH),连续静脉血液透析(CVVHD),连续静脉血液透析滤过(CVVHDF),连续静脉高通量血液透析,以及连续动脉静脉血液滤过(CAVH)。

实施例
实施例1:兔子中的单克隆抗体开发
通过皮下注射(SQ)用抗原/佐剂乳剂免疫雌性新西兰兔。用完全弗氏佐剂进行初次免疫,并将不完全弗氏佐剂用于所有后续加强免疫。每三周以每只兔子250μg蛋白质抗原SQ注射兔子(两个部位(臀部和肩胛骨)交替)。在第二次加强免疫后7天从边缘耳静脉取出测试出血。通过间接ELISA测定来测试该测试出血(免疫血清)以确定兔的免疫应答是否足以用于单克隆抗体开发。最好响应的兔子被给予最后的SQ加强免疫,并且四天后通过放血被安乐死。通过心脏穿刺收集全血。通过对靶抗原的间接ELISA鉴定产生目的抗体的B细胞,并分离免疫球蛋白基因。将重链和轻链克隆到单独的哺乳动物表达载体中,转染到HEK细胞中(瞬时转染),并收获含有兔单克隆抗体的组织培养上清液。
实施例2:小鼠中的单克隆抗体发育

实施例3:人源化
通过比较小鼠单克隆的鼠可变(V)区框架(FR)序列与Kabat数据库中的人抗体的序列(Kabat等人,Sequences of Proteins of Immunological Interest,5th ed.,U.S.Department of Health and Human Services,U.S.Government Printing Office,Ishington,D.C.,通过引用并入本文),发现人序列与小鼠单克隆的VK和VH结构域的FR具有最高程度的序列同源性。因此,选择这些人序列作为其上移植小鼠单克隆的CDR的人框架。
实施例4:组织学评价

实施例5:
Balb/c小鼠(20-24周龄,体重25-30g)购自Charles River LaboratoriesInternational,Inc。所有程序均由匹兹堡大学医学中心(UPMC)的机构动物护理和使用委员会(IACUC)批准。在1周的适应期后,在用异氟烷麻醉后对小鼠进行盲肠结扎和穿刺手术(CLP)以诱导败血症或仅进行剖腹手术(假手术)。具体而言,将盲肠外露,从顶部1/4处结扎,并用21号针刺穿3次。挤出一滴粪便,将结扎并刺破的盲肠置换到腹部中,并关闭腹壁。作为标准的手术后治疗,在颈背部皮下进行液体复苏(生理盐水40ml/kg)的一次施用,注射丁丙诺啡(0.05mg/kg),并在手术后立即并每12小时(持续3天)腹膜内施加包括头孢曲松(25mg/kg)和甲硝唑(12.5mg/kg)的抗生素。密切监测所有动物,并在恢复期间允许自由获取食物和水。手术后48小时处死小鼠并收获肾组织并获得血液样品用于进一步测量。
两种同种型的抗TIMP2抗体由Astute Medical,Inc.(San Diego,CA)提供。抗体TIMP2(NB172-77)是单克隆F(ab’)2,抗体TIMP2(NB251-47)是多克隆山羊IgG。在CLP手术后8小时将CLP动物随机分成三组:CLP(用媒介物安慰剂治疗)、CLP+腹膜内注射抗TIMP2抗体NB172-77,5mg/kg)、CLP+腹膜内注射抗TIMP2抗体NB251-47,5mg/kg)。
通过苏木精和曙红(H&E)染色组织学组织图像中的组织学评分肾脏损伤。它包括细胞浸润评分(0-3)和小管上皮损伤评分(0-5)。具体而言,通过对小管和血管壁周围的细胞(层)的近似数量进行评分来确定细胞浸润(评分:0=无,1=<10细胞(5层),2=10至20个细胞(5至10个细胞层)),3=>30个细胞(>10个细胞层);小管上皮的损伤基于显示细胞坏死、刷状缘丧失、管型形成、空泡化和小管扩张的小管的比例评分如下:0,无;1,<10%;2,11%至25%;3,26%至45%;4,46%至75%;和5,>76%。使用来自BioVision(MountainView,CA)的肌酐测定试剂盒测量血清肌酐(SCr)。
使用SPSS 14.0版软件(SPSS Inc.,Chicago,IL)分析用于比较组的统计学测试。使用t检验比较配对的实验组。p<0.05的差异被认为是显著的。结果表示为平均值±SEM。
如图1所示,与未治疗的败血症动物相比,用TIMP2(NB172-77)的抗TIMP2治疗显著降低了肾组织损伤的程度(双尾T检验,P值0.046)。正如预期的那样,抗TIMP2治疗对血清肌酐测量的肾功能没有影响(图2)。因此,TIMP2在败血症期间促进肾组织损伤,而与血清肌酐水平无关。
