Abstract

肺炎诊断用circRNA生物标志物及应用、引物和试剂
技术领域
本发明涉及生物技术领域,特别涉及肺炎诊断用circRNA生物标志物及应用、引物和试剂。
背景技术
肺炎按医院内外发病可分为社区获得性肺炎(CAP)和医院获得性肺炎(HAP)。近年肺炎其发病率和病死率都有升高趋势。肺炎病原体较复杂,各种感染和非感染之间,在临床表现、胸部X线等常有交叉重叠,难以鉴别。肺炎常见病原体有细菌、病毒、支原体、衣原体、立克次体、螺旋体、真菌、放线菌和寄生虫。但一般情况下病原体较难确定。
对肺炎的诊断主要依靠临床表现、实验室检查及胸部影像学检查来诊断。新型的分子标志物近年来也有很多进展,包括对肺炎病人代谢组学或者对肺炎病人miRNA组学的研究等。但这些研究都会存在一些问题,例如检测方法较复杂、标志物不稳定易降解等。
非编码RNA(ncRNA),包括短microRNA(miRNA)、长非编码RNA(lncRNA)和最近描述的环状RNA(circRNA),共占真核转录组总RNA的95%左右。circRNA具有独特的环状结构,稳定性强,是作为疾病的分子标志物的理想候选者。已报道circRNA可作为许多疾病的有效诊断和生物标志物,但目前仍然没有使用circRNA这种易于监测和高度稳定的分子靶标用于诊断肺炎,尤其是社区获得性肺炎这种常见且高度致命的疾病的报道。探索circRNA的表达与在肺炎患者的关系对了解肺炎感染的诊断具有重大意义。
发明内容
有鉴于此,本发明提供了肺炎诊断用circRNA生物标志物及应用、引物和试剂。该生物标志物选自hsa_circ_0018429、hsa_circ_0125357、hsa_circ_0026579、hsa_circ_0099188中的一种或几种。本发明具有的技术效果为:
利用本发明的circRNA:hsa_circ_0018429、hsa_circ_0125357、hsa_circ_0026579和hsa_circ_0099188中一种或几种,在全血中的表达水平能够很好的筛选肺炎感染患者;当四种标志物组合使用时,AUC可以达到0.8776,灵敏度和特异度分别可达到89%和71%,说明对肺炎感染具有较好的诊断效果,显示出了其作为生物标志物的优势。
本发明还提供一种用于肺炎感染检测的引物组合物,包括本发明所述用于检测circRNA的正向引物和反向引物。本发明所述的circRNA组合物具有在制备肺炎感染检测相关产品中应用的能力。
为了实现上述发明目的,本发明提供以下技术方案:
本发明提供了肺炎诊断用circRNA生物标志物,选自如下生物标志物中的一种或几种:
(1)hsa_circ_0018429:序列如SEQ ID NO:1所示;
(2)hsa_circ_0125357:序列如SEQ ID NO:2所示;
(3)hsa_circ_0026579:序列如SEQ ID NO:3所示;
(4)hsa_circ_0099188:序列如SEQ ID NO:4所示。
hsa_circ_0018429位于第10号染色体上,全长19579个碱基。
hsa_circ_0125357位于第4号染色体上,全长13253个碱基。
hsa_circ_0026579位于第12号染色体上,全长24620个碱基。
hsa_circ_0099188位于第12号染色体上,全长341个碱基。
本发明还提供了该circRNA生物标志物在制备肺炎诊断试剂中的应用。
优选地,肺炎为社区获得性肺炎。
本发明还提供了肺炎诊断用circRNA引物,选自如下引物组中的一种或几种:
(1)用于扩增hsa_circ_0018429的引物组;
(2)用于扩增hsa_circ_0125357的引物组;
(3)用于扩增hsa_circ_0026579的引物组;
(4)用于扩增hsa_circ_0099188的引物组。
作为优选,用于扩增hsa_circ_0018429的引物组由SEQ ID NO:5所示正向引物和SEQ ID NO:6所示反向引物组成。

在本发明中,肺炎诊断的方法为检测circRNA在肺炎感染患者和健康人群的全血样本中的表达水平。
上述应用中,表达水平可为绝对表达水平或相对表达水平。所述相对表达水平可为相对于β-ACTIN的相对表达水平。所述表达水平可为circRNA组合在全血中的表达水平。
本发明肺炎诊断用circRNA生物标志物的筛选方法如下:
1)建立统一标准的标本库和数据库:从医院采集血样样本,同时系统收集患者的病理学资料和临床资料;
2)环状RNA差异表达谱分析:选择肺炎感染病人和健康个体的全血,提取RNA,使用博奥晶典人类环状RNA v2芯片筛选技术,并通过差异分析,得到在肺炎感染病人和健康个体的差异表达的circRNA;
3)用实时荧光定量PCR方法筛选出可用作检测生物标志物的血清circRNA:对已筛选的血清差异表达的circRNA在大样本人群中使用上述引物组合进行定量分析验证,确定能够用于区分正常人和肺炎感染病人的circRNA:hsa_circ_0018429(SEQ ID No.1),hsa_circ_0125357(SEQ ID No.2),hsa_circ_0026579(SEQ ID No.3),hsa_circ_0099188(SEQID No.4)。
本发明使用的检测方法可以选自:RT-PCR方法、芯片技术、测序技术、荧光定量PCR方法等中的至少一种。

本发明提供一种用于检测肺炎感染的circRNA组合物及其及其制备方法与应用。本发明全血circRNA组合物选自下列中的至少一种circRNA:hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188。本发明还提供一种用于肺炎感染检测的引物组合物,包括本发明所述用于检测circRNA的正向引物和反向引物。本发明所述的circRNA组合物具有在制备肺炎感染检测相关产品中应用的能力。
本发明提供的肺炎诊断用circRNA生物标志物及应用、引物和试剂中所用试剂或仪器均可由市场购得。
下面结合实施例,进一步阐述本发明:
实施例1CAP病人与健康全血对照人群全血的circRNA表达分析
一、对象和方法
1.对象
对象来自2015年1月至2016年12月期间北京大学人民医院肺病科收集的样本,最终选取了6对社区获得性肺炎感染和健康人群的样本,样本来源均排除自身免疫性疾病和炎症病性疾病患者,年龄与性别比例与研究对象相似。
2.方法
(1)样本采集:入院后2天内,在无菌的预凝血管中收集外周静脉血样并立即离心;将得到的血清样品保存在-80℃直至分析。
(2)全血总RNA提取:根据产品说明书,用TRIzolTMLS试剂(Invitrogen,USA)提取总RNA。将提取的RNA溶解在不含RNase的水中,并使用Nanodrop 2000(Thermo Scientific,USA)定量。通过1%凝胶电泳确定RNA的完整性。
(3)对样品进行circRNA表达谱分析:
博奥晶典人类环状RNA芯片v2表达谱芯片检测以待测样品的totalRNA为起始,进行体外扩增和荧光标记,过程采用Ambion WT Expression试剂盒。
反转录合成First strand cDNA:以TotalRNA为起始,含有T7启动子序列的Random引物,使用First Strand Enzyme Mix合成First strand cDNA。合成Second strand DNA:用Second Strand Enzyme Mix将DNA-RNA杂合体中的RNA链转化为Second Strand cDNA,合成双链DNA。
体外转录合成cRNA:以Second Strand cDNA为模板,用T7Enzyme Mix合成cRNA。
cRNA纯化:使用RNA纯化柱纯化cRNA,除去反应中的盐、酶等试剂,并对cRNA进行定量、质控。
反转录:以cRNA为模板,RandomPrimer为引物,CbcScriptⅡ酶进行反转录。纯化回收反转录得到的cDNA并定量。
标记:以反转录的cDNA产物为模板,RandomPrimer为引物,用Klenow Fragment酶合成cDNA互补链并掺入带有荧光基团的dNTP(Cy3-dCTP),纯化并定量标记产物。带有荧光基团DNA即可用于芯片杂交。
博奥晶典人类环状RNA芯片v2杂交扫描后的tiff格式图片数据采用FeatureExtraction提数软件进行预处理分析,然后采用GeneSpring GX软件计算基因表达差异和统计学显著性p值。数据处理过程主要包括:芯片扫描tiff图用FeatureExtraction软件处理得到原始数据文件(.txt)。将原始数据文件(.txt)导入GeneSpring软件,写入分组等参数信息。进行每个样品的数据归一化和QC分析。用Cluster3.0软件进行Cluster分析和图形化展示。根据分组信息,进行差异比较,得到差异基因。
3.结果
关于CAP患者全血的circRNA芯片表达谱聚类分析见图1。最终得到8296个明显具有表达变化的差异circRNA,包括有3904个上调circRNA,4392个下调circRNA,并且发现这些异常表达的circRNA能够明显的将病人和健康人群进行分类。鉴于其潜在的诊断价值,本申请通过以下实例采用大规模样本进行筛选异常高表达的单个或circRNA分子标志物组合。
实施例2实时荧光定量PCR检测CAP患者全血中的circRNA分子标志物的表达
一、对象与方法
1.对象
对象来自2015年1月至2016年12月期间北京大学人民医院肺病科收集的样本,最终选取了30对肺炎感染和健康人群的样本,样本来源均排除自身免疫性疾病和炎症病性疾病患者,年龄与性别比例与研究对象相似。
2.方法
(1)样本采集:入院后2天内,在无菌的预凝血管中收集外周静脉血样并立即离心;将得到的血清样品保存在-80℃直至分析。
(2)全血总RNA提取:根据产品说明书,用TRIzolTMLS试剂(Invitrogen,USA)提取总RNA。将提取的RNA溶解在不含RNase的水中,并使用Nanodrop 2000(Thermo Scientific,USA)定量。通过1%凝胶电泳确定RNA的完整性。基于A260/A280值评估的RNA样品的质量;OD值在1.8和2.0之间的样品,浓度>90ng/μL,被认为适合进一步分析。
(3)定量PCR:根据产品说明书使用PrimeScript RT试剂盒(Takara Bio,Japan)进行定量RNA的逆转录。简言之,将630ng RNA(Nanodrop 2000)和4μL PrimeScript RTMaster Mix(Takara Bio,Japan)与不含RNase的水混合至终体积20μL。然后,将混合物在37℃的水浴中温育15分钟以完成逆转录并获得总cDNA。定量PCR用1.6μL的cDNA,5μL的SYBRgreen预混实施例的Taq(Takara Bio公司制,日本),3μL无RNA酶的水,和0.4μM引物(reverse和forward)执行,筛选出可作为分子标志物的circRNA所用引物如下表:

使用ABI PRISM7500序列检测系统(Applied Biosystems公司,LifeTechnologies,USA)测量,在95℃下保持30s预变性,之后在95℃下变性5s,60℃退火30s,延伸时间根据碱基长短决定,设定循环数为50。将circRNA的表达水平用作稳定的内源对照(β-ACTIN)用于标准化。使用2ΔCT方法定量circRNA的相对表达水平。
3.结果
根据倍数变化值选择排名前100位的上调的circRNA并且p<0.05。使用qPCR检查这些circRNA在包含30名CAP患者和30名对照受试者的中的差异表达。以在80%以上的CAP患者外周血样本中明显上调的表达作为选择标准,本申请鉴定了4种circRNA用于测试作为潜在的生物标志物,分别是hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188,CAP患者比健康对照人群的表达明显增高,且具有统计学意义(P<0.05)(图2)。为了评估全血中hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188的诊断价值,用ROC分析了4个circRNA单独作为标志物和作为一个组合对CAP的诊断能力,见下表。

之后绘制了CAP区别于健康对照人群的ROC曲线,见图3。曲线下面积(AUC)显示,全血hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188都具有较好的诊断价值,hsa_circ_0018429的AUC为0.8216,hsa_circ_0026579的AUC为0.7733,hsa_circ_0125357的AUC为0.7730,hsa_circ_0099188的AUC为0.6978,将hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188作为一个组合用于区分CAP和健康对照人群时,发现其AUC达到0.8776,说明hsa_circ_0018429,hsa_circ_0026579,hsa_circ_0125357,hsa_circ_0099188能够作为CAP诊断的分子标志物,且作为组合时,效果更好。
对比例1
在研发过程中,采用多组引物进行PCR扩增,但只有实施例2表1的引物组特异性强。现将其中一组对比引物的序列及对比试验结果呈现如下:

注:Ct值表示扩增量达到阀值时的循环数,本实验每种引物做3个重复,取均值。Ct值越小表示其扩增速度越高。“--”表示没有检测到循环数,即没有特异性产物被扩增。
结果显示,实施例2的引物组与上述对照引物及其他引物组进行对比发现,在相同的荧光定量PCR实验条件下,实施例2的引物组能够对circRNA进行特异性扩增,且扩增速度高于对比引物组。

Claims (4)

1. circRNA生物标志物在制备肺炎诊断试剂中的应用;所述circRNA生物标志物选自如下生物标志物中的一种或几种:
(1)hsa_circ_0018429:序列如SEQ ID NO:1所示;
(2)hsa_circ_0125357:序列如SEQ ID NO:2所示;
(3)hsa_circ_0026579:序列如SEQ ID NO:3所示;
(4)hsa_circ_0099188:序列如SEQ ID NO:4所示。
2.肺炎诊断用circRNA引物在制备肺炎诊断试剂中的应用,其特征在于,所述肺炎诊断用circRNA引物选自如下引物组中的一种或几种:
(1)用于扩增hsa_circ_0018429的引物组;所述用于扩增hsa_circ_0018429的引物组由SEQ ID NO:5所示正向引物和SEQ ID NO:6所示反向引物组成;
(2)用于扩增hsa_circ_0125357的引物组;所述用于扩增hsa_circ_0125357的引物组由SEQ ID NO:7所示正向引物和SEQ ID NO:8所示反向引物组成;
(3)用于扩增hsa_circ_0026579的引物组;所述用于扩增hsa_circ_0026579的引物组由SEQ ID NO:9所示正向引物和SEQ ID NO:10所示反向引物组成;
(4)用于扩增hsa_circ_0099188的引物组;所述用于扩增hsa_circ_0099188的引物组由SEQ ID NO:11所示正向引物和SEQ ID NO:12所示反向引物组成。
3.根据权利要求2所述的应用,其特征在于,所述肺炎诊断试剂还包括用于PCR扩增的其它试剂。
4.根据权利要求3所述的肺炎诊断试剂,其特征在于,所述用于PCR扩增的其它试剂为彩色荧光定量预混试剂。
