背景技术
阻塞性睡眠呼吸暂停(OSA)发生在上呼吸道中的组织在睡眠期间阻塞气道时。大脑会感知到二氧化碳的增加,并会将人唤醒以使呼吸恢复。这种事件被称为呼吸暂停。导致觉醒的局部气道阻塞称为呼吸不足。一个人不太可能记得这样的觉醒,但睡眠被中断。阻塞性睡眠呼吸暂停的严重程度通过觉醒的频率来衡量,如下表所示。
未经治疗的话,OSA不仅使患者慢性疲劳,而且还带来严重的健康后果。不幸的是,尽管人群中OSA的患病率很高,但绝大多数患者仍然未被确诊。据估计,未被确诊的阻塞性睡眠呼吸暂停患者人数高达85%。仅在美国,这可能意味着3500万未确诊的患者。这种大量未确诊的人群是睡眠医学中最大的公共健康问题之一。
确诊率低的原因有很多。睡眠医学领域相对较新。对临床睡眠问题及其原因的认识仍然相对较低。对睡眠呼吸障碍的理解仍在以恒定的速度发展,并且随着更多临床后果被表征,将进一步提高认识。此外,在一些人群中存在一种误解,即睡眠呼吸障碍不是严重的症状,并且通常被认为是简单的打鼾而不予理会。目前对该症状的认识明显低于严重程度。
确诊的又一个障碍是大多数患者必须通过才能被确诊的相对繁琐的过程。通常,初次就诊之后是第二次访问睡眠专家,然后是实验室内多导睡眠图(PSG)。当使用者睡觉时,多导睡眠图是一种非常彻底的观察。它在实验室环境中进行,患者身上至少放置22个电极和传感器,并经由视频整晚进行观察。许多患者觉得这种外部睡眠环境和观察不方便并且被侵扰。数据收集的本质干扰了测试对象的典型的、自然的睡眠习惯。一些患者倾向于尝试周末研究以尽量减少对他们的常规工作的中断量,而许多实验室对于研究、特别是周末研究的等待时间也很长。实验室内PSG研究的成本也很高,通常每晚为2000-3000美元,这对医疗保健系统来说是一个沉重的负担。最近,家庭睡眠测试(HST)设备已被用于将4-8个传统实验室测试通道带入家庭环境。遗憾的是,它们仍然需要处方、不易使用,并且由于错误放置或不充分固定的电极而导致大约20%时间的数据不足。
一些患者报告不希望只是为了获得呼吸暂停诊断的坏消息而完成PSG的整个过程。这种形式的一厢情愿的拒绝会妨碍诊断率,并推迟治疗的开始及其益处。许多患者坚持多年,怀疑他们可能有睡眠呼吸暂停,但没有着手进行测试。这导致医疗保健支出的显著增加和生活质量的下降。研究已计算出未确诊的睡眠呼吸暂停患者的医疗保健利用成本每年额外增加2000美元至12000美元之间。一项研究估计OSA(确诊和未确诊)的总经济成本在每年65亿至1650亿美元之间。睡眠呼吸暂停的诸多影响很难完全估计,例如对患者的人际关系的影响。
除了这些缺点之外,PSG还面临在仅给出一晚数据的情况下尝试理解复杂的患者的挑战。由于PSG的不便和费用,大多数患者只被研究了一晚。然而,睡眠质量不同夜晚会变化很大。每晚许多因素都会不同程度地影响睡眠:运动,饮食,饮酒,压力,环境。PSG的一个显著局限性是只收集一晚的数据。理想情况下,贯穿由这些因素引起的典型波动而从每个睡眠夜晚收集数据,以更完整地表征患者的睡眠。
对于那些确诊的患者,一线治疗是气道正压通气(PAP)。它也被称为持续气道正压通气(CPAP)或自动气道正压通气(APAP)。本公开将所有这样的治疗称为“PAP”。PAP是对于OSA的使用最广泛、最有效的治疗。在PAP中,床边压缩机通过软管和面罩向患者的气道供应加压空气。气压设置得足够高,以在睡眠期间保持气道开放。临床数据显示了使用PAP治疗的显著益处。许多患者报告对他们的健康和生活质量有明显的积极影响。然而,许多患者并非总是感受到PAP治疗的益处。这可能导致患者认为治疗无效,尽管有相反的重要临床证据。一些这样的患者则会停止治疗,或仅偶尔进行治疗。临床数据表明坚持使用PAP治疗产生了最大的益处。遗憾的是,对于这些零星的使用者,在使用者的感知之外没有方便的方法来判断治疗是否真正起作用。
另一类患者将持续使用PAP治疗一段时间,然后一旦感觉良好就停止使用,想当然地认为他们可能已经治愈,或者不再需要PAP治疗。此后不久,他们将再次感到疲劳和认知障碍,让人联想起他们在PAP治疗前的时间。一旦他们认识到这一点,他们就会重新接受治疗。一些医疗保健提供者将此称为溜溜球组,因为他们像上下起伏的玩具溜溜球一样对治疗犹豫不决。
遗憾的是,对于所有PAP使用者来说,没有方便的方法以科学合理的方式确定每晚使用和不使用他们的PAP设备的效果。
睡姿在阻塞性睡眠呼吸暂停中起到非常显著和被极度低估的作用。当以仰卧姿势睡觉时,重力对舌头、扁桃体、软腭和上呼吸道的其它结构的影响更为明显。临床研究表明,大多数睡眠者在背卧睡觉时的阻塞量至少是侧卧睡觉时的两倍。仰卧睡觉时发生的阻塞更严重:它们的持续时间更长,并且导致血氧水平下降更严重。对于一些患者,仰卧的呼吸暂停-呼吸不足指数(“AHI”)与侧卧AHI的比率超过4比1。睡姿是发生呼吸暂停或呼吸不足的可能性的最大决定因素之一。因此,睡姿也是维持呼吸道通畅所需的PAP压力的最强决定因素之一。在实验室内滴定(titration)研究中,PAP压力设置在REM阶段/快速眼动睡眠期、仰卧姿势睡眠期间确定。这是因为在仰卧睡觉的REM阶段,气道最容易塌陷。因此,PAP压力设置通常被设定为覆盖最坏情况症状。事实上,除非存在REM仰卧睡眠,否则滴定研究不被认为是成功的。在一些研究期间,技术人员将进入房间并推动患者仰卧,以便获得REM仰卧睡眠和相关的压力设置。
研究表明,当未仰卧时,患者所需的PAP压力明显降低。一项研究发现,所有患者(不仅仅是姿势OSA患者)在侧卧睡觉期间所需的压力几乎低3cm水柱。对于那些有姿势性OSA的人,背卧和侧卧之间的压力差会大得多。
大多数人不需要研究来识别睡姿对呼吸的作用。几个世纪以来,据报道,人们在仰卧睡觉时会大声打鼾,并且常见的建议是肘击大声打鼾者以使他们侧身而安静下来。
尽管有这些证据,睡姿在OSA中的作用对于绝大多数OSA患者来说依然是一个谜。很少有患者了解睡姿在呼吸暂停中的作用。
因此,迫切需要一种以方便的方式确定姿势的作用和氧饱和度降低的严重程度的途径。这适用于未确诊以及确诊的患者、那些坚持PAP治疗的患者以及那些难以坚持使用PAP治疗的患者。所有人都将从更好地了解他们的OSA、睡姿的作用以及他们的治疗的每夜的影响中受益。除了PAP治疗之外,了解OSA替代治疗的效力以及睡姿在其中的作用的能力也是非常有益的。
发明内容
本发明提供一种姿势阻塞性睡眠呼吸暂停检测系统,其获得关于受试者睡姿的信息(例如,仰卧睡觉、侧卧睡觉)并将其与指示呼吸停止或减少的信息(例如,血氧水平)组合。在一些实施例中,检测系统将脉搏血氧计与姿势传感器组合,可选地一起组合在同一设备或壳体中。脉搏血氧计是每项睡眠研究跟踪夜间的血氧饱和度的核心。显著的呼吸暂停通常会使血氧饱和度降至标准水平以下。一些其它实施例可以增设可选的其它检测器以检测并可以分析例如鼾声(麦克风和/或加速度计)、呼吸速率(加速度计和/或心率算法)、呼吸努力(加速度计、心率和/或麦克风)、睡眠/觉醒周期(心率和/或加速度计)、鼻腔开口处的气流(热敏电阻、流量传感器和/或压力传感器)。可以使用这些信息中的一部分或全部,例如,以评估睡眠呼吸障碍(特别是睡眠呼吸暂停)的风险并可以诊断睡眠呼吸障碍。受试者也可以使用它来评估CPAP功效。

在一些实施例中,本发明提供了一种用于准确评估阻塞性睡眠呼吸暂停风险以及分析姿势在受试者阻塞性睡眠呼吸暂停中的作用的系统。在一些实施例中,本发明分别获得关于受试者的仰卧睡眠事件和侧卧睡眠事件的信息,以及混合的AHI。该系统可由受试者在家中使用,并且不需要使用睡眠实验室。
在一些实施例中,该设备由电池例如纽扣电池、碱性电池、LiPo电池或Li离子电池供电。该设备可具有充电电路,例如感应充电电路,并且它可以使用弹簧销与USB兼容。

阻塞性睡眠呼吸暂停在几乎所有病例中都依赖于姿势。仰卧睡觉的人将经历更频繁和更严重的气道阻塞,因为重力将舌头、扁桃体、软腭和其它结构拉入气道。大多数OSA患者在仰卧睡觉时会经历在侧卧睡觉时的至少两倍的事件。有些人在仰卧睡觉时会遇到4到5倍的阻塞。相比之下,患有中枢性睡眠呼吸暂停和其它导致睡眠期间血氧下降的病症的患者通常较少依赖于姿势。由于几乎每个人都会花时间进行仰卧和侧卧睡觉,因此可以使用脉搏血氧测量数据和姿势数据的组合来将OSA与中枢睡眠呼吸暂停(CSA)和其它非姿势相关的病症和状况进行区分。
本发明将用于光学测量的传感器与可以确定姿势的传感器组合。该设备可以在夜间被佩戴在皮肤上。姿势测量构件可以或可以不必在给定患者上校准,以能够将仰卧睡觉与侧卧睡觉进行区分。该设备将在一个或更多个晚上的持续时间内记录血氧饱和度和姿势。然后,重要的是,可以分析该信息以比较患者仰卧睡觉时的呼吸暂停事件和患者侧卧睡觉时发生的事件的频率和/或严重程度。此分析可以在设备本身上进行,也可以在使用通过设备收集的数据的其它设备上进行。该分析可以比较低血氧饱和度事件的频率、这些事件的严重程度、或两个睡姿的频率、严重程度和/或持续时间的组合。如果在仰卧睡觉期间血氧不足的频率、严重程度或持续时间明显大于侧卧睡觉期间,则数据分析将指示OSA,而血氧不足的次数、时长和严重程度的大致相等可指示CSA或其它另一种不常见的睡眠障碍的可能性。
在一些实施例中,生成血氧饱和度随时间的数据曲线图。曲线上方的表示患者记录的血氧饱和度与完全(100%)饱和度之间随时间的差异的区域可用于差异地诊断OSA和CSA。在患OSA的患者中,与侧卧睡觉相比,曲线上方的区域通常具有更高的仰卧睡觉相比侧卧睡觉的每分钟的事件比率。患CSA和其它病症或状况的患者具有较低的仰卧与侧卧的每分钟事件的比率。曲线上方的这个区域可以合并血氧饱和度降低的频率、持续时间和严重程度。
本文描述的系统能够在中枢性和阻塞性呼吸暂停之间进行区分。在一个实施例中,可以使用姿势数据和血氧饱和度SpO2数据的组合来确定姿势阻塞性睡眠呼吸暂停(pOSA)的程度。通常,pOSA定义为在处于仰卧姿势时具有至少两倍的事件(呼吸暂停或呼吸不足)数量。此外,在仰卧姿势发生的饱和度降低事件持续更长时间并且达到更低的饱和度水平。本文描述的发明提供了确定受试者姿势与每个饱和度降低事件之间的相关性的能力。这种相关性对于区分CSA和OSA很重要。在大多数时间,OSA是姿势驱使的。研究表明,高达75%的OSA患者是pOSA。几乎每个患者都在仰卧姿势有更多事件。相比之下,CSA远非如此姿势驱使。尽管有一些病例报告说CSA可能因仰卧睡觉姿势而加剧,但绝大多数CSA事件都与姿势无关。利用这些知识,我们可以估计使用者患有CSA的可能性。如果使用者有显著事件,但他们与仰卧睡觉姿势无关,则强烈怀疑CSA。对于这些使用者,本应用可以建议使用该设备的进一步研究,以及使用其它标准的护理测试设备的进一步研究。在一个实施例中,如果由于测试的初始结果而怀疑CSA,则指导使用者将设备放置在他们的胸部以用于后续测试。可以使用相同的设备和应用完成此测试,从而节省大量时间、金钱和便利。在随后的测试夜晚,使用者将设备放在胸部上以从该位置收集运动数据。对于患有CSA的患者,停止呼吸努力对于CSA来说是与CSA有区别并且独特的。患有CSA的患者将这样停止呼吸,其将在运动数据中显示为呼吸努力运动的明显缺失/失效。以这种方式,这里描述的系统可以非常合理地区分OSA和CSA。
实施例
在一个实施例中,传感器可位于头部或躯干上或两者上。通过在头部和躯干两者上放置传感器,可以收集更多数据,并且可以从来自两个位置的数据的比较中得出更精细的观察结果。例如前额上的头部传感器可以收集头部姿势、脉搏血氧饱和度和声音数据。躯干上的传感器可放置在颈部、锁骨、胸部、腹部、横膈膜上的任何位置。躯干传感器可以检测运动和姿势数据。根据该数据,可以确定身体姿势、呼吸速率、呼吸努力和心率。
胸部区域上的第二传感器的一个优点在于它可以提供关于呼吸努力的数据。通常在呼吸暂停期间,呼吸努力增加,因为身体试图获得更多的氧,但由于阻塞而无法获得。这种努力是明显的,并且可以通过躯干上的运动传感器来捕捉。增加这种附加通道功能还可满足某些健康系统的测试通道要求。
在另一实施例中,同一传感器装置在多个晚上被佩戴。多个晚上的数据有助于积累与使用者睡眠质量有关的其它信息。由于睡眠质量每晚变化,所以多个晚上的数据可以更好地指示使用者的睡眠问题。
在一个实施例中,传感器被佩戴在头部上多个晚上,以收集关于使用者睡眠的基准信息,例如氧合作用、运动和呼吸紊乱。一旦建立了这种模式,就可以在躯干、胸部或腹部佩戴同一传感器。在该位置,传感器将检测与呼吸努力相对应的横隔膜和胸部的运动。如果传感器检测到长时间的运动暂停,则这表明可能存在中枢性睡眠呼吸暂停。这样,可以在两个不同位置在多个晚上使用同一设备来区分OSA和CSA。
该系统由可佩戴传感器、数据分析平台和算法以及用于收集使用者输入和通信结果的使用者界面组成。
可佩戴传感器
该设备包括用于获得光电容积脉搏波描记(PPG)信号的光学传感器。光以至少两种不同的波长发射到组织中。在每个波长下测量反射回光学传感器的所得光。比较结果以确定氧合血红蛋白浓度。光学传感器可以测量血氧饱和度、脉搏率和心率变异性(HRV)。
该设备还包括用于确定姿势和加速度的传感器。在一个优选实施例中,这通过加速度计完成。加速度计数据用于确定睡眠者的头部姿势。如本文所讨论的,睡觉姿势对呼吸睡眠障碍至关重要。运动数据还可用于识别不宁腿综合征(RLS),这是一种涉及移动腿部的冲动的睡眠障碍。加速度计数据还可以指示使用者的唤醒和睡眠间隔,并且用于计算总睡眠时间。RLS的特征在于在睡眠和睡眠过渡期间移动腿部的冲动和感觉。RLS运动的独特标志可在加速度计数据中检测到。RLS运动是急动和重复的,其可以从典型的睡眠运动信号中辨别出来。在一个实施例中,可以从伴随app或调查问卷上的使用者输入数据或从前额加速度计数据中的测量值和标志来评估RLS风险。然后,如果怀疑是RLS,则可以使用该设备在额外的晚上进行观察和测量,其中将设备放在腿上以测量运动。由于在RLS事件期间腿上的特别明显的运动信号,这种方法可以非常确定地确认可疑的RLS。体动记录仪(Actigraphy)研究运动数据以确定生理事件。在一个实施例中,使用来自加速度计的运动数据来确定睡眠阶段。在REM睡眠期间,由于身体的禁止将梦境表现出来的保护机制,使用者通常几乎不动。因此,运动数据可以根据很少运动或没有运动的时间来辨别可疑的REM阶段睡眠。在阶段1和阶段2睡眠期间,在运动数据中发现的呼吸信号随着使用者过渡到睡眠而减慢。
也可以根据由设备收集的PPG波形来确定使用者的睡姿。波形特征根据从仰卧睡眠到侧卧睡眠的睡觉姿势而变化。甚至左右侧卧睡觉姿势也是可区分的。由于典型的睡眠者每晚改变姿势20次或更多次,该设备通常会有很多机会评估每个姿势在PPG波形上的样子。
由于睡觉姿势影响PPG波形,因此使用从加速度计收集的姿势数据来通知PPG数据的处理也是有利的。例如,由于我们知道PPG的AC分量的幅值在侧躺姿势减小,所以我们可以在这种情况下使用不同的滤波器来处理PPG以产生更准确的SpO2结果。相应地,当加速度计数据指示使用者正在仰卧睡觉时,我们可以在知道幅值将更大的情况下相应地处理PPG信号。
该设备可以确定使用者何时坐起、站起来和离开床。这对于跟踪在夜间上洗手间特别有用。夜尿——即夜间排尿的冲动——是睡眠呼吸暂停的症状。一旦使用诸如CPAP的治疗,许多使用者报告夜尿减少。该设备能够量化这种减少,为使用者提供正向强化。
通过加速度计收集的运动数据也可用于加强血氧计的测量。在脉搏血氧测量法中,在运动事件期间可能产生显著的信号误差。使用运动传感器,可以识别这些运动事件。在一个实施例中,设备算法将标记SpO2值,该SpO2值是根据由运动传感器确定的运动事件期间收集的数据计算的。在一个实施例中,该算法将丢弃与高于阈值的运动事件相关联的SpO2值,高于该阈值使得SpO2明显不准确。在典型使用情况下,传感器能够实现SpO2在约+/-3%内的精度。在运动事件期间收集的精度降低超过阈值如+/-±5%的数据可能会被丢弃。在另一实施例中,当存在运动时,使用考虑了运动环境的不同的系数组来计算SpO2。
在一个实施例中,PPG信号本身用于确定何时存在运动事件并因此滤除对SpO2的计算。运动事件在PPG波形中以明显的方式显示为增加的信噪比。在另一实施例中,PPG中的该运动标志可以与来自运动传感器的数据耦合,以确定何时丢弃或调整SpO2数据计算。
运动数据还可以识别咳嗽,从而可以测量疾病和过敏。咳嗽通常导致加速度计数据的3轴中占主导地位的一个中发生显著的、突然的运动。该标志由软件识别并记录为咳嗽事件。测量和跟踪咳嗽使使用者能够理解其睡眠环境中可能导致睡眠中断的触发因素。
在另一实施例中,运动数据用于识别由打鼾引起的振动。来自打鼾的运动信号标志的特征在于持续典型的1-2秒的稳定增加的振动,并且记时为与呼吸周期一致。打鼾每分钟最多发生15次,但更常见的是每分钟6-12次。运动数据表现出随着它们增加而波动的值,然后在运动中更快地下降。在一些打鼾者中,数据显示长时间没有明显的运动,随着突然打鼾将暂停打破而会被更重要的运动事件打断。
可以根据设备传感器数据来确定使用者的呼吸速率。PPG波形包含充足的用于使用本领域中描述的信号处理技术来推导呼吸速率的信号。来自加速度计的体动记录仪数据也能够揭示呼吸速率。呼吸速率在运动数据中作为正弦模式显示。可以根据该信号的处理来确定呼吸速率。将从运动传感器得到的运动数据与PPG信号数据相结合可以增强呼吸速率计算的精度。另外,可以根据数据的处理来确定呼吸努力,尤其是当存在呼吸窘迫时。呼吸信号中显著增加的运动指示呼吸窘迫。这可以在呼吸暂停或呼吸不足期间发生,并且提供另一信号以识别和确认呼吸事件。
在一些实施例中,麦克风捕获声音信号,并允许分析呼吸中的打鼾声音和暂停。麦克风可以结合在设备中。或者,系统可以使用智能手机、电话、平板电脑或其它连接设备中的现有麦克风。
在一些实施例中,该设备包括用于前额的传导性传感器,其用于收集脑电图(EEG)信号。EEG可用于指示大脑活动、睡眠分段和识别睡眠期间的觉醒。
在一些实施例中,包括压力和流量传感器以评估气流。这可以通过在鼻孔正下方放置开口管来实现,该开口管与放置在设备中的压力传感器连通。或者,可以在鼻孔正下方放置一个具有无线连接性的附加贴片,并利用压力、温度、声音或流量传感器来评估呼吸气流。或者,放置在鼻孔下面的附加贴片可以使用用于电源和/或数据通信的细线电缆连接到设备。
在一些实施例中,附加传感器包括用于评估睡眠环境中的环境光量的环境光传感器、用于评估关键呼吸气体如在调节呼吸驱使中起关键作用的CO2的量的气体传感器、以及用于评估睡眠环境的空气质量的其它环境传感器。
传感器装置包括电子器件壳体、电子器件和柔性皮肤附接元件。在一个优选实施例中,电子器件壳体是可重复使用的并且可以再充电。在另一实施例中,电子器件和壳体是一次性的。可以为每次使用附接新的柔性皮肤附接元件。该柔性皮肤附接元件可以是在壳体下方的粘合剂层。或者,它可以为较厚层,其在两侧具有粘合剂并且其间具有由诸如泡沫的间隔材料制成的柔性层。在另一实施例中,柔性皮肤附接元件可以是双面贴片(overpatch),该双面贴片(overpatch)具有粘合剂下侧面和施加至壳体的全部或一部分以将其固定到使用者的装置。又一替代实施例具有位于固定到前额上的头带内的电子器件壳体。另一实施例将电子器件集成在眼罩中。又一实施例将电子器件固定在耳塞中,该耳塞装配在耳道的中途。另一实施例将电子器件结合到睡帽中,从而将传感器放置在前额上。
传感器位置
在一个实施例中,该设备被放置在前额上。前额为收集感兴趣的信号提供了理想的位置。在该实施例中,该设备可以被放置在前额的上半部分上,相比于眉毛更靠近发际线。上前额具有良好的灌注,从而产生良好的信号。该设备被放置在两眼之间对齐、眉毛上方和发际线下方的任何地点。放置在这种被描述的窗口(box)的上部上通常产生最佳信号。前额的上半部分的脉管系统的特征在于多个较小的血管,从而导致更好的灌注。该区域在PPG上具有更好的AC/DC信号比率,这允许更好的信噪比。
可供放置设备并实现充足信号的其它地点尤其包括耳垂、耳道,耳后颅骨上、鼻孔、鼻中隔、锁骨、颈部、喉结、背部、胸骨、胸部或腹部。
使用者界面
在一个实施例中,智能手机、平板电脑或类似的通信设备(以下称为智能设备)与系统一体化。智能设备上的应用程序从使用者收集信息。该应用程序引导使用者通过简化的使用者数据集合,包括年龄、性别、身高、体重和自我报告的睡眠质量。该集合可包括基于临床验证的睡眠调查问卷的调查,例如STOP-BANG、Epworth嗜睡量表、柏林调查问卷。已经证明这些问题可以准确地识别呼吸暂停。将这些数据纳入我们的系统可以提高评估的准确性。此外,该应用程序包括被设计为识别可能有更复杂的睡眠障碍并可能从多导睡眠图测试中受益的个体的某些问题。如使用者或同床伴侣所报告的关于睡眠期间腿部运动的问题可以帮助识别不安腿综合征(RLS)。应用程序调查还可以包括识别具有更复杂的健康问题如充血性心力衰竭或肺功能不全的使用者的问题,这些问题需要咨询医生。
数据分析
数据分析平台在一个或优选地多个晚上从传感器获取数据,并将其与调查问卷的结果相结合,以创建非常准确、真实和个性化的对使用者睡眠的评估。将测得的客观睡眠数据与来自验证问题的使用者报告数据相结合的能力是该设备所独有的。在一个实施例中,来自客观测量的数据将优先于使用者报告的数据。然而,在来自测量数据的临床结论不是很清楚的情况下,使用主观的使用者报告数据来确定使用者的下一步骤。例如,如果测得的隔夜SpO2数据没有显示出显著的去饱和度,但是使用者报告数据显示出显著的疲劳和睡眠抱怨,那么应用程序将建议进一步研究以检查疲劳的原因,因为不会怀疑患OSA。在这种情况下,该应用程序将建议针对呼吸努力相关觉醒(RERA)、失眠或其它睡眠障碍调查进一步的测试。这只是一个例子,用于说明将测得的睡眠数据与使用者报告数据相结合以帮助确定睡眠问题的原因的能力。可以任选地将该数据与历史数据进行比较,以使使用者了解他们与具有相似年龄、性别、体重指数和睡眠习惯的同龄人相比所处的位置。研究表明,提供对同龄人群组的参考可以增强睡眠研究结果的影响,并可以用作行为改变的强大动力。
影响行为
我们在此描述的系统的一个重要功能是主动影响使用者行为的能力。实现这一目标的一种关键方式是通过关于治疗的反馈环。除了识别和诊断睡眠呼吸暂停外,该系统还可以提供关于各种治疗疗法的使用反馈。PAP使用者可以使用该设备来验证PAP设备的功效。如果需要,可以使用该设备测试和评估不同水平的PAP治疗。对于新确诊的患者,反馈尤为重要。传感器可以为这些使用者提供有用的反馈。PAP治疗的大多数新使用者分为两类:立即感受到治疗效果并迅速成为坚持的使用者的人,以及感受到不是很直接的益处的人。对于后一组,有关PAP治疗在夜间将他们的呼吸改善了多少的定量反馈非常有助于说服他们获益如何超过治疗的不便。通过传感器,他们可以跟踪他们在PAP上的进度。研究表明,在PAP治疗的最初几周内使用非常表明长期依从性。
选择替代疗法的使用者也可以从呼吸暂停检测系统中受益。PAP治疗有几种替代方案。PAP治疗反复被证明是最有效的,但只有在使用的情况下才有效。对于不能选择或不选择使用PAP的患者,存在一些选择,包括:口腔器具、手术、姿势疗法、生活方式改变、口腔压力疗法。然而,这些疗法的功效难以评估。许多使用者没有密切关注或被提供这些疗法或手术后的睡眠测试评估。因此,如果他们的呼吸暂停未得到充分治疗,他们可能会冒着健康风险。呼吸暂停检测系统可以与所有替代疗法结合使用以评估其功效。这种改进的全面评估途径使使用者能够自信地使用替代疗法,或者停止在未从中受益的方法上花费时间和金钱。

图15示出了处于两个不同位置的可佩戴传感器组件100。一个位置在前额上。另一个位置在胸部、腹部或躯干上。这可以通过同时佩戴两个传感器或通过在不同的夜晚在每个位置佩戴一个传感器来实现。前额放置的优点在别处讨论。胸部放置提供了几个额外的优点。睡眠期间胸部的运动从传感器产生明显的运动标志。该信号可用于确定呼吸努力。具体而言,这可用于区分阻塞性睡眠呼吸暂停和中枢性或混合性睡眠呼吸暂停。中枢性睡眠呼吸暂停的特征在于是没有呼吸努力。因此,如果使用者在先前的测试中已经显示出呼吸暂停的证据,则使用放置在胸部上的传感器可以判断是否存在相应的呼吸努力的暂停。通过比较来自放置在前额上的传感器的结果与来自放置在胸部上的传感器的结果,可以判断中枢性睡眠呼吸暂停是否有责任。在一些情况下,这种信号差异将足以指示确诊。在一些情况下,信号差异可能仅足以指示进一步测试对于明确地诊断使用者是合理的。中枢性睡眠呼吸暂停(CSA)是一种神经系统疾病,其中即使气道畅通,呼吸也会在睡眠期间停止。OSA和CSA都会扰乱睡眠并带来严重的长期健康后果。这些睡眠障碍需要不同的治疗,因此对于诊断测试来区分它们是很重要的。
图16A以侧视图示出了放置在皮肤组织150上的可佩戴传感器组件100的一个实施例。传感器接口元件160从壳体105的周边部分向下延伸,以形成光学接合表面,从而将光学元件170与皮肤组织150直接耦合。这种直接耦合有助于确保通过光电检测器捕获高质量的原始光信号。图16B示出了本实施例的可佩戴传感器组件100的侧视图。示出了光学耦合元件170超过柔性皮肤附接元件110的表面突出到皮肤表面中的方式。该突出的典型距离约为0.5mm,典型范围为0.25mm至0.75mm。然而,该距离可以变化为小至0.1mm或大至2.5mm或以上。元件必须充分接触皮肤以产生所需的光学耦合并且没有气隙以获得最佳光学信号。然而,如果元件突出到皮肤中太远,则它们会显著减少血液灌注并导致较差的光电容积描记信号。如本文所述,需要最佳距离以产生有利信号。光学耦合元件170由光学透明材料组成。为了成功地将光学构件与皮肤光学地耦合,重要的是消除任何气隙。因此,可以消除气隙并且光学地透明的材料符合两个关键参数。用于本发明的一种这样的材料是双组分环氧树脂。这种材料在制造过程中可以流动以消除气隙,并且在硬化后光学透明。
图17示出了可佩戴传感器组件100的另一实施例的下侧视图,该侧与皮肤组织接触。值得注意的是接合所述组织的传感器接口元件160的光学接合表面。在该接口元件160内有光源180和光电检测器190。如在脉搏血氧测量中众所周知的,光源180可以发射两种不同波长的光(例如,红光和红外光),并且光电检测器190可以检测两个光波长吸收的基于时间的变化以用于计算使用者的血氧饱和度。关键尺寸由A表示,即从能量发射元件180的中心到能量检测元件190的中心的距离。在一个实施例中,该尺寸A为7mm。在另一些实施例中,它可以在5mm-10mm的范围内并且产生高质量信号。重要的是足够分开以使得从光源180发射的光能必须穿过足够的血液灌注组织——包括脉动血液——行进以产生具有足够时变或AC分量的信号,从而允许精确的血氧饱和度测量。发射元件和检测元件应当足够靠近,以便检测到的时变信号主要由来自发射件而不是任何其它源的光能组成。
图18A和18B分别示出了可佩戴传感器组件100的一个实施例的下侧视图和侧视图。凸起的传感器接口元件160的光学接合表面执行至少两个关键功能。一,它有助于将设备的光学构件与皮肤光学耦合,如本文所讨论的。二,它还由特别选择为不允许设备发射的感兴趣波长通过的阻光材料构成。一些可能的材料包括吸收感兴趣的波长的热固性和弹性体材料,包括但绝不限于:ABS,聚碳酸酯,SantopreneTM热塑性塑料,丁基苯乙烯等。感兴趣的波长通常为标称的670nm、850nm和900nm。
图18B示出了沿图18A中的线B-B截取的可佩戴传感器组件的一个实施例的侧视图。组件内的关键构件包括光电检测器元件、能量发射元件、加速度计、无线通信构件、微处理器和电池。还示出了充电端口,这里在图18A中被图示为USB连接器。为清楚起见,图中省略了带粘性的柔性皮肤附接元件。微处理器控制设备的功能。能量从能量发射元件发射到组织中。在一个优选实施例中,发射的能量是约650nm和930nm的两种不同波长的光。其它波长是可能的,其中一个在红色光谱中,另一个在红外光谱中。该光穿过组织,然后其一部分由光电检测器接收。光电检测器接收光能并输出与对应于每个波长的光强的强度对应的电压。根据该数据,确定PPG波形,并计算血氧饱和度SpO2。PPG是这些接收信号的强度的AC分量的随时间的曲线图。通过以下方式来计算SpO2:对于给定时间t,计算ΔAred=(Redt-Redt-1)/((Redt+Redt-1)/2)和ΔAIR=(IRt-IRt-1)/((IRt+IRt-1)/2);然后绘制ΔAred对ΔAIR,则最佳拟合线的斜率为R。然后通过系数调整R值以产生SpO2,例如使用:(x)R2+(y)R+z=SpO2。在一个实施例中,SpO2由设备上的微处理器计算。在另一实施例中,来自光电检测器和加速度计的原始信号数据由无线通信构件发送到另一设备或服务以进行处理。在又一实施例中,原始信号数据在由无线通信构件发送到另一设备或服务之前接受预处理以进行进一步处理。
图18C示出了壳体105中的光电检测器190的详细侧视图(图18B中标记为“C”)。值得注意的是渐缩的截锥形表面194,其从光电检测器190延伸并向下渐缩至壳体中的开口196。该向下的锥角对于允许最大量的光信号到达传感器是重要的。在一个实施例中,该锥角为45度,但其可以在5-85度的范围内。尺寸G表示孔洞的开口的直径。在一个实施例中,该尺寸G略小于光电检测器190的工作表面192。图18C还示出了尺寸H,即从光电检测器190的表面192到壳体中的开口196的距离。在一个实施例中,该距离被最小化以允许最佳信号通过。在于开口196与光电检测器190的工作表面192之间延伸的容积中设置有光学透明材料198。被支承在壳体内的其它设备构件包括姿势传感器(例如,加速度计)、控制器和通信器(例如,无线发射器)。
图18D示出了光源180的详细剖面侧视图。在一个实施例中,描述壳体中的光源开口184的W尺寸被大小确定为小于光源180的有效区域182。尺寸K示出了从能量发射元件的表面到壳体的外表面的距离。在一个实施例中,该距离被最小化以允许最佳量的光通过。在于开口184与光源180的有效区域182之间延伸的容积中设置有光学透明材料186。在一个实施例中,光源是一对发光二极管(LED)。在一个实施例中,这些LED在大约650nm和大约930nm的波长下操作。许多其它波长是可能的。发现效果良好的一些特定波长为656-657nm和930-932nm。
图19示出了根据本发明的一个实施例的用于将设备100放置在使用者的前额200上的区域。虽然可以在前额上的任何地方获得足够的读数,但是将设备100放置在前额的上半部分中导致明显更好的信号质量。图19中所示的区域位于两眼上方和其间并且由眉毛与发际线之间的区域限定的前额的上半部分。该区域的特征在于更多的微血管和更小的血管,这通常导致更好的灌注信号。这产生了PPG波形的较大AC分量,从而允许从信号中导出更多细节。从该区域获得的读数通常可以是双倍强度,是从前额的下半部分获得的灌注指数的两倍。对于最准确的信号结果,该上半区域是优选的。
图20A示出了取自处于仰卧姿势的受试者的PPG信号的一部分。该信号的幅值表示灌注指数,即脉动血流与非脉动血液的比率。该信号还显示具有相对窄的峰部的特征形状。相反,图20B和图20C中所示的波形呈现出约为图20A的一半的较低的幅值和灌注指数。另外,图20B中的波形在顶部是平整的并且显示出与图20A的波形截然不同的形状。同样,图20C中的波形在顶部也更圆,并且显示出与图20A的形状截然不同的形状。波形和灌注指数的这些变化可用于确定睡眠者的姿势。首先,在每个仰卧和非仰卧情形中表征PPG波形,这允许针对每种情形确定阈值水平。这些阈值水平用于与给定的PPG波形进行比较以输出睡眠者的姿势确定。在另一实施例中,使用数字信号处理技术来比较仰卧和非仰卧睡眠的波形,以表征每个波形的形状。这用于确定波形形状的阈值,然后将其应用于给定的PPG波形以确定使用者的姿势。
本文描述的系统和方法的另一个优点是识别其它与氧有关的疾病和病症的能力。这些病症包括高原病、肺病、慢性阻塞性肺病COPD、肺气肿、支气管炎、肺水肿、纤维化、肺炎、先天性缺陷、充血性心力衰竭、贫血、麻醉剂使用、麻醉剂使用和某些中毒如氰化物中毒。
Claims (40)
1.一种阻塞性睡眠呼吸暂停检测设备,包括:
光学接合表面,其适于接合使用者的皮肤;
光源,其适于从所述光学接合表面出射光;
光电检测器,其适于检测光学接合表面处的光并产生检测到的光信号;
姿势传感器,其适于确定患者的睡觉姿势;
控制器,其适于确定由所述检测到的光信号计算出的血氧饱和度值和来自所述姿势传感器的使用者姿势信息并将血氧饱和度值和使用者姿势信息记录在存储器中;和
壳体,其支承所述光学接合表面、所述光电检测器、所述光源、所述姿势传感器和所述控制器。
2.根据权利要求1所述的设备,还包括通信器,其适于传送血氧饱和度值和使用者信息姿势信息。
3.根据权利要求2所述的设备,其中,所述通信器包括设置在所述壳体中的无线发射器。
4.根据权利要求1-3中任一项所述的设备,其中,所述光学接合表面成形为接合使用者前额上的皮肤。
5.根据权利要求1-4中任一项所述的设备,其中,所述壳体还包括包围所述光学接合表面的外周表面,所述光学接合表面从所述外周表面向下延伸。
6.根据权利要求5所述的设备,其中,所述光学接合表面设置在距所述外周表面0.1mm至2.5mm处。
7.根据权利要求5所述的设备,其中,所述光学接合表面设置成距所述外周表面0.25mm至0.75mm。
8.根据权利要求5-7中任一项所述的设备,还包括设置在所述外周表面下方的柔性材料,所述光学接合表面从所述柔性材料的下表面向下延伸。
9.根据权利要求8所述的设备,其中,所述柔性材料包含粘合剂。
10.根据前述权利要求中任一项所述的设备,其中,所述光学接合表面包含优先地吸收与所述光源出射的光波长对应的光波长的材料。
11.根据前述权利要求中任一项所述的设备,其中,所述光源被配置为经所述光学接合表面中的光源开口出射光,并且所述光电检测器被配置为检测通过所述光学接合表面中的光电检测器开口的光,所述光源开口和光电检测器开口设置成相距5mm-10mm。
12.根据权利要求1-10中任一项所述的设备,还包括从所述光学接合表面中的开口延伸到所述光电检测器的工作表面的渐缩的截锥形表面。
13.根据权利要求12所述的设备,其中,邻近所述光电检测器的截锥形表面的直径小于所述光电检测器的工作表面的直径。
14.根据权利要求1至10中任一项所述的设备,还包括在所述壳体中的开口,所述邻近所述光源的开口的直径小于所述光源的工作区域的直径。
15.根据前述权利要求中任一项所述的设备,其中,所述姿势传感器包括加速度计。
16.根据前述权利要求中任一项所述的设备,其中,所述姿势传感器包括由所述控制器使用以根据检测到的光信号或血氧饱和度值来计算患者姿势的一组指令。
17.一种阻塞性睡眠呼吸暂停检测设备,包括:
光学接合表面,其适于接合使用者的皮肤;
光源,其适于经所述光学接合表面中的光源开口出射光;
光电检测器,其适于检测通过所述光学接合表面中的光电检测器开口的光并生成检测到的光信号;
姿势传感器,其适于确定患者的睡觉姿势;
控制器,其适于确定由所述检测到的光信号计算出的血氧饱和度值和来自所述姿势传感器的使用者姿势信息并将血氧饱和度值和使用者姿势信息记录在存储器中;和
壳体,其支承所述光学接合表面、所述光电检测器、所述光源、所述姿势传感器和所述控制器,所述壳体包括包围所述光学接合表面的外周表面,所述光学接合表面从所述外周表面向下延伸0.1mm至2.5mm。
18.根据权利要求17所述的设备,其中,所述光学接合表面设置成距所述外周表面0.25mm至0.75mm。
19.根据权利要求17所述的设备,还包括设置在所述外周表面下方的柔性材料,所述光学接合表面从所述柔性材料的下表面向下延伸。
20.根据权利要求19所述的设备,其中,所述柔性材料包含粘合剂。
21.根据权利要求17所述的设备,其中,所述光源开口和所述光电检测器开口设置成相距5mm-10mm。
22.根据权利要求17所述的设备,还包括通信器,其适于传送血氧饱和度值和使用者信息姿势信息。
23.根据权利要求22所述的设备,其中,所述通信器包括设置在所述壳体中的无线发射器。
24.根据权利要求17所述的设备,其中,所述光学接合表面包含优先地吸收与所述光源出射的光波长对应的光波长的材料。
25.根据权利要求17所述的设备,还包括从所述光电检测器开口延伸到所述光电检测器的工作表面的渐缩的截锥形表面。
26.根据权利要求25所述的设备,其中,截锥形表面的邻近所述光电检测器的直径小于所述光电检测器的工作表面的直径。
27.根据权利要求17所述的设备,其中,所述光源开口的直径小于所述光源的工作区域的直径。
28.根据权利要求17所述的设备,其中,所述姿势传感器包括加速度计。
29.根据权利要求17所述的设备,其中,所述姿势传感器包括由所述控制器使用以根据所述检测到的光信号来计算患者姿势的一组指令。
30.一种从受试者获得阻塞性睡眠呼吸暂停信息的方法,所述方法包括:
将检测设备的光学接合表面贴靠放置在所述受试者的皮肤表面上;
将光经所述光学接合表面传输到所述受试者的皮肤表面中;
检测从所述受试者的皮肤表面通过所述光学接合表面的光;
根据所述检测到的光来确定血氧饱和度;
利用所述设备内的姿势检测器获得受试者的姿势信息;以及
将血氧饱和度和姿势信息存储在存储器中。
31.根据权利要求30所述的方法,其中所述皮肤表面包括所述受试者前额上的皮肤表面。
32.根据权利要求30或31所述的方法,还包括将血氧饱和度与姿势信息时间关联。
33.根据权利要求30-32中任一项所述的方法,还包括由血氧饱和度来计算姿势信息。
34.根据权利要求30-33中任一项所述的方法,还包括传送血氧饱和度和姿势信息。
35.根据权利要求34所述的方法,其中,所述传送包括显示血氧饱和度和姿势信息。
36.根据权利要求34所述的方法,其中,所述传送包括无线地传输血氧饱和度和姿势信息。
37.根据权利要求36所述的方法,其中,所述传送包括从所述设备内的发送器无线地发送血氧饱和度和姿势信息。
38.根据权利要求30-37中任一项所述的方法,还包括从设置在所述受试者的躯干上的第二姿势传感器收集姿势信息。
39.一种针对睡眠障碍执行受试者风险评估的方法,包括:
获得关于所述受试者的睡眠调查问卷信息;
在睡眠期间从所述受试者获得血氧饱和度信息;
在睡眠期间从所述受试者获得头部姿势信息;
将所述睡眠调查问卷信息、所述血氧饱和度信息和所述头部姿势信息与历史数据的数据库进行比较;以及
从所述比较步骤确定受试者的睡眠呼吸暂停风险。
40.一种针对睡眠障碍执行受试者风险评估的方法,包括:
获得关于所述受试者的睡眠调查问卷信息;
在睡眠期间从所述受试者获得血氧饱和度信息;
在睡眠期间从所述受试者获得头部姿势信息;
将所述睡眠调查问卷信息、所述血氧饱和度信息和所述头部姿势信息与来自其它受试者人群的数据的数据库进行比较;以及
从所述比较步骤确定受试者的睡眠呼吸暂停风险。
