背景技术
肺癌是全球范围内最常见的恶性肿瘤之一,自20世纪50年代以后,肺癌的发病率和死亡率均呈明显上升的趋势,在我国目前肺癌已成为首位恶性肿瘤死亡原因。肺癌的发病与吸烟、大气污染、长期接触放射性物质、肺部慢性疾病等有关。目前主要的治疗手段包括手术、化疗、放疗、分子靶向治疗等。
化疗作为肺癌病人术后一个重要的辅助治疗手段,化疗药物常常引起恶心、呕吐、腹泻、味觉改变、食欲减退以及厌食甚至肝脏损伤影响营养物质的摄入。而肺癌常用的化疗药物顺铂属强致吐类药物,如果不加以控制,恶心和呕吐会造成液体或电解质的失衡、体重丢失以及衰弱,甚至恶液质,也可造成机体代谢紊乱及内稳态的失衡,加重病人的代谢负担,引起各种营养素的消化吸收障碍,导致病人营养不良。营养不良又会导致肿瘤病人对进一步的化疗等抗肿瘤治疗的耐受力下降。化疗造成营养不良的原因是多方面的,其直接原因是化疗引起的恶心、呕吐、腹泻、胃肠道黏膜损伤、食欲减退等症状,影响营养物质的摄人,同时与化疗药物干扰人体对营养物质的吸收利用有关。
肺癌是营养不良发生率最高的肿瘤之一,尤其在晚期肺癌病人中营养不良发生率可达30%以上。肺癌本身对食管产生压迫症状影响进食。肺癌引起的呼吸困难导致病人大脑缺氧,对化学感受器所传递的饥饿信号迟钝,对食物的味觉、嗅觉也会发生改变,进食的快感减少或消失,产生厌食。同时肺癌本身产生的一些细胞因子,也可以刺激和诱导宿主免疫细胞产生各种细胞因子,导致糖、脂肪、蛋白质代谢异常,引起营养不良。而机体的营养状态与免疫监视功能密切相关。加之化疗致使病人血细胞减少,免疫功能减退,更易并发感染。在病人化疗期间,营养不良不仅会影响病人自身的生理和心理状态,而且还会影响后期治疗效果以及病人的生活质量,甚至缩短生存期。早期发现并针对营养不良情况给予适当的治疗十分必要。改善化疗病人的营养状况,不仅有助于机体的恢复,提高免疫功能,而且还能提高对化疗的耐受性、改善治疗效果和生存质量,延长生存期。因此,如何给予营养支持,提高病人自身抵抗力,成为特殊医学用途食品配方设计工作的一个重要课题。
在美国、欧盟、日本等发达国家,特殊医学用途食品都有相应的规定和开发应用;而我国的特殊医学用途配方食品尚处于起步阶段,亟待开发研究。而此类食品市场潜力巨大,经济效益高,因此开发此类食品,具有重大的经济效益和社会意义。
特殊医学用途配方食品以肠内营养形式供给肺癌病人营养支持的首选途径,本发明的肺癌营养食品是依据18岁-64岁肺癌病人发病机制,针对性设计饮食配方,利用各元素的综合协同作用、发挥对肺癌病人化疗期间的营养补充和调节,避免病人的恶化或复发。其中该配方在保证热量前提下,高蛋白、高碳水化合物、低脂肪、高含量活性功能物质,能从氧化应激和免疫能力方面控制病人的发病机理,能调节病人情绪、心态。在保证效果的前提下,通过元素形式选择、搭配和销售方式的简化,降低产品价格。该食品既可以作为单一营养来源满足患者的营养需求,也可以与其他日常饮食搭配食用,丰富饮食类型和食用方式,激发患者饮食兴趣。

其中:

其中:挤压时的温度为60℃,螺杆转速为165r/min,模孔孔径为12mm;
酶解反应温度为38℃,酶解反应的pH值为7,酶解反应中豌豆蛋白浓度为8.1g/mL,中性蛋白酶的用量为豌豆蛋白用量的12%;
酶解反应后,豌豆肽的DPPH清除率、.OH清除率、O2 .-清除率分别为95%、88%、45%。

脂肪营养成分的原料灭菌后,平均分成5份,形成能量袋。
其中:灭菌功率为400W-500W、微波杀菌时间为100s-120s;
本发明所述的肺癌全营养配方食品制备方法食用方法如下:
目标人群为无严重黏膜炎、能耐受肠内营养的18岁-64岁之间肺癌化疗病人。
剂量包使用规格:0.4包/(kg·d),每日使用剂量≤25包;
推荐定量日常饮食用量为:碳水化合物用量≤200g/d。
按照以下两种方法之一食用:
(1)用100mL-350mL开水降温至40℃-60℃溶解功能袋和能量袋、搅拌、摇匀后,直接饮用或经管饲摄入;
(2)将功能袋添加到定量的日常饮食中食用;

本发明食用型式为肠内营养,肠内营养可以防止肠壁的萎缩,从而避免病人出现其他并发症。
本发明食品效果取决于其组成成分的各自作用和综合协同作用。
蛋白中豌豆蛋白或豌豆肽具有显著高效的清除各种自由基的功效,DPPH清除率、.OH清除率、O2 .-清除率分别为95%、88%、45%,还具有抗炎症、免疫调节、抑制肾素和ACE酶活性的作用,特别是具有防治癌症引起的与氧化应激有关的并发症作用,该豌豆蛋白或豌豆肽具有选择性清除引起肺癌的ROS物质。同时,豌豆商业品种尚无转基因品种,致敏性低,无饲喂不耐受性。

本发明根据肺癌发病机制和治疗机理,在配方中添加了特定的传冯,如褪黑素,并限定了维生素和矿物元素的型式、含量和比例,比如限定维生素A和维生素E的添加形式为非胡萝卜素和生育酚型式和含量,以及维生素B6、硒在饮食中比例。
谷氨酰胺对肺癌化疗病人有显著的辅助治疗作用,同时可减少化疗的副作用。通过强抗氧化性可以从根源上选择性清除自由基,保留对肺癌细胞有杀死作用的超氧离子自由基,此外,氧化应激状态调整了,也可以对改善肺癌病人血浆中ADA、GSH、PX、CAT、SOD、MDA的表达水平,从而调节病人的身体状态,有利于病人的康复、避免疾病的恶化或复发。
精氨酸增强的肠外营养改善晚期肺癌化疗病人的免疫功能,保护肠黏膜的免疫屏障,效果明显。
高蛋白、高碳水化合物、低脂肪配方适合肺癌食用者身体特点,有利于身体处康复。
本发明是基于病人的机体状态,在满足肺癌病人饮食要求前提下,突出配方食品改善病人氧化应激状态成分的比例,强化蛋白、碳水化合物、抗氧化成分、免疫作用增强组分,创造性调配维生素和矿物元素的比例搭配,适当添加改善别人抑郁情绪和心情成分。

将豌豆肽、乳清蛋白、豌豆膳食纤维、羟丙基淀粉、海带多糖、低聚木糖、赤藓糖醇、醋酸视黄酯、维生素D2、D-α-生育三烯酚、维生素K1、盐酸硫胺素、核黄素、盐酸吡哆醇、氰钴胺、烟酸、叶酸、D-泛酸钙、L-抗坏血酸、D-生物素、氯化钠、硫酸铜、氧化镁、富马酸亚铁、氧化锌、硫酸锰、磷酸二氢钙、磷酸二氢钾、碘酸钾、亚硒酸钠、氯化铬、谷氨酰胺、精氨酸、番茄红素、褪黑素、苹果香精,于500W的灭菌器中灭菌120s,灭菌后,平均分成5份,形成功能袋;
将大豆油、玉米籽油、紫苏油、鱼油于500W的灭菌器中灭菌120s,灭菌后,平均分成5份,形成能量袋;
豌豆肽的制备方法为:将含水量25wt.%的豌豆蛋白进行挤压,挤压后,将挤出物冷却、干燥、粉碎,再用中性蛋白酶进行酶解反应,酶解反应后,豌豆肽的DPPH清除率、.OH清除率、O2 .-清除率分别为95%、88%、45%。

制备每百克营养食品中的原料如下:

2.根据权利要求1所述的肺癌营养食品及其制备方法,其特征在于:
蛋白为豌豆蛋白或豌豆肽、乳清蛋白;
脂肪以富含ω-3不饱和脂肪酸为主的大豆油、菜籽油、花生油、玉米籽油、红花油、紫苏油等的一种或几种与海洋鱼油组成;
碳水化合物为低聚木糖、低聚半乳糖、赤藓糖醇、豌豆膳食纤维、豌豆抗性淀粉、海带多糖、麦芽糊精、羟丙基淀粉、多聚果糖或任何中华人民共和国国家食品药品监督管理总局批准的碳水化合物的或膳食纤维或抗性淀粉的几种组合。
3.根据权利要求1所述的肺癌营养食品及其制备方法,其特征在于:
维生素A添加形式为醋酸视黄酯、棕榈酸视黄酯中的一种或多种混合;
维生素D添加形式为维生素D2、维生素D3中的一种或多种混合;
维生素E的添加形式为D-α-生育三烯酚、D-α-醋酸生育三烯酚、D-α-琥珀酸生育三烯酚、dl-α-醋酸生育三烯酚、dl-α-生育三烯酚、维生素E琥珀酸钙中的一种或多种混合;
维生素K1添加形式为维生素K1;
维生素B1添加形式为盐酸硫胺素、硝酸硫胺素中的一种或多种混合;
维生素B2添加形式为核黄素、核黄素5’-磷酸钠中的一种或多种混合;
维生素B6添加形式为盐酸吡哆醇;
维生素B12添加形式为氰钴胺;
烟酸的添加形式为烟酸、烟酰胺中的一种或多种混合;
叶酸的添加形式为叶酸;
泛酸的添加形式为D-泛酸钙;

钠的添加形式为氯化钠;

铜的添加形式为硫酸铜、葡萄糖酸铜中的一种或多种混合;
镁的添加形式为碳酸镁、硫酸镁、氧化镁、氯化镁、L-苏糖酸镁中的一种或多种混合;
铁的添加形式为葡萄糖酸亚铁、富马酸亚铁、硫酸亚铁、乳酸亚铁、琥珀酸亚铁中的一种或多种混合;
锌的添加形式为硫酸锌、柠檬酸锌、柠檬酸锌(三水)、葡萄糖酸锌、氧化锌、乳酸锌中的一种或多种混合;
锰的添加形式为硫酸锰、葡萄糖酸锰中的一种或多种混合;

6.根据权利要求5所述的肺癌营养食品及其制备方法,其特征在于:酶解反应温度为38℃,酶解反应的pH值为7,酶解反应中豌豆蛋白浓度为8.1g/mL,中性蛋白酶的用量为豌豆蛋白用量的12%;酶解反应后,豌豆肽的DPPH清除率、-OH清除率、O2 -清除率分别为95%、88%、45%。
7.如权利要求1所述的肺癌营养食品的制备方法,其特征在于:以肺癌营养食品质量100g计,将蛋白、脂肪、碳水化合物、维生素A、维生素D、维生素E、维生素K1、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、叶酸、泛酸、维生素C、生物素、钠、钾、铜、镁、铁、锌、锰、钙、磷、碘、硒、铬、谷氨酰胺、精氨酸、番茄红素、褪黑素、苹果香精营养成分的原料灭菌后,平均分成5份,形成功能袋,最低单位剂量包的能量值等于295kJ,

8.如权利要求7所述的肺癌营养食品的制备方法,其特征在于:灭菌功率为400-500W,灭菌时间为100-120s。
9.一种权利要求1所述的肺癌营养食品的食用方法,其特征在于:

依据病人体重,剂量包使用规格:0.4包/(kg·d),每日使用剂量≤25包;
