实施例5:CAR-T细胞治疗CD30阳性淋巴瘤
筛选经临床确诊为CD30阳性的霍奇金淋巴瘤志愿患者共20例,所有志愿患者的年龄≥18岁,预期寿命>12周,经检测,肌酸酐<2.5mg/dl,ALT/AST<3×正常值,胆红素<2.0mg/dl,并具有足够的静脉通路进行血浆分离置换法,且对白细胞去除术无其他禁忌症。本实施例获得了所有参与临床试验的患者的知情同意书。
志愿患者在注射CAR-T细胞之前,进行5天的化疗治疗,化疗的目的是减少淋巴球以促进CAR-T细胞的移植成活率及稳态扩展,同时也减少了肿瘤负荷,化疗方案的选择将依据患者的基础疾病及之前的治疗方法,在化疗完成后的2天给予CAR-T细胞进行治疗。
使用CAR-T细胞治疗的方案具体为:采集50ml志愿患者的外周血于肝素抗凝管中,然后进行单个核细胞的分离,将患者T细胞感染包含实施例1的CAR3重组基因的慢病毒并且在体外进行大量扩增1周后,得到2.5-5×109个自体T细胞,然后将得到的CAR-T细胞回输给患者体内。
首次剂量按照1×106/kg体重的CART细胞,使用分割剂量执行,第0天(10%),第1天(30%),第2天(60%),每次10-15min完成输注,受试者的生命体征和血氧饱和度会在给药前,输注完成后及其后的1h内每15min监测一次,直到这些指标稳定且正常,输注前及完成输注后20min会各采集一份血样以确定CAR-T的基线水平。首次输注前及每次输注后2小时检测血钾和血尿酸值,输注后每周检测细胞因子干扰素γ,白介素2和白介素6以及QPCR定量检测CART的DNA含量。
本实施例中患者对首次剂量均耐受,然后第30天和第60天进行CAR-T细胞的第二次和第三次输注,剂量均为1×106/kg体重的CART细胞,输注后2小时抽血检测血钾及血尿酸值,输注后每周检测细胞因子干扰素γ,白介素2和白介素6以及QPCR定量检测CART的DNA含量。
本实施例制成的CART细胞足够,而且患者对CART细胞有反应,表现为首次输注一个月内肿瘤明显缩小,受试者继续进行更多次的输注,直至CART细胞使用完毕。在本实施例中,将总剂量分割成多个1×106/kg体重的CART细胞,采用间隔一个月的多次输注方式,使得CART细胞在体内持续存在,持续杀伤肿瘤细胞,而且主要的优势是输注后的细胞因子风暴表现非常轻微,患者可以在门诊完成输注,避免了反复住院的高额费用。
输注CAR-T细胞后的2-6月受试者每月返回医院一次,在这些研究随访中受试者接受:合并用药,体格检查,记录不良事件,抽血检验血液学、生化、CAR-T细胞的存活率以及PETCT检测肿瘤大小变化。
受试者输注后2年内进行季度评估,在这些研究随访过程中受试者接受:合并用药,体格检查,记录不良事件,抽血检验血液学、生化、CAR-T细胞的存活率。
结果显示:正常人血液内调节性T细胞为CD4阳性,杀伤性T细胞为CD8阳性,CD8/CD4的比例约为1:2,患者回输CART细胞后其CD8阳性T细胞比率显著升高,最高时CD8/CD4比例为11,如图7所示。
评价2年内霍奇金淋巴瘤志愿患者症状缓解的情况,结果见表2;
