Abstract
本发明属于医疗器械技术领域,具体涉及一种内镜逆行胰胆管造影术用全覆膜自膨式金属支架释放及回收器。所述装置包括内芯管、内套管、外套管、圈套器;内芯管尾端为双腔导管,内芯管的管腔Ⅰ通过圈套器金属杆,管腔Ⅱ与内芯管头端的单腔导管相通,内芯管单腔导管与双腔导管连接处有“凹”形槽,内芯管头端与一带孔的锥形帽相连;内套管套在内芯管外,外套管套在内套管外。本装置可在胆胰疾病的内镜下覆膜可回收金属支架治疗等领域广泛推广及应用。
Description
技术领域
本发明属于医疗器械技术领域,具体涉及一种内镜逆行胰胆管造影术用全覆膜自膨式金属支架释放及回收器。
背景技术
经内镜逆行胰胆管造影术(ERCP)置入全覆膜自膨式金属支架被广泛应用于炎性胆管狭窄、恶性胆管狭窄、医源性胆管狭窄、医源性胆管损伤、胆漏、胰管狭窄、内镜下乳头括约肌切开或球囊扩张后胆道出血等疾病的治疗中,与传统开腹手术相比具有创伤小、安全系数高、恢复快等优点。
目前全覆膜自膨式金属支架与支架推进器为一个整体,支架释放前需仔细定位;全覆膜自膨式金属支架的回收主要通过异物钳拖拉金属回收环,使金属支架与胆道分离并取出支架,上述金属支架推进器及金属支架取出方法存在以下不足:1、金属支架与推进器呈一个整体,其外包装盒较大,影响支架的运输;2、金属支架呈回收状态,金属支架两面的生物膜呈折叠状态,容易导致生物膜的破损及与金属丝的分离;3、金属支架释放一半后不能将支架回收到外鞘内;4、金属支架放置位置过高可压迫肝内胆管,位置过低则外露于十二指肠的支架段过长;5、位置放置不理想的金属支架只能取出,释放的金属支架重新放置到外鞘内较困难,造成支架的浪费及医疗费用的剧增;6、目前缺乏全覆膜金属支架回收专用工具;7、临床上往往通过活检钳牵拉金属回收环取出金属支架,其操作难度大、操作时间长、X线照射时间长;8、金属支架取出过程中,尾部收紧呈锥形的金属支架容易损伤胆管黏膜及十二指肠乳头,导致胆道出血及胰腺炎;9、金属支架经十二指肠镜钳道取出困难,且取出过程中容易损伤十二指肠镜CCD镜头、钳道内壁导致十二指肠镜漏水,而损伤十二指肠镜;10、金属支架经口取出过程容易损伤上消化道黏膜,二次进镜增加手术风险并延长手术时间。因此,需要对金属支架推进器进行改进。
发明内容
为解决现有技术中存在的缺陷,本发明提供一种全覆膜自膨式金属支架释放及回收器。
为实现上述发明目的,本发明采用如下技术方案:
全覆膜金属支架释放及回收器,包括内芯管、内套管、外套管、圈套器;内芯管尾端为双腔导管,内芯管的管腔Ⅰ通过圈套器金属杆,管腔Ⅱ与内芯管头端的单腔导管相通,内芯管单腔导管与双腔导管连接处有“凹”形槽,内芯管头端与一带孔的锥形帽相连;内套管套在内芯管外,外套管套在内套管外。
进一步地,内芯管尾端连有内芯管手柄,内套管尾端连有内套管手柄,外套管尾端连有外套管手柄,圈套器尾端连有套圈器手柄。
进一步地,所述外套管和内套管上分别有注水孔,注水孔近端的外套管和内套管内壁有硅胶内衬。
具体地,所述外套管直径4mm,内套管直径3mm,内芯管直径2.5mm,管腔Ⅰ腔道直径1mm,管腔Ⅱ腔道直径1mm,锥形帽最大直径4mm。
与现有技术相比,本发明具有以下优点:
1.本发明实现了金属支架与金属支架释放及回收器的分开存放与运输;
2.运输及存储中的金属支架呈弹开状态,使用时再与金属支架释放及回收器组装,避免金属支架表面生物膜的折叠性损伤及回收状态对金属支架的应力损伤;
3.金属支架释放全程均可实现支架的回收、重新定位及释放,避免了定位不准确导致的支架浪费及手术时间的延长;
4.一个金属支架释放及回收器可配置使用多枚金属支架,节约了医疗成分及费用;
5.金属支架释放及回收器可实现金属支架的快速回收及取出,节约了手术时间、X线暴露时间;
6.金属支架释放及回收器可实现金属支架的完整取出,不存在金属支架不能取出事件的发生;
7.使用金属支架释放及回收器回收金属支架过程中,不会损伤胆管、十二指肠乳头及上消化道黏膜,降低了手术风险;
8.取出的金属支架如外形完整,对非一次性使用的全覆膜自膨氏金属支架经清洁、消毒后可再次使用,节约了医疗费用。

具体实施方式
以下结合具体实施方式对本发明做进一步说明。
全覆膜金属支架释放及回收器,包括内芯管1、内套管2、外套管3、圈套器4;内芯管尾端为金属杆包裹的双腔导管,内芯管的管腔Ⅰ5通过圈套器金属杆14,管腔Ⅱ6与内芯管头端的单腔导管相通,内芯管单腔导管与双腔导管连接处有“凹”形槽7,内芯管头端与一带孔的锥形帽8相连,导丝可经管腔Ⅱ6和单腔导管通过锥形帽8的孔;内套管套在内芯管外,外套管套在内套管外;内芯管尾端连有内芯管手柄9,内套管尾端连有内套管手柄10,外套管尾端连有外套管手柄11,套圈器尾端连有圈套器手柄12;外套管和内套管上分别有注水孔13;外套管直径4mm,内套管直径3mm,内芯直径2.5mm,管腔Ⅰ腔道直径1mm,管腔Ⅱ腔道直径1mm,锥形帽最大直径4mm;外套管和内套管注水孔近端的内壁均有硅胶内衬18。
使用前先将全覆膜金属支架释放及回收器组装,组装方法如下:
(1)向金属支架释放及回收器注水孔内注入生理盐水;
(2)将金属支架15套在内芯管头端锥形帽上;
(3)固定内芯管手柄,向前推进圈套器手柄,释放圈套器;
(4)圈套器套住金属支架的回收线圈16;
(5)固定内芯管手柄,向后拉圈套器手柄,将金属支架回收线圈上的金属尾翼17固定到“凹”形槽内的圈套器头端;
(6)固定内芯管手柄,向前推进内套管手柄,金属支架的回收线圈进入内套管内,继续推进内套管,逐步收紧金属支架的回收线圈,并使金属支架的头端收紧,使其呈锥形,固定到“凹”形槽内;
(7)固定内套管手柄,向前推进外套管,则金属支架逐渐进入外套管内,继续推进外套管至金属支架完全进入外套管内,且内芯管头端锥形帽与外套管相接;完成金属支架、释放及回收器的组装。
释放全覆膜自膨式金属支架的操作方法:
(1)经管腔Ⅱ、内芯管头端锥形帽的导丝孔置入斑马导丝19;
(2)在导丝引导下置入金属支架释放及回收器,金属支架通过十二指肠乳头并越过狭窄,到达释放位置;
(3)固定外套管,向前推进内套管,金属支架逐步与外套管分离,最终完全释放金属支架;
(4)固定内套管,向前推进内芯管,金属支架的回收金属线圈逐步与内套管分离,直至完全释放回收金属线圈;
(5)固定内芯管,向前推进圈套器手柄,圈套器打开释放金属支架回收线圈上的金属尾翼;金属支架完全与本发明释放及回收器分离;
(6)撤出金属支架释放及回收器。
全覆膜自膨式金属支架回收操作:
(1)向金属支架释放及回收器注水孔内注入生理盐水;
(2)经十二指肠钳道置入导丝,导丝经十二指肠乳头置入到金属支架内;
(3)在导丝引导下经内芯管头端锥形帽置入金属支架释放及回收器,金属支架释放及回收器头端通过十二指肠乳头进入胆总管及金属支架内;
(4)固定内芯手柄,回拉外套管及内套管手柄,外套管及内套管经金属支架外露于十二指肠乳头外;
(5)固定内芯管手柄,向前推进圈套器手柄,释放圈套器;
(6) 圈套器套住金属支架的回收线圈16;
(7) 固定内芯管手柄,回拉圈套器手柄,将金属支架回收线圈上的金属尾翼17固定到“凹”形槽内的圈套器头端;
(8)固定内芯管手柄,向前推进内套管手柄,金属支架的回收线圈进入内套管内,继续推进内套管,逐步收紧金属支架的回收线圈,并使金属支架的头端收紧,使其呈锥形,固定到“凹”形槽内;
(9)固定内套管手柄,向前推进外套管,则金属支架逐渐进入外套管内,继续推进外套管至金属支架完全进入外套管内,且内芯管头端锥形帽与外套管相接;完成金属支架、释放及回收器的组装;
(10)经十二指肠钳道取出金属支架释放及回收器。
本发明可在胆胰疾病的内镜下覆膜可回收金属支架治疗等领域广泛推广及应用。
Claims (4)
1.全覆膜自膨式金属支架释放及回收器,其特征在于,包括内芯管、内套管、外套管、圈套器;内芯管尾端为双腔导管,内芯管的管腔Ⅰ通过圈套器金属杆,管腔Ⅱ与内芯管头端的单腔导管相通,内芯管单腔导管与双腔导管连接处有“凹”形槽,内芯管头端与一带孔的锥形帽相连;内套管套在内芯管外,外套管套在内套管外。
2.如权利要求1所述的全覆膜自膨式金属支架释放及回收器,其特征在于,所述内芯管尾端连有内芯管手柄,内套管尾端连有内套管手柄,外套管尾端连有外套管手柄,圈套器尾端连有套圈器手柄。
3.如权利要求1所述的全覆膜自膨式金属支架释放及回收器,其特征在于,所述外套管和内套管上分别有注水孔,注水孔近端的外套管和内套管内壁有硅胶内衬。
4.如权利要求1所述的全覆膜自膨式金属支架释放及回收器,其特征在于,所述外套管直径4mm,内套管直径3mm,内芯管直径2.5mm,管腔Ⅰ腔道直径1mm,管腔Ⅱ腔道直径1mm,锥形帽最大直径4mm。
