本发明公开了一种防移位易回收的胆道内支撑管及置入装置,所述防移位易回收的胆道内支撑管包括管腔依次连通的头端、第一管段、第二管段和尾端;头端上端敞开;第一管段包括多个以第一管段的轴线为轴均匀分布且向远离第一管段轴线方向凸起的弧形管壁,弧形管壁的两端分别与头端下端和第二管段上端连接;第二管段的侧壁具有与第二管段管腔连通的引流通孔;第二管段的下端管壁设有环形金属线,环形金属线的上端穿过第二管段的下端管壁,且下端具有环形金属线线结。本发明的环形金属线外露于十二指肠腔内,其抗肠液的腐蚀能力强,通过牵拉环形金属线很容易的将防移位易回收的胆道内支撑管从胆总管内取出。

技术领域
本发明涉及一种胆道内支撑管及置入装置,具体地说是一种防移位易回收的胆道内支撑管及置入装置。
背景技术
胆管狭窄是我国的常见病,包括炎性胆管狭窄、医源性胆管狭窄及肿瘤性胆管狭窄,狭窄上方胆汁排泄不畅,进而诱发急性胆管炎、梗阻性黄疸、肝损害等并发症,出现寒战、发热、腹痛、黄疸等表现,重者甚至导致感染性休克及肝功能衰竭,是危害人类健康的重要疾病。目前胆管狭窄的治疗方案包括开腹胆肠吻合术、腹腔镜胆肠吻合术、内镜逆行胰胆管造影(ERCP)途径的树脂或金属支架支架支撑术、经皮经肝胆管穿刺(PTC)途径的树脂或金属支架支架支撑术等。
对于高龄、基础疾病多、心肺功能障碍等高危患者,全麻手术风险较大。如果存在凝血功能障碍、腹水、肝内胆管无扩张,行PTC发生出血、胆漏及腹腔脓肿等并发症的风险较高,而持续性胆道外引流引起的胆汁丢失不易于机体的恢复,甚至发生代谢性酸中毒。目前多采取ERCP途径的胆道支架支撑术治疗胆管狭窄,其创伤小,狭窄解除率较好。目前所应用的胆道支架为两端设有防滑侧翼的树脂胆道引流管,上述引流管存在以下不足:1.胆道支架头端侧翼在置入到肝内胆管后,由于胆管侧壁的挤压,侧翼往往处于不同程度的闭合状态,继而导致引流管脱出概率较高;2、胆道支架尾端外露于十二指肠内,导致胆道密闭性的破坏,容易发生肠液反流,引起反流性胆管炎、细菌移位及支架堵塞;3、胆道支架放置于胆总管内,支架头端一旦顶住胆管壁,胆道支架难以取出;4、胆道内引流管的侧翼呈锥形,在取出过程中可损伤肝内胆管及十二指肠乳头黏膜,引起乳头炎、乳头出血,甚至因乳头水肿而发生急性胰腺炎。
发明内容
根据上述提出的技术问题,而提供一种防移位易回收的胆道内支撑管及置入装置。本发明采用的技术手段如下:
一种防移位易回收的胆道内支撑管,包括管腔依次连通的头端、第一管段、第二管段和尾端;
所述头端上端敞开,下端具有第一通孔;
所述第一管段包括多个以所述第一管段的轴线为轴均匀分布且向远离所述第一管段轴线方向凸起的弧形管壁,所述弧形管壁的两端分别与所述头端下端和所述第二管段上端连接;
所述第二管段管腔贯穿所述第二管段的上端和下端且直径大于所述第一通孔,所述第二管段的侧壁具有与所述第二管段管腔连通的引流通孔;
所述第二管段的下端管壁设有环形金属线,所述环形金属线的上端穿过所述第二管段的下端管壁,且下端具有环形金属线线结;
所述尾端管腔贯穿所述尾端的上端和下端且直径与所述第二管段管腔相一致。
所述头端呈类半椭球形(按垂直于椭球长轴划分),其上端敞开处设有与所述头端管腔直径相一致的头端开口;
所述头端的大端与所述弧形管壁连接;
所述头端管腔直径与所述第一通孔相一致。

所述内芯包括管腔依次连通的第一芯管、第二芯管和内芯手柄;
所述第一芯管经过所述第一通孔从所述头端上端穿出且外径小于所述第一通孔,所述第二芯管的外径小于所述第二管段管腔且大于所述第一通孔的直径,所述第二芯管与所述防移位易回收的胆道内支撑管相对应的管段上设有多个圆形突起;
所述引流管推进器头端管壁固定有线,所述线的另一端依次穿过位于所述第二管段最下方的所述引流通孔和所述尾端管腔从所述尾端和所述引流管推进器头端之间穿出且具有线结,所述线的位于所述防移位易回收的胆道内支撑管内的部分通过所述防移位易回收的胆道内支撑管和所述第二芯管夹持;
所述引流管推进器尾端设有推进器手柄;
所述环形金属线的下端从所述引流管推进器头端进入所述引流管推进器管腔,所述引流管推进器管腔与所述内芯之间具有容纳进入所述引流管推进器管腔内的所述环形金属线的空间。
所述引流管推进器头端呈类半椭球形(按垂直于椭球长轴划分)且与所述尾端相匹配,其大端与所述引流管推进器主体连接,所述引流管推进器主体的外径为7-10Fr,所述引流管推进器的长为2m;

防移位易回收的胆道内支撑管置入装置在导丝引导下经ERCP钳道及十二指肠乳头置入胆总管内,通过狭窄进入肝内胆管,至所述尾端完全进入胆总管,拽出内芯后多个弧形管壁自然释放,整体呈球形,拽出引流管推进器释放防移位易回收的胆道内支撑管,对狭窄胆管起支撑作用,并对狭窄上方胆管内胆汁起引流作用。
与现有技术相比,本发明具有以下优点:
1.本发明的第一管段包括多个弧形管壁,自然释放后,整体呈球形,通过对胆管壁产生弹性张力而发挥防移位作用;
2.本发明的弧形管壁外侧的中部区域内设有多条横向凸起,加强本发明的防移位作用;
3.防移位易回收的胆道内支撑管套在内芯上,第二芯管与防移位易回收的胆道内支撑管相对应的管段上设有多个圆形突起,同时第二芯管对头端起支撑作用,可保持弧形管壁在释放前呈闭合状态,易于防移位易回收的胆道内支撑管置入到肝内胆管;
4.防移位易回收的胆道内支撑管套在内芯上,第二芯管与防移位易回收的胆道内支撑管相对应的管段上设有多个圆形突起,对防移位易回收的胆道内支撑管产生固定作用,防移位易回收的胆道内支撑管位置放置不佳时,可以将防移位易回收的胆道内支撑管、引流管推进器及内芯一起经十二指肠钳道拽出;
5.防移位易回收的胆道内支撑管与引流管推进器由线连接,通过防移位易回收的胆道内支撑管和第二芯管夹持作用对线产生固定作用,内芯未完全释放前,可以通过回拉引流管推进器将防移位易回收的胆道内支撑管经十二指肠钳道拽出;
6.本发明的环形金属线外露于十二指肠腔内,其抗肠液的腐蚀能力强,通过牵拉环形金属线很容易的将防移位易回收的胆道内支撑管从胆总管内取出;
7.本发明的环形金属线下端具有环形金属线线结,使用异物钳、圈套器等均可以将其经十二指肠镜取出。
基于上述理由本发明可在医疗器具等领域广泛推广。

为使本发明实施例的目的、技术方案和优点更加清楚,下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。
实施例1
如图1所示,一种防移位易回收的胆道内支撑管,包括管腔依次连通的头端1、第一管段2、第二管段3和尾端4;
所述头端1上端敞开,下端具有第一通孔22;
所述第一管段2包括四个以所述第一管段2的轴线为轴均匀分布且向远离所述第一管段轴线2方向凸起的弧形管壁5,所述弧形管壁5的两端分别与所述头端1下端和所述第二管段3上端连接;
所述第二管段3管腔贯穿所述第二管段3的上端和下端且直径大于所述第一通孔22,所述第二管段3的侧壁具有与所述第二管段3管腔连通的引流通孔6;
所述第二管段3的下端管壁设有环形金属线7,所述环形金属线7的上端穿过所述第二管段3的下端管壁,且下端具有环形金属线线结8;
所述尾端4管腔贯穿所述尾端4的上端和下端且直径与所述第二管段3管腔相一致。
所述头端1呈类半椭球形,其上端敞开处设有与所述头端1管腔直径相一致的头端开口9;
所述头端1的大端与所述弧形管壁5连接;
所述头端1管腔直径与所述第一通孔22相一致。
所述引流通孔6呈椭圆形,且长轴与所述第二管段3轴线平行。
所述尾端4呈类半椭球形,其大端与所述第二管段3连接。
所述弧形管壁5外侧的中部区域内设有多条横向凸起10。
所述第二管段3外径为7-10Fr;
所述头端1管腔直径比所述第二管段3外径小3Fr,所述第一管段2、所述第二管段3和所述尾端4的管腔直径均比所述头端1管腔直径大2Fr;
所述防移位易回收的胆道内支撑管的长为5-12cm。
所述弧形管壁5的两端拉直状态下的间距为2cm,所述弧形管壁5的个数为四。
所述环形金属线7的上端至下端的距离为10cm,所述环形金属线的直径为1mm。
实施例2
如图2-图6所示,一种防移位易回收的胆道内支撑管置入装置,包括内芯11;
所述内芯11的外壁依次套接有如实施例1所述的防移位易回收的胆道内支撑管和引流管推进器12;
所述内芯11包括管腔依次连通的第一芯管13、第二芯管21和内芯手柄14;
所述第一芯管13经过所述第一通孔22从所述头端1上端穿出且外径小于所述第一通孔22,所述第二芯管21的外径小于所述第二管段3管腔且大于所述第一通孔22的直径,所述第二芯管21与所述防移位易回收的胆道内支撑管相对应的管段上设有多个圆形突起15;
所述引流管推进器12头端管壁固定有线16,所述线16的另一端依次穿过位于所述第二管段3最下方的所述引流通孔6和所述尾端4管腔从所述尾端4和所述引流管推进器12头端之间穿出且具有线结17,所述线16的位于所述防移位易回收的胆道内支撑管内的部分通过所述防移位易回收的胆道内支撑管和所述第二芯管21夹持;
所述引流管推进器12尾端设有推进器手柄18;
所述环形金属线7的下端从所述引流管推进器12头端进入所述引流管推进器12管腔,使得所述环形金属线7处于拉直状态,所述引流管推进器12管腔与所述内芯11之间具有容纳进入所述引流管推进器12管腔内的所述环形金属线7的空间。
所述引流管推进器12头端呈类半椭球形且与所述尾端4相匹配,其大端与所述引流管推进器12主体连接,所述引流管推进器12主体的外径为7-10Fr,所述引流管推进器12的长为2m;
所述第一芯管13外径比所述头端1管腔直径小1Fr;
所述第二芯管21外径比所述第二管段3管腔直径小1Fr;
所述内芯11管腔直径为1mm;
所述内芯11长为2.2m;
所述圆形突起15的直径为0.6Fr。
经口置入十二指肠镜,胆管插管成功后,将斑马导丝19通过狭窄胆管20进入到狭窄上方胆管内。将斑马导丝19尾端经内芯11头端置入,固定内芯手柄14及推进器手柄18,在斑马导丝19引导下将防移位易回收的胆道内支撑管置入到胆总管内,通过狭窄进入到肝内胆管内。所述尾端4完全进入到胆总管后,固定推进器手柄18,回拉内芯手柄14,内芯11与第一管段2分离,四个弧形管壁5自然释放,整体呈球形。固定推进器手柄18,继续回拉内芯手柄14,当内芯11头端完全进入到引流管推进器12管腔内后,此时防移位易回收的胆道内支撑管与引流管推进器12仅有线16连接。向后移动推进器手柄18,将引流管推进器12与防移位易回收的胆道内支撑管连接的线16经所述第二管段3最下方的所述引流通孔6拽出,此时引流管推进器12与防移位易回收的胆道内支撑管完全分离,所述环形金属线7外露于十二指肠腔内。在防移位易回收的胆道内支撑管释放过程中,回拉内芯手柄14,内芯11与第一管段2分离,四个弧形管壁5自然释放,整体呈球形,而内芯11仍位于防移位易回收的胆道内支撑管管腔内,如果防移位易回收的胆道内支撑管位置放置过深,可固定内芯手柄14及推进器手柄18,通过回拉推进器手柄18,将防移位易回收的胆道内支撑管拽出至最佳位置;如果防移位易回收的胆道内支撑管放置过浅,可固定内芯手柄14及推进器手柄18,通过向前推进推进器手柄18,将防移位易回收的胆道内支撑管推至最佳位置。内芯11已经完全进入引流管推进器12内,如果防移位易回收的胆道内支撑管位置放置过浅,向前推进推进器手柄18,将防移位易回收的胆道内支撑管推至理想位置;如果防移位易回收的胆道内支撑管位置放置过深,可固定推进器手柄18,向前推进内芯手柄14,使无圆形突起15的第一芯管13头端重新进入到防移位易回收的胆道内支撑管内,固定推进器手柄18及内芯手柄14,回拉推进器手柄18,通过线16将防移位易回收的胆道内支撑管拽出至理想位置。防移位易回收的胆道内支撑管位置调整最佳后,重新释放防移位易回收的胆道内支撑管。

Claims (10)
1.一种防移位易回收的胆道内支撑管,其特征在于,包括管腔依次连通的头端、第一管段、第二管段和尾端;
所述头端上端敞开,下端具有第一通孔;
所述第一管段包括多个以所述第一管段的轴线为轴均匀分布且向远离所述第一管段轴线方向凸起的弧形管壁,所述弧形管壁的两端分别与所述头端下端和所述第二管段上端连接;
所述第二管段管腔贯穿所述第二管段的上端和下端且直径大于所述第一通孔,所述第二管段的侧壁具有与所述第二管段管腔连通的引流通孔;
所述第二管段的下端管壁设有环形金属线,所述环形金属线的上端穿过所述第二管段的下端管壁,且下端具有环形金属线线结;
所述尾端管腔贯穿所述尾端的上端和下端且直径与所述第二管段管腔相一致。
2.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述头端呈类半椭球形,其上端敞开处设有与所述头端管腔直径相一致的头端开口;
所述头端的大端与所述弧形管壁连接;
所述头端管腔直径与所述第一通孔相一致。
3.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述引流通孔呈椭圆形,且长轴与所述第二管段轴线平行。
4.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述尾端呈类半椭球形,其大端与所述第二管段连接。
5.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述弧形管壁外侧的中部区域内设有多条横向凸起。
6.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述第二管段外径为7-10Fr;
所述头端管腔直径比所述第二管段外径小3Fr,所述第一管段、所述第二管段和所述尾端的管腔直径均比所述头端管腔直径大2Fr;
所述防移位易回收的胆道内支撑管的长为5-12cm。
7.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述弧形管壁的两端拉直状态下的间距为2cm,所述弧形管壁的个数为四。
8.根据权利要求1所述的防移位易回收的胆道内支撑管,其特征在于,所述环形金属线的上端至下端的距离为10cm,所述环形金属线的直径为1mm。
9.一种防移位易回收的胆道内支撑管置入装置,其特征在于,包括内芯;
所述内芯的外壁依次套接有如权利要求1~8任一权利要求所述的防移位易回收的胆道内支撑管和引流管推进器;
所述内芯包括管腔依次连通的第一芯管、第二芯管和内芯手柄;
所述第一芯管经过所述第一通孔从所述头端上端穿出且外径小于所述第一通孔,所述第二芯管的外径小于所述第二管段管腔且大于所述第一通孔的直径,所述第二芯管与所述防移位易回收的胆道内支撑管相对应的管段上设有多个圆形突起;
所述引流管推进器头端管壁固定有线,所述线的另一端依次穿过位于所述第二管段最下方的所述引流通孔和所述尾端管腔从所述尾端和所述引流管推进器头端之间穿出且具有线结,所述线的位于所述防移位易回收的胆道内支撑管内的部分通过所述防移位易回收的胆道内支撑管和所述第二芯管夹持;
所述引流管推进器尾端设有推进器手柄;
所述环形金属线的下端从所述引流管推进器头端进入所述引流管推进器管腔,所述引流管推进器管腔与所述内芯之间具有容纳进入所述引流管推进器管腔内的所述环形金属线的空间。
10.根据权利要求9所述的防移位易回收的胆道内支撑管置入装置,其特征在于,所述引流管推进器头端呈类半椭球形且与所述尾端相匹配,其大端与所述引流管推进器主体连接,所述引流管推进器主体的外径为7-10Fr,所述引流管推进器的长为2m;
所述第一芯管外径比所述头端管腔直径小1Fr;
所述第二芯管外径比所述第二管段管腔直径小1Fr;
所述内芯管腔直径为1mm;
所述内芯长为2.2m;
所述圆形突起的直径为0.6Fr。
