本发明涉及中药领域，具体地说，本发明涉及治疗颈肩腰腿痛的中药组合物。
二背景技术
颈肩腰腿痛是困扰很多人，严重影响工作和生活的常见疾病。引起颈肩腰腿痛的原因很多，除外伤、脊椎变形等病因病理清晰的因素较容易处置之外，肩周炎、风湿、类风湿等病理不明的疾病，特别是涉及免疫系统的疾病引起的颈肩腰腿痛中西医临床上均无确切有效的处置方法。其中，肩周炎是肩关节周围肌肉、肌腱、滑囊和关节囊等软组织的慢性无菌性炎症。炎症导致关节内外粘连，从而影响肩关节的活动。其病变特点是广泛，即疼痛广泛、功能受限广泛、压痛广泛。其病因并未完全研究清楚，一般认为源于受冷冻、外伤、感染或风湿所致。风湿一般认为是生活习惯或外部环境导致的自身免疫问题或风湿寒邪入侵所致。目前临床上西医对此类疾病一般采用氟比洛芬巴布、布洛芬、双氯芬酸等软膏剂或喷剂缓解疼痛，而中医常用治疗思路主要为补气通络、活血为主，如奇正消痛贴膏(主要成分为独一味和姜黄)和伤湿止痛膏(主要成分为草乌、川乌、乳香、没药)治疗肩周炎和风湿疼痛时都有明显优于西药的治疗效果，但这些中药制剂的主要缺点在于有效成分含量低，缓解疼痛时效短，更难以达到治本的效果。
三发明内容
基于申请人和其他中药研究人员对乳香和没药干燥树脂体组成和结构的研究，本申请在提取前对传统中药材乳香和没药进行了复合酶处理，使用木瓜蛋白酶、纤维素酶和果胶酶通过解离其中蛋白和多糖形成的包裹结构或片层结构有效的促进了有效成分β-乙酰乳香酸和β-榄香烯的释放，进而显著提高了含乳香和没药的膏贴剂的疗效。
一方面，本发明提供了一种治疗颈肩腰腿痛的中药组合物，包括以下重量份的原料药：丁香5-15、肉桂5-15、乳香10-15、没药10-15、草乌1-10、川乌1-10。
进一步地，所述的中药组合物由以下重量份的原料药制成：丁香5-15、肉桂5-15、乳香10-15、没药10-15、草乌1-10、川乌1-10、当归5-15、赤芍5-10、川芎5-10、白芷5-10、蛇床子5-10、威灵仙5-10、苏木5-15、小茴香5-15、冰片1-10、续断1-10、延胡索1-5、细辛1-5。
进一步地，所述的中药组合物由以下重量份的原料药制成：丁香10、肉桂10、乳香10、没药10、草乌5、川乌5、当归10、赤芍10、川芎10、白芷10、蛇床子10、威灵仙10、苏木10、小茴香10、冰片10、续断10、延胡索5、细辛5。
本发明的中药组合物可以为液体制剂、涂膜剂、膏贴剂等剂型，优选为膏贴剂型。
进一步地，本发明的中药组合物按照以下方法制成：
(1)称量所需重量的乳香和没药，粉碎，过200目筛，加入乳香和没药总重量10倍的纯净水，分别按照每克药物400U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6.5，40℃下反应2小时，加热至80℃灭活酶10min，再加入药粉总重量4倍的食用乙醇回流提取2小时，得乳香和没药提取物；
(2)称量所需重量的丁香、肉桂、草乌、川乌、当归、赤芍、川芎、白芷、蛇床子、威灵仙、苏木、小茴香、冰片、续断、延胡索、细辛，粉碎，过200目筛，加入药物重量5倍的纯净水，煮沸熬制2小时，得药液；
(3)合并步骤(1)和(2)中所得乳香和没药提取物以及药液；
(4)药液中加入适量增稠剂，制成药膏。
另一方面，本发明提供了所述中药组合物的制备方法，包括步骤：
(1)称量所需重量的乳香和没药，粉碎，过200目筛，加入乳香和没药总重量10倍的纯净水，分别按照每克药物400U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6.5，40℃下反应2小时，加热至80℃灭活酶10min，再加入药粉总重量4倍的食用乙醇回流提取2小时，得乳香和没药提取物；
(2)称量所需重量的丁香、肉桂、草乌、川乌、当归、赤芍、川芎、白芷、蛇床子、威灵仙、苏木、小茴香、冰片、续断、延胡索、细辛，粉碎，过200目筛，加入药物重量5倍的纯净水，煮沸熬制2小时，得药液；
(3)合并步骤(1)和(2)中所得乳香和没药提取物以及药液；
另一方面，本发明提供了一种提取乳香和没药中有效成分的方法，包括：
称量所需重量的乳香和没药，粉碎，过筛，加入适量纯净水，分别按照每克药物100U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6-7，40-60℃下反应1-2小时，加热至70-80℃灭活酶10-20min，再加入药粉总重量1-5倍的食用乙醇回流提取1-2小时。
四具体实施方式
实施例1本发明中药组合物和对照品的制备
主要试剂
所用中药材为正规药材供应商处市购，乳香和没药产自肯尼亚；
食品级木瓜蛋白酶：购自山东西唐生物科技有限公司，200000U/g；
食品级纤维素酶：购自湖南利尔康生物有限公司，10000U/g；
食品级果胶酶：购自湖南利尔康生物有限公司，40000U/g；
中药药膏用增稠剂：购自上海品纶工贸有限公司；
其余试剂均为常规国产型号。
A.按照以下方法制备本发明的中药组合物：
(1)称量所需重量的乳香和没药，粉碎，过200目筛，加入乳香和没药总重量10倍的纯净水，分别按照每克药物400U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6.5，40℃下反应2小时，加热至80℃灭活酶10min，再加入药粉总重量4倍的食用乙醇回流提取2小时，得乳香和没药提取物；
(2)称量所需重量的丁香、肉桂、草乌、川乌、当归、赤芍、川芎、白芷、蛇床子、威灵仙、苏木、小茴香、冰片、续断、延胡索、细辛，粉碎，过200目筛，加入药物重量5倍的纯净水，煮沸熬制2小时，得药液；
(3)合并步骤(1)和(2)中所得乳香和没药提取物以及药液得药液A；
(4)药液中加入适量增稠剂(加入重量为药液A重量的约50％)，制成药膏A。
B.基本按照A部分的方法制备，不使用酶处理乳香和没药，制得药液B和药膏B。
C.基本按照A部分的方法制备，仅使用每克药物100U和50U的纤维素酶和果胶酶处理乳香和没药(不使用木瓜蛋白酶)，制得药液C和药膏C。
实施例2本发明的酶处理方法增强乳香和没药的提取效果
检测对象为乳香和没药主要有效成分三萜酸和挥发油中的代表性成分β-乙酰乳香酸和β-榄香烯。
主要试剂
11-羰基-β-乙酰乳香酸标准品购自上海拓研生物科技有限公司；
β-榄香烯标准品购自南京贝伽生物科技有限公司；
其余试剂均为常规国产型号。
含量检测方法
β-乙酰乳香酸：waters HPLC系统、二极管阵列检测器、Empower工作站、C18柱、乙腈-水-磷酸(84∶16∶01)流动相、250nm检测波长；
β-榄香烯：waters HPLC系统、二极管阵列检测器、Empower工作站、C18柱、甲醇-乙腈-水流动相(38∶44∶18)；210nm检测波长；
具体检测步骤包括配制标准品溶液、绘制对照曲线、对照曲线验证、适当稀释后检测实施例1中A、B、C药液中β-乙酰乳香酸和β-榄香烯含量。

经称量并换算，实施例1的A部分中乳香和没药提取物中的β-乙酰乳香酸和β-榄香烯含量分别达到了9.21％和10.35％，明显高于现有技术中的水平(4-6％以及4％-8％左右)。配合表1的数据，可以证实本申请的方法在不显著增加成本和制备时间的情况下(使用廉价常用食品级酶且用量不大，处理时间仅2小时左右)，有效提高了乳香和没药的有效成分提取水平。

家兔肩周炎模型由浙江中医药大学动物实验中心提供：半岁龄，体重2kg(±0.2kg)的健康家兔60只，剪去右肩外侧毛后，将两后肢和左前肢固定于台上，以摇床200转/分钟平行摇动肩关节，每天持续4小时，连续3天；随后以水浸湿后装冰块的袖袋外敷兔右肩部，每天持续4小时，连续3天；一周后，按病理观察结果选择处于典型肩周炎僵硬期的家兔45只用于实验，随机均分为3组，另留5只对照。
使用药物
组1：伤湿止痛膏羚锐制药股份股份有限公司，按说明书每日三次；
组2：本发明中药组合物药膏A，每日三次；
组3：本发明中药组合物药膏B，每日三次；
对照：不做治疗，正常饲养；
用药共持续14天
指标检测：
5-HT是肩周炎疼痛的主要递质，PGE2除致痛作用外，也是炎症反应的重要介质。
治疗开始前、治疗结束后，取兔耳动脉血ELISA法检测血清5-HT和PGE2含量；
治疗结束后，麻醉，取右前肢肱二头肌组织块，置冰面上，生理盐水漂洗干净，吸干，质量，加入匀浆介质，用于冰水浴中剪碎组织块，冰浴中超声匀浆4℃2000r/min离心10min，取上清液ELISA法检测肌肉5-HT和PGE2含量。
治疗结束后，麻醉，取右前肢肱二头肌组织块，置冰面上，生理盐水漂洗干净，放入20％甲醛溶液固定7天后，石蜡包埋，切片显微镜观察肌肉形态。
实验结果：

数据表明，使用药物治疗后，各组的血清5-HT和PGE2水平均有明显下降，而对照组基本没有变化。其中使用药液A的第2组下降幅度明显大于第1和3组；使用药液A的第2组肌肉5-HT和PGE2也普遍低于第1和3组(第3组肌肉PGE2低除外)。上述结果进一步证实了本发明药膏镇痛兼治本的优点(止痛加消炎)，而且效果，特别是降低PGE2的效果优于现有中药制剂。
肌肉纤维观察也基本印证了上述结果：组2和组3的上皮细胞增生明显少于组1，更明显少于对照，肌纤维排列也较整齐。
实施例4：本发明药膏的治疗腿疼的实际应用数据
实验方案：
选取风湿性腿关节痛患者60人，随机分为4组。分别给予伤湿止痛膏(羚锐制药股份股份有限公司按说明书每日三次)、本发明药膏A(每日三次)、奇正消痛贴膏(奇正藏药股份有限公司按说明书每日两次)，持续治疗两周，不使用包括按摩和针灸在内的任何其他药物和治疗处置。
根据中医病症诊断疗效标准判断治愈、显效、好转和无效。

可见本发明的中药组合物实际临床效果也较为明显，优于常见外用药伤湿止痛膏和奇正消痛贴膏。

Claims (7)

1.一种治疗颈肩腰腿痛的中药组合物，包括以下重量份的原料药：丁香5-15、肉桂5-15、乳香10-15、没药10-15、草乌1-10、川乌1-10。
2.权利要求1所述的中药组合物，由以下重量份的原料药制成：丁香5-15、肉桂5-15、乳香10-15、没药10-15、草乌1-10、川乌1-10、当归5-15、赤芍5-10、川芎5-10、白芷5-10、蛇床子5-10、威灵仙5-10、苏木5-15、小茴香5-15、冰片1-10、续断1-10、延胡索1-5、细辛1-5。
3.权利要求2所述的中药组合物，由以下重量份的原料药制成：丁香10、肉桂10、乳香10、没药10、草乌5、川乌5、当归10、赤芍10、川芎10、白芷10、蛇床子10、威灵仙10、苏木10、小茴香10、冰片10、续断10、延胡索5、细辛5。
4.权利要求1-3任一项的中药组合物，其为膏贴剂型。
5.权利要求1-4任一项的中药组合物，其按照以下方法制成：
(1)称量所需重量的乳香和没药，粉碎，过200目筛，加入乳香和没药总重量10倍的纯净水，分别按照每克药物400U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6.5，40℃下反应2小时，加热至80℃灭活酶10min，再加入药粉总重量4倍的食用乙醇回流提取2小时，得乳香和没药提取物；
(2)称量所需重量的丁香、肉桂、草乌、川乌、当归、赤芍、川芎、白芷、蛇床子、威灵仙、苏木、小茴香、冰片、续断、延胡索、细辛，粉碎，过200目筛，加入药物重量5倍的纯净水，煮沸熬制2小时，得药液；
(3)合并步骤(1)和(2)中所得乳香和没药提取物以及药液；
(4)药液中加入适量增稠剂，制成药膏。
6.权利要求2或3的中药组合物的制备方法，包括步骤：
(1)称量所需重量的乳香和没药，粉碎，过200目筛，加入乳香和没药总重量10倍的纯净水，分别按照每克药物400U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6.5，40℃下反应2小时，加热至80℃灭活酶10min，再加入药粉总重量4倍的食用乙醇回流提取2小时，得乳香和没药提取物；
(2)称量所需重量的丁香、肉桂、草乌、川乌、当归、赤芍、川芎、白芷、蛇床子、威灵仙、苏木、小茴香、冰片、续断、延胡索、细辛，粉碎，过200目筛，加入药物重量5倍的纯净水，煮沸熬制2小时，得药液；
(3)合并步骤(1)和(2)中所得乳香和没药提取物以及药液。
7.一种提取乳香和没药中有效成分的方法，包括：
称量所需重量的乳香和没药，粉碎，过筛，加入适量纯净水，分别按照每克药物100U、100U、50U加入木瓜蛋白酶、纤维素酶和果胶酶，pH调至6-7，40-60℃下反应1-2小时，加热至70-80℃灭活酶10-20min，再加入药粉总重量1-5倍的食用乙醇回流提取1-2小时。
