背景技术
脊椎动物的免疫系统是非常复杂的,并且免疫系统的紊乱也是复杂的。脊椎动物免疫系统包括先天性免疫系统和适应性免疫系统。先天性免疫系统,也被称为非特异性免疫系统,包括以非特异性方式保护生物体的细胞。先天性免疫系统不同于特异性识别抗原并提供长期保护的适应性免疫系统。先天性免疫系统的特征在于抗原非依赖性应答,并且先天性免疫系统的暴露不导致免疫记忆。先天性免疫系统的细胞包括树突状细胞、肥大细胞、巨噬细胞、自然杀伤细胞、嗜中性粒细胞、嗜碱性粒细胞和嗜酸性粒细胞。

发明内容
本发明的一个方面提供了人源化免疫缺陷非肥胖糖尿病(NOD)小鼠,其中所述小鼠:(1)对于scid突变是纯合的;(2)具有IL-2受体γ链缺陷;(3)被移植CD34+人造血干细胞(HSC);(4)被接种人患者衍生异种移植物(patient-derived xenograft,PDX);其中所述HSC和PDX是非HLA匹配的(例如,仅部分匹配,或不匹配)。
在某些实施方式中,所述scid突变是Cg-Prkdcscid。
在某些实施方式中,IL-2受体γ链缺陷是遗传无效突变(genetic nullmutation),例如Il2rgtm1Wjl。在其他实施方式中,IL-2受体γ链缺陷是IL-2Rγ链中的截短突变(例如,封锁(latching)细胞外或细胞内的结构域)。
在某些实施方式中,小鼠是NOD.Cg-Prkdcscid Il2rgtm1Wjl/SzJ(即NOD scidγ(NSG))。
在某些实施方式中,小鼠是被进一步手术植入人胸腺和肝碎片的雌性NSG小鼠,例如hu-BLT NSGTM小鼠(BLT小鼠或BLT人源化小鼠)。

在某些实施方式中,CD34ig.+人HSC在小鼠的全身照射(例如以约1、2、3、4、5、10、20、100、200、300、400、500、600、700、800、900、1000、1100、1200或1300cGy的剂量,或在本文的任何两个记载剂量之间,例如100-300cGy或700-1300cGy等)后移植。
在某些实施方式中,人PDX在小鼠被移植CD34+人HSC后约2周被接种至小鼠。在某些实施方式中,人PDX在小鼠被移植CD34+人HSC后约12周被接种至小鼠。在某些实施方式中,人PDX在小鼠被移植CD34+人HSC后约1、2、3、4、5、6、7、8、9、10、11、12、13、14或15周(或由任何两个数值限定的范围)被接种至小鼠。
在某些实施方式中,人PDX来自原代患者样品。在某些实施方式中,人PDX来自已经被作为异种移植物传代至少一代的存档的(archived)肿瘤样品。在某些实施方式中,人PDX具有低传代数,例如已经被作为异种移植物或在培养物中传代不超过5、4、3、2或1代。在某些实施方式中,人PDX保留人类癌症的遗传和/或表型异质性,所述人PDX衍生自所述人类癌症。在某些实施方式中,人PDX来自未经治疗的患者。在某些实施方式中,人PDX来自治疗耐受患者。
在某些实施方式中,人PDX是杰克逊实验室维护和可从其获得的PDX LIVETM肿瘤的PDX中的任何一种或多种。杰克逊实验室提供使用更广泛的在更早的传代次数的患者衍生异种移植物(PDX)癌症模型,作为PDX LIVETM肿瘤移植NSG小鼠的集合。可容易获得的、现成的PDX肿瘤的这个集合可以被维持在NSG小鼠背景中,并且任何PDX也可以在本发明的非HLA匹配的人源化免疫缺陷小鼠(例如NSG、NSGS、BLT等)中。

在某些实施方式中,人PDX是来自卵巢癌、肺癌例如非小细胞肺癌(NSCLC)、膀胱癌、淋巴瘤(例如AML、CML、ALL、CLL、DLBCL(弥漫性大B细胞淋巴瘤))、乳腺癌例如三阴性乳腺癌(TNBC)、脑癌、胰腺癌、前列腺癌、结肠癌、结直肠癌、子宫内膜癌、胃/GIST癌、肝细胞癌、肾脏/肾癌、皮肤癌(例如黑素瘤)、软组织癌、肉瘤或癌细胞系的异种移植物。
在某些实施方式中,人PDX是来自表达PD-L1和/或PD-L2的肿瘤/癌症的异种移植物。
在某些实施方式中,约0.5-10×106细胞(例如约1-9×106细胞、约2-8×106细胞、约3-7×106细胞、约4-6×106细胞或约5×106细胞)的人PDX被接种。
在某些实施方式中,在PDX接种后约50天(或在HSC移植后约9周),在小鼠的外周血中的人CD45+细胞的百分比达到约20-30%。
在某些实施方式中,小鼠被施用抗癌化合物。例如,抗癌化合物可以是5-FU、阿瓦斯汀(Avastin)、顺铂、卡铂、keytruda、多西他赛或其组合。在某些实施方式中,抗癌化合物是化疗剂。在某些实施方式中,抗癌化合物是临床前药物。在某些实施方式中,抗癌化合物是免疫调节剂,例如PD-1或其配体/受体的调节剂,或CTLA-4或其配体/受体的调节剂。在某些实施方式中,抗癌化合物是抗PD-1和/或抗PD-L1试剂,例如抗PD-1抗体和/或抗PD-L1抗体。
抗PD-1抗体阻断PD-1与其配体PD-L1和PD-L2之间的相互作用,而抗PD-L1抗体阻断PD-L1与PD-1和B7-1(CD80)两者之间的相互作用,其涉及T细胞应答的下调。
多种PD-1和PD-L1抑制剂在广泛癌症的早期和晚期临床试验的临床开发中。任何一种或多种PD-1和PD-L1抑制剂可用作本发明的抗癌剂。
代表性的抗PD-L1试剂包括表1中的下列试剂,而代表性的抗PD-1试剂包括表2中的下列试剂,两者均改编自Dolan和Gupta,Cancer Control,21(3):231-7,2014(通过引用并入本文)。
例如,BMS-936559/MDX-1105是针对PD-L1的全人、高亲和力免疫球蛋白(Ig)G4单克隆抗体。MPDL3280A是靶向PD-L1的工程化人单克隆抗体。CT-011/pidilizumab是与PD-1结合的人源化IgG1单克隆抗体。BMS-936558/MDX-1106/nivolumab是针对PD-1的全人IgG4单克隆抗体。Pembrolizumab是具有针对PD-1的活性的高选择性、人源化IgG4-κ单克隆抗体。
在某些实施方式中,抗癌剂是CTLA-4拮抗剂,例如抗CTLA-4抗体(例如,伊匹单抗(ipilimumab)——FDA批准用于治疗黑素瘤的CTLA-4抑制剂;和Tremelimumab,原名为ticilimumab或CP-675,206,是由辉瑞生产的全人IgG2单克隆抗体,并且目前正在进行癌症治疗的人体临床试验)。

肿瘤还使用除了由CTLA-4和PD-1介导的这些以外的机制以逃避免疫应答。例如,多种髓性生长因子在许多肿瘤的微环境中被释放,以发信号通知具有独特免疫抑制能力的未成熟髓性细胞扩增,包括被称为肿瘤相关巨噬细胞(TAM)的髓性细胞亚群。TAM是实体瘤中的白细胞的丰富群体,其在许多情况下通过例如抑制TIL活性和增加肿瘤血管生成促进而非限制肿瘤进展。

在肿瘤中发现的其他髓性细胞包括骨髓衍生抑制性细胞(MDSC),其代表包含巨噬细胞、粒细胞和树突状细胞(由其抑制T细胞增殖和促进血管生成的能力定义)的前体的未成熟细胞的异质组。MDSC使用一系列免疫抑制机制以帮助肿瘤逃避免疫,其大部分作用是针对抑制T细胞。
在肿瘤免疫中重要的其他免疫细胞群包括树突状细胞(DC)和自然杀伤(NK)细胞。DC是“专职性抗原提呈细胞”并且能够处理独特的肿瘤特异性抗原以激活T和B细胞。因此,DC处于致力于开发肿瘤疫苗和扩大离体肿瘤特异性CTLS用于后续过继性免疫疗法的研究的中心。
NK细胞具有对于自身组织的免疫监视重要的独特细胞表面受体,并且其活性由在大多数正常细胞和肿瘤上发现的HLA I类抗原提呈分子的结合介导。保留HLA I类表达的肿瘤逃避NK细胞介导的细胞毒性,但失去表达的那些不再被NK细胞识别为“自体”并被杀死。促进NK细胞活化的化合物和使用异体NK细胞的过继性免疫疗法是临床前和临床研究的活跃领域。

具体实施方式
1.概述
传统癌症治疗方法利用对快速分裂细胞(例如肿瘤/癌细胞)具有毒性的广泛作用性化学试剂。这种化疗方法可以是成功的,但是可能由于各种各样的脱靶毒性而变得复杂,并且具有诱导耐药性的风险。哺乳动物免疫系统已经发展出许多有效的、高度特异性的用于消除靶细胞(包括被病原体感染的细胞和已经变成癌性的那些)的机制。作为回应,肿瘤细胞已经发展出其自身的一系列用于逃避免疫检测的机制。因此,对免疫效应细胞与肿瘤之间的相互作用获得更好的理解开辟了用于临床使用的刺激持久的、免疫介导的肿瘤消退的治疗策略的新的和有前途的大道。这类新的免疫-肿瘤治疗策略是非常令人鼓舞的,但该领域中的进一步研究可以获益于本发明的人源化的、基于小动物模型的(例如基于小鼠的)体内测试平台,其允许洞察人免疫和肿瘤细胞相互作用的更好的生物学理解,并且使当被转化成临床应用时具有较高成功可能性的新疗法的临床前测试成为可能。
本文所述的发明部分地基于人CD34+移植NSG小鼠中的患者衍生异种移植物(PDX)的生长速率不需要PDX和移植的人免疫细胞之间的完全HLA型匹配的令人惊讶的发现。
本文所述的发明还部分地基于癌细胞系移植的时机相对于人源化对异种移植物生长没有显著影响的令人惊讶的发现。另一方面,癌细胞系移植的时机对CD45+细胞群也没有显著影响。
因此,本发明的一个方面提供了人源化免疫缺陷非肥胖糖尿病(NOD)小鼠,其中所述小鼠:(1)对于scid突变是纯合的;(2)具有IL-2受体γ链缺陷;(3)被移植CD34+人造血干细胞(HSC);(4)被接种人患者衍生异种移植物(PDX);其中所述HSC和PDX是非HLA匹配的。
如本文所用,“非HLA匹配”是指不完全HLA匹配,包括仅部分HLA匹配,或非HLA匹配。在某些实施方式中,HSC和PDX之间仅部分HLA匹配。在某些实施方式中,HSC和PDX之间非HLA匹配。
在某些实施方式中,小鼠是NSG小鼠,或紧密相关的衍生物,例如NSGS小鼠、NSG-SGM3小鼠或人CD34+移植BLT小鼠。
本发明的人源化小鼠可用于广泛的生物学、医学和临床研究,包括,仅举几例,癌症生物学、免疫-肿瘤学、再生医学、人类造血、感染性疾病、移植、临床前药物功效测试研究以及免疫和自体免疫。
例如,本发明的人源化小鼠模型可用于研究癌症治疗中的免疫应答、感染性疾病的治疗、基因疗法和大分子药物的免疫原性等。

3.人源化肿瘤携带NSG小鼠
杰克逊实验室NSG小鼠(NOD.Cg-Prkdcscid Il2rgtm1Wjl/SzJ,库存号005557)也通常被称为NOD scidγ;NSG;NOD-scid IL2Rγnull;和NOD-scid IL2Rgnull。它们结合了NOD/ShiLtJ背景(杰克逊实验室库存号001976),严重联合免疫缺陷突变(scid)和IL2受体γ链缺陷的特征。因此,NSG小鼠缺乏成熟的T细胞、B细胞(并因此不产生小鼠抗体),以及功能性NK细胞,没有补体系统,并且具有细胞因子信号传导缺陷,导致比任何其他公布的小鼠品系更好的人造血干细胞(HSC)和外周血单核细胞(PBMC)的移植。NSG小鼠也具有缺陷性巨噬细胞和树突状细胞。最近的出版物已经证实该品系在胰岛移植、造血干细胞和癌症干细胞的研究中的突出效用。

NSG小鼠携带真实无效白介素-2受体γ链突变,与表达截短的白介素-2受体γ链的其他品系相反(参见Ohbo等,Blood,87:956-967,1996)。NSG小鼠根据材料转让协议(MTA)对于非营利性研究机构是可获得的,并且杰克逊实验室根据与NIH的协议分发NSG小鼠。
为了创建本发明的人源化NSG小鼠(HU-NSGTM),造血干细胞例如人CD34+HSC通过例如尾静脉注射、心内注射或肝内注射被引入至杰克逊实验室NSG小鼠中。HSC可以被引入至约2周龄、3周龄或4周龄的NSG小鼠中。通常,25号针头可用于尾静脉注射。也可以使用较小规格的针头,可能增加接种物中的细胞的剪切。
用于HSC递送的替代性位点包括眶后静脉窦、骨髓腔本身、和脾脏。注射到眶后窦中比使用尾静脉更容易执行,但是更具有侵入性,并且其需要受体小鼠被麻醉。干细胞归巢至骨髓依赖于引导干细胞从外周血至骨髓腔的分子,例如基质衍生因子1和干细胞。因此,干细胞递送到循环中(或原位地到骨髓中)增加了细胞将建立在新宿主的骨髓中的驻留的可能性。
任选地,就在引入HSC之前,将NSG小鼠暴露于全身照射(TBI)以清髓,其可以通过将NSG小鼠置于特别设计的照射器中,用设计成导致动物变得暂时性或慢性免疫抑制的一定剂量的全身γ照射而实现。

本文所述的本发明的非人类动物(例如小鼠)、组合物和方法目前代表某些说明性实施方式、示例性实施方式,并且另外并非旨在作为对本发明的范围的限制。本领域技术人员将想到其中的改变和其他用途。这样的改变和其他用途可以在不脱离如权利要求中阐述的本发明的范围的情况下作出。
实施例1 小鼠
NOD.Cg-PrkdcscidIl2rgtml Wjl/SzJ(NOD-scid IL2rγnull,NSG)小鼠是从在杰克逊实验室(Bar Harbor,ME)发育并维持的集群获得。所有动物在无特定病原体设施中、在微型隔离笼中居住,并被给予高压灭菌食物和在交替的星期用磺胺甲噁唑-甲氧苄氨嘧啶药水(Goldline Laboratories,Ft.Lauderdale,Fla.)和酸化的高压灭菌水供养。
实施例2 将人造血干细胞(HSC)移植到小鼠
如Pearson等(Curr.Protoc.Immunol.81:15.21.1-15.21.21,2008)所述用100cGy照射24至72小时龄(新生)NSG小鼠组。用含3×104CD34+造血干细胞(HSC)的CD3T细胞耗竭的人脐带血(UCB)以25-50μL体积通过心内注射注射照射的小鼠,如Brehm等(Clinic.Immunol.135(1):84-98,2010)所述。12周后,HSC受体的血液的流式细胞术分析量化人免疫系统的移植。对于实验性研究,仅使用具有>20%外周人CD45+细胞和>5%人CD3+T细胞的小鼠。
类似地,还可以用含3×104CD34+造血干细胞(HSC)的CD3T细胞耗竭的人脐带血(UCB)通过肝内注射或通过面部静脉注射注射亚致死照射的新生NSG小鼠,如Brehm等(Clinic.Immunol.,135(1):84-98,2010)所述。
此外,在使用标准技术通过侧尾静脉注射0.2-1×106CD34+HSC之前,约3周龄的NSG小鼠组可以以约200至1300cGy剂量进行全身亚致死照射。例如,在注射前称重每只动物,并且每次注射可以施用按体积计动物体重的至多约1%。在注射之前,温热动物5-10分钟(例如,在可商购加温盒中,或在顶置式加热灯下,或使用置于笼下的温水循环垫)以扩张静脉。然后将轻度麻醉的动物放置在其侧面上、在可再充电热袋或循环温水垫上以在麻醉期间保持它们温暖。然后插入小号(28-30)针头,斜向上,朝着动物头部方向进入静脉,尽量保持针头和注射器平行于尾部。针头应当顺利地前进到静脉中。注射之后,将动物关在笼子中并观察5-10分钟以确保出血没有重新开始。
对于BLT(骨髓/肝脏/胸腺)小鼠模型,约1mm3立方体的胎儿肝脏和胎儿胸腺被作为类器官植入至宿主NSG小鼠,其先前已经经受约200cGy的全身照射。然后使用标准技术通过侧尾静脉注射约0.2-1×106CD34+HSC。BLT小鼠模型允许稳健和一致的异种移植物(例如人)免疫系统发育,包含多种造血谱系;呈现持续的高水平T细胞发育;和T细胞在自体胸腺组织上被培育。这样的小鼠模型通常对病毒感染(例如EBV和HIV)具有可检测的T和B细胞应答。
在任何合适的人源化小鼠模型中,例如在人源化NSG小鼠中,患者衍生异种移植物(PDX)或癌细胞系在特定时间点(例如人CD34+细胞注射后2周或12周)被接种。异种移植物被允许生长,例如约7周,频繁监测体重和肿瘤体积(例如每周两次)。
在研究结束时可以从小鼠收集外周血,用于分析人CD45+供体细胞移植的程度。优选地,人CD45+供体细胞在外周血中必须达到至少约20-30%。
实施例3 预期的PDX生长速率的重演不需要HLA型匹配
基本上如上文实施例2所述进行NSG小鼠的人源化。简而言之,在使用标准技术通过侧尾静脉注射约0.2-1×106CD34+HSC之前,约3周龄的NSG小鼠组进行全身照射(约200cGy)。使用三种不同癌症样品(乳腺癌细胞BR0620、肺癌细胞LG1208和膀胱癌细胞BL0440)的PDX异种移植物被皮下植入到本发明的人源化NSG小鼠模型中,即具有来源于HLA错配的人HSC供体的已建立的且功能成熟的人免疫细胞的hu-CD34NSG小鼠,并在PDX移植后监测PDX移植的生长至多约55天。所有三种肿瘤显示稳健生长并且没有明显的排斥迹象(图1)。
结果显示在本发明的人源化NSG小鼠模型中重演预期的PDX生长速率不需要HLA型匹配。
实施例4 Hu-CD34NSGTM小鼠中PDX移植对肿瘤生长的时间评估
为了评估非HLA匹配的人源化hCD34+NSG小鼠中对PDX异种移植肿瘤生长的任何时间效应,根据基本上如上文实施例2或3所述的程序,在非HLA匹配的人CD34+细胞注射至NSG小鼠中2周或12周后,约5×106人SKOV3卵巢癌细胞被接种。异种移植物被允许生长约7周以上,每周两次监测体重和肿瘤体积。在研究结束时从小鼠收集外周血,用于分析人CD45+供体细胞移植的程度。每组7只小鼠的两个组(2周相对于12周)的平均结果显示在图2中。
在移植后2周时,人免疫力尚未显现。事实上,成熟的人T和B细胞需要至少12周以变得在hu-CD34NSG小鼠的PBL中可检测。然而,在肿瘤移植研究中,肿瘤成活(tumor take)在两组中均为100%(两者均为N=7),并且肿瘤体积随时间的增加在2周组中略微超过12周组(图2)。
结果显示两组小鼠之间没有显著差异,表明癌细胞系移植的时机相对于人源化对SKOV3卵巢癌细胞的生长没有显著的(如果有的话)影响。
在癌细胞接种50天后测试小鼠的人造血嵌合,并且在PBL中全部显示25-50%的huCD45+细胞,指示成功的移植(图3)。结果显示在非HLA匹配的人源化NSG PDX模型中癌细胞系移植的时机对CD45+细胞群没有显著的(如果有的话)影响。
实施例5 人源化对PDX生长动力学无显著影响
上述这些实验没有解决的一个重要问题是当与在正常、非人源化NSG小鼠中的生长速率相比时,人免疫细胞的存在是否影响肿瘤生长速率。
为了确定NSG小鼠模型中的人源化是否对非HLA匹配的PDX的生长动力学具有任何显著影响,使用与上述基本上相同的方法,三种新鲜的PDX肿瘤样品(乳腺癌BR0744(图4A)、肺癌LG0977(图4B)和软组织肉瘤SA0209(图4C))被独立地平行移植到NSG小鼠模型或huCD34-人源化NSG小鼠模型中,并相应地测量PDX生长曲线并绘图。
在NSG和人源化NSG模型两者中,所有三种肿瘤的成活率(take rate)为100%,并且肿瘤在每个移植宿主中发育。在NSG相对于hu-CD34NSG受体中,只有乳腺肿瘤以略微更快的速率生长(图4A),其他两种肿瘤在宿主两者中以相同的速率生长(图4B和4C)。但总的来说,在移植实验期间后的整个40-70天内,在NSG相对于hNSG模型中,PDX肿瘤的肿瘤生长曲线没有显著差异。
在所有hNSG实验组中,外周血中的hCD45+细胞为高于20%,表明人源化是成功的。还参见图5A-5C,显示在非人源化NSG小鼠中没有观察到hCD45+细胞。
在肿瘤生长研究结束时,还收集肿瘤并通过流式细胞术分析TIL的存在。所有三种肿瘤含有人CD4+和CD8+T细胞,但检测到很少CD19+B细胞(参见图6A-6C)。虽然不希望受任何特定理论的束缚,但TIL在hu-CD34NSG受体中减缓肿瘤生长的失败表明识别肿瘤的T细胞可能已经变得无变应性。
总的来说,这些结果证实hu-CD34NSG小鼠支持非HLA匹配的肿瘤生长,并且人免疫细胞的存在不显著影响肿瘤成活或生长速率。
实施例6 Hu-CD34NSGTM PDX小鼠是用于评价药物功效的功能平台
人源化NSG小鼠支持非HLA匹配的人肿瘤的生长的能力,如上文所证实的,是在该临床前测试平台的开发中的重要发现。
为了测试具有非HLA匹配的PDX的本发明的Hu-CD34 NSGTM小鼠是否可用于评估针对PDX的药物功效,使用临床相关标准护理(SOC)治疗,对阴性对照/载体组以及分别使用5-FU和阿瓦斯汀对抗结肠癌CN1572P5PDX的两个治疗组,获得在21天内的肿瘤生长曲线。具体地,5-FU以约20mg/kg体重的剂量静脉内施用,Q7d×2(每7天,总共2剂量)。阿瓦斯汀以约10mg/kg体重的剂量腹腔内施用,每周两次,总共5剂量。载体(D5W)被静脉内施用,Q7d×3。
图7A和7B中的结果显示,在本发明的人源化NSG模型中,5-FU和阿瓦斯汀两者均对非HLA匹配的结肠癌PDX有效。这证实本发明的非HLA匹配的Hu-CD34NSG PDX模型是用于评估临床相关标准护理(SOC)药物的功效的功能平台。
实施例7 Keytruda和顺铂抑制Hu-CD34 NSGTM中PD-L1+ MDA-MB-231乳腺癌肿瘤模型的生长
程序性细胞死亡蛋白1(也被称为PD-1和CD279)是人体中由PDCD1基因编码的蛋白质。PD-1是免疫抑制性细胞表面受体,其属于免疫球蛋白(Ig)超家族的CD28家族,并表达在T细胞、前B细胞、单核细胞、自然杀伤细胞和许多肿瘤浸润淋巴细胞(TIL)上。它是抑制T细胞应答的重要免疫"检查点"受体,并在调节T细胞功能中起到关键作用。
PD-1与两种配体PD-L1和PD-L2结合,其两者已经在肿瘤细胞上发现,并且其两者已经被肿瘤细胞用于衔接活化的T细胞上的PD-1受体以抑制活化的T细胞的功能,从而逃避针对肿瘤细胞的免疫应答。因此,PD-1及其配体通过阻止T细胞的活化而在下调免疫系统中发挥重要作用,其进而下调针对癌症的免疫应答,但也降低自身免疫力并促进自身耐受。PD-1的抑制作用被认为是通过促进淋巴结中的抗原特异性T细胞的凋亡(程序性细胞死亡),同时减少调节性T细胞(抑制性T细胞)的凋亡的双重机制而实现。
阻断PD-1的新一类药物,PD-1抑制剂,激活免疫系统以攻击肿瘤并因此可用于治疗癌症。例如,针对PD-1的单克隆抗体可以增强免疫系统,从而可用于治疗癌症。此外,许多肿瘤细胞表达免疫抑制性PD-1配体PD-L1。因此,抑制PD-1和PD-L1之间的相互作用可以增强体外T细胞应答并介导临床前抗肿瘤活性。
在共有296名患者的临床试验中,一种这样的抗PD-1抗体药物nivolumab(Opdivo-Bristol Myers Squibb)在非小细胞肺癌、黑素瘤和肾细胞癌中产生完全或部分的应答。Nivolumab也靶向PD-1受体,并于2014年7月在日本和2014年12月由美国FDA批准用于治疗转移性黑素瘤。
另一个这样的抗PD-1抗体药物pembrolizumab(Keytruda,MK-3475,Merck)靶向PD-1受体,并于2014年9月由FDA批准用于治疗转移性黑素瘤。在2015年3月已经通过英国早期获得药物计划(Early Access to Medicines Scheme,EAMS)使英国晚期黑素瘤患者可获得Pembrolizumab。它也正在美国被用于肺癌和间皮瘤的临床试验。
在早期开发中靶向PD-1受体的其他药物包括Pidilizumab(CT-011,Cure Tech)、BMS 936559(Bristol Myers Squibb)和MPDL328OA(Roche)。
另一方面,顺铂是现在是一类的含铂抗癌化疗药物中的第一个成员,该类别也包括卡铂和奥沙利铂。这些铂络合物在体内反应,结合DNA并引起DNA交联,这最终触发细胞凋亡(程序性细胞死亡)。
为了确定移植的PDX肿瘤是否也响应于临床相关免疫-肿瘤学检查点抑制剂,或者这样的检查点抑制剂是否可以在驻留的人免疫细胞中重新激活抗肿瘤应答,设计和进行一系列实验以解决这些问题。
首先,为了确定检查点抑制剂pembrolizumab(Keytruda)是否在本发明的非HLA匹配的PDX人源化NSG模型中有效,根据本发明的方法建立PDX huNSG模型。具体地,通过用基质胶皮下接种,人源化NSG小鼠(人CD45+细胞被发现在hNSG小鼠的外周血中超过25%,证实成功的huCD34移植)被移植每只小鼠5×106的非HLA匹配的PD-L1-阳性乳腺癌细胞系MDA-MB-231细胞。这个细胞系在肿瘤细胞表面表达非常高水平的PD-L1,其可以结合T细胞上的PD-1并诱导无变应性——约94.3%的MDA-MB-231细胞表达的PD-L1。然后用载体、顺铂或Prembrolizumab(Keytruda)处理肿瘤移植的人源化NSG小鼠。
对阴性对照/载体组以及分别使用顺铂和Pembrolizumab(Keytruda)对抗MDA-MB-231PDX的两个治疗组,获得在21天内的肿瘤生长曲线。具体地,顺铂以约2mg/kg体重的剂量静脉内施用,Q7d×2(每7天,总共2剂量)。Pembrolizumab(Keytruda)以约5-10mg/kg体重的剂量腹腔内施用,Q5d×4(每5天,总共4剂量)。载体(盐水)被腹腔内施用,Q5d×4。
顺铂是含铂的化学治疗剂,其在快速分裂的细胞中引起DNA交联和细胞凋亡。顺铂治疗仅少量降低人源化小鼠中MDA-MB-231肿瘤的生长速率。相反,Keytruda在开始治疗后约2周内显著延迟肿瘤生长(图8A和8B)。
在生长研究结束时,收集实验小鼠的外周血并分析人CD19+B细胞和人CD4+和CD8+T细胞。图9A-9D显示人T细胞,包括CD3+CD4+和CD3+CD8+T细胞两者,以及CD19+B细胞,存在于本发明的Hu-CD34 NSGTM非HLA匹配的MDA-MB-231 PDX小鼠的外周血中。
还收集肿瘤并分析人CD4+和CD8+浸润T细胞。图10A-10C显示人T细胞(CD3+CD4+和CD3+CD8+T细胞两者)存在于本发明的Hu-CD34 NSGTM非HLA匹配的MDA-MB-231 PDX小鼠的肿瘤组织中。
因此所有三个治疗组显示这些细胞与治疗无关的相似百分比。Keytruda处理的肿瘤中不存在额外的TIL表明较慢的肿瘤生长是由于驻留的TIL的重新激活而不是来自PBL或脾脏的TIL浸润的额外刺激。
数据再次证实本发明的非HLA匹配的Hu-CD34 NSG PDX模型是用于评估药物功效(包括可能依赖于移植的人免疫细胞的功能的免疫调节药物)的功能平台。
实施例8 Keytruda和顺铂抑制Hu-CD34 NSGTM中PD-L1+TNBC BR1126乳腺癌肿瘤模型的生长
在本实施例中获得与实施例7基本上相同的结果,其中非HLA匹配的PD-L1-阳性乳腺癌细胞系MDA-MB-231细胞被非HLA匹配的PD-L1-阳性乳腺癌细胞系BR1126细胞(三阴性乳腺癌(TNBC)细胞系)代替。三阴性乳腺癌是乳腺癌的侵袭性亚型,治疗选择有限。PD-L1表达已经在TNBC患者中被报道。当PD-L1表达在TIL中被评估时,其与较高等级和较大尺寸的肿瘤相关。肿瘤PD-L1表达也与TNBC中T调节细胞的浸润相关,这些发现表明PD-L1表达肿瘤和PD-1/PD-L1表达TIL在调节TNBC中的免疫应答中的作用。
具体地,通过皮下接种,人源化NSG小鼠被移植每只小鼠5×106的非HLA匹配的PD-L1-阳性乳腺癌细胞系BR1126细胞。这可以通过基质胶的存在而完成。约56.9%的BR1126细胞表达PD-L1。人CD45+细胞被发现在hNSG小鼠的外周血中超过25%。
对阴性对照/载体组,以及分别使用顺铂和Pembrolizumab(Keytruda)对抗BR1126PDX的两个治疗组,获得在21天内的肿瘤生长曲线。具体地,顺铂以约2mg/kg体重的剂量静脉内施用,Q7d×3(每7天,总共3剂量)。Pembrolizumab(Keytruda)以约5-10mg/kg体重的剂量腹腔内施用,Q5d×4(每5天,总共4剂量)。载体(盐水)被腹腔内施用,Q5d×4。
图11A和11B中的结果显示,针对本发明的人源化NSG模型中的非HLA匹配的PD-L1+乳腺癌PDX,与载体对照相比,顺铂和Keytruda两者显著降低肿瘤生长。
图12A-12D进一步显示,人T细胞,包括CD3+CD4+和CD3+CD8+T细胞两者,以及CD19+B细胞,存在于本发明的Hu-CD34 NSGTM非HLA匹配的BR1126 PDX小鼠的外周血中。
在研究结束时收集来自Keytruda处理的小鼠的肿瘤,并检查淋巴细胞浸润。图13A-13D显示人T细胞(CD3+CD4+和CD3+CD8+T细胞两者)以及CD19+B细胞存在于本发明的Hu-CD34 NSGTM非HLA匹配的BR1126 PDX小鼠的肿瘤组织中。因此,如在癌细胞系实验中那样,肿瘤也被人CD4+和CD8+T细胞以及人B细胞浸润,并且与载体处理的小鼠相比,用Keytruda处理不增加肿瘤浸润,再次表明较慢的肿瘤生长是由于驻留的免疫效应细胞的重新激活。
图13E-13H进一步显示,人T细胞(CD3+CD4+和CD3+CD8+T细胞两者)以及CD19+B细胞存在于本发明的Hu-CD34 NSGTM非HLA匹配的BR1126 PDX小鼠的脾脏中。
在类似的实验中,对单独化疗、抗PD-1剂Keytruda和抗CTLA4剂伊匹单抗治疗的PDX肿瘤样品进行免疫染色,针对CD45+CD8+浸润T淋巴细胞的存在。图16显示,与单独化疗相比,使用抗PD1剂Keytruda和抗CTLA4剂伊匹单抗的治疗导致浸润性CD45+CD8+T淋巴细胞的强烈存在。
这再次证实本发明的非HLA匹配的Hu-CD34 NSG PDX模型是用于评估药物功效(包括可能依赖于移植的人免疫细胞的功能的免疫调节药物)的功能平台。
实施例9 Keytruda+/-多西他赛抑制Hu-CD34NSGTM中PD-L1+LG1306肺癌肿瘤模型的生长
进行实验以确定hu-CD34 NSG小鼠中的肿瘤的组合治疗是否显示比任一单一试剂疗法更高的功效。
在本实施例中获得与实施例7和8基本相同上的结果,其中非HLA匹配的PD-L1-阳性乳腺癌细胞系MDA-MB-231细胞或BR1126细胞被非HLA匹配的PD-L1-阳性肺癌细胞系LG1306代替。
具体地,人源化NSG小鼠被移植每只小鼠5×106的非HLA匹配的PD-L1-阳性PDX肺癌细胞系LG1306。约89.1%的LG1306细胞表达PD-L1。人CD45+细胞被发现在hNSG小鼠的外周血中超过20%。
对阴性对照/载体组,以及使用Pembrolizumab(Keytruda)、有或没有抗有丝分裂化疗剂多西他赛对抗LG1306PDX的两个治疗组,获得在24天内的肿瘤生长曲线。具体地,多西他赛当存在时以约10mg/kg体重的剂量静脉内施用,Q7d×4(每7天,总共4剂量)。Pembrolizumab(Keytruda)以约5mg/kg体重的剂量腹腔内施用,Q5d×6(每5天,总共6剂量)。载体(盐水)被腹腔内施用,Q5d×6。
来自单独用Keytruda治疗的小鼠的肿瘤显示与来自载体处理的对照的肿瘤相比降低的生长,但是由于10只小鼠中的一只不应答Keytruda,它们的应答是高度可变的。当Keytruda与多西他赛组合时,肿瘤生长在治疗后10天内被显著抑制,小鼠-至-小鼠(肿瘤-至-肿瘤)变化性很低。然而,当一个不应答Keytruda的小鼠被剔除出计算时,在Keytruda组和组合组之间没有差别。两组均显示肿瘤生长的显著降低,并且在组合Keytruda和多西他赛时没有观察到加和效应。
因此,图14A和14B所示的结果显示,Pembrolizumab(Keytruda)、有或没有多西他赛,对本发明的人源化NSG模型中非HLA匹配的PD-L1+乳腺癌PDX是有效的。
总的来说,本文所述的实验,特别是实施例6-9中的实验,证实被移植到hu-CD34NSG小鼠中的人肿瘤能够应答标准护理化学疗法。然而,甚至更重要的发现是,被移植的肿瘤似乎逃避人免疫力,非常像它们在从其获得它们的患者中所做的那样。此外,用TIL检查点抑制剂治疗推测性地从无变应性释放T细胞并刺激其对肿瘤的细胞毒性。
数据证实hu-CD34 NSG小鼠是用于收集对人免疫细胞和肿瘤的相互作用的新洞察,并用于测试免疫-肿瘤学和组合疗法的有力平台。
总的来说,本文实施例中证实的结果证实本发明的人源化小鼠(例如NSG小鼠)中PDX肿瘤的移植和生长,移植的小鼠对标准护理(SOC)治疗的应答,以及在用检查点抑制剂治疗后免疫介导的肿瘤消退。这些结果支持本发明的人源化小鼠(例如NSG小鼠)作为用于免疫-肿瘤学疗法的新的临床前桥梁的用途。

19.权利要求1所述的小鼠,其中所述小鼠对于所述IL-2受体γ链缺陷是纯合的或半合的。
20.一种产生具有患者衍生异种移植物的人源化免疫缺陷非肥胖糖尿病小鼠的方法,所述方法包括:
(1)向免疫缺陷非肥胖糖尿病小鼠中引入CD34+人造血干细胞(HSC),其中所述小鼠:
(a)对于scid突变是纯合的;和
(b)具有IL-2受体γ链缺陷;
(2)用人患者衍生异种移植物(PDX)接种所述小鼠,
其中所述HSC和所述PDX是非HLA匹配的。
21.一种预测多种抗肿瘤试剂治疗肿瘤的功效等级顺序的方法,所述方法包括:
(1)将所述多种抗肿瘤试剂中的每一种作为单一试剂施用于权利要求1所述的小鼠,并测定功效,其中所述PDX代表所述肿瘤;
(2)对所述多种抗肿瘤试剂中的每一种的功效进行比较和/或分级,由此预测所述多种抗肿瘤试剂治疗所述肿瘤的功效等级顺序。
22.一种测试用于使用两种或更多种候选试剂治疗肿瘤的组合疗法的方法,所述方法包括:
(1)将所述两种或更多种候选试剂作为单一试剂或作为组合施用于权利要求1所述的小鼠,并测定功效,其中所述PDX代表所述肿瘤;
(2)对所述组合的功效和所述单一试剂的功效进行比较,
其中与所述单一试剂的加和功效相比,所述组合的更高功效表明所述组合是更好的。
23.一种测定用于使用试剂治疗肿瘤的给药方案的功效和/或安全性的方法,所述方法包括:
(1)将所述试剂施用于权利要求1所述的小鼠,其中所述PDX代表所述肿瘤,并且其中所述试剂根据所述给药方案施用;
(2)测定功效和/或安全性。
