Abstract
本发明涉及用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的试剂盒,所述试剂盒包含含有至少15重量%的烷醇胺和至少10重量%的络合剂的第一组分和含有至少一种酶的第二组分。该试剂盒使得能够使用非常少的用量来有效地清洗和消毒手术和医疗仪器。
Description
本发明涉及用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的试剂盒和方法。
在医院里,通常使用碱性清洗剂机械清洗并随后通过化学或热方式消毒手术仪器以及其它医疗设备。这类强碱性试剂可对敏感表面产生侵蚀性作用。通常将被血液污染的手术仪器在其使用后立即例如置于消毒溶液中,并且使其首先留在其中,直到清理放入洗涤机中进行清洗。消毒剂使血液凝结,且消毒活性物质使血液中所含的蛋白质成分变性。这类特别顽固的血液残留物通常只有通过碱性含活性氯的清洗剂才能去除。这些氧化性活性氯组分促使变性的蛋白质成分分解。消毒剂的其它成分例如碘也可形成难以去除的残留物。
碱性含活性氯的清洗剂具有以下缺点:它们包含要求标识的有害物质,在处理它们的过程中需要特别的预防措施以保护操作人员,并且它们是废水中不希望的环境污染物。
由US-A-4 243 546、EP-A-0 481 663和EP-A-0 730 024得知含酶的清洗剂,其特别可酶促降解血蛋白。
EP-A-1 021 519公开了用于清洗手术仪器的不含酶的清洗剂浓缩物。作为酶促清洗剂的缺点,在其中提到了酶活性可经较长时间的存放而降低。
EP-A-1 327 674公开了用于医疗仪器的清洗剂浓缩物,其包含烷醇胺、酶和作为络合剂的氨基多羧酸。
本发明的目的在于:提供有效清洗医疗和手术仪器和装置的可能方式,其在实践中便于操作并且在仅小体积的清洗剂的情况下就够用。
本发明通过用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的试剂盒来实现该目的,其特征在于,该试剂盒包含:
a. 第一组分,其包含至少15重量%的烷醇胺和至少10重量%的络合剂;
b. 第二组分,其包含至少一种酶。
首先解释一些本发明范围内所使用的术语。
用于医疗和/或手术元件和装置的清洗剂和/或消毒剂是在这类仪器的处理过程中至少降低污物-和细菌负载的这类试剂。
机械清洗在合适的洗涤机中,优选在医疗和/或手术仪器的常规清洗-和消毒设备(RDG)中进行。该术语在ISO 15883-1中定义。
本发明的试剂盒包含至少两种组分。第一组分(也称作碱性组分)包含相对高浓度的烷醇胺和络合剂。第二组分(也称作酶组分)包含至少一种酶,优选蛋白水解酶。这些蛋白水解酶的量优选介于0.05和1安森单位/克组分。
意料之外地显示,通过以要求保护的方式和方法将清洗剂的这些基本上已知的成分分到两种组分中,可使用非常少量的活性物质和尤其还使用非常小使用体积的这两种组分实现非常有效地清洗和/或消毒医疗仪器。
如下文实施例中进一步阐明那样,所述试剂盒的两种本发明组分以体积比1:1制备的的混合物可例如配制成0.2%的水溶液,并且对手术仪器的常见混合污染物具有良好效用。
因此根据本发明可在医院操作中将所述试剂盒的组分储存在容量为例如5至20 l的储存容器中。这类容器或罐可容易运输,并可在操作中贮存在所用清洗-和消毒设备的邻近处并与其相连。根据本发明并不要求将例如容量为200 l 的桶形式的大体积储存足够长的操作时间,这通常需要给复杂的供应管路系统,因为具有这种容量的桶通常不能与清洗-和消毒设备相邻贮存,而是必须设置在分开的贮存室中并因此通过长的管路与使用位点相连。
意料之外地显示,使用要求保护的最小浓度的烷醇胺作为所述碱性组分的成分导致在清洗水溶液中所述试剂盒组分的非常小的进料量和/或浓度的情况下特别良好的效用。根据本发明设定将清洗剂和/或消毒剂的成分分到两种组分中,这使得提供所谓的高度浓缩物,其中彼此不相容或难以相容的内容物(例如一方面的碱性烷醇胺和另一方面的酶)可以比具有仅一种组分的清洗剂而言以更高的浓度稳定贮存并且保持储备。
在本发明的一个优选实施方案中,第一组分的烷醇胺含量为15-30重量%,更优选15-25重量%。所述烷醇胺可优选具有以下结构:
其中R1为具有1至6个C原子的羟烷基并且其中R2和R3相互独立地为所述羟烷基或氢。
更优选地,所述烷醇胺选自单-、二-和/或三乙醇胺。
第一组分可包含优选10-30重量%,更优选15-25重量%的络合剂。所述络合剂用于使水软化并且可通过络合碱土金属离子而改进对手术(OP)典型污染物的清洗作用。所述络合剂可为基于丙烯酸或其碱金属盐的均聚物、共聚物或三元共聚物,还可为膦酸和/或其碱金属盐例如1-羟基乙烷-1,1-二膦酸、氨基三亚甲基膦酸、乙二胺基四亚甲基膦酸、膦酰基丁烷三甲酸;酒石酸、柠檬酸以及葡糖酸;以及次氮基三乙酸或乙二胺基四乙酸和/或它们的盐。优选使用螯合剂,尤其是氨基(多)羧酸及其盐,例如甲基甘氨酸二乙酸(MGDA)的三钠盐。
络合剂对阳极化铝表面等的腐蚀作用可通过添加磷酸和链长为C1至C22的脂族醇和/或链长为C2至C22的脂族二醇和/或脂族多元醇的至少一种单-和/或二酯得以减小或完全避免。特别优选磷酸一方面与丁醇和另一方面与乙二醇的二酯。该酯可以商品名Hordaphos® MDGB商购。根据本发明甚至在使用硬水的情况下获得良好的清洗效果,并且尽管如此对阳极化铝表面产生温和的作用。
第二组分可包含至少一种蛋白水解酶。合适的蛋白水解酶可例如以商品名Esperase® 8.0 L、Subtilisin®或Liquanase® Ultra 2.0 L从Novozymes公司获得。
第二组分可根据本发明包含表面活性剂,优选非离子表面活性剂。同样地可添加阴离子和阳离子表面活性剂。
非离子表面活性剂可根据本发明选自脂肪醇乙氧基化物、脂肪醇丙氧基化物、EO-PO嵌段共聚物、烷基葡糖苷、烷基聚葡糖苷、辛基酚乙氧基化物和壬基酚乙氧基化物。特别优选本领域技术人员常用的乙氧基化和/或丙氧基化脂肪醇。这类非离子表面活性剂例如可从Cognis或Julius Hoesch公司获得。
特别地可向第二组分添加常用的防腐剂,例如对羟基苯甲酸或其甲酯、5-溴-5-硝基-1,3-二噁烷、水杨酸、m-N-二甲基硫代苯氨基甲酸2-萘酯、5-氯-5-甲基-4-异噻唑啉-3-酮、2-甲基-4-异噻唑啉-3-酮、十六烷基三甲基氯化铵(CTAC)以及这些化合物的混合物。另一防腐剂为对羟基苯甲酸和/或其甲酯。借助这些防腐剂可避免组分的微生物和真菌侵害。
按需要可添加配制助剂(增溶剂),例如异丙苯磺酸钠、甲苯磺酸钠、二甲苯磺酸钠、脲,二醇,特别是聚丙二醇和聚乙二醇,甲基乙酰胺和脂肪醇例如鲸蜡醇。
可能的内容物的所述列举不是穷举。可额外地添加例如润湿剂、乳化剂、泡沫抑制剂等。有利的是例如添加N-酰基谷氨酸盐作为润湿剂。
本发明还涉及本发明试剂盒用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的用途。
这类机械清洗在自动程序运行期间无需人工辅助,尤其是在常规的仪器洗涤机(RDG)中进行。“清洗和/或消毒”的特征应表示本发明的试剂盒既可在唯一的方法步骤中将清洗和消毒组合着使用,还可在清洗步骤后进行单独的消毒步骤的程序运行下使用。在这种情况中,可在这两种步骤中使用本发明的试剂盒。
本发明同样涉及用于清洗医疗和/或手术仪器和/或装置的方法,其特征在于以下步骤:
a) 施加本发明试剂盒的组分的0.05%至0.5%水溶液,
b) 使所述溶液在室温直至该溶液沸点的温度下起作用,
c) 冲洗。
在本发明的方法中,优选通过喷雾施加清洗剂浓缩物的水溶液,但也可例如通过浸渍或灌注进行。百分率数据为重量百分率,除非在具体的个别情况中另有说明。
在施加清洗溶液后,起作用可包括静置的起作用,即不持续施加和/或喷雾或机械循环和/或移动该清洗溶液。优选在机械洗涤和/或消毒过程中,将清洗溶液在所使用的RDG中不断或间断地循环,在此优选将该溶液持续喷雾或冲到装置和/或仪器上。
可将所述试剂盒的组分直接计量加入用作清洗液(Flott)的水溶液中。
在步骤b)中的起作用优选在室温至55℃,优选在35至50℃,更优选在40至50℃下进行。
所述试剂盒的组分的水溶液优选具有9至11,更优选10至11的pH-值。只要在本申请的范围内测量清洗剂浓缩物的稀释溶液的pH-值,则使用去离子水(VE-水)作为溶剂。如果用常规自来水将所述浓缩物配制成即可使用的溶液,则根据水的性质pH-值可略有偏差。
将浓缩物/组分的pH值调节至期望的范围,这任选地优选通过添加酸和/或合适的缓冲体系来进行。优选添加至少一种选自具有2至6个碳原子的单-、二-或三羧酸的有机酸。在这些酸中,优选柠檬酸、酒石酸、苹果酸、乳酸、乙醇酸、乙醛酸、琥珀酸、己二酸和戊二酸。柠檬酸是特别优选的。优选向所述浓缩物添加少量的酸。所述有机酸中的某些例如柠檬酸在本发明的意义上同时为络合剂。
在步骤b)中的作用时间为优选1 min至30 min,更优选3至20 min,更优选5至15min。
在本发明方法的范围内可行的是,在清洗过程开始前,由合适的技术措施识别医疗仪器和装置的类型并且根据该类型选择合适的清洗和消毒方法。本发明方法的清洗-和消毒设备可根据本发明具有用于实施多种不同的清洗-和消毒方法的程序控制。该程序控制根据实施的清洗-和消毒方法被设计用于按需要计量加入这两种组分。该程序可优选具有不同浓度的这两种组分并且还设定所用组分相互的不同比率。例如可在处理传统手术仪器时选择比在处理用于微创手术的敏感仪器(MIC-仪器)或内窥镜时更高浓度的碱性组分。这两种组分相互的体积比根据本发明可优选为1:10至10:1,更优选1:5至5:1,更优选1:2至2:1。在标准清洗和/或-消毒的情况中,可为例如1:1。在本发明的方法中在清洗溶液中这两种组分的浓度之和为0.05至0.5重量%,优选0.08至0.3重量%。对于这两种组分中每一种的优选范围为0.02至0.3重量%,更优选0.04和0.15重量%。
不同的程序和在此期间分别使用的组分以及它们的浓度和浓度比可被确证为用于处理医疗产品的许可程序。这时所有处理过程的简单并完整的归档是可行的,包括所用组分的类型、用量和任选还有批号。
根据本发明可提供,提供用于识别要清洗和消毒的医疗仪器和装置的类型和用于根据该类型选择合适的清洗-和消毒方法和/或程序的设备。当分别清洗相同或类似仪器的批次,例如常规的手术仪器、MIC-仪器等时,这种操作方式可特别有意义。仪器类型可通过机械和光学方式识别,例如在仪器篮内扫描高度、光学识别仪器等。优选的是在仪器本身上或用于将其带入清洗-和消毒设备内的篮上存在有编码设备,其与清洗-和消毒设备的相应读取设备共同工作。这例如在此情况下是RFID-标签和RFID读取设备。
在本发明的方法中,所述仪器和装置的类型可选自:常规手术仪器、微创手术仪器、内窥镜及其部件、神经外科仪器、眼科手术仪器、麻醉用具、医疗仪器和装置的容器以及OP-鞋。
下文借助实施例阐明本发明。

实施例2
在该实施例中测试本发明的清洗性能。
大小为 1 × 9 cm 的粗糙DIN-金属薄片充当测试污染的基底。
将10ml肝素化羊血(Acila公司)和1ml的VE-水混合,并用150μl的硫酸鱼精蛋白(1000 I.E./ml用于灭活各1000 I.E.肝素)使其再次可凝结。将如此预备的血施涂在金属薄片上并且在室温下干燥过夜。
向1000 ml的烧杯中添加900 ml的清洗溶液。该清洗溶液根据本发明包含实施例1的各0.1体积%的两种组分。在第二烧杯中,清洗溶液包含EP 1 327 674 A1实施例1的1体积%浓缩物作为对比例(对比例)。
将清洗溶液加热至55℃并用磁力搅拌器在3.5级下搅拌。将金属薄片浸入所述溶液中5 min和随后用VE-水冲洗掉。随后将金属薄片干燥过夜,目测评估,随后用0.1%的酰胺黑溶液(Amidoschwarzlösung)染色以检测剩下的蛋白质残留量并且再次目测评估。
根据本发明实施例实现的清洗性能与对比例的清洗性能相同。该实施例因此证明,使用比用作对比例的现有技术而言显著更低浓度的清洗剂并因此也仅使用所用体积的一小部分的清洗组分就可以实现同样良好的清洗性能。这是出乎意料的并不能从现有技术中推导出。
实施例3
在RDG中清洗和/或消毒时,清洗溶液应尽可能有效地循环泵送。由于所用表面活性剂的不希望的起泡可阻碍该循环泵送,这导致泵压力下降且损害循环。
因为本发明试剂盒的组分以比现有技术清洗剂显著更低的浓度使用,可预期该不希望的起泡比现有技术更小程度地出现。
为了检测这一点,在RDG Miele PG 8536中在循环所述清洗溶液过程中在30℃的低温度范围内测量循环泵压力。在加热所述液体(Flott)所经历的该温度范围内,溶液对起泡特别敏感。
已显示,包含本发明试剂盒的各0.1体积%的两种组分的溶液展现出仅非常轻微的泵压力下降,而根据现有技术的溶液(EP 1 327 674 A1的1体积%的实施例1)表现出显著的泵压力下降。

1.用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的试剂盒,其特征在于,所述试剂盒包含:
a) 第一组分,其包含15-30重量%的烷醇胺和10-30重量%的络合剂,其中所述烷醇胺选自由二乙醇胺和三乙醇胺组成的组;
b) 第二组分,其包含至少一种酶。

9.根据权利要求1至8中任一项的试剂盒用于机械清洗和/或消毒医疗和/或手术仪器和/或装置的用途。
10.用于清洗医疗和/或手术仪器和/或装置的方法,其特征在于以下步骤:
a) 施加根据权利要求1至8中任一项的试剂盒的组分的0.05%至0.5%水溶液,
b) 使所述溶液在室温直至所述溶液沸点的温度下起作用,
c) 冲洗。
11.根据权利要求10的方法,其特征在于,在步骤b)中的起作用发生在室温至55°C。
12.根据权利要求10的方法,其特征在于,在步骤b)中的起作用发生在35至50°C。
13.根据权利要求10的方法,其特征在于,在步骤b)中的起作用发生在40至50°C。
14.根据权利要求10或11的方法,其特征在于,所述试剂盒的组分的水溶液具有9至11的pH值。
15.根据权利要求10或11的方法,其特征在于,所述试剂盒的组分的水溶液具有10至11的pH值。
16.根据权利要求10或11的方法,其特征在于,在步骤b)中的作用时间为1 min至30min。
17.根据权利要求10或11的方法,其特征在于,在步骤b)中的作用时间为3至20 min。
18.根据权利要求10或11的方法,其特征在于,在步骤b)中的作用时间为5至15 min。
19.根据权利要求10或11的方法,其特征在于,所述方法另外包括识别医疗仪器和装置的类型并根据所述类型选择合适的清洗和消毒方法的步骤。
20.根据权利要求19的方法,其特征在于,所述仪器和装置的类型选自常规的手术仪器、麻醉用具、医疗仪器和装置的容器以及OP鞋。
21.根据权利要求19的方法,其特征在于,所述仪器和装置的类型选自微创手术仪器、神经外科仪器和眼科手术仪器。
22.根据权利要求19的方法,其特征在于,所述仪器和装置的类型选自内窥镜及其部件。
