Abstract

背景技术
良性前列腺增生(BPH)是中老年男性的常见病,其中患病率随着年龄增加而增大。在年龄为70岁时,一半以上的男性具有有症状的BPH,而近90%的男性具有肥大前列腺的显微镜证据。证状的严重性也随着年龄增加而增大,60-70岁年龄段的患者有27%具有中等严重的症状,而70多岁的患者有37%具有中等严重的症状。
在生命的早期,前列腺具有核桃的尺寸和形状并且重约20克。前列腺扩大似乎是正常过程。随着年龄增长,前列腺的尺寸逐渐增加到它的正常尺寸的两倍或更多。在腺体达到一定尺寸之后,外部前列腺囊的纤维肌性组织限制扩张。由于对扩张的这种限制,所以囊内组织将压紧并限制前列腺尿道,因此引起对尿流的阻力。

参见图2A-2C,前列腺的结构可分类成三个区:外围区、过渡区和中心区。外围区PZ是形成腺体的下后方方面的区域,它在正常前列腺(图2A-2C)中含70%的前列腺腺元素。大多数前列腺癌(高达80%)在外围区PZ中产生。中心区CZ围绕射精管122并含有约20-25%的前列腺体积。中心区常常是炎性过程的部位。过渡区TZ是良性前列腺增生在其中发展的部位,并在正常前列腺(图2C)中含腺元素的体积的约5-10%,但在BPH情况下可构成高达80%的这种体积。过渡区TZ由两个侧向前列腺叶和用标号130表示的尿道周腺区域组成。如从图2A-2C能理解的,在过渡区TZ周围有天然屏障,亦即,前列腺尿道120、前纤维肌性基质FS、及在过渡区TZ和外围区PZ之间的纤维平面FP。在图2A-2C中,前纤维肌性基质或纤维肌性区可以看到,并且主要是纤维肌性组织。
BPH通常是当患者陈述麻烦的排尿困难而寻求药物治疗时诊断的。BPH的主要症状是尿频和尿急。BPH还能引起尿潴留在膀胱中,这又能导致下尿道感染(LUTI)。在许多情况下,LUTI能上升到肾中并引起慢性肾盂肾炎,最终可能导致肾功能不全。BPH也能导致与睡眠障碍有关的性功能异常或严重排尿困难所引起的心理焦虑。因此,BPH可能随着男性群体年龄的增长而显著地改变生活质量。
BPH是前列腺的腺细胞的连续生成和自然死亡(调落现象)之间不平衡的结果。这种细胞的过度产生导致前列腺尺寸增大,最显著的是在横过前列腺尿道的过渡区中。
在BPH的早期阶段情况下,治疗能减轻症状。例如,α-阻滞剂通过使在前列腺和膀胱颈中发现的平滑肌组织舒张来治疗BPH,这可以使尿更容易从膀胱中流出。这类药物能证明在腺元素引起前列腺中极度细胞生产之前是有效的。
然而,BPH的更晚期阶段只能通过外科手术治疗。已研究出许多方法采用电外科学或机械组织提取、及热切除或冷冻切除囊内前列腺组织。在许多情况下,这类介入仅提供暂时缓解,且经常有相当大的手术前后不舒服和发病率。
在现有技术的热切除方法中,将RF(射频)能量输送到前列腺组织上,如在图3A和3B中示意示出的,图3A示出现有技术中细长的RF针,该针穿透到前列腺叶内多个部位中。在该现有技术方法的第一方面,细长的RF针通常长度约为20mm,包括穿入前列腺叶中的绝缘体。因此产生的RF治疗切除远离前列腺尿道120的组织,但不以接近和平行于前列腺尿道120的组织为目标。在现有技术RF方法的另一方面,RF能量的施加通常延长1-3分钟或更长时间,这使切除作用的热扩散可以到达囊周边。这种现有技术的RF能量输送方法可能不产生耐久效果,因为平滑肌组织和α-肾上腺素受体在前列腺尿道周围未被均匀地切除。结果,叶中的组织能持续生长并对尿道产生影响,因此限制治疗的长期效率。
发明内容
在一些实施例中,提供了用于治疗患者的前列腺的良性前列腺增生的方法,该方法包括将蒸汽输送针穿过患者的尿道壁插到前列腺叶的多个部位中,将可冷凝的水蒸汽穿过该针输送到前列腺中每个部位处,以及切除与尿道壁平行的连续的叶区域。
在一些实施例中,连续的叶区域处于患者的膀胱颈和精阜之间。
在一些实施例中,插入步骤包括将15mm或更少的蒸汽输送针的尖头(末梢)穿过尿道壁插入到前列腺叶中。

具体实施方式
一般,本发明的用于治疗BPH的一种方法包括将热蒸汽沿间隙引入前列腺的内部中,其中蒸汽可控制地切除前列腺组织。该方法能在基于诊所的治疗中利用蒸汽来对每个叶施加50卡和200卡之间的能量。该方法能促成前列腺组织的局部切除,更具体地说,由蒸汽施加的能量能局部切除邻近尿道的组织,而不损坏不邻近尿道的前列腺组织。

在一个实施例中,本发明旨在在邻近前列腺尿道的区域中利用对流加热来治疗前列腺。
在一个实施例中,切除治疗法配置成以平滑肌组织、α-肾上腺素受体、以及平行于膀胱颈区域和精阜区域之间的前列腺深度小于2cm的交感神经结构为目标。
在一个实施例中,该系统包括输送水蒸汽的蒸汽输送机构。该系统可利用蒸汽源,该蒸汽源配置成提供温度为至少60℃、80℃、100℃、120℃或140℃的蒸汽。
在另一实施例中,该系统还包括计算机控制器,该计算机控制器配置成以范围从1秒到30秒的间隔输送蒸汽。
在另一实施例中,该系统还包括供与蒸汽一起输送的药理剂或其它化学药剂或化合物的源。药剂可以是麻醉剂,和抗菌素或毒素如该药剂也可以是密封剂、粘合剂、胶水、超级胶或诸如此类。
本发明的另一种方法提供对于BPH的治疗,该方法能使用经直肠途径,该经直肠途径将TRUS(超声波系统)用作成像机构以对前列腺成像,并将蒸汽输送工具导引到治疗部位。
在本发明的另一种方法中,工具或针工作端可以手动地或至少部分地通过弹簧机构前进。
在本发明的另一方面,该系统可以在切除治疗期间同时输送冷却流体到尿道,以便保护尿道的内部衬层。
图4、5和6示出本发明的系统的探针100的一个实施例,该探针适合于经尿道接近前列腺,并且该探针提供观察机构以便当将该探针导引到患者的前列腺内部部位时观察尿道。探针100还携带可伸展且可收缩的微导管件105(图5和6),该微导管件105具有远端尖头部分108(图4),该远端尖头部分能穿入前列腺叶中的精确瞄准的部位,以便切除目标组织体积。
柄和插管器部分
在图4中,可以看到,探针100具有用于插入尿道中的细长插管器部分110和供用人手握紧的柄部分111。插管器部分110的关键结构部件包括沿着纵向轴线113延伸的刚性插管器套管或延伸套管112,该套管112具有近端端部114a和远端端部114b。刚性延伸套管中的孔115沿着纵向轴线116延伸。在一个实施例中,参见图4和5,延伸套管112包括薄壁不锈钢管,该薄壁不锈钢管具有孔115,该孔115的尺寸设计成可容纳市售观察镜或内窥镜118。图5的示意性剖视图示出联接到延伸套管112的中间部分122上的结构或堵壁(bulkhead)120。该结构或堵壁120包括结构件,该结构件上联接有模制的柄,该模制的柄具有手枪式握把124,尤其是右侧和左侧配对柄部分125a和125b(图4)。堵壁可以塑料模制部分,该塑料模制部分可固定到套管112上或者旋转式联接到套管112上。
参见图5和6,其中示出模制柄的左侧部和右侧部,可以看到,套管112中的孔115具有近端开口端部130,内窥镜118可插入该开口端部130中。延伸套管112的近端端部114a联接到适配器机构132上,该适配器机构132可松开地接合内窥镜118并旋转式使内窥镜118与插管器部分110对准。内窥镜118具有近端观察端135和光连接器136,该光连接器136从观察端135向外延伸以将光源140联接到内窥镜上。图7示出,套管112中的孔115具有从约2mm至5mm的直径以用于容纳不同的内窥镜118,同时提供环形空间138以用于使冲洗液能穿过孔115并从插管器部分向外流动。
在系统100的一个实施例中,参见图5-8,可伸展且可收缩的微导管105包括具有尖锐尖头的薄壁柔性聚合物管,该聚合物管可沿轴向在插管器部分110的通道148中滑动。图4、7和9示出,插管器部分110包括用塑料或其他合适材料制成的细长插管器主体144,该插管器主体环绕延伸套管112。插管器主体144延伸到远端工作端部分145,该工作端部分145具有用于穿过尿道前进的钝的鼻状部或尖头146。细长插管器主体144还配置有容纳微导管件105的通道148,如下所述。参见图8和9,插管器主体144的远端端部部分145配置有通向中心开口区域162的开口160,该中心开口区域162远离内窥镜118的远端透镜164,该远端透镜164可用于在导引期间通过内窥镜的透镜164观察尿道。内窥镜118可具有30°、12.5°或其它角度的透镜以用于通过开口130观察。如在图8和9中能看到的,开口160之间具有桥接元件165,该桥接元件165起防止组织落到插管器主体144的中心开口区162的作用。在图8中可以看到,柔性导管轴105的工作端部分105设置在开口区域162附近并因此能通过内窥镜透镜164观察。
微导管和弹簧致动器
图10和11示出与探针100脱离配合的柔性微导管件或针105,以便表明它的休止形状。在一个实施例中,微导管105具有第一(近端)较大横断面部分170,该第一较大横断面部分170颈状收缩到第二(远端)横断面部分175,其中较小横断面部分175具有弯曲的休止形状,其中该曲线配置成无显著阻力地与微导管105的工作端108所遵循的路线的弯曲轴线177的轮廓相符,该路线是当该工作端从它的未伸展的位置移动到它的伸展的位置时的路线,如图1、8和9中所示。在一个实施例中,参见图10-12,微导管的第一横断面部分170包括薄壁外套管180,该薄壁外套管180从使得微导管件105的长度延长的内微导管185同心地向外。如在图12中能看到的,外套管180提供围绕内管状件185的绝热气隙188。在图12所示的一个实施例中,外套管180配置有间歇的突起190,所述突起保持外套管180的内表面192和内导管的外表面193之间的气隙188。图9示出,外套管180具有颈缩部分194,该颈缩部分194通过任何合适的手段如超声波接合、胶粘剂等接合到内微导管185上。返回图10,外套管180和内管状件都可包括适合于输送高温蒸汽的耐高温聚合物如如下所述。在一个实施例中,微导管185的外径为0.050”,其中内腔195约为0.030”。参见图8-9,用于向组织输送蒸汽介质的工作端部分108的一个实施例具有薄壁198,该薄壁中具有多个排出口200,这些排出口配置用于将蒸汽介质排放到组织中,如下所述。排出口的数量可为约2-100个,在一个实施例中包括十二个排出口,每个排出口都具有0.008”的直径,它们分成六排,每排两个排出口,这六排排出口围绕工作端108交错排列,如图10中所示。在图10-11所示的一个实施例中,微导管185的最远端尖头202具有变尖的锥形构型,该锥形构型可由微导管185的塑料材料形成。如下面将说明的,现已发现,聚合物针和针头202对热特性有用,因为在蒸汽输送期间它的热容对蒸汽质量不产生影响。
图10-11还示出,微导管185的远端尖头部分108具有至少一个标记204,该标记204与适合于通过内窥镜118的透镜164观察的微导管185的颜色形成对比。在一个实施例中,远端尖头部分具有一组第一颜色的环形标记204,该第一颜色与微导管185的第二颜色形成对比,其中当微导管185处于未伸展位置时,这些标记通过内窥镜透镜164不可见。在微导管185伸入组织中后,标记通过透镜164可见,这表示微导管185已伸入组织中。
现在返回图5和6,柄部分111的剖视图示出处于未伸展位置的微导管件105和相关组件。图5示出,扳机致动器210的凸缘208a和208b设置在致动器环212的两侧,该致动器环212联接到可滑动的微导管件105的近端端部214上。如从图5中能理解的,向下延伸的扳机致动器210适合于将凸缘208a、208b和微导管105扳到待击发位置,该待击发位置相当于微导管105的未伸展位置。在图5中,致动器210示出为处于第一位置B(虚线图)和第二位置B’,该第二位置B’在用食指致动之后,因此将微导管件105从它的伸展位置B扳到第二可松开的未伸展位置(或待击发位置)B’。凸缘208a和致动器210还用虚线图示出处于用标号208a’表示的松开位置。在图5中,凸缘208a、208b和相关组件配置用于进行以A表示的轴向行程范围,该范围从约8mm到15mm,它对应于微导管105的行程并且更一般地对应于组织伸入深度。在图5的实施例中,凸缘208a、208b和微导管件105可以弹簧致动,以便通过围绕套管112设置的螺旋弹簧215从未伸展位置移动到伸展位置。如在图5中能看到的,弹簧215设置在可滑动的凸缘208b和扳机座218之间,该扳机座218包括适合于将微导管105从它的待击发位置松开的松开扳机220的上面部分。
图5还示出松开扳机220,该松开扳机可松开地将凸缘208a和微导管105保持处于它的待击发位置,在该待击发位置中,扳机220的齿部分222接合凸缘208a的下边缘。从图5中能理解,松开扳机220配置成当扳机220通过医生的手指致动而被沿近端方向按下时围绕活铰接部分224挠曲或枢转。在致动扳机和松开微导管105以使之朝远端移动之后,该组件的轴向行程配置成随着凸缘208a接触至少一个如图6中所示的缓冲元件230轻柔地而非突然地终止。缓冲元件230可包括任何弹簧或弹性元件,在图6中示出为装在螺旋弹簧中的弹性体元件,该弹体元件用来缓冲和阻尼该弹簧致动式微导管组件的行程结束。缓冲元件230联接到凸缘235上,该凸缘235则配置成固定在左侧和右侧柄部分125a和125b之间(见图4)。
现在转到该系统的能量输送方面,设有蒸汽源250以用于通过微导管件105输送蒸汽介质从而切除组织。蒸汽源可以是蒸汽发生器,该蒸汽发生器能输送蒸汽介质如水蒸汽,并具有精确控制的品质以便提供精确的例如按每秒卡数度量的热能输送量。合适的蒸汽发生器的说明可从下列美国专利申请中找到:Nos.11/329,381、60/929,632、61/066,396、61/068,049、61/068,130、61/123,384、61/123,412、61/126,651、61/126,612、61/126,636、61/126,620,它们全都整体包括在本文中以作为参考。蒸汽发生系统还可以包括类似于专利申请61/123,416、61/123,417、61/126,647中说明的感应加热系统。该系统还包括控制器255,该控制器255能设定成控制蒸汽输送的不同参数,例如,控制器能设定成针对选定的治疗间隔、选定的压力或选定的蒸汽品质输送蒸汽介质。
参见图5,在一个实施例中,蒸汽源250远离柄124,蒸汽介质通过柔性导管262运送到该柄,该柔性导管262联接该柄和其中的单向阀264。在一个实施例中,蒸汽能在导管262中再循环直至蒸汽源中的螺线管被致动而产生蒸汽流以便由此提供增加的流体压力,该增加的流体压力打开单向阀265并允许蒸汽介质通过柔性管268流到能通过扳机275用手指致动的阀270。在图5所示的一个实施例中,扳机275被弹簧277推向未按压的位置,该未按压的位置相当于阀270的关闭位置。扳机275还能通过电线(未示出)联接到控制器255上。因此,致动扳机275能导致控制器致动蒸汽发生器中的电磁阀以使蒸汽流过减压阀。作为安全机构,柄中的阀270只通过它的致动打开,以便因此允许蒸汽介质流过柔性管278,该柔性管278与环212的流入口部分280连通,该流入口部分280又与微导管105中的内腔195连通。因此,图5示出流动路线和致动机构,它根据需要提供从蒸汽源到微导管105的工作端108中的蒸汽出口200的蒸汽流。
如图5中能看到的,该柄还能提供互锁机构,如果微导管松开扳机处于待击发位置,则该互锁机构防止致动蒸汽流,其中联接到松开扳机220上的边缘部分292能接合扳机275中的缺口294以防止按下所述扳机275。
仍参见图5,该系统的一个实施例包括流体冲洗源300,该流体冲洗源300操作性地联接到延伸件112的孔115上,以便将流体从孔115向外输送到探针工作端145的开口区域162(见图8)。如在图7中能看到的,孔115的尺寸设计成为围绕内窥镜118的流体冲洗流提供空间138。在图5中可以看到,流体源300—它可以是生理盐水或其它流体的滴注袋或受控制的压力源—可拆卸地联接到柄中的管系302上,该管系延伸到阀305,该阀305能在柄的一侧从致动器308用拇指操作。拇指致动器308还能通逐渐向前移动致动器308,例如将阀打开一更宽的开口,来控制冲洗液的流动速率。流体从阀305经由管道312流到延伸套管112中的口或开口315以便因此进入套管的孔115。
图5还示出操作式联接到柄124中管系322上的吸入源320,该吸入源320也能通过阀305致动,其中拇指致动器308能向后扳动,以允许抽吸力通过阀305施加到管系312上,该管系312延伸到延伸件中的口315上,这是冲洗流的相同路线。因此,在治疗期间抽吸力或吸入力能将流体从装置的工作端抽出。
参见图4、5、6和8,本发明的探针100的一个实施例的另一方面是当探针100相对于柄部分111的手枪式握把124的取向离开工作端145时微导管或针105的取向。在下面进一步说明的方法使用中,插管器通常是用手枪式握把以“握把向下”的取向GD穿过尿道插入(图13A),而手枪式握把126向下取向对医生来说是舒适的。治疗通常包括将探针旋转式再取向,如图13A中所示,以便微导管或针105能相对于握把向下的位置以90°至约135°穿入前列腺叶中。图13A和13B示意性示出探针100在前列腺中的正视图,其中所展开的微导管105示出柄的手枪式握把124的、展开的微导管105和连接器内窥镜136的取向,该取向表示内窥镜118的旋转取向并因此表示摄像机图像在监视器上的取向。如在图4-6中能看到的,插管器110、微导管105和内窥镜118的组件在凸缘235A和235B内的柄内可旋转。在一个实施例中,该系统具有在不同角度下的锁定光圈,如相对于图13A的握把向下取向GD在75°和135°之间每隔15°一个锁定光圈。因此图13A-13B和14A-14B示出外科医生可以使用的任选方法。
图13A和13B示出医生朝一个旋转方向锁定探针100的所有部件,并简单地用将他的手和手枪式握把124从握把向下位置GD旋转到大于90°的选定取向,然后松开微导管105以便穿透到前列腺叶中。在致动蒸汽输送扳机之后,蒸汽切除以标号400表示的区域。可以理解,内窥镜118旋转而使监视器上的图像也旋转。此后,医生如图13B中所示旋转探针以治疗另一前列腺叶。对于精通解剖学界标、选择简单性并习惯于在相对于患者解剖学的实际垂直轴线旋转的监视器上观察图像的医生来说,该方法可能是优选的。
图14A和14B示出医生利用探针的旋转特点并保持柄的手枪式握把124处于握把向下的定向GD以及将插管器110和微导管105旋转到合适角度以便治疗前列腺的第一和第二叶。该方法也适合于精通解剖学界标并习惯于在手术室(OR)中观察监视器上的旋转图像的医生。
图15A和15B示出医生利用探针的另一实施例来治疗两个前列腺叶。在图5-6的实施例中可以看到,内窥镜118用插管器110和微导管105—但不用柄的手枪式握把—锁定在旋转取向。应该能很容易理解,探针能制成允许在插管器110和微导管105之间相对于柄的手枪式握把124进行旋转调节—但提供旋转式将内窥镜118锁定到柄的手枪式握把124上的托架。图15A-15B示出这一实施例的使用,其中医生能保持柄的手枪式握把124处于握把向下的取向GD,然后只旋转插管器110和微导管105。在该实施例中,监视器上的图像保持垂直而不旋转,这也许被习惯腹腔镜检查的医生优选,在该腹腔镜检查中,当操纵仪器时图像不在监视器上旋转。
在本发明的另一方面,参见图10-11,微导管105在其中远端位置处携带温度传感器或热电偶405,例如如图10所示。热电偶操作式连接到控制器255上以便控制蒸汽输送。在一个实施例中,在通过致动扳机275开始蒸汽输送之后,算法读出来自热电偶的405输出信号,在正常工作时,由于蒸汽的流动,该热电偶将指示瞬时温升。结果,该算法和热电偶405不表示在致动扳机275时的典型温升,因而当算法反映已防止能量输送的系统故障时,该算法能终止能量输送。
在另一实施例中,再参见图10-11,微导管105能在该微导管105的存在于插管器主体144的通道148中的部分中承载另一温度传感器或热电偶410。该热电偶410也操作式连接到控制器255和蒸汽源250上。在一个实施例中,在开始蒸汽输送和致动致动器308之后,算法读出来自热电偶410的输出信号,该致动器308将冲洗液从源300输送到探针的工作端145。冲洗液的输送将使热电偶的区域中的温度保持在治疗间隔期不切除组织的预定峰值水平,例如低于55℃、低于50℃或低于45℃。如果温度超过预定峰值水平,则算法和控制器可终止蒸汽能输送。在另一实施例中,控制器根据感测的温度运算和调制冷却流体流入的速率,和/或响应于感测的温度调制蒸汽流动。在可供选择的实施例中,热电偶410可被插管器主体144的暴露于微导管存在于其中的通道148的部分承载。
使用的方法
参见图16A和16B,本发明的装置和方法提供了对第一和第二侧向前列腺叶(或右侧叶和左侧叶)中以及此外具有肥大的中叶的患者中受影响的中叶中组织的精确而受控制的热切除治疗。尤其是,切除治疗配置成切除基质或平滑肌组织,切除α-肾上腺素(肌收缩)受体、和切除交感神经结构。更特别地,该切除治疗的方法配置成以平滑肌组织、α-肾上腺素受体和与膀胱颈区域420和精阜区域422间的前列腺尿道平行的交感神经结构为目标,如图16A-16B中所示。目标切除区域425具有在图16A-16B中用D表示的深度,该深度D距前列腺尿道120小于2cm,或小于1.5cm。根据患者的前列腺尿道120的长度,切除能量输送的数量可在从2到4的范围内,通常是2或3。
在使用的方法中,医生首先使患者为经尿道插入探针100的延伸部分110作好准备。在一个例子中,在手术前15-60分钟,患者可口服或舌下含轻度镇静药如安定、劳拉西泮等。特别有意义的是,由于没有与注射可冷凝的蒸汽有关的疼痛,所以不需要前列腺阻滞(注射)或其它麻醉形式。然后医生例如用食指致动针收缩致动器210,以便通过致动器的轴向运动使微导管105收缩并扳动扳机(见图4-6)。通过观察柄124,医生可看到,微导管105通过扳机210的轴向位置扳动扳机。可设置安全锁机构(未示出)以便将微导管105锁定处于待击发的位置。
接下来,医生使探针100的延伸部分110经尿道前进,同时在与内窥镜118相联接的观察监视器上观察探针的插入。在导引超过精阜422到达膀胱颈420之后,医生将定向到解剖学界标。前列腺尿道的界标和长度能相对于根据较早诊断的超声波图像或其他图像如MRI图像制定的手术前计划进行考虑。
医生能使带微导管的探针绕其轴线旋转以将微导管定向在图13A中所示的角度处来治疗第一叶。此后,治疗包括扳动扳机并松开微导管继之以输送蒸汽,移动并重复蒸汽注射,在每个叶中共3次注射。图17是本发明的方法的示意图,其中在前列腺叶中按序进行三次穿透微导管105和其中通过蒸汽能量提供能量输送以便产生略微叠加的切除作用或伤害来切除平滑肌、α-肾上腺素受体和与前列腺尿道平行的区域中的交感神经结构。本发明的方法与现有技术相比减轻了切除的组织的负担,并因此减少了导致更快的组织吸收和更快的临床改善的总体炎性响应。
图18是患者在手术之后示例性BPH治疗1周的矢量MRI图像,其中治疗包括下列步骤和能量输送参数。患者的前列腺根据超声波诊断重量为44.3克。在手术之前30分钟给患者施药Amparax(劳拉西泮)。在图18中的患者的治疗中,每个治疗间隔包括在六个部位中的每个部位处的10秒的蒸气输送(在每个前列腺叶中3次注射)。因此,在右和左前列腺叶中实际能量输送的总持续时间为60秒。输送的能量为6卡/秒(cal/sec),或者每个治疗部位425(图16A)60卡且总体总量为360卡路里,以便产生平行于前列腺尿道的切除作用,这能在图18的MRI中看到。在涉及图18的MRI图像的患者中,中叶还用单次10秒的蒸汽注射或50卡的能量治疗。蒸汽能配置成在5-10卡/秒的范围内输送能量。
通过将本发明的方法(图17)和现有技术(图3A-3B)进行比较,应该能理解,本发明的方法和设备与现有技术显著地不同。图3A示意示出现有技术的RF针,该RF针是细长的,通常长约20mm,它切除远离前列腺尿道的组织而不以接近或平行于前列腺尿道的组织为目标。其次,现有技术的RF能量输送方法施加RF能量1-3分钟或更长时间,这使热扩散效应能到达囊周边,不像本发明的方法大大限制热扩散的非常短的治疗间隔。第三,现有技术的RF能量输送方法不产生均匀切除邻近和平行于前列腺尿道的组织来切除平等地于前列腺尿道的区域中的平滑肌组织、α-肾上腺素受体、和交感神经结构。
本发明的一个方法用图19的框图示出,该方法包括以下步骤:使探针经尿道前进到患者的前列腺;将能量施加器或微导管伸入到前列腺叶的多个部位中至一小于2cm的深度;然后在每个部位处施加能量以便在平行于前列腺尿道的至少一部分的连续区域中产生切除区。
本发明的另一方法用图20的框图示出,该方法包括下列步骤:使探针经尿道前进到患者的前列腺;将能量施加器或微导管伸入到前列腺叶的多个部位中;在每个部位处施加能量小于30秒钟以便由此防止热扩散到前列腺叶的外围部分。
本发明的另一方法在图21中示出,该方法包括以下步骤:使探针经尿道前进到患者的前列腺;将能量施加器或微导管伸入到前列腺叶的多个部位中;在每个部位处施加能量一选定的时间间隔并在整个能量输送的选定的时间间隔中用冷却流体冲洗尿道。现已发现,这种冷却流体的流动也许是有用的,最重要的是冷却流体的流动能持续治疗间隔的时间,因为这种时间短,例如10-15秒。这种持续流动方法能在现有技术方法如图3A和3B的RF切除方法中使用,因为冷却流体体积聚集在患者的膀胱中,且长治疗时间间隔造成膀胱被快速充满。这会导致额外的步骤以便拨出探针、除去过量流体和然后重新开始治疗。
尽管上面详细说明了本发明的一些特别实施例,但应该理解,该说明仅用于举例说明目的且本发明的上述说明不是详尽的。本发明的特殊功能部件在某些图中示出而在另一些图中未示出,这仅是为了方便起见及任何功能部件都可以和本发明的另外功能部件组合。许多改变和可供选择的方案本领域技术人员都明白,这些可供选择的方案和改变都包括在权利要求书的范围内。在从属权利要求中存在特殊功能部件能组合并属于本发明的范围内。本发明还包括像从属权利要求参照其它独立权利要求用多个从属权利要求格式可供选择地写出的实施例。
Claims (15)
1.一种蒸汽疗法系统,包括:
适合于插入男性尿道中的轴;
该轴中的蒸汽输送针,该蒸汽输送针包括蒸汽排出口;
蒸汽源,所述蒸汽源与所述蒸汽输送针的蒸汽排出口流体联接;和
蒸汽输送致动器,该蒸汽输送致动器适合于将蒸汽从蒸汽源输送到蒸汽输送针中并使之从蒸汽排出口输出,
所述蒸汽疗法系统的特征在于该系统还包括:
蒸汽针展开机构,该蒸汽针展开机构适合于大致横向于该轴移动该蒸汽输送针的尖头;
冲洗液源和处于该轴中并从冲洗液源延伸到冲洗液出口的冲洗通道;
冲洗致动器,该冲洗致动器配置成利用冲洗液源通过冲洗液出口冲洗冷却流体;和
互锁装置,该互锁装置配置成防止在不冲洗冷却流体的情况下进行蒸汽输送。
