本发明提供一种适用于糖尿病的特殊医学用途配方食品及其制备方法,生产每吨产品的主要原料包括:牦牛奶100~2500kg,鲜牛奶100~2500kg,乳清粉200~500kg,乳清蛋白粉1~300kg,乳糖1~120kg,奶油1~100kg,精炼植物油50~200kg,中链甘油三酯1-150kg,魔芋粉1-200kg或淀粉1~150kg;或牦牛奶100~400kg,水解乳清蛋白100~300kg,奶油0~100kg,精炼植物油50~200kg,中链甘油三酯0~150kg,果糖20~300kg,魔芋粉1-200kg或淀粉1~150kg,麦芽糊精1~300kg。本发明仅用物理方法加工制成,以满足各种进食受限、糖代谢紊乱或特定糖尿病状态人群对各种营养成分的需求。

技术领域
本发明属于特殊医学食品生产技术领域,具体分别涉及一种应用牦牛奶辅以其它辅料、维生素及矿物质的低血糖指数的适合于糖尿病人食用的特殊医学用途的特定全营养配方食品。
背景技术
特殊医学用途配方食品的开发对临床患者的肠内营养支持和营养治疗方面有着非常重要的作用。
对于糖尿病这类特殊人群的核心问题是由于胰岛素的缺乏或抵抗而导致的代谢紊乱、血糖升高及消化吸收问题。糖尿病是原发于胰岛的一种糖代谢异常,胰岛是一种大的腺体,位于脊柱前方,可以向消化道分泌消化酶、胰岛素和其他一些激素。胰岛素依赖型糖尿病通常在年轻时就发病,可以导致胰岛功能衰竭。但是非胰岛素依赖型糖尿病通常在中年时才发病,很少导致胰岛功能衰竭。
据不完全统计,糖尿病在中国20岁以上人群的患病率为9.7%,患者中仅有40%进行了治疗。2011年全球共有460万人死于糖尿病或其并发症,这个数字是非常惊人的,2011年全世界有3.6亿人患糖尿病,预计到2030年可达5亿,其中大部分为Ⅱ糖尿病,其临床的主要标志症状为易口渴、饥饿、尿多,餐后高血糖,视力及神经系统病变,病久后可引起多个器官损害、系统代谢紊乱、酮症酸中毒,对于一些本身就有糖类不耐受的人群,在发生创伤、感染或怀孕等应激状况时易引发糖尿病,且糖尿病患者易发并发症,易引发伤口不愈合、神经系统损伤如视神经损害、高渗性昏迷;同时易患癌症,癌细胞易转移扩散。 而目前,医学界还没有能完全治愈这种病症的特效药。临床研究也证实,药物治疗和控制饮食相结合对病情控制很有利,正如俗话所说:“三分治,七分养”。 糖尿病人的饮食治疗是控制糖尿病的首要手段,其重要性不亚于药物治疗,低血糖指数膳食不仅可以帮助糖尿病患者有效控制血糖,而且在延缓饥饿、增强饱腹感、预防低血糖、防治心血管并发症、减重以及预防糖尿病方面都有帮助。通过改善患者的营养状况,维持人体所需要的各种营养成分的平衡,提高患者的生存与生活质量。
因此,特殊医学用途配方食品的开发对临床糖尿病患者的肠内营养支持和营养治疗方面有着非常重要的作用。现有的用于糖尿病患者的配方食品也很多,如中国专利申请公开的一种适用于糖尿病患者食用的配方食品及其制备方法(201510057275.3),由荞麦、小麦纤维、山楂、葛根、低聚异麦芽糖、木糖醇、食品添加剂、维生素预混料经过提炼、浓缩、干燥、混合等步骤制成。其可有效降低患者的血糖,缓解患者的临床症状。但其不能为患者提供合理的蛋白质需求,难以增强患者的免疫能力,不能避免冠心病、肾病等并发症的发生。
发明内容
本发明的目的在于提供一种适用于糖尿病的特殊医学用途配方食品,其利用牦牛奶、鲜牛奶、乳清粉、乳清蛋白粉、奶油、精炼植物油、中链甘油三酯、果糖、乳糖、魔芋粉或淀粉、麦芽糊精、水解乳清蛋白等加入适量的维生素和矿物质或其他营养成分,仅用物理方法加工制成的以满足各种进食受限、糖代谢紊乱或特定疾病状态人群对各种营养成分的需求。
为实现上述目的,本发明的技术方案为:一种适用于糖尿病的特殊医学用途配方食品及其制备方法,生产每吨产品的主要原料包括:
(1)含乳糖配方(不适用于乳糖不耐症):牦牛奶 100 ~2500kg,鲜牛奶100~2500kg,乳清粉(D90)200~500 kg,乳清蛋白粉1 ~300 kg,乳糖1~120 kg,奶油1~100kg,精炼植物油50~200kg ,中链甘油三酯1~150kg,魔芋粉1~200kg或淀粉1~150kg。
或(2)低乳糖配方(适用于乳糖不耐症): 牦牛奶 100 ~400kg,水解乳清蛋白100~300 kg,奶油1~100kg,精炼植物油50~200kg ,中链甘油三酯1~150kg,果糖20~300 kg,魔芋粉1~200kg或淀粉1~150kg,麦芽糊精1~300 kg。
为了给糖尿病患者提供均衡及更多的营养物质,还添加有:醋酸视黄酯50~100g;胆钙化醇10~80g;dl-ɑ-醋酸生育酚40~600g;植物甲萘醌10~200g;盐酸硫胺素5~80g;核黄素1~400g;盐酸吡哆醇4~80g;氰钴胺5~50g;烟酸10~100g;叶酸6~40g;D-泛酸钙15~200g;L-抗坏血酸700~3500g;D-生物素1~50g;肌醇1~700g;牛磺酸1~700g;氯化胆碱200~1500g;碳酸钙1~6000g;硫酸亚铁100~450g;硫酸锌60~330g;硫酸锰1~100g;硫酸铜6~40g;柠檬酸钾200~3000 g;氧化镁1~3500g;亚硒酸钠1~100g;二水钼酸钠10~100 g;六水三氯化铬10~100 g;碘酸钾1~60g;氯化钾100-5000g;磷酸氢二钠1000~4800 g;柠檬酸钠200~3000g;核苷酸100~300g;二十二碳六烯酸DHA 1~20kg;花生四烯酸ARA 2~40 kg。
各类营养物质的纯度:醋酸视黄酯9.76%;胆钙化醇0.25%;dl-ɑ-醋酸生育酚50%;植物甲萘醌1.00%;盐酸硫胺素≥99%;核黄素10%;盐酸吡哆醇≥98%;氰钴胺0.1%;烟酸≥99.5%;叶酸10%;D-泛酸钙97~102%;L-抗坏血酸≥98%;D-生物素1%;肌醇≥98%;牛磺酸≥98.5%;氯化胆碱98%;碳酸钙≥98%;硫酸亚铁≥98%;硫酸锌≥98%;硫酸锰10%;硫酸铜≥99%;柠檬酸钾≥98%;氧化镁≥99%;亚硒酸钠1%;二水钼酸钠5%;六水三氯化铬5%;碘酸钾10%;磷酸氢二钠≥98%;柠檬酸钠99~100.5%;氯化钾≥99.0%;核苷酸≥98%;二十二碳六烯酸DHA 7%;花生四烯酸ARA 10%。
作为优化,所述乳清粉为D90,乳清蛋白粉为WPC34和WPC80。
作为优化,所述精炼植物油为向日葵油、橄榄油、棕榈油或大豆油。
本发明还提供一种上述适用于糖尿病的特殊医学用途配方食品的制备方法,包括以下步骤:
(1)经杀菌、净化处理的牦牛奶、鲜牛奶、纯净水经板式换热器预热至50℃~55℃,吸入真空混料机与混料罐形成闭合循环;
(2)将原料中除维生素、矿物质的干粉原料经振动筛机输入到高速在线混合机中,用步骤(1)处理后的牦牛奶和鲜牛奶去混合溶解循环,同时将奶油、植物油、中链甘油三酯和溶解好的维生素、矿物质用泵输入到混合机的管路内进行在线混合乳化循环,此配料的浓度应在11~13%之间;
(3)全部溶解混合好的物料进行均质,均质压力14~16Mpa,均质后的物料通过冷却板式换热器将物料温度降到4℃~6℃;
(4)均质后将物料用泵送入蒸发器进行杀菌浓缩,杀菌温度90~93℃,且物料在蒸发器的杀菌管道内的停留时间不能少于36秒,经多流程浓缩后物料浓度在41~44%之间,再经高压泵送入低温干燥塔进行喷雾干燥并通过静床和振动流化床进行二次附聚干燥成粉末状成品。
作为优化,步骤(2)中,在溶解维生素类物质和矿物质时,先溶解除二水钼酸钠和六水三氯化铬外的矿物质,间隔10分钟后再溶解维生素,再间隔5分钟后将二水钼酸钠和六水三氯化铬溶解进行物料循环。
作为优化,步骤(4)所得粉末状成品压成片或块状,以便携带、食用方便。
对于糖尿病人来说,膳食中蛋白质的来源选择非常重要,动物蛋白含丰富的必须氨基酸,生物价值高、利用率好,而植物蛋白所含的必须氨基酸较少,因此,在本发明中的蛋白质来源选择是动物蛋白中优质的牛奶蛋白,其中一大部分为牦牛奶蛋白,牛奶本身就含有各种必须氨基酸,而牦牛奶中必需氨基酸是黑白花牛奶高1.897倍,并且含有人体所需要的18种氨基酸,包括天门冬氨酸ASP、苏氨酸THR、丝氨酸SER、谷氨酸GLU、甘氨酸、GLY、丙氨酸ALA、缬氨酸VAL、蛋氨酸MET、异亮氨酸ILE、亮氨酸LEU、酪氨酸TYR、苯丙氨酸PHE、赖氨酸LYS、组氨酸HIS、精氨酸ARG、脯氨酸PRO、色氨酸TRP、胱氨酸CYS,其中有8种是人体内不能合成的必须氨基酸,比普通牛奶高1.897倍。糖尿病患者的膳食中碳水化合物对血糖的影响非常大,既不能多也不能少,而且碳水化合物的种类选择也很重要,过多的碳水化合物不利于患者血糖的控制,过少会引起机体糖异生作用增强,脂肪分解增加,会产生代谢性酸中毒和饥饿性酮症,反而会使耐糖量降低,而适量和适当的碳水化合物不仅可改善耐糖量,还可降低胆固醇和甘油三酯。在控制餐后血糖急剧上升和改善糖耐量方面,可溶性膳食纤维效果最佳。利用血糖指数低的膳食纤维、乳糖、果糖、麦芽糊精等不会使血糖升高,增加饱腹感,降低血脂和胆固醇、提高免疫力、促进维生素和矿物质元素的吸收和防止腹泻等方面具有重要的生理作用。低血糖指数膳食还可通过降低血液中总胆固醇和低密度脂蛋白,增加高密度脂蛋白等作用来预防心血管病。由于有利于改善血管健康,因而低血糖指数膳食对眼病、肾衰竭和腿部神经麻木等并发症的预防都有一定帮助,摄食低血糖指数食物引起轻低的血胰岛素水平,促使身体燃烧更多的脂肪,而减少脂肪的储存。本发明中选择的所有含碳水化合物的原料都为低血糖指数GI<55的,尤其是魔芋粉为高膳食纤维食品,血糖指数GI应为0,牛奶的GI值为27,乳糖的GI值为46,果糖的GI值为23。众所周知,进食血糖指数越低的食物,餐后血糖升高的越慢,对糖尿病患者越有利,在本发明的配方中,只有牛奶中所固有的乳糖的GI值为46,相对于其它成份是较高的,但也是小于55这个公认的指标,都是符合糖尿病人的饮食要求的。利用中链甘油三酯中的中链脂肪酸能更好地维持血甘油三酯水平,对伴有高甘油三酯血症的Ⅱ糖尿病患者有利,中链甘油三酯对Ⅱ型糖尿病(T2DM)患者减轻体重,降低发生冠心病等并发症的危险性有很大帮助。Ⅱ型糖尿病(T2DM)患者通常比非糖尿病者具有更高的体重,减轻体重可降低T2DM发生冠心病等并发症的危险性。通过调整饮食来减肥,安全性明显优于药物作用,但长期进行传统低脂膳食可能导致脂溶性维生素的丢失,所以在控制总能量的基础上,改善脂类摄入品质和构成非常重要。中链甘油三酯(MCT)容易吸收、直接通过门静脉转运至肝脏并且不依赖肉毒碱就可直接迅速进行氧化,不易造成脂肪储存,可以代替部分传统膳食油脂长链甘油三酯(LCT)。使用富含单不饱和脂肪酸的向日葵油、橄榄油、棕榈油等复合油脂可以降低患者血浆中甘油三酯和总胆固醇的水平,减少心血管病危险,改善免疫功能,由于单不饱和脂肪酸只含有单个不饱和双键,不易发生过氧化,对血脂影响较小。牦牛奶中天然含有的铬元素与钙、铁、锌、钾等一样丰富,铬元素可使胰岛素的活性增加,当缺乏铬时,就很容易表现出糖代谢失调,如不及时补充这种元素,就会患糖尿病,诱发冠状动脉硬化导致心血管病,严重的会导致白内障、失明、尿毒症等并发症。本发明中再额外补充三价铬来进一步满足糖尿病人对铬元素的需求。
总而言之,本发明以牦牛奶和鲜牛奶作为乳类原料,进行合理正确的配比,特别采用牦牛奶中丰富的、天然的氨基酸、共轭亚油酸、维生素、钙、铁、锌、铬等,避免了单独使用普通牛奶时需要补充大量的化工合成的有机和无机营养素,能更好地吸收代谢,减少了对胃肠道的刺激;所有含碳水化合物原料都为低血糖指数GI<55的,能够减缓患者血糖升高,添加魔芋粉从而利用其膳食纤维中的葡甘聚糖,葡甘聚糖、乳糖、果糖、麦芽糊精等不会使血糖升高,增加饱腹感,降低血脂和胆固醇;使用富含单不饱和脂肪酸的向日葵油、橄榄油、棕榈油等复合油脂可以降低患者血浆中甘油三酯和总胆固醇的水平,减少心血管病危险,改善免疫功能;利用中链甘油三酯中的中链脂肪酸能更好地维持血甘油三酯水平,对伴有高甘油三酯血症的Ⅱ糖尿病患者有利,中链甘油三酯对Ⅱ型糖尿病(T2DM)患者减轻体重,降低发生冠心病等并发症的危险性有很大帮助;牦牛奶中的天然共扼亚油酸具有清除自由基,增强机体的抗氧化能力和免疫能力,促进脂肪氧化分解,促进人体蛋白合成对有着重要作用。在低乳糖配方中,乳糖含量≤2%,是专为乳糖不耐受、有腹泻的糖尿病患者配置的特殊膳食类的营养食品。同时使用经过酶水解的乳清蛋白,由于高效的蛋白消化率,通过非常好的氨基酸模式,更容易被吸收,减少了肾脏的负担,对乳蛋白过敏、不吸收的患者是更好的营养补充食品。
具体实施方式
本发明公开了一种适用于糖尿病人的特殊医学用途牦牛奶配方食品的产品配方及生产工艺,本领域技术人员可以借鉴本发明内容,对原料和添加剂在国家相关标准规定范围内做适当的替换和调整,但需要特别说明的是,所有的替换和调整都被视为包括在本发明的范围中。因特殊医学用途配方食品是针对有特殊医学状况的人群而设计的,对其内含营养成分有着严格的控制要求,本发明的配方已经过检测、实践和应用,是符合国家食品安全标准的,并通过较好的实例进行如下描述,相关人员在使用本发明时不应脱离本发明内容和范围进行改动、变更或组合。

上述实施例中提供的糖尿病患者的特殊医学用途配方食品的具体食用量与食用方法在产品包装标签上明示,具体食用方法如:“每14g粉用90ml水冲调成液态后再食用、每日的食用量根据病情如何调整”等应用食用表来明示,,产品标签应对产品的配方特点进行说明,且产品的类别和适用人群,同时还在标示“不适用于非目标人群”,应醒目标示:“请在医生或临床营养师指导下使用”,还应标示:“本产品禁止用于肠外营养支持和静脉注射”。
Claims (8)
1.一种适用于糖尿病的特殊医学用途配方食品,其特征在于,主要原料包括:
(1)牦牛奶 100 ~2500kg,鲜牛奶100~2500kg,乳清粉(D90)200~500 kg,乳清蛋白粉(WPC34和WPC80)1 ~300 kg,乳糖1~120 kg,奶油1~100kg,精炼植物油50~200kg ,中链甘油三酯1~150kg,魔芋粉1~200kg或淀粉1~150kg;
或(2)牦牛奶 100 ~400kg,水解乳清蛋白100~300 kg,奶油1~100kg,精炼植物油50~200kg ,中链甘油三酯1~150kg,果糖20~300 kg,魔芋粉1~200kg或淀粉1~150kg,麦芽糊精1~300 kg。
2.根据权利要求1所述的适用于糖尿病的特殊医学用途配方食品,其特征在于,原料中还添加有:醋酸视黄酯50~100g;胆钙化醇10~80g;dl-ɑ-醋酸生育酚40~600g;植物甲萘醌10~200g;盐酸硫胺素5~80g;核黄素0~400g;盐酸吡哆醇4~80g;氰钴胺5~50g;烟酸10~100g;叶酸6~40g;D-泛酸钙15~200g;L-抗坏血酸700~3500g;D-生物素1~50g;肌醇0~700g;牛磺酸1~700g;氯化胆碱200~1500g;碳酸钙1~6000g;硫酸亚铁100~450g;硫酸锌60~330g;硫酸锰1~100g;硫酸铜6~40g;柠檬酸钾200~3000 g;氧化镁1~3500g;亚硒酸钠1~100g;二水钼酸钠10~100 g;六水三氯化铬10~100 g;碘酸钾1~60g;氯化钾100-5000g;磷酸氢二钠1000~4800 g;柠檬酸钠200~3000g;核苷酸100~300g;二十二碳六烯酸DHA 1~20kg;花生四烯酸ARA 2~40 kg。
3.根据权利要求2所述的适用于糖尿病的特殊医学用途配方食品,其特征在于,各类营养物质的纯度:醋酸视黄酯9.76%;胆钙化醇0.25%;dl-ɑ-醋酸生育酚50%;植物甲萘醌1.00%;盐酸硫胺素≥99%;核黄素10%;盐酸吡哆醇≥98%;氰钴胺0.1%;烟酸≥99.5%;叶酸10%;D-泛酸钙97~102%;L-抗坏血酸≥98%;D-生物素1%;肌醇≥98%;牛磺酸≥98.5%;氯化胆碱98%;碳酸钙≥98%;硫酸亚铁≥98%;硫酸锌≥98%;硫酸锰10%;硫酸铜≥99%;柠檬酸钾≥98%;氧化镁≥99%;亚硒酸钠1%;二水钼酸钠5%;六水三氯化铬5%;碘酸钾10%;磷酸氢二钠≥98%;柠檬酸钠99~100.5%;氯化钾≥99.0%;核苷酸≥98%;二十二碳六烯酸DHA 7%;花生四烯酸ARA 10%。
4.根据权利要求1所述的适用于糖尿病的特殊医学用途配方食品,其特征在于,所述乳清粉为D90,乳清蛋白粉为WPC34和WPC80。
5.根据权利要求1所述的适用于糖尿病的特殊医学用途配方食品,其特征在于,所述精炼植物油为向日葵油、橄榄油、棕榈油或大豆油。
6.一种适用于糖尿病的特殊医学用途配方食品的制备方法,其特征在于,包括以下步骤:
(1)经杀菌、净化处理的牦牛奶、鲜牛奶、纯净水经板式换热器预热至50℃~55℃,吸入真空混料机与混料罐形成闭合循环;
(2)将原料中除维生素、矿物质的干粉原料经振动筛机输入到高速在线混合机中,用步骤(1)处理后的牦牛奶和鲜牛奶去混合溶解循环,同时将奶油、植物油、中链甘油三酯和溶解好的维生素、矿物质用泵输入到混合机的管路内进行在线混合乳化循环,此配料的浓度应在11~13%之间;
(3)全部溶解混合好的物料进行均质,均质压力14~16Mpa,均质后的物料通过冷却板式换热器将物料温度降到4℃~6℃;
(4)均质后将物料用泵送入蒸发器进行杀菌浓缩,杀菌温度90~93℃,且物料在蒸发器的杀菌管道内的停留时间不能少于36秒,经多流程浓缩后物料浓度在41~44%之间,再经高压泵送入低温干燥塔进行喷雾干燥并通过静床和振动流化床进行二次附聚干燥成粉末状成品。
7.根据权利要求6所述的适用于糖尿病的特殊医学用途配方食品的制备方法,其特征在于,步骤(2)中,在溶解维生素类物质和矿物质时,先溶解除二水钼酸钠和六水三氯化铬外的矿物质,间隔10分钟后再溶解维生素,再间隔5分钟后将二水钼酸钠和六水三氯化铬溶解进行物料循环。
8.根据权利要求6所述的适用于糖尿病的特殊医学用途配方食品的制备方法,其特征在于,步骤(4)所得粉末状成品压成片或块状。
