本发明涉及中药领域,尤其涉及一种治疗颈动脉斑块的中药药膏及制备方法。
背景技术
颈动脉斑块是颈动脉粥样硬化的表现,好发于颈总动脉分叉处,目前认为与老年人缺血性脑卒中的发生密切相关。其引起缺血性脑卒中的机制可能为:斑块增大致颈动脉管径狭窄引起颅内低灌注及斑块脱落形成栓子,导致颅内动脉栓塞。临床上,通过对颈动脉的狭窄程度及斑块的形态学测定,来对颈动脉斑块进行评价,判断其危害性。
颈动脉斑块的形成与动脉粥样硬化相同,受多种因素影响。其中年龄大于60岁、男性、长期吸烟史、高血压病史、糖尿病史及高脂血症等是颈动脉斑块形成的危险因素。根据是否产生相关的脑缺血症状,分为有症状性和无症状性两大类:症状性(1)短暂性脑缺血发作(TIA),可表现为一过性单侧肢体感觉、运动障碍、单眼失明或失语等,一般仅持续数分钟,发病后24小时内完全恢复。发作过后查体无明确阳性体征,影像学检查无局灶性病变;(2)缺血性脑卒中,可有相应神经系统定位体征,影像学检查可见局灶性病变。无症状性许多颈动脉硬化性疾病患者临床上没有任何神经系统症状或仅有一些非特异性表现,如头晕、头痛、晕厥等。
目前首选的无创性颈动脉检查手段,广泛应用于颈动脉硬化病变的筛查及随访。不仅可以显示斑块的部位和大小、管腔狭窄部位和严重程度,还能进行血流动力学测定,并可对斑块进行形态学评价。彩色多普勒超声通过检测颈动脉内膜中层厚度(IMT)来确定是否有动脉粥样硬化斑块形成。正常IMT应小于1.0mm,IMT在1.0~1.2mm间为内膜增厚,大于1.2mm为斑块形成。超声下根据形态和回声特点,斑块可分为:①低回声脂质性软斑;②中等回声富含胶原组织的纤维性扁平斑块;③强回声伴声影的钙化性硬斑块;④回声强弱不等的溃疡性混合型斑块。其中,软斑、扁平斑和混合斑属于不稳定斑块,是引起缺血性脑卒中的重要原因之一。
目前治疗颈动脉斑块多以药物治疗和手术治疗为主,但疗程较长,易产生耐药性,且有一定的副作用,不宜长久服用,而手术治疗,风险较大,费用较高,容易给患者带来很大的精神和经济负担。
发明内容
本发明提供了一种治疗颈动脉斑块的中药药膏及制备方法;本发明中药药膏所选药材配伍相宜,符合中医药学和现代医药学理论,并具有祛湿消斑、活血化瘀、散郁开结之功效,对治疗颈动脉斑块具有独特疗效,且疗效显著,使用方便,疗程较低,见效较快,治愈后不复发,安全无毒副作用,能够达到标本兼治的目的,成本较低,奇效实用,值得临床上广泛推广应用。

一种治疗颈动脉斑块的中药药膏的制备方法,包括以下工艺步骤:
(1)将千日红、苘麻、辣椒茎、泽兰和匙叶草洗净,放入容器内,加入8~10倍量的蒸馏水,浸泡5~6小时后,煮沸3~4小时,提取;再次加入6~8倍量蒸馏水,煮沸2~3小时,提取;最后,加入4~6倍量蒸馏水,加热煮沸1~2小时,提取;合并三次提取液,过滤,得滤液,浓缩成60℃下相对密度为1.14~1.17的稠膏,备用;
(2)将月季花、鸡娃草和八角莲放入容器内,进行粉碎处理,研末,得到过180目的细粉,备用;
(3)将茜草、雀梅藤、金银花、雉子筵、番红花和玉柏洗净,浸入5倍量的70%的乙醇溶液浸泡1~2小时,加热提取3小时,回收乙醇,滤过,得滤液,浓缩为65℃下相对密度为1.12~1.15的稠膏,备用;
(4)将青葙子洗净,切碎,放入容器内,绞汁,滤杂质,留汁液备用;
(5)将秦艽去杂质洗净,切片,置于炒锅内,以文火加热,炒至药片表面微有焦斑为宜,取出摊晾,置于研钵内,研末,得到过200目的细粉,备用;
(6)将上述步骤(1)~(5)所制得的稠膏、细粉以及汁液制成药膏,即得所述的治疗颈动脉斑块的中药药膏。
本发明所用中药的药性如下:
千日红:味甘,性平。祛痰平喘,清肝散结,止咳定喘。用于慢性支气管炎,喘息性慢性支气管炎;治头风,目痛,气喘咳嗽,痢疾,百日咳,小儿惊风,瘰窃,疮疡。《广西中药志》载:“花序:凉血消肿,止痉咳。治百日咳;外治疮疡肿痛。民间治月经不调。全株煲水外洗,治跌打疮疖。”现代医学研究,千日红含有人体所需的氨基酸、维生素C、E及多种微量元素,具有平肝熄火、明目散结、止咳定喘、清新润肺、调整内分泌紊乱、解郁降火、补血、健脾胃、通经络、消炎、祛斑、特别是治疗内分泌紊乱引起的黄褐斑、雀斑、肝斑、色斑、暗疮有明显疗效。
月季花:味甘,性温;归肝经。活血调经,散毒消肿。用于月经不调,痛经,痈疖肿毒,淋巴结结核;淋巴结结核,跌打损伤;跌打损伤,白带,遗精。《现代实用中药》载:“活血调经。治月经困难,月经期拘挛性腹痛.外用捣敷肿毒,能消肿止痛。”
茜草:味苦,性寒。归肝经。凉血活血,祛瘀,通经。用于吐血、衄血、崩漏下血、外伤出血、经闭瘀阻、关节痹痛、跌扑肿痛。凉血止血,活血祛瘀:本品止血而不留瘀,用于热证出血、经闭腹痛、跌打损伤。配乌贼骨止血力更强。
青葙子:味苦,性寒;归肝经。祛民热,清肝火,明目退翳。主目赤肿痛,眼生翳膜,视物昏花,高血压病,鼻衄,皮肤风热瘙痒,疮癣。《药性论》载:“治肝脏热毒冲眼,赤障、青盲、翳肿。主恶疮疥瘙,治下部虫露疮。”
苘麻:味苦,性平,归脾、胃经;清热利湿,解毒开窍。主治痢疾、中耳炎、耳鸣、耳聋、睾丸炎、化脓性扁桃体炎、痈疽肿毒。《上海常用中草药》载:“全草:解毒,祛风。治痢疾,中耳炎,耳鸣,耳聋,关节酸痛。”
雀梅藤:味甘、淡,性平;归肺、肾经。降气,化痰,祛风利湿。主咳嗽,哮喘,胃痛,鹤膝风,水肿。《广西药植名录》载:“祛毒,生肌。治疮疥。”
秦艽:味辛、微苦,性平;归胃、肝、胆经。祛风湿,清湿热,止痹痛。用于风湿痹痛,筋脉拘挛,骨节酸痛,日晡潮热,小儿疳积发热。现代医学研究证实,秦艽乙醇浸液在体外对炭疽杆菌、葡萄球菌、伤寒杆菌、肺炎杆菌、痢疾杆菌、霍乱弧菌均有抑制作用,其水浸剂在试管内对常见皮肤真菌也有不同程度的抑制作用。
金银花:味甘,微苦,性寒;归肺、胃、心、大肠经。清热祛邪,宣散风热,清解血毒。现代药理研究表明,金银花中的有效成分具有抗菌及抗病毒作用,对金黄色葡萄球菌、溶血性链球菌、痢疾杆菌、伤寒杆菌、脑膜炎双球菌、肺炎双球菌、绿脓杆菌以及流感病毒等都有明显抑制作用,并且能够增强免疫功能,抗炎、解热,对炎症有明显抑制作用。
辣椒茎:味辛,性热,无毒。除寒湿,逐冷痹,散瘀血凝滞。治风湿冷痛,冻疮。
雉子筵:味甘,性温;归肺、脾经。活血化瘀,养阴清热。主疝气,干血痨。《陕西草药》载:“益中气,补阴虚。治疝气及干血痨。”
泽兰:味苦、辛,性微温;归肝、脾经。活血化瘀,行水消肿。用于月经不调,经闭,痛经,产后瘀血腹痛,水肿。《日华子本草》载:“通九窍,利关脉,养血气,破宿血,消症瘕,产前产后百病,通小肠,长肉生肌,消扑损瘀血,治鼻洪吐血,头风目痛,妇人劳瘦,丈夫面黄。”
番红花:味甘,性平;归心、肝经。活血祛瘀,散郁开结,凉血解毒。主痛经,经闭,月经不调,产后恶露不净,腹中包块疼痛,跌扑损伤,忧郁痞闷,惊悸,温病发斑,麻疹。
匙叶草:味苦、辛,性寒;归肺、肝、大肠经。活血化瘀,清热止咳。主腹内血瘀痞块,劳伤咳嗽。
玉柏:味酸,性温,无毒;归肺、肾经。祛风通络,祛风除湿,舒筋通络,活血化瘀。主治风湿疼痛,肢体麻木,跌打扭伤及鸡爪风等症,小儿麻痹症后遗症。《本草拾遗》载:“取根茎浸酒,去风血,除风瘙,宜老。”
鸡娃草:味苦,性寒。杀虫止痒,腐蚀疣痣。主体癣,头癣,手足癣,神经性皮炎,疣痣。《陕甘宁青中草药选》载:“杀虫止痒。治神经性皮炎,牛皮癣。”
八角莲:味苦,性凉。清热解毒,活血化瘀。主治:毒蛇咬伤,跌打损伤;外用治虫蛇咬伤,痈疮疖肿,淋巴结炎,腮腺炎,乳腺癌。广州部队《常用中草药手册》载:清热解毒,燥湿泻火。治淋巴结炎,腮腺炎,痈疮。”

具体实施方式
下面结合具体实施例对本发明做进一步阐述。
实施例1
本发明中药药膏有效成分的原料组成及重量为:
一种治疗颈动脉斑块的中药药膏的制备方法,包括以下工艺步骤:
(1)将千日红、苘麻、辣椒茎、泽兰和匙叶草洗净,放入容器内,加入8~10倍量的蒸馏水,浸泡5~6小时后,煮沸3~4小时,提取;再次加入6~8倍量蒸馏水,煮沸2~3小时,提取;最后,加入4~6倍量蒸馏水,加热煮沸1~2小时,提取;合并三次提取液,过滤,得滤液,浓缩成60℃下相对密度为1.14~1.17的稠膏,备用;

临床资料:
1、病例选择:
作为治疗颈动脉斑块的中药药膏,选择确诊的颈动脉斑块患者118例临床观察,病人随机分成治疗组和对照组两组,治疗组59例,其中男性30例,女性29例,年龄60~81岁,平均年龄70.5岁,平均病程5个月;对照组59例,其中男性29例,女性30例,年龄61~80岁,平均年龄70.5岁,平均病程5个月。治疗组和对照组两组病例的病程、症状轻重程度基本一致,无显著差异,具有可比性。
2、药物选择:
治疗组使用本发明中药药膏,外用,温水清洁患处后,取适量本发明中药药膏均匀涂抹于患处,按摩至药膏完全吸收,每日3~5次,7天为1个疗程,连续使用3~5个疗程,病情严重者遵医嘱酌增用量。
对照组服用阿司匹林,口服,一日75mg~160mg,每日1次,疗程视病情而定,或遵医嘱。
3、疗效判定:
(1)治愈:颈动脉斑块及其临床症状全部消失,身体恢复正常。
(2)显效:颈动脉斑块及临床症状明显减轻,身体明显好转。
(3)有效:颈动脉斑块及临床症状有所减轻,身体有所好转。
(4)无效:通过治疗后颈动脉斑块及临床症状无减轻或加重。
4、结果:
以上结果显示,治疗组的治愈率和总有效率明显高于对照组。

1、薛某,女,60岁,天津汉沽区人。患者体检时发现颈动脉斑块,去医院治疗,治疗效果不佳,经使用本发明中药药膏治疗,使用1个疗程后,症状明显减轻,继续使用2个疗程后,症状全部消失,痊愈,半年后随访无复发。
2、丁某,男,72岁,河北衡水人。患者因短暂性脑缺血发作,去医院治疗,疗效一般,且反复发作。颈动脉超声示:双侧颈动脉稍弯曲,左侧颈动脉分叉段前壁见稍增强实质斑块,右侧颈总动脉分叉段的前臂见稍增强实质斑块。经医生建议使用本发明中药药膏治疗,使用1个疗程后,左侧斑块和右侧斑块明显缩小,使用3个疗程后,左侧斑块和右侧斑块全部消失,痊愈,1年后随访无复发。
3、魏某,女,65岁,重庆南川区人。患者因右上肢麻木4月余,去门诊治疗,治疗效果不佳,检查颈动脉超声示有右侧颈动脉稍弯曲,双侧颈动脉管壁毛糙,左侧颈总动脉分叉段的前臂见稍增强,有实质斑块;右侧颈总动脉分叉段的后壁见稍增强,有实质斑块。使用本发明中药药膏治疗,使用1个疗程后,右上肢麻木症状明显好转,再行颈动脉超声示:左侧斑块和右侧斑块有所缩小,使用3个疗程后,右上肢麻木症状消失,再行颈动脉超声示:左侧斑块和右侧斑块消失,1年后随访无复发。
4、叶某,男,67岁,广东阳江人。患者无明显诱因出现发作性右侧肢体无力,去医院治疗,治疗效果不佳,且反复发作性左侧肢体无力,经查颈部动脉B超示:右侧颈内动脉起始处可见低回声斑块,医生推荐使用本发明中药药膏治疗,使用2个疗程后,复查颈部动脉B超示:右侧颈内动脉起始处低回声斑块缩小,继续使用2个疗程后,右侧颈内动脉起始处低回声斑块全部消失,痊愈,随访至今无复发。
5、孟某,女,61岁,安徽安庆人。患者患发作性左侧肢体无力,并伴有头晕,无恶心、呕吐,轻微头痛,经B超检查,左侧颈内动脉起始处可见低回声斑块,用过多种方法治疗,治疗效果不明显,经人介绍,使用本发明中药药膏治疗,使用1个疗程后,症状明显减轻,经B超复查,左侧颈内动脉起始处可见低回声斑块缩小,继续使用2个疗程后,经B超复查,左侧颈内动脉起始处可见低回声斑块症状消失,痊愈,2年后随访无复发。
6、阎某,男,69岁,上海金山区人。患者因无明显诱因出现头晕,伴恶心、呕吐,轻微头痛,无肢体抽搐,无肢体活动障碍,去医院就诊,入院后查颈部动脉B超示:左侧颈内动脉起始处可见1.3cm×1.6cm大小低回声斑块,给予本发明中药药膏治疗,使用1个疗程后,症状明显缓解,复查颈部动脉B超示:左侧颈内动脉起始处可见1.2cm×1.4cm大小不均质回声斑块,使用3个疗程后,症状全部消失,痊愈,随访至今无复发。

1.一种治疗颈动脉斑块的中药药膏,其特征在于:制成所述中药药膏有效成分的原料组成及重量份数为:
千日红20~38份月季花19~37份茜草19~37份青葙子18~36份
苘麻17~35份雀梅藤17~35份秦艽16~34份金银花15~33份
辣椒茎15~33份雉子筵14~32份泽兰14~32份番红花12~30份
匙叶草10~28份玉柏8~27份鸡娃草8~27份八角莲7~25份。
2.根据权利要求1所述的一种治疗颈动脉斑块的中药药膏,其特征在于:制成所述中药药膏有效成分的原料组成及重量份数为:
千日红23~35份月季花22~34份茜草22~34份青葙子21~33份
苘麻20~32份雀梅藤20~32份秦艽19~31份金银花18~30份
辣椒茎18~30份雉子筵17~29份泽兰17~29份番红花15~27份
匙叶草13~25份玉柏12~24份鸡娃草12~24份八角莲10~22份。

4.根据权利要求1所述的一种治疗颈动脉斑块的中药药膏,其特征在于:制成所述中药药膏有效成分的原料组成及重量份数为:
千日红29份月季花28份茜草28份青葙子27份
苘麻26份雀梅藤26份秦艽25份金银花24份
辣椒茎24份雉子筵23份泽兰23份番红花21份
匙叶草19份玉柏18份鸡娃草18份八角莲16份。
5.一种如权利要求1~4任一项所述治疗颈动脉斑块的中药药膏的制备方法,其特征在于:包括以下工艺步骤:
(1)将千日红、苘麻、辣椒茎、泽兰和匙叶草洗净,放入容器内,加入8~10倍量的蒸馏水,浸泡5~6小时后,煮沸3~4小时,提取;再次加入6~8倍量蒸馏水,煮沸2~3小时,提取;最后,加入4~6倍量蒸馏水,加热煮沸1~2小时,提取;合并三次提取液,过滤,得滤液,浓缩成60℃下相对密度为1.14~1.17的稠膏,备用;
(2)将月季花、鸡娃草和八角莲放入容器内,进行粉碎处理,研末,得到过180目的细粉,备用;
(3)将茜草、雀梅藤、金银花、雉子筵、番红花和玉柏洗净,浸入5倍量的70%的乙醇溶液浸泡1~2小时,加热提取3小时,回收乙醇,滤过,得滤液,浓缩为65℃下相对密度为1.12~1.15的稠膏,备用;
(4)将青葙子洗净,切碎,放入容器内,绞汁,滤杂质,留汁液备用;
(5)将秦艽去杂质洗净,切片,置于炒锅内,以文火加热,炒至药片表面微有焦斑为宜,取出摊晾,置于研钵内,研末,得到过200目的细粉,备用;
(6)将上述步骤(1)~(5)所制得的稠膏、细粉以及汁液制成药膏,即得所述的治疗颈动脉斑块的中药药膏。
