Abstract

一种治疗三叉神经痛的外用复合油及其制备方法。本发明的治疗三叉神经痛的外用复合油；由以下原料药制成：小驳骨6～9份、木藤蓼6～9份、豨莶草9～12份、功劳叶8～10份、一叶兰12～15份、羌活12～15份、木贼草8～10份、橄榄油40～45份、鸸鹋油25～30份。制备复合油的方法，包括以下步骤：A、取小驳骨、木藤蓼、豨莶草、功劳叶、一叶兰，加95%乙醇，提取后，烘干，制成300目粉末；B、取羌活、木贼草制成300～400目粉末；C、取橄榄油、鸸鹋油加入搅拌机中搅拌均匀；D、将步骤A中的粉末、步骤B中的粉末、步骤C中的混合物，加入到超声波搅拌机中，在60～65℃条件下，超声搅拌1～1.5小时，冷却即成。

一种治疗三叉神经痛的外用复合油及其制备方法
技术领域
本发明属于医药制备技术领域，具体为一种治疗三叉神经痛的外用复合油及其制备方法。
背景技术
三叉神经痛是最常见的脑神经疾病，临床表现主要为一侧面部三叉神经分布区内反复发作的阵发性剧烈疼痛，发病骤起骤停，病情顽固，疼痛呈周期性发作，间歇期如常人，多发生于中老年人，发病率女多于男，长期慢性疼痛，对患者的生存和工作质量造成严重影响。三叉神经痛在中医中属“头痛”和“头风”等范畴，气滞血瘀是其主要病机。目前尚缺乏有效治疗手段，临床常用的药物治疗、立体定向放射治疗和外科治疗等疗效不佳，治疗有效率低，复发率高，不良反应增多 。
1.药物治疗:卡马西平治疗，当疼痛停止后可考虑逐渐减量。不良反应可见头晕、嗜睡、口干、恶心、消化不良等，出现皮疹、共济失调、再生障碍性贫血、昏迷、肝功能受损、心绞痛、精神症状时需立即停药。如卡马西平无效可考虑改用苯妥英钠。上述两药无效时可试用氯硝西泮。不良反应有嗜睡和步态不稳，老年患者偶见短暂性精神错乱，停药后消失。可同时辅用大剂量维生素B12，肌内注射，部分患者可缓解疼痛。偶有一过性头晕、全身瘙痒、复视等不良反应。
2.封闭治疗:服药无效者可试行无水乙醇或甘油封闭三叉神经分支或半月神经节，破坏感觉神经细胞，可达止痛效果。不良反应为注射区面部感觉缺失。
3.经皮半月神经节射频电凝疗法:X线监视或CT导向下将射频针经皮刺入三叉神经节处，射频发生器加热使针头温度达65～75℃，维持1分钟。选择性破坏半月神经节后无髓鞘Aδ及C纤维（传导痛、温觉），保留有髓鞘Aα及β粗纤维（传导触觉），疗效达90%以上。适用于年老体衰有系统疾病、不能耐受手术者。约20%应用此疗法的患者出现面部感觉异常、角膜炎、咀嚼肌无力、复视、带状疱疹等并发症。
4.手术治疗:可选用三叉神经感觉根部分切断术，止痛效果确切。三叉神经显微血管减压术，止痛同时不产生感觉及运动障碍，是目前广泛应用的手术方法，但可出现听力减退、气栓及滑车、展、面神经暂时性麻痹等并发症。
发明内容
本发明的目的是提供一种治疗三叉神经痛的外用复合油及其制备方法。
本发明的治疗三叉神经痛的外用复合油；由以下重量份数的原料药制成：小驳骨6～9份、木藤蓼6～9份、豨莶草9～12份、功劳叶8～10份、一叶兰12～15份、羌活12～15份、木贼草8～10份、橄榄油40～45份、鸸鹋油25～30份。
该治疗三叉神经痛的外用复合油，由以下重量份数的原料药制成：小驳骨7份、木藤蓼7份、豨莶草11份、功劳叶9份、一叶兰13份、羌活15份、木贼草9份、橄榄油42份、鸸鹋油28份。
制备该治疗三叉神经痛外用复合油的方法，包括以下步骤：
A、取所述重量份数的小驳骨、木藤蓼、豨莶草、功劳叶、一叶兰，加8倍重量的95%乙醇回流提取4次，每次3小时，合并煎液，滤过，浓缩成1.20～1.35 (60°   C)的浸膏，真空烘干后，制成300目粉末；
B、取所述重量份数的羌活、木贼草制成300～400目粉末；
C、取所述重量份数的橄榄油、鸸鹋油加入搅拌机中搅拌均匀；
D、将步骤A中的粉末、步骤B中的粉末、步骤C中的混合物，加入到超声波搅拌机中，在60～65℃条件下，超声搅拌1～1.5小时，冷却即成。
本发明治疗三叉神经痛的外用复合油组方中，小驳骨祛风湿；散瘀血；续筋骨。主风湿痹痛；月经不调；产后腹痛；跌打肿痛；骨折。
木藤蓼清热解毒，调经止血，行气消积。
豨莶草祛风湿，通经络，清热解毒，降血压；用于风湿痹痛，肢体麻木；中风，手足不遂；痈肿疮毒，湿疹瘙痒；高血压。
功劳叶清虚热、益肝肾、祛风湿；主阴虚劳热；咳嗽咯血；头晕目眩；腰膝酸软；风湿痹痛；白癜风。 
一叶兰能活血散瘀，补虚止咳；有助于治跌打损伤，风湿筋骨痛，腰痛，肺虚咳嗽，咯血功效。
羌活散表寒，祛风湿，利关节，止痛，主外感风寒，头痛无汗，油印寒湿痹，风水浮肿，疮疡肿毒。
木贼草疏风散热，解肌，退翳。治目生云翳，迎风流泪，肠风下血，血痢，脱肝，疟疾，喉痛，痈肿。
橄榄油富含丰富的单不饱和脂肪酸，即油酸及亚油酸、亚麻酸，还有维生素A、维生素B、维生素D、维生素E、维生素K及抗氧化物等。
鸸鹋油有深层渗透能力、能愈合烧伤、切口、皮疹和其他皮肤疾病；鸸鹋油还能对肌肉疼痛有止痛作用；对由于体育活动造成的受伤、关节炎症有治疗作用。
本治疗三叉神经痛的外用复合油，基质油由橄榄油和鸸鹋油复合制成，具有良好的镇痛和渗透效果；含有的多种有效药物成分，祛风通络、散表止痛，营养丰富，用于治疗三叉神经痛效果突出，安全有效。
具体实施方式
下面给出的实施例拟对本发明作进一步说明，但不能理解为是对本发明保护范围的限制，本领域技术人员根据本发明内容对本发明的一些非本质的改进和调整，仍属于本发明的保护范围。
实施例1, A、取小驳骨7克、木藤蓼7克、豨莶草11克、功劳叶9克、一叶兰13克，加8倍重量的95%乙醇回流提取4次，每次3小时，合并煎液，滤过，浓缩成1.20～1.35 (60°C)的浸膏，真空烘干后，制成300目粉末；
B、取羌活15克、木贼草9克制成300～400目粉末；
C、取橄榄油42克、鸸鹋油28克加入搅拌机中搅拌均匀；
D、将步骤A中的粉末、步骤B中的粉末、步骤C中的混合物，加入到超声波搅拌机中，在60～65℃条件下，超声搅拌1～1.5小时，冷却即成。
实施例2, A、取小驳骨6克、木藤蓼6克、豨莶草9克、功劳叶8克、一叶兰12克，加8倍重量的95%乙醇回流提取4次，每次3小时，合并煎液，滤过，浓缩成1.20～1.35 (60°C)的浸膏，真空烘干后，制成300目粉末；

实施例3, A、取小驳骨9克、木藤蓼9克、豨莶草12克、功劳叶10克、一叶兰15克，加8倍重量的95%乙醇回流提取4次，每次3小时，合并煎液，滤过，浓缩成1.20～1.35 (60°C)的浸膏，真空烘干后，制成300目粉末；
B、取羌活15克、木贼草10克制成300～400目粉末；
C、取橄榄油45克、鸸鹋油30克加入搅拌机中搅拌均匀；
D、将步骤A中的粉末、步骤B中的粉末、步骤C中的混合物，加入到超声波搅拌机中，在60～65℃条件下，超声搅拌1～1.5小时，冷却即成。
有关临床资料如下：
一般资料：选取2013年5月至2014年5月我院收治的符合相关诊断标准的三叉神经痛患者196例为研究对象。患者年龄范围为26~67岁，平均年龄(50．18±4.61)岁。将所有患者随机均分为四组，每组49例患者。各组患者在性别、年龄、病程等方面差异无统计学意义(P>0.05)，具有可比性。
诊断及排除标准：参照1988年国际头痛协会(IHS)制订的三叉神经痛(原发性三又神经痛)的诊断标准确诊 。经必要相关辅助检查如血常规、CT或MRI等，排除继发性三叉神经痛，包括牙颅内肿瘤、牙痛、颞颌关节痛、带状疱疹后神经痛、丛集性头痛、慢性阵发性偏头痛、耳源性三叉神经痛、舌咽神经痛等，查体无阳性体征。
治疗方法：对照组，卡马西平片(上海信谊黄河制药有限公司，国药准字H31020306)0.1 g，每日3次口服，服用2周。服药期间忌食辛辣食品。治疗组共三组分别使用实施例1、2、3中复合油，每日4次，每次涂1~2克到率谷穴、风池穴、百会穴和四神聪穴；一个月后观察疗效。
疗效标准：治愈：疼痛完全消失，随访6个月内无复发；显效：疼痛完全消失，随访6个月内复发，但发作次数较前减少50％ 以上；有效：疼痛明显减轻，发作频次较前减少25％ ～50％ ；无效：未达到有效标准 。疗效结果见表1。
统计学分析：采用SPSS 19.0统计学软件处理，，计量资料用均数±标准差(x±s)表示，采用t检验；计数资料率的比较采用x2 检验。P<0.05为差异有统计学意义。

1.一种治疗三叉神经痛的外用复合油；其特征在于是由以下重量份数的原料药：小驳骨6～9份、木藤蓼6～9份、豨莶草9～12份、功劳叶8～10份、一叶兰12～15份、羌活12～15份、木贼草8～10份、橄榄油40～45份、鸸鹋油25～30份制成。
2.根据权利要求1所述的外用复合油，其特征在于是由以下重量份数的原料药：小驳骨7份、木藤蓼7份、豨莶草11份、功劳叶9份、一叶兰13份、羌活15份、木贼草9份、橄榄油42份、鸸鹋油28制成。
3.一种制备权利要求1～2任一项所述外用复合油的方法，其特征在于包括以下步骤：
A、取所述重量份数的小驳骨、木藤蓼、豨莶草、功劳叶、一叶兰，加8倍重量的95%乙醇回流提取4次，每次3小时，合并煎液，滤过，浓缩成1.20～1.35 (60°C)的浸膏，真空烘干后，制成300目粉末；
B、取所述重量份数的羌活、木贼草制成300～400目粉末；
C、取所述重量份数的橄榄油、鸸鹋油加入搅拌机中搅拌均匀；
D、将步骤A中的粉末、步骤B中的粉末、步骤C中的混合物，加入到超声波搅拌机中，在60～65℃条件下，超声搅拌1～1.5小时，冷却即成。
