背景技术
与胃肠道(GI)和肝胆道以及与胃肠道有关的器官/组织有关的紊乱包括诸如胃癌、食道癌、肝癌、和胰腺癌等癌症。特别地,胰癌(例如,胰腺癌)是胰腺的一种恶性增长,其主要出现在胰管的细胞内。该疾病是第九种最常见的癌症形式,然而在男性与女性癌症死亡的主要原因中,分别位居第四与第五。胰腺癌几乎总是致命的,其中5年存活率低于3%。

获得GLF的方法

表征靶分子的方法
本文中提供的所述方法和组合物的一些实施方式包括表征GLF或粪便样品中的靶分子。表征靶分子可包括,例如,鉴定靶分子、检测靶分子和/或定量靶分子。鉴定、检测和定量靶分子的方法是现有技术众所周知的。

荧光检测也可在本文提供的某些方法中用于检测靶分子。有用的荧色物包括DAPI、荧光黄、Hoechst33258、R-藻青蛋白、B-藻红蛋白、R-藻红蛋白、若丹明、德克萨斯红和丽丝胺。荧光黄或若丹明标记的抗体,或荧光黄或若丹明标记的二级抗体可用于本发明中。
放射免疫测定法(RIA)也可用于本文提供的某些方法中。此类测定法是现有技术众所周知的。放射免疫测定法可例如与125I-标记的一级或二级抗体(Harlow and Lane,supra,1988)一起使用。
可例如使用检测来自发色底物的颜色的分光光度计;检测辐射的辐射计数器如用于检测125I的γ计数器;或在一定波长的光存在下检测荧光的荧光计来分析来自可检测试剂的信号。当使用酶联测定法时,可按照制造商的说明书使用分光光度计如EMAXMicroplate Reader(Molecular Devices;Menlo Park,Calif.)来进行靶分子数量的定量分析。如果期望,本发明的测定法可以是自动的或通过机器进行的,而且可同时检测来自多个样品的信号。

在一些实施方式中,可按照如下步骤鉴定生物标记:测量来自至少一个具有病症或生理状态的测试受试者的测试GLF或测试粪便样品和来自至少一个不具有所述病症或生理状态的对照受试者的对照GLF或对照粪便样品中至少一种靶分子的水平;比较所述测试GLF或测试粪便样品中的所述至少一种靶分子的水平与所述对照GLF或对照粪便样品中的所述至少一种靶分子的水平,其中所述至少一种靶分子水平的显著差异鉴定生物标记。一些实施方式包括测量并比较来自多个具有病症或生理状态的测试受试者的测试GLF或测试粪便样品和来自多个不具有病症或生理状态的对照受试者的对照GLF或对照粪便样品中的多种靶分子。在一些实施方式中,可测量并比较至少5、10、15、20、25、30、35、40、45、50、55、60、65、70、75、80、85、90、95或100种靶分子。在一些实施方式中,GLF或粪便样品可从至少5、10、15、20、25、30、35、40、45、50、55、60、65、70、75、80、85、90、95或100个测试受试者中获得。在一些实施方式中,GLF或粪便样品可从至少5、10、15、20、25、30、35、40、45、50、55、60、65、70、75、80、85、90、95或100个对照受试者中获得。在一些实施方式中,与对照GLF或对照粪便样品相比,测试GLF或测试粪便样品中靶分子水平的显著差异可以是统计学上显著的。
试剂盒
本文提供的所述方法和组合物的一些实施方式涉及用于检测GLF或粪便样品中的靶分子、确定GLF或粪便样品中是否存在靶分子、定量GLF或粪便样品中的靶分子,或鉴定GLF或粪便样品中的靶分子的试剂盒。一些此类试剂盒可包括用于向受试者口服施用的灌洗组合物。在一些实施方式中,所述灌洗液可包括成分如聚乙二醇、硫酸镁、硫酸钠、硫酸钾、柠檬酸镁和比沙可啶。在一些实施方式中,所述灌洗液可包括聚乙二醇和电解质溶液,任选还包括比沙可啶或抗坏血酸(例如,GOLYTELY、HALFLYTELY、NULYTELY、MOVIPREP)。在一些实施方式中,所述灌洗液可包括磷酸盐(例如FLEET'S PHOSPHO-SODA)。在一些实施方式中,所述灌洗液可包括硫酸盐如硫酸钠、硫酸镁或硫酸钾(例如,SUPREP)。在一些实施方式中,所述灌洗液可包括柠檬酸镁。在一些实施方式中,所述灌洗液可包括匹可硫酸钠。

疾病
本文提供的所述方法和组合物的一些实施方式涉及特定疾病的诊断、预后。一些实施方式包括与胃肠道以及与其相关器官有关的疾病和紊乱。示例性疾病包括胃肠道和与其相关器官的癌症,例如胃癌、肝癌、胰癌。疾病的更多实例包括胰腺炎、胰腺癌、胃肠神经内分泌肿瘤、胃腺癌、结肠腺癌、肝细胞癌、胆管癌、胆囊腺癌、溃疡性结肠炎和克罗恩氏病。一些疾病涉及炎症性肠病(IBD)。如本文所使用,术语"炎症性肠病"可以指以至少部分胃肠道炎症为特征的一大类疾病。IBD症状可包括肠炎症并导致腹绞痛,以及持续性腹泻。炎症性肠疾病包括溃疡性结肠炎(UC)、克罗恩氏病(CD)、未定型结肠炎、慢性结肠炎、不连续或斑块状疾病、回肠炎症、肠外炎症、响应破裂腺管(ruptured crypts)的肉芽肿性炎症、口疮性溃疡、透壁性炎症、显微镜结肠炎、憩室炎和改道性结肠炎。疾病的更多实例包括口炎性腹泻、吸收不良紊乱和消化道、肝、胰腺和胆管树的其它病症。

更多实施方式包括通过评价多个生物标记来确定受试者的生理状态的方法。一些此类方法包括确定多个生物标记的存在、不存在和/或水平。多个生物标记的存在、不存在和/或水平可与受试者的生理状态的可能性(如受试者发展疾病的可能性和/或受试者对治疗特定疾病的治疗方案的可能响应)相关联。在一些此类方法中,受试者的"临床风险评分"可通过关联多个生物标记的存在、不存在和/或水平以确定受试者患有疾病或将发展疾病的可能性来测定(参见,例如,Soonmyung P.等,(2004)New Eng.J.ofMedicine351:2817-2826;和Cho C.S.等,(2008)J.Am.Coll.Surg.206:281-291,以其整体通过引用引入本文)。

实施例
实施例1-基于硫酸盐的GLF的蛋白质组分析
在该分析中,评价基于硫酸盐的GLF支持蛋白质组分析的能力。为了鉴定使用基于硫酸盐的灌洗组合物获得的GLF中的靶分子,向三位人类受试者施用SUPREP,并通过质谱法分析所得到的GLF中的蛋白质。在该实施例中,作为结肠镜检查程序的一部分,从受试者收集所述GLF。
在收集时,添加完整的蛋白酶抑制剂片剂(ROCHE),并在4℃下,在1000rpm下旋转样品30分钟。在14,000x g下再旋转上清液30分钟以沉淀细菌和碎片。用6倍体积的丙酮使1.8ml上清液沉淀,再用相等体积的氯仿萃取,接着在C-2反相SPE柱(Sep-Pak,Waters)上分离。用3倍柱体积的0.1%三氟乙酸(TFA)、0.1%TFA中的10%、20%和30%乙腈(ACN)洗涤所述柱,并用3倍柱体积的0.1%TFA中的60%ACN洗脱。通过离心冷冻干燥法干燥样品,重悬于100μl的50mM碳酸氢铵/10mM三(2-羧乙基)膦中,并用2μl的10mM测序级胰蛋白酶(Promega,Madison,WI)消化。
使用来自LC系统的输入端,在LTQ-Orbitrap质谱仪上采集数据。A溶剂含有在水中的3%B和0.2%甲酸。B溶剂含有在乙腈中的3%A和0.2%甲酸。溶剂是来自Fisher的HPLC级。对于120分钟的运行,起始溶剂是5%B,保留7分钟。在13分钟时,将梯度改为10%,83分钟时改为40%,103分钟时改为90%,然后在111分钟时,从90%降低至5%。然后再平衡,以进行下一次注射。为了测定的可重复性,对各样品进行三次注射。
在每秒400m/z至2000m/z的质量范围内扫描所述MS(Orbitrap),同时LTQ(Trap)采集至多5个并联的MSMS(肽序列)谱。使用标准Thermo Xcalibur软件采集数据。MS数据(Orbitrap)对2-3ppm是稳定的,本底离子用于质量漂移评价。MSMS数据(LTQ)测量为约0.6Da,而从低ppm Orbitrap数据获得母质量。使用一式三份注射,从C18LC柱洗脱肽,以确保所述数据的可靠性和可重复性。由来自各灌洗制剂(患者样品)的一式三份注射创建检索文档,并使用Xcalibur和Mascot软件包的组合,将其转化成MGF(Mascot Generic Format)文档。
使用Mascot搜索引擎(Matrix Science,UK)对RefSeq数据库(http://www.ncbi.nlm.nih.gov/RefSeq/)进行数据库检索,其中将分类指定为人类(智人),母离子(MS)的质量精确性为10ppm和碎片离子(MS/MS)为0.6Da,并且选择"没有酶"。由于样品中存在可修饰或截断正在检查的肽的消化酶,因而进行没有酶特异性的检索。由于这些抗体序列不是所述标准RefSeq列表的一部分,因而通过添加包括在SwissProt蛋白质数据库中的抗体序列来补充所述RefSeq数据库。
Mascot评分越高表示蛋白质匹配越好,并可与相对的蛋白水平相关联。如由基于没有酶特异性的该数据库的检索的Mascot评分体系所确定的,">40"的评分阈值表示<0.05的p值显著性;40的评分与p<0.01是一致的。使用标准Mascot评分,即使在多次MS/MS期间取样,对于各检测的肽,凭此也只加上最高评分。对于所包括的全部数据,在每个蛋白质系至少一个样品中,所有的评分>40。对于额外的置信度,还记录显著肽的数目,并选择至少2个肽的最小标准。很少具有小于3个肽。所计数的所有显著肽表示来自它们各自蛋白质的不同序列(个别肽)。以x/y形式记录显著的肽的评分和数目,其中x是评分,y是显著肽的数目。如果在特定样品中未检测到蛋白质,则将其列为"ND"。蛋白质记录为蛋白质名称,并提供由NCBI的蛋白质数据库规定的"gi"编号。整个本申请中列出的NCBI数据库中的每个"gi"编号中含有的序列通过引用引入本文。当以其前体蛋白或其它非成熟形式命名蛋白质时,同样隐含所述蛋白质的成熟形式,包括如去除信号序列和添加翻译后修饰的改变。在所有情况下,所述蛋白质根据其基因衍生的序列命名,以提供一致性。
表1列出了在来自定义为患者3、4和6的三位单独患者的GLF中鉴定的最丰富的蛋白质的实例,以上述形式呈现。如从表1中可见,可从GLF鉴定许多蛋白质,并且许多这些蛋白质可能与胰腺有关。其它蛋白质包括可能与结肠癌及其它癌症有关的DMBT1(gi#148539840)。还鉴定了抗体和假定的糖基化蛋白质。

在另一实验中,根据制造商的指导向受试者施用SUPREP,并在即将结肠镜检查之前,由所述受试者自己将所得到的GLF收集到放置在盥洗室的收集容器中。如上所述通过MS分析所述GLF的蛋白质组。将表示所存在的最丰富种类的Mascot评分的结果汇总在表2中。所述结果表明一些尿污染。在结肠镜检查期间随后收集的样品中观测到类似的蛋白质组谱。表2显示在由受试者收集的GLF中鉴定了许多不同的蛋白质。鉴定的蛋白质包括DMBT1、胰蛋白质和抗体,与表1中的数据一致。

实施例2-基于聚乙二醇的GLF的蛋白质组分析

实施例4-在组合使用GLF和SSL-7富集获得的样品中检测IgA-1
和IgA-2抗体
在该分析中,评价组合使用GLF和SSL-7富集获得的样品检测IgA-1和IgA-2抗体的能力。为了鉴定对金黄色葡萄球菌超抗原样蛋白7(SSL-7)具有亲和性的GLF中的靶分子,向人类受试者施用基于硫酸盐的灌洗组合物(SUPREP),并使用SSL-7亲和珠(affinity beads)在每个GLF中富集蛋白质。作为结肠镜检查程序的一部分,从受试者收集所述GLF。
具体地说,使用SSL-7亲和性珠来分离IgA-1和IgA-2。将20μl的SSL-7琼脂糖(Invitrogen,圣地亚哥,CA)添加至1ml样品,并在4℃的辊上培养过夜。将管在1,000xg下旋转2分钟,以使珠沉淀,并弃去上清液。用1X磷酸盐缓冲盐水洗涤珠4次,再在600rpm和37℃下,在振荡器中,用pH为2.7的20μl的100mM甘氨酸洗脱1小时。用60μl的100mM碳酸氢铵/10mM三(2-羧乙基)膦稀释洗脱的抗体,再用2μl测序级胰蛋白酶(Promega,Madison,WI)消化。如上所述进行质谱分析和数据库检索。将存在于对SSL-7具有亲和性的GLF中的最丰富的鉴定蛋白质和它们的相应Mascot评分汇总在表7中。如在之前实施例中所观测的,抗体存在于所述GLF中,且可以使用亲和性试剂进行这些的富集和分析,从而能够具体分析所述GLF中的该亚蛋白质组。最丰富的抗体是IgA。一致报道IgA存在于肠道中。

和IgM
在该分析中,评价组合使用GLF与蛋白质L富集获得的样品检测IgA和IgM抗体的能力。为了鉴定对蛋白质L具有亲和性的GLF中的靶分子,向人类受试者施用基于硫酸盐的灌洗组合物(SUPREP),并使用蛋白质L亲和珠在每个GLF中富集蛋白质。作为结肠镜检查程序的一部分,从受试者收集所述GLF。
使用蛋白质L亲和珠分离含有κ轻链的抗体。将20μl蛋白质L琼脂糖(Santa Cruz Biotechnology,Santa Cruz,CA)添加至1ml样品,并在4℃的辊上培养过夜。将管在1,000x g下旋转2分钟,以使珠沉淀,并弃去上清液。用1X磷酸盐缓冲盐水洗涤珠4次,再在600rpm和37℃下,在振荡器中,用pH为2.7的20μl的100mM甘氨酸洗脱1小时。用60μl的100mM碳酸氢铵/10mM三(2-羧乙基)膦稀释洗脱的抗体,再用2μl测序级胰蛋白酶(Promega,Madison,WI)消化。将存在于对蛋白质L具有亲和性的GLF中的最丰富的鉴定蛋白质和它们的相应Mascot评分以及显著肽的数目汇总在表8中。正如所料,再次检测到IgA和来自抗体的相关链。由于蛋白质L对于IgA抗体不是完全特异性的,因而还检测到IgM抗体(gi#193806374)。

实施例6–在使用GLF获得的样品中检测细菌来源的蛋白质
在该分析中,评价GLF促进细菌来源的蛋白质检测的能力。为了鉴定GLF中与细菌相关的靶分子,向两位人类受试者施用基于硫酸盐的灌洗组合物(SUPREP);作为结肠镜检查程序的一部分,从所述受试者收集所得到的GLF。用来自每种GLF的100μl接种Super Optimal Broth(SOB)培养基,并在37℃和220rpm振摇下培养过夜。在珠磨器(bead-beater)中,在8M脲中溶解沉淀,将溶解产物稀释在50mM碳酸氢铵/10mM三(2-羧乙基)膦的2M脲中,并用测序级胰蛋白酶(Promega,Madison,WI)消化。如之前描述的,在使用120分钟运行的Orbitrap MS系统上采集数据。产生MGF检索文档,并用Mascot搜索引擎(Matrix Science,UK)对照RefSeq数据库(http://www.ncbi.nlm.nih.gov/RefSeq/)进行检索,其中将分类指定为真细菌,母离子(MS)的质量精确性为10ppm,碎片离子(MS/MS)为0.6Da。将存在于GLF中的与细菌相关的最丰富的鉴定蛋白质和它们的相应Mascot评分以及显著肽的数目汇总在表9中。在所示样品中,培养的细菌是大肠杆菌。其它样品显示不同细菌,表明所述灌洗液还残留一些肠道细菌。

实施例7-来自几个受试者的组合样品的蛋白质组分析
为了进一步促进使用GLF获得的样品中大量可检测的目标蛋白质的鉴定,将由分别从12位受试者采集的数据形成的检索文档连接成单个检索文档,并使用之前对Orbitrap数据规定的参数进行检索。在各个GLF样品中分析许多蛋白质,并选择(主要)具有至少3个用p<0.05(Mascot评分约41)的阈值检测的独特显著肽的蛋白质。只有3种列出的蛋白质(gi's5031863、6466801和115430223)具有小于3个的显著肽,但这些具有约400的Mascot评分,大大超过蛋白质鉴定的95%置信水平。将在该联合分析中鉴定的蛋白质与所记录的特定蛋白质来源和所记录的相关癌症一起列在表10中。对于各独特的鉴定蛋白质,表10还列出了具有40或以上Mascot评分的鉴定的肽的SEQ ID NO.。已报告许多鉴定的蛋白质存在于胰液中。本申请中提供了表10中列出的参考文献。

实施例8-粪便样品的蛋白质组分析
在该分析中,评价粪便样品支持蛋白质组分析的能力。为了鉴定粪便样品中靶分子,不向人类受试者施用灌洗组合物。在正常排便过程中,使用放置在盥洗室中的收集容器收集来自所述受试者的粪便样品。在0.1%TFA中均匀化少量所述粪便(大便)样品,然后在13000x g下离心。用6倍体积的丙酮沉淀所述蛋白质,重悬于0.1%TFA中,用等体积的氯仿萃取,然后在如实施例1所述的SPE柱中处理。将最丰富的鉴定蛋白质和它们的相应Mascot评分以及显著肽的数目汇总在表11中。在所述样品中检测到主要可能来自胰腺的蛋白质,这表明,在GLF中发现后,粪便是检测这些生物标记蛋白质的来源。然而,所述样品还含有许多其它非人类的材料,其使得所述分析更加受限制,特别是对于生物标记的发现。

在该分析中,评价在使用GLF的样品中获得的样品中检测聚糖的能力。为了鉴定和分析GLF中包括聚糖的靶分子,从人类受试者收集GLF。将1.8mL的GLF添加至12mL冰冷却的丙酮,并培养1小时,以使所述蛋白质沉淀。在12,000x g下离心所述样品15分钟,并除去丙酮。在用冰冷却的丙酮洗涤后,将沉淀重悬于0.1%TFA中,再通过5mL注射器类型的SepPak C2柱。用60%乙腈/40%0.1%TFA洗脱蛋白质。在真空下除去所述溶剂后,将所述蛋白质级分再溶解在100μL的50mM碳酸氢铵中,并在振荡器中,在37℃下,用2μL PNGaseF去糖基化过夜。在用1mL的0.1%TFA淬灭后,将从使用真空歧管的1mL注射器类型的SepPakC18柱的流出级分收集为聚糖。通过将25μL衍生化溶液(含有35mM ABEE和100mM2-PB的90:10的MeOH:HAc)添加至所述干燥的聚糖进行还原胺化,以用4-ABEE(4-氨基苯甲酸乙酯)标记所述干燥的聚糖,并在65℃下培养2小时。通过添加1mL乙醚、搅拌并弃去所述醚,除去过量的ABEE。在第二次乙醚萃取后,将所述样品短暂地放进SpeedVac以除去残余的醚。然后在HPLC上运行所述标记的聚糖,并用来自Agilent C8反相柱的20-25%乙腈洗脱。真空干燥该级分,在50μL的0.1%TFA中再溶解,并在WatersQ-TOF LC-ESI-MS上运行来进行聚糖分析。用0.2%甲酸中约20-25%乙腈从Q-TOF MS上的所述C18反相柱洗脱出衍生的聚糖。以MS专用模式从每秒m/z150-2000扫描质谱仪以获得所述衍生的聚糖的剖面数据。图1汇总了这些结果,并描绘了存在于一部分胃肠灌洗液中的各种糖蛋白衍生的聚糖结构的相对丰度的图。如图1所示,一些糖蛋白衍生的聚糖结构包括与链截断有关的特定修饰。由于已知细菌能够消化和消耗来自蛋白质的聚糖,因而这些修饰可能是由于GLF样品中存在的细菌活动。然而,此类修饰还可以与疾病、特别是癌症有关,其中异常的糖基化已与所述疾病关联。
前述分析表明可在使用GLF获得的样品中检测到大量聚糖。
实施例10-在使用GLF获得的样品中检测代谢产物
在该分析中,评价在使用GLF获得的样品中检测代谢产物的能力。为了鉴定和分析GLF中包括代谢产物的靶分子,向人类受试者施用基于柠檬酸镁的灌洗组合物,并分析所得到的GLF中的代谢产物如胆酸及其它胆盐。作为结肠镜检查程序的一部分,从所述受试者收集所得到的GLF。
在最大速度下,离心3ml GLF20分钟,并用0.1%TFA酸化上清液。将所述上清液施加至C18SPE柱(Waters Sep-Pak),用3倍体积的0.1%TFA洗涤,再用0.1%TFA中的50%ACN洗脱。通过离心冷冻干燥法干燥所述洗脱物,再溶于500μl的0.1%TFA中。
使用来自LC系统的输入端在Waters Q-TOF质谱仪上采集数据。所述A溶剂含有在水中的3%B和0.2%甲酸。所述B溶剂含有在乙腈中的3%A和0.2%甲酸。溶剂是来自Fisher的HPLC级。起始溶剂是5%B,保留5分钟,然后在25分钟时改为40%,在30分钟时改为90%,然后在36分钟时重置为5%。在每秒m/z100至m/z2000的质量范围内扫描所述MS。使用标准MassLynx软件采集数据。具有标记的胆酸峰的洗脱化合物汇总在图2中。在Orbitrap仪器上观测到类似的峰轮廓,其中使用标准物和MS/MS数据鉴定所述胆酸峰。鉴定包括胆酸的代谢产物。
前述分析表明可在使用GLF获得的样品中检测到代谢产物。

42.权利要求40-41中任一项所述的方法,其中所述靶分子选自由多肽、核酸、胆汁酸、抗体、代谢产物、聚糖及其组合组成的组。

48.权利要求40-47中任一项所述的方法,其中所述生物标记与疾病、对治疗的阳性响应、或对治疗的阴性响应有关。
49.权利要求40-48中任一项所述的方法,其中所述生物标记与癌症的存在或癌症倾向有关。

58.权利要求55-57任一项所述的试剂盒,其中所述靶分子包含多肽、抗体、胆汁酸、代谢产物或聚糖。

60.权利要求59所述的试剂盒,其中所述生物标记与疾病、对治疗的阳性响应、或对治疗的阴性响应有关。
