Abstract

利什曼原虫属寄生虫具有两种主要的形态形式:前鞭毛体和无鞭毛体。前鞭毛体形式为狭长的、有鞭毛的、活动的,具有单核和动基体(动基体位于前部与核之间),在媒介昆虫的消化道中繁殖。无鞭毛体形式为圆形形式,具有未发育的鞭毛、杆状动基体,且它们在哺乳动物宿主的吞噬细胞-单核细胞系统的细胞中繁殖(Grimaldi,G.Jr.&Tesh,R.B.Leishmaniasis of the New World:current concepts andimplications for future research.Clin.Microbiol.Res.,第6卷,第230-250页,1993)。
在不同物种的哺乳动物中啮齿类和犬类为寄生虫的天然贮库,且为传病媒介的感染源。犬可以被确定为内脏利什曼病(VL)的主要家庭宿主。狐狸和狼为该病的森林贮库,而有袋类动物和啮齿类可以为引起皮肤利什曼病(CL)的物种的贮库。雌性白蛉(双翅目:Psicodidae)为该病的传病媒介,其在美国属于罗蛉属(Lutzomyia),且在旧大陆国家属于白蛉属(Phlebotomus)(SACKS,D.&KAMHAWI,S.Molecularaspects of parasite-vector and vector-host interactions inleishmaniasis.Annu.Rev.Microbiol.,第55卷,第453-483页,2001)。

已显示了临床和流行病学重要性的另一个方面是HIV病毒和利什曼原虫属之间的协同感染。利什曼病可以改变HIV引起的疾病的进程,并促进在世界若干国家导致所述疾病发生的病毒所引起的免疫抑制。

Cunninghan(2002)报道了约10至25%的使用五价锑剂治疗的VL患者对治疗有抗性或出现复发。Sundar(2001)也报道了显著百分比的患有由杜氏利什曼原虫引起的内脏病的患者对的治疗有抗性(CUNNINGHAM,A.C.Parasitic adaptive mechanisms ininfection by Leishmania.Exp.Mol.Pathol.,第72卷,第131-141页,2002)。对于这些病例,可以使用戊烷脒(pentamidine)和两性霉素B,尽管这些药物具有高毒性和高成本(Grimaldi&Tesh,1993)。
在VL的家庭循环中犬是重要的贮库,且由于强的犬类患病率(与人感染相比)被认为是白蛉感染的主要来源。感染的犬,即使无症状,皮肤也具有大量寄生虫,使得昆虫传病媒介由该贮库感染,并因此传递至人类。应注意人传染对作为重要的传染来源的生物循环没有影响(Tesh,1995和WHO,2003)。与未治疗VL的致死率相关的该事实使得巴西的卫生部采取当犬对利什曼原虫属抗原血清反应阳性时将其消灭的传染控制措施。然而,通常用于在犬诊断该疾病的血清学方法、诸如IFA和ELISA可以具有不同的灵敏度和特异性,且因此真正的感染率可以是低估的。这使得受感染动物被保留,这是该疾病控制失败的一个原因(Tesh,1995)。
用于治疗犬VL的市售药物、诸如别嘌呤醇(alopurinol)、五价锑剂和两性霉素B作为控制该疾病的措施不可行,这是因为它们的高成本且治疗过的和临床治愈的犬经常复发,由此为传病媒介保留了感染源(Tesh,1995)。此外,在大量犬VL治疗中使用这些药物带来另一个担心,这可能增加了选择对已用于治疗人的那些药物有抗药性的菌株的风险(Reithinger等人,2002)。
根据上文所提及的多个事实,注意到需要进行集中于发现新的用于治疗人和犬对抗该疾病的替代疗法的新的研究。
因此,鉴别有以下特性的新化合物/产物/药物变得非常有吸引力:对患者有较低毒性,在经济上更可行(在现已使用的药物中未观测到这一现实性);且其给药途径改善以引起患者可能具有的最少的不适感。关于这点,提出使用真菌姬松茸(以在含所述产物的药物制剂中存在的其水提物和/或其纯化的级分的形式)最小化和/或解决所述问题。
当使用水提物时,目标是将其掺入制剂中,且对于该目标,第一步是将其放入与组分相容的制剂中,所述制剂保持其稳定性且促进其药理作用。此外,文献中有关于当经口服途径用于人类患者时所述真菌的非毒性的研究,所以证明在个体中没有副作用或毒性作用。因此所述真菌提取物及其级分的局部施用(用于CL)不能被认为是侵入性的,且当与通常用于锑剂的途径(在这种情况下,为肌内和静脉内途径)相比时,患者的不舒适会当然少得多。此外,固体和/或液体形式的药物制剂在VL的情况中还将被开发用于口服施用。
除所有这些优势之外,还有这一事实:该真菌在我们的植物群中很丰富,且目前ANVISA已发布用于以功能性食品形式商品化。

技术详述
本发明特征为与药学上可接受的赋形剂组合的姬松茸的水提物及其衍生的蛋白和非蛋白质级分在治疗犬和人的皮肤和内脏的利什曼病中的应用。更具体地,本发明公开了优选用于局部和口服应用的组合物,所述组合物形式为选自凝胶剂、霜剂、软膏剂、糊剂、通常情况下为乳剂、溶液剂、片剂和胶囊的固体、半固体和液体形式的药物制剂。
所述组合物可以经口、肌内、静脉内、腹膜内、皮下、透皮途径施用,或者作为可以被植入或注射的装置;但它们优选局部施用。
通过以下的非限制性实施例可以更好的理解本文提出的技术:
实施例1-寄生虫的培养
这里使用亚马逊利什曼原虫的IFLA/BR/1967/PH-8株、恰氏利什曼原虫的MHOM/BR/1970/BH46株和硕大利什曼原虫的MHOM/IL/1980/Friedlin株。将寄生虫在完全施耐德培养基中培养,所述培养基由补充有20%灭活的胎牛血清(Sigma)、20mM的L-谷氨酰胺、50μg/mL庆大霉素、200U/mL青霉素和100μg/mL链霉素的施耐德培养基(Sigma)组成,pH为7.4。将寄生虫在25°C保持在培养物中,以使得培养物继续培养以保持各株,并获得稳定生长期前鞭毛体形式。
实施例2-姬松茸的水提物的制备
为制备姬松茸的水提物,称取约1克的真菌,并在1mL的pH7.0的Tris-HCl10mM缓冲液中研磨。在4°C温孵1小时后,将提取物以9000rpm(SORVAL LC5C centrifuge)离心1小时30分钟,回收上清液,并通过Bradford法判断其浓度(Bradford,1976)。
实施例2.1--使用真菌水提物进行的生物测定
使用姬松茸的水提物的杀利什曼原虫活性试验
测定真菌姬松茸水提物对三种不同的利什曼原虫属物种的稳定期无鞭毛体-状形式的杀利什曼原虫活性。
在25°C使用25至200μg/mL的真菌水提物在96孔细胞培养皿(Nunc,)中、在最终体积100μL的RPMI-PR培养基中培养4x105的亚马逊利什曼原虫、硕大利什曼原虫和恰氏利什曼原虫的前鞭毛体和无鞭毛体-状形式达24小时。然后加入50uLMTT试剂(98%噻唑蓝溴化四唑;存储浓度为5mg/mL),并将培养皿在25°C培养4小时。在显微镜下分析细胞以证实甲(formazan)结晶的形成。不久之后加入60μL的SDS10%/HCL0.1M溶液以溶解甲结晶,并将培养皿温孵18小时。然后使用分光光度计在570纳米(nm)的波长进行吸光度读数(参见图1和2;表1和2)。
通过实验中收集的吸光度读数,制作表1和2以表示水提物针对以两种方式测试的三种利什曼原虫属物种的杀利什曼原虫活性的百分比。使用吸光度读数基于以下公式计算寄生虫死亡的百分比:

真菌提取物抑制寄生虫进入哺乳动物巨噬细胞中的功效
为证实真菌提取物抑制寄生虫进入哺乳动物巨噬细胞中的功效,将亚马逊利什曼原虫、恰氏利什曼原虫和硕大利什曼原虫的前鞭毛体和的无鞭毛体-状形式用真菌的水提物进行处理,然后以10:1的比例与来自BALB/c小鼠的的腹腔巨噬细胞一起温孵。将寄生虫(4x106)用200μg/mL的真菌的水提物处理,并在25°C温孵1小时,然后用其感染巨噬细胞(比例为10个利什曼原虫属寄生虫比1个巨噬细胞)(B)。作为对照,感染未处理的寄生虫(A)。数据表示具有相似结果的三个实验。
将感染的巨噬细胞定量,并使用显微镜测定每个感染的巨噬细胞的寄生虫数量。结果在图3(前鞭毛体)和图4(无鞭毛体-状)中显示。
随后测定被前鞭毛体和无鞭毛体-状形式感染的巨噬细胞的数量的百分比和与每个感染的巨噬细胞的寄生虫的数量之间的比例。数据在表4和5中显示。

使用亚马逊利什曼原虫、硕大利什曼原虫和恰氏利什曼原虫的稳定生长期前鞭毛体感染从BALB/c小鼠获得的腹腔巨噬细胞,比例为每个巨噬细胞10个寄生虫,并在25°C使用200μg/mL的真菌姬松茸的水提物处理48小时(A)。感染细胞并不接收治疗作为对照(B)。结果在图5中显示。
实施例2.1-从姬松茸的水提物获取蛋白质级分
为从真菌姬松茸的水提物制备并提取蛋白质级分,将约28克新鲜的和洁净的蘑菇(使用子实体)加入50mL补充有50μL蛋白酶抑制剂混合剂(SIGMA,CODE P8340)的milli-Q水中。在研钵和研棒的帮助下,将该物质在冰浴中匀化。随后将该内容物在4°C静置1小时,并经滤纸过滤以除去不溶的物质。

随后,通过Lowry法定量样品,并用于接下来的生物测定(表6)。
表6–从每个Amicon管的真菌姬松茸的水提物中获得蛋白质级分。通过梯度离心将该蛋白质级分分离,并通过Lowry法定量。
在10%的真菌姬松茸的水提物的纯化的级分中的聚丙烯酰胺凝胶电泳。
在Amicon管中纯化后,将新的级分在10%进行SDS-PAGE凝胶电泳以验证它们的蛋白质特性。如图6所示,其中显示了2种凝胶。
实施例2.2-:纯化的新的级分对亚马逊利什曼原虫、恰氏利什曼原虫和硕大利什曼原虫的稳定生长期前鞭毛体的杀利什曼原虫活性。
测试级分对亚马逊利什曼原虫、恰氏利什曼原虫和硕大利什曼原虫的稳定生长期前鞭毛体的杀利什曼原虫活性,结果在图7中显示。
通过细胞活力试验方案通过浸渍MTT试剂进行该实验,使用50μg/mL浓度的真菌的水提物和纯化的级分。使用50μg/mL浓度的两性霉素B作为对照(图7)。
通过在之前的实验中收集的吸光度读数,得到的表以表示真菌水提物及其纯化的蛋白质级分对三种利什曼原虫属物种的稳定生长期前鞭毛体形式的杀利什曼原虫活性的百分比。数据在表7中显示。
表7:使用真菌姬松茸的水提物及其纯化级分的亚马逊利什曼原虫、恰氏利什曼原虫和硕大利什曼原虫的稳定期前鞭毛体形式的平均的死亡百分比。使用两性霉素B作为该实验的对照。结果显示各实验组的标准平均值±偏差。显示的数据表示具有相似结果的三个实验组。
级分F4和F5的生物测定
由于注意到级分F4和F5对于不同的利什曼原虫属物种显示最佳的致死结果,所以选择它们继续进行实验。计算它们的使50%的寄生虫失活的有效浓度,并在表8中显示。

使用寄生虫感染腹腔巨噬细胞,比例为每个细胞10个寄生虫,然后,使用级分F4和F5进行处理。结果在图8中显示。为了研究在使用纯化的级分刺激后巨噬细胞杀利什曼原虫活性的机制是否通过一氧化氮(NO)的产生而发生,使用级分F3,F4和F5刺激感染的或未感染的巨噬细胞,并通过格里斯反应测定产生产生的NO。结果在图9中显示。
还分析了级分F4和F5在哺乳动物细胞中的细胞毒性,结果在图10中显示。为此目的,从BALB/c得到巨噬细胞,并在25°C使用级分F4和F5处理24小时。通过MTT试验评价细胞活力。
实施例3-从真菌姬松茸的水提物得到的级分F5的纯化
在明确使用级分F4和F5用于上文引用的实验之后,由于它们已显示出杀利什曼原虫活性的最佳指标,且对于哺乳动物巨噬细胞不显示任何显著的细胞毒性,所以我们决定选择级分25继续进行实验,该决定建立于纯化后其具有较高的最终收率的这一事实。
因此,在FPLC系统中进行级分F5的纯化,并得到新的最纯的级分。为该目的,将合并的级分F5使用离子交换MonoQ HR5/5柱(作为固定相)、以NaCl溶液作为流动相洗脱。使用NaCl浓度梯度(0至1M)处理样品,收集,渗析和冻干。
也将它们定量,并在SDS-PAGE系统中以10%进行蛋白电泳(图11)。
测试了从F5得到的新的级分对亚马逊利什曼原虫的稳定期前鞭毛体形式的杀利什曼原虫活性,结果在图12中显示。使用50μg/mL浓度的药物两性霉素B作为对照。通过细胞活力试验方案使用MTT试剂进行该实验,且使用浓度20μg/mL的级分。
在一些通过离子交换纯化的新的级分中可以观察到高的杀利什曼原虫活性,诸如级分F2、F3、F18、F19和F27。所述级分,即使浓度低于之前的级分F5,也显示了比级分F5更高的活性。还测试了通过离子交换纯化的级分的毒性(图13),且显示没有显著的细胞毒性。
实施例4-使用亚马逊利什曼原虫感染并使用姬松茸的水提物和级分F5治疗的BALB/c小鼠的体内治疗。
使用所述姬松茸的水提物和称为F5的级分进行对于亚马逊利什曼原虫感染的小鼠的治疗。
在此还测试了2种治疗:预防性治疗(称为化学预防)和常规治疗。
由于体外试验已证实水提物能够在巨噬细胞中降低寄生虫的侵入,对实验性化学预防模型进行详细描述以评价是否可能使用姬松茸的水提物或级分F5进行预治疗以在BALB/c小鼠中降低感染并预防或抑制疾病发展。
对于化学预防模型,将动物预先使用水提物或级分F5治疗5天,并使用5x105的亚马逊利什曼原虫的稳定期前鞭毛体形式在右足垫进行感染。感染后,通过管饲法经口使用每天2mg的单次剂量的水提物或级分F5将动物进一步治疗20天。
对于常规的治疗模型,对动物使用5x105的亚马逊利什曼原虫的稳定期前鞭毛体形式在右足垫进行感染,然后通过管饲法经口使用每天2mg的单次剂量的水提物或级分F5治疗20天。
作为对照,使用1mg/Kg/天的药物两性霉素B(脱氧胆酸盐)进行同样时间(20天)的治疗,作为感染对照,将一些动物感染,并接受盐水20天。
感染后,对受感染动物的爪进行每周测定以检测疾病的进程(图13)。感染10周后将动物处死,并收集一些器官诸如脾、病损和腿弯部淋巴结,并处理用于寄生虫学和免疫分析。
在图14的分析中,与接受盐水的感染动物的爪部肿胀相比,从使用真菌水提物的两种计划的治疗(化学预防和常规治疗)中均可观测到动物的爪部肿胀显著减少。在一些测定中还观察到感染动物的爪部肿胀甚至比两性霉素B治疗的动物的肿胀更少。
在图15的分析中,与接受盐水的感染动物的爪部肿胀相比(感染对照),观测到使用级分F5治疗的动物的爪部肿胀显著减少。所述减少甚至呈现了与使用两性霉素B治疗的动物感染的爪部所观测到的相似的值。与感染对照相比,化学预防法没有显示受感染动物的爪部肿胀的明显减少。
然而,应注意到化学预防进行中的动物没有显示器官毒性,在感染的动物和使用两性霉素B治疗的那些动物中观测到一个事实:在这些动物中肝的炎性酶类尤其显著增加。
在感染的第10周将动物处死用于寄生虫的定量分析,如图16和17中所示。
如图15中所观测到的,使用水提物的常规治疗和化学预防能显著减少在受感染动物的所评价的器官中(脾、腿弯部淋巴结和感染的爪)寄生虫的数量,所述减少大于使用两性霉素B治疗的动物所发现的值。
尽管与感染对照(使用盐水治疗的动物)相比,化学预防模型已显示了好的结果,但使用常规治疗得到最好的结果,所以在该情况下这些动物的脾中没有发现寄生虫,且在引流淋巴结和受感染的爪中仅有少量寄生虫,这表明使用姬松茸的水提物治疗提供了显著的保护。
尽管与对照组相比,受感染并使用级分F5进行化学预防的动物的爪部肿胀没有显著的不同的值,但与感染对照相比观测到在进行化学预防的动物中寄生虫数量显著减少(图16)。还注意在感染并使用级分F5治疗的动物中寄生虫的数量显著减少,所以也不可能在所述动物的脾中检出寄生虫。因此,使用级分F5进行化学预防和常规治疗在不同的被测定器官均显示了寄生虫的数量显著减少,所述减少甚至大于使用两性霉素B治疗的动物所观测到的相关的值。
因此,总之,我们证实了在感染亚马逊利什曼原虫的BALB/c小鼠中,真菌姬松茸的水提物和称为F5的级分具有重要的体内杀利什曼原虫活性。
应注意所述研究进行了两次,且结果相似。目前,在感染恰氏利什曼原虫的BALB/c小鼠中使用水提物和级分F5的实验正在进行中,且化学预防和受感染动物的常规治疗中的结果均是有希望的。
Claims (5)
1.液体、半液体或固体药物形式的杀利什曼原虫制剂,其特征为其包含蘑菇姬松茸的水提物和/或其具有相应的50至80kDa的蛋白质命名为F4和F5的纯化的级分和所述产物中存在的非蛋白质组分、诸如鞣酸类、皂甙和多糖类,以及至少一种药学上可接受的赋形剂。
2.根据权利要求1的杀利什曼原虫制剂,其特征为其优选用于选自凝胶剂、霜剂、软膏剂、糊剂、通常情况下为乳剂、溶液剂、片剂和胶囊的药物形式中。
3.根据权利要求1和2的杀利什曼原虫制剂,其特征为其通过口服和/或局部途径施用。
4.根据权利要求1至3的杀利什曼原虫制剂的应用,其特征为其用于制备药物,所述药物用于预防感染或治疗感染了利什曼原虫属的哺乳动物。
5.根据权利要求1至4的杀利什曼原虫制剂的应用,其特征为其应用于犬和人对抗利什曼病的预防或临床治疗中。
