总之,IL-2是一种具有高度多效性特性的细胞因子,其在不同细胞群体的生物学活性中是非常有重大意义的。该特性使得IL-2在免疫应答的调节中为一个重要的节点,这使其转变为用于免疫调节疗法的有吸引力的且复杂的靶标。
IL-2已经在癌症的治疗中使用了数年。特别地,其以高剂量的使用是在数个国家中被批准用于治疗转移性黑素瘤和肾癌的疗法。然而,IL-2在患者中的直接使用严重受限于它的毒性效应和低效力。以致,仅20%的合格患者接受该治疗,而且仅17%的经治疗的患者显示出重大的客观应答。对于该在临床中的引人注目的失败的可能解释是,用天然IL-2进行的治疗还刺激调节T细胞群体(Ahmadzadeh,M.等人,(2006)Blood.107,2409-14),其相反地作用于用它所追求的免疫刺激。现在,许多临床前证据支持了该想法。特别地,在鼠类模型中的实验显示,体内注射的IL-2的首要活性是天然调节T细胞的内环境稳定性扩增。
已开发了数种策略,目的是减轻用IL-2进行的治疗的毒性效应。这些策略中的一些基于使用IL-2的突变型变体,所述突变型变体被设计用于增加该分子主要通过高亲和力受体(α、β和γ链)而不是通过中等亲和力受体(β和γ链)来进行信号传导的能力。基本的想法是促进T细胞中的信号传导而非NK细胞中的信号传导,NK细胞是被认为对于所观察到的毒性效应负有责任的细胞。在该系列的工作中,包括下列发明:美国专利7,186,804、美国专利7,105,653、美国专利6,955,807、美国专利号5,229,109、美国专利申请20050142106。无论如何,重要的是注意,这些发明中没有一个涉及在体内具有比天然IL-2更大的治疗效力的IL-2的突变蛋白质,基于其降低的刺激天然调节T细胞的能力。
已制造了其他的IL-2的突变型变体,目的是增加其药理学活性。例如,改善其折叠或增加其在血液中的生存期。下列发明尤其在该系列的工作之中:美国专利号4,959,314、美国专利号5,116,943、美国专利号4,853,332。再次地,这些突变蛋白质中没有一个具有降低的激活调节T细胞的能力,也没有显示出更大的治疗效力。

发明简述
本发明涉及IL-2的突变型变体的获得,所述突变型变体在鼠类可移植肿瘤模型中显示出比天然IL-2更大的治疗效力。这些突变蛋白质的特征在于:其是IL-2的活性的不全激动剂,并且因其特别低的在体外和/或在体内刺激天然调节T细胞(CD4+CD25+FoxP3+T)的能力而被选择出来。这些突变蛋白质的体内治疗效力提出了用于改善在恶性肿瘤中用IL-2进行的治疗的实际解决方案。特别地,这些突变蛋白质使得能够克服在用天然IL-2进行的治疗中观察到的限制,这些限制源自其经证实的在体内扩增天然调节T细胞的能力。
本发明涉及多肽,其具有与人IL-2相同的一级序列,除了几个氨基酸被突变之外。所引入的突变实质上降低了这些多肽在体外和在体内刺激调节T细胞(CD4+CD25+FoxP3+T)的能力并且赋予其在鼠类可移植肿瘤的治疗中更大的效力。本发明还包括这些突变型变体(单独地或与疫苗相组合地)用于治疗疾病例如癌症或其中调节T细胞(Tregs)的活性是有关联的感染的治疗性应用。
本发明允许目前的基于IL-2的免疫调节策略的实质性改善,在癌症的直接治疗中和在其与不同疫苗的组合中。特别地,用在本发明中所描述的突变型变体代替天然IL-2使得能够避免调节T细胞的扩增,这些调节T细胞显著地减小所希望的治疗效果。
发明详述
IL-2的类似多肽的获得
本发明涉及大小为100至500个氨基酸,优选地140个氨基酸的多肽,其表观分子量为至少15kD。这些多肽保持高的与天然IL-2的序列同一性,大于90%的同一性,在其序列的一个区域中包含3至6个突变(相比于天然IL-2而言)。在所述位置处,通过引入与在天然IL-2中的相同位置处存在的氨基酸残基不同的氨基酸残基来使这些多肽突变。就具有与原始氨基酸非常不同的物理化学特性来选择置换原始残基的残基,尤其是极性的残基改变为非极性的残基,带电荷的残基改变为不带电荷的残基,大的残基改变为小的残基,酸性的残基改变为碱性的残基。
可以将本发明的多肽不加区别地称为,尤其是免疫调节多肽、IL-2的类似物或IL-2的突变蛋白质。这些多肽从IL-2-受体复合物的3D结构(在公共数据库PDB中可得)开始来进行设计,通过优选地在IL-2的相应于明显暴露于溶剂的氨基酸的和在不同物种的IL-2(从数据库Swissprot中获得的序列)中高度保守的位置处引入突变。通过使用用于蛋白质结构的可视化的生物信息学程序,例如RASMOL、SwissPDBviewer等,来鉴定上面所提及的该类型的暴露于溶剂的氨基酸。通过使用用于序列的多重比对的生物信息学程序,例如Fasta、ClusterW等,来鉴定在IL-2的序列中的保守位置。
本发明的多肽可以通过不同途径来获得,尤其是通过蛋白质的合成。还可以通过基因工程技术来获得,例如通过在细菌(例如尤其是大肠杆菌(E.coli))中、在哺乳动物细胞(例如尤其是NSO细胞)中表达它们。还可以借助于聚合酶链反应,通过定向诱变技术来获得在特定位置处的点突变。
令人惊讶地,发明人发现了这些突变蛋白质相比于传统的天然IL-2的使用而言的实质性优点。该优点在于提高了其在抗肿瘤治疗中的效力,这源自其避免调节T细胞扩增的能力。
就其生物学活性来选择IL-2的类似多肽
通过下列特性来选择本发明的多肽:
1)天然IL-2的激动剂作用。可以在使用IL-2依赖性细胞系(例如CTLL2或Kitt225)的体外增殖试验中,或者用使用小鼠和/或人的T淋巴细胞的混合物的试验来直接评价该特性。在这些试验中,这些突变蛋白质应当具有以5至50倍的程度比天然IL-2低的比刺激活性。
2)相比于天然IL-2而言,在体外和/或在体内刺激调节T细胞群体的能力的丧失。可以例如通过研究本发明的突变蛋白质相比于天然IL-2而言直接诱导CD4+CD25+T细胞(其从未接触过抗原的小鼠中纯化,并且在体外用抗-CD3抗体刺激)扩增的能力来评价该特性。还可以评价在调节T细胞群体(CD4+CD25+FoxP3+T)的增殖率的扩大或增加方面,在五天期间在小鼠中腹膜内或皮下注射这些突变蛋白质或天然IL-2的效应。在这些试验中,突变型IL-2对于调节T细胞的活性应当以至少1000倍的程度比天然IL-2低。
3)在动物模型中相比于天然IL-2而言增加的治疗效果。可以例如通过比较突变蛋白质与天然IL-2独自地在可移植肿瘤模型(例如B16黑素瘤)中的抗肿瘤或抗转移效应来评价该特性。还可以通过增强对于目的疫苗的细胞和/或体液应答的效应来进行评价。在包含相同的IL-2和突变蛋白质的总蛋白质量的剂量下,这些突变蛋白质应当显示出比天然IL-2更大的治疗效力。
本发明特别地涉及在表1中详细描述的突变蛋白质。这些突变蛋白质包含多个氨基酸置换,其整体地赋予它们以上面所提及的特性。表1:具有在本专利中所描述的三个基本特性的构建出的突变蛋白质。

本发明还包括除了上面所提及的,尤其是在表1中所描述的IL-2突变蛋白质类别之外的额外修饰。要么为了增加其与IL-2受体的特定组分的亲和力,而不影响甚至改善其不刺激调节T细胞的激动剂特征;要么为了改善其体内药效动力学:增加生存期或减少T细胞对其的内化。这些额外的突变可以经由使用生物信息学工具的推理性设计,或者通过使用不同性质的组合分子文库(噬菌体文库、在酵母中的基因表达文库或在细菌中的基因表达文库)来获得。
另一方面,本发明涉及融合蛋白,其包含与运载蛋白相偶联的上面所描述的任一种免疫调节多肽。所述运载蛋白可以为白蛋白或人免疫球蛋白的Fc区。
IL-2的类似多肽的治疗性应用
本发明还包括药物组合物,其包含本发明所公开的IL-2的突变蛋白质及其类似物作为活性成分;以及其可能的治疗性应用,目的是增强在疾病例如癌症或其中调节T细胞是特别有关联的慢性感染中天然的或由疫苗诱导的免疫应答。
对于其治疗性用途,应当向携带疾病的受试者独立地或者与其他多肽或与其他物质(它们有助于或增强其治疗作用)相联合地施用本发明的多肽。施用途径可以是在现有技术中被描述用于药物的肠胃外施用的施用途径中的任一种。优选地,可以通过静脉内途径、肌内途径、皮下途径或肿瘤内途径进行施用。
还可以通过代替天然IL-2而作为在癌症和慢性感染性疾病的治疗中有用的或者用于增强对于疫苗的细胞和/或体液应答的药物组合物的一部分来施用本发明中所描述的多肽。本发明的多肽可以与用于癌症的治疗性疫苗或者与在其中调节T细胞是有关联的感染性疾病中的预防性疫苗相组合地进行使用。
为了获得所希望的治疗效果,应当以足够高的剂量来施用本发明的多肽,该剂量足以保证其在对于所研究的疾病来说有重大意义的淋巴结或外周位点中的浓度,其在所述突变蛋白质显示出免疫刺激效应的浓度范围之内。因此,应当依据所研究的疾病类型和施用途径来调整所考虑的剂量。例如,在治疗肿瘤的情况下,应当调整剂量以达到保证刺激抗肿瘤免疫应答的在肿瘤内部和/或在局部区域淋巴结中的突变蛋白质的浓度。待探索的剂量范围可以从数百微克/剂至数百毫克/剂变化。对于其中所述突变蛋白质代替使用天然IL-2的传统疗法的那些应用来说,待使用的突变蛋白质剂量应当小于或者在活性(通过使用采用CTLL2细胞系的试验而测定的)方面等价于对于天然IL-2传统上使用的剂量。
待施行的施用次数也应当依照所考虑的突变蛋白质的生物分布来进行调整。通常,应当达到使上面所提及的有效浓度维持连续的2天至30天的一段时间。例如,应当注意,如果将突变蛋白质与运载蛋白相偶联,那么其施用频率应当相应地进行调整。对于其中代替天然IL-2的应用,突变蛋白质的施用方案可以与在传统疗法中所施行的相似。
“治疗作用”应当理解为疾病症状的完全或部分缓解。在癌症中,尤其地,肿瘤体积的减小或复发时间的增加是疾病缓解的标准。
本发明的多肽在肿瘤(例如尤其是黑素瘤和肾肿瘤)的治疗中是特别有用的。

实施例
实施例1.IL-2的突变蛋白质的设计
通过使用在数据库PDB(Protein Data Bank)中所报道的人IL-2的结构和在数据库Swissprot中可得的不同物种的IL-2的氨基酸序列作为基础,依据生物信息学技术,借助于计算机来设计突变蛋白质。设计了一些突变蛋白质,其在暴露于溶剂的和在进化中高度保守的残基处包含3至6个突变(引入非保守性氨基酸置换)。在大肠杆菌中,从在载体pET28a中的基因构建物之中表达出这些突变蛋白质,它们在氨基末端处包含具有6个组氨酸的识别序列。通过反相色谱法来纯化突变蛋白质(图1),以高纯度(>95%)获得。依据其在体外和体内实验检验中的特性来选择所获得的突变蛋白质,以显示在本发明的主体中所描述的3个基本特性。在所有构建出的突变蛋白质之中,在表1中描述了一整套特定突变,其具有所希望的特性,即为IL-2的活性的激动剂,而不明显地刺激调节T细胞,并且在鼠类可移植肿瘤的治疗中显示出比天然IL-2更大的治疗效力。表2指出了其他的构建出的突变蛋白质,但它们不显示所希望的特性。

实施例2.所设计的IL-2的突变蛋白质的激动剂特征的证明
图2举例说明了在表1中提及的突变蛋白质如何结合至CTLL2细胞系表面上的IL-2受体的组分(图2a)。构建出的突变蛋白质结合至CTLL2细胞,已知所述CTLL2细胞在其表面上具有对于IL-2的高亲和力受体和中等亲和力受体。结果是,在我们的试验中检测出的结合与用天然IL-2所获得的那种相似。图2b则图解说明了在表1中给出的突变蛋白质刺激CTLL2细胞系生长的能力(图2b)。结果是,在该试验中,这些突变蛋白质是IL-2的活性的不全激动剂。它们的比活性以5至50倍的程度小于天然IL-2。
实施例3.IL-2的突变蛋白质对于调节T细胞的效应
表1中所描述的突变蛋白质显示出非常低的在体外刺激调节T细胞的能力(图3)。正如在该图中所观察到的,天然IL-2却能够使调节T细胞(CD4+CD25+FoxP3+T)(其用粘附在培养平板底部上的抗-CD3抗体进行刺激)显著地增殖。以比天然IL-2显著地高的质量浓度的表1中所描述的突变蛋白质不刺激调节T细胞。应当加以注解的是,上面所描述的结果是正确有效的,即使增加待使用的突变蛋白质的量以致于采用能够在用CTLL2细胞系进行的增殖试验中显示出与天然IL-2等价的活性的那种量。通常,表1中所描述的突变蛋白质所展示出的刺激调节T细胞的能力以至少1000倍的程度小于天然IL-2。
实施例4.所设计的突变蛋白质的体内免疫刺激活性的表征
表1中所描述的突变蛋白质显示出增加的体内免疫刺激能力。图4a,b显示,在用两个20μg突变蛋白质的腹膜内日剂量进行治疗五天之后,突变蛋白质如何在首次用于实验的小鼠中诱导比天然IL-2大的脾肿大。该刺激与效应细胞群体(例如CD8+T淋巴细胞)的明显增加有关。作为有重大意义的观察结果,该用突变蛋白质进行的治疗不刺激调节T细胞(CD4+CD25+FoxP3+T)的扩增,这与对于天然IL-2所观察到的不同(图4c,d)。
实施例5.在可移植肿瘤的鼠类模型中突变蛋白质的治疗效力的测量
在可移植肿瘤的鼠类模型中证明了所设计的突变蛋白质的治疗效力的增加。表1中所描述的突变蛋白质显示出增加的对于治疗在MB16鼠类黑素瘤模型中诱导的肺转移的效力。图5显示了用两个20μg表1的突变蛋白质之一的腹膜内日剂量进行治疗5天,如何显示出强劲的抗转移效应,其在用相同剂量的天然IL-2以相同方式进行治疗的组中未观察到。
实施例6.突变蛋白质增强抗肿瘤疫苗的效应的能力的测量
证明了所设计的突变蛋白质增强疫苗的抗肿瘤效应的能力。使用了EG7细胞系的原发性肿瘤模型,EG7细胞系是经改造以表达抗原OVA的肿瘤细胞系。单独地或与突变蛋白质相组合地用佐以VSSP的抗原OVA对携带肿瘤的小鼠进行免疫接种。图6显示,肿瘤生长的降低在用疫苗与突变蛋白质的组合进行治疗的小鼠的组中比在单独地用疫苗进行治疗的组中大。
