技术领域
本发明涉及软组织工程化。更确切地,本发明涉及非细胞脂肪组织提取物、包含所述提取物的植入物、其制备方法及其用途。
背景技术
软组织包括体内的连接和支撑结构,诸如肌肉、结缔组织、脂肪组织和血管。软组织工程化目的在于为由例如外伤(烧伤和瘢痕)、手术切除或先天畸形引起的软组织缺陷制造替代部件。另外,美容用途,如填充面部皱纹,是再造软组织的重要应用。
在组织工程化中已经使用了异体材料,诸如硅和牛胶原蛋白。然而,这些材料可能导致严重的排斥反应和过敏反应。因此目前优选使用天然移植材料。
在软组织工程化中期望使用自体移植物。自体脂肪组织移植是一种老的治疗方法,其中将成熟的脂肪细胞或脂肪组织本身移植到缺陷位置。对于临床用途而言,脂肪组织丰富、易于收集并且容易获得。然而,自体脂肪组织移植物的使用可能仍然导致若干问题,诸如再吸收、过度的结缔组织(瘢痕)形成、炎症反应以及移植物的硬化和凝固。此外,根据所使用的方法和执行所述方法的人员的技能,通过脂肪组织移植获得的长期结果是不可预知的和多变的。
脂肪干细胞已经被用于软组织工程化。这些细胞能够增殖并分化为成熟的脂肪组织。然而,由于细胞外基质在细胞增殖和分化中具有重要作用,因此不太可能仅使用细胞来修复大的或深的软组织缺陷。为了再生为功能性组织,移植的细胞需要人造细胞外基质,即生物材料支架,以帮助细胞附着、增殖和分化。
新血管的形成,即新生血管形成,对再生软组织中的营养供给和废物清除是至关重要的。迄今为止,单一的生长因子(例如FGF-2或VEGF)已经为了这种目的被用作生物活性物质,但是结果并不令人满意。似乎需要不同生长/分化因子的群,而不仅仅是单一的因子。然而,分化因子的正确组合是未知的。在单个植入物中使用生物工程化生长因子的群会造成非常高的成本。
迄今为止,还没有软组织缺陷再造的合适方法。因此,在本领域中普遍需要开发植入物,其诱导快速的体积增加以填充缺陷、使移植组织保持不具有时间依赖性体积损失、并诱导快速新血管形成。
发明内容
新生脂肪生成是软组织工程化有希望的方法。本发明基于尝试提供适于细胞增殖和分化的微环境从而导致在没有细胞的外源移植的情况下形成脂肪组织的研究。最佳的微环境加强脂肪干细胞从周围组织的迁移并诱导细胞分化为成熟脂肪细胞。在发育的组织中需要新血管形成以避免移植组织的坏死、瘢痕形成和再吸收,以及从而分泌若干分化因子。
本发明提供一种元细胞脂肪组织提取物,其包含预定量的VEGF、FGF-2和IGF-1。在一些实施方案中,所述提取物包含每1mg总蛋白质至少1pg的VEGF、至少70pg的FGF-2和至少50pg的IGF-1。在其他实施方案中,每1mg总蛋白质的VEGF含量为至少7pg。所述提取物可以在软组织修复或工程化中使用以及在组织工程化中的血管生成诱导中使用,例如在伤口愈合中、在烧伤治疗中和在缺血性病症治疗中使用。
本发明还提供一种植入物,其包含本发明任一实施方案的无细胞脂肪组织提取物和生物相容基质。所述提取物优选是外源的,而所述基质优选是水凝胶并可以选自透明质酸,壳聚糖,血纤蛋白,胶原蛋白,藻酸盐,基于聚乳酸、乳酸羟基乙酸共聚物、聚己内酯的聚酯,及其混合物。在一些实施方案中,所述植入物是可注射的。所述植入物可以在软组织修复或工程化中使用,以及在组织工程化中的血管生成诱导中使用,例如在伤口愈合中、在烧伤治疗中和在缺血性病症治疗中使用。
本发明还进一步提供一种制备本发明任一实施方案的无细胞脂肪组织提取物的方法。所述方法包括以下步骤:a)提供包括不同细胞的非均匀化的脂肪组织样品;b)将所述样品培养预定的时间;以及c)收集提取物。还提供根据所述方法获得的无细胞脂肪组织提取物。
此外,本发明提供一种制备本发明任一实施方案的植入物的方法。所述方法包括将所述脂肪组织提取物与生物相容材料混合或将所述脂肪组织提取物吸收到生物相容材料中。
另外,本发明提供诱导脂肪生成的方法和诱导血管形成的方法。所述方法包括给有需要的受试者植入本发明任一合适实施方案的植入物。

图3示出血管生成实验中的管形成。图3A代表阴性对照,其显示没有管形成。图3B和图3C分别代表用900μg/ml(蛋白质含量)或1300μg/ml的ATE处理的细胞。图3D示出了用已知的血管生成生长因子,10ng/ml的VEGF和1ng/ml的FGF-2,处理的阳性对照细胞。在图3B、图3C和图3D中可以清楚地观察到管形成。与阳性对照3D的血管生成潜力相比,两种ATE处理具有相等但是剂量依赖性的血管生成诱导潜力。

具体实施方式
脂肪组织包括各种分化阶段的脂肪细胞、内皮细胞、成纤细胞、周细胞以及能够分化成若干谱系的细胞的脂肪干细胞和充质干细胞。新生脂肪生成,即新脂肪组织的形成,是软组织工程化的有希望的方法。本发明基于尝试提供适合细胞增殖和分化的微环境从而导致在没有细胞的外源移植的情况下形成血管和脂肪组织的研究。最佳的微环境加强脂肪干细胞从周围组织的迁移并诱导内皮细胞和脂肪前体细胞分化为成熟脂肪细胞、疏松结缔组织和肌肉细胞以及血管细胞。发育组织中的新血管形成始终是必须的以避免坏死和瘢痕形成,即从而使功能性成熟脂肪组织能够发育。
本发明涉及一种无细胞脂肪组织提取物(ATE),其具有在不施用细胞的情况下在植入位置诱导脂肪组织和维管结构快速形成的潜力。所述ATE能够为新生脂肪生成和血管生成创造最佳的微环境,并因此可以在软组织修复和/或工程化中使用。
本文中术语“脂肪组织提取物”(ATE)是指生物活性物质的混合物,所述生物活性物质优选为脂肪形成因子和血管形成因子,其由脂肪组织细胞即各种分化阶段的脂肪细胞、内皮细胞、成纤细胞、周细胞以及脂肪干细胞分泌。所述提取物是无细胞的或非细胞的。所述脂肪组织提取物与传统条件培养基不同,例如,所述提取物采集自小组织块或活细胞。制备方法包括无细胞培养,所得提取物比条件培养基浓度要高得多,并且培养时间短,在若干分钟到几天的范围内变化。
可以由例如通过吸脂术或外科手术获得的脂肪或脂肪组织样品制备脂肪组织提取物。根据需要,将所述组织样品切成小块以使细胞基本上保持活力。吸脂术材料可以直接使用。换言之,所述组织样品的加工不涉及匀浆化。通过在培养基中、在无菌盐溶液中、在磷酸盐缓冲盐水中或在其他合适的等渗性水缓冲溶液中培养细胞来提取生物活性因子,其中在培养期间细胞或组织块将生物活性因子释放到液相中。然后收集所述ATE,即没有细胞的液相。也可以在使用之前将所述ATE过滤以使所述提取物消毒并产生非细胞液体。所得ATE是细胞因子和其他生物活性物质的无细胞蛋白质混合物。
不仅可以使用同源性脂肪组织,也可以使用异源性脂肪组织作为ATE原料,并为了上述目的将其加工。这种异源性提取物能够被吸收到并用于例如冷冻干燥的“成品”软组织支架中。这种产品的一个实例由交联的透明质酸(例如Restylane)和异源性ATE制成。由于提取物是非细胞的,因此即使在相同的提取物和植入物适合若干患者的异源性使用中也不太可能发生免疫反应和过敏反应。这得到了在实施例5中更详细描述的动物研究的支持,其中在大鼠中植入的人ATE(异种移植物)不引起任何炎症反应或其他并发症。
ATE是通过脂肪组织细胞表达的脂肪形成因子和血管形成因子的最佳的细胞因子混合物。可以通过现有技术中已知的常规方法(例如ELISA)检测ATE的生物活性物质。本文中,检测了在各种ATE中120种细胞因子的表达(参见实施例1)。长培养时间,例如24小时或更长,导致更多的血管生成提取物,而短培养时间和长培养时间都产生脂肪形成混合物。
可以将ATE调配(tailor)为包含期望量的或预定量的脂肪形成因子和血管形成因子,所述脂肪形成因子和血管形成因子包括血管内皮细胞生长因子(VEGF)、基本的成纤细胞生长因子(FGF-2)和类胰岛素生长因子(IGF-1)。期望的组成可以取决于预期的用途。根据当前的知识和本发明的支持,VEGF和FGF-2被认为是刺激血管生成的重要因子,而FGF-2和IGF对刺激脂肪生成是重要的。因此,在一个实施方案中,ATE包含每1mg总蛋白质至少1pg的VEGF、至少70pg的FGF-2和至少50pg的IGF-1。这种ATE特别适用于例如在软组织修复和工程化中刺激脂肪生成。在另一个实施方案中,ATE包含每1mg总蛋白质至少7pg的VEGF、至少70pg的FGF-2和至少50pg的IGF-1。这种ATE特别适用于在例如诱导伤口愈合和治疗缺血性病症和烧伤中刺激血管生成。不同的所调配的ATE可以进一步含有不同量的若干其他细胞因子,虽然它们对于特定的应用而言可能不总是至关重要的(参见表1)。总之,ATE应当含有约2∶1至约1∶2的IGF-1和FGF-2,以及约1∶100至约1∶10的VEGF和FGF-2。
可以通过在ATE的制备中使用不同的培养时间和温度来调配脂肪组织提取物的含量。通常,培养时间在若干分钟至若干小时的范围内,例如过夜。例如,约1小时的培养时间通常得到约1.7mg/ml至2.5mg/ml的总蛋白质含量、约200pg/ml至约1300pg/ml的IGF-1含量、约200pg/ml至约1400pg/ml的FGF-2含量和约2pg/ml至约25pg/ml的VEGF含量。换言之,1小时的培养时间通常得到每1mg总蛋白质约150pg至约550pg的IGF-1含量、每1mg总蛋白质约100pg至约500pg的FGF-2含量以及每1mg总蛋白质约3pg至约20pg的VEGF含量。这种类型的提取物是脂肪形成性的和血管形成性的。然而,如果在提取物中需要特别高浓度的VEGF(约200pg/ml至约1400pg/ml,即每1mg蛋白质约25pg至约500pg的VEGF),例如特别是对于在期望的目标组织中的血管生成诱导而言,则应当将培养时间延长至例如约24小时。24小时的培养时间通常得到每1mg总蛋白质约60pg至约200pg的IGF-1含量、每1mg总蛋白质约130pg至约500pg的FGF-2含量以及每1mg总蛋白质约25pg至约500pg的VEGF含量。此外,可以使用各种培养温度来调整脂肪组织提取物,所述培养温度通常在室温至约37℃的范围内。然而,在一些实施方案中,可以使用较低的温度,如4℃。在再其他实施方案中,只要蛋白质未发生变性,可以使用更高的温度。
更普遍地,所述提取物中VEGF的浓度可以在约1pg/ml和约1400pg/ml之间,优选在约10pg/ml和约1000pg/ml之间,更优选在约7pg/ml和约700pg/ml之间,并且最优选在约10pg/ml和约100pg/ml之间变化。所述提取物中FGF-2的浓度可以在约20pg/ml和约1500pg/ml之间,优选在约100pg/ml和约1300pg/ml之间,更优选在约150pg/ml和约1000pg/ml之间,并且最优选在约200pg/ml和约800pg/ml之间变化。IGF-1的浓度可以在约100pg/ml和约1500pg/ml之间,并优选在约150pg/ml和约800pg/ml之间,并且最优选在约200pg/ml和约500pg/ml之间变化。另一方面,所述提取物的总蛋白质浓度可以在约0.75mg/ml和约3.5mg/ml之间变化。优选地,总蛋白质浓度为约1.4mg/ml至约2.7mg/ml,更优选为约1.8mg/ml至约2.7mg/ml,再更优选为约2mg/ml至约2.7mg/ml,并且最优选为约2.1mg/ml至约2.5mg/ml。
还可以以与总蛋白质浓度的关系定义提取物中生物活性物质的浓度。因此,含有1mg总蛋白质的提取物中VEGF的量(每1mg蛋白质的生长因子含量)可以在约1pg和约800pg之间,并优选在约3.5pg至约500pg之间,更优选在约7pg至约300pg之间,并且最优选在约20pg至约100pg之间变化。含有约1mg总蛋白质的提取物中FGF-2的量(每1mg蛋白质的生长因子含量)可以在约1pg和约1200pg之间,优选在约1pg至约600pg之间,更优选在约70pg至约500pg之间,再更优选在约110pg至约450pg之间,并且最优选在约250pg至约350pg之间变化。IGF-1的量(每1mg蛋白质的生长因子含量)可以在约50pg和约700pg之间,并优选在约100pg至约500pg之间,再更优选在约100pg至约300pg之间,并且最优选在约110pg至约230pg之间变化。
ATE还可以天然地包含或为了进一步调配ATE而向其中添加若干其他脂肪形成和血管形成因子(表1所示),如生长因子、白细胞介素、补体系统产品、糖皮质素、前列腺素、脂蛋白和游离脂肪酸以及细胞外基质成分,例如胶原蛋白I、胶原蛋白IV和胶原蛋白VI、蛋白聚糖、弹性蛋白和乙酰透明质酸。另外,在使用之前,例如可以用任何期望的药物或药剂补充所述ATE。
另一方面,在使用之前,可以通过去除诸如单一生长因子或细胞因子的物质来进一步调配ATE。例如,如果所述ATE要被用于例如在治疗缺血性病症中诱导初级血管生成,则可以去除血管生成抑制剂如TIMP-1和TIMP-2以及初级脂肪形成因子如IGF-1、脂连蛋白和/或瘦蛋白。去除或分离期望的物质的方法是本领域容易得到的,包括通过免疫吸附分离生长因子。
调配ATE可以进一步包括浓缩(例如通过离心分离)或稀释(例如用培养基、无菌生理盐溶液或任何其他缓冲等渗性溶液),从而获得适合特别用途的ATE。
在细胞培养研究中测试脂肪组织提取物的脂肪形成潜力,其中在存在或不存在本实施方案的提取物的情况下培养人脂肪源性干细胞(hASC)。如图2所示和在实施例2中更详细地描述,如通过增加的油-红-O颜色富集(甘油三酯(triglyseride)形成)的判断,所述提取物清楚地诱导多数干细胞的脂肪形成分化。
此外,在体外管形成实验中评价本实施方案提取物的血管形成潜力。在存在或不存在脂肪组织提取物或VEGF和FGF-2的情况下在成纤细胞培养物之上培养人内皮细胞。在所有情况下,清楚地观察到血管生成,即管形成,如图3所示和在实施例3中更详细地描述。然而,ATE的血管形成潜力明显是剂量依赖性的。
在一些实施方案中,本发明的ATE可以用作细胞培养基或细胞培养基补充物以在体外研究脂肪生成或血管生成。所述ATE导致分化的脂肪细胞在细胞培养物中均匀的分布,并因此提供用于反映人体内条件的脂肪生成的优异的细胞模型。迄今为止,还没有用于脂肪形成分化的合适的细胞模型。
在细胞培养中,特别是在临床应用中存在代替异种血清的总目标。人血清是及其昂贵的,因此不是最佳的溶液。在本发明的一些实施方案中,ATE可以用作细胞培养物成分或者在细胞培养基中用作血清替代物。这不仅节省成本,而且对于产生组织工程化的人构造是有用的,特别是在动物源性成分不合适的情况下。
本发明还涉及包含同源性或异源性无细胞脂肪组织提取物和生物相容性基质的植入物。仅使用脂肪组织提取物修复大的或深的软组织缺陷是困难的,但是所提供的植入物适于填充软组织缺陷,承受机械负载,并且还使新组织再生。此外,所述植入物提供稳定的长期结果,感觉上并看起来与自然组织相似,易于使用,无毒并且是惰性的,但是另一方面,允许营养物和代谢物适当的扩散。所述生物相容基质必须是对人体无毒的、在组织中相当惰性的、能够结合和释放小分子(例如生长因子),并且在组织中可降解。
在本发明的植入物中使用的生物相容基质优选为水凝胶。本文中所用术语“水凝胶”是指含有亲水性聚合物链、能够将水吸收到其结构中、溶胀成其原始体积的百分之数百或数千并仍然保持其结构的高度含水的材料。天然聚合物水凝胶具有若干有希望的性质。它们可降解,它们能够在温和条件下加工,并且它们是细胞外基质(ECM)的成分或具有与其相似的结构。天然聚合物通常是无毒的并且在体内与周围的组织良好地相互作用。根据特定植入物的要求,可以修改水凝胶的硬度或刚度、降解速率和生物活性物质结合速率或物质释放速率。这些修改的方法易于在现有技术中获得的。
一种合适的水凝胶材料是透明质酸或乙酰透明质酸,其是广泛分布的形成结缔组织细胞外基质的天然多糖。其包含与N-乙酰葡萄糖胺链连接的D-葡萄糖醛酸。透明质酸是通常使用的天然生物材料,因为其是完全无免疫原性的并且在本领域已经被安全使用了几十年。交联的透明质酸是市售的,例如商品名(Q-Med,瑞典)、(Anika Therapeutics,USA)、(L.E.A.Derm,法国)、(Bioha laboratories,中国)、(Genzyme,USA)、Prevelle(Genzyme,USA)、(Teoxane laboratories,瑞士)、(Merz Aesthetik,德国)、(Auragenix Biopharma,荷兰)和Rofilan Hylan(Cairo Tech,埃及)。这些交联的透明质酸中的一些(例如Restylane)已经被FDA批准为在临床应用中的软组织填充物质。
适用于本发明的另一水凝胶是壳聚糖,其是含有葡萄糖胺和N-乙酰葡萄糖胺的聚合物的天然多糖。由于是缓慢生物可降解的生物材料并且对人体无毒,因此在工业和研究中壳聚糖已经被广泛地用作制药试剂。在此之前,改性壳聚糖已经被用作激素、药物或蛋白质的控释递送系统,其中通过扩散和生物材料的降解发生释放。
适用于本发明的其他天然水凝胶形成材料是本领域技术人员容易获得的,包括例如胶原蛋白、藻酸盐、明胶、血纤蛋白和琼脂糖。
合成水凝胶如基于聚乳酸(PLA)、乳酸羟基乙酸共聚物PLGA、聚己内酯(PCL)的聚酯也可以在本发明实施方案的植入物中用作水凝胶。
根据材料,有可能实现更多的脂肪生成或血管生成。通过选择不同的支架材料或通过材料改性(例如乙酰透明质酸交联),有可能提高植入物的脂肪形成或血管形成效果,并使所述效果集中于更多的血管形成或更多的脂肪形成潜力。
本发明实施方案的植入物还可以包含水凝胶的混合物,如壳聚糖和透明质酸的混合物。还可以通过共混、共聚或功能化组合水凝胶。
在一些实施方案中,所述生物相容基质可以包含纳米或微米颗粒。例如,可以例如通过离子凝胶化法由壳聚糖制成微米颗粒,或者可以通过水包油包水法制备合成的PLA、PLGA或PCL微米颗粒,并可以在内部混合第二水凝胶材料,例如透明质酸。因此,在植入物中可以以纳米或微米颗粒和包围所述颗粒的水凝胶的形式提供脂肪组织提取物。总之,ATE可以与任何可降解的药物释放装置组合。
生物相容基质的量相对于植入物中脂肪组织提取物的量通常为约37%至约50%生物相容基质和约50%至约63%提取物。在生物相容基质为水凝胶如透明质酸的情况下,植入物中水凝胶含量的降低导致较弱的凝胶结构,导致在体内较快的降解速率。在一个具体的实施方案中,所述植入物包含约45.5%透明质酸、和约54.5%脂肪组织提取物。这种植入物具有足够弱的凝胶强度以允许直接注射到受体组织中。
所述水凝胶或其组合还可以通过交联、冷冻干燥、共混和/或共聚进行改性,并且可以使用不同浓度的水凝胶组合。植入物的大小也可以在非常小(几微升)至几十微升的范围内变化。通常,例如为了面部重建的目的,使用1至5微升的植入物。
植入物中的脂肪组织提取物含量可以变化。通常,植入物包含约300μg/ml至约1.2mg/ml的蛋白质,优选在600μg/ml和1mg/ml之间,再更优选接近800至900μg/ml。
本发明的植入物可以通过任何合适的方式植入。注射提供可行的植入方法。植入的替代形式是本领域中熟知的,如支架、薄膜或膜。
在动物实验中证实了本发明的植入物的脂肪形成和血管形成潜力。将植入物植入到大鼠体内皮肤之下,在植入位置处监测脂肪组织和血管形成(实施例4,图4和图5)。已经经过2周之后,观察到脂肪组织积累和血管形成。在4至6周时,在与植入物紧密接触的位置中观察到大量的血管形成和动脉状结构以及一些脂肪组织形成。在12至20周时,在与植入物紧密接触的位置处检测到成熟脂肪组织的高度血管化的、厚的、密集的层。在40周时,植入物已经降解;然而,发育的成熟脂肪组织仍然存在,证明通过本发明实施方案的植入物可以获得长效的结果。因此,这些结果证明本发明实施方案的植入物的确能够用于体内软组织工程化和修复。
因此,本发明提供一种诱导新生脂肪组织形成和血管生成的植入物,以及提供一种已经被捕集并固定到水凝胶结构中的脂肪形成和/或血管形成因子的长期递送系统。本发明的生物活性植入物有助于诱导宿主细胞渗透并且使组织再生更加有效率。所述植入物的效果是局部的且位置特定的,降低了某种效果需要的脂肪形成和血管形成因子的总剂量。这不仅节省成本还是避免有害全身效应的方法。
本发明还提供制备本发明植入物的方法。所述方法包括将脂肪组织提取物与生物相容基质混合或将其吸收到生物相容性基质中。所述混合通常在室温下在无菌注射器中小心地进行直到ATE和水凝胶材料完全地混合,避免气泡形成。所述提取物立即被捕集并吸收到所述水凝胶材料中并准备用于植入。
此外,本发明提供本发明的脂肪组织提取物和植入物在软组织修复或工程化中的用途。因此,所述提取物和所述植入物可以在修复或工程化软组织的方法中使用以实现成熟脂肪组织和足够血管形成的发展。在一个具体实施方案中,所述植入物通过注射施用。
此外,本发明提供本发明的脂肪组织提取物和植入物在血管生成诱导和/或缺血组织修复中的用途。因此,所述提取物和所述植入物可以在修复或工程化毛细血管的方法中使用以实现足够血管形成的发展。在一个具体实施方案中,所述植入物通过注射施用。与本发明相关,已经发现,对于脂肪生成而言,在组织中存在生存脂肪细胞或脂肪组织干细胞通常是有利的。如果所述植入物例如被施用到没有脂肪细胞的组织中,则所述植入物的效果可能更多地或主要地是血管形成性的而不是脂肪形成性的。
由于短培养时间和短混合时间的原因,本发明的一个优点在于ATE的制备容易并且其调配快速。因此,获得脂肪组织、将其加工成ATE并进一步加工成植入物的整个过程以及植入可以在一次手术中完成。
对于本领域技术人员明显的是,随着技术进步,本发明的概念可以以不同的方式实施。本发明及其实施方案不限于下述的实施例,而是可以在权利要求的范围内变化。
实施例1.用于细胞培养和动物研究的脂肪组织提取物的制备
根据需要,将从吸脂术中获得的并且作为得自外科手术的皮下组织的人脂肪组织样本以及得自被处死的大鼠的大鼠脂肪组织样本切成小块。将组织块或吸脂术材料移至50ml试管(Nunc)中。向试管中加入没有任何补充物的无菌盐溶(用于动物研究)或DMEM/F12培养基(用于细胞培养研究),并培养至少45分钟。在培养期间轻柔震荡试管若干分钟。在不同时间点收集脂肪组织提取物(ATE)的样品(在每个收集点,将培养基或盐溶液完全移除并用新鲜液体替换),以12000rpm离心5分钟,并且在细胞培养物或动物实验中使用之前无菌过滤。在ATE制备的24小时之后,通过分离细胞并将其进一步培养来检测细胞的生存力。基质血管部分细胞是形态正常的,并且保持生物活性且正常增殖。
通过BCA蛋白质测定法(Pierce Biotechnology,Rockford,IL,USA)分析源自不同患者和/或在不同时间点(1hr、2hr或24hr)收集的80个人ATE样品的总蛋白质浓度。总蛋白质浓度总是在时间点1hr(1等分试样)和2hr(2等分试样)处于最高值,并且蛋白质浓度在该时间内降低(图1A)。通常,总蛋白质浓度在第一等分试样(1hr)中为约1.8mg/ml至约2.5mg/ml,在第二等分试样(2hr)中为约1.5mg/ml,以及在第三等分试样(24hr)中为约1mg/ml。在无菌盐溶液或在细胞培养基中培养的提取物具有相当的总蛋白质含量。在将相等体积的培养溶液(培养基或无菌盐溶液)和切碎的脂肪组织用于提取的情况下,实现最佳蛋白质制备。这种条件导致蛋白质浓度接近2mg/ml,对于细胞培养物研究而言其在合适的范围。然而,可以并且通常不得不将较高的浓度稀释到合适的范围。任何过多的培养溶液导致过低的蛋白质浓度,这需要通过在离心柱中浓缩来进一步调整。
此外,在不同时间点(1hr、2hr和24hr)收集的31至40个人脂肪组织提取物样品中每个都通过酶联免疫吸附测定法(ELISA,分别为Quantikine人VEGF免疫测定法、Quantikine人FGF基本免疫测定法或者Quantikine人IGF-I免疫测定法,R&D Systems,Abingdon,UK)检测血管内皮细胞生长因子(VEGF)、基本成纤细胞生长因子(FGF-2)或类胰岛素生长因子(IGF-1)的浓度。根据制造商的说明进行ELISA。所有标样和样品都双份检测。
根据结果,在1小时培养之后IGF-1浓度处于最高水平(图1B),而FGF-2浓度在培养过程中都非常高(图1C)。VEGF似乎在培养24小时之后急剧增加(图1D)。虽然根据测试,在1小时和2小时处的VEGF浓度对于脂肪生成或血管生成诱导是合适的,但是仅在24小时培养之后VEGF才达到非常高的水平。
此外,使用人细胞因子抗体阵列仪C系列1000(Human Cytokine Antibody Array C Series 1000,RayBioTech,Inc.,Norcross,GA,USA)从在两个不同时间点(1hr和24hr)的2个不同提取物样品中检测出全部120种生长因子和细胞因子。根据制造商的说明操作所述阵列仪。所有样品均以双份检测,并以半定量获得结果。表1中表示为(-)的因子是未被表达的,或其表达很低从而在阵列仪中检测不到。
根据细胞因子检测,在阵列仪的检测限内的ATE的细胞因子图案如下:1hrATE样品表达大量的血管生成素、脂肪生成素、TIMP-1和TIMP-2、MIF、IGFBP-6、NAP-2、瘦蛋白、PDGF-BB、GRO和少量的若干其他因子,如表1中所示。24hr ATE样品表达大量的血管生成素、IL-6、NAP-2、MCP-1、脂肪生成素、GRO、TIMP-1、TIMP-2和少量的若干其他因子,如表1中所示。在24小时,总体上比在1小时表达更多的细胞因子(参见表1)。瘦蛋白、PDGF-BB、MIF和IGFBP-6的表达在1小时处很高,而在24小时处很低。在24小时,MCP-1、IL-6、CCL5和VEGF表达很高,但在1小时处仅检测到很低的量。
因此,通过改变培养时间和调整生长因子浓度,有可能制备和调配用于不同用途的ATE。

实施例2.脂肪组织提取物的脂肪形成潜力
根据从Zuk PA et al.(Mol Biol Cell.2002Dec;13(12):4279-95)修改的方案分离人脂肪源性干细胞(hASC)。简言之,根据需要将得自外科手术或吸脂术的人脂肪组织样本切成小块,并在37℃下在振荡器中使其在用0.05%胶原酶I补充的DMEM/F12培养基中酶消化60至90分钟。为了易于消化,在培养期中偶尔将组织吸出。为了使基质血管部分与干细胞群分离,在室温下将消化的组织在600xg下离心10分钟。首先通过具有100μm孔径的过滤器然后通过具有40μm孔径的过滤器将消化的组织过滤。将获得的人干细胞群在用15%人血清(HS,Cambrex)、1mM L-谷氨酰胺和1%抗菌-抗霉菌混合物(Gibco)补充的DMEM/F12中培养。在恒定湿度的5%CO2空气氛下将细胞保持在37℃。使细胞附着过夜。次日,将细胞清洗若干次并更换培养基以去除碎片。
按以上方式培养hASC并且每2至3天更换培养基。将源自2或3个不同患者的早期传代(p1至p5)融合干细胞培养物用胰蛋白酶处理、汇集在一起并以10000细胞/cm2的密度铺板于培养基中。次日,采用6个不同的培养条件:
1)用人血清补充的普通培养基:DMEM/F12、15%人血清、1%抗菌-抗霉菌混合物、1mM L-谷氨酰胺;
2)用ATE补充的培养基:DMEM/F12、15%人血清、1%抗菌-抗霉菌混合物、1mM L-谷氨酰胺;以及300μg/ml(蛋白质浓度)的ATE(培养体积的约25%)
3)用ATE补充的培养基:DMEM/F12、15%人血清、1%抗菌-抗霉菌混合物、1mM L-谷氨酰胺;以及600μg/ml(蛋白质浓度)的ATE(培养体积的约50%)
4)用ATE补充的培养基:DMEM/F12、15%人血清、1%抗菌-抗霉菌混合物、1mM L-谷氨酰胺;以及950μg/ml(蛋白质浓度)的ATE(培养体积的约75%)
5)用15%人血清、1%抗菌-抗霉菌混合物、1mM L-谷氨酰胺补充的ATE(1200μg/ml蛋白质浓度;最终培养体积的83%);
6)脂肪形成培养基:DMEM/F12、10%胎牛血清(FBS)、1%抗菌-抗霉菌混合物、1%L-谷氨酰胺、33M生物素、17M泛酸盐、100nM胰岛素、1M地塞米松(Sigma)和0.25mM异丁基-甲基黄嘌呤(IBMX,Sigma)。仅在最初24小时将IBMX置于脂肪形成培养基中。
每2至3天更换培养基。在限定的时间点(2至3周和5周)通过用油-红-O(Sigma)将细胞培养物染色来评价脂肪形成分化。除非另有说明,细胞培养试剂得自Gibco。
基于油-红-O染色检测的细胞中的甘油三酯积累评估不同培养条件下的脂肪形成分化。简言之,通过将油-红-O溶解在100%异丙醇中来制备0.5%油-红-O溶液。将所述溶液与蒸馏水以2∶3的比例混合,然后在室温下放置10分钟。使用标准滤纸过滤所得溶液。在染色之前,将细胞培养介质从孔中吸出并用PBS温和地将孔冲洗2至3次。用4%低聚甲醛溶液将细胞固定20分钟。然后用蒸馏水冲洗孔。将细胞在60%异丙醇溶液中培养2至5分钟,然后加入预先制备的油-红-O溶液并培养5分钟。用蒸馏水冲洗孔直到水澄清。用与CCD摄像单元连接的Nikon TS-100(Nikon,Tokyo,日本)拍摄染色细胞的相差显微照片。通过评价细胞内红色染色区域的范围来检测脂肪形成分化的量。
在每个干细胞群中,脂肪组织提取物显示在培养1至3周之后诱导培养物中多数细胞发生剂量依赖性同源脂肪形成转变(图2)。当使用至少200μg/ml的ATE时观察到脂肪形成作用,并且在测试最多2mg/ml的更高浓度ATE时脂肪形成作用更加广泛。在实验中使用的ATE根据实施例1中的描述制备,并且包含1.8mg/ml蛋白质、143pg/ml IGF-1、123pg/ml FGF-2和3.6pg/ml VEGF。
实施例3.脂肪组织提取物的血管形成潜力
在用10%胎牛血清、1%L-谷氨酰胺、1%非必需氨基酸(Gibco)和1%抗菌性-抗霉菌性混合物(Gibco)补充的MEM(Gibco)中培养BJ成纤细胞(CRL-2522;美国典型培养物保藏所(American Type Culture Collection),Manassas,VA,USA)。在恒定湿度的5%CO2空气氛中将细胞保持在37℃。每2至3天更换培养基并将融合细胞按1∶4分开。
从脐带样本中分离人脐静脉内皮细胞(HUVEC),在内皮生长培养基(EGM-2,Lonza)中培养最多7次传代。在恒定湿度的5%CO2空气氛中将细胞保持在37℃。每2至3天更换培养基并将融合细胞按1∶4分开。
为了研究本发明的脂肪组织提取物的血管形成潜力,将BJ成纤细胞以20000细胞/cm2的密度铺板于48孔培养板中并培养至融合(3至4天)。然后,将HUVEC以4000细胞/cm2的密度铺板于融合成纤细胞培养物之上。铺板HUVEC之后,将培养基更换为EGM-2培养基(得自EGM-2bullet kit,Lonza)。次日,不同地处理共培养物:
1)阴性对照:内皮细胞基础培养基(EBM-2,得自EGM-2bullet kit);
2)阳性对照A:用10ng/ml VEGF(R&D Systems)和1ng/ml FGF-2(Sigma)补充的EBM-2;
3)阳性对照B:用50ng/ml VEGF(R&D Systems)和5ng/ml FGF-2(Sigma)补充的EBM-2;
4)用440μg/ml(蛋白质浓度;细胞培养基总体积的约25%)的ATE补充的EBM-2;
5)用880μg/ml(蛋白质浓度;细胞培养基总体积的约50%)的ATE补充的EBM-2;
6)用1320μg/ml(蛋白质浓度;细胞培养基总体积的约75%)的ATE补充的EBM-2。
所有培养介质用2%FBS、1%L-谷氨酰胺、0.1%GA-1000(庆大霉素硫酸盐和两性霉素B的溶液,得自EGM-2bullet kit)补充。在实验中使用的ATE根据实施例1中的描述制备并且含有1.76mg/ml蛋白质、143.4pg/ml IGF-1、123.1pg/ml FGF-2和3.56pg/ml VEGF。
在用内皮细胞特异性抗体(抗冯维勒布兰德因子,anti-von Willebrand factor)免疫细胞化学染色之前将细胞培养7天。在培养期间更换一次介质。
按以下步骤进行免疫细胞化学染色:用PBS将细胞清洗3次,用冰冷的70%乙醇固定20分钟,用0.5%Triton X-100(JT Baker,Phillipsburg,NJ,USA)透化15分钟并用10%BSA(Gibco)阻断非特异性染色30分钟。在阻断之后,在4℃下用冯维勒布兰德因子一级抗体(在兔子中制备的抗-vWF,Sigma,1∶500)培养细胞过夜。次日,用PBS清洗细胞3次,用二级抗体(兔子的多克隆抗体IgG FITC,Acris Antibodies GmbH,Hiddenhausen,德国,1∶500)培养30分钟,用PBS清洗3次。在染色之后,在细胞培养孔中保留500μl PBS并用石蜡封口膜将孔密封。用配置有Nikon DS摄像控制单元DS L-1的Nikon Eclipse TS100显微镜(Nikon,Tokyo,日本)观察荧光,并用Adobe Photoshop软件(Adobe Systems,San Jose,CA,USA)处理图像。
从第4或第5天起脂肪组织提取物在共培养物中诱导管状结构的形成。在7天之后,在阳性对照A和B中(分别为图3E和图3F)以及在用880μg/ml(图3C)或1320μg/ml(图3D)的ATE处理的细胞中观察到清晰的管形成。阳性对照A中的生长因子处理清楚地刺激典型的长、细、支化毛细管状结构的形成。然而,在阳性对照B中任何过量的生长因子多少会导致紊乱或簇生管结构。阳性对照A中的生长因子处理代表组织中的体内环境。用1320μg/ml的ATE和甚至用880μg/ml的ATE获得的结果与用已知血管形成生长因子(阳性对照A)获得的结果非常相似。脂肪组织提取物对HUVEC细胞产生剂量依赖性作用并产生毛细管网络。
实施例4.可注射植入物的制备
配制两种不同的本发明的可注射植入物用于动物研究:1)包含54.5%大鼠ATE(总蛋白质浓度1.3mg/ml)和45.5%透明质酸的植入物;和2)包含54.5%人ATE(总蛋白质浓度2.5mg/ml)和45.5%透明质酸的植入物。在这些制剂中,使用的透明质酸为非动物源性的稳定化的透明质酸(NASHA)20mg/ml,Restylane(Q-Med,Uppsala,瑞典)。也可以使用不同形式的交联透明质酸产品(例如Restylane Perlane或Restylane Touch或Macrolane),这取决于缺陷的类型。
通过将1ml的Restylane与1.2ml的ATE(按照实施例1中的描述制备的)小心地混合来制备植入物。置于大鼠皮下组织之下的最终植入物体积始终为100μl并包括45.5μl透明质酸和54.5μl合并的液体。这种植入物是足够粘稠的以避免立即再吸收到组织中或从注射位置泄漏出来,但是含有大量的提取物。
按照上述制备含有54.5%磷酸盐缓冲盐水(PBS)和45.5%透明质酸的对照植入物。
为了研究蛋白质从植入物中的释放,将120μl的每种植入物注射到具有0.2μm孔径的96孔板条插入物(96-well plate strip insert)(Nunc)中。将插入物置于96孔细胞培养板中,并在37℃下在150μl的PBS中培养若干周。每3天更换PBS。收集等分试样并在-20℃下存储用于总蛋白质和ELISA分析。在2周的时间里,对照植入物不释放蛋白质,而含有ATE的植入物每3天释放最多350μg/ml的蛋白质以及最少100μg/ml的蛋白质。这些释放实验表明,从植入物中释放出蛋白质,因此所述植入物是功能性的。
实施例5.植入物的体内脂肪形成和血管形成活性(动物研究)
所有动物实验根据芬兰动物保护法进行并得到西芬兰国家地方办公室社会福利和卫生服务部(Department for Social Welfare and Health Services of State Provincial Office of Western Finland)的批准。
用Domitor(1mg/ml;0.5mg/kg)和Ketalar(10mg/ml;75mg/kg)的混合物麻醉27只雄性斯普拉-道来大鼠(平均重量300g)。用1ml注射器和27计量注射针,将按照实施例4中的描述制备的植入物和对照以100μl的最终体积注射到大鼠的背部皮下组织之下。使植入物在皮下组织之下保留1、2、3、4、6、8、12或20周以及9个月,然后用二氧化碳将动物处死。从植入位置采集组织样品并处理,用于组织分析。实验中使用的人ATE按照实施例1中的描述制备并含有2.5mg/ml蛋白质、200pg/ml IGF-1、863pg/ml FGF-2和61pg/ml VEGF。在实验中使用的大鼠ATE按照实施例1中的描述制备并含有1.3mg/ml蛋白质。
将组织样品在4%低聚甲醛中固定过夜,用分级乙醇序列脱水并包埋于石蜡中。用切片机(Microm HM 430,Microm GmbH,Waldorf,德国)将样本切成5μM厚的薄片并用苏木精-伊红(H&E)染色,用于组织分析。用Nikon Microphot FXA显微镜(Nikon,Tokyo,日本)采集图像并使用Corel Draw 10和Adobe Photoshop 7.0处理。
组织样本的苏木精-伊红染色表明植入区域中的新血管形成和脂肪组织形成(图4和图5)。在植入2周之后采集的样品中检测到许多小血管和一些存在红细胞的成熟大血管。检测到仅有少量或没有脂肪组织形成。在植入4周之后采集的样品中,检测到一些与剩余的植入物和形成的血管结构紧密接触的白色圆形脂肪组织沉积物。检测到的小尺寸脂肪组织沉积物表明它们是新形成的。在植入4周或6周之后采集的样品中,已经进行了血管形成,能够观察到大量的小血管和与小动脉和小静脉相似的结构,以及一些脂肪组织形成。与对照相比,形成的血管数量上更多并且直径更大。在12至20周,在与植入物紧密接触的地方检测到成熟脂肪组织的高度血管化的、厚的、密集层。
在实验中,对炎症反应、植入物降解速率、脂肪生成和血管生成追踪观察到9个月。没有由于植入物注射操作或从麻醉术后恢复的原因产生的并发症。未检测到炎症或被膜形成。所有植入物和对照都诱导一些小血管形成,可能是因为由外来透明质酸材料引起的温和反应。然而,仅在用含有大鼠和人ATE的植入物注射的大鼠中血管形成是广泛的。在大鼠中,人ATE比大鼠ATE产生更多的血管形成,但是它们似乎都良好地诱导脂肪生成。
实施例6.临床预实验
通过直接吸入无菌注射器中由吸脂术样品制备人脂肪组织提取物。为了使蛋白质提取最大化,在无菌注射器中将2.5ml吸脂术的脂肪与1.5ml无菌等渗盐溶液混合并在室温下培养45分钟。在培养期间将注射器震荡若干次。所得提取物的总蛋白质浓度为2.5mg/ml,生长因子浓度为17pg/ml VEGF和177pg/mlFGF-2。
在培养之后,通过0.2μm过滤器(Acrodisc注射器过滤器,Pall Gelman Laboratory,An Arbor,USA)将提取物无菌过滤到通过接头与第一注射器连接的第二无菌注射器中。将装有提取物的第二注射器通过接头与装有足够量Restylane的第三无菌注射器连接。将所得无菌脂肪组织提取物与Restylane混合,比例为0.9ml(45.5%)Restylane和1.2ml(54.5%)脂肪组织提取物。通过将材料在第二和第三注射器之间来回注射将液相和凝胶相混合。当脂肪组织提取物的浅粉色均匀地悬浮在材料中时,认为植入物材料是足够均匀的。最终的植入物含有1.2mg/ml蛋白质。
将获得的植入物立即用30G针头皮下注射到健康志愿者腿部的两个不同的位置。第一个位置包含老的、存在的瘢痕组织,在其中注射450μl植入物。通过去除皮下脂肪组织在腿部的第二个位置提供2个缺陷,以使组织中所得圆形缺陷位置直径约为1cm。一个缺陷保持完整,作为对照,而用900μl植入物填充另一个缺陷。
在植入之后或在随访期间未观察到过敏反应或其他并发症。在预实验开始3个月(12周)和6个月(28周)之后从植入位置和对照组织中采集活组织检查。观察不到炎症反应,并且在3个月之后,仍然存在大量植入材料。在植入之后,在瘢痕组织中观察到增加的血管形成。然而,仍然不存在过度的脂肪组织形成。在6个月时(图6C),与对照相比,真皮中的组织体积极大地膨胀,并且可以观察到正常的、高度血管化的疏松结缔组织。在对照中(图6A),能够观察到极少量的血管。在6个月时,在真皮的下部观察到过度的血管(小动脉状血管)形成(b=血管),并且还存在新形成的脂肪组织(a=脂肪组织)(图6C)。根据临床初步研究,所述材料耐受性及其良好,诱导脂肪组织和血管形成,并且即使在植入物本身降解之后也存在新形成的组织。
因此,本发明提供一种植入物,其诱导新生脂肪组织形成和血管生成,并提供一种被捕集到并固定在水凝胶结构中的脂肪形成和血管形成因子的长期递送系统。本发明的生物活性植入物有助于宿主细胞渗透的诱导并使组织再生更有效率。所述植入物的作用是局部的和位置特定的,降低了用于某种效果的脂肪形成和血管形成因子的总剂量。这不仅节约成本还是避免有害全身效应的方法。
Claims (17)
1.一种无细胞脂肪组织提取物,包含预定量的VEGF、FGF-2和IGF-1。
2.如权利要求1所述的提取物,其中每1mg总蛋白质的生长因子含量为至少1pg的VEGF、至少70pg的FGF-2和至少50pg的IGF-1。
3.如权利要求2所述的提取物,其中每1mg总蛋白质的VEGF含量为至少7pg。
4.如权利要求2所述的提取物,还包含血管生成素、脂肪生成素、TIMP-1和TIMP-2、MIF、IGFBP-6、NAP-2、瘦蛋白、PDGF-BB和GRO。
5.如权利要求2或3所述的提取物,还包含血管生成素、IL-5、IL-6、lL-8、CCL5、NAP-2、MCP-1、脂肪生成素、GRO、TIMP-1、TIMP-2。
6.用于软组织修复或工程化、伤口愈合、治疗烧伤或治疗缺血性病症的如权利要求1至5中任一项所述的提取物。
7.包含权利要求1至5中任一项所述的无细胞脂肪组织提取物和生物相容基质的植入物。
8.如权利要求7所述的植入物,其中所述生物相容基质为水凝胶。
9.如权利要求8所述的植入物,其中所述水凝胶选自透明质酸,壳聚糖,血纤蛋白,胶原蛋白,藻酸盐,基于聚乳酸、乳酸羟基乙酸共聚物、聚己内酯的聚酯,及其混合物。
10.如权利要求7至9中任一项所述的植入物,所述植入物是可注射的。
11.如权利要求7至10中任一项所述的植入物,其中所述提取物是异源性的。
12.用于软组织修复或工程化、伤口愈合、治疗烧伤或治疗缺血性病症的如权利要求7至11中任一项所述的植入物。
13.制备包含预定量VEGF、FGF-2、和IGF-1的无细胞脂肪组织提取物的方法,所述方法包括以下步骤:
a)提供包含活细胞的非匀浆的脂肪组织样品,
b)培养所述样品预定时间,以及
c)收集提取物。
14.通过如权利要求13所述的方法获得的无细胞脂肪组织提取物。
15.制备如权利要求7至11中任一项的植入物的方法,包括将所述脂肪组织提取物与所述生物相容材料混合,或将所述脂肪组织提取物吸收到所述生物相容材料中。
16.一种诱导脂肪生成的方法,所述方法包括将如权利要求7至11中任一项的植入物植入到有需要的受试者中。
17.一种诱导血管生成的方法,所述方法包括将如权利要求7至11中任一项的植入物植入到有需要的受试者中。
