Abstract

Description
本发明涉及用于调节细胞的免疫应答的方法。
通过利用成本低且高效的内源性防御机制,主动免疫接种首次使得能够广泛防治最具威胁性的传染病,并且甚至在一些情况下实现了全球性根除所述传染病。因此,已进行了努力并且还将进行努力以针对各种适应症开发预防性和治疗性的疫苗接种方法。然而,有效的接种取决于导致获得保护性免疫力的免疫应答的诱导。但是,免疫接种抗原缺乏免疫原性导致没有取得所期望的效果。已经开发出了高度令人关注的特异性抗原制剂以用于预防和治疗疟疾、HIV、流感或者肿瘤疾病(仅为了列举几个突出的例子)。然而,这些疗法并不成功,这例如是由于免疫接种抗原缺乏免疫原性。此外,即使广泛使用的疫苗也导致缺乏免疫原性的问题,例如仅对于大约80%的受治疗者建立起真正保护性的免疫应答滴度的乙型肝炎疫苗。免疫系统缺乏反应性的主要原因是,这些抗原没有被识别为“外源的”。在哺乳动物中,主要是T-细胞决定将由抗原呈递细胞(APC)所呈献的结构识别为内源的还是外源的。为了诱导免疫应答,基本上需要两个分开的且相互独立的信号。该机制应当防止免疫系统的过度出击。首要的前提是,T-细胞受体也识别由APC所呈献的抗原。如果并非这种情况,则不会发生进一步的反应。此外,为了诱导免疫应答,绝对必不可少的是T-细胞表面上的CD28受体与B7的相互作用,只有当APC将所述抗原结构归类为危险时,B7才表达在APC上。在使用仅具有边缘免疫原性的接种抗原进行疫苗接种的情况下,通过B7和CD28之间的相互作用的共刺激可能不会发生,这继而不引起免疫应答,而是导致出现在T-细胞水平上的耐受。然而,能够证明,可以通过关闭E3-遍在蛋白连接酶Cbl-b而规避对于共刺激的需要。该酶是在免疫反应性的控制中的决定性转换点(Chiang等人,J Clin Invest(2007)doi:10.1172/JCI29472)。在Cbl-b不存在时,所施用的几乎没有免疫原性的物质也可以导致诱导出强烈的免疫应答。此外,Cbl-b缺陷型小鼠(纯合基因敲除)是能存活的,并且它们的免疫系统能够有效地识别自体诱导的肿瘤,并针对其建立主要基于CD8+T-细胞的裂解性免疫应答(Loeser等人,JEM(2007)doi:10.1084/iem.20061699)。然而,在用超级抗原进行免疫接种后,该酶的所描述的完全关闭也导致增加的自身免疫。因此,Loeser等人可以证明,作为负调节物的Cbl-b决定性地负责T-细胞的“免疫反应性”。
对于Cbl-b也已经描述了用于减弱特定基因表达的siRNA技术,具有较低的效率。US 2007/0087988涉及用于调节HPK1的方法,所述HPK1的表达可以通过提高Cbl-b表达而得到提高,反之亦然(例如,通过Cbl-b siRNA抑制)。
US 2007/0054355描述了Cbl-b肽和Cbl-b相关蛋白,特别是POSH,以及其用于治疗Cbl-b相关疾病的用途。
WO 2004/078130 A2涉及用于治疗POSH相关疾病(例如,病毒性疾病、癌症或神经学病症)的组合物。POSH可以尤其与许多POSH相关蛋白(包括Cbl-b)一起成为可供使用的。
US 2006/0292119A1涉及用于通过抑制细胞中的负免疫调节剂来提高免疫细胞的免疫应答的方法。此类负免疫调节剂选自与分子稳定性有关(例如通过遍在蛋白化、脱遍在蛋白作用和苏素化)的蛋白质,以及选自诱导NFkB-抑制剂表达的转录因子,或者NFkB-靶基因的转录的阻抑子。
然而,还没有描述Cbl-b介质的适合于临床应用的使用。因此,本发明的一个目的是提供可供使用的用于调节免疫反应性的实际适用的方法。
因此,本发明涉及用于提高接触过抗原的免疫系统细胞的免疫反应性的体外或离体方法,包括降低或者抑制这些细胞的Cbl-b功能,从而提高所述细胞针对该抗原的免疫反应性。
在现有技术中已经详细描述了Cbl-b基因及其基因产物(UniGene Id.Hs.3144和Hs.381921)。Cbl-b序列已经公开,例如在GenBank数据库中,登录号为NM_008279和NP_009112。抗-Cbl-b抗体、siRNA和反义抑制剂均是商业上可得的。例如在US2007/0054355中公开了某些适合于降低或抑制Cbl-b表达并因此降低或抑制Cbl-b功能的siRNA,所述siRNA具有混合的RNA/DNA核苷酸以及大约20个碱基的长度。
为了应对免疫系统过度反应(其可以导致例如自身免疫反应性的诱导)的风险,T-细胞中Cbl-b功能的抑制/关闭只可以在时间上严格限定的时间段内发生。因此,对于在治疗上辅助的疫苗接种成套方案而言,必需的是,仅在有限的时间段期间受控制地减弱Cbl-b,以便促进建立免疫接种抗原特异性免疫应答,但通过及时恢复免疫学“正常状态”而防止自身免疫性疾病。因此,根据本发明,在体外或离体仅对某些选择出的分离的免疫系统细胞进行处理,并重新将其引回患者中。因此,用于有效的体外或离体Cbl-b减弱的成套方案(Ansatz)是增强免疫反应性的前提条件。
优选地,通过降低或抑制Cbl-b的表达来降低或抑制Cbl-b功能。术语“降低”或“抑制”涉及相比于未改变的天然功能而言,Cbl-b的功能(或表达)下降,直至功能的完全抑制。优选地,将功能(或表达)降低至少30%、40%、50%、60%、70%、80%、90%或95%。
在优选的实施方案中,瞬时地实现Cbl-b的功能的降低或抑制。也就是说,功能如上所述仅暂时降低并且随后其可以重新恢复,例如通过消耗或降解抑制剂(例如Cbl-b siRNA),或者通过新生或Cbl-b未受损的细胞(体内)。在此还可以反复地进行免疫细胞中Cbl-b的瞬时降低,例如直至达到治疗成功。
优选地,通过使用Cbl-b反义RNA或者siRNA来降低或抑制Cbl-b的表达。为此,使用与靶(Cbl-b)mRNA序列的部分互补的短DNA和/或RNA序列,从而使得所述短DNA和/或RNA序列与靶(Cbl-b)mRNA序列杂交并令其失活。这些序列的长度优选地为至少15、18、20、22、25、28、30、35、40、50、60、70、80、90、100、120、140、160、180或200个碱基直至完整的靶序列,优选地直至2502、2000、1500、1000、500或300个碱基。优选地,使用SEQ ID No.1、2、3、4、5、6、7和/或8的序列。
同样地,可以通过许多其他已知的组分来降低或抑制Cbl-b的功能,例如通过使用Cbl-b拮抗剂、抑制剂,特别是适配体或Intramer。根据本发明可以使用抑制Cbl-b的作用或功能的任何拮抗剂或抑制剂,以提高细胞的免疫反应性。优选地,使用拮抗剂或抑制剂来制备药剂,所述药剂用于根据本发明在体外、离体或甚至在体内提高免疫系统细胞的免疫反应性。这使得能够治疗具有被抑制的或无效力的免疫系统的疾病,特别是癌症,以及使得能够提高针对(接种)抗原的免疫应答,所述(接种)抗原可以在体内或离体与免疫系统细胞相接触。
本发明还涉及用于降低免疫系统细胞的免疫反应性的方法,包括降低或抑制所述细胞的c-Cbl功能,从而降低所述细胞针对抗原的免疫反应性,优选地通过瞬时的降低或抑制,特别是通过使用c-Cbl反义RNA或者siRNA。为了增强免疫反应性,并非绝对必需使c-Cbl与Cbl-b一起减弱。如实施例中所显示的,减弱c-Cbl反而产生通过Cbl-b抑制而导致的效应的逆转。因此,Cbl-b和c-Cbl具有拮抗性的功能。c-Cbl还执行迄今未知的、在精细调节T-细胞反应性中的功能,这是因为它的减弱导致提高的免疫耐受性。因此,降低或抑制c-Cbl功能适合于免疫抑制,并因而使得其能够在例如炎症或变态反应中应用。因为减弱的程度和方向依赖于Cbl-b或c-Cbl(类似于本文中所描述的Cbl-b)的降低或抑制的剂量,所以可以相组合地在其功能方面降低这两种因子。为了提高免疫反应性,Cbl-b的降低超过c-Cbl的降低,而为了降低免疫反应性则反之。c-Cbl反义或者siRNA可以具有与上面对于Cbl-b所描述的相同的序列行(Sequzenzl inien)。优选地使用SEQ ID No.9、10、11、12、13、14、15和/或16的序列。
尤其是,在特别的实施方案中,使用这样的细胞,所述细胞已摄取了该抗原并且优选地呈递抗原片段,或者在HLA的情形下更好地识别抗原片段并且由此被激活。
在优选的实施方案中,根据本发明所使用的细胞为抗原呈递细胞、PBMC(外周血单核细胞)、T-淋巴细胞、B-淋巴细胞、单核细胞、巨噬细胞和/或树突细胞,特别是T-淋巴细胞,CD8+T-淋巴细胞、CD4+T-淋巴细胞,特别是Th1、Th2、Th17、Tregs(调节性T细胞)或CTL(细胞毒性T细胞)、NK细胞或NKT细胞。同样地,通常也可以使用CD3/CD19阴性淋巴细胞。NK细胞形成其中特别优选的一组。抗原优选地被细胞摄取,并且所述细胞优选地呈递抗原片段。在此特别优选地,将PBMC和T-细胞相组合地用于由此进行处理,以便诱导特别强的抗原特异性反应。在其他的实施方案中,特别是为了普遍提高免疫反应性(例如,为了治疗免疫缺陷),各种T-细胞单独地足以达到广泛的作用。根据本发明而提高的免疫反应性优选地由这些细胞(特别是CD8或CD4细胞以及NK或NKT细胞)来促成。
为了用Cbl-b抑制剂(例如Cbl-b siRNA)或敲除Cbl-b构建体来转染细胞,特别是T-细胞或NK细胞,优选地使用电穿孔。为此可以使用任何导致转染(也就是说,适合于Cbl-b的抑制)的培养基。此类培养基例如为Optimem(Gibco,#31985-047)。
此外,还可以用免疫刺激物质(例如,免疫刺激细胞因子或其他免疫刺激受体(如TLR,toll样受体)的配体或针对表面分子(优选地,CD3和/或CD28)的抗体)来处理或刺激所述细胞,以便促进由所述细胞进行的免疫反应。
同样地,在由树突细胞所支持的疫苗接种(优选地,抗肿瘤疫苗接种)的范围内,也可以采用Cbl-b的抑制。
备选地或另外地,也可以将Cbl-b受抑制的细胞与从患者中获得的树突细胞(其优选地荷载有肿瘤(细胞)抗原)进行体外共培养,以及将所述共培养物用于本发明的目的。
本文中所使用的“疫苗接种”不应当以绝对的意义(也就是说,施用免疫原,这导致由免疫系统实施的绝对保护)进行理解,而是应当理解为进行免疫学施用以便提高由免疫系统实施的保护或者针对该疫苗抗原而激活免疫系统(特别是免疫系统细胞)。
在一个特别的方面,本发明涉及Cbl-b抑制剂或拮抗剂在制备药物组合物中的用途,所述药物组合物用于提高患者中针对抗原的免疫反应性或者提高免疫反应性本身,包括分离患者的免疫系统细胞,通过使用Cbl-b抑制剂或拮抗剂在体外或离体提高所述细胞的免疫反应性,并且将所述细胞再植入所述患者中,其中通过降低或抑制所述细胞的Cbl-b功能来提高免疫反应性。
可以通过在小部分的循环T-细胞中降低Cbl-b表达,而与疫苗接种(施用抗原)相伴地实现免疫反应性的(优选地在时间上受限的)增强。为此,可以从患者(其理想地已在几天(例如5天)前进行了免疫接种)的全血或者来自骨髓和来自肿瘤组织自身(TIL)的血细胞中获得PBMC(外周血单核细胞),并且在体外或离体用Cbl-b-特异性siRNA减弱成套方案来进行处理。该方法非常快地进行。理想地,在将它们取出后仅几分钟就可将该细胞制备物再次施用给该患者。可选地,在将细胞再植入之前,可以通过适合于各自细胞的离体刺激方案来使所述细胞增殖或扩增。在患者的T-细胞群体中仅占几个百分点的在体外激活的T-细胞在它们于受者生物体中循环时在淋巴结中遇到抗原呈递细胞,所述抗原呈递细胞由于已发生的免疫接种而摄取了抗原并迁移到那里。由于经体外处理的T-细胞不需要共刺激信号,因而它们将会在识别免疫接种抗原之后立即增殖并分泌细胞因子,所述细胞因子全身性地有助于诱导细胞水平和体液水平上的免疫应答。采用这种成套方案,即使弱免疫原性的抗原也将会导致建立长期持续的接种保护。同样地,如此可以引起癌症患者中对自体肿瘤的排斥。在此,Cbl-b拮抗剂增强免疫反应性,其中Cbl-b拮抗剂要么在肿瘤学疾病的单独治疗中或者在肿瘤学疾病的相伴的化学疗法/放射疗法治疗中使用,要么与被动免疫接种(例如肿瘤抗原特异性抗体)相组合地使用。
因此,该方法适合于用于治疗先天性或获得性免疫缺陷,特别是AIDS,多发性骨髓瘤,慢性淋巴性白血病,药物引起的免疫抑制或者癌症疾病,可选地其中选择疾病特异性抗原。在此,特别优选的是治疗形成实体瘤的癌症疾病。
为了提高疗法的成功机会,癌症疾病的这种治疗优选地与其他抗肿瘤疗法相组合地进行,所述其他抗肿瘤疗法特别是化学疗法、放射疗法、施用治疗性生物制剂、或者由树突细胞所支持的(肿瘤)疫苗接种。在由树突细胞所支持的疫苗接种(优选地,抗肿瘤疫苗接种)的范围内,可以采用Cbl-b的抑制。备选地或另外地,也可以将Cbl-b受抑制的细胞与从患者中获得的树突细胞(其优选地荷载有肿瘤(细胞)抗原)进行体外共培养,以及将所述共培养物用于本发明的目的。
在体外或离体提高免疫反应性可以在上面所描述的治疗方法中进行,其中可在取出细胞之前就已经使细胞暴露于抗原或者在取出细胞之后使细胞暴露于抗原。
另外,在治疗性移植的情况下,下述的时间顺序是重要的:给予免疫接种抗原,所述免疫接种抗原被抗原呈递细胞摄取,以及进一步地这些细胞迁移至局部淋巴结,在那里所摄取的物质被呈递给已成为具反应性的T-细胞。因此,优选地,用所述抗原对所述患者进行接种,优选地还是在分离细胞之前,特别优选地在分离细胞之前至少1、2、3、4或5天和/或至多20、16、12、10、9、8、7、6、5、4、3、2或1周。备选地,也可以进行事后的疫苗接种或者在体外或离体用抗原处理所述细胞。
此外,还可以使用抗原呈递细胞,所述抗原呈递细胞优选地来源于患者自身并且可与相应的抗原相接触,然后与给予Cbl-b受抑制的免疫细胞(优选地,T-细胞)一起地,或者在给予Cbl-b受抑制的免疫细胞之前或之后不久地促成特定免疫反应性的提高。
优选地,对于某种抗原而言特异的细胞或者包含某种抗原的细胞就对于所述抗原的特异性或所述抗原的存在来进行选择,其中提高选择出的细胞的免疫反应性。通过选择某种抗原或者对其具有免疫增强特异性的细胞,可以使免疫反应靶向地指向患者中的某靶标。此类靶标特别是肿瘤(通过选择至少一种或更多种肿瘤抗原)或病原体。
可以设想,与细胞分离平行地,已将转染成套方案中的与之相应的siRNA置于同一个无菌的一次性使用小管中。因此,在另一个方面,本发明涉及优选地无菌的容器,例如一次性使用小管,其包含Cbl-b抑制剂,特别地用于提高免疫系统细胞针对抗原的免疫反应性。
同样地,本发明也计划了试剂盒,其包含用于容纳免疫系统细胞的容器,特别是一次性使用小管,以及Cbl-b抑制剂(如siRNA),所述试剂盒特别地用于浓照根据本发明的方法来提高免疫系统细胞针对抗原的免疫反应性。
所述容器或所述试剂盒另外还可以包含免疫刺激物质,优选地细胞因子或其他受体(例如TLR)的配体或针对表面分子(优选地,CD3和/或CD28)的抗体,以便额外地增强刺激。同样地,在稳定组分、稳定培养基或稳定缓冲液(用于细胞的稳定化)之外,所述试剂盒或所述容器还可以包含转染或核转染溶液,优选地细胞培养基,如RPMI或Optimem。

实施例2:Cbl-b表达的瞬时降低
在本实施例中表明,可离体影响T-细胞的免疫反应性。
利用CPT小管(Vacuta iner)从捐献者取全血,并通过离心从中分离出PBMC。在进一步的步骤中,从该制备物中富集CD8+细胞。使用Amaxa转染仪,用Cbl-b-特异性siRNA转染所述CD8+细胞(详细实验方案见实施例3),并继续培养。作为对照,采用相同的实验方案,用非特异性siRNA转染相同的一份细胞。由于估计到Cbl-b和c-Cbl的功能的潜在重叠,因而用c-Cbl-特异性siRNA以及由c-Cbl-特异性siRNA和Cbl-b-特异性siRNA组成的组合处理另外两份细胞。所有这些份细胞均培养两天。通过随后的Western印迹分析表明,所述转染导致所预期的Cbl-b表达的减弱。为了诱导Cbl-b表达,用CD3-特异性抗体刺激培养物,并且在另一份中用CD3-和CD28-特异性抗体进行刺激。在所有的实验中,在经转染的制备物中Cbl-b的表达均被压低至对照转染的强度的5%以下,如图2中所示的。由于siRNA的稳定性并因而有效的表达抑制具有有限的持续时间并且不转递至其他细胞,因而所选择的成套方案为Cbl-b表达的瞬时减弱,这与Cbl-b siRNA的存在严格相关。
实施例3:转染方案
在人T-细胞中用siRNA进行核转染(Nukleofektion)
与另一位实验室雇员一起进行核转染。一人承担siRNA Oligo的吸移工作,而另一雇员将样品转移到培养基中。这样显著地加快了该方案过程。
1.制备用于细胞-RPMI的培养基(+pen/strep.,+L-glut,+10%FCS)。
2.将培养基吸移到至少两个50ml Falcon小管/构建体中(一个用于收集经核转染的细胞,另一个用于杯池洗涤培养基),将1ml/l核转染样品吸移到每个小管中。将管带至37℃。
3.对于每种核转染样品,给Eppendorf管作上标记。
4.将经核转染的细胞离心(410g)7分钟并移去上清液。添加Optimem(Gibco,#31985-047)。
5.从而使得细胞密度为40×106/ml。将100μl(=4×106个细胞)吸移到每个Eppendorf小管中。
6.将1.5~2.5μM siRNA-Oligo添加到包含有即将进行核转染的细胞的Eppendorf小管中。通过吹吸进行混合,并将溶液转移到杯池(Küvette)中(避免气泡)。在桌上轻敲杯池以去除气泡。
7.合上盖子,并将杯池置于Amaxa转染装置(电穿孔仪)中。(将U-14而不是V-24制定为在使用Nukleofektor溶液Optimem情况下的最佳选择)。按下x-按钮并在OK信号之后移去杯池。(在下一次电穿孔之前再次按下x-按钮。)
8.立即向核转染杯池中添加500μl RPMI(37℃),并使用Pasteuer吸移管通过吹吸小心混合。将细胞转移到收集用Falcon小管中。用500μl经预先加热的RPMI清洗杯池一次,并将其余的细胞转移到收集用Falcon小管中。
9.用每一样品重复步骤5~7。
10.将Fa lcon小管置于培养箱中,直至所有样品均经核转染。
11.将4ml细胞悬浮液吸移到6-孔平板的孔中(=2份样品),并将平板置于培养箱中。
12.第二天,收集细胞,对其进行计数,并在如上所述进行的核转染后24小时开始培养。取一等分试样的细胞用于RNA分离,以便检查在激活的时刻靶基因是否被下调。基于蛋白质表达动力学,取蛋白质样品。

实施例5:T-细胞反应性的瞬时增强--测量IFN-γ
因此,表明了能够以至少95%的效率抑制人CD8+细胞中的Cbl-b表达。作为另一结果,现在将表明,同样可以增强T-细胞群体的反应性。为了确保所期望的效果完全是Cbl-b-特异性的并且在减弱Cbl-b表达的情况下不能通过c-Cbl而绕开,在另一份细胞中同样也减弱了c-Cbl,并且在第三份细胞中减弱了c-Cbl和Cbl-b。将所有这些CD8+细胞培养物保持于培养状态两天,并且一方面借助于CD3进行刺激,以及另外借助于CD3和CD28进行刺激,并且与未刺激的组进行比较。T-细胞通常需要CD3和CD28的共刺激以便进行增殖,这可以通过炎性细胞因子分泌到培养物上清液中而容易地检测出。为了掌握此T-细胞激活情况,测量了上清液中的IFN-γ滴度。在图3中描绘了通过siRNA处理而导致的表达减弱对于T-细胞反应性的影响的图解图示。在转染后2天,如所描述的,刺激用Cbl-b-和/或c-Cbl-特异性siRNA转染的所有CD8+细胞培养物,并与用非特异性siRNA转染的对照组进行比较。未刺激的细胞基本上不显示出IFN-γ表达。在仅CD3刺激后,所有培养物均显示出提高的反应性,从而可以在上清液中检测到至少500pg/ml IFN-γ。在此,关于c-Cbl-特异性siRNA和经非特异性转染的细胞的信号非常相似(低)。只有用Cbl-b-特异性siRNA转染的细胞显示出明显更高的反应性,这随着大约3ng/ml的IFN-γ滴度而出现。然而,用c-Cbl-和Cbl-b-特异性siRNA共转染的细胞制备物的反应性低于在Cbl-b-特异性siRNA处理后的反应性,只达到1.2ng/ml。在所有情况下,正如所预期的,CD3和CD28特异性共刺激产生比仅CD3特异性刺激明显更高的信号。用非特异性siRNA处理的对照制备物产生处于1.2ng/ml的量的IFN-γ滴度,而用Cbl-b-特异性siRNA处理的培养物具有3.8ng/ml的IFN-γ浓度。值得注意的是,经Cbl-b-和c-Cbl-特异性共减弱的培养物显示出处于1.7ng/ml的量的滴度,这因而明显低于Cbl-b被减弱的组中的滴度。也出乎意料的是,用c-Cbl-特异性siRNA处理的细胞群体具有显著更小的反应性,在上清液中仅测量出500pg/ml的IFN-γ。该浓度明显低于经非特异性处理的经共刺激的对照组的浓度,并且与仅用抗-CD3刺激的对照组的值相当。因此,与鼠类系统不同,通过实验排除了在人CD8+T-细胞中Cbl-b和c-Cbl的冗余,并且仅通过Cbl-b(及其上游调节物)而非经由Cbl-b/c-Cbl组合的治疗性成套方案对于提高免疫反应性来说是合适的。然而,抑制c-Cbl使得能够获得用于其他应用(例如,治疗炎症或变态反应)的免疫抑制。
此外,这种要么设计为单一疗法要么随疫苗接种一起进行的、相伴的治疗性成套方案能够识别也在外周中的散布的肿瘤细胞,并通过建立免疫应答而持久地对抗这些肿瘤细胞。若在诊断出癌症疾病后立即使用,由此同样也防止原发性肿瘤的散布。
实施例6:T-细胞反应性的瞬时增强--测量IL-2
通过测量相同上清液中的IL-2浓度产生了非常类似的情景,如从图4中可以推断出的。无刺激时没有可测量的应答,而抗-CD3特异性处理在所有组中导致明显的可测量的信号。因此,在Cbl-b被减弱的组中测量出>200pg/ml。在Cbl-b和c-Cbl被共减弱的群体中IL-2浓度却明显更低,为<100pg/ml。抗-CD3和抗-CD28特异性共刺激则产生更高的信号。不依赖于siRNA处理,在所有组中测量到>800pg/ml的IL-2浓度。对照组所产生的滴度与Cbl-b被减弱的组的滴度非常相似,分别为1.3和l.4ng/ml。有趣的是,IL-2浓度在c-Cbl-特异性减弱后明显更低,仅为800pg/ml,无论在该成套方案中是仅唯一使用了c-Cbl还是将c-Cbl与Cbl-b一起使用。
实施例7:免疫反应性的瞬时增强
通过所述体外或离体处理有效地增强了T-细胞反应性,从而可能通过仅刺激T-细胞受体来诱导T-细胞的增殖。这对于所描述的治疗性成套方案来说是基本前提条件。由于使用了RNAi技术,这种减弱具有瞬时的性质。
这些经修饰的细胞的适用性有助于增强疫苗的免疫原性,以及普遍提高免疫系统的反应性。基础免疫接种后五天,从患者中取全血至CPT小管中。大约20分钟后,通过离心从中分离出PBMC。用Cbl-b-特异性siRNA转染该细胞制备物,之后立即将其重新再植入患者中。第10天,再次抽取全血,并从中获得血清。测量血清中的促炎细胞因子滴度,并且与对照组进行比较。同样地,就Th1控制的免疫应答的其细胞调整(升高的IFN-γ、IL-2和IL-12滴度)或Th2指导的免疫应答的体液趋向(升高的IL-4、IL-5和IL-10滴度)而分析了所诱导的免疫应答的性质。如有必要,以14天间隔进行进一步的加强免疫接种,有或没有PBMC处理(图5)。
实施例8:CD4 T-细胞和CD19B-细胞的核转染
如上所述分离出PBMC,并且在与实施例3中相同的条件(U14)下进行转染。用于转染的Oligo(siGLO Red)以2μM的浓度进行使用,并借助于FACS来检测特异性摄取,通过使用CD8-FITC和CD3-APC的同时双重染色来区分CD4和CD8细胞(图6A)。
如上所述制备PBMC,并分离出其中的CD8细胞。然后,如上所述用siGLO Red再次转染其余的CD8阴性细胞(然而,Oligo浓度为3.3μM)。通过使用CD3-FITC和CD19-APC的同时双重染色,借助于FACS来检测特异性摄取(图6B)。有趣的是,在该实验中也观察到,在CD3/CD19阴性淋巴细胞级分(其主要由NK-细胞组成)中同样也发生了所使用的Oligo的有效摄取。
因此,本实施例表明,在与CD8细胞相同的转染条件下,其他免疫细胞也能够被高效地转染。
实施例9:在mRNA和蛋白质水平上,人CD4细胞中Cbl-b表达的瞬时降低
通过耗竭CD8细胞而从PBMC中分离出CD4细胞,并借助于PHA/抗-CD3/28刺激来进行培养。2周后,使用Amaxa转染方案(参见实施例3和8),用Cbl-b-特异性siRNA转染所述CD4细胞。使用相同的方案,用非特异性siRNA转染相同的一份细胞作为对照。在转染后,用IL-2(5ng/ml)将所述细胞再培养一天,并在第二天用抗-CD3/28抗体进行刺激。
与对照转染相比较,在经过24小时刺激的CD4细胞中,经转染的制备物中的Cbl-b mRNA表达降低了大约85%(图7A)。在此,Cbl-b mRNA的剧烈减少与Cbl-b蛋白量的相对剧烈的减少(如在Western印迹中所检测的(图7B))相关联。
实施例10:CD4T-细胞反应性的增强--测量具有抗肿瘤活性的细胞因子
在由T-细胞介导的免疫应答中,CD4细胞的主要任务之一是产生炎性细胞因子。文献中提到细胞因子IL-2、IFN-γ和TNF-α对于抗肿瘤活性而言特别重要。
因此,利用ELISA测定了这三种细胞因子的表达。在抗-CD3/28刺激后24小时,在人的Cbl-b被沉默的CD4T-细胞中这三种细胞因子显著提高(图8)。
实施例11:通过具有抗肿瘤活性的细胞因子的产生增加来增强CD4T-细胞反应性的瞬时过程
为了达到Cbl-b被沉默的T-细胞的有效抗肿瘤活性,重要的是,在T-细胞刺激后在一段时间段期间仍然还保持增加的细胞因子产生。然而,这一时间段也应当受到限制,以便使针对患者的非恶性组织的不希望的自身免疫的持久建立这一风险降至最低。
因此,还在不同时间点,在FACS中借助于细胞内染色分析了IFN-γ的产生。图9中的图清楚地表明了,IFN-γ产生的剧烈提高保持了至少48小时,但在刺激后6天,其重新落回到与对照中相当的值。
实施例12:T-细胞反应性的增强--具有功能特性或刺激标志物功能的表面分子的表达的提高
分别测定作为关于在人T-细胞中Cbl-b的在功能上成功沉默的标志物的细胞因子产生在技术上是更费时费力的,并因而按趋势可以不如此同期地进行。因此,借助于FACS也测定了在功能上更重要的表面标志物的表达。
在文献中将CD107a定义为关于细胞毒性T-细胞的分泌活性的表面标志物,因此在24小时的抗-CD3/28刺激后在Cbl-b受抑制的CD8T-细胞上进行了测定。在此,用Cbl-b siRNA转染的T-细胞表现出大大提高的分泌活性(图10A)。
由于CD107a分子参与了T-细胞中的囊泡输运,因而其也可被用作关于CD4T-细胞的分泌活性的主要标志物。图10B显示,与在其他方面经相同处理的对照细胞相比较,在用Cbl-b siRNA转染的细胞中CD107a的表达也是显著的。
另外两种通过T-细胞刺激可诱导的表面分子为CD40L和ICAM。这两种分子具有在功能上的重大意义,CD40L特别是对于与抗原呈递细胞的相互作用以及对于B细胞的刺激/增殖具有重要意义,ICAM对于与抗原呈递细胞的相互作用和从血管系统中迁移到(恶性)组织中具有重要意义。图10B显示,在用Cbl-b siRNA转染的人CD4T-细胞中,这两种表面分子的表达显著提高。
所描述的负责T-细胞反应性的提高的机制之一是在细胞表面上存在的CD3受体的较不显著的减弱。图10B显示,在用Cbl-b siRNA转染的人CD4T-细胞中,仍存在于细胞表面上的CD3受体的数量也明显提高。
有趣的是,传统的T-细胞激活标志物CD69的细胞表面表达也提高了,而CD25的表达保持不变。这可能具有特别的在功能上的重大意义,因为尽管CD25也被定义为刺激标志物,但其功能与所谓的T-调节细胞的存在和存活特别相关。
总之,因此,图10表明,通过Cbl-b siRNA转染,可以在细胞表面上以明显更大的量检测到其他在功能上重要的或者可用作表面标志物的分子。
实施例13:在体内肿瘤模型中,Cbl-b缺陷型T-细胞和树突细胞的共同转移是有效的治疗方法。
在野生型小鼠中通过皮下注射10万个EG7ova细胞而诱导了肿瘤。然后,在第5和6天,注射CD8T-细胞和树突细胞,并且通过测量肿瘤生长来连续追踪该所采取的细胞疗法的效果。图11A显示,相比于通过野生型T-细胞所达到的而言,通过转移Cbl-b缺陷型T-细胞可以显著更强地和更持久地抑制肿瘤生长。
此外,图11B还表明,用Cbl-b缺陷型T-细胞进行的疗法可以保证很大一部分的经治疗的小鼠长期存活,直至80天的观察期结束。与此相反,与对照组相比较,用野生型T-细胞进行的治疗仅导致轻微的生命延长。因此,图11B表明,相比于用正常的T-细胞进行的疗法而言,用Cbl-b缺陷的或Cbl-b受抑制的T-细胞进行的疗法具有显著的优点。
Claims (22)
1.用于提高接触过抗原的免疫系统细胞的免疫反应性的体外或离体方法,包括降低或者抑制这些细胞的Cbl-b功能,从而提高所述细胞针对该抗原的免疫反应性。
2.根据权利要求1的方法,其特征在于,通过降低或抑制Cb l-b的表达来降低或抑制Cbl-b功能。
3.根据权利要求1或2的方法,其特征在于,Cbl-b的功能的降低或抑制是瞬时的。
4.根据权利要求1~3中任一项的方法,其特征在于,通过使用Cbl-b反义RNA或者s iRNA来降低成抑制Cbl-b的表达。
5.根据权利要求1~4中任一项的方法,其特征在于,通过使用Cbl-b拮抗剂、抑制剂、适配体或Intramer来降低或抑制Cbl-b的功能。

10.Cbl-b抑制剂或拮抗剂在制备药物组合物中的用途,所述药物组合物用于通过下述方式来提高患者中针对抗原的免疫反应性:分离患者的免疫系统细胞,通过使用Cbl-b抑制剂或拮抗剂在体外或离体提高所述细胞的免疫反应性,并且将所述细胞再植入所述患者中,其中通过降低或抑制所述细胞的Cbl-b功能来提高免疫反应性。
11.根据权利要求10的用途,用于治疗先天性或获得性免疫缺陷,特别是AIDS,多发性骨髓瘤,慢性淋巴性白血病,药物引起的免疫抑制或者癌症疾病。

17.根据权利要求10~16中任一项的用途,其特征在于,对于某种抗原而言特异的细胞或者包含某种抗原的细胞就对于所述抗原的特异性或所述抗原的存在来进行选择,并且提高选择出的细胞的免疫反应性。
18.根据权利要求17的用途,其特征在于,所述抗原为肿瘤抗原。
19.根据权利要求1~18中任一项的用途,其特征在于,所述细胞在再植入之前进行扩增。
20.容器,优选地一次性使用小管,其包含Cbl-b抑制剂,特加地用于提高免疫系统细胞针对抗原的免疫反应性,优选地如权利要求1~9中任一项所定义;或者用于权利要求10~19中任一项的用途。
21.用于实施权利要求1~9的方法或者用于权利要求10~19的用途的试剂盒,其包含用于容纳细胞的容器,优选地一次性使用小管,Cbl-b抑制剂,和优选地细胞培养基和/或转染缓冲液。
22.根据权利要求20的容器或者根据权利要求21的试剂盒,其包含免疫刺激物质,优选地细胞因子,特别是CD3和/或CD28。
