技术领域
本发明涉及含有来源于植物、动物或矿物原料药的医用配制品,特别涉及一种治疗颅脑外伤后遗性头痛的中药制剂及其制备方法。
背景技术
颅脑外伤后遗性头痛症多属瘀血头痛。中医学认为,头为诸阳之会,五脏六腑精华清阳之气均汇集于此。笔者通过多年的临床观察,颅脑外伤患者多因颅内血肿或开颅减压术后,对颅脑神经及细胞存在着不同程度的损伤,瘀血停积,阻塞脑络而引起头痛。颅脑损伤的发生率占全身损伤的10%~15%,仅次于四肢骨折,头痛是颅脑外伤后最常见的症状之一,持续性的头痛往往会给患者带来巨大的痛苦。
颅脑外伤后头痛,发生的机理尚未完全明了。现代医学认为:部分病人是在颅脑器质性损害之后的病理基础上引起的,如蛛网膜下腔出血,或炎症致蛛网膜粘连,囊肿压迫,脑脊液循环障碍,脑血管和神经根间接地受累。另外,脑实质性损伤后导致脑皮质功能弱化,兴奋与抑制失调,皮质下中枢调节功能紊乱,血脑屏障的破坏,钾离子运转失常,脑膜或脑实质瘢痕形成,以及脑外伤后神经细胞的退行性变等均是导致颅脑外伤后头痛不可忽视的因素。
祖国医学认为颅脑外伤后头痛属中医“头痛”的范畴。“头为诸阳之会”,又为“髓海所在”,主要依赖肝肾精血濡养及脾胃运化水谷精微,输布气血上充于脑,则耳聪目明,神清气爽;脑外伤后瘀血阻滞,脉络瘀阻,脑失所养,气血逆乱,变生诸症。一般认为本病有虚实之分,实证多责之为气滞血瘀、湿滞血瘀等,多从行气活血、祛瘀等方面立法;虚证有气血阴阳之别,治疗多从补益入手。笔者认为本病迁延较久,以虚证居多,过多应用活血化瘀之剂并未取得较好效果,特别是对一些久治不愈患者,反使正气一伤再伤,有犯“虚虚”之忌。《内经》有“脑为髓之海,髓海有余,则轻劲有力,自过其度;髓海不足,则脑转耳鸣,胫酸眩晕,目无所见,懈怠安卧。”脑外伤后髓海震损,又因久治不愈,阴血暗耗,故见则头痛隐隐时而剧痛,经久不愈,甚至导致脑转耳鸣,胫酸眩晕,精神懈怠。故治宜滋阴养血为主,辅以活血行气止痛之药。
颅脑外伤病人在恢复期后,仍有约25%的患者还遗留某些症状,头痛往往是最主要症状,头痛可以暂时也可以长久地影响生活、工作,甚至丧失劳动能力。不少人甚至部分医务人员也认为脑外伤后头痛属于不治之症或无较好疗法,对病人造成严重的心理负担和和巨大的痛苦;目前治疗颅脑外伤后遗性头痛药物尚存在许多不足,如西医治疗仍以化学药物治疗为主,治疗周期长,副作用大,疗效不确切,且不易坚持,中医传统治疗方法也没有专门的治疗颅脑外伤后遗性头痛中药问世。而中医千百年来的临床积累,有许多对颅脑外伤后遗性头痛行之有效的方药。
发明内容
本发明所要解决的技术问题在于,提供了一种治疗颅脑外伤后遗性头痛的中药制剂及其制备方法,利用各药物的功效产生协同作用,能够起到去痛解痹,除烦安神、养心补虚、安神益智、化痰通窍、养心护肝、行气解郁、解除忧郁、焦虑、失眠之功效,从而有效地治疗颅脑外伤后遗性引起的头痛。

所述治疗颅脑外伤后遗性头痛的中药制剂,所述药物剂型可以为:蜜炼丸剂、水合丸剂、胶囊、口服液、颗粒剂、胶剂、糖浆剂、针剂或滴剂;在煎汤剂服用时,可以每日1剂,水煎分两次服,每7天为一疗程。
为解决上述技术问题,本发明还提供了一种治疗颅脑外伤后遗性头痛的中药制剂的制备方法,所述中药制剂剂型为蜜炼丸剂,其制备方法包括:将所述原料药加水浸泡30分钟至1小时,加热煮沸30分钟,过滤,滤液备用;滤渣加水,第二次加热,煮沸30分钟,再过滤,滤液备用;滤渣再加水,第三次加热煮沸40分钟,过滤;将三次滤液合在一起,加热浓缩至糊状,放入烘箱,控制在80℃,烘干后冷却,与蜂蜜混在一起搓成细条;腊封包装。
为解决上述技术问题,本发明又提供了一种治疗颅脑外伤后遗性头痛的中药制剂的制备方法,所述中药制剂剂型为胶囊剂,其制备方法包括:
a.将全蝎,蜈蚣按照所述原料药重量份数比放入乙醇中加热回流提取;
b.再取所述中药制剂的剩余原料药,加水加热回流提取;
c.将上述两种提取液合并,减压回收乙醇,浓缩抽滤吸附,得到药粉,装入胶囊制为胶囊剂。
所述步骤a中,优选将所述全蝎,蜈蚣放入乙醇中,所述乙醇的用量为所述全蝎和蜈蚣重量之和的10倍量,加热回流提取2次,每次30分钟~1小时,将2次提取液合并静置。
所述步骤b中,优选将剩余原料药放入水中,所述水的用量为所述剩余原料药重量之和的10倍量,加热回流提取3次,每次30分钟~1小时,将3次提取液合并静置。
所述步骤c中,优选将所述步骤a、b中两种静置后的提取液合并,减压回收乙醇,浓缩至药液浓度为0.5g生药/mL,抽滤至滤液的相对密度为20℃时1.08;上述滤液经体积为10L的大孔吸附树脂柱吸附,后用10倍树脂柱体积的去离子水或蒸馏水洗脱,再用5倍树脂柱体积的95%乙醇洗脱,收集乙醇洗脱液,去除溶剂,得到药粉,装入胶囊制为胶囊剂。每粒胶囊可以装0.3g。
本发明有益的技术效果在于,与现有技术相比教,本发明具有给药方便,药剂易于制造,成本低廉等优点。本发明采用不同药性的中药材,进行了科学配伍,能达到理想的治疗效果,且安全,毒副作用小,使用方便,治愈时间短,治愈后不易复发。能创造较好的社会价值及经济利益。

颅脑外伤性头痛临床比较多见,常为脑挫裂伤、脑震荡和脑外伤后综合征引起。据《内经》之“有所堕坠,恶血留内”的理论,故应从瘀论治。瘀除脉通,通则不痛。颅脑损伤的发生率占全身损伤的10%~15%,仅次于四肢骨折,头痛是颅脑外伤后最常见的症状之一,持续性的头痛往往会给患者带来巨大的痛苦。颅脑外伤后头痛,发生的机理尚未完全明了。现代医学认为:部分病人是在颅脑器质性损害之后的病理基础上引起的,如蛛网膜下腔出血,或炎症致蛛网膜粘连,囊肿压迫,脑脊液循环障碍,脑血管和神经根间接地受累。另外,脑实质性损伤后导致脑皮质功能弱化,兴奋与抑制失调,皮质下中枢调节功能紊乱,血脑屏障的破坏,钾离子运转失常,脑膜或脑实质瘢痕形成,以及脑外伤后神经细胞的退行性变等均是导致颅脑外伤后头痛不可忽视的因素。
祖国医学认为颅脑外伤后头痛属中医“头痛”的范畴。“头为诸阳之会”,又为“髓海所在”,主要依赖肝肾精血濡养及脾胃运化水谷精微,输布气血上充于脑,则耳聪目明,神清气爽;脑外伤后瘀血阻滞,脉络瘀阻,脑失所养,气血逆乱,变生诸症。一般认为本病有虚实之分,实证多责之为气滞血瘀、湿滞血瘀等,多从行气活血、祛瘀等方面立法;虚证有气血阴阳之别,治疗多从补益入手。补虚当有法度,做到补泻兼施。肝肾属阴,脾亦属阴。脾为后天之本,亦当补益,以增强其运化之力。此三阴同补,可谓“壮水之主,以制阳光。”补中有泻,则可宣泄肾浊,平降肝火,渗湿健脾,去除滋阴药腻胃滞脾的弊端。再则,补血当先补气。因气为血之帅,血为气之母,气为血之子,气行则血行。故方中精选补气药物,实现气血双补。笔者认为本病迁延较久,以虚证居多,过多应用活血化瘀之剂并未取得较好效果,特别是对一些久治不愈患者,反使正气一伤再伤,有犯“虚虚”之忌。只宜平补平泻,不可猛补峻泻。当是缓补及里,变虚亏为充实。总之“,从阴阳则生,逆之则死。”本发明主要从内因着手,滋肝健脾益肾,益气补血,达到调经养血,恢复人体正常内循环的作用。经症或称脑震荡后综合
方中熟地黄滋阴养血;黄芪补气,气旺则阴血自生;丹参清心活血,使补而不滞;香附、羌活行气止痛;川芎,三七辛温,擅疏通,上行头目,下行血海,为血中气药,为止痛圣药;酸枣仁、柏子仁养心安神;当归补血润燥,通经化瘀;五味子益气敛阴,助补气生阴之力;牡丹皮滋阴润燥、清热除烦;茯苓、远志养心安神;红花载药上行,使药力上行,可行气血,使诸药滋而不腻,补不留瘀;蜈蚣,全蝎以毒化瘀;诸药合用补行兼施,共奏益气滋阴养血,行气活血止痛之功。
丹参苦,微寒。归心、肝经。祛瘀止痛,活血通经,清心除烦。用于月经不调,经闭痛经,症瘕积聚,胸腹刺痛,热痹疼痛,疮疡肿痛,心烦不眠。用于胸肋胁痛,风湿痹痛,症瘕结块,疮疡肿痛,跌仆伤痛,月经不调,经闭痛经,产后瘀痛等。治疗胸肋疼痛、症瘕结块,以及月经不调、经闭经痛具有良效。.《纲目》载:丹参,按《妇人明理论》云,四物汤治妇人病,不问产前产后,经水多少,皆可通用,惟一味丹参散,主治与之相同。盖丹参能破宿血,补新血,安生胎,落死胎,止崩中滞下,调经脉。是活血化瘀,去痛之良药。
当归:其味甘而重,故专能补血,其气轻而辛,故又能行血,补中有动,行中有补,诚血中之气药,亦血中之圣药。大约佐之以补则补,故能养营养血,补气生精,安五脏,强形体,益神志,凡有形虚损之病,无所不宜。佐之以攻则通,故能祛痛通便,利筋骨,治拘挛、瘫痪、燥、涩等证。营虚而表不解者,佐以柴、葛、麻、桂等剂,大能散表卫热,而表不敛者,佐以大黄之类,又能固表。惟其气辛而动,故欲其静者当避之,性滑善行,大便不固者当避之。凡阴中火盛者,当归能动血,亦非所宜,阴中阳虚者,当归能养血,乃不可少。
黄芪以补虚为主,能补气固表,利尿托毒,排脓,敛疮生肌。用于气血不足、疮疡内陷、脓成不溃或久溃不敛者。黄芪具有很好的托毒生肌的功能,即久不愈合的脓肿化脓生肌。现代医学研究表明,黄芪内含而多种抗菌有效成分,而且能增强机体的免疫功能,因此还能用于预防某些传染病的发生。《本草逢原》载:“黄芪能补五脏诸虚,治脉弦自汗,泻阴火,去肺热,无汗则发,有汗则止。”是增进抵抗力和防御疾病的良药。
白芷性温味辛,归肺、胃经。功效:祛风散寒,通窍止痛,消肿排脓,燥湿止带。功能主治祛风湿,活血排脓,生肌止痛。用于头痛、牙痛、鼻渊、肠风痔漏、赤白带下、痈疽疮疡、皮肤瘙痒。祛风,燥湿,消肿,止痛。治头痛,眉棱骨痛,齿痛,鼻渊,寒湿腹痛,肠风痔漏,赤白带下,痈疽疮疡,皮肤燥痒,疥癣。《本经》载其:主女人漏下赤白,血闭阴肿,寒热,风头(头风)侵目泪出,长肌肤,润泽。《别录》:疗风邪久渴(久渴或疑作′久泻′),呕吐,两胁满,风痛头眩,目痒。《药性论》:治心腹血刺痛,除风邪,主女人血崩及呕逆,明目、止泪出,疗妇人沥血、腰腹痛;能蚀脓。是去痛化瘀的良药。
三七性温,味甘、微苦。性温,归肝、胃经,以根、根状茎入药。是名贵中药材,生用可止血化瘀、消肿止痛,是云南白药主要成分,同棵植物的花叶也能入药,当茶饮。功能主治:散瘀止血,消肿定痛。用于各种内、外出血,胸腹刺痛,跌扑肿痛。(1)具有良好的止血功效、显著的造血功能;(2)能加强和改善冠脉微循环,扩三七入药历史悠久,作用奇特被历代医家视为药中之宝,故有“金不换”之说法。三七“味微甘而苦,颇似人参之味。”“凡杖扑伤损,瘀血淋漓者,随即嚼烂罨之即止,青肿者即消散。若受杖时,先服一、二钱,则血不冲心,杖后尤宜服之,产后服亦良。大抵此药气温,味甘微苦,及阳明、厥阴血分之药,故能治一切血病”。《本草纲目新编》记载:“三七根,止血之神药也。无论上、中、下之血,凡有外越者,一味独用亦效,加入于补气补血药中则更神。盖此药得补而无沸腾之患,补红得此而有安静之休也。”
夜交藤养心,安神,通络,祛风,治失眠症,劳伤,多汗,血虚身痛,痈疽,瘰疬,风疮疥癣。甘微苦,平。《本草再新》:“味苦,性温,无毒。”《饮片新参》:“苦涩微甘。”《陕西中草药》:“性平,味甘。”入心、脾、肾、肝经配酸枣仁,滋心阴,宁心神;配生地,养血补阴;配天门冬、麦冬,清虚火、养心阴;配羌活、独活,祛风胜湿、舒利关节。
柏子仁性平味甘,心、肾、大肠经。具养心安神、润肠通便的功效。治惊悸、失眠、遗精、盗汗、便秘等症,能养心安神,润肠通便。用于虚烦不眠,心悸怔忡,肠燥使秘等症。《药品化义》:柏子仁,香气透心,体润滋血。同茯神、枣仁、生地、麦冬,为浊中清品,主治心神虚怯,惊悸怔仲,颜色憔悴,肌肤燥痒,皆养心血之功也。又取气味俱浓,浊中归肾,同熟地、龟版、枸杞、牛膝,为封填骨髓,主治肾阴亏损,腰背重病,足膝软弱,阴虚盗汗,皆滋肾燥之力也。味甘亦能缓肝,补肝胆之不足,极其稳当,但性平力缓,宜多用之为妙。
鸡血藤性苦、甘,温。归肝、肾经,补血,活血,通络。用于月经不调,血虚萎黄,麻木瘫痪,风湿痹痛活血舒筋;养血调经,《本草纲目拾遗》载其能医手足麻木;肢体瘫痪;风湿痹痛;凌霄花,鸡血藤内服是治疗血淤不散,养血化淤活血的良药。
羌活辛、苦,温。归膀胱、肾经。功能散寒,祛风,除湿,止痛。用于风寒感冒头痛,风湿痹痛,肩背酸痛。羌活功能发散风寒,祛风止痛,用于感冒风寒,兼有头痛、身痛为主,常配防风、白芷等药同用。羌活祛风湿的作用也甚为显著,为祛风胜湿常用之品,但一般认为本品以风湿痹痛在身半以上者为宜,如周身痹痛,可配防风、独活等同用。对于头痛病症,多配合川芎、细辛等应用。作为发汗解表药时,应将它的祛风止痛功效密切结合起来,即在临床上用于风寒表症时,必须兼有头痛或骨节疼痛等症,才考虑使用。至于用治风湿痹痛,不论有无表症,都可应用。根据近年来临床实践体会,本品退热的功效很好,可配合清热药如蒲公英、板蓝根等品治风热表症,而且一般在热退之后无再度发热现象。但本品气味浓烈,用量过多,易致呕吐,故在使用时必须注意患者的胃那情况,掌握适当剂量。
防风味辛、甘,性微温。归膀胱、肝、脾经。祛风解表,胜湿止痛,止痉。外感表证,风疹瘙痒,风湿痹痛,破伤风。用于感冒头痛,风疹瘙痒。治风寒表证,头痛身痛、恶风寒者,常配伍荆芥、羌活、独活等;治外感风湿,头痛如裹、身重肢痛者,与羌活、藁本等同用;治风疹瘙痒,多配伍苦参、荆芥、当归等。防风,古代名“屏风”(见《(名医别录》),喻御风如屏障也。其味辛甘,性微温而润,为“风药中之润剂”。临床随证配伍,具有不同的双向作用,兹琐志之。能发汗,又能止汗“用防风必兼荆芥者,以其能入肌肤宣散故耳”(见《本草求真》),“若属外感证,用麻桂嫌热、嫌猛;用银翘嫌寒时,荆防用之最宜”(见《施今墨对药临床经验集》),可见荆芥与防风相配有达腠理、发汗散邪之效,二者相辅相成。
川芎辛,温。入肝、胆经。行气开郁,法风燥湿,活血止痛。治风冷头痛旋晕,胁痛腹疼,寒痹筋挛,经闭,难产,产后瘀阻块痛,痈疽疮疡。用于月经不调,经闭痛经,瘕腹痛,胸胁刺痛,跌扑肿痛,头痛,风湿痹痛。《本经》载其:主中风入脑头痛,寒痹,筋挛缓急,金创,妇人血闭无子。《别录》载其:″除脑中冷动,面上游风去来,目泪出,多涕唾,忽忽如醉,诸寒冷气,心腹坚痛,中恶,卒急肿痛,胁风痛,温中内寒。″是行气化瘀的良药。
远志苦、辛,微温。归心、肾、肺经。功能主治安神益智,祛痰,消肿。用于心肾不交引起的失眠多梦,健忘惊悸,神志恍惚,咳痰不爽,疮疡肿毒,乳房肿痛。应用于失眠多梦,心悸怔忡,健忘。本品苦辛性温,性善宣泄通达,既能开心气而宁心安神、又能通肾气而强志不忘,为交通心肾、安定神志、益智强识之佳品。主治心肾不交之心神不宁、失眠、惊悸等症,常与茯神、龙齿、朱砂等镇静安神药同用,如远志丸(《张氏医通》);治健忘证,常与人参、茯苓、菖蒲同用,如开心散(《千金方》),若方中再加茯神,即不忘散(《证治准绳》)。《本草正》:远志,功专心肾,故可镇心止惊,辟邪安梦,壮阳益精,强志助力。以其气升,故同人参、甘草、枣仁,极能举陷摄精,交接水火
熟地性味甘,性微温。归肝、肾经。主治具有补血滋阴功效用于血虚萎黄,眩晕,心悸失眠,月经不调,崩漏等症。可用于肾阴不足的潮热骨蒸、盗汗、遗精、消渴等,如六味地黄丸。补精益髓功效用于肝肾精血亏虚的腰膝酸软,眩晕耳鸣,须发早白等。熟地味甘微温质润,既补血滋阴,又能补精益髓。如熟地配以当归、白芍、川芎就是大名鼎鼎的治疗血虚症的“四物汤”。熟地配白芍能养肝,配柏子仁养心,配龙眼能养脾。
香附辛微苦甘,平,入肝、三焦经功能主治:理气解郁,调经止痛。用于肝郁气滞,胸、胁、脘腹胀痛,消化不良,月经不调,经闭痛经,寒疝腹痛,乳房胀痛。《本草述》:香附,主治诸证,当审为血中之气病,乃中肯,不漫同于诸治气之味也。......故上焦心包络所生病,如七情抑郁者能开之,以心包络主血也;中焦脾胃所生病,如霍乱吐逆及饮食积聚、痰饮痞满能畅之,以胃生血,脾统血也;下焦肝肾所生病,如膀胱连胁下气妨,如下血、尿血及女子崩漏、带下、月候不调等证,亦以胃脾为血之元,肝固血之脏,肾乃血之海也。
五味子温;酸、甘;归肺、心、肾经。收敛固涩,益气生津,补肾宁心。用于久嗽虚喘,梦遗滑精,遗尿尿频,久泻不止,自汗,盗汗,津伤口渴,短气脉虚,内热消渴,心悸失眠。果实作中药功能益气生津、敛肺滋肾、止泻、涩精、安神,可治久咳虚喘、津少口干、遗精久泻、健忘失眠等症。药理试验证明能调节中枢神经系统的兴奋和抑制过程,促进肌体代谢,调节胃液和胆液分泌,对肝炎恢复期转氨酶升高者有降低作用。
酸枣仁养肝,宁心,安神,敛汗。治虚烦不眠,惊悸怔忡,烦渴,虚汗。《本经》:主心腹寒热,邪结气聚,四肢酸疼,湿痹。《别录》:主烦心不得眠,脐上下痛,血转久泄,虚汗烦渴,补中,益肝气,坚筋骨,助阴气,令人肥健。治虚劳虚烦,不得眠治心脏亏虚,神志不守,恐怖惊惕,常多恍惚,易于健忘,睡卧不宁,梦涉危险,一切心疾《纲目》:酸枣仁,甘而润,故熟用疗胆虚不得眠,烦渴虚汗之证;生用疗胆热好眠。皆足厥阴、少阳药也,今人专以为心家药,殊昧此理
红花性温,味辛。功能主治应用于活血通径、散瘀止痛。用于经闭、痛经、恶露不行、症瘕痞块、跌打损伤。《本草汇言》:红花,破血、行血、和血、调血之药也。主胎产百病因血为患,或血烦血晕,神昏不语;或恶露抢心,脐腹绞痛;或沥浆难生,;或胞衣不落,子死腹中,是皆临产诸证,非红花不能治。若产后血晕、口噤指搦;或邪入血室,谵语发狂;或血闷内胀,僵仆如死,是皆产后诸证,非红花不能定。凡如经闭不通而寒热交作,或过期腹痛而紫黑淋漓,或跌扑损伤而气血瘀积,或疮疡痛痒而肿溃不安,是皆气血不和之证,非红花不能调。
鸡血藤性苦、甘,温。归肝、肾经,补血,活血,通络。用于月经不调,血虚萎黄,麻木瘫痪,风湿痹痛活血舒筋;养血调经,《本草纲目拾遗》载其能医手足麻木;肢体瘫痪;风湿痹痛;妇女有经不调;痛经。是活血良药。
全蝎辛,平;有毒。归肝经。功能息风镇痉,攻毒散结,通络止痛。用于小儿惊风,抽搐痉挛,中风口歪,半身不遂,破伤风,风湿顽痹,偏正头痛,疮疡,瘰疬。对脑神经的作用:蝎毒毒素对大鼠脑神经细胞线粒体的作用蝎毒毒素对鼠脑神经细胞线粒体结构和功能具有显著的影响。初分毒素(SVc)抑制线粒体细胞色素氧化酶和琥珀酸脱氢酶的活力,降低呼吸控制率(RCR)、氧化磷酸化效率(ADP/0)和耗氧速率(Q02),增加线粒体膜的流动性。蝎毒毒素多肽SV III与线粒体短时间作用,明显抑制线体的Q02,所需浓度为SVc的1/10左右,如果它与线粒体长时间作用,则有延缓线粒体老化的效应,减慢线粒体因放置而导致的Q02降低速度。SVc和SVIII对鼠脑神经细胞线粒体作用的大小不与加入的浓度成正比关系,仅在某一特定的浓度区域呈现最强的作用。《本草求真》载其:全蝎,专入肝祛风,凡小儿胎风发搐,大人半边不遂,口眼眼斜,语言蹇涩,手足搐掣,疟疾寒热,耳聋,带下,皆因外风内客,无不用之。
蜈蚣成、辛,温。有毒。归肝、脾、肺经。功能主治息风镇痉,功毒散结,通络止痛。用于小儿惊风,抽搐痉挛,中风口歪,半身不遂,破伤风,风湿顽痹,疮疡,瘰疬,毒蛇咬伤,败毒抗癌、息风解痉、退炎治疮,是活血化瘀的良药。
茯苓甘、淡,平。归心、肺、脾、肾经。功能利水渗湿,健脾宁心。用于水肿尿少,痰饮眩悸,脾虚食少,便溏泄泻,心神不安,惊悸失眠。渗湿利水,益脾和胃,宁心安神。治小便不利,水肿胀满,痰饮咳逆,呕哕,泄泻,遗精,淋浊,惊悸,健忘。《本经》载其:″主胸胁逆气,忧恚惊邪恐悸,心下结痛,寒热烦满,咳逆,口焦舌干,利小便。″《别录》:″止消渴,好睡,大腹,淋沥,膈中痰水,水肿淋结。开胸腑,调脏气,伐肾邪,长阴,益气力,保神守中。″《药性论》:″开胃,止呕逆,善安心神。主肺痿痰壅。治小儿惊痫,心腹胀满,妇人热淋。″
牡丹皮治热入血分,发斑,惊痫,吐、衄、便血,骨蒸劳热,经闭,症瘕,痈疡,扑损。清肝经之热,用于治疗肝经郁热所致的胁痛、头痛、月经不调。活血化瘀,治疗血瘀肿块,经闭不通,腹部胀痛。《本草经疏》:牡丹皮,其味苦而微辛,其气寒而无毒,辛以散结聚,苦寒除血热,入血分,凉血热之要药也。寒热者,阴虚血热之候也。中风瘛疭、痉、惊痫,皆阴虚内热,营血不足之故。热去则血凉,凉则新血生、阴气复,阴气复则火不炎而无因热生风之证矣,故悉主之。痈疮者,热壅血瘀而成也。凉血行血,故疗痈疮。辛能散血,苦能泻热,故能除血分邪气,及症坚瘀血留舍肠胃。脏属阴而藏精,喜清而恶热,热除则五脏自安矣。
本发明利用各药物的功效产生协同作用,能够起到去痛解痹,除烦安神、养心补虚、安神益智、化痰通窍、养心护肝、行气解郁、解除忧郁、焦虑、失眠之功效,从而有效地治疗颅脑外伤后遗性引起的头痛。
本发明蜜炼丸剂的制作过程为:取熟地3000g,三七3000g,丹参3000g,柏子仁3000g,香附3000g,夜交藤3000g,羌活3000g,防风3000g,白芷3000g,川芎3000g,红花3000g,当归3000g,鸡血藤3000g,黄芪3000g,五味子3000g,茯苓3000g,酸枣仁3000g,远志3000g,牡丹皮3000g,全蝎1000g,蜈蚣1000g。把药材加水浸泡30分钟至1小时,加热煮沸30分钟,过滤,滤液备用;滤渣加水,第二次加热,煮沸30分钟,再过滤,滤液备用;滤渣再加水,第三次加热煮沸40分钟,过滤;将三次滤液合在一起,加热浓缩至糊状,放入烘箱,控制在80℃,烘干后冷却成粉末状与蜂蜜混和在一起搓成细条;以每5g为一丸腊封包装。
本发明胶囊剂的制作过程为:取全蝎1000g,蜈蚣1000g放入10倍量乙醇中,加热回流提取2次,每次45分钟,提取活性成份,将2次提取液合并静置。取熟地2000g,三七2000g,丹参2000g,柏子仁2000g,香附2000g,夜交藤2000g,羌活2000g,防风2000g,白芷2000g,川芎2000g,红花2000g,当归2000g,鸡血藤2000g,黄芪2000g,五味子2000g,茯苓2000g,酸枣仁2000g,远志2000g,牡丹皮2000g,加10倍量水,加热回流提取3次,每次1小时,将3次提取液合并;合并上述两种提取液,减压回收乙醇并浓缩至药液浓度为0.5g生药/mL,抽滤后,滤液的相对密度约为1.08(20℃);上述滤液经体积为10L的大孔吸附树脂柱,先用10倍树脂柱体积的去离子水或蒸馏水洗脱,再用5倍树脂柱体积的95%乙醇洗脱,收集乙醇洗脱液,去除溶剂,得到组份药;装入胶囊制为胶囊剂,每粒胶囊装0.3g。可以每12粒为一板。
本发明粉剂中成药的制作过程为:取熟地1500g,三七1500g,丹参1500g,柏子仁1500g,香附1500g,夜交藤1500g,羌活1500g,防风1500g,白芷1500g,川芎1500g,红花1500g,当归1500g,鸡血藤1500g,黄芪1500g,五味子1500g,茯苓1500g,酸枣仁1500g,远志1500g,牡丹皮1500g,全蝎500g,蜈蚣500g。所述原料药按配比混合,投入粉碎机粉碎成细粉,细粉过筛后放入耐酸碱浸渍锅内,在常温下,与乙醇共同浸渍10~20天,将浸渍好的液体及药渣进行压榨过滤,分离后取滤液,加热浓缩至糊状,放入烘箱,控制在100℃,烘干后冷却,粉碎后以每袋4~6g包装。
本发明中药制剂药效学研究:
1、心气虚模型小鼠应用本发明中药制剂后,对悬挂引起的紧张有明显抑制作用;使睡眠时间延长;肌张力增加、站立时间延长;采用水迷宫法测得记忆力增加;常压下耐缺氧时间延长;使心气虚小鼠心率恢复;
2、正常小鼠应用本发明中药制剂后,对强迫游泳法和悬挂法引起的紧张有明显的镇静作用;使小鼠的睡眠时间明显延长;对免疫功能明显的提高,使单核吞噬细胞吞噬指数和吞噬活性明显增加,小鼠血清深血素显著升高;
3、对神经内分泌型高血压模型大鼠的高血压有明显的降低作用;心率明显恢复;睡眠时间显著延长,与戊巴比妥钠有明显的协同作用;用水迷宫法试验可见,使模型大鼠的记忆力增加,在迷宫中停留的时间明显缩短;用断头法试验可见,本发明中药制剂使大鼠断头后的张嘴时间明显延长。
急性毒性实验:
本发明中药制剂的小鼠口服最大给药量为210g生药/kg,按公斤体重计算,相当于成人日用量8.8g的1215倍,按体表面积计算,相当于167倍;腹腔给药测得其LD50为41.375生药/kg,其95%可信限为35.243~42.068g生药/kg。说明本发明中药制剂口服毒性低,临床应用较安全。
长期毒性实验:
本发明中药制剂对小鼠按7.69、19.18和43.21g生药/kg连续用药15周(1.0ml/100g体重,每天2次)及停药3周后,结果表明:本发明中药制剂对大鼠的毛发、行为、大小便、体重、脏器重量、血象、肝肾功能、血糖、血脂等指标均无明显影响,脏器肉眼没有发现异样变化和组织学检查结果表明,用药15周及停药3周后,大鼠各脏器均无明显改变。说明本发明中药制剂对大鼠长期用药后毒性小,停药后也没有异样反应,应用安全。

1.1诊断标准(1)中医诊断标准:参照国家中医药管理局颁布实施的《中医病证诊断疗效判定标准》头痛证候,主症为头痛经久不愈,其痛如刺,固定不移,或头痛有外伤史者,舌质紫或有瘀点等。次症为头晕伴恶心,脉细涩。(2)西医诊断标准:参照中华人民共和国中医药行业标准,高等院校实施的《实用神经内科诊断学》和《神经病学》确定诊断标准。以头痛为主症,或前额、一侧颞部或全头痛,头痛性质多为刺痛等。
1.2入选标准①有明确的头部外伤史;②伤后经手术、脱水等急性期治疗后仍遗留反复头痛;③头颅CT检查:未见异常或仅有损伤区域脑软化灶改变。
1.3排除标准①无明确外伤史的头痛;②伤后仍处脑外伤急性期患者;③头颅CT检查有颅内出血、血肿、创伤性脑梗塞及其它占位改变者。
1.4头痛程度分级方法按六点行为评分法(6point behavioralrating scale,BRS-6):将疼痛分为6级:1级(0分):无疼痛;2级(1分):有疼痛但可被轻易忽视;3级(2分):有疼痛,无法忽视,不干扰正常生活;4级(3分):有疼痛,无法忽视,干扰注意力;5级(4分):有疼痛,无法忽视,所有日常活动都受影响。但能完成基本生理需求:如进食和排便等,6级(5分):存在剧烈疼痛,无法忽视,需休息或卧床休息。1~2分为轻度头痛;3~4分为中度头痛;5分为重度头痛。
1.5对象与分组将300例颅脑损伤后头痛患者,按轻、中和重度头痛分层随机分为治疗组和对照组。其中男性155例,女性145例;平均年龄38.25岁;按格拉斯哥分级法(GCS),轻型颅脑损伤130例,中型颅颅脑损伤116例,重型颅颅脑损伤54例,(包括外伤性蛛网膜下腔出血,脑挫裂伤,硬膜外血肿,硬膜下血肿)。两组在例数、性别、年龄和外伤程度上的差异无统计学意义,两组间有可比性。
2方法
2.1治疗组单纯给予本方剂的胶囊剂内服。每日3次,疗程为一个月。连续服药2周后暂停服药2天,观察疗效。疗程结束后随访一个月。
2.2对照组给予西医药治疗。止痛药可选用:盐酸颅痛定片60mgTid,盐酸曲马多片100mg Tid,芬必得片30mg Bid。以上三种止痛药先使用盐酸颅痛定,无效时选用盐酸曲马多片或芬必得片,或三种药物轮流选用直至止痛或疗程结束,同时予谷维素20mg tid。疗程为一个月。连续服药2周后暂停服药2天,观察疗效。疗程结束后随访一个月。
两组病例在治疗观察期间一律不使用其他影响疗效的药物。
2.3疗效评定标准
显效:经治疗四周后疼痛解除(即达BRS-6的1级0分水平),一个月内不再发作,患者可自由活动。显效:经治疗四周后,疼痛减轻2级以上,偶有轻微头痛,但不影响工作、生活。有效:经治疗四周后,疼痛减轻1级以上,发作频率减少,强度减弱,但发作时仍影响工作、生活。无效:经治疗四周后,疼痛无减轻或一个月内疼痛再次发作。
2.4统计学数据处理
治疗结果应用spss3.0软件包进行秩和检验及x 2检验。
3结果
3.1治疗14天后两组头痛改善的情况比较,见表1。
根据服药前头痛程度分层分别进行比较,治疗14天后,治疗组疗效优于对照组,差异均有显著性(P<0.01),治疗组中轻、中度头痛之间疗效差异无显著性(P>0.05),重度与轻度、重度与中度头痛之间比较疗效差异有显著性,重度头痛疗效差于轻、中度头痛(P<0.05)。

3.2治疗一个月后两组头痛改善的情况比较见表2。
根据服药前头痛程度分层分别进行比较,经过治疗一个月后,治疗组疗效优于对照组(P<0.01),差异有显著性(P<0.01),治疗组疗中
不同头痛程度的疗效差异无显著性(P>0.05)。

头部外伤后头痛的程度、性质及持续时间可因颅脑损伤的种类各异影响药物的疗效,本次试验采用分层随机化分组,治疗组和对照组头部外伤程度上分布均衡可比,同时一般状况(如性别、年龄等)两组无明显差异,说明两组各基本因素(对疗效影响较大的非处理因素)基本均衡,治疗组和对照组分组合理,使治疗效果的分析具有可比性。观察发现,头痛程度会影响到治疗效果,统计分析时根据头痛程度进行分层分析。本发明中药制剂和西医对症治疗治疗14天后,治疗组疗效明显优于对照组,轻、中度头痛优于重度头痛。一个月后,治疗组疗效优于对照组。服用本发明中药制剂治疗颅脑外伤性头痛有较好的疗效。
具体实施例1:郑某,男,26岁,于2007年5月23日来院就诊。患者因被木棍砸伤头痛,2h后出现频繁呕吐,4h后昏迷。右侧瞳孔散大,光反射消失,即行手术治疗,取颅板减压,清除血肿。半月后患者生命体征稳定,但一直头痛,舌淡微暗、苔薄,此由颅脑外伤,瘀血内阻,脑络不通所致。方用熟地10g,三七10g,丹参15g,柏子仁10g,香附10g,夜交藤10g,羌活15g,防风10g,白芷10g,川芎15g,红花10g,当归10g,鸡血藤10g,黄芪10g,五味子10g,茯苓10g,酸枣仁10g,远志10g,牡丹皮10g,全蝎5g,蜈蚣5g。连服5d,症情缓解,再进5剂,疼痛明显减轻。30剂后症状完全消失,随访3次,1年未复发。
实施例2:曹某,女,44岁,工人。2006年9月23日初诊。半年前因车祸昏迷4~5分钟,清醒后即感头昏痛。嗣后症状时轻时重。近20天以来睡眠差,仅2~3小时,多梦,记忆力差。眼底检查及脑超声波探查无异常。CT示右额颞部见带状低密度区,右侧侧脑室略受压,较左侧偏小。诊为脑外伤后综合症。方用:熟地10g,三七20g,丹参20g,柏子仁10g,香附10g,夜交藤20g,羌活15g,防风10g,白芷15g,川芎15g,红花10g,当归15g,鸡血藤15g,黄芪15g,五味子10g,茯苓20g,酸枣仁10g,远志20g,牡丹皮10g,全蝎5g,蜈蚣5g。服药5天后自觉症状缓解,睡眠可达4~5小时。仍按原方加减继服15天,症状缓解后改用散剂,3个月后随访已能胜任工作。
实施例3:方某,男,19岁,大学二年级学生,高中时与同学打闹头部被碰撞后,出现头痛、头昏,心慌、失眠,怀疑自己头部血管破裂,继而出现厌食、情绪低落、学习成绩下降,神经系统及内科查体未见异常,头部CT检查、心电、脑电等各项检查均正常,排除了器质性病变,使用以下中药治疗:熟地10g,三七20g,丹参15g,柏子仁20g,香附10g,夜交藤10g,羌活15g,防风10g,白芷15g,川芎15g,红花10g,当归15g,鸡血藤10g,黄芪10g,五味子10g,茯苓20g,酸枣仁10g,远志20g,牡丹皮10g,全蝎5g,蜈蚣5g。
服用水煎剂,汤药日1剂服用2次,服药4周后上述症状明显缓解,进行血、尿常规,肝、肾功能及心电图检查未见异常。一直使用半年后告愈。随访无复发。
实施例4:李某,男,47岁,教师,2006年1月5日初诊。1年前因车祸导致头部外伤,当时即感头痛头晕、恶心呕吐,不伴意识障碍,肢体活动正常。头颅CT扫描未见明显异常。西医诊断:头部外伤性反应。当时在我院急诊科观察,给予对症治疗,症状缓解后,回家调养。半月后,患者又感头部胀痛,时作时止,心烦易怒,口干口苦,头晕耳鸣,纳差,舌质紫暗、苔薄白,脉弦。据患者舌脉症,结合其病史,证属瘀血阻络,肝气郁滞。予中药活血通络、舒肝理气。拟方:熟地10g,三七20g,丹参20g,柏子仁10g,香附10g,夜交藤20g,羌活15g,防风10g,白芷15g,川芎15g,红花10g,当归15g,鸡血藤15g,黄芪10g,五味子15g,茯苓20g,酸枣仁10g,远志20g,牡丹皮10g,全蝎5g,蜈蚣5g水煎服,每日1剂,连服6剂。1月12日二诊:头痛明显减轻,发作次数减少,口干口苦及头晕耳鸣减轻,饮食增加,仍感心烦易怒,舌质暗红、苔薄白,脉弦。上方每日1剂,并嘱患者保持心情舒畅,连服2周。1月26日三诊:头痛、头晕消失,继续治疗1个月。随访1年,头痛未发,临床基本治愈。

1.一种治疗颅脑外伤后遗性头痛的中药制剂,其特征在于,各种原料药的重量份数比为:熟地10~30份,三七10~30份,丹参10~30份,柏子仁10~30份,香附10~30份,夜交藤10~30份,羌活10~30份,防风10~30份,白芷10~30份,川芎10~30份,红花10~30份,当归10~30份,鸡血藤10~30份,黄芪10~30份,五味子10~30份,茯苓10~30份,酸枣仁10~30份,远志10~30份,牡丹皮10~30份,全蝎10~15份,蜈蚣10~15份。

4.根据权利要求1所述治疗颅脑外伤后遗性头痛的中药制剂,其特征在于,各种原料药的重量份数比为:熟地15~25份,三七15~25份,丹参15~25份,柏子仁15~25份,香附15~25份,夜交藤15~25份,羌活15~25份,防风15~25份,白芷15~25份,川芎15~25份,红花15~25份,当归15~25份,鸡血藤15~25份,黄芪15~25份,五味子15~25份,茯苓15~25份,酸枣仁15~25份,远志15~25份,牡丹皮15~25份,全蝎10~15份,蜈蚣10~15份。
5.根据权利要求1~4中任一项所述治疗颅脑外伤后遗性头痛的中药制剂,其特征在于,所述中药制剂的剂型为:蜜炼丸剂、水合丸剂、胶囊、口服液、颗粒剂、胶剂、糖浆剂、针剂或滴剂。
6.一种如权利要求1~4中任一项所述治疗颅脑外伤后遗性头痛的中药制剂的制备方法,其特征在于,所述中药制剂剂型为蜜炼丸剂,其制备方法包括:将所述原料药加水浸泡30分钟至1小时,加热煮沸30分钟,过滤,滤液备用;滤渣加水,第二次加热,煮沸30分钟,再过滤,滤液备用;滤渣再加水,第三次加热煮沸40分钟,过滤;将三次滤液合在一起,加热浓缩至糊状,放入烘箱,控制在80℃,烘干后冷却,与蜂蜜混在一起搓成细条;腊封包装。
7.一种如权利要求1~4中任一项所述治疗颅脑外伤后遗性头痛的中药制剂的制备方法,其特征在于,所述中药制剂剂型为胶囊剂,其制备方法包括:
a.将全蝎,蜈蚣按照所述原料药重量份数比放入乙醇中加热回流提取;
b.再取所述中药制剂的剩余原料药,加水加热回流提取;
c.将上述两种提取液合并,减压回收乙醇,浓缩抽滤吸附,得到药粉,装入胶囊制为胶囊剂。
8.根据权利要求7所述治疗颅脑外伤后遗性头痛的中药制剂的制备方法,其特征在于,所述步骤a中,将所述全蝎,蜈蚣放入乙醇中,所述乙醇的用量为所述全蝎和蜈蚣重量之和的10倍量,加热回流提取2次,每次30分钟~1小时,将2次提取液合并静置。
9.根据权利要求7所述治疗颅脑外伤后遗性头痛的中药制剂的制备方法,其特征在于,所述步骤b中,将剩余原料药放入水中,所述水的用量为所述剩余原料药重量之和的10倍量,加热回流提取3次,每次30分钟~1小时,将3次提取液合并静置。
10.根据权利要求7所述治疗颅脑外伤后遗性头痛的中药制剂的制备方法,其特征在于,所述步骤c中,将所述步骤a、b中两种静置后的提取液合并,减压回收乙醇,浓缩至药液浓度为0.5g生药/mL,抽滤至滤液的相对密度为20℃时1.08;上述滤液经体积为10L的大孔吸附树脂柱吸附,后用10倍树脂柱体积的去离子水或蒸馏水洗脱,再用5倍树脂柱体积的95%乙醇洗脱,收集乙醇洗脱液,去除溶剂,得到药粉,装入胶囊制为胶囊剂。
