Abstract

Description
技术领域
本发明涉及一种用于体外处理含粒子的液体的可再生的过滤器及其应用。本发明的应用领域是医学领域,尤其是直接血液处理领域。
技术状态
数千年来都在施用含有有效药物的物质来对疾病的治愈产生支持性的影响。另一种治疗性影响是可通过体外处理从血液中除去有毒物质。这种发展的起始点是经典的放血疗法,两千多年来这种放血疗法是某些疾病的标准治疗方法。50多年前新材料和新技术以及由血型研究中得到的知识使之可能在临床应用中引入血液透析法,进而导致产生换血治疗法,所述换血治疗法后来被血浆置换法取代。低特异性、高成本和感染风险限制了血浆置换法的应用。
血液过滤法、血液透析过滤法、双过滤法和血浆吸附法(或者还有治疗性血采集法)是体外治疗方法应用中的里程碑。使用血浆吸附法时,首先从血液中除去大于白蛋白的物质。使用非特异性因子或特异性因子结合流动的血液或血浆中的高分子物质。
作为血液基质与成分部分之间的静电或者疏水性相互作用的结果,如今可以常规地从血液中除去LDL、β2微球蛋白、内毒素、免疫球蛋白和循环免疫复合物。
蛋白A对于IgG的Fc受体的特异性亲和性使得开发免疫吸附剂成为可能,例如这些免疫吸附剂用于排除IgG以治疗重症类风湿性关节炎
特异性标识序列(抗体、肽)使得从血液中除去具有明确限定的特异性的目标结构成为可能。它们尤其用于除去LDL(LDL )、Lp(a)(Lp(a))、乙酰胆碱受体抗体()、抗β1肾上腺素能抗体()或者炎症介质(EP 1163004)。
把患者自身离解的免疫复合物用作患者特异性的免疫吸附剂的配体(DE 19538641)是越来越有目的且个性化的治疗中的一种特殊形式。
在全连续式血采集法中,借助于体外循环中血泵,主要以60-120ml/min的速度从外周静脉或者中央静脉导管中连续地取血,并且在除去了病原以后经不同的外周静脉重新输入。提供这种可间歇使用的体外循环处于类似于体外血液透析的情况。
在多数血采集法中,在实际血浆处理前需要初级分离血浆和血细胞。这种初级分离既可以借助于血浆离心分离法进行也可以借助于血浆过滤分离法进行。在这两种方法中平等地考虑正面效果和负面效果。基本上,血浆过滤分离法在处理上不太复杂,并且可以使血浆不含血小板。其缺点是在血浆过滤器中形成次级膜,这限制了有效过滤的次数。而在另一个方面,通过血浆离心分离法可以不间断地得到实际上无限量的血浆。稍微沾染血小板可能会对次级分离产生不利的影响。
通常,在初级分离中过滤流量是血液流量(血浆流量约20-30ml/min)的约30%。取决于适应症,通常将患者的血浆容积处理一倍或者两倍的。在处理一个或者两个患者血浆容积时(假设为无再分布、合成或者分解代谢的单隔室模型)理论上可以达到把病原最多减少到初始值的37%或者14%。然而在实践中多数达不到这些数字。
非选择性血采集法(血浆置换法)
在非选择性血浆置换法中,借助于膜式血浆分离器或者离心机在体外循环中将血浆与血细胞分离,全部血浆被丢弃并且被加入人血白蛋白或者新鲜血浆的电解液等容(isovolaemically)替代。这种替代液与分离的血细胞结合并且被重新输入患者体内。非选择性血浆置换法的优点是建立体外循环简单、对所有可接受洗血治疗的病原的方法的通用性、在不确知的病原结构(例如在乙酰胆碱受体抗体阴性的重症肌无力)中有疗效和相对低的体外容积。其缺点是免疫球蛋白丢失和凝血因子丢失、替代的外部蛋白的不耐受性风险和超渗替代风险,以及潜在的在替代液传染病原时的感染风险。
因为所述缺点的原因,当前只有在选择性方法不可行(例如在TIT、乳糜微粒血症、抗体阴性重症肌无力时)时才采用非选择性血浆置换法。
膜式血浆分离器包括带有合成膜(例如聚乙烯或者聚砜)的中空纤维模块。表面积在0.2至0.5m2之间,微孔尺寸为0.2-0.5μm。为了监控所述体外循环,使用专门为此研发的装置;作为一种替代方案,还可以使用用于血液灌注或血液过滤的装置。
选择性血采集法在选择性血采集法中,在次级循环中,通过进一步过滤过程(次级分离)、或通过吸附(免疫吸附或物理化学吸附)、或通过沉淀,病原从经过血浆过滤器分离(初级分离)的血浆中除去,然后,清洗的血浆输回到患者中。。选择性血采集法需要监控体外循环和次级循环的特定装置。
双过滤法(分级过滤法、膜差异过滤法)
该方法在分离血浆之后的次级循环中使用具有较小微孔尺寸(截止值为25-40nm)的第二过滤器。其目的是尽可能大量地重新获得白蛋白,另一个方面是在次级过滤器中保留较大分子的病原蛋白,所述次级过滤器按所谓的“闭塞末端”模式(闭塞中空纤维的末端排放)工作。由于该方法根据分子尺寸(分子量和空间分子结构)进行分离,因此它只适于除去大分子病原,譬如IgM、纤维蛋白原或者α-2巨球蛋白。因此适应症是(例如)高粘滞性综合征、巨球蛋白血症、冷球蛋白血症和高胆固醇血症。使用双过滤血采集法来治疗微循环障碍称为流变疗法(rheopheresis)。
与非选择性血浆置换法相比,该方法的长处是不需要替代液并且尤其可以选择性地除去流变学活性的蛋白而不引起止血障碍。其缺点是:由于可能以非常高的起始值阻塞中空纤维,所述次级过滤器的容量有限,以及可能丢失免疫球蛋白(其随着方便的不同而改变)。
免疫吸附法
免疫吸附法在临床上是指免疫活性分子与(例如)固定的氨基酸、肽或者蛋白结合。这些基于吸附的方法除去某些种类的蛋白,或者特异性除去病原抗体。相反的是,在过程工程学中将LDL结合到抗载脂蛋白B抗体上称为LDL免疫吸附法。
脂蛋白的消除
系统(Kaneka、Osaka;Hospal、Planegg)基于LDL和Lp(a)从血浆中吸附到葡聚糖硫酸酯/纤维素(DSC)上。该机制基于葡聚糖硫酸酯的带负电荷的硫酸根基团与上述两种脂蛋白的带正电荷的Apo B的静电相互作用,HDL、免疫球蛋白和白蛋白仅被在轻微的程度上吸附。
免疫球蛋白的消除
系统(ASAHI/Diamed,Cologne)基于色氨酸(TR-350L)或者苯丙氨酸配体(PH-350L)与聚乙烯醇凝胶基质结合而使用不可重复使用的吸附剂。
冷过滤法
在冷过滤法(Asahi Medical,Tokyo;Diamed,Cologne)中,分离的血浆被冷却到4℃,以在膜差异过滤法中使冷球蛋白沉淀,在分离沉淀物后,接着加热到体温,然后借助于低温过滤器重新输回。
全血采集法
在全血采集法中,借助于以粒子形态处于吸附器柱中的吸附性物质(活性炭、交换器树脂),在体外循环中或多或少选择性地直接从血液中除去毒物。这类似于活性炭血液灌注法(其用于一系列中毒的重症)。吸附器柱的尺寸必须保证吸附剂的足够的交换表面和接触时间。
通过系统(Fresenius HemoCare公司的脂蛋白直接吸附系统,St.Wendel)使得从全血中直接吸附LDL和Lp(a)成为可能。单次使用吸附柱包含带负电荷的聚丙烯酸酯配体,所述聚丙烯酸酯配体固定在聚丙烯酸酰胺上并且通过静电方式结合致动脉粥样硬化脂蛋白。
方法、过滤和吸附可以以各种方式结合。Matson等(US 6,287,516)描述了一种血液过滤系统,其包括具有下游吸附器的血液过滤器。来自过滤器的超滤液(排除分子量#50,000道尔顿的物质)通过软管系统被泵入吸附器单元中,在该吸附器单元中脓毒介质被结合。按照这种方式处理的超滤液可以通过进一步的泵/软管/阀门系统与初级过滤的血液合并,然后重新输回患者体内。
特异性吸附的优点是明显的,因为仅仅除去(致病的)目标物质,而不会因为非特异性吸附“治愈性”血浆成分对标准调控产生负面影响。全特异性吸附法的缺点是使用的配体主要是特异性抗体,使得产品较为昂贵。这意味着是总治疗成本,尤其是对于不得不治疗数天或数周的疾病的负担。在EP 0139949中描述了一种用于血液清洗的装置,其既在壳体中含有血浆过滤用中空纤维,也在血浆区域中含有吸附基质。Shettigar(WO 93/02777)描述了一种非常类似的装置。两者的共同之处在于吸附基质都是不能再生的。US 6497675B1中清楚的提供了再生的可能性。此处的缺点是尽管吸附器单元整合到血浆过滤器中,但为了再生的目的不得不把它从壳体中取出。这种操作使得高过程变得复杂并增加了混淆的风险,因此增加了传染疾病的风险。再生的吸附器单元在另一个工作步骤中被拧入新的血浆过滤器壳体中。
发明内容
本发明期望将膜的优点(不产生任何粒子、可以自由地选择孔径、价格便宜)与粒子的优点(考虑到在材料种类、尺寸、表面积、活性和偶联配体方面大的可变通性)结合在一个壳体中,这样既允许昂贵的功能化粒子再生,同时又使血浆过滤器膜的内部没有生物膜(其通常限制所述膜的可用性)。
本发明的目的是一种全血处理单元,其特征在于通过(例如)取消泵和额外的软管连接来建立一种简单的系统,并且结合膜和粒子的优点。根据本发明的全血处理单元使得下列方面成为可能:具有较高的血液流量(高达160ml/min)、缩短处理时间、以及重要的是可再生的。
所述目标通过下列事实而实现:将通常孔径为0.2-0.5μm的通常的血浆过滤器(膜式中空管)插入自身封闭的生物相容性壳体中(参见图1和图1a)。纤维素衍生物或合成材料(例如聚砜或者聚酰胺)可以用作膜材料。壳体同时起到功能化粒子的容器的作用。例如,聚砜、聚丙烯腈、聚甲基丙烯酸甲酯、聚乙烯醇、聚酰胺、聚碳酸酯和纤维素衍生物可以用作粒子的材料。加入流动的血液后,血浆流过以压力梯度和孔径作为函数的膜在膜的外部,血浆在此流过吸附器凝胶,所述吸附器凝胶包含非特异性或者特异性地功能化的微粒,所述微粒的粒径大于膜的孔径。按照这种方式特异性地清除了某些生物活性物质的血浆与腔内血浆过滤器血流在壳体中合并,并作为清洗的全血重新输回患者体内。过滤器系统防止微粒进入血流中。
本发明的基本特征是下列事实:三通阀被按照这样的方式设置在过滤器系统中,该方式使得将液体泵入粒子区域并且从入口孔流出成为可能(参见图2和图2a)。通过操作这些三通阀,在吸附潜力耗尽后在壳体的出口处停止血流/血浆流。使用通常的缓冲液通过粒子储存容器进行再生。再生缓冲液通过血浆过滤器的微孔进入过滤器膜的空腔内部并且脱离生物膜的表面,结果是在完成相应的清洗程序以后,过滤器可用于再次处理循环中。根据本发明,处理系统还可以包括两个或者多个过滤器,结果是在一个吸附器再生的同时另一个吸附器继续进行处理,并且按照这种方式可以进行连续的血液处理。根据本发明三通阀的设置首次使得在不必从壳体中除去吸附器单元的条件下再生功能化粒子成为可能。按照这种方式,可以更快速并且以较少的技术努力的方式进行再生步骤。最重要的是这是非常有意义的,原因在于其消除了不同患者的吸附器单元的混淆。该优点在医院中的常规运行上应当是特别重要的。另外,使用根据本发明的装置在置换时不存在污染物渗入吸附器单元的风险。
总而言之,可以看到与要求持续数日的一次性产品相对,通过使用根据本发明的装置可以显著地降低治疗成本。
下面通过实施方案的例子更加详细地说明本发明。
实施方案
实施方案1
材料和方法
·将3升牛源柠檬酸盐血与1.5μg重组人IL6混合并且通过图1所示的装置泵入一个封闭的循环中(参数见下文)。
·从全血中除去抗原的实验用装置包括中空纤维血浆分离器(A),所述血浆分离器的下部壳体部分用插入的圆筒(B)代替,在所述圆筒中吸附剂凝胶包围所述中空纤维。由于跨膜压力释放的血浆不得不通过吸附器并按照这种方式脱离目标物质。
·血浆分离模块包括0.4m2的中纤维过滤器(A)。
·吸附器容器含有60ml的Sepharose,每ml的Sepharose共价结合5mg的IgY(从接种IL6的小鸡的蛋中得到的卵黄抗体)
·吸附器的结合容量达72μg的IL6。
·血流速度设定为约120ml/min。
·在此血浆流量达到20-25ml/min。
·在完成了1.5升的血浆通流后,结束吸附并且测定血液中的IL6浓度(人-IL6-ELISA,Milenia Biotec)。
结果:
·可以没有任何问题地操作该模块。
·血浆处理量是1.5升。
·没有观察到溶血。
·在历时60分钟的吸附过程中,IL6的浓度由初始的500pg/ml降至200pg/ml。这对应于全部血浆体积单次通过根据图1所示的装置(还参见图1a)的吸附器时清除率为60%。
实施方案2
Sepharose 4FF的活化和特异性抗-IL6抗体的偶联
材料和方法
溶液A:在100ml丙酮中溶入10g溴化氰(CNBr)
溶液B:在100ml丙酮中溶入15.2g三乙胺
溶液C:混合300ml丙酮和700ml H2O
溶液D:混合600ml丙酮和400ml H2O
缓冲液A:在0.2M碳酸盐缓冲液中加入0.5M NaCl,pH8.5

Sepharose 4FF(Amersham/GE)
将50mg特异性拮抗人IL-6的卵黄抗体(IgY)溶解到pH8.5的PBS中。
实施方法
·根据氰基转移法(Kohn J和Wilchek M.The use of cyanogen bromide andother novel cyanylating agents for the activation of polysaccharide resins.ApplBiochem Biotechnol.1984;9:285-305)活化Sepharose 4FF。从原容器中取出15ml的在20%丙酮中的Sepharose,并且用水(10×20ml)在微细粒子过滤器上洗涤。
·此后,用3:7丙酮洗5次,然后用6:4丙酮洗5次。
·将吸取后的Sepharose转移到合适的容器中,并悬浮在8ml的6:4丙酮中。
·将悬浮液在-20℃下搅拌(10min)
·然后加入2ml CNBr溶液,均化,滴入2ml TEA溶液。
·将混合物在微细粒子过滤器上吸取,然后用冰水洗涤。
·此后,根据(Kohn J,Wilchek M.A colorimetric method for monitoringof Sepharose by cyanogen bromide.Biochem Biophys Res Commun.1978;84(1):7-14)测定活性,其中测得Sepharose上的氰酸酯基团的浓度。
·将吸干后的活化的Sepharose搅拌加入IgY溶液中,并且在室温下振荡2小时。

实施方案3
全血处理单元的制造
1.制造包括通常的中空纤维血浆过滤膜(0.3m2)和20ml的具有拮抗IL-6功能化的吸附剂的粒子(Sepharose 4FF)的壳体。
2.插入数捆膜,然后浇铸两端开口以形成与壳体密封的连接体。切掉所述中膜的突出端。
3.用封闭盖封闭双端,所述封闭盖支撑连接分枝。
4.上连接分枝处设置三通阀(V1),该三通阀起到使血液流动或排出再生液的作用。
5.下连接分枝处设置三通阀(V2),该三通阀起到排出处理后的血浆或引入再生液流体和洗涤液的作用。
6.下连接分枝处设置三通阀(V3),该三通阀起到将处理后的血浆与细胞血液成分合并从而形成处理后的全血,以及在关闭该三通阀后防止再生液渗入所述全血中的作用。
7.通过在柱体上的侧部进料连接加入功能化的Sepharose。通过第二侧部连接分枝进行排气。
实施方案4
体外从血中除去IL6
材料和方法
·在CNBr活化后,Sepharose 4FF(Amersham/GE)与从接种IL6的小鸡的蛋黄中得到的卵黄抗体(IgY)偶联
·偶联密度:5mg IgY/ml充满的Sepharose
·重组人IL-6(R&D Systems GmbH)
·肝素化全血
·IL6-ELISA试剂盒(Milenia Biotec)
实施方法:
·在三个不同的混合物中,在每种情况下均向2ml健康人的全血中刺入146ng重组IL-6
·向该血样中加入0.1ml Ig Y-Sepharose(特异性拮抗人IL-6)
·在室温下用振荡器摇动样品。
·借助于作为时间函数的平均ELISA值来测定血液的IL-6含量。
结果:
在用IL6的成批试验(图3)中,可重复地证明了负载拮抗IL-6-IgY(5mg/ml)的Sephrose能够从全血中结合IL-6。所使用的IL-6量大致对应于其在重病患者(例如重症脓毒病和感染性休克患者)的血液中产生的量。

通过泵(P1)传送的含粒子的液体(F)经过处于位置的三通阀(V1)进入装置AB。由于在中空纤维(a)的通路中建立的跨膜压力,取决于膜的孔径,流出没有粒子的液体组分进入到处于膜外的空间中。液体(T)经过装置AB的部分B,该部分B含有能够特异性地吸附溶解的物质的粒子(细节参见图1a)。经过处于位置的三通阀(V2)和处于位置的三通阀(V3),清洗的液体(T)和含有粒子的浓缩液合并。在该处理过程的得出物中,原液(F)不含溶解的、不利的成分部分。

膜(a)的微孔因其尺寸只允许液体组分(F)通过。由于吸附器粒子(B)该组分不含不利的、可溶解的成分部分,然后离开所述吸附器。通过对应于所述处理过程切换三通阀(V2)和(V3),可以达到与直接流过的含有粒子的浓缩液的重新合并。

通过沿着与过滤器的工作方向相对的方向传送合适的洗涤液来再生所述过滤器。
由泵(P3)传送的洗涤液(Reg)经三通阀(V2)进入吸附器区域B。在该区域进行吸附器粒子的再生。此后,液体(Reg)在与分离方向相反的方向上清洗中空纤维(a),并因此恢复原有效能(细节参见图2a)。此后,负载污染物的介质(W)通过处于位置的三通阀(V1)离开吸附器,而不接触所述要清洗的液体。阀(V3)的位置防止污染储存容器中的液体(F)。

吸附器模块必须再生以重复使用。为此通过阀(2)在与正常工作方向相反的方向上将合适的液体(Reg)引入吸附器中。在第一阶段中,存在吸附器粒子的解吸附反应。此后,在相反方向流经膜(a),微孔中或微孔上不含沉积物。恢复吸附器模块的过滤效能和吸附效能。对应于处理过程切换三通阀(V1-3),防止由(Reg)或者(W)污染储备液。

Claims (17)
1.用于膜式分离和吸附处理含有粒子的液体的过滤器系统,包括
a.壳体,其中
b.中空纤维被按照这样的方式设置,该方式使得用于所述液体的入口孔和出口孔在所述反应容器的外部,所述中空纤维膜和所述壳体的内壁之间的区域旨在填充微粒,在所述出口孔前面有
c.滤网,所述滤网将所述微粒从所述血浆区域中分离,以及
b.三通阀被按照这样的方式设置,该方式使得将液体泵入所述粒子区域并且从所述入口孔流出成为可能。

17.根据权利要求10至16中任一项所述的过滤器系统在治疗由于先天和/或获得性免疫系统的失调而激发或保持的疾病中的应用。
