Abstract

技术领域

背景技术
生物学行为反馈及其对于糖尿病控制的关键重要性是各种病症的复合体,这些病症的特征是共同的最后高血糖要素,该病症起因于病症正在发生,或者通过在生物系统组织的所有级别(从分子经过荷尔蒙至人体状态)作用的机制来确定病症正在发生。使用胰岛素并通过内服来将血糖维持在接近正常水平的加强治疗显著地降低了类型1(34,40)以及类型2糖尿病的(T1DM,T2DM,42)的慢性并发症,但是可能潜在地造成危及生命的严重低血糖(SH)危险。现有技术水平中仅有不完善的胰岛素替代可以降低导致认识功能障碍、昏呆、昏迷或者猝死(16,17,31,39)的对于低血糖的荷尔蒙预防和警告症状。此外,近来的研究表明低血糖可以引起胰岛素敏感性增加(18,32)。因此,低血糖已经被鉴定为最佳糖尿病管理(9,11)的主要障碍。具有T1DM和T2DM的人将面临终生的“行为方面控制优化问题”,以在不增加其低血糖危险的情况下,降低高血糖波动幅度并维持严格的血糖控制。
关键在于,优化问题提供并适当地利用了对于系统状态和动力学的可用反馈。从系统生物学观点来研究,T1DM优化的任务需要研究若干个生物系统级别的反馈环:(i)对于病人改变人类行为的外部反馈;(ii)胰岛素-葡萄糖相互作用;以及(iii)随着从一次低血糖发作至下一次低血糖发作的复发性低血糖而出现的荷尔蒙反馈,通过低血糖伴随的自主神经功能衰竭(HAAF,10)来反映。图1代表了这三个主要的反馈环。
本发明的背景是反馈环1(图1),即血糖和行为的时间模式。该过程受到许多外部因素的影响,包括所注射的胰岛素的定时和剂量、所吃的食物、身体活动等。换言之,糖尿病中的血糖(BG)波动是复杂动态系统作用的可测量结果,该可测量结果受到许多内部和外部因素的影响。该系统的宏观(人类)级别的优化取决于自我治疗行为。因此,这种优化必须基于利用领域内可用数据的反馈(诸如血糖自我监测(SMBG))、HbA1c的知识,以及症状的监测和自我治疗做法。这些宏观级别的信息源处于各种发展阶段并且是临床认可的,HbA1c化验和SMBG现在的操作规程:
HbA1c:该平均血糖状态经典指标(market)(1)已经与糖尿病的长期并发症相关联,并且被确认为T1DM和T2DM(36)的黄金标准。然而,已经反复证实了HbA1c是病人急剧血糖波动的一种无效评估指标。DCCT(糖尿病控制与并发症试验)得出的结论是,仅仅约为8%的SH发作可以根据已知的变量(包括HbA1c(39))预测;后来通过使用SH历史、认识和自主神经功能得分(15)的结构化等式模型,该预测提高到18%。在我们的研究中,HbA1c根本与SH没有显著关联(6,23,27)。
SMBG:现代的家用BG测量计提供了一种通过SMBG(3,7,41)用于频繁且准确的BG确定的方便工具。大多测量计能够存储上百个SMBG读数及每个读数的日期和时间,并且具有用于将这些读数下载至PC的接口。测量计通常带有具有基本数据分析(例如,均值BG的计算、前两周期间的平均BG的估计、目标百分比、低血糖和高血糖区等)、数据日志和图像表示(柱状图和圆饼图)能力的软件。然而,虽然这些装置提供了在某个时间点处与BG的当前状态相关的信息,但在给出BG结果时,这些装置当中没有一个装置提供了病人总体血糖控制、BG模式和趋势或者自我治疗有效性的评估。在一系列研究中,我们已经证明特定的SMBG数据分析和低BG指数(LBGI)可以捕获到针对低血糖的倾向于增加危险的长期趋势(23,24,29),并且可以识别24小时周期的低血糖的增加危险(20)。这些分析基于BG测量刻度(scale)的特定非对称性认识,并且基于校正该非对称性的非线性变换(22,25)。由于我们的首次宣布(6),我们重新改进并进一步验证了我们的方法,并且提供了一种BG数据危险分析(28)的结构性理论。该理论成为我们在T1DM(21,27)中使用SMBG来综合评估血糖控制的算法的基础。
测试及行为评估:为了从形式上来描述行为自我治疗的过程,我们创造了自我调节行为(Self-Regulation Behavior)的随机(stochastic)模型,该随机模型给出了对于反馈模式内部情况-症状感知/察觉-鉴定-自我调节决策(26,图2)的描述。该模型背后的思想在于,诸如低(或高)BG发作的内部事件之后是自我调节行为序列,如果不适当,则可能导致病人SH或者极端的高血糖,如果适当,则会使得避免这些极端情况。可以区分出四个顺序步骤:内部情况(例如,低/高BG)-症状感知/察觉-鉴定-判断/自我调节决策。这些步骤通过可能通路的连续区间来链接,即,如图2的步骤1至步骤2所示,存在各种低/高BG水平的可能感知,但如图2中步骤2至步骤3所示,不存在单一可能的感知鉴定,并且如图2的步骤3至步骤4所示,不存在状况鉴定之后唯一预定的决定。该模型是随机(stochastic)模型,该模型是通过转变概率(26,33)族表示的从一个步骤到下一步骤的转变。自我治疗行为的模型提供了本发明公开的理论基础--对于人类的适当反馈将改变人的感知和察觉,从而产生更好的自我调节决定以及改善的血糖控制。此前提通过下面段落中所述的研究得到了实验证实。
行为干涉的使用:一些研究已经证明避免低BG事件(<70mg/dl)几周就能够改善症状的感知并逆转不察觉低血糖(8,12,13,14)。虽然这种干涉的前提是降低SH危险,但是它需要进行密切病人监测以确保没有危害到新陈代谢控制。我们以前已经开发出BGAT,一种很好地证明对于患T1DM的人有效的精神生物行为干涉。BGAT的积极效果包括BG检测、BG轮廓、社会心理状态、知识、判定的改善以及降低诸如严重低血糖和高血糖(4,5)的危及生命的事件。另外,我们已经证明BGAT改善了具有无损低血糖症状的病人和觉察降低的病人两者的BG检测。Joslin Clinic的研究已经发现BGAT保存了在病人经受加强胰岛素治疗时的逆调节完整性。
本发明各种实施例中的一个方面集中在反馈环#1(图1)-血糖和行为的时间模式,但并不局限于此。前提是能够通过血糖和自我测试的特应性时间模式的算法识别使得行为改变。特别地,识别出高血糖以及低血糖模式、增加的葡萄糖变化性以及无效的自我监测,并将这些消息实时地传送回个人。
正如自我治疗行为的随机模型(图2)所假定的那样,继而促使了又引起血糖控制改善的一系列的意识增加、鉴定提高、自我治疗行为改善。与本发明相关的特别具有重要性的是特应性反馈的定时。根据本发明的各种实施例的一个方面,在与该消息发送相关的日期的时间帧之前,向病人给出与血糖和自我测试的时间模式的识别相关的反馈。优选地,当病人在测试他们的BG时给出该消息发送,并且该消息发送应当与通常出现在随后时间周期的模式相关,所述随后的时间周期无论如何可以从大约2小时至8小时长。该消息发送使得病人能够在给出了信息的时间周期之前立即采取正确的措施,以避免任何有害的血糖模式重新出现。洞察的定时以及措施的结果促动因素有望对于改善自我管理意识和行为具有强烈影响,并最终改善病人血糖控制。
发明内容
本发明的各种实施例的一个方面包括但并不局限于,四种用于识别下列模式的方法和算法:(i)高血糖;(ii)低血糖;(iii)葡萄糖变异性增加,和(iv)无效的自我测试。该方法使用了跨过2-6周的周期收集的日常SMBG数据。SMBG被定义为糖尿病病人在自然环境下得到的血糖的阶段性非自动确定(通常每天2次或者更多次)。用户、患者或者病人可以对自己进行测试或者依赖于其他人的帮助,例如外行、熟人、临床医师、其他医疗专家等。
本发明的各种实施例可以尤其直接与下列内容相关:
■通过引入能够评估时间葡萄糖模式的智能数据解释部件而实现的现有SMBG装置的增强,以及通过相同特征实现的未来SMBG装置的支持;
■通过相同特征实现的旨在辅助糖尿病管理的手持装置(个人数字助理,PDA、移动电话/电子邮件装置、胰岛素注射器等)的增强;
■通过相同特征实现的检索SMBG数据的软件的增强--这种软件实际上由每个家用BG监视装置的制造商来产生,并且通常由病人和解释SMBG数据的卫生保健提供方使用。该软件可以驻留在病人的个人计算机中,或者可以经过互联网入口来使用;

本发明各种实施例的一个方面在于但并不局限于:向病人提供与即将到来的可能高血糖、可能低血糖、葡萄糖变异性增加或者测试不足或者测试过度的周期相关的实时信息将会促使适当治疗反应,并且将会因此引起更好的糖尿病控制。图3示出了该基本概念:
在每次SMBG测试时并且在呈现SMBG的结果之前,装置估算血糖的历史模式,并基于该估算发布对于下一时间周期的警告。这些警告包括高血糖或者低血糖的高危险、血糖变异性增加、测试不足或者测试过度(图3)。
支持该消息发送系统的系统、方法和算法是本发明公开的主题。该理论背景已经通过我们的BG数据(27,28,30)危险分析理论建立,并遵循先前研发和公开的技术。所有方法和算法都已首先根据葡萄糖总平均或者最优人类模式,使用特定时间周期内用于葡萄糖级别偏差或测试模式的通用统计假定而形成。然后,将结果得到的算法应用至大数据集(N=335患者),以确认该算法并确定算法参数的范围。表1表示了该数据集中的参与者的人口统计特征:

警告消息所覆盖的下一时间周期的预定持续时间可以是2和8小时之间的任何时间,优选地为6小时。一天即2 4小时时间周期可以以预定的持续时间来划分成时间段(time bin)。为了简化说明,在该公开中,我们假定时间周期具有4小时时间周期的预定持续时间,在夜间具有8小时时间周期。

本发明方法的一个方面包括基于在每个时间周期的葡萄糖值的计算,为下一时间周期即随后的时间周期提供高血糖危险、低血糖危险、高葡萄糖变异性的危险以及用户无效测试的指示。危险或者无效测试的指示可以在下面的步骤完成之后发生:多个SMBG数据点的采集,对具有预定持续时间的时间周期内的SMBG数据点的分类,以及每个时间周期内的葡萄糖值的估算。该指示可以是在下一时间周期之前立刻发布给用户的指示危险或者无效测试的消息的形式。该指示可以发生在一下时间期间,但并不局限于此:在下一时间周期开始之前即刻,在多个SMBG数据点采集的24小时内,在多个SMBG数据点采集的12小时内,在多个SMBG数据点采集的6小时内,几乎与最后SMBG测试同时,以及实时发生。
读数次数:
SMBG读数的周数目可以是大约2周或者超过6周,但是优选地为大约2周至6周,尤其是大约4周。优选地,每个时间周期有5个读数。SMBG读数的总数可以从至少30读数开始,但是优选从60开始。
1.根据SMBG数据识别高血糖和低血糖模式的算法
识别高血糖和低血糖危险增加模式的算法通过下面详述的若干序列步骤来工作。该思想在于,将个人的每天24小时的SMBG轮廓,分割成具有预定持续时间的固定时间周期,从SMBG读数的时刻开始,或者在另外的预定时间开始。然后,基于历史SMBG数据,估算每个时间周期内的平均葡萄糖,以得到对于高血糖或者低血糖的偏差。这些偏差评估为两个级别:
■超过总体数据、文献以及已接受的临床应用指南所识别的绝对阈值;以及
■超过由每个个体的血糖模式分析而得到的特应性阈值,即个体阈值。
如果存在这两种情况中的任一种,则宣布该时间周期是高血糖或者低血糖的高危险周期。每种条件的判断受一对高血糖参数和一对低血糖参数支配:
对于高血糖危险:
■BG阈值参数(α1),反映了高BG的总体级别的定义。例如,α1=180mg/dl或者α1=200mg/dl就是可接受的值。
■个体的复合概率阈值(β1),反映了在该特定时间周期期间该特定个体BG为高的特应性可能。例如,β1=0.4至0.6就是可以接受的值。
对于低血糖危险:
■BG阈值参数(α2),反映了低BG的总体级别的定义。例如,α2=70mg/dl或者α2=75mg/dl就是可接受的值。
■个体的复合概率阈值(β2),反映了在该特定时间周期期间该特定个体BG为低的特应性可能。例如,β2=0.01至0.2就是可以接受的值。
识别高血糖和低血糖模式的算法的步骤:这两算法的最开始七个步骤相同:
(1)检索最近2至6周监测期间采集的所有SMBG数据以及每个读数的时间:
(2)在SMBG读数的时间,以预定的时间间隔(2-8小时)将每天24小时分割成M个时间段。所述时间段被定义为允许在2-6周上累计充足SMBG读数数目的充足持续时间的周期。例如,假定在11PM进行SMBG读数,那么可以如下定义M=5个时间段:1~[11PM-7AM);2~[7-11AM);3~[11AM-3PM);4~[3-7PM),5~[7-11PM),其中“]”意思是包括;
(3)将所有的SMBG读数分类到这些时问段中:让Xk1,Xk2,…..,XkNk为落至SMBG的最近30天的时间段k中的SMBG读数,例如k=1,2,...M;此处Nk=时间段k中SMBG读数的数目,
(4)针对每个时间段k,如下计算BG的平均值和SD:

其中Yk是除了k之外的4个时间段中均值的平均,SD1是Xk-Yk的SD的估计。例如,对于k=2,Y2=(X1+X3+X4+X5)。
假定时间段k的均值不高于其他时间段的均值的虚假设,则统计tk将具有近似的t分布,对于N>30,t分布将以中心正态分布来逼近。在有效数据集合中,该逼近的平均绝对值误差为0.0009(SD=0.001),因此该正态逼近对于该算法的实际实施而言是可以接受的。(注意:直接计算t分布是非常困难的,这正是推荐正态逼近的原因)。tk>0的概率的正态近似可以计算为P(tk>0)=Φ(tk),其中Φ(tk)是中心正态分布的分布函数(均值为0,SD=1)。

算法的阈值:算法使用的特定阈值αi和βI(i=1,2)应当由使用该算法的设备的制造商、使用该算法管理病人的临床医生或者用户来确定,并且应当基于消息频率的可接受性与消息的实用性来确定。我们已经使用在本部分开始所提供的数据库编制了表2和表3,其中表2提供了给定各种阈值下,所述算法针对高血糖发布的消息的频率,表3提供了所述算法针对低血糖发布的消息的频率。对于每个患者,需要30天内至少60个SMBG读数以及每个时间段至少5个读数以输入计算。

图2和图3中的百分比如下进行计算:我们计算是否对于每个患者以及对于每个时间段“发布”消息。如果对患者“发布”了至少一个消息,则该患者作为1计数到表格中的频率计数中。
说明性实例:
图4-6示出了识别高血糖时间模式的计算。在图4中,通宵周期被识别为高血糖的高危险期。在图5中,两个时间周期,3-7PM和7-11PM被识别为高血糖的高危险期。在图6中,自11PM到11AM的时间周期被识别为低血糖的高危险期。
2.根据SMBG数据来识别葡萄糖变异性增加模式的算法
识别葡萄糖变异性增加模式的算法的逻辑与识别高血糖模式的算法的逻辑类似。然而不是测量平均BG,而是检验包括每个时间段内变异性的测量。例如,这种测量可以是BG的标准偏差(SD),或者被转换成危险间隔(risk space)(28)的这些值的危险标准偏差(RSD)。在该实现中,我们使用RSD,这是因为该测量对于高血糖和低血糖的葡萄糖变异性同样敏感。
使用ADRR(平均日常危险范围)来计算人的总体变异性。但是还可以使用SMBG读数的总体标准偏差、M-value(37)、MAGE(38)、不稳定指数(35)或者任何其他可接受的变异性的测量来计算(例如参见附录A,30,概率一览表)。SMBG读数的标准偏差将使得变异性的轮廓对于高血糖波动幅度更加敏感,而对低血糖波动幅度不那么敏感。由于ADRR在其敏感性以及极端血糖波动幅度的预测能力方面表现优良,并且因为ADRR已经清楚地识别出了总体阈值(附录A,30),因此在该实现中我们使用了ADRR。
如前面部分中一样,假设SMBG读数在11PM读取,并且将随后24小时时间周期以预定的持续时间分成多个时间段。然后,基于历史SMBG数据,估算每个时间段中的葡萄糖读数,以得到对更高变异性的偏差。这些偏差因超过经由每个个体的血糖模式分析而确定的特应性阈值而被评估。此外,相对于总体参数来对人的总体ADRR进行分类。使用总体ADRR和特应性偏差的结合将时间周期宣布为变异性增加高危险期。每种条件的判断由两个参数控制。
■ADDR阈值参数(α),反映了高葡萄糖变异性的总体级别的定义。例如,对于α=30,或α=40mg/dl就是可接受的值。
■个体的概率阈值(β),反映了在该特定时间周期期间内,该特定个体变异性为高的特应性可能。例如,β=0.6至0.8就是可以接受的值。
算法的步骤:
(1)检索最近监测的2至6周期间内采集的所有SMBG数据以及每个读数的时间;
(2)在SMBG读数的时间,以预定的时间间隔(2-8小时)将每天24小时分割成M个时间段。所述时间段被定义为允许在2-6周上累计足够SMBG读数数目的充足持续时间的周期。例如,假定在11PM读取了读数,那么M=5个时间段可以定义如下:1~(11PM-7AM];2~(7-11AM];3~(11AM-3PM];4~(3-7PM],5~(7-11PM],其中“)”意思是包括;
(3)将所有的SMBG读数分类成这些时间段:让Xk1,Xk2,......,XkNk为落至SMBG的最近30天的时间段k中的SMBG读数,例如k=1,2,...M;此处Nk=时间段k中SMBG的数目。
(4)使用先前介绍的公式:f(BG,a,b)=c.[(ln(BG))a-b}]将每个BG读数变换成“危险间隔”,其中该函数的参数取决于BG刻度,并且为如下所示:如果BG以mg/dl测量,那么a=1.084,b=5.381,c=1.509;如果BG以mmol/l测量,那么a=1.026,b=1.861,c=1.794(28)。
(5)对于每个时间段,使用下面公式来计算危险标准偏差RSD:

(6)使用上面公式计算总体RSD,但是包括跨过所有时间段的所有SMBG读数。
(7)对于每个时间段,计算比值Zk=5*(RSDk/RSD-1)。正如通过大数据库(N=233个患者)的分析所示出的一样,这些比值已经近似于中心正态分布,因此这些比值可以以与前面部分中相同的方式用于特应性偏差的测试。
(8)对于每个时间段,将Zk>0的每个个体的概率计算为P(Zk>0)=Φ(Zk),其中Φ是通过在前面部分中给出的多项式来计算的中心正态分布的分布函数。P(Zk>0)是特定时间段将会比其他时间段具有更高变异性的个体概率。
(9)使用2006年12月12日公开并在最近公布的(30)算法来将每人的ADRR计算为变异性的总体指标(overall marker)。简而言之,ADRR的计算可以通过下列公式来完成:
对于每个SMBG读数,计算r(BG)=10.f(BG)2,其中f(BG)在上面的(4)定义;

算法的阈值:该算法使用的特定阈值α和β应当由设备制造商基于消息频率的可接受性与消息的实用性来确定。我们已经使用上述的数据库编制了表4,表4代表了给定各种阈值下通过算法发布的消息频率。对于患者,需要30天内至少60个SMBG读数以及每个时间段至少5个读数以输入计算。

说明性实例:图7示出了识别变异性增加模式的计算:周期3-7PM被识别为高变异性周期;ADRR阈值设置为40,概率阈值设置为0.6。
3.识别无效SMBG测试模式的算法
与前面所述的算法类似,我们假设在11PM读取SMBG读数,然后识别无效测试模式的算法将一天分成5个时间段:1~[11PM-7AM);2~[7-11AM);3~[11AM 3PM);4~[3-7PM);以及5~[7-11PM)。然后,该算法计算每个时间段中包含的SMBG读数的百分比。
如果发现一个时间段中包括小于α%或大于β%的人的SMBG读数,则该时间段分别被识别为测试不足周期或者测试过度周期。参数α%和β%可以设置在合理的值,例如α%=5%,β%=50%。如果超过了这些阈值中一个或者两个,将发布消息,如图3所呈现的消息。该消息将会包括在测试频率低于α%时测试不足的警告,以及测试频率高于β%时测试过度警告。
说明性实例:图8示出了无效SMBG测试模式。无效测试的阈值设在5%,患者A在夜间仅仅有他/她SMBG读数的3.2%,而患者B在下午仅仅具有4%的SMBG读数。
在用于验证这些方法的数据库中,,没有在时间段内β%=50%的过度采样实例。在早晨的时间段内(7-11AM),跨过所有时间段的所有患者的最高测试频率为48.4%。
实例性系统:
本发明的方法可以使用硬件、软件或者软硬件的结合来实现,并且可以在一个或多个计算机系统或者其他处理系统(诸如个人数字助理(PAD))中或者直接在配备有足够存储和处理能力的血糖自我监测装置(例如SMBG存储计(memory meter))中实现。在实例实施例,本发明可以以在如图9所示的通用计算机900上运行的软件来实现。计算机系统900可以包括一个或多个处理器,诸如处理器904。处理器904可连接至通信基础结构(诸如通信总线、跨接带(cross-over bar)或者网络)。计算机系统900可以包括显示器接口902,用于转发来自通信基础结构(或者来自未示出的帧缓冲器)906的图形、文本或者其他数据,以在显示单元930上进行显示。显示单元930可以是数字和/或模拟的。
计算机系统900还可以包括主存储器908,优选地包括随机访问存储器(RAM),并且还可以包括辅助存储器910。辅助存储器910可以包括硬盘驱动912和/或可移动存储驱动914,可移动存储驱动914代表软盘驱动、磁盘驱动、光盘驱动、快闪存储器等。可移动存储驱动914以公知的方式从可移动存储单元918进行读取或者对其进行写入。可移动存储单元918代表由可移动存储驱动914读取和写入的软盘、磁带、光盘等。正如将会理解的那样,可移动存储单元918可以包括其中存储有计算机软件和/或数据的计算机可用存储介质。
在备选实施例中,辅助存储器910可以包括用于计算机程序或者其他指令加载到计算机系统900的其他装置。这种装置可以包括例如可移动存储单元922和接口920。这种可移动存储单元/接口的实例包括程序盒式存储器和盒式存储器接口(诸如可以在视频游戏设备中找到的程序盒式存储器和盒式存储器接口)、移动存储器芯片(诸如ROM、PROM、EPROM或EEPROM)和相关插槽,以及其他允许软件和数据从可移动存储单元922转移到计算机系统900的可移动存储单元922和接口920。
计算机系统900还可以包括通信接口924。通信接口924允许软件和数据在计算机系统900和外部设备之间转移。通信接口924的实例可以包括调制解调器、网络接口(诸如以太网卡)、通信端口(例如串行或者并行等)、PCMCIA卡槽和卡等。经由通信接口924转移的软件和数据可以是信号928的形式,信号928可以是电子信号、电磁信号、光信号或者能够由通信接口924接收的其他信号。信号928可以经由通信路径(即,信道)926提供至通信接口924。信道926承载了信号928并可以使用导线或者电缆、光纤、电话线、蜂窝电话链路、RF链路、红外链路和其他通信信道来实现。
在本文中,术语“计算机程序介质”和“计算机可用介质”用于一般性地指代诸如可移动存储驱动914、安装在硬盘驱动912中的硬盘以及信号928等。这些计算机程序产品是用于为计算机系统900提供软件的装置。本发明包括这种计算机程序产品。
计算机程序(也称作计算机控制逻辑)可以存储在主存储器908中和/或辅助存储器910中。计算机程序还可以经由通信接口924来接收。当被执行时,这种计算机程序能够使计算机系统900执行此处讨论的本发明的特征。尤其是当被执行时,计算机程序可以使得处理器904执行本发明的功能。因此,这种计算机程序代表了计算机系统900的控制器。
在本发明使用软件实现的实施例中,软件可以存储在计算机程序产品中,并使用可移动存储驱动914、硬盘驱动912或者通信接口924加载至计算机系统900中。当由处理器904执行时,控制逻辑(软件)使得处理器904执行此处描述的本发明的功能。
在另外的实施例中,本发明主要使用例如像是专用集成电路(ASIC)硬件部件以硬件来实现。执行此处所述功能的硬件状态机的实现对于相关领域技术人员而言是显而易见的。
在另一实施例中,本发明使用软件和硬件两者的结合来实现。
在本发明的软件实施例的实例中,上述的方法可以以SPSS控制语言来实现,但是还可以以其他程序语言来实现,例如C++程序语言或者本领域技术人员可以使用的其他程序,但并不局限于此。
图10-12示出了本发明的替代实施例的方框图示。参考图10,示出了系统1010的方框图示,系统1010基本包括供病人1012使用以进行记录的葡萄糖测量计1028,该葡萄糖测量计1028尤其是记录胰岛素计量读数和测量的血糖(BG)等级。通过葡萄糖测量计1028得到的数据优选地通过适当的通信链路1014或者数据调制解调器1032、或者经由互联网入口,传送到处理器、处理台或者芯片1040,诸如个人计算机、PDA或者蜂窝电话。例如,存储的数据可以存储在葡萄糖测量计1028内,并且可以经过适当的接口电缆直接下载至个人计算机104,然后经由互联网传输至处理位置。一个实例是LifeScan公司制造的包括用于下载数据至个人计算机的接口电缆的ONE TOUCH监测系统或者监测计,其与IN TOUCH软件兼容。应当理解的是,葡萄糖测量计1028以及任何计算机处理模块或者存储器模块可以集成在单个外壳内或者提供在独立的外壳中。
葡萄糖测量计在工业上是常见的,并且基本包括任何可以作为BG采集结构工作的装置。BG测量计或者采集结构、装置、工具或系统包括用于每次检验各种旨在抽取血样(例如针刺手指)并且使用通过电磁方法读取葡萄糖浓度的仪器来确定葡萄糖级别的各种传统方法。近来,已经开发了用于确定血液分析物的浓度而无需进行抽血的各种方法。例如,授予Yang等人的美国专利No.5,267,152(在此引入以供参考)描述了一种使用近IR辐射漫反射激光光谱学来测试血糖浓度的无创性技术。在授予Rosenthal等人的美国专利No.5,086,229和授予Robinson等人的美国专利No.4,975,581中也描述了与近IR光谱测量类似的设备(其内容包含在此以供参考)。
在授予Stanley的美国专利No.5,139,023(在此引入以供参考)描述了皮肤(transdermal)血糖监测装置,该皮肤血糖监测装置依赖于透过性增强剂(例如胆汁盐)来促进间质液与接收介质之间沿着浓度梯度建立的葡萄糖的皮肤运动。授予Sembrowich的美国专利No.5,036,861(在此引入以供参考)描述了通过皮肤贴来收集排汗的无源葡萄糖监测器,其中胆碱能药(cholinergic agent)用来刺激从外分泌汗腺排汗。在授予Schoendorfer的美国专利No.5,076,273和在授予Schroeder的美国专利No.5,140,985中公开了类似的排汗收集装置(其内容包含在此以供参考)。
此外,授予Glikfeld的美国专利No.5,279,543还描述了使用电离子透入疗法来无创性地经过皮肤将物质采样至皮肤表面上的接受器中。Glikfeld教导该采样程序可以与葡萄糖专用的生物传感器或者葡萄糖专用的电极结合以监测血糖。另外,对于Tamada的国际公开No.WO96/00110(在此引入以供参考)描述了用于目标物质的皮肤监测的电离子透入疗法装置,其中电离子透入电极用于将分析物移至收集容器,生物传感器用于检测容器中存在的目标分析物。最后,在授予Berner的美国专利No.6,144,869(在此引入以供参考)描述了一种用于测量存在分析物的浓度的采样系统。
另外,BG测量计或者采集机构可以包括体内的导尿管和皮下组织流体采样。
计算机、处理器或者PDA 1040可以包括根据预定的流程顺序处理、分析和解释自动记录的糖尿病病人数据并生成适当的数据解释输出所需的软件和硬件。根据计算机1040上存储的病人数据的数据分析和解释的结果可以以通过与个人计算机940关联的打印机生成的纸件报告形式来显示。可替代的方式是,数据解释程序的结果可以直接显示在与计算机940相关的视频显示单元上。另外,结果还可以显示在数字或者模拟显示设备上。优选地,结果可以根据图7或8所呈现的特征来显示。个人计算机1040可以经过通信网络1036将数据传送至保健提供计算机1038。经过通信网络1036传送的数据可以包括自动记录的糖尿病人数据或者数据解释程序的结果。
图11示出了具有糖尿病管理系统的备选实施例的方框图示,该糖尿病管理系统是一个病人操作的装置1110,装置1110具有优选足够小型的外壳以使得装置1110可以由病人手持或者携带。用于接收血糖试片的试片导槽(guide)(未示出)位于外壳1116的表面上。试片接受来自病人的血样。该装置可以包括微处理器1122和连接至微处理器1122的存储器1124。微处理器1122设计成执行存储在存储器1124中的计算机程序,以执行在上面更加详细讨论的各种计算和控制功能。键区1116可以经过标准键盘解码器1126连接至微处理器1122。显示器1114可以经过显示器驱动1130连接至微处理器1122。显示器1114可以显示图7或8中的特征。显示器1114可以是数字和/或模拟的。扬声器1154和时钟1156还可以连接至微处理器1122。扬声器1154在微处理器1122的控制下进行操作,以发出警告病人可能的未来低血糖或高血糖危险的听得到的声调。时钟1156为微处理器1122提供当前日期和时间。
存储器1124存储微处理器1122所使用的病人1112的血糖值、胰岛素剂量值、胰岛素类型以及参数,以计算未来的血糖值、补偿的胰岛素剂量和碳水化合物补偿。每个血糖值和胰岛素剂量值可以与对应的日期和时间一起存储在存储器1124中。存储器1124优选地为非易失性存储器,诸如电可擦除只读存储器(EEPROM)。
装置1110还可以包括连接至微处理器1122的血糖测量计1128。血糖测量计1128可以设计成测量血糖试片上接收的血样并根据血样的测量结果提供血糖值。正如前面所提及的那样,这种血糖测量计在本领域是公知的。血糖测量计1128优选地是产生直接输出至微处理器1122的数字值的类型。备选地,血糖测量计1128可以是产生模拟值类型的测量计。在备选的实施例中,血糖测量计1128经过模数转换器(未示出)连接至微处理器1122。
装置1110可以进一步包括连接至微处理器1122的输入/输出端口1134,优选地是串行端口。端口1134可以通过接口(优选地是标准RS232接口)连接至调制解调器1132。调制解调器1132用于经过通信网络1136在装置1110与个人计算机1140或与保健提供计算机1138之间建立通信链路。用于经过连接塞绳连接电子设备的专用技术在本领域中是公知的。另外的替代实例是“蓝牙”通信技术。

Claims (104)
1.一种用于识别和/或预测用户的高血糖模式的系统,所述系统包括:
采集模块,用于采集多个SMBG数据点;以及
处理器,所述处理器包括:
用于对具有预定持续时间的时间周期内的所述SMBG数据点进行分类的部件;
用于估算每个时间周期中的葡萄糖值的部件;和
用于基于所述估算来指示随后时间周期的高血糖危险的部件。
2.根据权利要求1所述的系统,其中所述估算部件包括:
用于基于所述葡萄糖值来确定至高血糖的个体偏差的偏差确定部件;
用于基于个体和绝对偏差确定每个所述时间周期内的复合概率的部件;和
用于将每个时间周期中的所述复合概率与预先设置的阈值进行比较的部件。
3.根据权利要求2所述的系统,其中所述偏差确定部件包括用于计算每个所述时间周期SMBG的平均和标准偏差的部件。
4.根据权利要求2所述的系统,其中所述偏差确定部件包括用于计算每个所述时间周期的偏差对比度的部件。
5.根据权利要求4所述的系统,其中所述偏差对比度计算为:

29.根据权利要求1所述的系统,进一步包括显示模块,所述显示模块用于如果出现高血糖危险时向用户显示消息。

34.一种用于识别和/或预测用户的低血糖模式的系统,所述系统包括:
采集模块,用于采集多个SMBG数据点;以及
处理器,所述处理器包括:
用于对具有预定持续时间的时间周期内的所述SMBG数据点进行分类的部件;
用于估算每个时间周期中的葡萄糖值的部件;和
用于基于所述估算来指示随后时间周期的低血糖危险的部件。
35.根据权利要求34所述的系统,其中所述估算部件包括:
用于基于所述葡萄糖值来确定至低血糖的个体偏差的偏差确定部件;
用于基于个体和绝对偏差确定每个所述时间周期内的复合概率的部件;和
用于将每个时间周期中的所述复合概率与预先设置的阈值进行比较的部件。
36.根据权利要求35所述的系统,其中所述偏差确定部件包括用于计算每个所述时间周期SMBG的平均和标准偏差的部件。

采集模块,用于采集多个SMBG数据点;以及
处理器,所述处理器包括:
用于在具有预定持续时间的时间周期内对所述SMBG数据点进行分类的部件;
用于估算每个所述时间周期中的血糖变异性的部件;和
用于基于所述估算来指示随后时间周期的更高变异性的危险的部件。
68.根据权利要求67所述的系统,其中所述估算部件包括:
用于确定具有的变异性高于其他所述时间周期的变异性的每个时间周期的个体概率的部件;
用于根据变换函数,利用或者不利用变换所述SMBG数据点来确定变异性整体指标的部件;和
用于将每个所述时间周期的所述个体概率和所述变异性整体指标与预先设置的阈值进行比较的部件。
69.根据权利要求68所述的系统,其中所述变换函数计算为:
f(BG,a,b)=c.[(ln(BG))a-b}],
其中,如果BG以mg/dl来测量,那么a=1.084,b=5.381,c=1.509,如果BG以mmol/l来测量,则a=1.026,b=1.861且c=1.794。
