Abstract
【解決手段】最適な糖尿病の管理を維持する方法、システム、コンピュータプログラム製品であって、血中グルコース自己監視装置によって収集された血中グルコース測定値に基づいて、次の期間における、低血糖、高血糖、グルコース変動の増大、不十分または過剰な検査を予測する。本方法、システム、コンピュータプログラム製品は直接的には、使用者に、高血糖、低血糖、グルコース変動の増大、無効な検査の期間を予測して警告することができるデータ知的解釈部を導入することによる既存の家庭用血中グルコース監視装置を改善、及び同機能を導入することによる血中グルコース自己監視装置の改善に関する。予測することによって、高血糖、低血糖、グルコース変動の増大による悪影響を防止できる。

Description
本特許出願は、その全開示が、参照によりその全てが本明細書に組み込まれる、2006年12月21日に出願された、発明の名称を「Systems, Methods and Computer Program Codes for Recognition of Patterns of Hyperglycemia and Hypoglycemia, Increased Glucose Variability, and Ineffective Self−Monitoring in Diabetes」とする米国特許仮出願第60/876,402号に基づく優先権を主張する。
・定期的なグルコースパターンを評価可能なデータ知的解釈部を導入することによる、既存のSMBG装置の改善、及び同機能による未来のSMBG装置の実現。
・同機能による糖尿病管理の支援を目的とする携帯可能な装置(電子手帳、PDA、携帯電話、電子メール機器、インスリンポンプ等)の改善
・同機能によるSMBGデータを取り出すソフトウェアの改善。かかるソフトウェアは、家庭用BG監視装置の製造業者がほぼ製造し、通常、SMBGデータを解釈するために、患者又は健康管理者が使用する。このソフトウェアは、患者のパーソナルコンピュータに常駐させる、或いは、インターネットポータルを介して利用できる。
・様々な糖尿病治療(インスリン、プラムリンタイド、エクセナタイドなどの変動低下剤)の有効性の評価。
・新しいインスリン送達装置(インスリンポンプ)又は閉ループ糖尿病管理システムの有効性の評価。
警告メッセージによってカバーされる次の期間の所定の時間は、2〜8時間の間の任意の時間であってよく、好ましくは6時間である。1日、すなわち24時間の期間を、所定の時間を有する時間ビンに分割してよい。説明を簡潔にするため、本開示の全体において、我々は、所定の時間を4時間、夜間は8時間を有する期間と仮定する。
SMBG測定を行う週は、約2週間以上、又は6週間を超えてもよいが、好ましくは約2〜6週間程度、特に好ましくは約4週間である。好ましくは、各期間毎に5回の測定を行う。SMBG測定の合計回数は、少なくとも30回の測定でもよいが、60回が好ましい。
1.SMBGデータから高血糖又は低血糖パターンを特定するアルゴリズム
これらの偏移は、以下の2つのレベル、即ち:
・集団のデータ、文献、及び許容される標準的治療法ガイドラインによって特定される絶対的なしきい値を上回っていること、及び
・固有のしきい値、すなわち、各個人の血糖パターンの解析によって決定される個人のしきい値を上回っていることによって評価される。
・集団レベルの高BGの定義を反映したBGのしきい値のパラメータ(α1)。
例えば、α1=180mg/dl又はα1=200mg/dlが許容される値である。
・特定の個人において、特定の期間の間にBGが高くなる固有の確率を反映した、個人毎の複合確率のしきい値のパラメータ(β1)。例えば、β1=0.4〜0.6が、許容される値である。
・集団レベルの低BGの定義を反映したBGのしきい値のパラメータ(α2)。例えば、α2=70mg/dl又はα2=75mg/dlが許容される値である。
・特定の個人において、特定の期間の間にBGが低くなる固有の確率を反映した、個人毎の複合確率のしきい値のパラメータ(β2)。例えば、β2=0.01〜0.2が、許容される値である。
(1)最近の2〜6週間の監視の間に収集されたSMBGデータを、それぞれの測定時間と共に読み出し;

図4〜図6は、高血糖の時間パターンを特定する計算結果を示す。図4において、夜間の期間が、高血糖のリスクが高いと特定された。図5において、午後3〜7時、及び午後7〜11時の2つの期間が、高血糖のリスクが高いと特定された。図6において、午後11時〜午前11時の期間が、高血糖のリスクが高いと特定された。
2.SMBGデータからグルコースの変動の増大のパターンを特定するアルゴリズム
・集団レベルの高いグルコース変動の定義を反映した、ADRRしきい値のパラメータ(α)。
例えば、α=30又はα=40mg/dlが、許容値である。
・この所定の期間の間にこの特定の個人について、変動が高くなる固有の確率を反映した、それぞれの確率のしきい値(β)。
例えば、β=0.6〜0.8が許容値である。
(1)最近の2〜6週間の監視の間に収集されたSMBGデータを、それぞれの測定時と共に読み出す。
(2)SMBG測定時に、それぞれの日の24時間を、所定の時間(2〜8時間)を有するM個の時間ビンに分割する。時間ビンは、2〜6週間にわたって適当な数のSMBG測定値を蓄積できるようにするために定義された、適当な長さを有する期間であると定義される。例えば、測定が午後11時に行われると仮定すると、M=5個の時間ビンを、以下のように定義することができる:1〜[午後11時〜午前7時);2〜[午前7時〜11時);3〜[午前11時〜午後3時);4〜[午後3時〜7時);5〜[午後7時〜11時)。ここで、“]”は、その時間を含むことを意味する;

(4)それぞれのBG測定値を、既に導入された式:f(BG,a,b)=c・[(ln(BG))a−b}]を用いて、「リスク空間」に変換する。ここで、この関数のパラメータは、以下のとおりである。BGは、mg/dl単位で測定した場合、a=1.084、b=5.381、及びc=1.509である。BGがmmol/l単位で測定した場合、a=1.026、b=1.861、及びc=1.794である(28)。
(5)それぞれの時間ビンについて、全てのリスク標準偏差RSDを、下式のように計算する。
(7)それぞれの時間ビンについて、比Zk=5*(RSDk/RSD−1)を計算する。
大量のデータベース(N=233名の被験者)の解析により示したように、これらの比は、ほぼ中心正規分布を有しており、したがって、前節と同様の方法により、固有の偏差の検査に用いることができる。
(8)それぞれの時間ビンについて、Zk>0であるそれぞれの個人の確率をp(Zk>0)=Φ(Zk)として計算する。ここで、Φは、前節で示された多項式近似によって計算された中心正規分布の分布関数である。p(Zk>0)は、ある時間ビンが、他の時間ビンよりも高い変動を有する個人の確率である。

図7は、変動の増大のパターンの計算を示し、午後3〜7時の期間が、変動の大きな期間として特定されている。ADRRのしきい値は40に設定され、確率のしきい値は0.6に設定されている。
3.無効なSMBG検査のパターンを特定するアルゴリズム
図8に、有効でないSMBG測定のパターンを示す。有効でない検査に対するしきい値を5%に設定した場合、被験者Aについては、夜間にSMBG測定値の3.2%しか測定されていないのに対し、被験者Bについては、午後にSMBG測定値の4%しか測定されていない。
本発明の方法は、ハードウェア、ソフトウェア、又はこれらの組み合わせを用いて実装することができ、又は1若しくは複数のコンピュータシステム、又は携帯情報端末(PDA)等の他の処理システム、又は適当なメモリ及び処理能力を有するグルコース自己診断装置(SMBGメモリ計測器等)に直接実装することができる。実施の形態の一例において、本発明は、図9に示したように、汎用コンピュータ900上で動作するソフトウェアに実装することができる。コンピュータシステム900は、プロセッサ904等の1つ又は複数のプロセッサを含んでいてよい。プロセッサ904は、通信基盤906(通信バス、交差バー、ネットワーク等)に接続されていてもよい。コンピュータシステム900は、通信基盤906(又は図示しないフレームバッファ)からの画像、テキスト、又は他のデータを表示装置930に伝送する表示インターフェース902を含んでいてもよい。表示装置930は、デジタル及び/又はアナログであってもよい。
認識されるように、着脱式記録装置918には、コンピュータソフトウェア及び/又はデータが記録された、コンピュータが使用可能な記録媒体が含まれる。
このような手段としては、例えば、着脱式記録装置922及びインターフェース920が挙げられる。このような着脱式記録装置/インターフェースの例としては、(ビデオゲーム装置において見られるもの等の)プログラムカートリッジ及びインターフェース、着脱式メモリチップ(ROM、PROM、EPROM、又はEEPROM等)及び接続ソケット、並びにソフトウェアを、着脱式記録装置922からコンピュータシステム900に転送できる他の着脱式記録装置922及びインターフェース920が挙げられる。

キーパッド1116が、標準的なキーパッドデコーダ1126を介してマイクロプロセッサ1122に接続されていてよい。表示装置1114が、表示装置ドライバ1130を介してマイクロプロセッサ1122に接続されていてよい。表示装置1114は、図7又は8に示された指標を表示することができる。表示装置1114は、デジタル及び/又はアナログであってよい。スピーカ1154及び時計1156がマイクロプロセッサ1122に接続されていてもよい。スピーカは、マイクロプロセッサ1122の制御下で動作し、患者が低血糖又は高血糖の危険を示していることを警告するための可聴音を発生する。
