技术领域

背景技术

美国专利nos.5282863和4748260公开了使用塑性的非金属杆的柔性脊柱稳定系统和方法。美国专利no.2003/0083657公开了使用柔性延长件的柔性脊柱稳定装置的另一个例子。这些装置具有柔性,但是它们不能很好地适合于经受长期的轴向负载和压力。此外,期望的柔性对刚性的程度随患者的不同而不同。存在的柔性固定装置的设计不能很好地适合于提供变化的柔性水平,以对每个单独的候选者提供最佳结果。例如,美国专利no.5672175公开了一种柔性脊柱固定装置,其使用由金属合金及/或复合材料制成的柔性杆。此外,为了沿期望的方向在椎骨上提供反旋力的目的,压缩或展开的弹簧绕杆盘绕。然而,此专利主要关注于提供允许脊柱“沿(该)垂直轴相对的纵向平移滑动运动”的脊柱固定装置,并且没有传授也没有建议可提供各种柔性特征的连接单元(例如杆或板)的任何特别设计。诸如在美国5672175中提及的现有技术的柔性杆,为了提供期望水平的柔性,典型地具有相对较小的直径的实心结构。因为它们典型地非常细以提供合适的柔性,这些现有技术的杆易于发生机械故障,并且已知道其植入患者后易破裂。
因此,传统的脊柱固定装置不能对与治疗脊柱疾病有关的问题提供全面的均衡的解决方案。许多现有装置的特征在于过于僵硬,这可导致上述的一些问题,同时其它的尽管提供一些柔性,但是不能很好地适合于提供长期稳定性且/或变化的柔性程度。因此,需要一种改善的动态脊柱固定装置,给脊柱的受伤部分提供期望的柔性水平,同时还给脊柱提供长期的耐用性和一致稳定性。
此外,在传统的将脊柱固定装置固定到脊柱的外科方法中,医生切割背部的中线至大约10-15厘米,然后将其剖开且移回到两侧。这样,医生完成肌肉解剖以暴露小平面关节的外部。接下来,解剖之后,医生使用射线照相装置(例如C臂荧光检查)发现到椎弓根的入口点,然后将脊柱固定装置(称为椎弓根螺钉)的固定件插入到椎弓根内。此后,为了给脊柱的受伤部分提供支撑和稳定性,将连接单元(例如杆或板)连接到椎弓根螺钉的上部。因而在传统的脊柱固定操作中,患者的背部被切开大约10~15厘米,结果对于维持脊柱非常重要的背部肌肉被切开或受伤,导致患者巨大的术后疼痛以及缓慢的恢复期。
最近,为了减少患者损伤,已产生一种最低程度侵入的外科手术方法,其能够穿过在外科手术位置患者背部内形成的相对小的洞或“窗”执行脊柱固定手术。通过使用内窥镜或显微镜,最低程度的侵入手术允许患者被损伤的区域的更小的切口。通过更小的切口,脊柱固定装置的两个或多个固定件(例如椎弓根螺钉)使用导航系统拧入相应的椎弓根区域。此后,使用特殊工具将固定装置的稳定件(例如杆或板)连接到固定件。替换地或附加地,外科手术可包括将逐级扩展器插入到切口内然后逐渐增加扩展器的直径。此后,管状牵引器插入到已扩张区域内,以牵引患者的肌肉并且提供用于手术的可见区域。建立此可见区域之后,如果需要,可执行减压和融合操作,随后进行固定操作,其包括以下步骤:寻找椎弓根的位置;将椎弓根螺钉插入到椎弓根内;使用内窥镜或显微镜;将稳定件(例如杆或板)固定到椎弓根螺钉,以稳定和支撑虚弱的脊柱。
执行此最低程度侵入的脊柱固定手术的最具挑战性的方面中的一个是在内窥镜或显微镜显像下,为椎弓根螺钉确定进入点的位置。通常地使用解剖界标及/或射线照相装置来寻找进入点,但是由于狭窄的工作空间,经常难于确定清楚的解剖关系。此外,此最低程度侵入手术要求大量的软组织必须被切除,以显露解剖区域用于插入椎弓根螺钉。软组织的切除导致在损伤区域流血,从而增加寻找正确的位置以插入固定件的困难,并且导致破坏围绕手术区域的肌肉和软组织。此外,由于难于为固定件精确地确定插入点的位置,因而传统的手术是不必要地创伤性的。
射线照相技术已被提议和执行,以试图更加准确地且快速地发现固定件将插入其内的椎弓根的位置。然而,通常由于外科手术中使用的金属工具和设备导致射线照相干扰,使用射线照相技术难于获得清晰的图像用于寻找椎弓根的相应的位置。此外,读取和解析射线照相的图像是一项需要大量训练和专门技术的任务。射线照相引起的进一步的问题在于患者暴露于大量的辐射中。
尽管一些引导系统已研制,其引导将椎弓根螺钉插入到椎弓根上期望的进入点,这些现有系统已证实难于使用,此外妨碍手术操作。例如,现有的用于椎弓根插入的引导系统使用较长的线,其穿过插入患者的背部肌肉和组织的引导管而插入。引导管的插入位置由射线照相装置(例如C臂荧光检查仪)确定,并被驱动直到引导管的第一端到达椎弓根的表面上的期望位置。此后,典型地由生物相容性的金属材料制成的引导线的第一端插入到引导管内,并且被推入椎弓根骨内,同时线的相反端保持在患者的背部外伸出。在引导线已固定到椎弓根骨之后,去除引导管,并且以引导线为中心的孔被扩大且牵引。最后,具有配置成穿过其以容纳引导线的轴向孔或通道的椎弓根螺钉通过引导线被引导到椎弓根骨上期望的位置,其中椎弓根螺钉被螺旋拧入椎弓根。
尽管线引导系统的想法不错,然而在实际中引导线非常难使用。因为它是相对较长且细的线,在尝试驱动线的一端进入椎弓根骨内时,引导线的结构完整性经常失效,使得此过程不必要的费时费力。此外,因为在插入期间线弯曲且卷曲,其不能提供光滑且可靠的支撑点,用于将随后的工具和椎弓根螺钉引导到椎弓根上的进入点。此外,目前经由皮肤的线引导系统与C臂荧光检查仪(或其它射线照相装置)协同使用,但没有使用内窥镜或显微镜得到的直接的显像。因此,目前的线引导系统引起误放置或椎弓根破坏的潜在危险。最后,由于线的一端保持在椎弓根螺钉的头部和患者背部外伸出,线妨碍外科医生在执行各种随后的包括脊柱固定手术的操作中运动的自由。因而,有必要提供改善的引导系统,适合于在内窥镜或显微镜显像下用于最低程度侵入的椎弓根螺钉固定手术中,其更容易地植入椎弓根,并且不会妨碍随后的由外科医生执行的手术。
如上所讨论,现有的用于治疗脊柱疾病的方法和装置需要大量的改进。大多数传统的脊柱固定装置太刚硬且不柔软。过多的刚度导致脊柱的进一步的异常和疾病,以及对于患者的巨大的不舒适。尽管一些现有的脊柱固定装置的确提供了一定水平的柔性,但这些装置没有被设计或制造,使得可容易地获得变化的柔性水平,来为每个特殊患者提供期望的柔性水平。此外,现有技术的具有柔性连接单元(例如杆或板)的装置引起机械失效的更大的危险,并且不提供脊柱的长期耐用性和稳定性。另外,执行脊柱固定手术的现有方法,由于难于寻找其中脊柱固定装置将固定的椎弓根或脊椎的骶椎的精确位置,对于患者是不必要的创伤性的。
发明内容
本发明通过提供用于稳定受伤或虚弱的脊柱的改善的方法和系统,解决上述和其它的需求。
为了克服传统脊柱固定装置的不足,在一实施例中,本发明的发明人已发明一种新颖的柔性的具有改善的结构和设计的脊柱固定装置,其耐用并且提供期望水平的柔性和稳定性。
作为长期研究的结果,以减少用于最低程度侵入的脊柱手术的操作时间,减少损伤手术区域附近的组织,在另一实施例中,本发明提供了用于精确地且快速地寻找,脊柱固定装置的,固定件将插入其中的,脊柱的位置的方法和装置。一种新颖的引导/标记装置用于指示脊柱中的固定件将插入的位置。

具体实施方式
在下文中将参考附图对本发明进行详细描述,其中自始至终同样的标号表示同样的元件。
图1描述了根据本发明的实施例的脊柱固定装置。此脊柱固定装置包括两个固定件2(指定为2’和2”),以及配置成容纳且固定在接头组件14内的柔性固定杆4,如下文中关于图3的进一步的详细描述。每个固定件2包括螺钉类型的螺纹轴10,其配置成插入且拧入患者的椎弓根内。如图1所示,螺钉类型的轴10包括在轴10的长度上形成的螺旋状外螺纹12,以及配置成在指定位置插入到患者的脊柱内的轴10的端部的圆锥端。固定件2的其它已知形状可连同设置有固定件2的本发明一起使用,固定件2可插入且固定到脊柱内并且可确保与杆4配合。
如上所述,通过将固定件2固定在脊柱内期望的位置,脊柱固定装置用于脊柱疾病的外科治疗。在一实施例中,杆4穿过两个或多个脊柱的椎骨延伸,并且通过固定件2固定,以致稳定两个或多个椎骨的运动。
图2示出了根据本发明的另一实施例的脊柱固定装置的透视图。图2中的脊柱固定装置,除了杆4包括并列于杆4的两刚性端部9之间的柔性中间部8以外,与图1中的脊柱固定装置类似。
图3根据本发明的实施例,提供了图1和2中的固定件2的示出接头组件14的各种组件的分解图。如图3所示,配合件14包括位于螺钉类型的轴10的顶端的圆柱头16、沿圆柱头16的内壁表面的部分形成的螺旋状螺纹或凹槽18,以及配置成将杆4容纳在其内的U型底座凹槽20。接头组件14进一步包括外螺纹螺母22,其具有在螺母22的侧面外部形成的螺旋状螺纹24,其中螺旋状螺纹24配置成与圆柱头16的螺旋状内螺纹18相配。在另一实施例中,接头组件14包括固定帽26,其配置成安装在圆柱头16的一部分之上,以覆盖和保护外螺纹螺母22,并且更加确保将杆4保持在底座凹槽20内。在一实施例中,固定帽26的内直径配置成确保与圆柱头16的外直径相配。其它的将固定帽26固定到圆柱头的方法,比如对应地定位的凹口或凹槽(未示出),对于本领域技术人员将是很显然的。在优选地实施例中,固定件2的元件和部件可由例如不锈钢、钢铁、钛或钛合金的高刚性和耐用的生物相容性的材料制成。此外地或替换地,非金属的生物相容性的材料也可使用,例如聚合体、弹性体、树脂、陶器及其复合材料。这些材料在本领域是已知的。如本领域也已知的和在此使用的生物相容性材料,涉及那些在植入到患者的体内后不会产生任何化学的或免疫的副作用的材料。
如图1和2所示,在优选的实施例中,通过将杆4水平地固定垂直于固定件2的螺纹轴10的长度方向的配合件14的底座凹槽20内,杆4配合到固定装置2。然后外螺纹螺母22在杆4上方被容纳且拧入圆柱头16内,以致将杆4固定在底座凹槽20内。固定帽26此后放置在圆柱头16上面,以覆盖、保护且更稳固地将元件固定在圆柱头16的内腔中。根据本发明,图4-7示出了可用于固定装置的杆4的各种实施例的透视图。图4示出了图1中的杆4,其中整个杆被制造且设计成可弯曲。在一实施例中,杆4包括具有预定厚度的圆柱壁5的金属管或管道。在替换实施例中,杆4可包括由生物相容性的合成金属混合材料或完全由生物相容性的合成材料制成。生物相容性的金属的例子有钛、不锈钢、锆、钽、钴、铬、镍及其合金。生物相容性的合成材料的例子有聚合体、弹性体、树脂、塑料、碳石墨及其复合材料。这些材料在本领域广为人知。
在一实施例中,为了给杆4提供柔性,圆柱壁5以螺旋方式沿杆4的长度被切割,以形成螺旋切口或凹槽6。如同对于本领域一名普通技术人员显然的,螺旋凹槽6的宽度和密度可调整以提供期望水平的柔性。在一实施例中,凹槽6由穿透杆4的圆柱壁的整个厚度的非常细的螺旋切口或切槽形成。如对于本领域技术人员所已知的,管状壁5的厚度和材料也影响柔性水平。
在一实施例中,杆4设计成具有大体上与正常的背部的柔性相似的柔性。本领域技术人员已知正常的背部的柔性范围,并且其中一个普通技术人员可容易地确定管状壁5的厚度和材料,以及凹槽6的宽度和密度,以在正常的背部的柔性范围内实现期望的柔性或柔性范围。当在此提及凹槽6时,术语“密度”涉及螺旋凹槽6的紧密度,或换句话说,例如如图4所示的相邻凹槽线6之间的距离。然而,应理解本发明不限于特殊的预定的柔性范围。在一实施例中,除了具有期望的横向柔性特征之外,杆4的刚性应能够承受相对于患者的其余正常脊柱以相似的方式沿脊柱的垂直轴施加到患者的脊柱的垂直轴向负载。
图5示出了图2中的杆4,其中仅中间部8制成且设计成柔性的,并且两端部9制成刚性的。在一实施例中,其内没有凹槽的金属末端环或帽9’可套在图4中的杆4的各端部上,使得端部9具有刚性。环或帽9’可使用诸如挤压及/或与金属焊接在一起的已知方法永久性地固定到杆4的端部。在另一实施例中,螺旋凹槽6仅沿中间部8的长度被切割,并且端部9包括没有凹槽6的管状壁5。由刚性金属或金属混合材料制成的没有凹槽6的管状壁5表现出较高的刚性。
图6示出了具有多个部分的杆4的另外实施例,两个柔性部8插入在三个刚性部9之间。例如此实施例可用于相对于彼此稳定三个相邻椎骨,其中三个椎弓根螺钉固定到相应的一个椎骨,并且三个刚性部9连接到相应的椎弓根螺钉2的接头组件14上,如上文中关于图3的描述。柔性部8和刚性部9的每一个可制成如上文中关于图5的描述。
图7示出了杆4的另一实施例,其具有预弯曲结构和构造,以符合且保持患者的被称为脊柱前弯症的脊柱的弯曲,同时稳定脊柱。一般地,患者的腰椎是“C”型形状,根据本发明的实施例,杆4当在图2中的脊柱固定装置中使用时,其结构形成与正常腰椎形状一致。在一实施例中,预弯曲杆4包括中间部8,其被制成且设计成柔性的,插在两刚性端部9之间。中间部8和端部9可制成如上文中关于图5的描述。制造各种大小、长度和预弯曲结构的金属或混合金属管状杆的方法在本领域是广为人知的。另外地或替换地,如下文中关于图23A将进一步详细描述,当两相邻椎弓根螺钉不是平行于彼此插入时,杆4的预弯曲结构和设计可偏置一歪斜角度。
用于产生柔性管状杆4或柔性中间部8的其它设计和材料将在下文中关于图8-10描述。图8示出了根据本发明的实施例的柔性管状杆4或杆部8的横截面透视图。在此实施例中,如上文中关于图4-7所描述的,柔性杆4、8由在其内切割有螺旋凹槽6的第一金属管5制成。在其内切割有螺旋凹槽31且具有比第一管5更小的直径的第二管30插入到第一管5的圆柱腔内。在一实施例中,第二管30具有螺旋凹槽31,其关于第一管5中切割的螺旋凹槽6沿相反的螺旋方向被切割,使得第二管30的旋转扭矩特征抵消第一管5的旋转扭矩特征的至少一部分。第二柔性管30插入到第一管的中心内,以给柔性杆4、8提供更多的耐用性和强度。第二管30可由与第一管5相同或不同的材料制成。在优选的实施例中,分别用于制造第一管5和第二管30的材料可以是下面的示例性的生物相容性金属中的任何一种或组合:钛、不锈钢、锆、钽、钴、铬、镍、铝、钒以及其合金。在替换的实施例中,管5和30可由生物相容性的合成金属混合材料或完全由生物相容性的合成材料制成。生物相容性的合成材料的例子有聚合体、弹性体、树脂、塑料、碳石墨及其复合材料。这些材料在本领域广为人知。
图9示出了根据本发明的另一实施例的柔性杆4、8的横截面透视图。在一实施例中,柔性杆4、8包括由生物相容性的金属缆线32制成的内芯,金属缆线32包括多个交叠的细的诸如钢纱、钛纱或钛合金纱的金属纱。缆线32由金属、或混合金属包围,如上所述柔性管5在其内切割有螺旋凹槽6。缆线32内的金属纱的数量和厚度也影响杆4、8的刚性和柔性。通过改变纱的数量、厚度或材料,柔性可增加或减小。因此,缆线32内的金属纱的数量、厚度及/或材料可调整以根据患者的特殊需求提供期望的刚性和柔性。本领域的普通技术人员可容易地确定纱的数量、厚度和材料,与管5的给定柔性相一致,以对于杆4、8实现期望的刚性柔性比特性。在替换的实施例中,如上所述,缆线32及多个纱可由生物相容性的合成金属混合材料或完全由生物相容性的合成材料制成。
图10示出了柔性杆4的另一实施例,其中柔性管5把非金属柔性芯34包起来。在各种实施例中,芯34可由已知的生物相容性的金属、生物相容性的形状记忆合金(例如NITINOL)或生物相容性的合成材料制成,诸如碳纤维、聚醚醚酮(PEEK)、聚芳醚酮酮醚酮(PEKKEK)或超高分子量聚乙烯。
图11示出了柔性杆35的另一实施例的透视图,其中如上文中关于图9所描述的,多个缆线32交织或编织在一起以形成编织缆线杆35。编织缆线杆35可由与上文所述的缆线32同样的材料制成。除了如上所述的缆线32的刚性和柔性的可变性之外,通过改变在编织结构35中使用的缆线32的数量和厚度,编织杆35的刚性和柔性可进一步修改以实现期望的特征。例如,为了在已知的正常的健康脊柱的弯曲范围内实现各种弯曲水平或范围,本领域的普通技术人员可容易地通过用于产生编织缆线杆35的缆线的不同规格、数量和材料提供的弯曲改变和测量而制造编织缆线杆35的各种设计。在另外的实施例中,编织缆线杆35的每一端由如上文中关于图5-7所描述的刚性帽或环9’包住,以提供具有柔性中间部8和刚性端部9的杆4。在另外的实施例(未示出)中,编织缆线杆35可用作由管5包住的柔性内芯,管5以与图8-10所示的实施例类似的方式在其内切割有螺旋凹槽6,以产生柔性杆4或杆部8。作为在此使用的术语“编织”或“编织结构”包括两个或多个缆线、带、绳、丝带及/或其它形状的材料以交叠方式交织。缆线、带、绳、丝带及/或其它形状的材料的各种交织方法在本领域已知。本发明包括这些交织技术。在另外的示例性的实施例(未示出)中,柔性杆35包括具有两个或多个带、绳或丝带以对角交叠的样式交织的编织结构。
图12A示出了柔性连接单元36的另一实施例,其具有两个刚性端部9’和插在端部之间的示例性数量的隔离物37。在一实施例中,刚性端部9’和隔离物可由如上所述的生物相容性的金属、混合金属、及/或合成材料制成。连接单元36还包括柔性件或缆线32,如上文中关于图9所讨论的,其穿过每个刚性端部9’和隔离物37中的轴向洞或孔(未示出)。图12B示出了连接单元36的分解图,其进一步示出了缆线32如何穿过刚性端部9’和隔离物37的纵向轴孔而插入。如图12B进一步所示,端部9’和隔离物37的每一个包括凸互锁件38,其配置成与紧邻端部9’或隔离物37的凹互锁洞配合。图12C示出了分解侧视图,且用虚线指出用于容纳相应的凸互锁件38的凹互锁洞39的位置和结构。
图13示出了根据本发明的另一实施例的柔性连接单元40的透视图。连接单元40与上述的连接单元36类似,然而,隔离物42配置成具有与刚性端部9’同样的形状和设计。此外,端部9’具有位于横向侧表面的穿过其缆线32可退出的退出口或槽44,其使用金属夹(未示出)或其它已知技术而被拉紧且夹紧或固定。由此柔性连接件36或40的长度在手术时可变化,以适合每个患者的特殊的解剖特征。在一实施例中,缆线32可使用金属夹或档止器(未示出)而固定。例如,夹或档止器可包括较小的具有比缆线32的直径稍大的内径的管状圆柱,以允许缆线32通过。在缆线32通过管状档止器拉至期望的拉紧状态之后,档止器被压缩以夹紧容纳在其内的缆线32。替换地,在制造具有预定数目的隔离物37、42在其内的连接单元36、40期间,缆线32可使用已知的技术预固定。
图14描述了根据本发明的另一实施例的脊柱固定装置。此脊柱固定装置包括:至少两个固定件2,其包括伸长的螺钉类型的具有外螺旋螺纹12的轴10,以及接头组件14。此装置还进一步包括板连接单元50,或简化为“板50”,其配置成与两固定件2的配合部14牢固连接。板50包括两个刚性连接件51,每个具有平面表面且通过柔性中间部8彼此连接。柔性中间部8可根据关于图4-11的上述任何实施例制成。每个连接件51包括配合孔52,其配置成容纳穿过其的接头组件14的第二螺纹轴54(图15)。
如图15所示,固定件2的接头组件14包括螺栓头56,其邻接第一螺纹轴10的顶端,且具有比第一螺纹轴10的周长更大的周长或直径。第二螺纹轴54从螺栓头56向上延伸。接头组件14进一步包括具有配置成与第二螺纹轴54相配合的内螺纹的螺母58,以及一个或多个垫圈60,用于夹紧连接件51顶着螺栓头56的上表面,从而牢固地将板50连接于椎弓根螺钉2。
图16A和16B示出了板连接单元40的两个实施例,其具有至少两个配合件51和插入期间且连接于两相邻连接件51的至少一个柔性部8。如图16A和16B所示,柔性中间部8包括如上关于图11所述的柔性编织缆线结构36。然而,柔性部8可根据关于图4-11的上述任何实施例或其结合而被设计和制造。图16C和16D分别示出了图16A中的板50的侧视图和顶视图。如上所述,具有不同类型的柔性中间部8的柔性连接单元50和58的不同实施例的制造,使用已知的冶金、有机聚合体、天然树脂,或复合材料,以及合适的制造和加工过程可容易地完成。
图16E示出了根据本发明的另一实施例的预弯曲板连接单元50’的侧视图。除了在板连接单元50’的制造期间,连接件51’从平行平面53以角度θ形成或弯曲之外,此板连接单元50’类似于板50。如上关于图7中的类似杆的预弯曲连接单元4的所述,此预弯曲结构设计成仿效且支撑脊柱的自然弯曲(例如脊柱前弯症)。另外地或替换地,如下文中关于图23A将进一步详细描述,当两相邻椎弓根螺钉不是平行于彼此插入时,此预弯曲结构可抵消歪斜角度。
图17示出了具有两个平面连接件62的板连接单元60的透视图,每个在其内具有用于容纳椎弓根螺钉2的第二螺纹轴44的配合孔64。柔性中间部8插入在两连接件62之间并且连接于其上。在一实施例中,柔性中间部8除了具有矩形结构而不是如图9所示的圆柱或圆形结构之外,其以类似于关于图9的上述缆线32的方式制造。然而,应理解的是柔性中间部8可根据前述任何实施例的设计和材料制成。
图18示出了图17中的板60的另一实施例的透视图,其中配合孔64包括一个或多个切削入连接件62的上部的螺母导引槽66,以将螺母58(图15)置于且固定到配合孔64内。螺母导引槽66配置成将螺母的至少一部分容纳且保持在其内,并且在连接件62已夹紧到椎弓根螺钉2的螺栓头56之后防止螺母58的横向滑动。
图19示出了板杆结合的连接单元70的透视图,其在连接单元70的一端具有如上关于图4-7所述的刚性类似杆的连接件4、9或9’,以及在连接单元70的另一端具有如上关于图14-18所述的类似板的连接件51或62。在一实施例中,插入在类似杆的连接件9(9’)和类似板的连接件52(64)之间的是柔性件8。此柔性件8可根据关于图8-13的上面描述的任何实施例而被设计和制造。
图20示出了使用图19中的板杆结合连接单元70的脊柱固定装置的透视图。如图20所示,此固定装置使用两类固定件2(例如椎弓根螺钉),如上关于图15所述第一固定件2’配置成牢固地保持板连接件42(64),并且如上关于图3所述第二固定件2”配置成牢固地保持杆连接件4、9或9’。

图23A示出了脊柱固定装置在植入到两相邻椎骨的椎弓根之后的侧视图。如此图所示,椎弓根螺钉2安装到椎弓根骨内,使得螺钉2的中心轴80从平行平面82偏移角度θ,并且螺钉2的中心轴80彼此大概偏移角度2θ。由于执行最低程度的侵入手术时有限的可使用的空间,椎弓根螺钉2的这种非平行插入经常发生。此外,由于患者的脊柱自然弯曲(例如脊柱前弯症),椎弓根螺钉2具有从平行到歪斜的趋势。因而,由于椎弓根螺钉2如何最后固定到椎弓根的非平行特征,当将杆或板连接单元连接于每个椎弓根螺钉2时,期望抵消此歪斜。
图23B示出了根据本发明的实施例的椎弓根螺钉的头部的侧视图。此螺钉2包括圆柱头84,除了圆柱头84包括配置成沿倾斜的方向容纳且保持柔性杆4的倾斜底座86,以抵消如上所述的椎弓根螺钉2的倾斜或歪斜θ之外,其与关于图3的上述圆柱头16类似。改进的椎弓根螺钉2还包括倾斜的稳定隔离物88,其配置成牢固地安装在圆柱头84的洞内,并且如倾斜底座86以同样的倾斜保持杆4向下。椎弓根螺钉2还包括外螺纹螺母22,其配置成沿圆柱头84的内表面(未示出)与螺旋螺纹配合,用于向下夹紧且将倾斜隔离物88和杆4固定到倾斜底座86,从而固定到椎弓根螺钉2的圆柱头84。
图23C示出了根据本发明的实施例的倾斜隔离物88的透视图。此隔离物88包括圆形中间部90以及两个从圆形中间部90的相对侧向外延伸的矩形端部92。图23D示出了隔离物88的侧视图,其进一步示出了补偿或抵消椎弓根螺钉2的歪斜角θ的从一端到另一端的倾斜。图23E示出了配置成将杆4和倾斜隔离物88容纳在其内的圆柱头84的顶视图。杆4穿过两个开口或槽94而容纳在圆柱头84的圆柱壁内,其允许杆4进入圆柱头84的圆形或圆柱形洞96,并且支撑在圆形或圆柱形洞94内形成的倾斜底座86的顶端上。在杆4定位在倾斜底座86上之后,倾斜稳定隔离物88容纳在洞96内,使得两矩形端部92容纳在两槽94内,由此防止隔离物88在圆柱洞96内的横向旋转。最后,外螺纹螺母22和固定帽26插入倾斜隔离物88的顶端上,以牢固地将隔离物88和杆4保持在圆柱头84内。
图24示出了标记和引导装置100的透视图,其用于在椎弓根螺钉2将插入的椎弓根上标记期望的位置,并且使用最低程度侵入的外科技术将椎弓根螺钉2引导至标记的位置。如图24所示,标记装置100包括管状空心引导件52,其空心内容纳内套针104,该内套针在一端具有穿透患者的肌肉和组织直到椎弓根的锋利端105。此内套针104还包括在一端的用于容易地插入和去除套针104的套针把手106。在一实施例中,标记和引导装置100包括引导件把手108,以允许更容易地操作装置100。
如图25所示,套针104是长管或圆柱形状,其具有比引导管102的空心的内径更小的直径,以便于插入到管状引导管102的空心内。套针104还包括用于穿过椎弓根穿透椎骨体的锋利的或尖的尖端105。套针104还包括具有比引导件管102的空心直径更大的直径的套针把手106,以挡止套针104完全穿过空心滑动。套针把手106还允许更容易地操作套针104。
图26A和26B提供了标记和引导装置100在插入到患者的背部且推动穿过肌肉和软组织到达椎弓根上期望的位置之后的透视图。使用已知的诸如X射线或射线照相成像的技术用相对较短的时间确定期望的位置。在标记和引导装置100已插入之后,患者不必延长地暴露于X射线。如图26B所示,在引导管102定位在椎弓根上期望的位置之后,移去内套针104,以允许基准针(未示出)插入到引导管102的空心内,且从而固定于椎弓根内。
图27A和27B分别示出了基准针110和112的两实施例的透视图。如上所述,根据本发明的基准针110和112在穿过空心引导管102之后,插入且固定在椎弓根内。针110和112具有圆柱形状,其直径比引导管102的空心的内径小,以穿过引导管102的空心。每个基准针的一端是锋利的尖端111,其配置成容易地插入且固定在脊柱的椎弓根内。在一实施例中,如图27B所示,基准针的另一端结合配置成与用于取回针112的取回装置(未示出)的内螺纹管配合的螺纹轴114。在下文中将关于图32对此取回装置进行更详细的描述。
为了容易地插入到椎弓根骨内,基准钉110、112优选地由耐用的且刚性的生物相容性的金属制成(例如不锈钢、钢铁、钛、钛合金)。对比现有技术的引导缆线,由于其相对更短的长度和更高的刚性结构,基准钉110、112可容易地被驱动进入椎弓根内,而没有弯曲或结构失效的危险。如上所述,现有技术的引导缆线的驱动过程经常非常困难且费时。基准钉110、112进入到椎弓根上的进入点对于外科医生更容易且便利,此外,不会由于在患者的背部外伸出的引导缆线而妨碍后续过程。
图28示出了具有比推动套针116的主体更大的直径的圆柱头118的圆柱形推动套针116。根据本发明,在基准针110或112已插入到引导管102的空心内之后,推动套针116插入到引导管102的空心内,以将基准针110或112驱动且固定到椎弓根内。在针插入过程中,医生用凿子或锤子敲打套针头118,以将基准针110和112驱动进入椎弓根内。在优选实施例中,推动套针116是圆柱管状,其具有比引导管112的空心的内径更小的直径。推动套针116还包括圆柱头118,其具有比推动套针116的直径更大的直径,以允许医生更容易地用凿子或锤子敲打。当然,在替换实施例中,锤子或凿子不是必须的。例如,取决于每种情况下的环境,外科医生可选择用他或她的手掌或其它物体推动或轻敲推动套针116的头部118。
图29A示出了锤子或槌棒及推动套针116如何用于驱动针110、112穿过引导管102的空心且进入椎弓根的指定位置内。图29B示出了两基准针110、112已被驱动且固定到两相邻的椎骨内之后脊柱的截面透视图。
如上所述,在基准针110或112已插入到椎弓根内之后,在一实施例中,产生以每个针110、112为中心的更大的孔或区域,以允许容易地将椎弓根螺钉2插入且固定到椎弓根内。此更大的孔用如图30所示的环锥112产生。此环锥122插在固定在椎弓根的期望位置的基准针110、112之上。此锥122是圆柱空心管,其中空心的内径比基准针110和112的外径要大,使得针110、112可插入到锥122的空心内。此锥122还包括在一端的用于切削和碾碎组织和骨的一个或多个锋利齿124,以便于产生更大的以基准针110、112为中心的进入点,使得椎弓根螺钉2可更容易地植入椎弓根内。图31示出了当环锥122插入到患者的背部内最低程度侵入的切口内在基准针110、112之上,来为椎弓根螺钉2(未示出)产生更大的插入孔时,患者的脊柱的截面透视图。如图31所示,牵引器130已插入到手术区域之上的最低程度侵入切口内,并且牵引器130的下管体向外扩张,推动周围的组织远离手术区域,且为外科医生提供更多的空间和视觉区域来手术。为了插入牵引器130,在一实施例中,最低程度侵入切口在患者的背部内用于插入两基准针110、112的引导管102的两进入点之间产生且连接。在牵引器130插入之前,最低程度侵入的切口的在先扩张典型地需要使用一系列分段扩张器(未示出),每个随后的扩张器具有比前面的扩张器更大的直径。在最后的扩张器到位之后,牵引器130在其下管体处于收缩未扩张的状态下插入。牵引器130朝向椎弓根推动到期望的深度之后,下管体被扩张,如图31所示。分段扩张器和牵引器的使用在本领域广为人知。
在一实施例中,插管锥122为椎弓根螺钉2产生更大的插入孔之后,基准针110、112被移去。如上所述,如果使用基准针112,可使用取回装置140,以在植入椎弓根螺钉2之前移去基准针112。如图32所示,取回装置140包括较长的管或圆柱部,其具有配置成与基准针112的外螺纹上部114相配合的内螺纹端142。取回装置端部142已拧到螺纹端114之后,医生可将基准针112拉出椎弓根。在另一实施例中,如果使用没有螺纹上部的基准针110,可使用合适的工具(例如特殊设计的针鼻钳)将针110取出。
在替换实施例中,基准针110、112不从椎弓根中拔出。相反地,特殊设计的椎弓根螺钉144可在针110、112之上插入到椎弓根内,而不用先移去针110、112。如图33所示,特殊设计的椎弓根螺钉144包括外螺纹轴10以及接头组件14(图3),其包括用来容纳柔性杆状的连接单元4(图4-13)的圆柱头16(图3)。替换地,接头组件14可配置成容纳如图14-20所示的板状连接单元。此椎弓根螺钉144还包括在螺纹轴10内的纵向轴向通道(未示出),螺纹轴10在其尖端具有开口146,且配置成将基准针110、112容纳到其内。
图34示出了使用插入装置150将椎弓根螺钉2插入到脊柱的第一椎弓根内之后,患者的脊柱的截面透视图。在本领域已知的各种类型的插入装置150可用于插入椎弓根螺钉2。如图34所示,在第一椎弓根螺钉2已植入之后,牵引器130被调整且稍微移动,以在第二基准针110、112的位置提供用于插入第二椎弓根螺钉的空间和视场。
图35提供了根据本发明的实施例的两个椎弓根螺钉2已经植入两相应的相邻椎弓根内之后,患者的脊柱的截面透视图。在椎弓根螺钉2已经到位之后,关于图4-20的如上所述的柔性杆、板或结合的连接单元可连接到椎弓根螺钉,以提供脊柱的柔性稳定。此后,移去牵引器130并且关闭且/或缝合最低程度侵入的切口。
图36A示出了根据本发明的另一实施例的用于脊柱固定的柔性杆200的透视图。杆200配置成由关于图1-3的如上所述的固定件2固定。在优选的实施例中,杆200以及下面将描述的杆210、220、230和240包括由已知的生物相容性的材料制成的实心的圆柱形状的杆,例如如上所述的不锈钢、钢铁、钛、钛合金、NITINOL以及其它合适的金属、合成金属混合材料或非金属材料或复合材料。如图36A所示,螺旋槽202沿杆200的圆柱体的至少一部分长度被切割或形成。在示例性的实施例中,杆的长度“l”可在4至8厘米之间,并且其圆柱直径“D”在4至8毫米之间。此螺旋槽202具有在0.1至0.5mm之间的宽度“w”,以及离水平线50至85度之间的螺纹角θ。螺旋槽202之间的距离可在3至6mm之间。然而,如本领域技术人员所了解的,上述尺寸仅是示例性的,并且可被改变以实现期望的适合于特殊患者或应用的柔性、扭矩和强度特征。
图36B示出了柔性杆200沿图36A中的线B-B的横截面图。如所示的,螺旋槽202朝向圆柱杆200的中心纵向轴被切割。此槽可以如用于实心或空心杆的螺旋或中断的螺旋的螺旋形式连续地形成,或者是如用于实心杆的断开的圆周槽。如果空心杆具有在其内形成的断开的圆周槽,此槽仅可部分地穿透杆材料以避免中断。在一实施例中,如图36B所示,槽202的深度大约等于杆200的圆柱半径,并且穿透如圆柱杆200的中心纵向轴一样深。然而,杆的横截面面积和形状、槽深、槽宽、槽横截形状以及纵向件的槽部的槽与槽的间距可改变,以调整如期望的机械和结构特征。例如,加深或加宽槽将增加柔性,而增加槽与槽的间距减小柔性。这可用于修改在固定的弯曲力下弯曲的杆的尺寸,定制杆的弯曲形状,并且补偿弯曲时杆内的机械应力,以最小化材料疲劳且改善杆的可靠性。
图37A示出了根据本发明的另一实施例的用于脊柱固定的柔性杆210。此杆210包括多个在杆210的主体内钻或形成的横向孔或通道212。在一实施例中,通道212经过圆柱杆210的中心纵向轴且与水平线成角度φ。用于每个相应的通道212的开口位于杆210的圆柱壁的相对侧,并且相邻通道212共有在圆柱壁的一侧的共用开口,如图37A所示,形成横向穿过杆210的中心纵向轴的Z字样式的内通道212。在一实施例中,取决于杆210的期望的机械和结构特征(例如柔性、扭矩和强度),每个通道212的直径D可在0.2至3mm之间改变。然而,应理解这些尺寸是示例性的,并且取决于使用的材料和期望的结构和机械特征可要求其它的直径D。同样地,与水平线的角度φ可变化以改变通道212的数目或相邻通道212之间的距离。

图39A示出了根据本发明的再一实施例的用于脊柱固定的柔性杆230。此杆230包括多个在杆230的主体内形成的横向通道232。此通道232大致上与关于图37A和37B的上述通道212相似,然而,通道232不是以Z字样式连接在一起。相反地,每个通道232大致上平行于其紧邻的通道232,并且一个通道232的开口不与相邻通道232的开口一致。如图39A所示,与水平线的角度φ在此实施例中大约为90度。然而,应理解根据本发明其它角度φ可引入。还应理解的是通道232(以及通道212)的尺寸、大小和形状可改变,以实现期望的机械和结构特征。例如,通道212和232的横截面形状不必是圆形的。例如,相反地它们可以是椭圆形或菱形,或其它期望的形状。

例如如图22B所示,有时对于多步脊柱固定手术,可能期望一个脊柱关节刚性地固定,而相邻脊柱关节动态地(柔性地)稳定。为实现此功能的纵向件的实施例如图41A所示。纵向件250的轴向部254被开槽,以提供用于弯曲的增加的柔性,然而轴向部252、256没有开槽且保持相对的刚性。孔258用于中止槽,以防止产生破裂且改善可靠性。使用这些具有扩张的直径以中止材料内的槽或缝的孔在本领域广为人知,做为减小材料内峰值机械应力并且减小材料失效的可能性的方法。
图41B示出了图41A中的杆250的装配,其使用至少三个固定件2(图3)配置成固定到患者的脊柱内,且具有设置在第一对固定件2之间的柔性部254以及设置在第二对固定件2之间的非柔性部252。
如图41C中示出的另一实施例,延伸的未开槽部252可提供一系列的用于放置单个固定件2的位置。在另一实施例中,延伸的未开槽部可对称地设置在开槽部的任意一端。应意识到部件252的延伸长度提供了“一个大小对所有适合”的纵向件250,其可提供相邻椎骨的椎弓根之间的各种距离。如图41C所示,相邻固定件2和2’之间的距离可通过选择在部件252上的固定件2的位置而调整。于是部件252的任何多余长度可裁剪掉或去除。
例如槽深、槽宽、槽的横截面形状或轮廓、以及开槽部254的槽与槽的间距的槽参数,可一律为常量,以使沿开槽部254的轴的结构和机械特征一致。有时对于纵向件具有轴向变化的结构和机械特征是有利的,以控制局部的机械应力水平、常规的定制的弯曲形状,或影响沿所有弯曲方向或沿选定的弯曲方向的弯曲阻力。圆柱(例如)空心纵向件的横截面可通过改变外径而变化,同时对于空心圆柱体保持不变的壁厚。另一实施例是通过调整内径(即圆柱体内腔的直径)而修改壁厚,同时保持空心圆柱体的外径不变。另一实施例同时地改变内径和外径。容易看到上述论点还应用于具有不是圆柱形的形状的纵向件。
图42A示出了根据本发明的实施例的柔性的螺旋开槽的稳定装置270的侧视图。螺旋开槽部271具有相对于未开槽部262和262’扩大的外径。然而,由于存在槽,如果螺旋开槽部271的外径与未开槽部262和262’的外径一样,螺旋槽给予部件271增加的柔性,因为在部件271内材料横截面积减小,其还将给予部件271相对于未开槽部262和262’更大的单位面积材料应力。对于螺旋开槽部271和未开槽部262及262′,在弯曲螺旋开槽部271期间,部件271的外径的扩大可保持可接受的材料应力水平。
在一实施例中,如果图42A中的纵向件是空心的,螺纹开槽部271的腔的内径可与未开槽部262和262’的腔的内径一样,然而开槽柔性部271的外径被增加,以减小弯曲及/或改变开槽部271的柔性时的材料应力。
图42A和42B(下文中描述)示出了纵向脊柱稳定装置的例子,其中柔性部相比纵向稳定装置的相对应的端部,具有不同的横截面轮廓(如果是圆柱杆的话是外径)或周长形状。
在另一实施例中,开槽柔性部的横截面轮廓(例如外径)与未开槽部件的横截面轮廓(例如外径)保持一样,然而开槽柔性部的腔的内径相对于未开槽部件的腔的内径减小了。这具有如上所述的类似的材料应力减小效应。
在本发明的再一实施例中,开槽柔性部的内径和外径可关于未开槽部件的内径和外径变化,以减小两部件之间的材料应力差。
图42B示出了本发明的另一实施的侧视图,其通过调整开槽部266的圆柱直径或横截面轮廓(同时对于空心纵向件这种情况保持内腔直径不变),实现沿纵向轴柔性的改变,以在分别处于开槽部266和未开槽部262和262’之间的过渡部264和264’的附近实现机械应力的减小。开槽部266的外径在接近开槽部266的中心部分处最小,并且朝向未开槽部262逐渐扩大。这提供了更大的横截面材料面积以分布力,从而在接近过渡部264和264’的开槽部266的区域内减小单位面积的应力。
在另一实施例中,槽深、槽宽、槽横截面形状以及槽与槽的间距的单独的或与圆柱横截面的如上所述变化结合的轴向变化也可实现柔性和机械特征的轴向改变。例如:(i)从接近开槽部的中心的最大值逐渐减小槽深,直到在具有非开槽部的边界处接近零(图43A);(ii)从接近开槽部的中心的最大值逐渐减小槽宽,直到在具有非开槽部的边界处接近零(图43B);(iii)从接近开槽部的中心的最大允许弯曲逐渐过渡槽的形状,直到在具有非开槽部的边界处提供减小的弯曲的形状;或(iv)从接近开槽部的中心的最小值逐渐扩大槽与槽的间距,直到在具有非开槽部的边界处的最大值(图43D)。
图44示出了具有围绕开槽部276的弹性体覆层278的纵向件。在此实施例中,弹性体覆层278仅覆盖开槽部276,并且不覆盖未开槽部272。还有可选择的锥体274在纵向件内形成,以在覆层和未覆层部之间提供光滑的表面过渡。这些可选择的锥体274还固定覆层的纵向位置。替换地,覆层可延伸到未开槽部272上。弹性体覆层可(i)仅接触纵向件的表面,(ii)此外穿透进入纵向件的槽内,或(iii)如果纵向件是空心的,另外穿透且至少部分地填充纵向件的内部。此弹性体覆层提供对纵向件的轴向的和弯曲的稳定性的另外控制,以及提供组织和开槽部之间的隔离物。
此弹性体覆层可由医用弹性体的任何种类组成,例如包括硅树脂、聚亚安酯(polyurethane)、聚碳酸氨基甲酸酯(polycarbonateurethane)和硅树脂-氨基甲酸酯共聚物(silicone-urethane copolymer)。使用本领域已知的各种技术,此覆层可应用于纵向件。在一技术中,热塑性的或热固的树脂可喷射到围绕纵向件的期望部分的加热的模具中,同时其粘附在模具内。此喷射成型法的优点是其可提供在室温和压力下通过替换方法用于应用的没有足够的低粘性的覆层材料。喷射成型法的另外优点在于覆层的外部形状由使用的模具的形状确定。喷射成型法的另一优点是可重复穿透槽空隙和空心纵向件的内部。替换的铸造技术包括压缩成型和传递成型。
其它的覆层应用方法包括流体喷射成型、浸入、喷镀或用诸如漆刷的机械涂敷器涂抹。这些方法需要覆层材料可在低粘性下应用。例如用于应用的树脂应悬浮于应用后挥发的溶剂中。在另一例子中,覆层材料在低粘性下应用,并且随后通过化学、热和辐射方法固化。这有时很有用,以遮盖纵向件的不期望应用覆层材料的部分。
图45A示出了由材料277制成的纵向件的柔性部的一致的横截面。图44B示出了作为由材料277制成的纵向件的柔性部的杆的非一致的横截面,其包括由另外的材料279制成的部分。明显地,图45A中施加有沿x和y方向的力的杆将展现相同的弯曲特征。如果部件320和330的材料具有不同的弯曲特征,图45B中的施加有沿x和y方向的力的杆将展现出不同的弯曲特征。例如,如果图45B中的材料279比材料277硬,杆沿x方向将比沿y方向更容易弯曲。
图46A示出了混合金属纵向件的另一实施例,其具有围绕纵向件的缆线部280的弹性体覆层278。在此实施例中,弹性体覆层278围绕两未覆层端部262之间的编织缆线280。此缆线可以是单独的缆线或未编织的多股缆线(未示出),并且可与端部262共轴,或如图46B所示的关于端部262的纵向轴离开中心地定位。缆线部280可是直的如图46A所示,或是弯曲的,例如图46C所示的缆线281。端部262之间的直缆线280比弯曲的缆线28 1提供更大的拉紧阻力,其伸直如同在拉紧状态下纵向件的伸长。在一实施例中,端部262和缆线280可由关于杆4和缆线32的上述任何期望的且适当的生物相容性的金属或合成金属混合材料制成。在另外实施例中,覆层278可由上述的适当的生物相容性的合成或非金属材料中的任何一种或组合而制成。
图46A-C中的混合金属纵向件的硬度可通过改变如上所述的弹性体覆层278内的缆线结构,或通过改变缆线280及/或覆层278的物理几何而修改。本领域技术人员应认识到例如通过改变缆线部280和覆层278的长度及/或直径,直径比或缆线的数量和位置,而可改变硬度。
图47A示出了柔性连接单元的另一实施例,其在两刚性端部9’之间具有一个或多个隔离物37,且具有覆盖一个或多个隔离物37的弹性体覆层278。连接单元还包括缆线32,其穿过每个隔离物37内的纵向轴向通道或孔。在一实施例中,如图47A所示,隔离物37大致上占据端部9’之间所有的间隔,使得多个隔离物37保持在端部9’之间沿纵向轴方向大致固定的位置处。换句话说,隔离物37不关于缆线32沿纵向方向大体上移动或滑动,因为端部9’之间没有间隔这样做。每个隔离物37邻近相邻的隔离物37及/或端部9’,使得其没有空间关于位于端部9’之间的隔离物37的纵向轴向通道内的缆线32或其它柔性件滑动。覆层278围绕每个隔离物37或围绕整个隔离物37组而形成。应意识到隔离物37和覆层278的结合形成复合或混合隔离物,其中隔离物37提供混合隔离物的第一材料,并且覆层提供混合隔离物的第二材料。在一实施例中,隔离物37可由如上所述的生物相容性的金属或合成金属混合材料制成,并且覆层278可由上述的恰当的生物相容性的合成或非金属材料中的任何一种或其组合制成。
在另一实施例中,如图47B所示,隔离物37可沿缆线32定位,使得在相邻隔离物37和端部9’之间有空间。覆层278围绕隔离物37和缆线32形成,使得相邻隔离物37和端部9’之间的大致上所有的间隔由覆层278占据。因而,覆层278限制包在其内的隔离物37和缆线32的运动,并且给端部9’之间的柔性部提供额外的刚性。覆层278、隔离物37以及缆线32可由任何适合的材料制成,例如包括关于图9、10和47A的上述材料。
图47A和B中的覆层278被显示以封装所有的端部9’之间的每个混合金属隔离物37。本领域技术人员将了解到连接单元36的硬度可通过仅覆盖隔离物37的一部分而改变,例如隔离物37的金属部和缆线32之间的间隔,或隔离物37之间的间隔以及隔离物37和端部9’之间的间隔。
在上述各种实施例中的柔性连接单元的硬度可通过选择各种生物相容性的材料而改变。例如,隔离物37可由生物相容性的金属(例如不锈钢、钛、钛合金、钽、锆、钴铬,以及这些材料的合金)制成。隔离物37还可由包括已知的刚性聚合体(例如UHMWPE、PEEK和聚亚安酯)或陶瓷(例如氧化铝或氧化锆)的材料制成。
169图48示出了柔性连接单元36的另一实施例,其具有在刚性端部9’之间的金属隔离物37,且弹性体覆层278围绕刚性隔离物37的至少一部分。连接单元36还包括穿过隔离物37和端部9’轴向定位的柔性缆线32,其中隔离物37、端部9’和缆线32都由弹性体覆层278物理分开。在此实施例中,柔性连接单元的所有元件可在机械负载下相对于任何其它元件移动,其仅由弹性体覆层278的弯曲、伸展和压缩特征限制。因而,元件的大小和形状可选择以经受人的脊柱上的负载,并且允许连接单元连接于其的椎骨的正常运动。金属隔离物37和覆层278一起形成合成金属混合或复合隔离物,其中弹性体278将金属隔离物37与相应的刚性端部9’和金属缆线32分开,使得它们不彼此摩擦,从而最小化磨损碎片的产生。此实施例的另一优点是连接单元沿所有方向或自由度都是柔性的,从而允许脊柱的弯曲、伸长、横向弯曲和轴向旋转,而没有沿任何方向的固定的或刚性的机械限制。图48中的弹性体覆层278与柔性缆线32共心。在另一实施例(未示出)中,缆线32可离开中心地位于隔离物37的轴向腔内,或多股缆线32可贯穿隔离物37的轴向腔而分布。
图48中所示的缆线32在两端从端部9’被物理隔开。这可通过单独地覆盖缆线32以产生混合金属缆线,并且先于覆盖隔离物37用端部9’装配而完成。替换地,混合金属缆线32可在装配前,通过挤压围绕缆线32的弹性体或将其滑动进入预先形成的挤压弹性体内而被覆盖。后一种制造方法具有允许缆线32在弹性体覆层278内沿其轴滑动的优点,从而相对于连接单元36的弯曲和剪切硬度,减小其轴向硬度。本领域技术人员将认识到如果柔性缆线32如在此描述的可自由滑动,那么连接单元36的弯曲和剪切硬度可通过改变缆线32的直径而改变,且最小化连接单元36的轴向硬度的改变。如本领域所已知的,“弯曲硬度”涉及物体可弯曲的量,并且“剪切硬度”涉及物体经受横向剪切力的量。“横向硬度”涉及物体可伸长或压缩的量。
图49示出了除了缆线32的每个端部与端部9’接触之外,其余与图48中的一样的柔性连接单元。在替换实施例(未示出)中,缆线32的一端可与一端部9’接触,同时缆线32的相对端通过如上所述的覆盖与另一端部9’分开。缆线32和端部9’之间的接触可以是滑动接触或诸如压配合装配、焊接装配或铜焊装配的固定接触。如果缆线32的两端与端部9’固定接触,例如刚性连接,柔性连接单元的轴向硬度可增加。仅缆线32的一端固定接触将对连接单元的轴向硬度几乎没有影响。
图48和49示出了混合金属隔离物,其包括环向围绕弹性体覆层278的金属隔离物37。因此隔离物37被首先覆盖在其内表面上。通过将隔离物37的金属部相对于模具中的端部9’保持到位,同时应用覆层278,这些实施例可容易地制造。此实施例的另外的优点是当连接单元36机械负载时,环向设置的隔离物37限制了覆层278的膨胀和弯曲。此限制效果导致连接单元36的硬度变化,特别是轴向压缩、弯曲和剪切。本领域技术人员将认识到连接单元36的硬度可通过改变隔离物37的内径、长度和数量而改变。
各种实施例中的弹性体覆层278可由各种方法形成,包括先前描述的各种模制技术、挤压、浸入和涂敷。在替换实施例中,弹性体覆层278使用喷射成型工艺和诸如聚碳酸氨基甲酸脂(PCU)的生物相容性的热塑性弹性体而被模制到位。PCU具有如下优点,有利于生物相容性、抵抗退化和破裂、有利于疲劳特性以及好的粘附于金属基底特性,另外其与喷射成型工艺兼容。然而,应理解覆层可由例如上述的其它合适的非金属材料制成。在另一实施例中,隔离物37和端部9’的表面准备有一种或多种特征或表面处理,以改善弹性体覆层278的固定的耐用性。
图50A-D示出了用于改善弹性体覆层278固定于任何刚性件28 1的表面的各种特征。图50A示出了刚性件281内的底切腔,其中腔282的主体比颈要大,从而将弹性体覆层278俘获在腔282内。腔还包括在腔的壁内的更小的底切槽283,用于交错连接弹性体覆层278。底切槽283和底切腔282也可独立使用。图50B示出了刚性件281上的围绕其弹性体覆层278被模制的外倒钩284。图50C示出了穿过刚性件281的壁的穿过其弹性体覆层278被模制的孔285。在一实施例中,弹性体覆层278覆盖围绕孔285的壁的内外表面。图50D示出了刚性件281在与弹性体覆层278界面具有的粗糙表面281’。粗糙表面可由各种方法形成,例如包括喷砂处理、喷丸处理、等离子体喷射、化学蚀刻以及各种加工技术。图50A-D示出的任何特征可彼此结合使用,或与诸如刚性件281的表面的清洗、钝化或化学涂漆的表面处理结合。
图51示出了隔离物37和端部9’的另一实施例,其中隔离物37和端部9’由弹性体覆层278物理隔开,且被配置使得当连接单元36机械负载时其可加固弹性体覆层278。隔离物37和端部9’包括重叠部,其物理限制端部9’相对于彼此的剪切位移,而不需要限制端部9’相对于彼此的轴向位移。图51是示例性的任何数量的隔离物37和端部9’的形状的组合,其可用于沿一个或多个方向改变连接单元36的硬度。本领域技术人员将认识到用重叠特征,或简单地通过增加或减小刚性隔离物37和端部9’之间的间隔,或通过增加附加隔离物(未示出)且改变相邻隔离物之间的间隔,可实现上述目的。
图52示出了连接单元284的另一实施例,其包括两刚性端部285和286,以及中间部,其中柔性件287连接端部285和286且穿过混合金属隔离物288内的轴向孔。在一实施例中,混合金属隔离物288由至少一种金属和一种弹性体材料形成,使得隔离物289的金属部配置成由诸如椎弓根螺钉或薄片状挂钩的固定件接受且保持,并且隔离物288的弹性体部290设置在金属部289的相对侧且邻近相应的端部285和286。
参考图52,当端部285和金属隔离物289由相应的例如固定件2(图2)保持,且附着于邻近的椎骨时,连接单元284提供稳定性,同时允许椎骨沿六个自由度(例如x轴、y轴、z轴、倾斜、滚转和偏转)运动。尽管端部285和286大体上限制混合金属隔离物288沿纵向轴向方向的运动,金属隔离物289两侧的弹性体部290的可压缩性和弹性允许金属隔离物289相对于端部285和286及/或柔性连接件287沿六个自由度的每一个稳定运动,同时还沿六个自由度的每一个提供运动的阻力和稳定性。因而,在一实施例中,连接单元提供更大范围的动态稳定运动。此外,在一实施例中,弹性体覆层290包括高摩擦力的材料,其抵抗混合金属隔离物288在柔性中间部287上滑动,从而进一步提供金属隔离物289沿纵向轴向方向运动的阻力。端部285和286使用任何上述技术或已知方法而与混合金属隔离物288的相应的端部连接。端部285配置成具有由椎弓根螺钉或其它类型的固定件接受且保持的足够的长度。当混合隔离物288的金属部289配合且固定到例如固定件2(图3)时,端部286伸出超过固定件2(在相对于两固定件2之间的隔离物的这侧)。因而,端部286配置成沿连接单元284的轴较短,以最小化伸出超过固定件2的连接单元284的长度。
在另一实施例(未示出)中,柔性件287可偏离连接单元284的中心纵向轴设置。取决于连接单元284的弯曲方向,此偏心结构提供不同水平的硬度。如果在脊柱伸展期间连接单元284被弯曲(例如当患者向后弯曲)时,期望提供更高水平的硬度,以及在脊柱弯曲期间连接单元284被弯曲(例如当患者向前弯曲)时期望更低水平的硬度时,这是有利的。另外地或替换地,通过在连接单元284的一侧上使用与连接单元284的其余侧不同的覆层290数量和厚度,可实现弯曲轮廓的不同水平的方向硬度。此外,在隔离物289的任一侧上可应用不同数量及/或类型的覆层材料290。因而,连接单元284可沿隔离物289的不同运动方向提供不同水平的硬度,且因此改变可提供给椎骨的不同方向运动的稳定性水平,该椎骨通过固定件2固定到隔离物289。在这些实施例中,其中连接单元284的硬度水平取决于弯曲方向,从而可将适当的标记(例如激光雕刻、物理特征等)设置在连接单元284上,以在将连接单元284固定到患者的脊柱之前指明连接单元284的恰当的方向。
图53示出了两连接单元284已经植入且固定到脊柱的相应的椎骨之后的位置。对于每个连接单元284,混合金属隔离物288固定到下椎骨291,并且端部285固定到上椎骨292。如图53所示,连接单元284提供考虑到脊柱的自然和解剖运动的容许柔性。因为椎间盘293和小平面关节(facet joint)294与上椎骨292的椎弓根相比更靠近下椎骨291的椎弓根,因此当由混合金属隔离物288提供的连接单元284的柔性部固定到下椎骨291时,其设置成偏离中心或靠近脊柱内自然关节的水平面,即椎间盘293和小平面关节294的水平面。在自然关节的水平面处的柔性便于自然地且解剖学上地纠正脊柱的运动。
当然,如果在另外的区域期望柔性,这可通过在连接单元294的相对端复制混合金属隔离物288和连接件287而实现,如图54所示。连接单元295配置成由连接于两隔离物288的相应固定件保持。连接单元295内的隔离物288可能比连接单元284内的隔离物288要长,使得可以提供椎骨之间的间距的可变性。在另外的实施例中,如上所述的连接单元可延伸以稳定在三个或更多的相邻椎骨之间的两个或多个关节或脊柱运动段,并且可通过三个或更多的固定件(例如椎弓根螺钉)附着于相应的椎骨。因而,在一实施例中,连接单元包括多个用来给多个关节或脊柱运动段提供柔韧的稳定性的混合金属隔离物288。此外,混合金属隔离物288可用刚性端部285根据外科医生需求以任何次序或结合来替换。这样,可设计混合的多层或多脊柱段连接单元,其中连接单元的每个段可提供适合于相应的每对将稳定的上下椎骨的期望的柔性水平。例如,稳定第一对椎骨的连接单元的第一部可非常刚硬,同时稳定第二对椎骨的连接单元的第二部相对于第一部可更加柔韧。根据本发明,大量期望的部件结合可实现,以产生混合的多层或多段连接单元。
在各种实施例中,如图52、54和55所示的柔性件287可为刚性材料的实心件,例如生物相容性的金属,优选地是如端部285和与端部285整体形成的286同样的材料,并且永久地固定到端部286。替换地,连接件287可以是缆线、多股缆线、编织的缆绳或用于连接端部285和286的其它结构。连接件的结构、长度和直径将影响连接单元284的柔性,这对于本领域技术人员是清楚的。类似地,混合金属隔离物288可由生物相容性的金属,优选地与端部285和286同样的材料,以及生物相容性的诸如硅树脂或聚亚安酯以及优选的聚碳酸氨基甲酸酯的弹性体制成。混合金属隔离物288显示为与刚性端部285和286大致上同样的外径。替换地,隔离物290的弹性体部可具有更大或更小的直径,或可改变直径。对于本领域技术人员清楚的是连接单元284的柔性可通过选择覆层材料或改变其尺寸而改变。
混合金属隔离物288的非金属部或弹性体部290可通过包括图50A-50D所示的那些方法的各种方法而连接于相应的端部285和286、金属隔离物289及/或柔性件287的表面。如图52-55所示,弹性体覆层290将金属隔离物289保持在关于端部285和286大致上固定的位置,同时当外力使得覆层沿任何方向弯曲或压缩时,其允许隔离物289一定的相对运动。因而,在一实施例中,当金属部289通过固定件2固定到椎骨时,连接单元284的柔性大致上由混合隔离物288的弹性体部290的可压缩性限制,其可通过隔离物288的金属部289的运动沿各种方向压缩。
图55示出了连接单元296的实施例,其具有由多于两种不同材料组成的混合金属隔离物297。隔离物297具有如图52所示的弹性体部290和金属部289,以及另外的显示在弹性体部290的外部的生物吸收部298。混合金属隔离物的生物吸收部298配置成大致上从隔离物289的金属部的每端延伸至相应的刚性端部285和286的最近端,以限制金属部297的运动,直到生物吸收部298在体内变软或降解。隔离物的生物吸收部298可由至少一种从已知的生物吸收材料组中选择的材料组成,其包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合。其它已知的甚至那些未来将发现的生物吸收材料可根据本发明使用。
在一实施例中,连接单元296可在脊柱融合手术后使用。在许多情况下,在融合操作手术后紧接的时期内,期望用植入装置刚性地固定脊柱,以允许治愈外科上放置的移植骨并且有效地将相邻椎骨融合在一起。成功地完成融合之后,期望移去植入装置以允许移植骨独立地稳定脊柱。这在移植位置引起负载,并且健康地改建用于可靠的长期固定的移植骨。然而,非常不期望实施第二次手术以去除植入装置。图55中的连接单元296在脊柱融合之后最初提供更刚硬的稳定性,然后经过自然过程连接单元296的生物吸收部298降解并且由身体吸收,从而减小连接单元296的硬度,并且允许移植骨分担更大百分比的负载以长期稳定脊柱。柔性连接单元296由此允许外科医生仅用一个外科程序可将柔性稳定性的水平从第一更刚硬的状态过渡到第二较少刚硬的状态。不必说,外科手术的消除从健康观点和经济角度对于患者都是巨大的优势。
柔性连接单元296可有利地用于任何期望提供变化的稳定性水平的情形。另外,引入到连接单元298内的生物吸收材料的相关量和类型可改变,以更改连接单元296的最初硬度,以及完全吸收所有的生物吸收部298所需要的时间。在一实施例中,可使用具有不同硬度特征及/或吸收时间的两种或多种不同类型的生物吸收材料,以提供从多级硬度的过渡。在另一实施例中,配置成稳定多个脊柱段的连接单元,可在连接单元的一个或多个柔性部内引入生物吸收材料,以通过多脊柱段连接单元的各种柔性部提供柔性状态的改变。此外,生物吸收材料298可应用来将连接单元的柔性部(例如混合金属隔离物部)完全封装,或简单地覆盖连接单元的选择部,或填充连接单元的间隙、间隔及/或通道。换句话说,一种或多种生物吸收材料298的应用可通过各种方法执行,以对于连接单元的一个或多个柔性部实现期望的最初和最终硬度特征。此外,不需要结合生物吸收覆层298和非生物吸收覆层290。因而,在一实施例中,图55所示的连接单元296的弹性体覆层290可完全地省略,或由生物吸收覆层298或其它具有不同硬度及/或降解/吸收特征的生物吸收覆层(未示出)替代。

Claims (61)
1.一种用于脊柱固定装置的柔性连接单元,包括:
具有第一和第二端的纵向件;
设置在所述第一和第二端之间的至少一个隔离物,其中所述至少一个隔离物包括由第一材料制成的第一部,以及由第二材料制成的第二部;
以及位于所述至少一个隔离物的纵向轴向通道内的至少一个柔性件,其中所述至少一个柔性件包括生物相容性的金属材料,并且所述第一和第二端大体上限制所述至少一个隔离物沿纵向轴向方向的运动。
2.如权利要求1所述的柔性连接单元,其中所述第一材料包括从包括金属、金属合金、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的至少一种材料。
3.如权利要求1所述的柔性连接单元,其中所述第二材料包括从包括金属、金属合金、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的至少一种材料。
4.如权利要求1所述的柔性连接单元,其中所述第一部包括金属隔离物,并且所述第二部包括围绕所述金属隔离物的非金属覆层。
5.如权利要求4所述的柔性连接单元,其中所述非金属覆层包括弹性体。
6.如权利要求1所述的柔性连接单元,其中所述第一部包括金属材料,并且所述第二部包括非金属材料,其中所述金属材料围绕所述非金属材料的至少一部分。
7.如权利要求6所述的柔性连接单元,其中所述非金属材料包括弹性体。
8.如权利要求1所述的柔性连接单元,其中所述第一部包括金属隔离物,并且所述第二部包括围绕所述金属隔离物和所述第一端之间的所述至少一个柔性件的第一部的第一非金属覆层,以及围绕所述金属隔离物和所述第二端之间的所述至少一个柔性件的第二部的第二非金属覆层。
9.如权利要求8所述的柔性连接单元,其中所述金属隔离物配置成由第一固定件固定到患者的脊柱,并且所述第二端配置成由第二固定件固定到患者的脊柱。
10.如权利要求9所述的柔性连接单元,其中当所述第一固定件固定到下椎骨并且所述第二固定件固定到上椎骨时,所述金属隔离物的位置大体上相应于患者脊柱的相邻上下椎骨之间的关节的位置。
11.如权利要求1所述的柔性连接单元,其中:
所述第一材料包括从包括金属、金属合金、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的至少一种材料;
所述第二材料包括从包括弹性体、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的至少一种材料;并且
所述至少一个隔离物还包括由包括生物吸收材料的第三材料制成的第三部。
12.如权利要求11所述的连接单元,其中所述生物吸收材料包括从包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合的一组材料中选择的至少一种材料。
13.如权利要求1所述的柔性连接单元,其中:
所述第一材料包括从包括金属、金属合金、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的至少一种材料;并且
所述第二材料包括生物吸收材料。
14.如权利要求13所述的连接单元,其中所述生物吸收材料包括从包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合的一组材料中选择的至少一种材料。
15.如权利要求1所述的柔性连接单元,其中所述至少一个柔性件包括单独的轴。
16.如权利要求1所述的柔性连接单元,其中所述至少一个柔性件包括具有多根纱线的至少一根缆线。
17.如权利要求16所述的柔性连接单元,其中所述至少一根缆线包括具有多根交织缆线的编织缆线。
18.如权利要求1所述的柔性连接单元,其中所述至少一个柔性件与所述第一和第二端整体形成。
19.如权利要求1所述的柔性连接单元,其中所述至少一个柔性件的至少一端不是直接连接到所述第一和第二端中相应的一个。
20.一种用于脊柱稳定的柔性连接单元,包括:
具有第一和第二端的纵向件;
位于所述第一和第二端之间的隔离物;
位于所述隔离物的纵向轴向通道内的柔性件;以及
配置成限制所述隔离物的运动的柔性覆层。
21.如权利要求20所述的柔性连接单元,其中所述柔性覆层封装所述第一和第二端之间的所述隔离物和柔性件。
22.如权利要求20所述的柔性连接单元,其中所述柔性覆层填充所述隔离物与所述柔性件及第一和第二端之间的一个或多个间隔,以限制所述隔离物相对于所述柔性件及第一和第二端的运动。
23.如权利要求20所述的柔性连接单元,其中:
所述隔离物包括从包括金属、金属合金、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的材料;并且
所述柔性覆层包括从包括弹性体、聚合体、树脂、塑料及其复合材料的一组生物相容性的材料中选择的材料。
24.如权利要求20所述的柔性连接单元,其中所述柔性覆层包括围绕所述隔离物和第一端之间的柔性件的第一部的第一段,以及围绕所述一个隔离物和第二端之间的柔性件的第二部的第二段。
25.如权利要求24所述的柔性连接单元,其中所述隔离物配置成由第一固定件固定到患者的脊柱,并且所述第二端配置成由第二固定件固定到患者的脊柱。
26.如权利要求25所述的柔性连接单元,其中当所述第一固定件固定到下椎骨并且所述第二固定件固定到上椎骨时,所述隔离物的位置大体上相应于患者脊柱的相邻上下椎骨之间的关节的位置。
27.如权利要求20所述的柔性连接单元还包括配置以给连接单元在最初植入到患者体内时提供增加的硬度的生物吸收覆层,其中由于所述生物吸收覆层在体内降解,所述连接单元硬度降低。
28.如权利要求27所述的连接单元,其中所述生物吸收材料包括从包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合的一组材料中选择的至少一种材料。
29.如权利要求20所述的柔性连接单元,其中所述至少一个柔性件包括单独的轴。
30.如权利要求20所述的柔性连接单元,其中所述至少一个柔性件包括具有多根纱线的至少一根缆线。
31.如权利要求20所述的柔性连接单元,其中所述至少一根缆线包括具有多根缆线的编织缆线。
32.如权利要求20所述的柔性连接单元,其中所述至少一个柔性件与所述第一和第二端整体形成。
33.如权利要求20所述的柔性连接单元,其中所述至少一个柔性件的至少一端不是直接连接到所述第一和第二端中相应的一个。
34.如权利要求20所述的柔性连接单元,其中所述连接单元的至少一侧上的覆层的厚度被改变,以在所述连接单元沿第一方向弯曲时提供第一水平的硬度,并且在所述连接单元沿第二方向弯曲时提供与所述第一水平硬度不同的第二水平的硬度。
35.如权利要求20所述的柔性连接单元,其中所述柔性件偏离所述纵向轴设置,以在所述连接单元沿第一方向弯曲时提供第一水平的硬度,并且在所述连接单元沿第二方向弯曲时提供与所述第一水平硬度不同的第二水平的硬度。
36.如权利要求20所述的柔性连接单元,还包括至少一个捕获和保持件,用于维持所述柔性覆层与所述第一和第二端的相应表面之间的物理连接。
37.如权利要求36所述的柔性连接单元,其中所述至少一个捕获和保持件包括由所述柔性覆层填充的底切区域。
38.如权利要求36所述的柔性连接单元,其中所述至少一个捕获和保持件包括由所述柔性覆层填充的通孔。
39.如权利要求36所述的柔性连接单元,其中所述至少一个捕获和保持件包括粗糙表面。
40.如权利要求36所述的柔性连接单元,其中所述至少一个捕获和保持件包括中间表面处理,以改善所述柔性覆层和相应的表面之间的接合。
41.一种脊柱固定装置,包括:
配置以固定到包括上椎骨和下椎骨的两相应并相邻的椎骨的第一和第二固定件,每个固定件包括接头组件;
纵向件,其具有配置以由所述第一固定件的接头组件容纳且固定的第一部,以及配置以由所述第二固定件的接头组件容纳且固定的第二部,其中所述纵向件还包括相对于所述第一和第二部偏离中心设置的柔性部,以便于当所述第一和第二固定件分别固定到所述上下椎骨中的相应的一个时,在或靠近所述上下椎骨之间的椎间盘处提供柔性。
42.如权利要求41所述的脊柱固定装置,其中所述柔性部包括金属混合隔离物,所述金属混合隔离物包括金属部和非金属部。
43.如权利要求41所述的脊柱固定装置,其中所述金属部包括金属隔离物,并且所述非金属部包括弹性体覆层。
44.如权利要求43所述的脊柱固定装置,其中第二部包括所述金属隔离物,并且所述金属隔离物配置成由所述第一和第二固定件的接头组件中的相应的一个容纳且固定。
45.一种用于脊柱固定装置的柔性连接单元,包括:
配置成使用至少三个固定件固定到患者的脊柱的纵向件;
其中所述纵向件的第一部配置成位于第一对固定件之间,并且所述纵向件的第二部配置成位于第二对固定件之间;并且
其中至少所述纵向件的第一部包括插入到两端部之间的至少一个隔离物,以及位于所述至少一个隔离物的纵向轴向通道内的至少一个柔性件,其中所述至少一个隔离物大体上相对于至少一个柔性件固定到位。
46.如权利要求45所述的柔性连接单元,其中所述第一部还包括非金属覆层,其相对于所述至少一个柔性件和所述端部限制所述至少一个隔离物的运动。
47.如权利要求46所述的柔性连接单元,其中所述第一部还包括配置以在最初植入到患者体内时给所述第一部提供增加的硬度的生物吸收覆层,其中由于所述生物吸收覆层在体内降解,所述第一部硬度降低。
48.如权利要求47所述的柔性连接单元,其中所述生物吸收材料包括从包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合的一组材料中选择的至少一种材料。
49.如权利要求45所述的柔性连接单元,其中所述第一部还包括配置以在最初植入到患者体内时给所述第一部提供增加的硬度的生物吸收覆层,其中由于所述生物吸收覆层在体内降解,所述第一部硬度降低。
50.如权利要求49所述的柔性连接单元,其中所述生物吸收材料包括从包括聚乳酸、聚羟基乙酸、聚乳酸羟基乙酸(polyglactic acid)、聚二氧环己酮、聚葡糖酸盐(酯)、硫酸钙、磷酸钙以及其组合的一组材料中选择的至少一种材料。
51.一种用于脊柱稳定的柔性连接单元,包括:
具有第一和第二端的纵向件;
位于所述第一和第二端之间且在其内具有纵向轴向通道的隔离物;
位于所述第一和第二端之间的隔离物的纵向轴向通道内的柔性件;
围绕设置在所述隔离物和第一端之间的柔性件的第一部的第一覆层;以及
围绕设置在所述隔离物和第二端之间的柔性件的第二部的第二覆层。
52.如权利要求51所述的柔性连接单元,其中所述第一端配置成与第一固定件配合,并且所述隔离物配置成与第二固定件配合。
53.如权利要求52所述的柔性连接单元,其中所述第一固定件配置成固定到上椎骨的椎弓根骨,并且所述第二固定件配置成固定到下椎骨的椎弓根骨,使得完全地通过所述隔离物相对于所述第一和第二端以及所述柔性件的运动,而不通过所述隔离物在所述第二固定件的接头组件内的运动,给下椎骨的椎弓根骨提供稳定的活动性。
54.如权利要求51所述的柔性连接单元,其中所述隔离物能够沿六个自由度运动,同时经历沿所述六个自由度中的每一个的运动阻力。
55.如权利要求51所述的柔性连接单元,其中所述第一和第二覆层中的至少一个的至少一侧上的覆层的厚度被改变,以在所述连接单元沿第一方向弯曲时提供第一水平的硬度,并且在所述连接单元沿第二方向弯曲时提供与所述第一水平硬度不同的第二水平的硬度。
56.如权利要求51所述的柔性连接单元,其中所述第一覆层的厚度与所述第二覆层的厚度不同。
57.如权利要求51所述的柔性连接单元,其中所述第一覆层包括与所述第二覆层不同的材料。
58.如权利要求51所述的柔性连接单元,其中所述第一和第二覆层通过位于所述隔离物内表面和所述柔性件之间的覆层材料的中间部而整体形成且连接。
59.如权利要求51所述的柔性连接单元,其中所述柔性件偏离所述纵向轴设置,以在所述连接单元沿第一方向弯曲时提供第一水平的硬度,并且在所述连接单元沿第二方向弯曲时提供与所述第一水平硬度不同的第二水平的硬度。
60.如权利要求51所述的柔性连接单元,还包括围绕至少所述第一覆层和第二覆层的生物吸收材料。
61.一种柔性连接单元配置成固定到上椎骨和下椎骨的相应的椎弓根骨,其中所述柔性连接单元完全通过位于或靠近所述下椎骨的椎弓根骨的所述连接单元的柔性部的柔性,给所述下椎骨的椎弓根骨提供稳定的活动性。
