Abstract

发明背景
神经再生是一种复杂的生物学过程。中枢和外周神经系统的神经损伤后的变性过程在某些方面是相类似的,但在另外一些方面却不同。最大的区别之一在于:在神经损伤后,外周神经具有明显更强的再生轴突能力(Fenrich,K.等.2004,Can J Neurol Sci.31(2):142)。
Schmidt和Leach最近综述了治疗神经损伤的许多方法(2003,Annu.Rev.Biomed.Eng.5:293)。目前治疗损伤导致神经缺陷的方法通常依靠来自病人的供体组织。这带来了下列问题:供体位置的功能损失、潜在疼痛神经瘤的形成、供体和受体神经结构的不同以及大范围修复时移植物短缺。为了克服这些问题,已经开发出了合成神经导管(NGCs),其通过将切断神经残端锁定在该管的两端来对神经间隙进行桥连(US5,0190,87)。许多装置,例如,Integra Neurosciences I型拼接(collage)管和SaluMedica的SaluBridgeTM神经封套已获美国食品与药物管理局批准。然而,这些装置是用于治疗相对较短的神经缺损,并且在大多数情况下,合成管并不具有神经自体移植物的功能(Schmidt & Leach2003,Annu.Rev.Biomed.Eng.5:293)。

NGF是第一个并且是研究最为透彻的神经-来源的因子,其作用的神经元群的种类相对有限,包括外周神经系统感觉神经元的交感亚群,和脑内的纹状体(striatial)及胆碱能神经元(Terenghi,G.1999,J.Anat194:1-14)。在正常情况下,NGF呈现出非常低的浓度,但是在实验性神经损伤动物模型中快速升高。NCF主要是由在损伤神经远残端的目标组织和施沃恩细胞来产生的,然后在作用神经元受体产生神经营养功效前以逆向方式运输入细胞体内。外周神经病中,在病态神经内的此类运输会受影响、减少或被完全阻断。尽管在早期NGF的递送能促进管内的神经再生,但是这种促进不会持续到1个月后,这可能是由于管内NGF浓度的快速降低,造成这种降低的原因在于NGF在37℃含水介质中降解、渗漏出管和/或由于液体进入而被稀释。此外,将神经营养因子导入NGCs的时间对于愈合或再生过程有着重要的影响:引入各种试剂太早或太晚都会抑制再生过程(US5584885)。
感觉神经元的细胞体位于背根神经节,小结节(nodule)位于每个脊神经的背根远端。背神经根和腹神经根位于硬膜鞘内并被脑脊液(CSF)包围,其穿过椎间孔,在其中背根形成了背根神经节,随后与腹根合在一起形成背神经根。从形态上看,DRG的体感神经元具有一个单极结构,并通过一个脊髓中的长的向上轴突而连接到中枢神经系统(CNS)上,并通过第二轴突分支下行通过脊神经根而连接到外周神经系统(PNS)上,并且进一步突出进入到外周神经中。从功能上说,DRG神经元负责从触觉、温度、痛感到本体感受的不同感觉通道的受体-传导刺激的信号传导。
发明简述

详细描述
本发明者令人惊奇的发现:将核酸载体施用入脊髓周围的脑脊液中是将外生核酸递送到背根神经节神经质中的有效方法。更具体地说,编码NGF的核酸载体被鞘内注射到脑脊液中后显示了对于神经导管内断裂坐骨神经再生的积极作用。
本申请所用术语“神经元细胞”和“神经元”采用本领域的通常含义可以交互使用,是指神经系统的任何传导细胞,其通常包括细胞体或细胞质,一些树突和一个轴突。该术语既包括单个细胞也包括多个细胞群,另有清晰说明的除外。
将重组载体鞘内施用入接近背根神经节的脑脊液使得背根神经节神经细胞可以一定方式进行转染,该方式不依赖于轴突传递并且以相对最小侵入技术来完成。在外周神经损伤之后,包围着损伤点的细胞和组织,包括施沃恩细胞,通常分泌因子,该因子被损伤或损坏的神经元轴突吸收。这些因子通常通过轴突传递被递送到损伤神经的细胞质中,其中它们与受体相互作用并且发挥生物学作用。许多病毒类基因治疗系统,包括利用疱疹和脊髓灰质炎病毒的治疗,都使用了这种轴突传递以将治疗用基因递送到其它方法不可到达的神经元中,包括中枢神经系统中的神经元。正如本领域技术人员所理解的,这些方法在某些情况下提供的不是最优基因递送,在这些情况中,轴突递送是被损害的,例如,在特定的神经病中,如糖尿病类神经病和外伤、压力或横切所引起的外周神经损伤中。
在外周轴突传递被损害或消失的情况下,可以通过使用对目标DRG神经元的鞘内注射,将置于适当启动子调控下编码外源基因的核酸载体递送到DRG的细胞。因此,可以通过将含核酸载体施用到脑脊液中,从而将所述核酸递送至外周神经系统的神经细胞体中。可以理解的,脑脊液中的注射位置取决于特定的目标神经元细胞,该细胞是核酸递送所需进入的背根神经节的神经细胞。更特别的,所挑选的施用位置足够接近目标DRG神经元细胞从而将核酸递送到目标细胞的细胞体中,即,递送可以被调节或被影响而与轴突传递无关。因此,核酸被递送到目标细胞可以不受轴突传递的影响。从外周神经系统的节段结构(segmental architecture),本领域技术人员可以理解到施用的位置取决于目的在于转染的DRG神经细胞。例如,为了将转基因基因产品递送到扩展至受试者食指的外周神经中,优选将载体鞘内施用入到环绕或靠近受试者的脊椎的CSF中其。可以参考普通解剖学文章来确定适宜的注射位置,例如,Introduction to HumanAnatomy 6th edition,Francis,CV.Mosby Company,1973。
在一个具体实施方案中,通过腰椎注射来施用载体。腰椎注射被认为是安全的,并且对受损的脊髓和神经根基本不产生影响,这是由于位于马尾水平上相对较宽的蛛网膜下隙中的脑脊液(CSF)在使得神经根对针头穿刺反应时可以具有一定程度的移动。因此,该方法确保了将基因递送到腰椎DRG中的安全多重施用。腰椎穿刺在临床中用作脊椎麻醉的可行方法,并且用于引入治疗用的或诊断用的试剂。
因此,鞘内施用的含核酸序列的载体可以用于指导任何外源核酸在背根神经节的神经元细胞中的表达,从而将治疗用产品提供到外伤或疾病后的外周神经元或用于研究外周神经元中的基因表达。
在不同的实施方式中,载体是病毒载体,“病毒载体”指的是为将外源核酸引入细胞而构建的重组病毒。病毒载体包括,例如,反转录病毒、腺病毒、腺伴随病毒(AAV)、杆状病毒、牛痘病毒、疱疹病毒、甲病毒类载体、甲病毒类复制和慢病毒载体。
通过病毒载体将核酸递送到细胞中需要病毒囊膜的外表面上的分子和将被转染的细胞上的分子之间的特异性相互作用,例如,糖蛋白D与细胞表面受体,疱疹病毒进入介质A或nectin-1,之间的相互作用,(Krummenacher等.2003,Journal of Virology 77(16):8985)。作为替代的,病毒基因递送系统可以涉及非特异性相互作用,例如哺乳动物细胞的杆状病毒转染。杆状病毒对哺乳动物细胞显示了广泛向性并且可以用非细胞-特异性的静电相互作用来调节病毒的进入(Sarkis等.2000,Proc.Nat Acad.Sci.97:14638)。取决于被转染的目标细胞的特性,本领域技术人员能很容易的确定最适宜的病毒基因递送系统。

利用杆状病毒载体来进行神经系统基因转染的可能性已经进行了两项研究。最初的报告描述了体外和体内神经元细胞的足量转导(Sarkis等,2000,Proc.Nat.Acad.Sci 97:14638)。在人类胚胎的脑原代细胞培养物中,神经上皮、成神经细胞和胶质细胞可以被转染,尽管使用成年裸鼠的体内试验表明主要是星形胶质细胞以及仅一些神经元被转导了。第二项研究检查了脑中杆状病毒-介导基因表达的细胞类型且鉴定脉络丛的立方上皮细胞是主要目标,内表皮细胞中有适度的基因表达,而其他类型的脑细胞仅有非常有限或没有表达,包括神经元和星形胶质细胞(Lehtolainen等,2002,Gene Therapy9:1693)。

本领域技术人员易于理解怎样构建用在本发明中的杆状病毒载体。重组杆状病毒载体可以根据商品化杆状病毒表达系统所附的说明书进行构建,例如,Bac-to-Bac表达系统(Invitrogen)。可以用分子生物方法来改进重组杆状病毒载体,包括PCR方法和其他克隆方法,正如技术人员熟知的或已经被描述的,例如在Sambrook等,《分子克隆实验手册》(第三版),Cold Spring Harbour出版社。

通常,非病毒的基因递送系统依靠目标核酸的直接递送或依靠非特异性内化方法。非病毒基因递送系统及其转染方法已经被本领域技术人员所熟知,包括,例如,裸露质粒、DEAE-葡聚糖、磷酸钙共沉淀、微注射、脂质体-介导转染、阳离子脂类、和聚阳离子聚合物。可以被本领域技术人员进一步理解的,其中的一些方法,例如微注射、脂质体-介导转染、聚阳离子聚合物都可以在体内和体外转染细胞。可以改进这些非病毒载体以提高神经-特异性转染,例如通过将载体与一种或多种配体相连的方法,该配体特异性地或优先与神经细胞结合。例如,可以通过将破伤风毒素的非毒性片段C与聚赖氨酸通过共价相连实现聚赖氨酸/DNA复合物的神经-特异性转染。
相对于真核细胞而言,含有细菌序列DNA的非病毒载体通常具有增高了的回文CpG序列,并且这些外来CpG序列在脊椎中可以作为强免疫刺激剂。因此,降低CpG含量是有利的,并且可以提高蛋白质表达,由于CpG序列在真核细胞宿主中被甲基化后会导致转录沉默(Chevalier-Mariette等.2003,Genome Biology 4:R53)。在一些实施方式中,非基于病毒DNA载体的DNA中的CpG含量降低。已发现第一代腺病毒的CMV增强子启动子-增强子内的胞嘧啶残基甲基化,包括CpG位置上的胞嘧啶残基甲基化,是转基因表达降低的主要机理(Brooks等2004,J.Gene Med.6:395)。本领域技术人员易于理解,可以利用常规生物学方法,如Chevalier-Mariette等2003,Genome Biology4:R53中所描述的寡核苷酸或PCR为基础的诱变,降低载体中的CpG二核苷酸含量。
在一些实施方式中,非病毒载体是一种聚乙烯基亚胺/DNA复合物(PEI/DNA)。聚阳离子PEI体外和体内中都具有较高的转染效率(Boussif等.1995,Proc.Nat.Acad.Sci.92:7297)。优选地,PEI/DNA中的DNA是质粒DNA。在PEI/DNA复合物中,PEI氮含量与DNA磷酸含量之比优选为6-30,更优选是6-20,最优选是6-15(Boussif等.1995,Proc.Nat.Acad.Sci.92:7297)。本领域技术人员易于制备PEI/DNA,例如,用已有的商业化方法。一旦进入神经系统中,PEI就可以介导DNA转染终末分化的非-分裂神经元(Boussif等.1995,Proc.Nat.Acad.Sci.92:7297)。直接脑注射后,PEI/DNA复合物可以提供的转基因表达水平高于HIV-衍生载体获得的转基因表达高,并且其与用腺病毒载体所取得的转基因表达含量在相同的范围内。PEI/DNA复合物中的PEI具有的平均分子量为800kD,50kD,或更优选为25kD(Abdallah等1996,Hum Gene Ther.7(16):1947)。可以用其它聚合物对PEI进行共价修饰,例如使用聚乙二醇来降低PEI/DNA复合物的细胞毒性。(Shi等2003,Gene Therapy 10,1179)。
在不同的实施方式中,所述载体包括与编码核酸序列可操作连接的启动子。该启动子可以是强病毒启动子,例如CMV或神经-特异性启动子。可以利用细胞特异性启动子来获得所选细胞类型中的特异性基因表达。
神经元特异性启动子可以是,能活化神经元或神经细胞内且基本不活化其他细胞类型内可操作连接的序列的转录的任一核苷酸序列并。如果那些细胞类型任一种中可操作连接序列的转录水平足够低以至于不会影响该细胞的生理功能,那么所述启动子基本不会活化转录。
神经元特异性启动子包括用于神经元基因,如SynapsinI、神经元特异性烯醇酶、神经丝-L和神经肽Y的启动子,以及特异性针对特定神经元细胞类型的启动子。例如,酪氨酸羟化酶基因启动子(4.8kb 血小板-衍生生长因子β-链(PDGFβ)启动子(Sasahara M,Fries JW,Raines EW,Gown AM,Westrum LE,Frosch MP,Bonthron DT,Ross R,Collins T.PDGF[beta]-chain in neurons of the central nervoussystem,posterior pituitary,and in a transgenic model.Cell 1991;64:217-227)已经显示了对于包括多巴胺能神经元在内的神经元细胞的特异性。在一个实施方式中,神经元特异性启动子是PDGFβ启动子。在一个具体实施方案中,所述神经元特异性启动子是人类PDGFβ启动子。
例如,启动子的转录活性可以较弱,提供低于理想水平的治疗用基因序列表达。在不同的实施方式中,所述启动子可以与增强子子可操作地连接。正如本领域技术人员所理解的,“增强子”是能提高可操作连接启动子转录活性的任何核苷酸序列,在神经元特异性启动子情况下,其能提高神经元细胞中启动子转录活性。许多增强子已被大家所熟知,并且本领域技术人员也知道如何来筛选新的增强子序列,例如,通过筛选能提高报道基因转录的核苷酸序列,例如通过功能图谱(functional mapping)。
当第一核酸序列与第二核酸序列置于功能连接时,那么就说所述序列是可操作地连接。例如,如果启动子活化了编码序列的转录,那么该编码序列与该启动子是可操作连接。类似的,当增强子增加了可操作连接序列的转录,那么启动子与增强子就是可操作连接。当与启动子分隔开时增强子仍能发挥作用,因此,那么所述增强子与所述启动子是可操作连接,即使该增强子与该启动子不相邻接。
在不同的实施方式中,增强子可以是一种异源增强子,即是天然状态下不与启动子相连的核苷酸序列,但一旦可操作连接后,则能提高启动子的转录活性。提高可操作连接序列的转录水平指的是,相对于只是启动子时所观察到的转录水平而言,可操作连接序列转录水平的可测增加,这可以用常规转录试验来测量,包括使用报道基因构建体。
所述增强子可以是已知的强病毒增强子元件,如劳氏肉瘤病毒(RSV)启动子(Gorman等1982.Proc.Nat.Acad.Sci.79:6777-6781),SV40启动子(Ghosh等1981.Proc.Nat.Acad.Sci.78:100),CMV启动子或包括CMV即时早期(IE)基因增强子(CMVIE增强子)的启动子(Boshart等1985(细胞)41:521;Niwa等,1991,Gene 108:193;参见美国专利5849522和5168062)。
在本发明的一个实施方式中,CMV增强子可操作地连接于PDGFβ启动子的上游。在另一个的实施方式中,CMV增强子可操作连接于PDGFβ启动子的上游并且这两个序列是相接触的。在另外的实施方式中,CMV增强子与CMV启动子可操作连接(CMVE/P)。
与增强子可操作连接的神经元特异性启动子的其他实例包括等位基因变体以及已知启动子和增强子序列的衍生物,描述见美国申请10/407009。

当序列最佳对齐时,序列间的同源程度用同一性百分比来表示,即序列间准确匹配的发生率。可以利用多种算法来实现进行同一性比较序列的最佳对齐,例如运用Smith和Waterman的局部同源性算法,1981,Adv.Apple.Math 2:482,Needleman和Wunsch的同源对齐算法,1970,J.Mol.Biol.48:443,Pearson和Lipman的相似性搜索方法,1988,Proc.Natl.Acad.Sci.美国85:2444,和这些算法的计算机化应用(如Wisconsin Genetics软件包中的GAP、BESTFIT、FASTA和TFASTA,GeneticsComputer Group,Madison,WI,U.S.A.)。另外还可以使用BLAST算法进行序列对齐,描述见Altschul等1990,J.Mol.Biol.215:403-10(利用已发表的缺省设置)。用于运行BLAST分析的软件可以从国家生物技术信息中心(National Center for Biotechnology Information)获得(通过网址http://www.ncbi.nlm.nih.gov)。在不同的实施方式中,变体和衍生物具有利用此类算法所确定的至少50%,至少80%、至少90%或至少95%的同一性。
在不同的实施方式中,载体包括一种编码标志蛋白质的基因,该蛋白质的表达及细胞或亚细胞定位易于测定。“标志蛋白质”指的是其存在或亚细胞定位易于测定的蛋白质,如绿色荧光蛋白质(GFP)或其任一增强衍生物。其他的标志蛋白质已为本领域技术人员所知。在不同的实施方式中,所述基因可以编码其表达通过下述方式易于测定的酶:提供特异性底物,然后检测酶促反转产物,例如向含有荧光素酶的细胞或细胞裂解物提供荧光素。在另外的实施方式中,所述标志蛋白质可以是任何可用免疫学方法测定其表达的蛋白质,例如通过提供可特异性识别该标志蛋白质的标记抗体。该抗体优选是单克隆抗体,并且根据本领域熟知方法对其进行直接或间接的标记,例如用荧光颜料进行标记并通过荧光显微法检测蛋白质表达。其他的免疫学检测方法也已为本领域技术人员所知,包括但不限于,免疫金染色法、同位素标记法、比色酶促沉淀法。
优选地,所述核酸载体包含治疗用基因或治疗用转基因,其表达可以产生治疗用产物。术语“基因”是根据其通常定义来使用的,即核酸序列的可操作连接群。在此所用的“治疗用产物”是指能实现预期结果的任何产品,例如,疾病的治疗、预防或改善。治疗用产物可以是治疗用蛋白质、治疗用肽或治疗用RNA,如小干扰RNA(siRNA)或反义RNA。
在一些实施方式中,治疗用产物是神经营养因子,例如神经生长因子。在此所用的“神经生长因子”指的是任何能促进神经细胞生长和/或神经再生的因子,例如神经营养蛋白、神经营养蛋白受体和神经营养因子。“神经营养蛋白”指的是能支持神经存活和/或神经再生的蛋白质家族。神经营养蛋白包括,例如,神经生长因子(NGF)、脑-源神经营养因子(BDNF),神经营养蛋白3(NT-3)、神经营养蛋白4/5(NT-4/5)以及神经营养蛋白的变体。“变体”是指具有下列特征的蛋白质:由于一个或多个氨基酸取代、添加或缺失,其序列与自然存在的蛋白质序列不同,但保留了自然存在蛋白质的一些生物活性。正如本领域技术人员所理解的,变体与自然存在蛋白具有约60%、70%、80%,优选90%、或更优选高于90%的同源性。在具体实施方案中,所述治疗用产物是NGF。在一个具体实施方案中,所述载体包含与CMVE/P可操作连接的编码NGF的基因。
“神经营养蛋白受体”指的是能与神经营养蛋白结合的蛋白质。神经营养蛋白受体包括,例如,低亲和力的p75受体和高亲和力的受体,例如trkA、trkB、和trkC及其变体。“神经营养因子”包括,例如睫状神经营养因子(CNTF)、海马-衍生神经营养因子(HDNF)、白血病抑制因子(LIF)(Yamamori等1989,Science 246:1412)、酸性和碱性成纤维细胞生长因子(aFGF,bFGF)及其变体。

在其他的实施方式中,所述治疗用产物是一种抗-凋亡因子,例如bcl-2或bcl-XL。对成人而言,外周神经损害后DRG中会有20-40%的细胞损失,这与凋亡很相似(Terenghi 1999,J.Anat.194:1)。并不限于任何特别的理论,认为将抗凋亡的蛋白质递送到DRG中的神经元细胞中可以减少或防止这种细胞损失。
制备重组载体的方法为本领域技术人员熟知,例如,下列文献中描述的方法,Sambrook等《分子克隆,实验室手册》(第三版)Cold SpringHarbour Laboratory出版社(2001)和其他的一些实验室手册,以及商品说明书中描述的方法。
为了有助于施用,可以将载体配制成药物组合物中的一种成分。所述组合物通常含有药用可接受浓度的盐、缓冲试剂、防腐剂和各种可兼容的载体或稀释剂。对于所有形式的递送,载体都可以配制在生理盐溶液中。
药用可接受稀释剂的比例和同一性可以根据选择的施用途径、与载体的相容性和常规药物试验来确定。通常,所述药物组合物用不会显著损害载体生物学活性的成分配制。适宜的载体和稀释剂的描述见,例如,Remington′s Pharmaceutical Sciences(Remington′s PharmaceuticalSciences,Mack Publishing Company,Easton,Pa.,USA 1985)。
可以用生理学适宜的缓冲液制备载体溶液。在常规储藏和使用条件下,这些制剂中含有能防止微生物增长但不会使载体失活的防腐剂。本领域技术人员知道怎样制备适宜的制剂。用于选择和制备适宜的制剂的常规步骤和成分的描述见,例如,Remington′s Pharmaceutical Sciences and inThe United States Pharmacopeia:The National Formulary(USP 24NF19),1999年出版。
在一些实施方式中,载体被施用到脊椎动物宿主。在一个具体的实施方式中,载体被施用到人类宿主。
在一些外周神经病类型中,DRG被认为是早期且重要的目标,这些外周神经病包括糖尿病神经病,最常见的外周神经病(Kishi等2002,Diabetes51:819;England & Asbury,2004 Lancet 363:2151)。糖尿病神经患者者显示了广泛的感觉和植物性神经症状但是没有明显的运动失调。人类和实验糖尿病神经病的病理学研究证明了在DRG中具有细胞损失,这可能是因可见的mylenated纤维损失和轴突萎缩导致的。DRG中的神经元还直接参与因物理外伤、压力或横切所引起的外周神经损伤的病理生理过程。外周神经断裂还会引发受影响DRG中存活神经元的核周体中神经肽、细胞质和转录因子表达的生物化学改变,及导致近端神经残端的萎缩(Stoll &Muller 1999,Brain Pathol.9(2):313)。这些病理变化必定会干扰感觉信号从身体部分例如皮肤、肌肉和内脏的向CNS的传递。结果,身体的某些部分会功能异常或根本无功能。
在DRG中,治疗用基因的表达,特别是那些编码神经营养蛋白的基因的表达能阻止实验性神经病中的神经变性(Glorioso等2003,Curr OpinMol Ther.5:483)。然而,将基因转移到DRG还是充满了挑战,原因在于基因的解剖特性。在先前的研究中,通过将病毒载体,主要是单纯疱疹病毒(HSV)肌肉注射或皮下注射来实现所述转移,所述病毒载体被神经末端吸收,然后在轴质中被运输到DRG的神经元的质体中(Haase等1998,J.Neurol Sci.160 Suppl.;Jacksondeng 2003,Virology 314:45;Goss等2001,Gene Therapy 8:551)。尽管有效,但是该方法依赖于包括神经末端内吞作用和轴突逆行传导的功能性细胞机理,而这些机理在外周神经病条件下已经被破坏了。
在另一种不依赖神经末端内吞作用和轴突逆行传导的方法中,Glatzel等(2000,Proc.Nat.Acad.Sci 97:442)和Xu等(2003,Biomaterials24:2405)最近使用显微神经外科技术将基因转移载体直接注射入到DRG中,获得沿感觉神经通道的报道基因强表达。这种注射方法需要移去一片脊椎以获得进入DRG的通道。此外对DRG组织也是侵入性的,并且当需要重复注射时如慢性病症的长期治疗中,其也不可行的。
因此根据本发明,将含编码治疗产物的基因的载体最小侵入程度地递送到背根神经节的神经元细胞体,有利于治疗外周神经病,促进受损外周神经的生长和/或再生。“外周神经病”,正如本领域技术人员所理解的,指的是伴有或无明显运动失调的外周神经元细胞或其功能的损失。因此此类细胞是受外周神经病影响的目标细胞,可以通过细胞内治疗用基因的表达来治疗。外周神经病是疾病或病症或全身性疾病所导致的。许多神经病的病因已经很清楚,例如,糖尿病、尿毒症、AIDS、莱姆疾病或营养不良。外周神经病的其他病因包括机械压力例如压紧或挤压、直接外伤、穿透损伤、挫伤、骨折或骨脱臼;作用表面神经的压力、神经内出血;暴露在寒冷或辐射中或,很少见,某些药物或有毒物质;和血管或胶原蛋白病症,例如,动脉硬化症、系统红斑狼疮、硬皮病、肉状瘤病、风湿性关节炎、多动脉神经炎。虽然外周神经病的病因是多样的,但是他们通常产生共同的症状,包括胳膊、手、腿和/或脚的虚弱、麻木、感觉异常(不正常的感觉例如灼热、挠痒、刺痛感或发麻)和疼痛。大量病例的病因不明。
优选通过鞘内注射来施用载体,所注射的量足以取得所需的效果,例如,在目标细胞中具有有效量的治疗用基因表达。
在一些实施方式中,可以通过腰椎穿刺来施用载体。腰椎穿刺是相对常规并且没有创伤的临床步骤,其对脊髓的危害极小。通常,用与注射器或微注射器相连的窄针头例如,26号针头将含载体的溶液施用入脑脊液。对于大鼠而言,针头的适宜鞘内位置可以通过轻微尾部运动来确证,这表明是进入到蛛网膜下空间的适宜注射。在注射后,针头在鞘内空间内保持一段时间,例如2分钟后再移去。
可以重复给予有效量的载体,这取决于初始治疗方案的效果。通常周期性地施用,同时监视任何反应。根据所选定的施用计划和程序,本领域技术人员知道应给出的较低或较高剂量。
例如,当载体被施用到人类患者时,载体以有效的剂量并保持足够的时间来被施用以获得所需的效果。例如,载体以所需的量和剂量被施用以递送治疗用基因,该基因产物的功能在于减缓、改进、调节、改善、稳定、防止扩散、减慢或延迟外周神经病的病程或治愈外周神经病。
被施用到患者的有效量可以改变,这取决于很多因素,例如,治疗用基因产品的药物动力学特性、施用模式、受试者的年龄、健康状况和体重,病症或病态的特性以及程度,治疗的频率以及如果有,共同治疗的类型。在使用病毒载体的实施方式中,所述有效量还取决于病毒的毒力和滴度。
基于上述因素,本领域技术人员可以确定出适宜用量。在最初载体可以适宜量被施用,其可以根据需要进行调整,这取决于患者的临床反应。可以根据经验确定载体有效量,这取决于该载体可以被安全施用的最大量。在一些实施方式中,载体对脊椎动物几乎没有细胞毒性,因而可以大量施用。然而,被施用的载体量应该是能产生所需效果的最小量。
在不同的实施方式中,大约109用量的重组杆状病毒颗粒被施用到人类患者中。在其他的实施方式中,约102-约109的重组杆状病毒颗粒、约106-约109的重组杆状病毒颗粒、约102-约107的重组杆状病毒颗粒、约103-约106的重组杆状病毒颗粒、约104-约105的重组杆状病毒颗粒以单一剂量被施用。在一些实施方式中,载体可以被施用超过一次,例如,通过重复注射。在其他实施方式中,通过一个与含有载体组合物的储液囊相连的鞘内导管将病毒载体重复施用,描述见Jackson等2001,Human GeneTherapy 12:1827。
在其他实施方式中,将含有约4μg DNA的非病毒载体以单一剂量的方式施用到宿主中。非病毒载体仅瞬时表达一次性转染入靶细胞的治疗用基因,效果不及最优转基因表达。在本发明的一些实施方式中,通过一个与含有载体组合物的储液囊相连的鞘内导管,将病毒载体重复施用,描述见Jackson等2001,Human Gene Therapy 12:1827。
对已断裂的外周神经最好按照下述进行治疗。将编码治疗用基因产物的载体,例如NGF等神经生长因子,鞘内施用到宿主DRG的神经元细胞中,该宿主的断裂的近端和远端神经残端被固定在神经导管中。载体被施用入背根神经节周围的脑脊液中,从而将所述核酸递送到已切断神经的细胞体中。例如,如果是坐骨神经断裂,可以通过腰部注射的方式将载体鞘内注射到L4和L5脊椎间脊内空间内的脑脊液中。
本申请所述的“神经导管”也被称为“神经导向通道”或简单称为“神经导向”,其指的是一种具有通过一个内在通道连接的第一和第二开口末端的装置,其中管内径足够大因而能在两端接受切割。神经导管可用于引导近端神经末端的轴突芽生,为受损神经末端分泌的生长因子的扩散提供通道,以及减少疤痕组织的渗透(schmidt等2003,Annu Rev BiomedEng5:293)。正如本领域所熟知的,神经导管可以来自生物材料,例如细胞外基质蛋白、例如胶原蛋白(US50190987)、层粘连蛋白、纤连蛋白、纤维蛋白/纤维蛋白原、透明质酸类物质或来自如硅胶、泡沫聚(四氟乙烯)。此外,合成神经导管还可以由生物可吸收或生物可降解物质制备而得,例如,聚(乳酸)(PLA)、聚(羟基乙酸)(PLG)、乳酸羟基乙酸共聚体(PLGA)、聚(己内酯)、聚(氨酯)、聚(寡)膦嗪、聚(3-羟基丁酸酯)和异丁烯酸酯类水凝胶。(参见Schmidt&Leach和本申请中的参考文献)。神经导管可以是不通透、通透或半通透性的,其可以引入神经元载体细胞、例如施沃恩细胞,其还可具有导向神经基底,以及其还可以有一个或多个腔内通道(Hudson等1999,Clin Plast Surg26:617)。
本领域技术人员知晓如何将已断裂的神经固定到神经导管中,例如按照Schmidt等2003,Annu Rev Biomed Eng 293。通常,通过例如9-0或10-0缝合将切断裂外周神经的近端和远端残端固定在神经导管的内通道内。优选地,神经导管内通道充满了溶液、悬浮液或轴突生长凝胶支持物。
在正常情况下,NGF呈现出非常低的浓度,但是在动物模型神经损伤处快速升高。切割时,NCF主要由损伤神经远残端的目标组织和施沃恩细胞产生,然后被逆向运输入神经细胞体内。背根神经节的神经元细胞体内的NGF外源表达可以被逆行传输而递送的NGF补充或取代。并不限于任何特殊的理论,NGF的外源表达会促进神经营养因子的顺行传输到已断裂神经的近端残端,并且这些因子会通过神经导管扩散并在导管内促进神经的再生。
正如本领域技术人员所理解的,可以在神经残端被固定在导管之前完成载体的施用,或更优选是在其之后。本领域技术人员易于确定有效促进宿主外周神经再生的载体施用时间。
在此列出的所有文献都全部引入以作参考。
虽然在此公开了本发明的不同实施方式,但是本领域技术人员用普通知识作出的适应和改进在本发明的范围内。此类改进包括用于本发明任一方面的已知等效物的替代以基本相同的方式取得相同的结果。在此所用的所有方法和科技术语与本发明所属领域普通技术人员普通理解的意思相同,另有定义的除外。

实施例
原料和方法
基因递送载体
在此测试了三种非-病毒基因递送系统,聚乙烯氨(PEI)/质粒DNA复合物、lipofectamineTM2000/DNA复合物和肽系统,以及两种病毒载体,重组杆状病毒和2型腺伴随病毒(AAV-2)。
为了制备PEI/DNA复合物,将质粒DNA pCMV E/P-luc(Liu等,2004,Gene Ther.11:52)或pcDNA3.1/NCF与PEI(25kDa;Sigma-Aldrich,SanDiego,CA)通过下述方式在5%葡萄糖溶液中进行混合:将适量的PEI溶液添加到DNA溶液中,通过涡旋方式简单混合,然后在室温下静置30分钟。通过将来自鼠脑cDNA文库的全长DNA片段插入经EcoR1消化后的pcDNA3中,构建得到pcDNA3.1/NGF。用于细胞转染和动物实验的PEI与DNA之比分别是每当量DNA磷对10和14当量的PEI氮。在一些实验中,在PEI与DNA混合前,用羰花青染料Cy3(Amersham、乌普萨拉市、瑞典)对PEI进行了标记。考虑1μgDNA含有3nmol的磷来计算所需质粒的量。为了制备lipofectamineTM2000/DNA复合物,将pCMV E/P-luc与lipofectamineTM2000/(Invitrogen,新加坡)通过下述方式在5%葡萄糖溶液中进行混合:将适量的lipofectamineTM2000添加到经稀释的DNA溶液中,通过涡旋方式简单混合,然后在室温下孵育20分钟。动物实验中的DNA(μg)与lipofectamineTM2000(μl)之比为1-3。
为了制备肽基因递送系统,使用肽NL4-10K和PEI600。所述肽含有来自NGF的含环4区域的29-氨基酸片段(aa80-108),该片段与作为DNA结合结构域的10-赖氨酸连接(Zeng等,J Gene Medicine,2004,出版中)。首先将DNA与PEI600以氮/磷比为5的比例进行第一次复合,然后以肽/DNA(nmol/g)为1.5的比例加入肽。
根据Bac-To-Bac杆状病毒表达系统手册(Gibco BRL,LifeTechnologies,USA)来构建重组杆状病毒载体。人类CMV E/P启动子或CMV E/PDGF启动子调控下的荧光素酶DNA来自Liu等所构建的载体(《基因治疗》,2004,11:52)。将启动子插入pFastBacl的NotI和XbaI位点之间,荧光素酶cDNA在启动子下游的XhoI和HindIII之间。在Sf9昆虫细胞内增殖重组杆状病毒。将来自昆虫细胞培养基的芽生(budded)病毒用0.2μm孔径的滤膜过滤,并以25000g超离60分钟进行浓缩。对于吸收试验而言,根据供应商提供的手册(Amersham、Uppsala,Sweden)用羰花青染料Cy3对PEI进行了标记。通过下述方式构建了重组AAV-2载体:将来自pCMV E/P载体的CMV E/P启动子或来自pCMV E/PDGF载体的CMVE/PDGF启动子亚克隆入侧连ITR序列的经修饰的pAAV质粒。这种pAAV质粒名为pAAV-MCS-luc,是通过用多重克隆点(MCS)-荧光素酶-聚A表达框取代质粒pAAV-MCS(Strtagen,La Jolla,CA)上两个NotI位点之间的原始序列构建得到的。该表达框从pGL3-为基础的质粒(Promega,美国)PCR扩增得到的,使用正向引物,5-ATTGCGCCCGCGGTACCGAG(SEQ IDNO:1)和反向引物,5-ATTGCGCCCGCTTATCGATTTTACCACATTTG(SEQ ID NO:2)。将启动子插入pAAV-MCS-luc中的KpnI和Hind III位点之间。使用该质粒与AAV-2包装质粒pAAV-RC及腺病毒辅助质粒pHelper(Stratagen,La Jolla,加拿大)一起转染HEK293(人胚胎肾)细胞。通过两轮的冷冻/解冻循环将包装入转染后HEK293细胞中的AAV-2载体从被收集细胞中释放出来,然后用一步重力-流(gravity-flow)肝素亲合柱对其进行了纯化。
动物与操作
整个试验中使用了成年雄性Wistar大鼠,重量为250-320g,由新加坡国立大学的实验动物中心提供。使用了50只大鼠来进行荧光素酶活性化验,其中30只大鼠用于时程研究(每个时间间隔5只),PEI、lipofectamineTM2000、AAV和杆状病毒载体试验各使用了5只。免疫组化试验使用了12只大鼠,其中的6只为假操作组,另6只为实验组。使用了12只大鼠来进行PCR分析,其中的6只在假操作组而6只在实验组。在注射后的不同时间,随机将大鼠分配到每个组中。将大鼠每笼放4只在恒温22℃和60%湿度下经过一个白天-黑夜循环(12h/12h)内,并用常规实验鼠粮进行喂养。所有动物的处理和饲养均按照,由国际卫生组织制定的且被新加坡国立大学实验动物中心采用的国际试验动物指导手册(International Guiding Principles)(1985)。
对于鞘内注射来说,通过腹膜内注射戊巴比妥钠来麻醉大鼠(60mg/60kg体重)。将大鼠的后背皮肤切开暴露脊柱。选择第4和5腰椎(L4-5)之间的脊柱内空间作为注射位置。一个与26号针头相连的10μl微注射器用来进行注射。鼠尾的轻微运动表明了正确注射进了蛛网膜下空间。每次注射使用4μg质粒DNA与1×108AAV-2颗粒或1×107杆状病毒颗粒的复合物20μl。2-5分钟的缓慢施用之后,针头被移出前在原处停留2分钟。注射后用手术夹闭合皮肤。
为了制备神经损伤和再生的鼠模型(Xu等2003,Biomate[eta]als 24:2504),通过2厘米长的皮肤切口来暴露被麻醉鼠的右坐骨神经。在去除7mm的一段神经后,接着将近端和远端神经残端拔入硅胶神经导管的每端开口内2mm(NGC,TygonID:0.05英寸,OD:0.09英寸,长:1.4厘米),并留有10mm的残端间隙。在近端残端被拔入管开口端前,将25微升的盐水填充到管内。用单根10/0神经束膜缝合线(Ethilon)将两个残端固定到管内。在NGC植入后,立即将含4μg pcDNA/NGF的PEI复合物20μl鞘内注射入L4和L5脊柱的脊内空间,对照大鼠注射PEI/pcDNA3/luc复合物。在该实验中使用了30只大鼠,其中15只注射了PEI/pcDNA3.1/NGF复合物,另外15只作为对照。
报道基因检测
进行了PCR扩增以检测DRG中运输的报道基因。在腰部鞘内注射PEI/DNA复合物或杆状病毒载体后2天,深度麻醉后用0.1MPBS(pH7.4)进行心脏内灌注使大鼠致死,并在注射位置收集了三对成对DRG(腰椎L4至L6)。用刀片进行切碎使得组织均质化,然后按照Dneasy Tissue试剂盒(Qiagen,Hilden,德国)常规方法提取DNA。根据荧光酶素基因序列设计了用于PCR的寡核苷酸引物,序列如下所示:5’引物,5’-AT TGC TCAACA GTA TGG GCA-3’(SEQ ID NO:3))))))))))),3’引物:5’-CGA AGA AGGAGA ATA GGG TTG-3’(SEQ ID NO:4)。扩增产物的预期大小是540bp。扩增循环为94℃5分钟,1循环;94℃,45秒,55℃30秒,72℃30秒,35个循环,最后72℃延伸7分钟。为了排除整个过程中的污染,对取自假操作大鼠的样本进行平行操作。
用荧光素酶活性试验来检验荧光素酶报道基因的表达。大鼠在深度麻醉后用0.1MPBS(pH7.4)进行心脏内灌注而使大鼠致死。在注射位置周围收集了六对成对DRG并在-80℃下保存至进行操作。加入PBS缓冲液后(100μlPBS/50mg组织),每份样本都在冰上用超声波均质化10秒钟,接着4℃下在微离心器中以13000rpm进行离心。使用10毫升上清液室温下进行荧光素酶活性试验,使用购自Promega(Madison,WI,美国)的试验试剂盒。使用单-孔光度计(Berthold Lumat LB9501)测量10秒钟。用组织提取物的总蛋白浓度标准化RLU,用蛋白质试验试剂盒(Bio-Rad,Hercules,CA,USA)进行测量。
对于免疫染色,注射后2天杀死大鼠。在深度麻醉后,首先用格氏溶液,接着用含2%多聚甲醛的0.1MPBS(pH7.4)灌注大鼠。灌注后,取出3对成对DRG(L4至L6),用相同的固定液后-固定(post-fixed)2-4个小时后转移到含15%蔗糖的0.1M PBS中。切出30μm厚的冷冻切片,然后封装(mount)于在包被后的载片上。该切片在0.1M PBS中洗涤20分钟,该PBS的pH为7.4,其中含有0.2%Triton X-100,接着用含5%正常山羊血清的PBS封闭1小时。然后用抗-荧光素酶多克隆一抗(Promega;1∶150稀释)和抗神经特异性核蛋白单克隆抗体(ChemiconInternational,美国;1∶500稀释)孵育切片过夜。在0.1M的PBS中洗涤该切片,然后用抗-兔IgG Fitc偶联物(Sigma-Aldrich公司,美国;1∶100稀释)和抗-小鼠IgG Fitc偶联物(Sigma-Aldrich公司,美国;1∶100稀释)再孵育1小时。在孵育后,在PBS中洗涤三次该切片,用DAKO荧光素酶固定介质固定,用盖玻片覆盖。对照切片没有用一抗孵育。用Olympus 500共焦激光扫描显微镜对切片进行检测。每个切片先用488nm激光线(laser line)和发射滤光器(emission filter)BP510-525扫描以检测Fitc荧光素;然后用543nm激光线和发射滤光器LP570来检测Tritc荧光素。
NGF试验
使用Cos7细胞进行体外转染,检验质粒pcDNA3.1/NGF的基因表达。将培养物细胞种植在60-70%融合的6-孔平板。过夜孵育后,用OptiMEM替换培养基,向每孔中加入25μl等份的含4μgpcDNA3.1/NGF的PEI/DNA复合物。在37℃下,用DNA/PEI复合物孵育细胞3个小时。接着用新鲜完全培养基替换上述培养基。继续孵育24小时后,收集细胞和培养基用于NGF试验,使用灵敏的NGF ELISA试剂盒(Boehringer Mannheim)。对于对照组,用pcDNA3-Luc代替pcDNA3.1/NGF。在体内NGF表达试验中,使用了15只大鼠,,其中10只用于实验组,第3天和第7天的时间间隔各5只。让4μg的pcDNA3.1/NGF或pcDNA3/NGF与20μl的PEI复合,然后如同上文所述注射入到每只大鼠中。收集DGR并进行均质化。上清液用于NGF ELISA。
神经再生评估
操作后四周,再次麻醉大鼠,将坐骨神经与NGC一起暴露,并将其小心地从周围组织中剥离出来。用镊子夹持住远离管的神经片段以确定神经再生的成功。后背肌肉的收缩或腿部的收回表明在被夹持片段中存在再生的感觉纤维,而如果没有反应则表示不存在此类纤维。
对于组织学检测,收集NGC中的再生神经链(nerve cable),并将其在2.5%戊二醛的PBS缓冲液(pH=7.4)中固定过夜。随后进行的固定、包埋、切片和染色步骤都如前所述(Xu等。2003《生物材料》24:2504)。接着用半薄(semi-thin)切片操作再生神经的末梢片段,并用TolubineBlue进行染色以进行形态分析。对取自10mm间隙(gap)中部的17个横向半薄切片进行分析,以确定再生轴突数量、纤维群和纤维直径。用MicroImage LiteTM(Olympus,图像分析软件)来进行定量测量和评估。选择感兴趣区域用于超薄切片。用柠檬酸铅对超薄切片(100nm)进行染色,在铜编织网上进行收集并用80-100kV的Philips EM 208s电子显微镜进行检测。评估样本的神经组织再生和抗聚合物的异体反应。
实施例1:通过腰部鞘内注射将基因转移到DRG中
首先,鞘内施用Cy3共价标记的杆状病毒载体或PEI/DNA复合物后,我们从评估DRG对基因载体的吸收开始。注射后2天,在注射位置附近收集DRG,并用抗-NeuN对其切片进行染色。在DRG中检测到了红色Cy3信号,主要位于高倍下的NeuN-阳性细胞的细胞质中(图1)。对注射后第1和第3天收集到的DRG样本进行PCR分析,结果表明CNS注射后在这些PNS区域中存在被运输的报道基因(图2)。
对带有荧光素酶报道基因的四种不同的基因递送系统,PEI/DNA复合物、lipofectamin/DNA复合物、杆状病毒载体和AAV-2载体,检测了它们在介导转基因转移进入DRG细胞的效果。腰椎鞘内注射2天后,很容易的检测了上述各种类型载体在DRG细胞中的荧光素酶活性(图3)。在研究来自PEI/DNA复合物的转基因荧光素酶表达的时程实验中,早在注射后第1天就检测到了酶活性,在第3天时达到峰值,然后在接下来的几周中逐渐降低,在注射后4周仍能检测到较低的活性(图3)。荧光素酶的免疫染色表明DRG中所检测到的蛋白质并不限于上皮组织,而且还存在于神经节细胞中。对荧光素酶和神经元特异性NeuN蛋白质的双重免疫染色表明:DRG中的荧光素酶阳性细胞其中的大部分也是NeuN-阳性的,只有很少的荧光素酶阳性细胞是非神经元细胞(图5)。
实施例2:通过神经导管的NGF cDNA转染和神经再生
NGF是一种主要作用于感觉和交感神经元的神经营养因子(Thorne,R.G.,and Frey,W.H.II 2001,Clin Pharmocokinet 40:907),选择NGF,通过神经导管(NGC)检测其在DRG中表达对外周神经再生的影响,神经导管是一种广泛用在前-临床试验中修复神经缺陷的装置(Schmidt等2003,Ann.Rev.Biomed.Eng.293)。我们对来自PEI-介导的基因递送的NGF cDNA表达进行了检测。在转染COS7细胞后,培养基中的NGF浓度升高,正如在敏感ELISA中所检测的一样,水平为约1ng/ml,比对照组高6倍(图6A)。类似的升高还发现于对细胞裂解物进行分析的时候(图6B)。在腰椎鞘内注射PEI/NGF cDNA复合物后,DRG中的NGF表达比对照组高3倍并且持续了至少7天。
实施例3:NGF表达对神经再生的影响
夹持实验显示:通过NGCs获得的神经再生成功率,NGF组在操作后4周时是87%,而对照组为67%。在从夹持实验阳性大鼠收集的管内发现了将两个神经残端10mm间隙桥连的再生组织链(tissue cable)。从NGF组的NGC中所收集到的组织链显示了比对照组更具改善的再生品质,神经纤维的直径显著增大,G-比率减小(Friende等1982,Brain Research235:335),G-比率为轴突直径与有髓纤维之比(图7)。所检测的另两个参数即纤维群和密度,尽管可以看出在NGF组中其有所增加,但是并没有发现显著的统计学差异,这可能是因为较大的标准差(图7)。
通过透射电子显微镜还可以检测到神经再生的形态标志。再生的神经链位于管内中心,四周包裹着精致的神经外膜。在NGF组和对照组中,所述链内均含有大量再生的有髓和无髓轴突的肌束。相对于对照组来说,NGF组中的有髓轴突分别具有更大和更高的数量、直径和密度(图8)。NGF组中的髓鞘也比对照组中的厚得多。
因此本研究证明了治疗用NGF基因对断裂坐骨神经的再生具有的积极的作用,为外周神经再生的基因治疗提供了一个实例。NGF是第一个并且是研究最为透彻的神经-来源的因子,其作用的神经元群的种类相对有限,包括外周神经系统感觉神经元的交感亚群,和脑内的纹状体(striatial)及胆碱能神经元(Terenghi,G.1999,J.Anat.184:1)。在正常情况下,NGF呈现出非常低的浓度,但是在实验性神经损伤动物模型中快速升高。该蛋白主要由损伤神经远残端的目标组织和施沃恩细胞产生,然后在作用神经元受体及产生神经营养功效前以逆向方式运输入细胞体内。外周神经病中,病态神经内的此类运输负性减少或深圳被完全阻断。保持运输机制从而维持受影响神经元正常功能的替代方式对于外周神经病的治疗来说是十分重要的。通过鞘内注射被递送基因载体到DRG神经元中不需要轴突传输就可到达细胞质中,其能有利地用于支持DRG中的神经元功能,从而减慢或终止相关轴突的变性过程。
形态分析和TEM分析都表明:DRG中的NGF转染、神经再生、特别是新轴突的直径都有所改善。轴突中神经丝的数量被认为是控制轴突直径的关键因素,其在感觉神经元中可能受到NGF的调节。
在外周神经断裂后,感觉神经元中的神经体大小、轴突直径、神经丝合成和神经丝的轴突传递均减小。本试验发现,NGF cDNA的转染会上调NGF蛋白质的表达、有助于提高神经丝合成并且显著的增大纤维直径。
正如本领域技术人员所理解的,在此描述的实施方式可以有很多改进。而本发明旨在包括落于权利要求限定范围内的所有这样的改进。

Claims (47)
1、一种将核酸递送到宿主外周神经系统的神经元细胞中的方法,其包括下述步骤:辨识背根神经节中的目标神经元细胞,然后将包含核酸的载体施用到宿主脑脊液中的位置,其中所述位置足够接近背根神经节的位置,以将核酸递送到目标神经元细胞的细胞体中。
2、一种治疗宿主外周神经病的方法,该方法包括:辨识受神经病影响的背根神经节中的目标神经元细胞,然后将包含治疗用核酸的载体施用到宿主脑脊液中的位置,其中所述位置足够接近背根神经节的位置,以将核酸递送到目标神经元细胞的细胞体中。
