Relatório Descritivo da Patente de Invenção para STENT PARA SER DISTRIBUÍDO POR CATETER PARA O TRATAMENTO DE UM ANEURISMA AÓRTICO ABDOMINAL.
Antecedentes da Invenção
Campo da Invenção [001] A presente invenção refere-se geralmente a um stent de distribuição intravascular para o reforço de um vaso sanguíneo, e mais particularmente a tal stent projetado especificamente para solucionar um aneurisma aórtico abdominal (AAA).
Discussão da Técnica Anterior [002] Um aneurisma aórtico é uma área enfraquecida na aorta, o vaso sanguíneo principal que transporta sangue do coração para o resto do corpo. A aorta se estende ascendentemente do coração no peito e então arqueia descendentemente, percorrendo o peito (a aorta torácica) e entrando no abdome (a aorta abdominal). O diâmetro normal da aorta abdominal é de cerca de 2,54 cm (uma polegada). À medida que o sangue flui através da aorta, a área enfraquecida se avoluma como um balão e pode explodir se o balão ficar muito grande.
[003] Os aneurismas aórticos mais comuns ocorrem na parte do vaso abaixo das origens da artéria renal. O aneurisma pode se estender para dentro do vaso que suprem os quadris e a pelve.
[004] Uma vez que um aneurisma atinge 5 cm de diâmetro, normalmente o tratamento é considerado necessário para evitar o rompimento. Abaixo de 5 cm, o risco de o aneurisma se romper é inferior ao risco da cirurgia convencional em pacientes com riscos cirúrgicos normais. O objetivo da terapia para aneurismas é evitar que o mesmo se rompa. Uma vez que um AAA se rompe, as chances de sobrevivência são muito baixas, com 80 a 90% de todos os aneurismas que se rompem resultando em morte. Essas mortes podem ser evitadas se o aneurisma for detectado e tratado antes de romper.

2/9 [005] A maior parte dos aneurismas aórticos ocorre na aorta abdominal, a causa principal sendo arteriosclerose. Essa é uma condição na qual os depósitos de gordura são criados nas paredes das artérias, que se tornam menos elásticas e mais fracas. Os fatores de maior risco para arteriosclerose são o tabagismo e pressão sanguínea alta além de fatores genéticos.
[006] AAA pode ser diagnosticado a partir de seus sintomas quando ocorrem, mas isso ocorre freqüentemente muito tarde. Os mesmos são normalmente encontrados em exames físicos de rotina e raios-X de tórax e abdominal. Durante o exame, o médico pode sentir uma massa pulsando no abdome. Se o médico suspeitar de um aneurisma, ele/ela solicitará provavelmente um ultra-som. Outros exames, tal como tomografia computadorizada (CT) e ressonância magnética (MRI) também podem ser realizados. Essas técnicas de varredura são muito úteis na determinação da posição exata do aneurisma.
[007] O procedimento cirúrgico para o tratamento de AAA envolve a substituição da parte afetada da aorta por um enxerto sintético, normalmente compreendendo um tubo feito de um material elástico com propriedades muito similares às de uma aorta normal e saudável. Essa operação crítica é normalmente bem-sucedida com uma mortalidade de entre 2 e 5 porcento. O risco de morte por um AAA rompido é de cerca de 50%, mesmo durante a cirurgia.
[008] Mais recentemente, em vez de se realizar uma cirurgia aberta para realização do reparo do aneurisma, cirurgiões vasculares têm instalado um stent endovascular e distribuído o enxerto no local do aneurisma utilizando cateteres alongados que são rosqueados através dos vasos sanguíneos do paciente. Tipicamente, o cirurgião realizará uma pequena incisão na área da virilha do paciente e então inserirá um cateter de distribuição contendo um stent/enxerto expansível tipo balão ou de auto-expansão, desmontado, para um local que forma

3/9 uma ponte com o aneurisma, ponto no qual o stent/enxerto é distribuído a partir da extremidade distal do cateter de distribuição e pode ou é expandido para aproximadamente o diâmetro normal da aorta nesse local. O stent/enxerto, obviamente, é uma estrutura tubular permitindo o fluxo de sangue através do lúmen do mesmo e removendo a pressão do aneurisma. Com o tempo, o stent/enxerto se torna endotelizado e o espaço entre a parede externa do stent e o aneurisma por fim preenche com sangue coagulado. Nesse momento, o aneurisma não está mais sujeito a pressões aórticas e, dessa forma, pode continuar a crescer.
[009] No tratamento de AAA, é importante que o stent seja localizado com precisão de forma a não obstruir o fluxo de sangue através das artérias renais que se ramificam a partir da aorta abdominal.
[0010] Em Amplatz et al., Patente U.S. N° 6.932.837, é descrito um stent/enxerto desmontável para enxerto de um lúmen de um vaso sanguíneo selecionado ou outro órgão tubular. O stent/enxerto compreende um tecido tramado ou trançado feito de uma pluralidade de fios de uma liga de efeito memória. O tecido é formado como um tubo e cada extremidade do dispositivo é aberta para permitir o fluxo de fluido através do mesmo. O dispositivo pode ser esticado longitudinalmente para, dessa forma, reduzir seu diâmetro, permitindo que o mesmo seja inserido no lúmen de um cateter de distribuição. Quando ejetado a partir da extremidade distal do cateter de distribuição, o stent/enxerto expandirá automaticamente para um diâmetro externo predeterminado suficiente para engatar a parede do vaso tubular sendo tratado.
[0011] Enquanto o dispositivo na patente '837 é adequado para uso como um stent coronariano, não é bem-adequado para o tratamento intravascular de AAA. Esse dispositivo tem uma tecedura uniforme, mas necessariamente tem uma densidade de fio que é insuficiente para limitar a exposição do aneurisma à pressão sanguínea aórtiPetição 870170044329, de 27/06/2017, pág. 12/24
4/9 ca. No caso desse stent/enxerto também crescer além de seus limites por sobre a ostia das artérias renais, será necessário se restringir o fluxo de sangue para os rins.
[0012] Existe, portanto, uma necessidade de se criar um stent/enxerto que possa ser colocado utilizando-se uma abordagem endovascular no tratamento de AAA, mas que não obstrua indevidamente o fluxo sanguíneo para os rins. A presente invenção fornece tal dispositivo.
Sumário da Invenção [0013] A presente invenção fornece um stent endovascular distribuído por cateter para o tratamento de AAA que compreende uma estrutura tubular mais interna possuindo um primeiro comprimento. Compreende uma pluralidade de fios de metal trançados de uma liga de efeito memória. A trama da trança é escolhida para definir aberturas suficientemente grandes de modo a não impedir materialmente o fluxo de sangue através da parede da estrutura tubular mais interna. Cercando a estrutura tubular mais interna encontra-se pelo menos uma estrutura tubular adicional de um diâmetro predeterminado, mas possuindo um comprimento que é inferior ao comprimento da estrutura tubular mais interna. A estrutura tubular adicional também compreende uma pluralidade de fios de arame trançados que são maiores em número do que o número de fios que criam a estrutura tubular mais interna. Os fios de arame da estrutura tubular trançada adicional também compreendem uma liga de efeito memória e a trança dos mesmos possui uma trama que define aberturas suficientemente pequenas de modo a substancialmente obstruir o fluxo de sangue através das mesmas. O estiramento longitudinal das estruturas tubulares mais interna e adicional dispostas de forma coaxial reduz o diâmetro externo do dispositivo o suficiente para permitir que o mesmo seja carregado dentro do lúmen de um cateter de distribuição endovascular. [0014] A parte da estrutura tubular mais interna que se estende além de um terminal distal da estrutura tubular adicional pode se sobrepor à junção das artérias renais do paciente com sua aorta abdominal quando a estrutura tubular adicional forma uma ponte para um aneurisma aórtico abdominal, mas sem bloquear o fluxo de sangue para os rins.

Descrição da Modalidade Preferida [0017] Com referência à figura 1, é indicada geralmente pela referência numérica 10 a modalidade preferida do stent/enxerto construída de acordo com a presente invenção. O stent/enxerto 10 é ilustrado no lugar em um segmento da aorta abdominal 12 possuindo um aneurisma 14. Em sua extremidade inferior, a aorta abdominal 12 ramifica para as artérias ilíacas comuns esquerda e direita 16 e 18. Também são ilustradas na figura 1 as artérias renais 20 e 22 levando aos rins (não ilustrados).
[0018] O stent/enxerto 10 compreende uma estrutura tubular mais interna 23 de um primeiro comprimento (L1). A estrutura tubular mais

6/9 interna compreende uma primeira pluralidade de fios de arame trançados 24, preferivelmente de uma liga de efeito memória. A trança compreendendo a estrutura tubular mais interna 23 possui uma trama predeterminada para definir as aberturas através da estrutura que são suficientemente grandes de forma a não impedir materialmente o fluxo sanguíneo através de sua parede com janelas. Os fios de arame podem ter um diâmetro na faixa de cerca de 0,005 cm a cerca de 0,02 cm (de 0,002 a 0,01 polegada) e a estrutura tubular 23 é projetada para fornecer uma força externa radial adequada necessária para autoexpansão do stent/enxerto 10.

[0022] De acordo com a presente invenção, aberturas ainda menores podem ser fornecidas através do comprimento L2 criando-se uma segunda estrutura trançada tubular mais externa 30 cercando de forma coaxial a estrutura tubular intermediária 26 que cerca a estrutura tubular mais interna 23. Essa segunda estrutura tubular mais externa 30 também pode se estender pelo comprimento L2 e pode ser idêntica em sua configuração trançada à estrutura tubular adicional 26, por exemplo, 144 fios de 0,0254 mm (0,001 polegada) de diâmetro de fio Nitinol trançados de modo a apresentarem aberturas de 0,0006 cm2 (0,0001 polegada ao quadrado).
[0023] É contemplado que o stent/enxerto 10 pode ser fabricado utilizando o método apresentado na Patente U.S. N° 6.123.715 de Curtis Amplatz, os ensinamentos da qual são incorporados aqui por referência. A estrutura mais interna 23 pode ser trançada para formar um tecido tubular como seria a estrutura ou estruturas tubulares adicionais 26. O tubo ou tubos trançados externos seriam então dispostos de forma concêntrica através da estrutura tubular mais interna e a combinação seria colocada em torno de um mandril cilíndrico do diâmetro externo desejado para o stent/enxerto. Esse conjunto poderia então ser aquecido a uma temperatura predeterminada e por uma duração de tempo suficiente para configurar com calor as estruturas tubulares ao diâmetro do mandril. Extremidades livres opostas 32, 34 dos fios compreendendo a estrutura tubular mais interna 23 podem ser alargadas radialmente para fora por 10 a 30 para fornecer sobreposição aperfeiçoada com a parede interna da aorta. Após a remoção do molde, os dois ou mais tubos trançados coaxiais podem ser mantidos unidos com alguns poucos pontos de sutura de poliéster.

8/9 forma liberável em sua extremidade proximal a um cateter de impulsão da forma descrita no pedido de patente co-pendente de Amplatz N° 11/121.386, depositado em 4 de maio de 2005, e intitulado System for the Controlled Delivery of Stents and Grafts. O stent seria então colocado dentro de um lúmen de um cateter de distribuição intravascular. O cateter de distribuição seria introduzido no paciente utilizando-se a técnica Seldinger bem-conhecida e então rosqueado através do sistema vascular até que uma extremidade distal do cateter de distribuição esteja perto de um aneurisma a ser tratado. Com o stent e o cateter de impulsão mantidos estacionários, o cateter de distribuição é colocado na direção proximal para ejetar o stent a partir da extremidade distal do cateter de distribuição onde o stent então se expande automaticamente para engatar a parede aórtica com a parte do comprimento L2 na figura 1 formando uma ponte para o aneurisma a ser tratado. A parte da estrutura tubular mais interna que se estende além da extremidade distal da estrutura tubular adicional pode se sobrepor a ostia das artérias renais 20 e 22. No entanto, devido à construção de tecedura aberta dessa parte da estrutura tubular interna, a mesma não impede substancialmente o fluxo de sangue através das artérias renais ou cria uma estenose. O comprimento adicional do stent/enxerto 10 fornecido pela extensão da estrutura tubular mais interna 23 além da extremidade distal das camadas adicionais 26, 30 serve para melhor estabilizar o stent/enxerto dentro da aorta abdominal, impedindo seu deslocamento antes da endotelização poder ocorrer.
[0025] Essa invenção foi descrita aqui com detalhes consideráveis a fim de se conformar aos estatutos de patente e para fornecer aos versados na técnica informação necessária para aplicação dos princípios de novidade e para construir e utilizar tais componentes especializados como necessário. Claims (6)
- REIVINDICAÇÕES1. Stent (10) para ser distribuído por cateter para o tratamento de um aneurisma aórtico abdominal, caracterizado pelo fato de compreender:a) uma estrutura tubular mais interna (23) de um primeiro comprimento (L1), a estrutura tubular mais interna (23) compreendendo uma primeira pluralidade de fios de arame trançados (24) de uma liga de efeito memória, a trança (24) possuindo uma trama predeterminada para definir aberturas suficientemente grandes de modo a não impedir materialmente o fluxo de sangue através das mesmas;b) pelo menos uma estrutura tubular adicional (26) de um diâmetro predeterminado localizado em uma relação circundante com uma parte de comprimento predeterminado (L2), inferior ao primeiro comprimento (L1), da estrutura tubular mais interna (23), a estrutura tubular adicional (26) compreendendo uma segunda pluralidade de fios de arame trançados (28) maior em número do que o da dita primeira pluralidade (24), e também de uma liga de efeito memória, a trança (28) da estrutura tubular adicional (26) possuindo uma trama que define aberturas suficientemente pequenas de forma a impedir efetivamente o fluxo de sangue entre as mesmas.c) onde o estiramento longitudinal das estruturas tubulares mais interna e adicional (23, 26) reduz o diâmetro externo do stent (10) o suficiente para permitir que o mesmo seja carregado para dentro de um lúmen de um cateter de distribuição intravascular, e a liberação do stent (10) do cateter de distribuição permite que o diâmetro externo expanda de volta para o dito diâmetro predeterminado.
- 2. Stent que pode ser distribuído por cateter, de acordo com a reivindicação 1, caracterizado pelo fato de que uma parte da estrutura tubular mais interna (23), a qual se estende além de umPetição 870170083747, de 31/10/2017, pág. 5/92/2 terminal da estrutura tubular adicional (26), é adaptada para se sobrepor à junção das artérias renais (20, 22) de um paciente com a aorta abdominal (12) do paciente quando a estrutura tubular adicional (26) atravessa um aneurisma aórtico abdominal.
- 3. Stent que pode ser distribuído por cateter, de acordo com a reivindicação 1, caracterizado pelo fato de que duas estruturas tubulares adicionais (26) são localizadas em uma relação circundante coaxial com a dita parte do comprimento predeterminado (L2).
- 4. Stent que pode ser distribuído por cateter, de acordo com a reivindicação 1, caracterizado pelo fato de que a liga de efeito memória compreende Nitinol.
