Abstract

유착은 일반적으로 수술 후, 염증, 창상 등이 회복되는 과정에서 발생하며, 상처 부위 주변 장기 및 조직이 들러붙는 현상을 말한다. 자세하게는 상처 치유 과정에서 염증 반응 및 응고 캐스캐이드(coagulation cascade)가 일어나 섬유소가 생성된다. 생성된 섬유소의 양이 과다하게 분비되면 섬유아세포(fibroblast)를 증가시키고 새로운 혈관이 생겨 조직 간 유착이 발생한다. 유착은 수술 후 환자의 복벽과 장 간에 60~93%의 높은 확률로 발생한다.
유착 현상은 심각한 문제를 초래한다. 복강 내 유착으로 인해 장의 일부가 조여질 때에는 장에 혈액 공급이 차단되어 허혈상태에 빠질 수 있으며, 신속한 수술적 치료가 이루어지지 않으면 장의 괴사까지 이어질 수 있다. 이 외에도 장기능 장애, 만성 통증, 불임 등의 문제를 유발할 수 있다. 이러한 유착을 방지하기 위해서 현재는 항염제를 사용하거나 물리적 장벽을 사용하고 있다.
기존에 발명된 유착방지제는 필름이나 직물 형태의 고분자 막 형태나 액제 또는 하이드로겔 형태로 판매되고있다. 막 형태의 유착방지제는 히알루론산과 카복시메틸셀룰로오스를 가교시킨 Genzyme사의 Seprafilm®, 산화 재생 셀룰로즈 (Oxidized Regenerated Cellulose, ORC)를 이용한 Johnson & Johnson Medical사의 Interceed®, poly-lactic acid를 사용한 Mast biosurgery사의 Surgiwrap®이 시중에 판매되고 있다. 이 제품들은 체내에 적용 시 장기 표면에 잘 부착되지 않는 점, 체액과 접촉 시 필름이 말리는 점, 건조상태에서 기계적 내구성이 미흡한 점, 부착 이후 장기의 운동으로 인해 적용한 부위에서 뭉치거나 이탈하는 점, 체내에서 생분해가 너무 늦어 제거수술을 해야 하는 점 등의 문제점들이 있다. 이러한 막 형태는 좁은 부위에 적용하기 어렵고 장기 표면에 잘 부착되지 않으며 장기의 움직임으로 인해 서로 뭉쳐지는 문제점 때문에 유착방지 효과가 미흡하거나 적용하기에 어려움이 있다. 액제 또는 하이드로겔 형태의 유착방지제는 물리적 장벽으로써의 기계적 내구성이 부족하고 상처 부위에 대한 부착력이 한계가 있다(도 1).
이와 관련하여 한국등록특허 제10-1062068호는 히알루론산 및, 하이드록시에틸스타치(Hydroxyethyl starch) 또는 키토올리고당 (Chitooligosaccharide)을 포함하는 조직 유착방지용 조성물로 상품명 Medicurtain®로 시판되고 있다. 그러나 해당 특허에서 사용되는 유착방지용 조성물은 겔 제형으로 상처부위에서의 점착력이 부족하여 국소적으로 사용하기에 어려움이 있다. 등록특허 제10-1062068호에서의 유착방지용 조성물의 사용량은 5ml로 국소적인 상처부위에 비해 매우 많은 양을 사용하여야 한다. 하지만 국소적인 상처부위에만 적용되는 양인 0.5ml를 사용할 경우 유착방지 효과가 부족하여 유착이 일어나는 것을 본 발명의 '대조예4' 에서 확인할 수 있다.
본 발명에서 사용되는 오일은 유착을 억제하는 성분으로 알려져 있지만 상처 부위에서 잘 머무를 수 없어 충분한 유착방지 효과를 나타낼 수 없다. 하지만 수용성 고분자와 오일을 적절하게 유화시키면 상처부위에서 오래 머물러 유착을 효과적으로 억제하게된다.
본 발명에서 사용된 친수성 고분자는 점도를 증가시키는 점증제의 일종으로 약제학적으로 허용가능한 부형제이다. 훌륭한 생체 적합성을 가지므로 시럽, 현탁액, 정제류 등을 포함한 제약 제품 등에 광범위하게 사용되고 있다.
이에, 본 발명에서는 기존에 발명된 여러 형태의 유착방지제 단점인 상처 부위에 대한 부착력과 기계적 내구성을 보완하면서 효과적인 유착방지제를 연구한 결과로, 친수성 고분자와 동ㆍ식물 또는 광물에서 유래된 오일을 사용한 신규한 제형의 유착방지제를 완성하였다(도 2).
본 발명의 목적은 수상 성분 및 동ㆍ식물 또는 광물에서 유래된 오일을 포함하는 유상 성분을 포함하며, 친수성 고분자, 계면활성제 및 보조계면활성제(보조용매)를 포함하는 조직 유착방지용 조성물, 바람직하게는 수상유형 유제 조성물을 제공하는데 있다. 본 발명은 친수성 고분자와 동ㆍ식물 또는 광물에서 유래된 오일을 포함하는 조성물을 통해 물리적 점탄성을 높여 수술 부위의 물리적 장벽을 효과적으로 형성해 상처가 치유되는 기간 동안 수술 부위에 충분히 잔존하여 유착을 방지하는데 목적이 있다. 본 발명은 유제 형태의 제형으로 특정 외과적 수술 기법에 제한되지 않고 다방면으로 사용될 수 있다.
본 발명에 따른 유착 방지용 조성물은, 친수성 고분자; 동ㆍ식물 또는 광물에서 유래된 오일; 계면활성제; 및 에탄올(ethanol), 글리세롤(glycerol), 폴리에틸렌글리콜(polyethyleneglycol), 프로필렌카보네이트(propylene carbonate), 프로필렌글리콜(propylene glycol), 디메틸폼아마이드(dimethylformamide, DMF), 디메틸설폭사이드(dimethyl sulfoxide, DMSO), 디메틸아민(dimethylamine, DMA), 트리아세틴(triacetin), N-메틸-2-피롤리돈(N-methyl-2-pyrrolidone, NMP), 디에틸렌 글리콜 모노에틸 에테르(diethylene glycol monoethyl ether), 폴리옥실글리세리드(polyoxylglyceride) 및 이들의 조합으로 구성되는 군으로부터 선택되는 보조계면활성제를 포함할 수 있다.
일 실시예에서, 동ㆍ식물 또는 광물에서 유래된 오일은 총 중량을 기준으로 1 내지 50 중량%의 범위를 가질 수 있다.
일 실시예에서, 친수성 고분자는 총 중량을 기준으로 0.1 내지 20 중량%의 범위를 가질 수 있다.

일 실시예에서, 동ㆍ식물 또는 광물에서 유래된 오일은 스쿠알렌(squalene), 라놀린(lanolin), 미네랄 오일(mineral oil), 파라핀(paraffin), 바셀린(vaseline), 아몬드유(almond oil), 옥수수유(corn oil), 면실유(cottonseed oil), 올리브유(olive oil), 낙화생유(peanut oil), 홍화씨유(safflower seed oil), 포도씨유(grapeseed oil), 참기름(sesame oil), 콩기름(soybean oil), 아르간오일(argan oil), 카놀라유(canola oil), 피마자유(castor oil), 코코넛유(coconut oil), 해바라기씨유(sunflower seed oil), 팜유(palm oil), 아보카드유(avocado oil) 및 이들의 조합으로 구성되는 군으로부터 선택될 수 있다.

본 발명에 따른 유제 형태의 유착방지용 조성물은 우수한 생체 적합성, 체내 지속성, 조직 부착성을 나타내고, 이를 통하여 궁극적으로 뛰어난 유착방지 효과를 나타낼 수 있다. 본 발명은 유제 형태의 제형으로 특정 외과적 수술 기법에 제한되지 않고 다방면으로 사용될 수 있다. 또한 하기의 실시예들은 본 발명의 예시로, 실시예로 본 발명이 한정되는 것은 아니다.

이하는 본 발명에 대하여 구체적으로 설명한다.
본 발명에서 용어, "유착(adhesion)"이란 서로 떨어져 있는 피부나 막 등이 서로 들러붙는 현상을 의미할 수 있다. 외과수술 후 혹은 염증, 이물, 출혈, 감염, 창상, 마찰, 화학적 치료 등으로 조직 손상이 발생하면 상처의 치유 과정에서 혈액이 유출되어 응고하고, 이에 의해 주변 조직과 비정상적 접합이 일어나는 유착 현상이 발생한다. 이러한 유착은 특히 수술 후에 빈번히 발생하며, 골반에 유착이 발생하는 경우에는 만성통증, 성기능 장애 등의 원인이 되기도 하고, 갑상선 제거수술 후 유착으로 흉부의 통증, 연하곤란 등의 부작용이 발생하며, 척추 수술에 의한 유착이 발생하면 신경이 압박되어 심한 통증을 가져오며, 자궁 내 유착은 불임, 무월경, 습관성 유산을 초래하는 것으로 알려져 있다.
본 발명의 유착방지용 조성물은 멸균 후에도 장기 조직에서 흘러내리지 않고 조직 표면에 부착할 수 있도록 적절한 농도의 친수성 고분자를 포함할 수 있다. 상기 친수성 고분자는 예를 들어 전분, 히알루론산, 소듐 알지네이트, 펙틴, 소듐 카복시메틸셀룰로오스, 메틸셀룰로오스, 하이드록시프로필메틸셀룰로오스, 저치환 하이드록시프로필셀룰로오스, 하이드록시프로필셀룰로오스, 덱스트린, 덱스트란, 풀루란, 폴리비닐피롤리돈, 폴리아크릴산, 폴리비닐알코올, 폴리에틸렌글리콜, 키토산, 젤라틴, 카라기난, 구아검, 로커스트 콩 검, 트라가칸타, 잔탄검, 젤란검, 아가로스 및 이들의 조합으로 구성되는 군으로부터 선택될 수 있지만 이에 제한되지 않는다. 상기 친수성 고분자들은 농도가 증가함에 따라 점도가 증가하는 친수성 고분자들이며, 바람직하게 친수성 고분자의 농도는 유착방지용 조성물의 총 중량을 기준으로 0.1 중량% 내지 20 중량% 함유 될 수 있다. 더 바람직하게는 친수성 고분자의 농도는 유착방지용 조성물의 총 중량을 기준으로 1 중량% 내지 10 중량% 함유 될 수 있다. 친수성 고분자가 0.1 중량% 미만인 경우 낮은 점도로 조직부착성이 떨어지며, 20 중량% 초과인 경우 너무 높은 점도로 인해 균일한 유착방지제의 제조가 불가능한 단점이 있다.
본 발명의 유착방지용 조성물은 유제 형성을 위해 적합한 계면활성제를 첨가할 수 있다. 계면활성제는 예를 들어 라우릴, 세틸, 스테아릴 및 올레일 알코올로부터 유래된 폴리옥시에틸렌 기반 지방 에터, 카프릴로 카프로일 마크로골 글리세라이드, 폴리에톡실화 피마자유, 에틸옥사이드/12-하이드록시 스테아르산, 자당지방산에스테르, 폴리옥시에틸렌 소르비탄 지방산 에스테르, 소르비탄 에스터, 폴리옥시에틸렌 및 폴리옥시프로필렌의 블록공중합체, 소듐 라우릴 설페이트, 소듐 올레에이트, 소듐 디옥틸 설포석시네이트, 4급 암모늄염, 이미다졸린염, α-토코페롤-폴리에틸렌 글리콜-1000-숙시네이트, 레시틴 및 이들의 조합으로 구성되는 군으로부터 선택될 수 있지만 이에 제한되지 않는다. 계면활성제는 유착방지용 조성물의 총 중량을 기준으로 0.1 중량% 내지 10 중량%을 포함할 수 있다. 바람직하게는, 계면활성제는 유착방지용 조성물의 총 중량을 기준으로 0.1 중량% 내지 3 중량%를 포함할 수 있다. 계면활성제가 0.1 중량% 미만으로 첨가될 경우 유제가 형성되지 않고, 10 중량%를 초과하면 유화과정에서 과다한 기포가 발생하여 제조하기에 적합하지 않다.
본 발명의 유착방지용 조성물은 제형 안정성을 위해 적절한 보조계면활성제(보조 용매)를 선택할 수 있다. 상기 보조계면활성제(보조 용매)는 예를 들어 에탄올, 글리세롤, 폴리에틸렌글리콜, 프로필렌카보네이트, 프로필렌글리콜, 디메틸폼아마이드, 디메틸설폭사이드, 디메틸아민, 트리아세틴, N-메틸-2-피롤리돈, 디에틸렌 글리콜 모노에틸 에테르, 폴리옥실글리세라이드 및 이들의 조합으로 구성되는 군으로부터 선택될 수 있지만 이에 제한되지 않는다. 보조계면활성제(보조용매)는 유착방지용 조성물의 총 중량을 기준으로 1 중량% 내지 50 중량%가 함유 될 수 있다. 바람직하게는, 보조계면활성제(보조용매)는 전체 유착방지용 조성물의 총 중량을 기준으로 15 중량% 내지 45 중량%가 함유될 수 있다. 보조계면활성제(보조용매)가 1 중량% 미만으로 함유할 경우 안정한 제형이 형성되지 않아 상분리가 일어나며, 50 중량%를 초과할 경우 조성물의 흐름성이 과도하게 증가하여 조직에 부착되기에 적합하지 않다.
본 발명의 유착방지제는 동ㆍ식물 또는 광물에서 유래된 오일을 사용할 수 있다. 상기 오일은 유상으로 체내에 투여하여 천천히 흡수되게 유지시키는 역할을 하며, 복강 내 상처부위에 적용 시 유착 발생을 억제시킨다. 유착방지효과가 있는 오일의 예로는 콩기름, 카놀라유, 아르간오일 등이 있다. 적절한 동ㆍ식물 또는 광물에서 유래된 오일은 예를 들어 스쿠알렌, 라놀린, 미네랄 오일, 파라핀, 바셀린, 세레신, 아몬드유, 옥수수유, 면실유, 올리브유, 낙화생유, 홍화씨유, 포도씨유, 참기름, 콩기름, 아르간오일, 카놀라유, 피마자유, 코코넛유, 해바라기씨유, 팜유, 아보카드유 및 이들의 조합으로 구성되는 군으로부터 선택될 수 있으며 이에 제한되지 않는다. 동ㆍ식물 또는 광물에서 유래된 오일은 유착방지용 조성물의 총 중량을 기준으로 1 중량% 내지 50 중량% 범위를 가질 수 있다. 바람직하게는, 동ㆍ식물 또는 광물에서 유래된 오일은 유착방지용 조성물의 총 중량을 기준으로 10 중량% 내지 50 중량% 범위를 가질 수 있다. 동ㆍ식물 또는 광물에서 유래된 오일이 1 중량% 미만인 경우 유제가 형성되지 않고, 50 중량%를 초과하면 제형적으로 안정하지 않아 상이 나뉘는 현상이 발생한다.
본 발명의 유착방지용 조성물은 완충용액을 포함할 수 있다. 상기 완충용액은 유착방지제의 pH가 급격히 변화하지 않게 유지시키는 역할을 하며, 완충용액의 바람직한 예로는 인산 완충용액, 구연산 완충용액, 히스티딘 완충용액, 트리스 완충용액 등이 있으며 이에 제한되지 않는다.
본 발명의 유착방지용 조성물은 등장화제를 포함할 수 있다. 상기 등장화제는 유착방지제를 체내에 투여할 때, 체내와 유착방지제의 삼투압에 차이를 감소시킨다. 따라서 체내에 투여하기에 적합한 유착방지제의 삼투압을 유지시키는 역할을 한다. 이러한 등장화제의 바람직한 예로는 다가알콜, 아미노산, 염화나트륨, 염화칼륨 등이 있으며 이에 제한되지 않는다.
본 발명의 유착방지용 조성물은 장기 조직에서 흘러내리지 않고 조직표면에 부착할 수 있는 적절한 점도를 가질 수 있다. 본 발명의 유착방지제는 전단율(share rate)이 1/s에서 점도가 바람직하게는 0 내지 300,000 mPaㆍs 일 수 있으며, 보다 바람직한 점도는 1,000 내지 250,000 mPaㆍs 일 수 있고, 보다 더 바람직한 점도는 10,000 내지 200,000 mPaㆍs 일 수 있다. 또한 본 발명의 유착방지제는 유사소성 흐름을 갖고 있기 때문에 상처부위에 적용할 때에 점도가 일시적으로 낮아져 상처부위에 적용이 간편하고 적용 후에는 다시 높은 점도로 복원되는 특징을 갖고 있다. 따라서, 본 발명의 유착방지제는 상처부위에서 이탈하지 않는 장점이 있다. 결과적으로, 본 발명에 따른 유착방지제는 우수한 유착방지 효과뿐만 아니라 숙련되지 않은 사용자도 쉽게 사용이 가능한 이점이 있다.
본 발명의 유착방지제가 적용 가능한 수술 내용은 한정되지 않는다. 본 발명의 유착방지제가 적용 가능한 수술은 생체 기관의 표면을 대상으로 한 수술일 수 있다. 본 발명의 유착방지제가 적용 가능한 부분은 수술의 직접적인 대상은 아니나 수술의 결과 상처가 형성되는 부분도 포함할 수 있다. 기관 표면에 유착방지제를 접촉시킬 때, 유착방지제의 형태나 유효성분의 비율은 기관, 조직의 종류, 외상 부분의 상태, 면적 등에 따라 적절히 선택될 수 있다. 유착방지제의 국소 기관 표면에의 접촉량은 국소 기관 표면을 덮을 정도일 수 있다.
본 발명의 유착방지제는 분말, 과립, 겔, 졸, 유제, 분산계, 크림, 용액 및 필름 제형을 포함하는 군으로부터 선택된 제형일 수 있으나, 이에 제한되는 것은 아니며, 의학적으로 적용되는 범위에 따라 다양화될 수 있다. 예컨대, 유제 형태의 유착방지제는 수상에 보조계면활성제와 친수성 고분자를 용해한 다음 계면활성제를 첨가하여 교반한 후 이의 혼합물에 동ㆍ식물 또는 광물에서 유래된 오일을 고압 균일 혼합함으로써 제조할 수 있다.
본 발명의 상기 유착방지 조성물은 유제 제형인 것이 바람직하다. 상기 유제 제형은 생체 적합성이 우수하여 다량의 물을 흡수한 상태에서는 생체의 조직과 유사한 거동이 나타나기 때문에 수술 기법에 관계없이 임상에서 널리 적용될 수 있다.
한편, 수술 후 발생하는 유착 방지를 목적으로 다양한 생체 유래 고분자와 비생체 유래 고분자를 조합한 유착방지제를 제조하려는 시도가 이루어지고 있다. 그러나, 유착방지 효과가 있다고 알려진 고분자를 혼합하여 유착방지제를 제조하는 경우 생체적합성이 뛰어난 반면 조직부착성, 체내 지속성 및 유착방지에 있어 기대만큼의 효과를 얻을 수 없거나, 어느 정도의 유착방지 효과는 나타내는 반면 생체적합성이 낮아 부작용을 일으킨다. 따라서, 적절한 생체 유래 고분자와 비생체 유래 고분자를 선택하여 우수한 생체적합성, 조직 부착성, 체내 지속성 및 특히 우수한 유착방지 효과 모두를 나타내는 유착방지제의 개발은 어려운 실정이다. 이러한 상황에서, 본 발명자들은 뛰어난 생체적합성, 조직 부착성, 체내 지속성뿐만 아니라, 이를 통하여 궁극적으로 우수한 유착방지 효과를 나타내는 물질의 조합을 찾기 위해 노력한 결과, 친수성 고분자, 동ㆍ식물 또는 광물에서 유래된 오일을 포함하는 유착방지제를 개발하였다.
구체적으로, 본 발명의 실시예 1은 소듐 카복시메틸셀룰로오스, 글리세롤, 폴리옥시에틸렌 소르비탄 모노팔미테이트(polyoxyethylene sorbitan monopalmitate) 및 카놀라유를 혼합한 조성물이며 시중에 판매 중인 유착방지제 (대조예2, 대조예3, 대조예4)과 비교하여 유착방지 효과를 확인하였다. 부검 시 육안소견 및 조직학적 관찰에 의한 유착방지 동물실험 결과로, 본 발명의 유착방지제는 시판 중인 유착방지제에 비해 유착 강도 및 유착 면적을 바탕으로 측정한 유착 정도에 있어 우수한 유착방지 효과를 나타내었다(도4). 따라서, 본 발명의 유착방지제는 수술 후의 유착방지를 위한 목적으로 유용하게 사용할 수 있다.
비교예 1
물(5150mg)에 소듐 카복시메틸셀룰로오스(600mg)을 넣고 60℃에서 3시간 동안 교반하여 용해했고, 폴리옥시에틸렌 소르비탄 모노팔미테이트(250mg)를 첨가 후 2시간 동안 교반하여 균일한 용액을 제조했다. 그 용액에 카놀라유(4000mg)을 첨가하여 고압 균질기로 13500 rpm, 60℃ 조건으로 15분 동안 균질화하여 조성물을 제조하였다.

물(1700mg)과 글리세롤(3450mg)을 혼합한 후, 아가로스(600mg)를 넣고 60℃에서 3시간 동안 교반하여 용해했다. 혼합한 용액에 폴리옥시에틸렌 소르비탄 모노팔미테이트(250mg)를 첨가 후 2시간 동안 교반하여 균일한 용액을 제조했다. 그의 용액에 카놀라유(4000mg)를 첨가하여 고압 균질기로 13500 rpm, 60℃ 조건으로 15분 동안 균질화하여 조성물을 제조하였다.

대조예 1: 생리식염수
마찰 손상 부위에 생리 식염수를 처리한 음성 대조군을 설정하였다.
대조예 2: 한미약품의 Guardix®-sol
히알루론산과 카복시메틸셀룰로오스로 구성된 유착 방지제로 시중에 판매되고 있는 제품(Guardix-sol,한미약품)을 사용하였다.
대조예 3: 한미약품의 Guardix®-SG
알지네이트와 염화칼슘 그리고 온도 감응성 고분자인 폴록사머를 사용한 유착방지제로 저온에서는 졸 상태였다가 실온 이상에서는 겔 상태가 되는 특징을 가진 시판 중인 유착방지제(Guardix®-SG)를 사용하였다.
대조예 4: 신풍제약의 메디커튼®
히알루론산과 하이드록시에틸스타치로 이루어진 유착방지제로 항염증성 및 항혈액응고능을 가진 하이드록시에틸스타치를 사용한 것이 특징이다. 시판 중인 신풍제약의 메디커튼®을 사용하였다.
실험예 1: 제형 안정성 시험
본 발명의 상기 비교예 1, 비교예 2, 실시예 1 내지 5를 제조하여 제형 안정성을 시험하였으며 방법은 다음과 같다. 제조한 제형을 에펜돌프 튜브(Eppendorf tube)(1.5mL)에 1g 정량하여 넣었다. 제형이 담긴 튜브를 -20℃에서 20시간 동안 냉동시키고, 60℃에서 4시간 동안 보관하여 해동했다. 냉동(-20℃)과 해동(60℃)과정을 3회 반복한 후 원심분리기로 5000 rpm의 속도로 15분 동안 원심분리했다. 원심분리한 해당 제형이 안정한지 육안으로 판단했다.
본 시험의 결과는 도 3에 도시된다. 비교예 1, 비교예 2는 상분리가 일어났으며 실시예 1 내지 5는 모두 제형적으로 안정한 것을 확인할 수 있다. 결과적으로, 비교예 1과 같이 보조계면활성제를 사용하지 않는 경우나 비교예 2와 같이 오일을 과량 포함하는 경우에는 제형적으로 안정한 상이 형성되지 않는 것을 확인할 수 있다.
실험예 2 유착방지제의 유착방지 효능 동물 실험 - 부검 시 육안 소견
부검 시 육안소견의 관찰을 통하여 유착방지 효능을 알아보기 위한 실험을 실시하였다. 마취 후 수술 부위에 제모를 실시하고, 포비돈으로 소독한 다음 복강의 정중선을 따라 4 ~ 5 cm 정도 절개하였다. 복강막에 1.5 × 1.5 cm 크기로 사포를 이용하여 찰과상을 내어 복벽과 조직의 유착을 유도하였다. 여기에 각각의 시험물질을 0.5mL 주입하였다. 시험종료일까지 생존한 동물에 대하여 CO2 가스 흡입법을 사용하여 안락사 시킨 후 부검하여 육안적 병리검사를 실시하고 개체별로 유착세기 및 유착면적을 토대로 유착정도를 평가하였다. 음성 대조군은 유착을 유도한 부위에 생리식염수를 처리한 대조예 1로 설정하였고, 양성 대조군은 유착을 유도한 부위에 시판 중인 유착방지제를 처리한 대조예 2 내지 4로 설정하였으며, 본 발명의 예시인 실시예 1과 실시예 6 내지 21에 대한 유착방지 효능을 평가하였다. 육안소견의 관찰에 의한 유착의 평가는 Ntourakis, Dimitrios 등 (Ntourakis, Dimitrios, Michail Katsimpoulas, Anna Tanoglidi, Calypso Barbatis, Panayotis E Karayannacos, Theodoros N Sergentanis, Nikolaos Kostomitsopoulos, and Anastasios Machairas. "Adhesions and Healing of Intestinal Anastomoses: The Effect of Anti-Adhesion Barriers." Surgical Innovation 23, no. 3 (2016): 266-76.))의 방법에 따라 실시하였다. 구체적으로, 유착면의 유착 크기, 두께, 유착을 분리하는데 필요한 힘에 따른 분류(0~4, 표 24)로 유착 정도를 평가하고, 그 결과를 표 25에 나타내었다.

도 4에 나타난 바와 같이, 카복시메틸셀룰로오스 소듐, 글리세롤 및 카놀라유를 혼합한 본 발명의 유착방지제(실시예 1)가 생리식염수를 처리한 군(대조예 1)이나 시판 중인 한미약품의 Guardix®-sol(대조예 2), 한미약품의 Guardix®-SG(대조예 3), 신풍제약의 메디커튼®(대조예 4)에 비하여 유착 정도, 유착 강도 및 유착되는 면적 등에 있어서 보다 효과적으로 유착을 방지할 수 있음을 확인하였다(도 4). 본 발명의 실시예 6 내지 11을 통해 유상의 종류에 따른 유착 방지 효과를 확인하였다. 또한 본 발명의 실시예 12 내지 21이 뛰어난 유착 방지 효과가 있음을 확인하였다(도 5).

Claims (7)
- 유착방지용 조성물로서,친수성 고분자;동ㆍ식물 또는 광물에서 유래된 오일;계면활성제; 및에탄올, 글리세롤, 폴리에틸렌글리콜, 프로필렌카보네이트, 프로필렌글리콜, 디메틸폼아마이드, 디메틸설폭사이드, 디메틸아민, 트리아세틴, N-메틸-2-피롤리돈, 디에틸렌 글리콜 모노에틸 에테르, 폴리옥실글리세라이드 및 이들의 조합으로 구성되는 군으로부터 선택되는 보조계면활성제를 포함하며,상기 보조계면활성제는 총 중량을 기준으로 15 내지 45 중량%의 범위를 가지며,상기 동ㆍ식물 또는 광물에서 유래된 오일은 총 중량을 기준으로 10 내지 50 중량%의 범위를 갖는,유착 방지용 조성물.
- 제1항에 있어서,상기 친수성 고분자는 총 중량을 기준으로 0.1 내지 20 중량%의 범위를 갖는,유착 방지용 조성물.
- 제1항에 있어서,상기 계면활성제는 총 중량을 기준으로 0.1 내지 10 중량%의 범위를 갖는,유착 방지용 조성물.
- 제1항에 있어서,상기 유착 방지용 조성물의 점도는 0 내지 300000 mPaㆍs인,유착 방지용 조성물.
- 제1항에 있어서,상기 친수성 고분자는 전분, 히알루론산, 소듐 알지네이트, 펙틴, 카복시메틸셀룰로오스 소듐, 메틸셀룰로오스, 하이드록시프로필메틸셀룰로오스, 저치환 하이드록시프로필셀룰로오스, 하이드록시프로필셀룰로오스, 덱스트란, 풀루란, 폴리비닐피롤리돈, 폴리아크릴산, 폴리비닐알코올, 폴리에틸렌글리콜, 키토산, 젤라틴, 카라기난, 구아검, 로커스트 콩 검, 트라가칸타, 잔탄검, 젤란검, 아가로스 및 이들의 조합으로 구성되는 군으로부터 선택되는,유착 방지용 조성물.
- 제1항에 있어서,상기 동ㆍ식물 또는 광물에서 유래된 오일은 스쿠알렌, 라놀린, 미네랄 오일, 파라핀, 바셀린, 아몬드유, 옥수수유, 면실유, 올리브유, 낙화생유, 홍화씨유, 포도씨유, 참기름, 콩기름, 아르간오일, 카놀라유, 피마자유, 코코넛유, 해바라기씨유, 팜유, 아보카드유 및 이들의 조합으로 구성되는 군으로부터 선택되는,유착 방지용 조성물.
- 제1항에 있어서,상기 계면활성제는 라우릴, 세틸, 스테아릴 및 올레일 알코올로부터 유래된 폴리옥시에틸렌 기반 지방 에터, 카프릴로 카프로일 마크로골 글리세라이드, 폴리에톡실화 피마자유, 에틸옥사이드/12-하이드록시 스테아르산, 자당지방산에스테르, 폴리옥시에틸렌 소르비탄 지방산 에스테르, 소르비탄 에스터, 폴리옥시에틸렌 및 폴리옥시프로필렌의 블록공중합체, 소듐 라우릴 설페이트, 소듐 올레에이트, 소듐 디옥틸 설포석시네이트, 4급 암모늄염, 이미다졸린염, α-토코페롤-폴리에틸렌 글리콜-1000-숙시네이트, 레시틴 및 이들의 조합으로 구성되는 군으로부터 선택되는,유착 방지용 조성물.
