本発明は、筋ジストロフィー治療剤、中心静脈栄養用組成物、筋組織の炎症抑制剤および筋ジストロフィーの抗炎症用食品組成物に関する。
筋の再生および肥大化は、負荷などの刺激により筋細胞の基底膜と形質膜との間に局在する筋衛星細胞が活性化および増殖して開始する。成熟した筋組織には骨格筋細胞の前駆細胞である筋衛星細胞が存在し、活性化により細胞周期に入り、増殖し、筋芽細胞に分化する。筋疾患患者では筋の再生能力が低下し、または筋の再生能力があっても筋線維が容易に崩壊し、筋萎縮を招く場合が多い。このような筋原性疾患として、筋ジストロフィーがある。
筋ジストロフィーは、筋線維の破壊と変性(筋壊死)と再生とを繰り返しながら、次第に筋萎縮と筋力低下が進行していく遺伝性筋疾患である。筋ジストロフィーの主な症状は運動機能の低下であって、呼吸機能障害、嚥下機能障害などの機能障害を併発する。
筋ジストロフィーの治療薬として、桔梗抽出物を有効成分として単独で含む筋肉疾患の予防又は治療用薬学組成物がある(特許文献1)。桔梗抽出物が筋タンパク質の合成及び筋肉細胞の分化を介して筋肉の形成に関与し、筋量を増加させるという。また、桑科植物由来成分を有効成分として含む筋肉疾患予防又は治療用薬学組成物(特許文献2)、および副甲状腺ホルモン(PTH)またはPTH誘導体の1種または2種以上、あるいは141個のアミノ酸からなりPTH様作用により高カルシウム血症をもたらすタンパク質である副甲状腺ホルモン関連タンパク質を有効成分として含有する筋疾患治療剤(特許文献3)がある。
一方、筋ジストロフィーでは経時的に運動機能が低下するため、日常の食生活の管理も重要である。例えばデュシェンヌ型筋ジストロフィーの場合、多種の食材をバランス良く摂取することが重要で、定期的に身長、体重およびBMIを測定することで栄養管理の目安とすることができる。タンパク質、カルシウム、ビタミンDおよび鉄の摂取が特に重要で、デュシェンヌ型筋ジストロフィーの患者はエネルギー摂取量が不足してタンパク質不足を起こしやすく、低アミノ酸血症を呈することがあるという。なお、筋肉増強を目的として、グルコシルセラミドを有効成分とする筋分化誘導促進剤(特許文献4)がある。また、三大栄養素の一つである脂質に関し、中鎖脂肪酸を含む総ケトン体濃度上昇剤(特許文献5)、およびがん細胞がブドウ糖を主な栄養源とするため糖質を制限したケトン食をがん患者に投与する試みもある(非特許文献1)。

筋ジストロフィーなどの慢性疾患の進行には、長期にわたる過度の筋組織の炎症がその一因となることが知られている。筋ジストロフィーは遺伝性疾患であり、筋破壊が筋衛星細胞の活性化および増殖より勝って筋萎縮や筋力低下が進行する。例えば、筋芽細胞が増殖しても、そこに損傷のシグナルが入ると全ての細胞が静止期に戻らず分化し、筋の幹細胞である筋衛星細胞の貯蔵量が減少する。このため、増殖促進とは別に筋衛星細胞の静止状態の維持が筋の幹細胞のプールの維持に重要である。筋線維は生体内で摂取した栄養成分によって形成されるため、日々の食事として摂食した成分によって炎症を抑制し、筋線維の再生および肥大化が促進されることが好ましい。したがって、筋ジストロフィー罹患者の炎症を抑制し、筋線維量を増加し、食事やその補充として使用しうる筋ジストロフィー治療剤の開発が望まれる。
筋ジストロフィーでは筋力低下により咀嚼能力および嚥下能力が低下しやすく、流動食の摂取が必要となる。誤嚥リスクが高い場合は経管栄養および胃瘻造設の他、中心静脈栄養などによる栄養摂取が必要となる。したがって、経管栄養または中心静脈栄養としても使用できる筋ジストロフィー治療剤の開発が望まれる。
本発明は、上記実情に鑑みてなされたものであり、筋ジストロフィー治療剤および筋組織の炎症抑制剤を提供することを目的とする。
また、上記筋ジストロフィー治療剤を含む中心静脈栄養用組成物および上記筋組織の炎症抑制剤を含む筋ジストロフィーの抗炎症用食品組成物を提供することを目的とする。
本発明者等は、従来癌患者およびアルツハイマー病患者に使用されていた中鎖脂肪酸トリグリセリド(以下、MCTと称する。)をジストロフィン遺伝子変異ラットに与えたところ、マクロファージの浸潤量で評価される筋組織の炎症細胞浸潤を改善することができ、クレアチンキナーゼ値(CK値)、前脛骨筋重量、筋線維径および筋力を増加させうることを見出し、本発明を完成させた。
すなわち本発明の第1の観点に係る筋ジストロフィー治療剤は、MCTを有効成分とする。

本発明によれば、MCTを含む、筋ジストロフィー治療剤および筋組織の炎症抑制剤が提供される。また、上記筋ジストロフィー治療剤を含む中心静脈栄養用組成物および上記筋組織の炎症抑制剤を含む筋ジストロフィーの抗炎症用食品組成物が提供される。
本実施の形態に係る筋ジストロフィー治療剤は、MCTを有効成分とする。中鎖脂肪酸とは炭素数8~12の脂肪酸である。本実施の形態におけるMCTとしては、グリセロールにエステル結合する脂肪酸として、MCTを構成する全脂肪酸に対する炭素数8~12の脂肪酸の割合が60~100モル%、より好ましくは70~100モル%のものが挙げられる。より好ましくは、MCTを構成する脂肪酸が、炭素数8のカプリル酸および炭素数10のカプリン酸を50モル%以上、好ましくは60モル%以上、より好ましくは70モル%以上含有する。MCTとしては動植物からの抽出物であってもよい。本実施の形態では、MCTとしてココナツオイルを使用することができる。
脂肪酸の代謝産物であるケトン体は、グルコースに代わるエネルギー源となる。長鎖脂肪酸トリグリセライド由来の遊離長鎖脂肪酸はカイロミクロンを経てリンパ管から大循環系に入り脂肪組織などに貯蔵され、グリコーゲン枯渇時などに分解され消費されるが、遊離中鎖脂肪酸はカイロミクロンを作らずに門脈に入り肝臓へ運ばれ、速やかにエネルギー源となって代謝される。このため中鎖脂肪酸は、糖質摂取が少ない条件において肝臓でケトン体を速やかに産生することができる。このため、前記非特許文献1に示すように、ブドウ糖を主な栄養源とする癌細胞に糖質を制限したケトン食を与える試みがなされている。しかしながら、筋ジストロフィーでは筋細胞膜の裏打ちをする細胞骨格タンパクが欠損しているため構造的に筋肉が壊れやすく、壊死と再生を繰り返した結果、筋量および筋力が低下する。癌における兵糧攻め的な効果と相違し、筋ジストロフィーは構造タンパク質の欠如による筋量の低下を生じるため、ケトン食を投与した場合でも作用メカニズムが異なる。しかしながら驚いたことに、筋ジストロフィー発症モデルラットに本実施の形態に係る筋ジストロフィー治療剤を摂取させると、後記する実施例に示すように、対照群より血清CK値上昇が抑制され、筋組織におけるマクロファージの浸潤量が低減して炎症細胞浸潤量が低下した。筋力および筋線維径を測定したところ、筋力および筋線維径も増加していた。本実施の形態に係る筋ジストロフィー治療剤は、投与対象の筋組織の炎症を抑制し、炎症による筋線維の崩壊を抑制し、かつ筋線維の再生を促進して筋ジストロフィーにおける筋力低下を抑制することができる。
したがって、本実施の形態に係る筋ジストロフィー治療剤は、「MCTを有効成分とする筋組織の炎症抑制剤」、「筋組織の炎症抑制剤を含む、筋ジストロフィーの抗炎症用食品組成物」ということができる。なお、筋組織としては、骨格筋、平滑筋および心筋のいずれでもよいが、特に運動障害の生じやすい骨格筋に有効である。
本実施の形態に係る筋ジストロフィー治療剤は、有効成分であるMCTに賦形剤を加えて粉末状、顆粒状、ペースト状、その他に加工して使用することができる。このような賦形剤としては、薬学的に許容されるものであれば特に限定されない。
本実施の形態に係る筋ジストロフィー治療剤および筋組織の炎症抑制剤は、タンパク質および糖質をさらに含んでもよい。これにより、筋ジストロフィー治療剤を栄養補充用食品または食事代替用食品として使用することができる。配合割合は、MCT、タンパク質および糖質の合計量に対してMCTが40~75質量%、より好ましくは42~75質量%、特に好ましくは45~75質量%であって、糖質が5~40質量%、好ましくは7~30質量%、特に好ましくは10~20質量%であって、タンパク質が5~40質量%、好ましくは7~30質量%、特に好ましくは10~30質量%である。MCTの配合割合を上記範囲とすることで、筋線維の再生を促進して筋ジストロフィーにおける筋力低下を抑制することができる。
配合するタンパク質としては、特に制限されないが、例えば、コーングルテン、小麦グルテン、大豆タンパク質、小麦タンパク質、ホエイタンパク質およびカゼインなどの乳タンパク質、食肉または魚肉から得られる動物性タンパク質、卵白、ならびに卵黄などがある。
糖質としては、単糖類、二糖類および多糖類を含む。単糖類としては、グルコース、フルクトースおよびガラクトース等が挙げられる。二糖類としては、マルトース、スクロースおよびラクトース等が挙げられる。多糖類としては、アミロース、アミロペクチン、グリコーゲンおよびデキストリン等が挙げられる。これらは食事により摂取されると数時間で代謝され、グルコースおよびエネルギーを発生しうる糖類である。なお、ペクチン、βグルカン、フルクタン、イヌリン、アガロース、アルギン酸ナトリウム、カラギーナン、フコイダン、セルロース、キチン、キトサンおよびエリスリトールなどの食物繊維等は、炭水化物であるが速やかにグルコースおよびエネルギーに代謝されない。このため、本実施の形態に係る筋ジストロフィー治療剤の糖質の配合量、後記するケトン比の算出などには、ペクチン等の食物線維は「糖質」に含めないものとする。
筋ジストロフィー治療剤には、有効性を維持する範囲で、MCT以外の油脂、水、ビタミン類、ミネラル類、有機酸、有機塩基、果汁、フレーバー、機能性成分および食品添加物など、通常の食品に含まれる成分を添加することができる。MCT以外の脂質としては、例えば、ラード、魚油等、これらの分別油、水素添加油およびエステル交換油等の動物性油脂;パーム油、サフラワー油、コーン油、ナタネ油、ヤシ油、これらの分別油、水素添加油およびエステル交換油等の植物性油脂などがある。ビタミン類としては、ビタミンA、カロテン類、ビタミンB群、ビタミンC、ビタミンD群、ビタミンE、ビタミンK群、ビタミンP、ビタミンQ、ナイアシン、ニコチン酸、パントテン酸、ビオチン、イノシトール、コリンおよび葉酸などが挙げられる。ミネラル類としては、例えば、カルシウム、カリウム、マグネシウム、ナトリウム、銅、鉄、マンガン、亜鉛およびセレンなどが挙げられる。有機酸としては、例えば、リンゴ酸、クエン酸、乳酸および酒石酸などが挙げられる。機能性成分として、例えばオリゴ糖、グルコサミン、コラーゲン、セラミド、ローヤルゼリーおよびポリフェノールなどが挙げられる。更に、乳化剤、安定剤、増粘剤、ゲル化剤、甘味剤、酸味料、保存料、抗酸化剤、pH調整剤、着色剤および香料などの食品添加物が筋ジストロフィー治療剤に配合されてもよい。
筋ジストロフィー治療剤が、MCTを少なくとも一部とする脂質、タンパク質および糖質を含む場合は、糖質及びタンパク質の合計質量に対する脂質の割合で定義されるケトン比が0.7~4、好ましくは0.7~2.9、より好ましくは0.8~2.9となるように各成分を配合することが好ましい。例えば、MCT40質量部にMCT以外の脂質20質量部、タンパク質と糖質との合計が60質量部の場合のケトン比は1であり、MCT75質量部にMCT以外の脂質25質量部、タンパク質と糖質との合計が25質量部の場合のケトン比は4となる。
ケトン比が上記範囲であれば、後記する実施例に示すように、筋ジストロフィー罹患者の食後のケトン体の血中濃度を0.8~2mmol/Lに維持することができる。その結果、筋組織におけるマクロファージの浸潤量が低減して炎症細胞浸潤量を低下させ、筋力および筋線維径を増加させることができる。また、同時に筋ジストロフィー発症モデルラットの血清CK値を対照群の1/2以下に抑制することができる。しかも、尿素窒素値は対照群と差がなく、筋ジストロフィー発症モデルラットにおけるタンパク質不足は生じさせない。このような血清CK値上昇抑制効果は、筋壊死抑制によると考えられ、この効果はケトン比を上記範囲に設定することで達成される。
本実施の形態に係る筋ジストロフィー治療剤によれば、筋ジストロフィー罹患者に、上記ケトン比が、0.7~4、好ましくは0.7~2.9、より好ましくは0.8~2.9となるように投与することができる。また、筋ジストロフィー罹患者が筋ジストロフィー治療剤以外の食物等を摂取する場合は、上記ケトン比になるように本実施の形態に係る筋ジストロフィー治療剤の組成を調整し、使用することができる。本実施の形態に係る筋ジストロフィー治療剤は、筋ジストロフィー罹患者の栄養補充用食品または食事代替用食品として使用することができる。
本実施の形態に係る筋ジストロフィー治療剤の投与量は、筋ジストロフィー罹患者の1日の必要カロリーの50~100%となるように投与することが好ましく、より好ましくは70~100%、特に好ましくは80~100%となるように投与される。これにより血中ケトン体濃度を向上させることができ、筋ジストロフィー罹患者の筋組織の抗炎症効果、筋力増加等の効果を得ることができる。
本実施の形態に係る筋ジストロフィー治療剤は経口投与であってよく、経管投与であってもよい。また、投与回数に制限はなく、筋ジストロフィー罹患者の消化吸収能力、嚥下能力、食欲、耐糖能および肝機能に応じて適宜選択することができる。
本実施の形態に係る筋ジストロフィー治療剤の形態としては、特に制限されず、ペースト剤、ゲル状剤、散剤、液剤、懸濁剤、乳剤、顆粒剤、錠剤、丸剤およびカプセル剤等であってもよい。筋ジストロフィー罹患者は嚥下能力が低下しやすい。このような嚥下能力、投与形態その他に応じて、好ましい剤型を選択することができる。また、これらの剤型は、常法により調製することができる。
例えば、本実施の形態に係る筋ジストロフィー治療剤を食事に混合して摂取できるように、ペースト剤、ゲル状剤、散剤、液剤、懸濁剤、乳剤および顆粒剤などとしてもよい。また、筋ジストロフィー治療剤に他の配合物を加えて、マヨネーズ、ドレッシングおよびカレーペーストなどの調味料として調製して、投与してもよい。
特に筋ジストロフィーの進行に伴い嚥下機能が低下した場合には、経鼻胃管、胃瘻、腸瘻栄養法および中心静脈栄養などの経管投与が適用できる。経管投与法に応じて添加する栄養成分、流動性、pH、浸透圧および添加する乳化剤などを調整することができる。本実施の形態に係る筋ジストロフィー治療剤は、経管栄養組成物、特に中心静脈から投与する中心静脈栄養用組成物として好適に使用することができる。
筋ジストロフィーとしては、デュシェンヌ型筋ジストロフィー、ベッカー型筋ジストロフィー、肢帯型筋ジストロフィー、顔面肩甲上腕型筋ジストロフィー、先天性筋ジストロフィー、筋強直性ジストロフィー、遠位性ミオパチー、眼咽頭型筋ジストロフィーおよびエメリー・ドライフス型筋ジストロフィーなどが知られている。いずれも骨格筋の変性および壊死を主病変とし、臨床的には進行性の筋力低下をみる点で共通する。したがって、いずれの筋ジストロフィーであっても本実施の形態に係る筋ジストロフィー治療剤の投与対象とすることができる。
別の実施の形態では、MCTまたは上述の筋ジストロフィー治療剤を対象、特には筋ジストロフィー患者に投与することを含む、筋ジストロフィーの治療方法が提供される。別の実施の形態では、MCTまたは上述の筋組織の炎症抑制剤を対象に投与することを含む、筋組織の炎症抑制方法が提供される。別の実施の形態では、筋ジストロフィーまたは筋組織の炎症を治療するための医薬の製造のためのMCTの使用が提供される。別の実施の形態では、筋ジストロフィーまたは筋組織の炎症の治療のためのMCTの使用が提供される。
以下、実施例により本発明をさらに具体的に説明する。但し、本発明はこれらに限定されるものではない。
(製造例1)
以下に従い、各配合飼料を調製した。
リサーチダイエット社から購入したペースト飼料に、MCTパウダー(Quest Nutrition社製)を重量比で1.215倍、スクロース(リサーチダイエット社製)を0.0433倍、コーンスターチ(リサーチダイエット社製)を0.3466倍、カゼイン(オリエンタル酵母社製)を0.267倍混合し、ペレット化して下記表1に示す組成の配合飼料1を調製した。また、この方法に準じて配合飼料2を調製した。なお、飼料3は、リサーチダイエット社製のラット用普通食であり、配合飼料AIN-93Mである。飼料の組成、ケトン比=脂質/(糖質+タンパク質)で定義されるケトン比、およびカロリーを併せて表1に示す。また、配合飼料に含まれる各栄養素のカロリーを表2に示す。

試験例1
<ジストロフィン遺伝子変異ラット(筋ジストロフィーモデルラット)の飼育>
東京大学大学院農学生命科学研究科獣医生理学研究室にて開発された、Wistar-Imamichi系統、ジストロフィン遺伝子の遺伝子改変(PMID: 25005781)によりデュシェンヌ型筋ジストロフィーを発症するラット(3週齢)を用いた。離乳後にラットを被験群と対照群の2群にわけ、対照群6匹には表1に記載する飼料3を12週齢まで摂食させ、被験群6匹には表1に記載する飼料2を10日間与え、その後は表1に示す飼料1を12週齢まで与えた。給餌の際はペアフィーディングを行い、各群の摂取量を揃えた。なお、飼料2から飼料1への切り替えは、てんかん治療の際に予め高ケトン比を摂取させ、その後、ケトン比を低減して摂取させる態様にならったものである。

<体重およびBUN(尿素窒素)の血中濃度の推移>
被験群および対照群の3週齢から12週齢の体重の変化を図1に示す。被験群と対照群とでほぼ同等の体重量の増加が確認された。

また、飼料2を10日間与えた後にテールカット法で血液を採取し、富士ドライケムスライド尿素窒素キットを用いて、血清BUNを測定した。被験群および対照群の結果を図2に示す。なお、データはn=4の結果である。t-検定による統計的有意差をもって飼料2投与によるBUN値の増加が確認された。BUNの窒素は、タンパク質を構成するアミノ酸の最終代謝産物であり、タンパク質の摂取状況をダイレクトに反映する。表1に示すように、飼料2と飼料3のタンパク質量はほぼ同等であるが、BUN値は被験群が対照群より高く、飼料2の摂取によるタンパク質不足は生じていないことが確認された。
<血中グルコース・ケトン体濃度の測定>
各群の9週間の給餌後、24時間絶食した各個体に、被験群には飼料2を、対照群には飼料3を1時間摂餌させた。飼料摂取前と飼料摂取直後に、テールカット法により数滴の血液を採取し、precision xceed(プレシジョンエクシード)を用いて血中グルコースおよびケトン体濃度を測定した。結果を図3および図4に示す(図中、***はp<0.001、被験群n=6、対照群n=7)。なお、飼育開始10日後(被験群では飼料2を10日間給餌した後)の対照群と被験群の血中ケトン体は、それぞれ0.7mM、3.5mMであり、飼育17日目(被験群では飼料2を10日間給餌した後に資料を1週間給餌した後)の対照群と被験群の血中ケトン体は、それぞれ0.73mM、3.85mMであった。飼料2の10日間の投与により血清ケトン体濃度は対照群の5倍に増加し、かつケトン比1.5の資料に切り替えた後も血中ケトン体濃度を高く維持されていた。
各群の9週間の給餌後、24時間絶食した各個体に、被験群には飼料2を、対照群には飼料3を1時間摂餌させた。飼料摂取前と飼料摂取直後に、テールカット法により数滴の血液を採取し、precision xceed(プレシジョンエクシード)を用いて血中グルコースおよびケトン体濃度を測定した。結果を図3および図4に示す(図中、***はp<0.001、被験群n=6、対照群n=7)。なお、飼育開始10日後(被験群では飼料2を10日間給餌した後)の対照群と被験群の血中ケトン体は、それぞれ0.7mM、3.5mMであり、飼育17日目(被験群では飼料2を10日間給餌した後に資料を1週間給餌した後)の対照群と被験群の血中ケトン体は、それぞれ0.73mM、3.85mMであった。飼料2の10日間の投与により血清ケトン体濃度は対照群の5倍に増加し、かつケトン比1.5の資料に切り替えた後も血中ケトン体濃度を高く維持されていた。
結果:表2に示すように、飼料1、飼料2、飼料3は、いずれもタンパク質の含有量がほぼ同等であり、主に炭水化物を脂質に置き換えることでケトン比が調整されている。デュシェンヌ型筋ジストロフィー発症ラットに9週間給餌すると、被験群では飼料摂取直後の血糖値が対照群と比較して統計的有意差をもって低減し、血中ケトン値は統計的有意差をもって増加した。また、対照群では食後血糖値が食前血糖値の1.7倍上昇したが、被験群では食前血糖値と食後血糖値とがほぼ同等であり、食事による血糖値の上昇が抑制されていた。このことから、被験群では、食後高血糖によるインスリンの過度な分泌がなく、筋組織の代謝が解糖系からケトン体・脂肪の代謝へと切り替えられた(そのため炎症が起こりにくい)と推察される。
<体重、脂肪重量、前脛骨筋重量、ヒラメ筋重量の測定>
被験群と対照群について、体重、前脛骨筋重量、ヒラメ筋重量、および脂肪重量を測定した。結果を図5~図8に示す(図中、被験群n=6、対照群n=7、*はp<0.05)。なお、脂肪重量は、精巣上体脂肪をサンプリングし、その重量を脂肪重量とした。
被験群と対照群について、体重、前脛骨筋重量、ヒラメ筋重量、および脂肪重量を測定した。結果を図5~図8に示す(図中、被験群n=6、対照群n=7、*はp<0.05)。なお、脂肪重量は、精巣上体脂肪をサンプリングし、その重量を脂肪重量とした。
結果:デュシェンヌ型筋ジストロフィー発症ラットに9週間給餌したところ、体重差は、対照群と被験群との間では観察されなかった。一方、被験群では脂肪の重量が有意に少ない。被験群では、ケトン体・脂肪を消費してエネルギーを得る代謝系に変換され、脂肪の燃焼が促進された結果と思われる。同様に、被験群では、前脛骨筋およびヒラメ筋の体重比における重量が対照群よりも高く、脂質が代謝されエネルギー効率が改善し、筋タンパク質の分解が抑制された結果と推定される。加えて、体タンパクの異化や筋ジストロフィーで発生する炎症を抑制したことで筋組織の破壊が抑制され、筋重量が維持された可能性もある。なお、前脛骨筋は主に速筋、ヒラメ筋は主に遅筋で構成され、速筋と遅筋では筋組織の同化・異化に関わる遺伝子発現に相違がある。飼料1、飼料2は、前脛骨筋などの速筋に与える影響が大きいと推定された。
<筋力の測定>
9週間の飼育後、ラットの筋力をグリップテストにより測定した。各個体10回ずつグリップテストを行い、前半5回のうち最大値と最大値を除く3回の平均(メーカー推奨のメソッド)として筋力を求め、体重あたりの筋力に換算して求めた。結果を図9、図10に示す(図中、被験群n=6、対照群n=7、*はp<0.05、**はp<0.01)。

結果:被験群では、グリップテストによる筋力の統計的有意な増加が確認された。筋量の増加のみならず、実生活に必要なグリップ力が増加されることが判明した。
<CK値の測定>
9週間の飼育後、麻酔下で開腹し腹大動脈から採血し、血清CK(クレアチンキナーゼ)値を測定した。結果を図11に示す(被験群n=6、対照群n=7)。

結果:クレアチンキナーゼは、筋収縮の際のエネルギーの代謝に関する酵素成分であり、血清CK値を測定することで逸脱酵素量を測定することで筋疾患の病状を分析することができる。被験群で血清CK値が対照群より低下している。ジストロフィーでは、CK値の上昇が観察されるが、被験群では筋組織の破壊が抑制された結果、血清CK値が減少したと推定される。
<筋線維径の測定および筋線維化領域の定量>
9週間の飼育後、前脛骨筋をサンプリングし、凍結した後、クライオスタットを用いて切片を作成した。その後、ヘマトキシリン・エオジン染色し、筋線維のフェレ径を測定し、その最小値を筋線維径とし、筋線維径のヒストグラムを作成した。ヘマトキシリン・エオジン染色の結果を図12のAに示し、ヒストグラムの結果をBに示す。対照群では、筋線維径に大小不同が見られ、極度に萎縮した矮小な繊維から、肥大した筋までほぼ一定の割合で存在する。これに対し、被験群では筋線維径の大小不同が少ないことが観察された。また、対照群では、炎症細胞浸潤が観察され、間質の開大と線維化が観察された。

9週間の飼育後、前脛骨筋をサンプリングし、凍結した後、クライオスタットを用いて切片を作成した。その後、マッソントリクローム染色し、線維化の程度を観察した。結果を図13に示す。図13に示す画像から、CellProfiler(バージョン3.1.8)を使用して青く染色された膠原線維の領域(線維化領域)を抽出した。抽出した線維化領域を切片全体の面積で割ることで、筋組織における線維化領域の割合を算出した。線維化領域の割合を図14に示す(被験群n=6、対照群n=7、*はp<0.05)。
結果:被験群では対照群に比べて、前脛骨筋の筋線維径が太く、筋組織の破壊を抑制する傾向が観察された。この結果は、上記CK値の結果と符合するものである。
<炎症細胞浸潤量の測定>
9週間の飼育後、大腿四頭筋をサンプリングし、遠心分離により免疫細胞の画分を抽出し、フローサイトメトリーを用いて、制御性T細胞(Treg)、ヘルパーT細胞(Th)、マクロファージ(MΦ)を測定し、生細胞に対するマクロファージ(MΦ)の割合、ヘルパーT細胞に対する制御性T細胞(Treg)の割合を算出した。結果を図15および図16に示す(被験群n=3、対照群n=4)。
9週間の飼育後、大腿四頭筋をサンプリングし、遠心分離により免疫細胞の画分を抽出し、フローサイトメトリーを用いて、制御性T細胞(Treg)、ヘルパーT細胞(Th)、マクロファージ(MΦ)を測定し、生細胞に対するマクロファージ(MΦ)の割合、ヘルパーT細胞に対する制御性T細胞(Treg)の割合を算出した。結果を図15および図16に示す(被験群n=3、対照群n=4)。
結果:長期にわたる過度の筋組織の炎症は、筋ジストロフィーなどの慢性疾患の進行の一因となることが知られている。被験群では、マクロファージの浸潤量が対照群より低減し、抗炎症性の免疫細胞である制御性T細胞の割合の増加が観察され、被験群は対照群より炎症が抑えられていた。飼料1、飼料2は、筋組織の炎症を抑制することで筋ジストロフィーなどの治療や予防に効果があると推定された。
<筋衛星細胞の分化レベルの測定>
9週間の飼育後、大腿四頭筋をサンプリングし、遠心分離により筋衛星細胞の画分を抽出し2日間培養した。その後筋衛星細胞の未分化~分化途中のマーカー遺伝子であるPax7と、分化途中のマーカー遺伝子であるMyoDの陽性細胞数を比較し、筋衛星細胞の未分化性がどれくらい保たれているかの評価を行った。結果を図17に示す。
9週間の飼育後、大腿四頭筋をサンプリングし、遠心分離により筋衛星細胞の画分を抽出し2日間培養した。その後筋衛星細胞の未分化~分化途中のマーカー遺伝子であるPax7と、分化途中のマーカー遺伝子であるMyoDの陽性細胞数を比較し、筋衛星細胞の未分化性がどれくらい保たれているかの評価を行った。結果を図17に示す。
結果:衛星細胞は、成熟した筋組織に見られる細胞質をほとんど持たない単核の前駆細胞であり、骨格筋細胞の前駆細胞であり、その活性化により細胞周期に入り、増殖および筋芽細胞に分化することが出来る。筋ジストロフィーでは、度重なる筋線維の破壊と再生の繰り返しの中で、筋組織の幹細胞である筋衛星細胞が恒常的に活性化され、その未分化性を失いほとんどが分化して筋線維になり、幹細胞の総量が減少すると考えられている。Pax7を発現する衛星細胞数と、MyoD陽性で検出される活性化衛星細胞数を測定したところ、被験群の大腿四頭筋では、Pax7に対するMyoD陽性細胞の数が抑制されていた。幹細胞の未分化性が保たれ、幹細胞自体の総量が維持され再生に寄与している可能性が示唆された。
<筋組織切片の免疫蛍光染色>
前脛骨筋を採取後直ちに、液体窒素で冷却したイソペンタンに浸して急速凍結した。クライオスタット(ライカバイオシステムズ社製)を用いて凍結筋組織を薄切し、スライドグラスに貼り付けた。

前脛骨筋の凍結切片(厚さ7μm)を、室温下で30分間、十分に風乾させた。その後4%パラホルムアルデヒドで15分間、室温で固定した後、それぞれ以下の方法で免疫染色を行った。
IgGの免疫染色では、非特異的タンパク質結合を、0.1%Tritonを含むリン酸緩衝生理食塩水(PBS)中の5%ウシ胎児血清でブロックした。切片をラミニン一次抗体と室温で2時間インキュベートし、PBSで洗浄した後、1:100に希釈したAlexaFluorコンジュゲート二次抗体(ヤギ抗ウサギIgG(H+L) F(ab’)2 Fragment Alexa Fluor 488(#4412;Cell signaling technology社製)及びヤギ抗ラットIgG H&L Alexa Fluor 647抗体(ab150159;Abcam社製))と室温で1時間インキュベートした。核はHoechst 33258でカウンターステインした。すべてのサンプルは、BZ-X810オールインワン蛍光顕微鏡(キーエンス社製)を用いて可視化した。前脛骨筋の全切片を写真撮影した。筋壊死は、写真上においてブラインドで数えた全線維に対するIgG陽性線維の比率(%)で定量した。
CD11bおよびeMHCの免疫染色では、0.1%Triton(ブロッキングバッファー)と0.6%H2O2とを含むリン酸緩衝生理食塩水(PBS)中の5%ヤギ血清で非特異的タンパク質結合をブロックし、内因性ペルオキシダーゼ活性をクエンチした。切片を一次抗体とともに室温で2時間インキュベートした後、PBSで洗浄し、Histofine Simple stainRat MAX-PO(ニチレイバイオサイエンス社製)とともに室温で1時間インキュベートした。3,3-ジアミノベンジジン(DAB)反応後、核をヘマトキシリンでカウンターステインした。すべてのサンプルは、DP73デジタルカメラ(オリンパス社製)を装備したBX51蛍光顕微鏡(オリンパス社製)を用いて可視化した。
Pax7、MyoDおよびKi67の免疫染色では、非特異的なタンパク質の結合を、0.1%Tritonを含むリン酸緩衝液(PBS)中の5%ロバ血清(ブロッキングバッファー)でブロックした。切片は以下の一次抗体と4℃で一晩インキュベートし、PBSで洗浄した後、1:400に希釈したAlexaFluor-conjugated secondary donkey anti-mouse IgG(H+L) Alexa Fluor 594(1:400、Jackson Immuno Research Laboratories製)、およびヤギ抗ウサギIgG(H+L)Alexa Fluor 488(1:400、JacksonImmuno Research Laboratories製)抗体を用いて、室温で1時間染色した。核はHoechst 33258でカウンターステインした。すべてのサンプルは、BZ-X810オールインワン蛍光顕微鏡を用いて可視化した。
Pax7、MyoDおよびKi67陽性の細胞数は、蛍光顕微鏡(Keyence社製)の10×対物レンズでランダムに選択した5つのフィールドで盲検的にカウントした。

<遺伝子発現の評価>
解剖によりサンプリングした前脛骨筋から、リアルタイムポリメラーゼ連鎖反応(PCR)により遺伝子発現を評価した。凍結した前脛骨筋の筋を、マイクロスマッシュMS-100R(トミー精工社製)を用いて、RNAiso Plus(タカラバイオ社製)で破砕し、total RNAを抽出した。DNAを除去し、PrimeScriptTM RT Reagent Kit及びgDNA Eraser(タカラバイオ社製)をそれぞれ用いてcDNAを合成した。SYBR(商標) Premix Ex TaqTM II(タカラバイオ社製)、LightCyclerTM(Roche Diagnostics社製)及び各遺伝子に対するフォワードプライマーとリバースプライマーとを用いて、リアルタイムPCRを行った。リアルタイムPCRの増幅条件は、95℃で10秒間に続いて、95℃で5秒間、57℃で10秒間及び72℃で10秒間を1サイクルとしての45サイクルである。筋損傷実験におけるリアルタイムPCRの内部標準として一般的に用いられるHprt遺伝子の発現量で、すべての遺伝子の発現量を正規化した。

評価の対象とした遺伝子は、組織の線維化を促進する因子であるTGFβ1遺伝子および筋衛星細胞の増殖を促進する因子であるFGF2遺伝子である。Hprt遺伝子に対するフォワードプライマー及びリバースプライマーの塩基配列をそれぞれ配列番号1及び2に示す。TGFβ1遺伝子に対するフォワードプライマー及びリバースプライマーの塩基配列をそれぞれ配列番号3及び4に示す。FGF2遺伝子に対するフォワードプライマー及びリバースプライマーの塩基配列をそれぞれ配列番号5及び6に示す。
結果:図18は、12週齢のラットにおけるラットIgG、ラミニンおよびHoechstでの染色像の一例を示す。当該染色像では壊死した筋線維(ラットIgG)、基底膜(ラミニン)、核(Hoechst)はそれぞれ赤、緑および青で示される。図19は、図18から定量したIgG陽性筋線維の割合を示す。飼料1および飼料2の投与によって、壊死筋の割合および炎症を軽減させた。図20は、TGFβ1遺伝子の発現量を示す。図20に示すように、飼料1および飼料2の投与による線維化の軽減はTGFβ1の発現の減少を介していることが示唆された。
図21は、12週齢のラットにおけるCD11bでの染色像の一例を示す。図22は、図21から定量したCD11b陽性細胞数を示す。CD11b陽性細胞数の抑制は、図15および図16に示された筋組織の炎症の抑制を支持する。
図23は、12週齢のラットにおけるeMHCの染色像の一例を示す。図23の染色像における染色箇所はeMHC陽性筋線維を示す。図24は、図23から定量した全筋線維中のeMHC陽性筋線維の割合を示す。飼料1および飼料2の投与によって再生筋の数が減ることが示唆された。これは再生能の低下ではなく、壊死筋の数が減ったことを反映した結果であると考えられる。
図25は、12週齢のラットにおける筋衛星細胞の染色像の一例を示す。Pax7およびMyoDは筋衛星細胞のマーカーである。Ki67は増殖細胞のマーカーである。Hoechstは核のマーカーである。図26はHoechst陽性の細胞中のPax7陽性細胞(Pax7陽性筋衛星細胞)の割合を示す(被験群n=6、対照群n=7)。図27はHoechst陽性の細胞中のMyoD陽性細胞(MyoD陽性筋衛星細胞)の割合を示す(被験群n=6、対照群n=7)。
図28はPax7陽性細胞中のKi67陽性細胞の割合を示す(被験群n=6、対照群n=7、*はp<0.05)。飼料1および飼料2の投与によって、Pax7陽性細胞中のKi67陽性細胞の割合が増えた。すなわちPax7陽性の筋衛星細胞の増殖が促進された。図29は、MyoD陽性細胞中のKi67陽性細胞の割合を示す(被験群n=6、対照群n=7、*はp<0.05)。飼料1および飼料2の投与によって、MyoD陽性細胞中のKi67陽性細胞の割合が増えた。すなわちMyoD陽性の筋衛星細胞の増殖が促進された。図30はFGF2遺伝子の発現量を示す(被験群n=6、対照群n=7、**はp<0.01)。飼料1および飼料2の投与による筋衛星細胞の増殖の促進は、FGF2の発現量の増大を介していることが示唆された。
試験例2
上記の試験例1ではラットが12週齢になるまでを解析したのに対し、試験例2では、9か月齢になるまでの長期飼育後のラットについて評価した。

<ジストロフィン遺伝子変異ラット(筋ジストロフィーモデルラット)の飼育>
試験例1と同様にデュシェンヌ型筋ジストロフィーを発症するラットを用いた。9か月齢での検体のためのラットについては、離乳後にラットを被験群と対照群の2群に分け、対照群には表1に記載する飼料3を9か月齢まで摂食させ、被験群には表1に記載する飼料2を10日間与え、その後は表1に示す飼料1を9か月齢まで与えた。摂餌は両群ともに自由摂餌とした。
試験例1と同様にデュシェンヌ型筋ジストロフィーを発症するラットを用いた。9か月齢での検体のためのラットについては、離乳後にラットを被験群と対照群の2群に分け、対照群には表1に記載する飼料3を9か月齢まで摂食させ、被験群には表1に記載する飼料2を10日間与え、その後は表1に示す飼料1を9か月齢まで与えた。摂餌は両群ともに自由摂餌とした。
デュシェンヌ型筋ジストロフィーを発症するラットの同腹仔から野生型のラットを実験に用いた。離乳後のラットに表1に記載する飼料3を9か月齢まで自由摂餌で摂食させた。n数はどの群も4~12である。なお、棒グラフの上についたアルファベットは、異なるアルファベットがついた群間でのp<0.05の有意差を示す。
被験群、野生型群および対照群の9か月齢までの体重の変化を図31に示す。被験群と対照群とでほぼ同等の体重量の増加が確認された。試験例1と同様に、グリップテストにより測定した9か月齢のラットの体重あたりの筋力を図32に示す。飼料1および飼料2を投与された被験群では、筋力の低下が軽減された。
試験例1と同様に、筋組織切片をヘマトキシリン・エオジン染色し、筋線維の最小フェレ径を測定した。ヘマトキシリン・エオジン染色の結果を図33に示す。対照群では、筋線維径に大小不同が見られ、極度に萎縮した矮小な繊維から、肥大した筋までほぼ一定の割合で存在する。これに対し、被験群では筋線維径の大小不同が少ないことが観察された。図34は最小フェレ径を示す。飼料1および飼料2の投与によって、筋線維径の縮小が軽減されたことが示された。
図35は前脛骨筋から作成した切片のマッソントリクローム染色の染色像を示す。図35に示す画像から上述のように抽出した線維化領域を切片全体の面積で割ることで、線維化領域の割合を算出した。線維化領域の割合を図36に示す。飼料1および飼料2の投与によって、線維化が軽減されることが示された。
飼料1および飼料2の投与によって、筋の壊死および炎症が抑えられ、筋衛星細胞の増殖が促進された。この効果は病態後期である9か月齢になっても継続し、筋力低下の軽減、線維化領域の軽減、および筋線維径の増大をもたらした。
本発明は、本発明の広義の精神と範囲を逸脱することなく、様々な実施の形態及び変形が可能とされるものである。また、上述した実施の形態は、本発明を説明するためのものであり、本発明の範囲を限定するものではない。すなわち、本発明の範囲は、実施の形態ではなく、請求の範囲によって示される。そして、請求の範囲内及びそれと同等な発明の意義の範囲内で施される様々な変形が、本発明の範囲内とみなされる。
