Abstract
본 발명은 LSP1(Leukocyte-specific protein 1)의 결핍된 T 세포-기반 면역 항암 요법에 관한 것으로, 본 발명에 따르면, LSP1의 낙아웃에 의해 종양 성장, 종양의 부피 및 크기가 감소하였으며, T 세포에서의 LSP1 결핍이 T 세포의 수, 분포 빈도, 이동 및 침윤을 증가시키고, 항 종양 사이토카인의 생산을 증가시킴으로써 종양의 성장을 억제하고 살상능을 향상시키므로, 이를 T 세포-기반 면역 요법에 이용할 수 있다.
Description
본 발명은 LSP1(Leukocyte-specific protein 1)의 결핍된 T 세포-기반 면역 항암 요법에 관한 것이다.
의학 연구에서 수많은 진보에도 불구하고, 암은 문명세계를 통하여 사망의 주요 원인으로 남아있다. 수술, 방사선요법 및 일반화된 화학요법등과 같은 암 치료에 대한 비 특이적인 접근은 순환하며 천천히 자라는 고형암의 선택적인 군의 치료에서 성공적이었다. 그러나, 많은 고형 종양은 이런 접근에 상당한 내성이 있고, 그 경우에 예후(豫後)는 매우 나쁘다. 표준 치료는 세포절제술후에 방사선 요법 또는 화학요법을 포함하나, 완치가 어렵고, 재발이 많다. 이와 같은 암치료에 있어서 부상하는 분야는 면역요법이다. 일반원리는 치료받는 환자에게 특히 약성세포에 대하여 실제로 거부반응인 것을 배치하는 능력을 부여하는 것이다. 1. 다양한 종류의 자극된 자가세포를 사용하는 차용 면역요법(adoptive immunotherapy); 2. 동종이계 림프구의 전신전달; 3. 면역학적으로 반응성인 세포의 종양내 이식; 4. 전신-종양 특이적 면역반응을 발생시키는 먼 부위에서의 백신접종을 포함하는 수 많은 면역학적 전략이 개발중이다. 이들 가운데, 차용 면역요법으로 종양 항원-특이적 림프구의 차용 전달(adoptive transfer)이 유망한데, 지금까지 이들 시도는 말초 혈관으로부터의 단핵 세포 또는 신규 종양 피검체로부터의 종양 침윤 림프구(tumour infiltrating lymphocyte, TIL)에 주로 근거하였다. 최근 악성흑색종 환자의 치료 시도에서, 증식된 TIL의 자가 전달 치료 시도는 51%까지의 객관적 반응률을 보인다고 보고되었다. TIL 세포는 수가 적으며, 종양으로부터의 면역억제 메커니즘으로 인하여 자주 무력(anergic)하게 되므로 증식이 일어나는데 오랜 주기 (수 개월)가 걸린다.
종양 침윤 백혈구(tumor-infiltrating leukocytes, TIL)의 밀도, 조성 및 기능적 상태로 구성되는 면역구조(Immune contexture)는 PD-1(programmed cell death protein 1)에 대한 항체(Ab)-기반 면역요법을 포함하는 항-종양 면역요법의 종양 진행 및 효능을 결정한다 (Fridman WH, Zitvogel L, Sautes-Fridman C, et al. Nat Rev Clin Oncol 2017;14(12):717-34.). 여러 연구에서 고밀도의 T 세포가 결장암, 비소세포폐암, 간세포암, 췌장암, 위암 및 흑색종을 포함하는 다양한 암에 걸린 환자에서 유리한 예후 및 생존과 양의 상관관계가 있다고 보고했다. 따라서, 항원-활성화 된 T 세포, 특히 키메라 항원 수용체(chimeric antigen receptor, CAR)-T 세포를 사용한 차용 세포 전달은 항암 요법의 효능을 향상시키는 유망한 전략 중 하나로 떠오르고있다 (Lim WA, June CH. Cell 2017;168(4):724-40.). 예를 들어, CD19-표적화 CAR-T 세포 요법이 재발성 또는 난치성 B-세포 급성림프구성백혈병 및 림프종을 포함하는 혈액 악성 종양 환자에서 현저히 높은 차도를 나타냈다.
혈액 악성 종양에서의 성공에도 불구하고 CAR-T 세포 요법은 고형암에서는 항상 효과적이지 않았고 오히려 실망스러웠다. 이와 같은 T-세포 기반 암 면역요법의 주요 장애물 중 하나는 불충분한 T 세포의 종양 덩어리로의 트래피킹일 것이다. 임상 및 전임상 연구에서, 생체 외 확장 후 다량의 T 세포의 주입에도 불구하고 전달된 T 세포의 소량만이 종양 조직의 내부에 도달할 수 있다 (Slaney CY, Kershaw MH, Darcy PK. Cancer Res 2014;74(24):7168-74.). 어째서 T 세포의 트래피킹, 침윤 및 침투가 불충분한지는 아직 밝혀지지 않았지만, 고형 종양의 모양이 보다 섬유성이고 덜 침습적이어서 궁극적으로 면역억제성 종양 환경(TME)을 구축하기 때문일 수 있다. 따라서, 고형 종양에 대한 T-세포 기반 면역요법의 효과를 최대화하기 위해서는, 섬유성 및 면역억제성 TME를 파괴 또는 우회함으로써 종양 덩어리 내부에 면역적격성(immunocompetent) T 세포를 성공적으로 전달할 수 있는 방법을 개발할 필요가 있다.

본 발명의 조성물의 치료적으로 유효한 양은 여러 요소, 예를 들면 투여방법, 목적부위, 개체의 상태 등에 따라 달라질 수 있다.

본 발명의 조성물은 목적하는 방법에 따라 비 경구 투여(예를 들어 정맥 내, 피하, 복강 내 또는 국소에 적용)하거나 경구 투여할 수 있으며, 경구 투여 하는 것이 가장 바람직하다. 투여량은 개체의 체중, 연령, 성별, 건강상태, 식이, 투여시간, 투여방법, 배설률 및 질환의 중증도 등에 따라 그 범위가 다양하다.

본 발명에서 용어, "개체"란 암이 이미 발병되었거나 발병될 수 있는 인간을 포함한 모든 동물을 의미한다. 본 발명의 발현 억제제 또는 세포를 포함하는 조성물을 개체에 투여함으로써, 상기 질환을 효율적으로 예방 및 치료할 수 있다. 예를 들어, 본 발명의 조성물로 다양한 암 또는 전이성 암이 발병한 인간을 치료할 수 있다. 또한 본 발명의 조성물로 다양한 암종 또는 종양이 발병한 쥐 또는 돼지를 치료할 수 있다. 본 발명의 조성물은 기존의 항암제 또는 항암보조제와 병행하여 투여할 수 있다.
본 발명의 조성물은 약제학적으로 유효한 양으로 투여한다. 용어 "약제학적으로 유효한 양"은 의학적 치료에 적용 가능한 합리적인 수혜/위험 비율로 질환을 치료하기에 충분한 양을 의미하며, 유효 용량 수준은 개체의 종류 및 중증도, 연령, 성별, 감염된 바이러스 종류, 약물의 활성, 약물에 대한 민감도, 투여 시간, 투여 경로, 배출 비율, 치료 기간, 동시 사용되는 약물을 포함한 요소 및 기타 의학 분야에 잘 알려진 요소에 따라 결정될 수 있다. 본 발명의 조성물은 개별 치료제로 투여하거나 다른 치료제와 병용하여 투여될 수 있고 종래의 치료제와는 순차적 또는 동시에 투여될 수 있다. 그리고 단일 또는 다중 투여될 수 있다. 상기 요소를 모두 고려하여 부작용 없이 최소한의 양으로 최대 효과를 얻을 수 있는 양을 투여하는 것이 중요하며, 당업자에 의해 용이하게 결정될 수 있다.
또한, 본 발명은 LSP1 유전자가 결실된 T 세포를 유효성분으로 포함하는 약학적 조성물을 개체에 투여하는 단계;를 포함하는 암의 예방 또는 치료 방법을 제공한다.

실시예 1. LSP1 억제에 의한 종양 성장 억제
1-1. . LSP1 억제에 의한 종양 성장 억제 효과
5Х105 개의 B16 흑색종(melanoma)을 PBS에 재부유시킨 뒤 8-12 주령 WT 마우스 및 Lsp1 KO 마우스 (Dr. Laurent Sabbagh, University of Montreal, Montreal, Quebec, Canada)의 우측 옆구리에 피하주사하고 2-3일마다 캘리퍼로 종양 부피를 측정하고 하기 수학식 1로 계산하여 3주 동안 종양의 성장을 확인하였다.

그 결과, WT 마우스에 비해 Lsp1 KO 마우스에서 종양 성장이 현저하게 감소되었다 (도 1A). 또한, 종양 접종 14일 후 Lsp1 KO 마우스에서 종양의 부피 및 중량이 WT 마우스에서보다 현저히 낮게 나타났다 (도 1B 및 1C).
1-2. LSP1 억제에 의한 T 세포의 종양 침윤 촉진 효과
상기의 흑색종을 갖는 마우스에서 Lsp1 결핍에 의해 영향을 받는 종양 미세환경(tumor microenvironments, TME)에서의 면역 구조(immune contexture)를 확인하기 위해, WT 마우스에서 평균 종양 부피가 약 700 mm3에 도달하였을 때 종양을 가지고 있는(tumor-bearing) WT 및 Lsp1 KO 마우스의 종양 침윤 백혈구(tumor-infiltrating leukocytes, TILs)를 유세포 분석하였다. 구체적으로, TILs를 분리하기 위해, WT 마우스의 평균 종양 부피가 500 또는 700 mm3에 달했을 때 WT 및 Lsp1 KO 마우스를 희생시키고 원발성 종양을 기계적으로 절제하고 분리하였다. 그 후 70 μm 세포 여과기로 여과하여 종양 세포 현탁액을 얻고, Ficoll-Hypaque (GE Healthcare, Chicago, IL) 밀도 구배 원심분리기를 이용하여 TILs를 분리하였다. 분리한 단일-세포 현탁액을 PBS로 제척한 뒤 유세포 분석을 수행하였다. 유세포 분석을 위해, CD45 (30-F11, BD Pharmingen, Franklin Lakes, NJ), CD3 (145-2C11, Invitrogen, Carlsbad, CA), CD4 (GK1.5, Biolegend, San Diego, CA) 및 CD8 (53-6.7, Biolegend)와 같은 플로오로크롬-표지된 항-마우스 항체들로 표면 염색을 30분 동안 4 ℃에서 수행하였다. 이 후, 세포를 FACS 버퍼에 재부유하고 DIVA 소프트웨어로 FACS CantoⅡ(BD Biosciences) 또는 LSR Fortessa (BD Biosciences)를 이용하여 유세포 분석하였고, 이를 통해 얻은 데이터는 FlowJo 소프트웨어 (FlowJo, LLC, Franklin Lakes, NJ)로 분석하였다. 그 결과, Lsp1 KO 마우스의 종양이 WT 마우스의 종양보다 현저히 많은 수의 침윤된 CD45+ 세포, CD3+, CD4+ 및 CD8+ T 세포를 가지는 것으로 나타났다 (도 1D). 이를 통해, Lsp1 KO 마우스에서 흑색종의 성장 감소가 T 세포의 구성보다는 증가된 세포수와 관련될 수 있음을 유추할 수 있다.
또한, CD8+ T 세포 및 CD4+ T 세포를 면역조직화학염색하여 종양에서의 T 세포의 침윤을 확인하였다. 구체적으로, OCT-임베딩된 종양 조직의 7 μm 절편을 차가운 아세톤으로 -20℃에서 10분 동안 고정하고 절편을 상온에서 30분 동안 0.3% H2O2에서 인큐베이션함으로써 내인성 퍼옥시다아제를 퀀칭한 뒤, 조직을 10% 정상 당나귀 혈청으로 상온에서 1 시간 동안 블로킹하였다. 그 후, 조직 절편을 랫 항-마우스 CD4 (1:1000, Biolegend) 또는 랫 항-마우스 CD8 (1:1000, Biolegend) Ab로 4℃에서 하룻밤 동안 인큐베이션하고 각 슬라이드를 PBS로 3회 세척하고 항-랫 이차 Ab (1:100, Vector Laboratories, Burlingame, CA)를 이용하여 VECTASTAIN Elite ABC HRP kit (Vector Laboratories)로 검출하였다. 양성 세포는 3,3'-diaminobenzidine tetrahydrochloride (DAB; Vector Laboratories)를 이용하여 검출하고 헤마톡실린으로 대조염색하였다. 이미지는 Pannoramic MIDI slide scanner (3DHISTECH)로 얻었으며, 염색 양성 세포를 마우스마다 상이한 두 개의 슬라이드, 각 슬라이드의 6개 필드에서 수동으로 계수하였다. 그 결과, CD4+ T 세포와 달리, CD8+ T 세포의 침윤이 WT 마우스보다 Lsp1-결핍 마우스에서 종양의 중심 및 가장자리 둘 다에서 현저히 높게 나타나 (도 1E 내지 1H), Lsp1 결핍이 CD8+ T 세포의 종양 중심으로의 침윤을 촉진하는 것을 확인할 수 있었다.
1-3. 면역 세포 집단 확인
TME를 구성하는 T 세포 외의 다른 면역 세포 집단에 대한 LSP1 억제에 의한 종양에서의 영향을 확인하기 위해, 종양 내 NK1.1+ NK 세포, CD19+ B 세포, 조절 T 세포 (Treg cells: Foxp3+ CD4+ T cells) 및 종양 관련 대식세포(tumor-associated macrophages , TAMs)의 분포 빈도 및 수를 Lsp1 KO 마우스 및 WT 마우스의 종양에서 유세포 분석으로 확인하였다.
그 결과, TILs의 절대수가 Lsp1 KO 마우스의 종양에서 현저히 많았음에도 불구하고 종양 내 NK1.1+ NK 세포 및 CD19+ B 세포의 분포 빈도에 차이가 없었으며 (도 2A), 조절 T 세포 (Treg cells: Foxp3+ CD4+ T cells)의 분포 빈도도 차이가 나타나지 않았다 (도 2B). 또한, CD11b+ 골수성 세포(myeloid cell)가 Lsp1 KO 및 WT 마우스에서 유사하게 나타난 반면, CD11b+Ly6ClowF4/80high 종양 관련 대식세포 (TAMs)의 분포 빈도는 WT 마우스에 비해 Lsp1 KO 마우스의 종양에서 현저히 감소하였다 (도 2C). 상기 결과를 통해 주목할만한 것은, Lsp1 KO 마우스의 종양에서 TAM들 중에 전-염증성 M1-유사 (CD206low MHCⅡhigh) TAMs가 현저히 증가했고, 항-염증성 M2-유사 (CD206high MHCⅡlow) TAMs가 현저히 감소한 것이다 (도 2C).
상기 결과들을 통해, Lsp1-결핍 마우스가 CD8+ T 세포의 침윤을 향상시키고, M2-유사 TAM에 비해 전-염증성 M1-유사 TAM의 침윤을 향상시킴으로써 항-종양 면역 환경을 조성하는 것을 알 수 있다.
실시예 2. LSP1 과발현에 의한 종양 성장 촉진
2-1. . LSP1 과발현에 의한 종양 성장 촉진 효과
종양 발달 동안 LSP1의 T 세포-특이적 효과를 확인하기 위해 T 세포에서 Lsp1를 과발현하는 형질전환(transgenic, Tg) 마우스를 제작하였다 (도 3A). 구체적으로, 마우스 Lsp1의 cDNA를 인간 CD2 프로모터를 포함하는 백혈구-특이적 발현 카세트에 클로닝하고, 컨스트럭트를 수정란의 원핵에 직접 주입하고 유전체 DNA를 PCR로 분석하여 형질전환 파운더(transgenic founder)를 선별하였다. 그 후, RNeasy Mini kit (Qiagen, Hilden, Germany)를 이용하여 이의 비장의 CD4+ 및 CD8+ T 세포로부터 총 RNA를 분리하고 RevertAid Reverse Transcriptase (Thermo Fisher Scientific)를 이용하여 cDNA로 변환하였다. 그 후, SYBR Green PCR premix (Bio-Rad, Hercules, CA) 및 프라이머 (5'-CCAGCCCTTTGGCCTTAGAA-3' 및 5'-TGGAAATGGGCAAGGTTGGT-3')를 이용하여 CFX96 real-time PCR 시스템으로 Lsp1 mRNA의 발현 정도를 분석하여 WT 마우스와 비교함으로써 Lsp1 Tg 마우스를 검증하였다. 이렇게 제작한 Lsp1 Tg 마우스와 WT 마우스에 상기 실시예 1에서와 같이 흑색종을 피하 접종하고 종양의 성장 정도를 확인한 결과, WT 마우스에 비해 CD4+ 및 CD8+ T 세포가 Lsp1 mRNA를 현저히 많이 발현하는 Lsp1 Tg 마우스 (도 3B 및 C)가 Lsp1 KO 마우스와는 대조적으로 3주 동안 WT 마우스에 비해 현저히 종양 성장이 가속화되는 것으로 나타났다 (도 3D). Lsp1 Tg 마우스의 종양의 부피 및 중량 또한 종양 접종 14일차에 WT 마우스에 비해 현저히 높게 나타났다 (도 3E 및 F).
2-2. 면역 세포 집단 확인
Lsp1 Tg 마우스의 TME에서 면역 세포 집단을 평가하기 위해, WT 마우스의 종양 부피가 약 500 mm3에 도달할 때 종양을 수집하여 유세포 분석함으로써 TIL의 수 및 분포 빈도를 분석하였다.
그 결과, 종양-침윤 CD3+ T 세포 및 CD4+ T 세포의 분포 빈도가 WT 및 Lsp1 Tg 마우스 사이에 유사한 것으로 나타났으나, Lsp1 KO 마우스에서의 결과와는 반대로 CD8+ T 세포의 분포 빈도가 Lsp1 Tg 마우스에서 WT 마우스에 비해 감소하는 것으로 나타났다 (도 3G). 또한, 종양 내 CD45+ T 세포, CD3+ T 세포, 특히 CD8+ T 세포의 절대수가 WT 마우스에 비해 Lsp1 Tg 마우스에서 현저히 낮았다 (도 3H). 아울러, Lsp1 과발현이 T 세포에 특이적이기 때문에 종양 내 NK1.1+ NK 세포, CD19+ B 세포 및 CD11b+ 골수성 세포의 분포 빈도 차이는 나타나지 않았다 (도 4A 내지 C).
상기 결과들을 통해, T 세포에서 Lsp1의 특이적 과발현이 B16 흑색종의 성장을 향상시키고, 이는 TILs, 특히, CD8+ T 세포의 수 및 분포 빈도의 감소와 관련되어 있음을 알 수 있다.
실시예 3. LSP1의 T 세포 이동 조절
3-1. LSP1의

+
T 세포 이동 억제
상기 실시예에서 유전적 Lsp1 억제(결핍)이 B16 흑색종에서 T 세포 침윤을 조절하였으므로, LSP1가 직접적으로 T 세포의 이동을 조절하는지 확인하기 위해, CD8+ T 세포를 종양 미세환경으로 트래피킹하고, 항-PD-1 요법 동안 TME에서 종양-특이적 T 세포 및 수지상 세포 사이의 상호 작용을 촉진하는 CXCL9 및 CXCL10에 LSP1가 응하여 CD8+ T 세포 이동을 조절하는지 확인하였다. 구체적으로, WT 및 Lsp1 KO CD8+ T 세포의 주화성(Chemotaxis)을 확인하기 위해 5 μm의 공극 크기를 가지는 Transwell 삽입물 (Corning Inc., Corning, NY)이 장착된 24-웰 플레이트의 하부 챔버에 뮤린 CXCL9 및 CXCL10 (R&D Systems, Minneapolis, MN)을 이동 배지 (0.1% FBS 포함 RPMI1640)에 희석하여 위치시키고, 50만 개의 CD8+ T 세포를 상부 챔버의 이동 배지에 분주하였다. 37℃에서 인큐베이션하고 4시간 후, 하부 챔버로 이동된 세포들을 헤모사이토미터(hemocytometer)로 계수하였다.
그 결과, Lsp1-결핍 CD8+ T 세포가 WT CD8+ T 세포보다 CXCL9 및 CXCL10에 현저한 화학주성반응(chemotactic response)을 보이는 것을 확인하였다 (도 5A). 반면, Lsp1-과발현 CD8+ T 세포는 WT 및 Lsp1-결핍 CD8+ T 세포에 비해 감소된 화학주성 이동을 나타내 (도 5A), LSP1이 CD8+ T 세포의 이동을 음성적으로 조절하는 것을 확인할 수 있었다. 그러나, 10% FBS로 자극된 CD8+ T 세포 이동은 CD8+ T 세포의 3 가지 유형 (도 5A) 간에 차이가 나타나지 않아, LSP1의 CD8+ T 세포 이동 조절이 CXCL9 및 CXCL10에 특이적임을 시사하였다.
3-2. TIL 및 LSP1의 병리적 관련성 확인
종양 조건에서 T 세포와 LSP1의 병리적 관련성을 확인하기 위해, B16 흑색종의 TILs에서 LSP1 발현 수준을 유세포 분석으로 확인하였다. 구체적으로, 정상 마우스 또는 종양-보유 마우스의 비장에서 세포를 분리하고 단일-세포 현탁액으로 준비하였다. CD4+ T 세포 또는 CD8+ T 세포를 항-CD4 비드 또는 항-CD8 비드 (Miltenyi Biotec, Bergisch Gladbach, Germany)를 이용하여 마그네틱 분리(magnetic separation)로 정제하였다. T 세포에서 LSP1 발현을 검출하기 위해, 정제한 CD4+ T 세포 또는 CD8+ T 세포를 완전배지(complete media)에서 재조합 IFN-γ (10 ng/ml, R&D Systems), TGF-β(transforming growth factor beta) (2 ng/ml, R&D Systems), IL-10(interleukin 10) (10 ng/ml, R&D Systems) 또는 항-마우스 CD3 Ab (1 μg/ml, 145-2C11, Invitrogen)와 함께 항-마우스 CD28 Ab (1 μg/ml, 37.51, Invitrogen)로 72시간 동안 자극시켰다. 배양된 세포들을 수거 및 염색하여 세포 내 LSP1 발현을 유세포 분석으로 검출하였다. 또한, 상기 T 세포들을 파쇄 버퍼로 파쇄한 뒤 12% SDS-PAGE 젤에서 전기영동하여 단백질들을 분리하고 PVDF 멤브레인에 트랜스퍼한 뒤 일렉트로블로팅하였다. 멤브레인들을 LSP1 (1:1000, Cell Signaling Technology) 항체 또는 β-tubulin (1:1000, Abcam, Cambridge, U.K.) 항체와 인큐베이션하고 enhanced chemiluminescent detection system (Thermo fisher scientific)을 이용하여 밴드를 시각화하였다.
LSP1 발현 수준을 확인한 결과, B16 흑색종에 침윤된 CD4+ T 세포는 종양이 없는 마우스의 비장 T 세포보다 LSP1의 발현 수준이 현저히 높게 나타났다 (도 5B). 또한, B16 흑색종에 침윤된 CD8+ T 세포도 유사한 결과를 나타내, T 세포에서 LSP1의 고발현이 B16 흑생종에 의해 유도될 수 있음을 알 수 있었다. 이와 같은 in vivo 종양 침윤 T 세포에서 LSP1 발현의 상향 조절이 어떻게 일어나는지 확인하기 위해, 어떤 종류의 종양-관련 자극이 LSP1 발현을 유도할 수 있는지 상기 유세포 분석을 통해 확인한 결과, 항-CD3/CD28 Abs 또는 전-염증 사이토카인인 IFN-γ로 T 세포를 자극하자 CD4+ 및 CD8+ T 세포 모두에서 LSP1 발현이 강하게 증가한 반면, TGF-β 및 항-염증 사이토카인인 IL-10는 LSP1 발현을 상향조절하지 못했다 (도 5C). 아울러, 이러한 T 세포 수용체 활성화 및 IFN-γ에 의한 LSP1 발현 증가를 웨스턴 블롯 분석으로도 확인하였다 (도 5D).
상기 실시예를 통해 T 세포에서 LSP1이 T 세포의 이동을 음성 조절하는 것을 확인한 것을 고려하면, 이와 같은 결과는 B16 흑색종이 TME 내의 T 세포에서 LSP1 발현을 상향조절함으로써 숙주의 T 세포의 항-종양 활성을 회피할 수 있음을 시사한다.
실시예 4.

-결핍 T 세포의 세포 독성 증가
4-1.
Lsp1
결핍에 의한 T 세포 세포독성 증가 확인
종양 성장에서 LSP1의 조절이 온전히 T 세포의 이동 조절에 의한 것인지 확인하기 위해, 종양 침윤 T 세포가 종양 성장을 억제하는 필수 단계인 T 세포의 세포 독성에 대한 LSP1의 영향을 확인하고자 하였다. 이를 위해, Lsp1 KO T 세포 (GSE75123)의 유전자 발현 프로파일을 T 세포-매개 세포 독성에 중점을 두어 WT T 세포와 비교 분석하였다. 구체적으로, 항-CD3/CD28 Abs로 6시간 동안 자극된 Lsp1 KO 및 WT 마우스의 비장 T 세포로부터 총 RNA를 분리하고 역전사 및 증폭한 뒤 24,241 개의 주석달린 유전자 (GSE75123)에 대한 62,976 개의 프로브가 포함된 어레이 칩 (SurePrint G3 Mouse GE 8x60K Microarray, Agilent)에 혼성화하였다. 정규화 후 각 유전자의 log2 fold 변화 값 및 P 값을 계산하였다. Lsp1 KO T 세포의 DEGs의 컷오프 값은 |fold change values| > 0.58였고 P values < 0.05였다. 또한, GSEA(Gene Set Enrichment Analysis)을 clusterProfiler (R package, ver.3.4.6)로 수행하였으며 GSEA 도표는 enrichrplot (R package)로 생성하였다. 아울러, Lsp1 KO T 세포에서의 DEGs와 세포독성-관련 유전자들 간의 오버랩의 유의성을 순열 검증(permutation test strategy)으로 결정하였다. 오버랩된 DEGs의 경헙적 P-값을 계산하기 위해 총 100,000 개의 무작위 치환 시료가 사용되었다.
그 결과, 세포독성 및 세포 사멸과 관련된 10 GOBP(Gene Ontology Biological Process) 용어를 대표하는 171 개의 유전자 중, 24 개의 유전자 (14%)가 Lsp1 KO T 세포의 DEGs(differentially expressed genes)와 유의하게 오버랩된 것으로 나타났다 (도 6A). 또한, GSEA(Gene Set Enrichment Analysis) 결과에서도 세포 사멸 및 백혈구 매개 세포독성의 생물학적 과정이 Lsp1 KO T 세포에서 증가한 반면 (도 6B 및 C), 세포 사멸의 음성 조절이 감소되었음 (도 6B)을 확인할 수 있었다. 이를 통해, T 세포에서 LSP1 결핍이 T 세포의 종양-사멸 세포독성의 증가와 연관되어 있음을 알 수 있다.
4-2.
Lsp1
결핍에 의한 T 세포 세포독성 이펙터 기능 촉진 확인
Lsp1 결핍에 의해 T 세포의 세포독성 이펙터 기능이 촉진되는지 확인하기 위해, 대표적인 항-종양 이펙터 사이토카인인 IFN-γ및 TNF-α의 발현 수준을 WT 및 Lsp1 KO 마우스의 비장 및 종양-침윤 T 세포에서 유세포 분석 방법으로 측정하였다.
그 결과, 종양 보유 WT 및 Lsp1 KO 마우스 간에 비장 크기, 비장 세포 수, 및 비장 CD4+ T 세포 및 CD8+ T 세포는 차이가 없었으며 (도 7A, B 및 C), 두 군의 비장에서 CD4+ 및 CD8+ T 세포 중 TNF-α+ 및 IFN-γ+ 세포의 분포 빈도는 유사한 정도로 나타났다 (도 7D 및 E). 반면, 종양에서는 WT 마우스에 비해 Lsp1 KO 마우스의 침윤된 CD4+ 및 CD8+ T 세포에서 TNF-α+ 및 IFN-γ+ 세포의 분포 빈도가 현저히 높게 나타나 (도 8A 내지 도8D), Lsp1 결핍이 종양-침윤 T 세포에서 TNF-α+ 및 IFN-η+ 발현을 유도함으로써 항-종양 면역성을 증가시키는 것을 알 수 있었다.
4-3.
Lsp1
과발현에 의한 T 세포 세포독성 이펙터 기능 저하 확인
Lsp1 Tg 마우스가 WT 마우스의 비장 중량 및 비장 세포수를 확인하였을 때, Lsp1 Tg 마우스의 비장 중량 및 비장 세포수가 WT 마우스와 비교하여 감소함을 확인하였다(도 9A 및 B).
또한, 상기 실시예와 반대로 Lsp1 Tg 마우스와 WT 마우스의 비장 및 종양-침윤 T 세포에서 IFN-γ 및 TNF-α의 발현 수준을 유세포 분석으로 확인한 결과, 종양 접종 후 WT 마우스에 비해 Lsp1 Tg 마우스에서 비장 CD4+ 및 CD8+ 세포의 TNF-α및/또는 IFN-γ+ 세포의 분포 빈도가 현저히 낮게 나타났다 (도 9C, D 및 E). 또한, 종양에서 Lsp1-과발현 CD4+ 및 CD8+ T 세포는 WT CD4+ 및 CD8+ T 세포에 비해 TNF-α+ 및/또는 IFN-γ+의 분포 빈도가 각각 현저히 감소된 것으로 나타났다 (도 9C, D 및 E). Lsp1 Tg 마우스의 비장 CD8+ T 세포가 WT 마우스보다 덜 팽창된 반면, Lsp1 Tg 마우스의 비장 CD4+ T 세포가 WT 마우스보다 더 팽창되었다 (도 9C). 이러한 결과들을 통해, T 세포에서 Lsp1가 과발현되면 침윤된 CD8+ T 세포를 현저히 감소시키고, CD8+ T 세포에 의한 TNF-α+ 및 IFN-γ+ 생산을 하향조절함으로써 흑색종의 성장을 촉진하는 것을 알 수 있다.
실시예 5.
Lsp1
결핍에 의한 항-PD-1 항체의 항-종양 활성 강화 효과
5-1.
Lsp1
-결핍의 T 세포에서의 항-종양 매개 효과
상기 결과들을 바탕으로 Lsp1-결핍 T 세포가 증가된 T 세포 트래피킹과 세포독성능에 의해 종양 성장을 더욱 효과적으로 억제한다고 가정하고, 성숙 T 세포 및 B 세포가 결핍된 Rag1 KO 마우스에서 Lsp1 KO 및 Lsp1 Tg T 세포를 이용한 차용 전달(adoptive transfer) 실험을 수행하여 이를 확인하였다. 구체적으로, Rag1 KO 마우스의 오른쪽 옆구리에 1Х105 개의 B16 흑색종을 피하 접종하고 하루 뒤, Pan T cell isolation kit (Miltenyi Biotec)를 이용하여 마그네틱 분리로 종양이 없는 Lsp1 KO 마우스 및 Lsp1 Tg 마우스의 바장으로부터 T 세포를 분리하였다. 비히클로서의 PBS 또는 Lsp1 KO 마우스 또는 Lsp1 Tg 마우스 유래 1Х107 개의 T 세포를 상기 B16 흑색종을 가진 Rag1 KO 마우스에 정맥 주사하였다. 종양 성장은 2일마다 기록되었다.
그 결과, 차용 전달 실험 전에는 Lsp1 KO T 세포 및 Lsp1 Tg T 세포가 CD3+ T 세포에서 유사한 CD4/CD8 비율을 가졌으나 (도 10A), 차용 전달 실험 후, Lsp1-과발현 T 세포 및 비히클 단독에 비해 Lsp1-결핍 T 세포가 B16 흑색종이 차용 전달된 Rag1 KO 마우스에서 종양 진행을 더욱 강하게 억제하였고 (도 10B), 이는 T 세포에서 Lsp1의 결핍이 항-종양 효과를 특이적으로 매개하는 것을 증명한다.
5-2.
Lsp1
-결핍의 항-PD-1 항체와의 병용처리에 의한 효과 확인
흑색종을 포함한 다양한 진행된 암 환자에게 면역요법으로 사용되어 온 면역체크포인트 억제제인 항-PD-1 차단(Anti-PD-1 blockade)과 본 발명의 접근법이 서로 상이한 항-종양 메커니즘 (억제성 면역 관문의 차단/T 세포 트래피킹)을 가지므로, Lsp1 결핍에 의한 항-종양 효과가 항-PD-1 Ab와의 병용 투여에 의해 영향을 받는지를 확인하였다. 구체적으로, 항-PD-1 차단 요법을 위해, 5Х105 개의 B16 흑색종을 WT 마우스 및 Lsp1 KO 마우스의 오른쪽 옆구리에 피하 접종하고 (0일), 3, 6, 9 및 12일 뒤에 항-PD-1 항체 (RMP1-14, Bio X cell, Lebanon, NH) 또는 이의 아이소파입 대조군 항체인 랫 IgG2a 아이소타입 대조군 (2A3, Bio X cell)을 10 mg/kg로 복강 내 투여하였다 (도 10C).
그 결과, 아이소타입 대조군 Ab를 처리한 WT 마우스에서 종양 부피가 1500 mm3에 달했을 때, 항-PD-1 Ab를 처리한 WT 마우스에서 종양 부피가 1000 mm3이고, 항-PD-1 Ab를 처리한 Lsp1 KO 마우스에서 종양 부피가 500 mm3였다 (도 10D). 즉, 항-PD-1 Ab의 처리가 WT 마우스에서 흑색종의 성장을 현저하게 감소시켰으며, 특히, 항-PD-1 차단(anti-PD-1 blockade)이 처리된 Lsp1 KO 마우스가 항-PD-1항체가 있거나 없는 WT 마우스보다 항-종양 효과가 현저하게 나타나 (도 10D), Lsp1 결핍에 의한 항-종양 효과가 항- PD-1 항체 처리와 관계없이 유지되는 것을 알 수 있다.
이를 통해, T 세포에서 Lsp1의 유전적 제거(genetic ablation)가 흑색종에 대한 항-PD-1 Ab를 포함하는 면역 관문 억제제의 치료 효능을 향상시키는 유용한 전략임을 시사한다(도 10E).
Claims (20)
- LSP1(Leukocyte-specific protein 1) 발현 억제제를 유효성분으로 포함하는 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, 발현 억제제는 siRNA(small interfering RNA), shRNA(small hairpin RNA), miRNA, 안티센스 올리고뉴클레오타이드 또는 핵산 앱타머인, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, T 세포-특이적 LSP1 발현 억제제인, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, DNA 절단 활성이 불활성화된(nuclease-deactivated) Cas12a 또는 Cas9 단백질, 반복적인 염기 서열(direct repeat, DR) 및 LSP1 유전자에 상보적인 프로토스페이서(protospacer)를 포함하는 crRNA(CRISPR RNA)를 포함하는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, 암은 고형암인, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, T 세포의 수 및 분포 빈도를 증가시키는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, T 세포의 종양으로의 침윤을 증가시키는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, TNF-α 및 IFN-γ 발현 및 분비를 증가시키는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 1항에 있어서, 면역체크포인트 억제제(immune checkpoint inhibitor)를 추가로 포함하는, 암의 예방 또는 치료용 약제학적 조성물.
- LSP1 유전자가 결실된 T 세포를 유효성분으로 포함하는 암의 예방 또는 치료용 약제학적 조성물.
- 제 10항에 있어서, T 세포는 LSP1 결실된 CAR(chimeric antigen receptor)-T 세포인, 암의 예방 또는 치료용 약제학적 조성물.
- 제 11항에 있어서, CAR은 암세포 항원-결합 도메인, 막횡단 도메인, 및 세포내 신호전달 도메인을 포함하는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 10항에 있어서, 면역체크포인트 억제제를 추가로 포함하는, 암의 예방 또는 치료용 약제학적 조성물.
- 제 13항에 있어서, 면역체크포인트 억제제는 PD-1 억제제 또는 CTLA-4 억제제인, 암의 예방 또는 치료용 약제학적 조성물.
- LSP1 발현 억제제 또는 LSP1 유전자가 결실된 T 세포를 유효성분으로 포함하는 T 세포에서 LSP1가 과별현되어 유발되는 암의 예방 또는 치료용 약제학적 조성물.
- LSP1 발현 억제제 또는 LSP1 유전자가 결실된 T 세포를 유효성분으로 포함하는 항암제 활성 증진을 위한 항암 보조제.
- 제 16항에 있어서, 면역체크포인트 억제제를 추가로 포함하는, 항암 보조제.
- 대상으로부터 분리된 T 세포에서 LSP1의 발현 수준을 확인하는 것을 포함하는 암의 진단 또는 예후를 예측하기 위한 정보를 제공하는 방법.
- LSP1(Leukocyte-specific protein 1) 발현 억제제를 유효성분으로 포함하는 약학적 조성물을 개체에 투여하는 단계;를 포함하는 암의 예방 또는 치료 방법.
- LSP1 유전자가 결실된 T 세포를 유효성분으로 포함하는 약학적 조성물을 개체에 투여하는 단계;를 포함하는 암의 예방 또는 치료 방법.
