Description

旋钮组件及圆管型吻合器 技术领域
本发明涉及医疗器械技术领域,具体涉及一种旋钮组件及圆管型吻合器。
背景技术
消化道疾病是人类高发的疾病之一,在治疗过程中,常使用圆管型吻合器代替医生的手工操作对消化道等生理组织进行吻合。圆管型吻合器是一种常见的外科手术器械,大多采用轴向内装订方式,在手术时对食管、胃、肠道等生理组织形成端对端的、或者端对侧的吻合,吻合时两段组织内敛收容于吻合器内,击发完成后在组织上形成圆形吻合口,重建了人体通道。
现有技术中,圆管型吻合器包括吻合器本体、活动连接所述吻合器本体的操作把手以及与所述本体配合的钉砧组件。所述吻合器本体包括设于远端的钉仓组件,所述钉仓组件包括环形钉仓、切刀;以及设于本体近端的旋钮,所述旋钮可相对于本体旋转,所述吻合器本体的内部还包括从近端向远端设置的钉砧轴。此处远端和近端是相对于操作者来说的,距离操作者较近的一端为近端,距离操作者较远的一端,即更靠近手术位置的一端为远端。在吻合器中,内侧与外侧是相对与吻合器的轴心来说的,靠近轴心的一侧为内侧,远离轴心的一侧为外侧。所述钉砧组件包括钉砧、设置在钉砧远端的钉砧帽和设置在钉砧内部的刀砧,钉砧组件可以与钉砧轴的远端直接连接或通过钉砧轴连接件连接。在肿瘤手术过程中,离断去除肿瘤组织后,钉砧轴从残端组织的荷包穿出,配置于吻合器本体的远端,旋转旋钮,使钉砧组件和钉仓之间的距离逐渐减小达到一个合适的距离后,达到可击发状态,才能握持操作把手击发器械完成吻合。随着医疗器械的不断发展,圆管型吻合器也越来越广泛地应用于痔疮等疾病的治疗。
在旋转旋钮,使钉砧组件向钉仓靠近的过程中,有时候组织闭合到位后,旋钮还是可以旋转的,这样钉砧组件与钉仓之间就会继续靠近,继续压缩组 织,对组织造成过度压榨,甚至可能会压碎组织,对手术结果十分不利。
发明内容
针对现有技术中的问题,本发明的目的在于提供一种旋钮组件及圆管型吻合器,旋转旋钮驱动件时,当旋钮本体所受的阻力达到阻力预设值后,旋钮组件将无法再传递更大的扭矩,此时的旋钮组件进入失效模式,钉砧组件和钉仓之间的间隙保持不变,而不会使得组织被过度压榨。
本发明实施例提供一种旋钮组件,用于圆管型吻合器,所述吻合器的内部设置有一丝杆;所述旋钮组件包括:
旋钮驱动件和旋钮本体,所述旋钮本体套设于所述丝杆的外部,所述旋钮本体旋转时带动丝杆沿吻合器的轴向移动;
第一配合件和第二配合件,所述第一配合件为弹性配合件,且所述第一配合件具有阻挡状态和让位状态,所述第一配合件处于阻挡状态时,所述第一配合件与第二配合件联动,所述第一配合件处于让位状态时,所述第一配合件与第二配合件不联动;
所述第一配合件设置于所述旋钮驱动件,且所述第二配合件设置于所述旋钮本体;或,所述第一配合件设置于所述旋钮本体,且所述第二配合件设置于所述旋钮驱动件。
可选地,所述旋钮驱动件受外力而旋转,且所述旋钮本体的旋转阻力大于预设阻力值时,所述第一配合件受到第二配合件的作用力产生变形而进入让位状态。
可选地,所述第一配合件为具有凸起部的弹片。
可选地,所述旋钮驱动件或所述旋钮本体上开设有弹片安装槽,所述弹片的两端安装于所述弹片安装槽中。

下面将参考附图并结合实施例,来详细说明本发明。
为了解决现有技术中的技术问题,本发明提供了一种用于圆管型吻合器的旋钮组件,所述吻合器的内部设置有一丝杆,所述旋钮组件包括选择性地联动的旋钮驱动件和旋钮本体,其中所述旋钮本体套设于所述丝杆的外部,所述旋钮本体旋转时带动丝杆沿吻合器的轴向移动。旋钮驱动件和旋钮本体上分别设置有两个配合件中的一个,两个配合件包括第一配合件和第二配合件。即,第一配合件和第二配合件分别设置于所述旋钮驱动件和旋钮本体,或者所述第一配合件和第二配合件分别设置于所述旋钮本体和旋钮驱动件。本发明进一步还提供了一种圆管型吻合器,包括所述旋钮组件。
其中,所述第一配合件为弹性配合件,且所述第一配合件具有阻挡状态和让位状态,所述第一配合件处于阻挡状态时,所述第一配合件与第二配合件联动,所述第一配合件处于让位状态时,所述第一配合件与第二配合件不联动。
当操作者沿第一方向旋转旋钮驱动件时,旋钮驱动件通过两个配合件传递旋转力矩,在旋钮本体所受的旋转阻力达到阻力预设值之前,第一配合件处于阻挡状态,即阻挡旋钮驱动件和旋钮本体之间的相对运动,旋钮驱动件 可通过两个配合件带动旋钮本体一起转动,即两个配合部处于联动状态,旋钮本体进一步通过丝杆带动钉砧组件沿吻合器的轴向移动而靠近钉仓组件。
随着钉砧组件逐渐靠近钉仓组件,当钉砧组件与钉仓组件之间到达合适的距离,同时对组织的压榨力达到合适的预定值时,再旋转旋钮,旋钮本体会受到阻碍第一方向旋转的阻力,此阻力即对应于钉砧组件和钉仓组件之间的组织压榨力,此时,只有在旋钮本体上施加大于该阻力的旋转扭矩才能沿第一方向转动旋钮本体。即,当旋钮本体所受的旋转阻力达到阻力预设值之后,第一配合件和第二配合件之间的相互作用力大于弹性配合件的克服弹性变形的力,第一配合件产生变形而进入让位状态,即不再阻挡旋钮驱动件和旋钮本体之间的相对运动,旋钮驱动件与旋钮本体不再联动,此时的旋钮组件进入失效模式,钉砧组件和钉仓之间的间隙保持不变,而不会使得组织被过度压榨。
下面结合附图对本发明两个实施例提供的圆管型吻合器和其中的旋钮组件进行详细介绍。其中,图1~8示出了本发明第一实施例的旋钮组件的结构,其中,第一配合件设置于旋钮驱动件,第二配合件设置于旋钮本体;图9~13示出了本发明第二实施例的旋钮组件的结构,其中,第一配合件设置于旋钮本体,第二配合件设置于旋钮驱动件。
如图1所示,示出了第一实施例的吻合器的结构。其中,吻合器包括吻合器本体和钉砧组件83,其中,吻合器本体包括吻合器壳体81、位于吻合器壳体81的远端的钉仓组件82、位于吻合器壳体81的近端的旋钮组件以及从近端向远端设置的丝杆6,钉仓组件82包括环形钉仓和切刀,钉砧组件83包括钉砧和钉砧轴,钉砧通过钉砧轴连接至丝杆的远端。旋钮组件旋转时, 依次通过所述丝杆6和钉砧轴带动钉砧组件83轴向移动,远离或靠近钉仓组件82。
当然,在另一类的圆管型吻合器中,例如前文提到的做痔疮手术的圆管型吻合器,其钉砧组件83是直接与吻合器本体连接的,钉砧组件83不能从吻合器本体上取下来,此时即钉砧组件83不包含钉砧轴,钉砧直接与丝杆的远端相连接。
如图2~8所示,所述旋钮组件包括旋钮本体1和旋钮驱动件。在该实施例中,旋钮驱动件为旋钮外壳2,操作者在操作旋钮组件时,直接接触旋钮外壳2,但本发明不限于此。所述旋钮本体1套设于所述丝杆6的外部,且旋钮本体1通过内部的本体内螺纹13与丝杆6配合。所述旋钮本体1沿第一方向旋转时可带动丝杆6朝吻合器的近端移动,并进一步带动钉砧组件83向钉仓组件82靠近。所述旋钮本体1沿第二方向旋转时,可带动丝杆6朝吻合器的远端移动,以使得钉砧组件83远离钉仓组件82。该实施例中,第一配合件设置于旋钮外壳2上,第二配合件设置于旋钮本体1上。其中,所述第一配合件为弹性配合件,且所述第一配合件具有阻挡状态和让位状态,所述第一配合件处于阻挡状态时,所述第一配合件与第二配合件联动,旋钮外壳2和旋钮本体1联动,所述第一配合件处于让位状态时,所述第一配合件与第二配合件不联动,旋钮外壳2和旋钮本体1不联动。
在该实施例中,第一配合件为具有凸起部211的弹片21。在旋钮外壳2的内侧面设置有弹片安装槽22,弹片21的第一端212和第二端213安装于所述弹片安装槽22中。弹片21可以为金属弹片或塑料弹片,或其他可以弹性变形且去除外力后可以恢复初始状态的结构。此处仅为弹性配合件的一种 设置方式,在其他替代的实施方式中,弹性配合件也可以采用弹簧和活动块配合的结构等等,均属本发明的保护范围之内。
所述第二配合件包括至少一个配合结构,所述配合结构为凸起或凹槽。在该实施例中,第二配合件包括多个凸起11,多个凸起11均匀分布在旋钮本体1的外侧面上。凸起11具有第一侧面111和第二侧面112,在旋钮外壳2朝向第一方向旋转时,凸起11的第一侧面111与弹片21接触,旋钮外壳2朝向第二方向旋转时,凸起11的第二侧面112与弹片21接触。
操作者操作旋钮组件时,首先施加外力使得旋钮外壳2受外力而旋转,旋钮外壳2与旋钮本体1通过两个配合件将外力向旋钮本体1传递,第一配合件会给第二配合件一个旋转作用力,如果此时旋钮本体1的旋转阻力小于预设阻力值,第二配合件给第一配合件的反作用力小于弹片21的克服弹性变形的力,弹片21不会发生变形,第一配合件处于阻挡状态,旋钮外壳2通过弹片21和凸起11的配合带动旋钮本体1一起旋转。操作者施加外力驱动旋钮外壳2旋转时,如果旋钮本体1的旋转阻力达到或大于预设阻力值,操作者需要施加更大的外力,此时凸起11给弹片21的反作用力大于弹片21的克服弹性变形的力,弹片21受到凸起11的作用力产生变形而进入让位状态,即凸起11和弹片21无法传递外力至旋钮本体1上,旋钮外壳2无法与旋钮本体1联动,旋钮外壳2进行的是无效的空转,并且可以发出凸起11和弹片21脱扣时的哒哒声。此时旋钮组件处于失效状态,钉砧组件83和钉仓组件82之间的钉成型间隙保持不变,组织不会被过度压榨。操作者如果听到哒哒声或者根据失效状态时旋转旋钮组件的手感可以判定出当前钉砧组件83已经移动到位,可以执行吻合器的击发操作。
在该实施例中,所述弹片21包括第一引导面214,所述旋钮外壳2朝向第一方向旋转时,所述弹片21的第一引导面214与所述凸起11接触。所述第一引导面214可以为弧面或斜面。所述弹片21还可以包括第二引导面215,所述旋钮外壳2朝向第二方向旋转时,所述弹片21的第二引导面215与所述凸起11接触,所述第二引导面215可以为弧面或斜面。
如图2所示,所述旋钮本体1的外侧面设置有第一凸台14,所述第一凸台14的近端侧还设置有第二凸台15,所述凸起11设置于第二凸台15。并且如图3和图4中示出的,第二凸台15和多个凸起11形成一个齿轮圆盘的结构。此处仅为一种实施方式,在其他替代的实施方式中,也可以只设置一个凸台,或者将第二凸台15设置于第一凸台14的远端侧等等,均属于本发明的保护范围之内。
在该实施例中,第二凸台15上凸起11的高度低于第一凸台14的高度;适应地,旋转壳体2的远端设有一台阶面与第一凸台14配合,以限定旋钮外壳在轴向上的运动;旋转壳体2的远端内侧上设置弹片21与凸起11配合。
进一步地,所述旋钮组件的近端还设置有旋钮后盖,所述旋钮后盖包括第一旋钮后盖5和第二旋钮后盖7,第一旋钮后盖5的远端设置有后盖内螺纹51,所述旋钮本体1的近端设置有与所述后盖内螺纹51相配合的本体外螺纹12,第一旋钮后盖5通过螺纹连接于旋钮本体1的近端配合,第二旋钮后盖7可以安装于第一旋钮后盖5的近端。
在图3和图4中的视角中,第一方向为顺时针方向,第二方向为逆时针方向。如图3和图4所示,在该实施例中,所述凸起11的第一侧面111朝向第一方向倾斜,使得旋钮外壳2在顺时针方向旋转且不与旋钮本体1联动时, 变形后的弹片21更容易越过凸起11。凸起11的第二侧面112为近似直角面或第二侧面112朝向第二方向倾斜,以阻碍旋钮外壳向第二方向旋转,以给医生以触觉反馈,提示医生旋转方向。此处凸起11的第二侧面112为近似直角面指的是第二侧面112基本上垂直于旋钮本体1的外侧面。如果凸起11的第二侧面112朝向第二方向倾斜,则第二侧面112的倾斜程度小于第一侧面111的倾斜程度。这样设置,可以使得在旋钮外壳2沿第二方向旋转时,弹片21不易越过凸起11,从而防止旋钮组件在沿第一方向旋紧之后,当需要反向操作时,无法带动旋钮本体1沿第二方向旋转增大钉仓组件82和钉砧组件83的距离。
如图9~13所示,示出了本发明第二实施例的旋钮组件的结构。第二实施例与第一实施例的区别在于:第一配合件设置于旋钮本体3,第二配合件设置于旋钮壳体4。
在图10的视角中,第一方向为逆时针方向,第二方向为顺时针方向。第一配合件为包括凸起部311的弹片31,第二配合件包括设置于旋钮壳体4的内侧面的多个凸起41。旋钮本体3沿第一方向旋转时,弹片31与凸起41的第一侧面411接触,旋钮本体3沿第二方向旋转时,弹片31与凸起41的第二侧面412接触。同样地,第一侧面411的倾斜程度大于第二侧面412的倾斜程度,以使得旋转外壳4在沿第一方向旋转时,旋钮本体3的旋转阻力大于阻力预设值时,弹片31更容易与凸起41脱扣,且保证在旋转外壳4沿第一方向旋紧后,可以再沿第二方向通过旋钮外壳4带动旋钮本体3旋转。
旋钮本体3的外侧面设置有弹片安装槽32,弹片31的两端312、313分别设置于所述弹片安装槽32中。弹片31包括第一引导面314和第二引导面 315。旋钮本体3沿第一方向旋转时,弹片31的第一引导面314与凸起41的第一侧面411接触,旋钮本体3沿第二方向旋转时,弹片31的第二引导面315与凸起41的第二侧面412接触。
在该实施例中,旋钮本体1上设置有第一凸台33和第二凸台34,第二凸台34设置在第一凸台33的近端侧。第一凸台33与旋钮外壳4的远端相作用以限制旋钮外壳的轴向运动。弹片安装槽32设置于第二凸台34的外表面。旋钮外壳4的远端与第一凸台33配合。
采用该第二实施例的旋钮组件,其工作原理与第一实施例的旋钮组件时类似。操作者操作旋钮组件时,首先施加外力使得旋钮外壳4旋转,旋钮外壳4通过两个配合件将外力向旋钮本体3传递,第一配合件会给第二配合件一个旋转作用力,如果此时旋钮本体3的旋转阻力小于预设阻力值,第二配合件给第一配合件的作用力小于弹片31的克服弹性变形的力,弹片31不会发生变形,第一配合件处于阻挡状态,旋钮外壳4通过弹片31和凸起41的配合带动旋钮本体3一起旋转。操作者施加外力驱动旋钮外壳4旋转时,如果旋钮本体3的旋转阻力达到或大于预设阻力值,操作者需要施加更大的外力,此时凸起41给弹片31的作用力大于弹片31的克服弹性变形的力,弹片31受到凸起41的作用力产生变形而进入让位状态,即凸起41和弹片31无法传递外力至旋钮本体3上,旋钮外壳4无法与旋钮本体3联动,旋钮外壳4进行的是无效的空转,并且可以发出凸起41和弹片31脱扣时的哒哒声。此时旋钮组件处于失效状态,钉砧组件83和钉仓组件82之间的钉成型间隙保持不变,组织不会被过度压榨。操作者如果听到哒哒声或者根据失效状态时旋转旋钮组件的手感可以判定出当前钉砧组件83已经移动到位,可以执行 吻合器的击发操作。
可以理解的是,无论是在第一实施例当中,还是在第二实施例当中,所述弹片的个数可以是两个或者多个,例如,当所述弹片为两个时,可以对称设置,以加强结构的稳定性。
本发明给出的两个实施例仅为示例,在实际应用中,还可以将两个实施例中的一些特征进行组合,得到新的技术方案。或者对其中的某个或某些特征进行变更,得到新的技术方案,均属于本发明的保护范围之内。例如,第一配合件可以不采用弹片的结构,而采用活动块和弹簧配合的结构,克服弹性变形的力的大小即为弹簧的弹簧力,第二配合件也可以不采用凸起,而采用凹槽的结构,且配合结构的数量和排布方式也可以根据需要选择。
本发明所提供的旋钮组件及圆管型吻合器具有下列优点:
本发明提供了一种用于圆管型吻合器的旋钮组件,将旋钮组件分为旋钮本体和旋钮驱动件,旋钮驱动件与旋钮本体选择性地联动,当旋钮本体所受的旋转阻力达到阻力预设值之前,旋钮驱动件可带动旋钮本体一起转动,旋钮本体可通过丝杆带动钉砧组件沿吻合器的轴向移动而靠近钉仓,当旋钮本体所受的旋转阻力达到阻力预设值之后,由于弹性配合件产生变形,旋钮驱动件无法驱使旋钮本体转动,此时的旋钮组件进入失效模式,钉砧组件和钉仓之间的间隙保持不变,而不会使得组织被过度压榨。
以上内容是结合具体的优选实施方式对本发明所作的进一步详细说明,不能认定本发明的具体实施只局限于这些说明。对于本发明所属技术领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干简单推演或替换,都应当视为属于本发明的保护范围。

Claims (17)

  1. 一种旋钮组件,其特征在于,用于圆管型吻合器,所述吻合器的内部设置有一丝杆;所述旋钮组件包括:
    旋钮驱动件和旋钮本体,所述旋钮本体套设于所述丝杆的外部,所述旋钮本体旋转时带动丝杆沿吻合器的轴向移动;
    第一配合件和第二配合件,所述第一配合件为弹性配合件,且所述第一配合件具有阻挡状态和让位状态,所述第一配合件处于阻挡状态时,所述第一配合件与第二配合件联动,所述第一配合件处于让位状态时,所述第一配合件与第二配合件不联动;
    所述第一配合件设置于所述旋钮驱动件,且所述第二配合件设置于所述旋钮本体;或,所述第一配合件设置于所述旋钮本体,且所述第二配合件设置于所述旋钮驱动件。
  2. 根据权利要求1所述的旋钮组件,其特征在于,所述旋钮驱动件受外力而旋转,且所述旋钮本体的旋转阻力大于预设阻力值时,所述第一配合件受到第二配合件的作用力产生变形而进入让位状态。
  3. 根据权利要求2所述的旋钮组件,其特征在于,所述第一配合件为具有凸起部的弹片。
  4. 根据权利要求3所述的旋钮组件,其特征在于,所述旋钮驱动件或所述旋钮本体上开设有弹片安装槽,所述弹片的两端安装于所述弹片安装槽中。
  5. 根据权利要求3所述的旋钮组件,其特征在于,所述弹片包括第一引导面,所述旋钮驱动件朝向第一方向旋转时,所述第一配合件的第一引导面 与所述第二配合件接触。
  6. 根据权利要求5所述的旋钮组件,其特征在于,所述第一引导面为弧面或斜面。
  7. 根据权利要求5所述的旋钮组件,其特征在于,所述弹片包括第二引导面,所述旋钮驱动件朝向第二方向旋转时,所述第一配合件的第二引导面与所述第二配合件接触。
  8. 根据权利要求7所述的旋钮组件,其特征在于,所述第二引导面为弧面或斜面。
  9. 根据权利要求1所述的旋钮组件,其特征在于,所述第二配合件包括至少一个配合结构,所述配合结构为凸起或凹槽。
  10. 根据权利要求9所述的旋钮组件,其特征在于,所述配合结构包括第一侧面,所述旋钮驱动件朝向第一方向旋转时,所述配合结构的第一侧面与所述第一配合件接触,所述第一侧面朝向第一方向倾斜。
  11. 根据权利要求10所述的旋钮组件,其特征在于,所述配合结构包括第二侧面,所述旋钮驱动件朝向第二方向旋转时,所述配合结构的第二侧面与第一配合件接触。
  12. 根据权利要求11所述的旋钮组件,其特征在于,所述配合结构的第二侧面为直角面,或所述配合结构的第二侧面朝向第二方向倾斜,且所述配合结构的第二侧面的倾斜程度小于配合结构的第一侧面的倾斜程度。
  13. 根据权利要求1所述的旋钮组件,其特征在于,所述旋钮驱动件包括旋钮外壳,所述旋钮驱动件的内侧面设置有所述第一配合件或第二配合件。
  14. 根据权利要求13所述的旋钮组件,其特征在于,所述旋钮本体的外 侧面设置有第一凸台,所述第一配合件设置于所述旋钮外壳的内侧面,所述第二配合件设置于所述第一凸台的近端侧或远端侧;或
    所述第二配合件设置于所述旋钮外壳的内侧面,所述第一配合件设置于所述第一凸台的近端侧或远端侧。
  15. 根据权利要求14所述的旋钮组件,其特征在于,所述旋钮本体的外侧面还设置有第二凸台,所述第二凸台位于第一凸台的近端侧;
    所述第二凸台上设置有所述第一配合件或第二配合件,且设置于所述第二凸台的第一配合件或第二配合件的高度低于所述第一凸台的高度,所述旋钮本体的远端与所述第一凸台相配合。
  16. 根据权利要求1所述的旋钮组件,其特征在于,还包括位于所述旋钮本体的近端的旋钮后盖,所述旋钮后盖的远端设置有后盖内螺纹,所述旋钮本体的近端设置有与所述后盖内螺纹相配合的本体外螺纹。
  17. 一种圆管型吻合器,其特征在于,包括权利要求1至16中任一项所述的旋钮组件。
