Abstract
拡張体と生体組織との位置ずれを抑制し、目標部位に対して正確にエネルギーを付与できる医療デバイス及び処置方法を提供する。 長尺なシャフト部(20)と、シャフト部(20)の先端部に設けられ径方向に拡縮可能な拡張体(21)を有する医療デバイス(10)であって、拡張体(21)は、生体組織を把持する基端側把持部(51a)及び先端側把持部(51b)を有する把持部(51)と、把持部(51)を把持方向に開閉させる可動部(52)と、を有する医療デバイス(10)である。
Description
本発明は、生体組織にエネルギーを付与する維持処置要素を備える医療デバイス、及び生体組織にエネルギーを付与する処置方法に関する。
心臓疾患の一つとして、慢性心不全が知られている。慢性心不全は、心機能の指標に基づいて収縮不全と拡張不全に大別される。拡張不全に罹患した患者は、心筋が肥大化してスティッフネス(硬さ)が増すことで、左心房の血圧が高まり、心臓のポンプ機能が低下する。これにより、患者は、肺水腫などの心不全症状を呈することとなる。また、肺高血圧症等により右心房側の血圧が高まり、心臓のポンプ機能が低下することで心不全症状を呈するような心臓疾患もある。
近年、これらの心不全患者に対し、上昇した心房圧の逃げ道となるシャント(貫通孔)を心房中隔に形成し、心不全症状の緩和を可能にするシャント治療が注目されている。シャント治療は、経静脈アプローチで心房中隔にアクセスし、所望のサイズの貫通孔を形成する。このような心房中隔に対するシャント治療を行うための医療デバイスとして、例えば特許文献1に挙げるようなものがある。
特許文献1の医療デバイスは、シャフト部の先端部に設けられる拡張体であるバルーンにより、シャント孔を大きくし、バルーンに設けた電極によって、シャント孔を維持する。しかし、この医療デバイスは、電極などの維持処置要素を生体組織に固定した状態で、シャント孔の拡張を行うことができない。そのため、拡張の際に、維持処置要素と生体組織の位置ずれを生じる可能性があり、それによって、治療効果が低減する可能性がある。
本発明は、上述した課題を解決するためになされたものであり、拡張体と生体組織との位置ずれを抑制し、目標部位に対して正確にエネルギーを付与できる医療デバイス及び処置方法を提供することを目的とする。
上記目的を達成する本発明に係る医療デバイスは、長尺なシャフト部と、前記シャフト部の先端部に設けられ径方向に拡縮可能な拡張体と、を有し、前記拡張体は、生体組織を把持する基端側把持部及び先端側把持部を有する把持部と、該把持部を把持方向に開閉させる可動部と、を有する。
上記目的を達成する本発明に係る処置方法は、径方向に拡縮可能な拡張体を有する医療デバイスを用いて、生体組織の貫通孔を拡張させる処置方法であって、前記拡張体が有する把持部を生体組織の貫通孔に位置決めするステップと、前記把持部により生体組織を前記貫通孔の両側から把持するステップと、前記拡張体を拡張させて前記貫通孔の径を大きくするステップと、前記把持部が有する維持処置要素により維持処置を行うステップと、を有する。
上記のように構成した医療デバイスは、把持方向に開閉可能な把持部により、生体組織を両側から把持するので、拡張体の位置ずれを抑制することができる。
上記のように構成した処置方法は、これにより、把持部で生体組織を把持した状態で維持処置が行われるので、維持処置要素の位置ずれを抑制しつつ、目標部位に対して正確にエネルギーを付与することができる。
以下、図面を参照して、本発明の実施の形態を説明する。なお、図面の寸法比率は、説明の都合上、誇張されて実際の比率とは異なる場合がある。また、本明細書では、医療デバイス10の生体内腔に挿入する側を「先端」若しくは「先端側」、操作する手元側を「基端」若しくは「基端側」と称することとする。
以下の実施形態における医療デバイスは、患者の心臓Hの心房中隔HAに形成された貫通孔Hhを拡張し、さらに拡張した貫通孔Hhをその大きさに維持する維持処置を行うことができるように構成されている。
[第1実施形態の医療デバイス]
図1に示すように、本実施形態の医療デバイス10は、長尺なシャフト部20と、シャフト部20の先端部に設けられる拡張体21と、シャフト部20の基端部に設けられる操作部23とを有している。拡張体21には、前述の維持処置を行うための維持処置要素(エネルギー伝達要素)22が設けられる。
図1に示すように、本実施形態の医療デバイス10は、長尺なシャフト部20と、シャフト部20の先端部に設けられる拡張体21と、シャフト部20の基端部に設けられる操作部23とを有している。拡張体21には、前述の維持処置を行うための維持処置要素(エネルギー伝達要素)22が設けられる。
シャフト部20は、先端部に拡張体21を保持しているアウターシャフト31と、アウターシャフト31を収納する収納シース30と、を有している。収納シース30は、アウターシャフト31に対して軸方向に進退移動可能である。収納シース30は、シャフト部20の先端側に移動した状態で、その内部に拡張体21を収縮状態で収納することができる。拡張体21を収納した状態から、収納シース30を基端側に移動させることで、拡張体21を露出させることができる。
アウターシャフト31の内部には、牽引シャフト33が収納されている。牽引シャフト33は、アウターシャフト31の先端から先端側に突出しており、その先端部が先端部材35に固定されている。牽引シャフト33の基端部は、操作部23より基端側に導出されている。牽引シャフト33の先端部が固定されている先端部材35は、拡張体21には固定されていなくてよい。これにより、先端部材35は、拡張体21を圧縮方向に牽引することが可能である。また、拡張体21を収納シース30に収納する際、先端部材35を拡張体21から先端側に離すことによって、拡張体21の延伸方向への移動が容易になり、収納性を向上させることができる。
操作部23は、術者が把持する筐体40と、術者が回転操作可能な操作ダイヤル41と、操作ダイヤル41の回転に連動して動作する変換機構42とを有している。牽引シャフト33は、操作部23の内部において、変換機構42に保持されている。変換機構42は、操作ダイヤル41の回転に伴い、保持する牽引シャフト33を軸方向に沿って進退移動させることができる。変換機構42としては、例えばラックピニオン機構を用いることができる。
拡張体21についてより詳細に説明する。図2に示すように、拡張体21は、周方向に複数の線材を有している。本実施形態において拡張体21の線材は、周方向に4本が設けられており、それぞれ径方向に拡縮可能である。拡張体21の線材は、基端部から先端側に向かって拡径方向に延びる基端側拡張部56と、先端部から基端側に向かって拡径方向に延びる先端側拡張部57とを有している。基端側拡張部56の先端部には基端側把持部51aが、先端側拡張部57の基端部には先端側把持部51bが、それぞれ設けられ、基端側把持部51aと先端側把持部51bとで、生体組織を把持可能な把持部51を形成している。基端側拡張部56の基端部は、アウターシャフト31の先端部に固定されている。先端側拡張部57の先端部55は、拡張体21の自己拡張に伴い、先端部材35より基端側に位置している。
図3に示すように、拡張体21の軸方向中央部には、軸方向に対向する基端側把持部51a及び先端側把持部51bと、これらを連結する可動部52とで、凹状部53が形成されている。可動部52は、把持部51より細径状の曲がり部54を有している。曲がり部54は、把持部51より細径状であることで、把持部51よりも曲げ強度が小さい。このため、曲がり部54は、拡張体21の拡張時において曲がりやすく、把持部51を扇状に開閉させることができる。
拡張体21を形成する線材は、例えば、円筒から切り出した平板形状を有する。拡張体21を形成する線材は、厚み50~500μm、幅0.3~2.0mmとすることができる。ただし、この範囲外の寸法を有していてもよい。また、線材はその他にも円形の断面形状や、それ以外の断面形状を有していてもよい。
基端側把持部51aには、維持処置要素22が設けられている。把持部51が心房中隔HAを把持する際には、基端側把持部51aは右心房側に位置するので、維持処置要素22からのエネルギーは、心房中隔HAに対して右心房側から伝達される。ただし、維持処置要素22は、先端側把持部51bに設けられていてもよいし、基端側把持部51aと先端側把持部51bの両方に設けられていてもよい。維持処置要素22は、把持部51の表面から突出した凸部に配置されることが望ましい。
維持処置要素22は、例えば、外部装置であるエネルギー供給装置(図示しない)から電気エネルギーを受けるバイポーラ電極で構成される。この場合、把持部51に配置された維持処置要素22間で通電がなされる。維持処置要素22とエネルギー供給装置とは、絶縁性被覆材で被覆された導線(図示しない)により接続される。導線は、シャフト部20及び操作部23を介して外部に導出され、エネルギー供給装置に接続される。
維持処置要素22は、他にも、モノポーラ電極として構成されていてもよい。この場合、体外に用意される対極板との間で通電がなされる。また、維持処置要素22は、エネルギー供給装置から高周波の電気エネルギーを受給して発熱する発熱素子(電極チップ)でもよい。さらに、維持処置要素22は、マイクロ波エネルギー、超音波エネルギー、レーザー等のコヒーレント光、加熱した流体、冷却された流体、化学的な媒体により加熱や冷却作用を及ぼすもの、摩擦熱を生じさせるもの、電線等を備えるヒーター等のように、貫通孔Hhに対してエネルギーを付与可能なエネルギー伝達要素により構成することができ、具体的な形態は特に限定されない。
拡張体21を形成する線材は、金属材料で形成することができる。この金属材料としては、例えば、チタン系(Ti-Ni、Ti-Pd、Ti-Nb-Sn等)の合金、銅系の合金、ステンレス鋼、βチタン鋼、Co-Cr合金を用いることができる。なお、ニッケルチタン合金等のバネ性を有する合金等を用いるとよりよい。ただし、拡張体21の材料はこれらに限られず、その他の材料で形成してもよい。
シャフト部20は、アウターシャフト31の内部にインナーシャフト32を有しており、インナーシャフト32の内部に牽引シャフト33が収納されている。牽引シャフト33及び先端部材35には、軸方向に沿ってガイドワイヤルーメンが形成されており、ガイドワイヤ11を挿通させることができる。
シャフト部20の収納シース30、アウターシャフト31、インナーシャフト32は、ある程度の可撓性を有する材料により形成されるのが好ましい。そのような材料としては、例えば、ポリエチレン、ポリプロピレン、ポリブテン、エチレン-プロピレン共重合体、エチレン-酢酸ビニル共重合体、アイオノマー、あるいはこれら二種以上の混合物等のポリオレフィンや、軟質ポリ塩化ビニル樹脂、ポリアミド、ポリアミドエラストマー、ポリエステル、ポリエステルエラストマー、ポリウレタン、ポリテトラフルオロエチレン等のフッ素樹脂、ポリイミド、PEEK、シリコーンゴム、ラテックスゴム等が挙げられる。
牽引シャフト33は、例えば、ニッケル-チタン合金、銅-亜鉛合金等の超弾性合金、ステンレス鋼等の金属材料、比較的剛性の高い樹脂材料などの長尺状の線材に、ポリ塩化ビニル、ポリエチレン、ポリプロピレン、エチレン-プロピレン共重合体などの樹脂材料を被覆したもので形成することができる。
先端部材35は、例えば、ポリオレフィン、ポリ塩化ビニル、ポリアミド、ポリアミドエラストマー、ポリウレタン、ポリウレタンエラストマー、ポリイミド、フッ素樹脂等の高分子材料またはこれらの混合物、あるいは2種以上の高分子材料の多層チューブ等で形成することができる。
図3に示すように、収納シース30の外部に露出した拡張体21は、その自己拡張力によって、径方向に拡張した状態となる。凹状部53は、拡張体21の拡張時において、把持部51が扇状に開いた状態(図4(a))から、曲がり部54が屈曲して把持部51が把持方向に閉じた状態(図4(b))に変形する。拡張体21を収縮状態で収納する収納シース30は、把持部51を開いた状態に維持する規制要素であり、規制要素を解除、すなわち、収納シース30の外部に拡張体21を露出させることにより、把持部51が把持方向に閉じることができる。後述するように、拡張体21は、凹状部53が心房中隔HAに位置決めされた状態で拡張するので、把持部51が把持方向に閉じることにより、心房中隔HAを両側から把持することができる。
図5に示すように、牽引シャフト33を基端側に移動させることにより、先端部材35が基端側に移動し、これによって拡張体21の先端部55を基端側に移動させる。これに伴い、拡張体21は径方向にさらに拡張する。この状態から、牽引シャフト33を先端側に移動させることにより、拡張体21は図3の状態に戻ることができる。
[第1実施形態の医療デバイスによる処置方法]
医療デバイス10を使用した処置方法について説明する。本実施形態の処置方法は、一例として、心不全(左心不全)に罹患した患者に対して行われる。より具体的には、図7に示すように、心臓Hの左心室の心筋が肥大化してスティッフネス(硬さ)が増すことで、左心房HLaの血圧が高まる慢性心不全に罹患した患者に対して行われる処置の方法である。
医療デバイス10を使用した処置方法について説明する。本実施形態の処置方法は、一例として、心不全(左心不全)に罹患した患者に対して行われる。より具体的には、図7に示すように、心臓Hの左心室の心筋が肥大化してスティッフネス(硬さ)が増すことで、左心房HLaの血圧が高まる慢性心不全に罹患した患者に対して行われる処置の方法である。
図6に示すように、本実施形態の処置方法は、心房中隔HAに貫通孔Hhを形成するステップ(S1-1)と、貫通孔Hhに拡張体21を配置するステップ(S1-2)と、拡張体21によって心房中隔HAを把持するステップ(S1-3)と、拡張体21によって貫通孔Hhの径を拡張させるステップ(S1-4)と、貫通孔Hh付近における血行動態を確認するステップ(S1-5)と、貫通孔Hhの大きさを維持するための維持処置を行うステップ(S1-6)と、維持処置が施された後の貫通孔Hh付近における血行動態を確認するステップ(S1-7)と、を有している。
術者は、貫通孔Hhの形成に際し、ガイディングシース211及びダイレータ212が組み合わされたイントロデューサ210を心房中隔HA付近まで送達する。イントロデューサ210は、例えば、下大静脈Ivを介して右心房HRaに送達することができる。また、イントロデューサ210の送達は、ガイドワイヤ11を使用して行うことができる。術者は、ダイレータ212にガイドワイヤ11を挿通し、ガイドワイヤ11に沿わせて、イントロデューサ210を送達させることができる。なお、生体に対するイントロデューサ210の挿入、ガイドワイヤ11の挿入等は、血管導入用のイントロデューサを用いるなど、公知の方法で行うことができる。
図8に示すように、S1-1のステップにおいて、術者は、右心房HRa側から左心房HLa側に向かって、穿刺デバイス200を貫通させ、貫通孔Hhを形成する。穿刺デバイス200としては、例えば、先端が尖ったワイヤ等のデバイスを使用することができる。穿刺デバイス200は、ダイレータ212に挿通させて心房中隔HAまで送達する。穿刺デバイス200は、ダイレータ212からガイドワイヤ11を抜去した後、ガイドワイヤ11に代えて心房中隔HAまで送達することができる。
S1-2のステップにおいては、まず、図9に示すように、予め挿入されたガイドワイヤ11に沿って、医療デバイス10を心房中隔HA付近に送達する。このとき、医療デバイス10の先端部は、心房中隔HAを貫通して、左心房HLaに達するようにする。また、医療デバイス10の挿入の際、拡張体21は、収納シース30に収納された状態となっている。
次に、図10に示すように、拡張体21を先端側に移動させることにより、拡張体21の先端側の部分を左心房HLa内に露出させる。これにより、拡張体21のうち先端側拡張部57は、径方向に拡張する。ここで、拡張体21の先端側把持部51bを心房中隔HAの左心房HLa側の面に押し当てる。これにより、拡張体21の位置決めがなされる。
次に、図11に示すように、収納シース30を基端側に移動させることにより、拡張体21の全体を露出させる。これにより、拡張体21のうち基端側拡張部56が、右心房HRa内で径方向に拡張する。このとき、前述のように、凹状部53では、可動部52が曲がり部54によって屈曲変形し、基端側把持部51aと先端側把持部51bとによって心房中隔HAが両側から把持されることで、S1-3のステップが行われる。S1-3のステップにおいて、可動部52は収納シース30の外径より大きく拡張せず、貫通孔Hhをほぼ拡張させない。すなわち、S1-3のステップでは、拡張体21のうち、先端側拡張部57と基端側拡張部56とが径方向に拡張し、可動部52が径方向にほぼ拡張しない拡張体21の第1次拡張が行われることで、把持部51で心房中隔HAが把持される。
S1-4のステップにおいて、術者は、把持部51によって心房中隔HAが把持された状態で、図12に示すように、操作部23を操作することで、牽引シャフト33を基端側に移動させる。牽引シャフト33の先端部に設けられた先端部材35により、拡張体21は径方向にさらに拡張する。この際には、拡張体21の基端側拡張部56と先端側拡張部57及び可動部52が、全て径方向に拡張する。この拡張体21の第2次拡張により、把持した貫通孔Hhを径方向に押し広げる。このように、把持部51によって心房中隔HAを両側から把持した状態で、径方向に対する拡張動作を行うことにより、心房中隔HAの貫通孔Hhを効果的に拡径させることができる。
貫通孔Hhを拡張させたら、S1-5のステップにおいて血行動態の確認を行う。術者は、図7に示すように、下大静脈Iv経由で右心房HRaに対し、血行動態確認用デバイス220を送達する。血行動態確認用デバイス220としては、例えば、公知のエコーカテーテルを使用することができる。術者は、血行動態確認用デバイス220で取得されたエコー画像を、ディスプレイ等の表示装置に表示させ、その表示結果に基づいて貫通孔Hhを通る血液量を確認することができる。
また、血行動態の確認は、各部の圧力測定によっても可能である。シャフト部20のガイドワイヤルーメンを通じて、左心房側の圧力測定を行うことができ、同時に、収納シース30とアウターシャフト31との隙間を通じて右心房側の圧力測定を行うことができる。圧力測定は、既知の圧力測定装置をシャフト部20に対して接続することで行うことができる。このように、圧力測定を行うことで、左心房の圧力が低下したことや、右心房の圧力が上がりすぎないことを確認できる。これにより、シャント治療の効果向上と、貫通孔Hhを大きくしすぎるリスクの低減を図ることができる。
次に、S1-6のステップにおいて、術者は、貫通孔Hhの大きさの維持するために維持処置を行う。維持処置では、維持処置要素22を通して貫通孔Hhの縁部に高周波エネルギーを付与することにより、貫通孔Hhの縁部を高周波エネルギーによって焼灼(加熱焼灼)する。維持処置要素22を通して貫通孔Hhの縁部付近の生体組織が焼灼されると、縁部付近には生体組織が変性した変性部が形成される。変性部における生体組織は弾性を失った状態となるため、貫通孔Hhは拡張体21により押し広げられた際の形状を維持できる。
維持処置要素22が配置された把持部51は、前述のように心房中隔HAを把持しており、その状態で維持処置が行われる。このため、維持処置の際に維持処置要素22の位置ずれを防止することができる。また、把持部51と可動部52との曲げ強度の差を調整することで、維持処置要素22の生体組織に対する押圧力及び接触面積をコントロールできる。これにより、焼灼のばらつきを小さくでき、確実な維持処置が可能となる。
維持処置要素22は、把持部51の凸部に配置される。このため、把持部51が心房中隔HAに押し付けられることで、維持処置要素22が生体組織に埋没した状態で、維持処置が行われる。これにより、維持処置時に維持処置要素22が血液に触れないようにし、電流が血液に漏洩して血栓等を生じることを抑制できる。
維持処置後には、S1-7のステップにおいて再度血行動態を確認し、貫通孔Hhを通る血液量が所望の量となっている場合、術者は、拡張体21を縮径させ、収納シース30に収納した上で、貫通孔Hhから抜去する。さらに、医療デバイス10全体を生体外に抜去し、処置を終了する。
[第2実施形態の医療デバイス]
次に、第2の実施形態の医療デバイス15について説明する。本実施形態の医療デバイス15は、拡張体100以外の構成については、第1の実施形態と同じであり、共通部分については説明を省略する。図13に示すように、拡張体100は、周方向に複数の線材を有し、この線材は、基端側拡張部106と先端側拡張部107及び可動部103を有している。拡張体100は、軸方向の中央部に凹状部104を有している。凹状部104は、基端側把持部102a及び先端側把持部102bを有する把持部102と、把持部102間に設けられる可動部103とで形成されている。例えば、可動部103には、把持部102より大径状で、把持部102より曲げ強度が大きい曲がり部105が設けられている。このため、拡張体100は、把持部102が大きく開口し、可動部103が牽引シャフト33から収納シース30の外径方向に離れるように形状付けされている。
次に、第2の実施形態の医療デバイス15について説明する。本実施形態の医療デバイス15は、拡張体100以外の構成については、第1の実施形態と同じであり、共通部分については説明を省略する。図13に示すように、拡張体100は、周方向に複数の線材を有し、この線材は、基端側拡張部106と先端側拡張部107及び可動部103を有している。拡張体100は、軸方向の中央部に凹状部104を有している。凹状部104は、基端側把持部102a及び先端側把持部102bを有する把持部102と、把持部102間に設けられる可動部103とで形成されている。例えば、可動部103には、把持部102より大径状で、把持部102より曲げ強度が大きい曲がり部105が設けられている。このため、拡張体100は、把持部102が大きく開口し、可動部103が牽引シャフト33から収納シース30の外径方向に離れるように形状付けされている。
曲がり部105は、把持部102より曲げ強度が大きいので、拡張体100が収納シース30から露出して径方向に拡張する際に、曲がり部105はあまり曲がらないまま、拡張体100が径方向に大きく拡大する。これにより、図13に示すように、心房中隔HAの貫通孔Hhに配置された拡張体100は、基端側拡張部106と、先端側拡張部107、及び可動部103の全てが径方向に拡張し、貫通孔Hhの径を拡張させる。貫通孔Hhの径を拡張させた後に、牽引シャフト33を基端側に移動させることにより、図14に示すように、曲がり部105が屈曲し、把持部102が把持方向に閉じて、心房中隔HAを両側から把持する。
このように、可動部103により把持部102を扇状に開閉させる場合には、把持部102と可動部103の曲げ強度のバランスを変化させることにより、拡張と把持の順番を変えることができる。
また、把持部と可動部の曲げ強度のバランスを調整することにより、拡張体の拡張動作と把持動作を同時に行わせるようにすることもできる。この場合、拡張体を貫通孔Hhで拡張させることにより、貫通孔Hhを両側から把持しつつ、同時に貫通孔Hhの径を拡大させることができる。
[第2実施形態の医療デバイスによる処置方法]
本実施形態の医療デバイス15を使用した処置方法について説明する。図15に示すように、本実施形態の処置方法は、心房中隔HAに貫通孔Hhを形成するステップ(S2-1)と、貫通孔Hhに拡張体100を配置するステップ(S2-2)と、拡張体100によって貫通孔Hhの径を拡張させるステップ(S2-3)と、拡張体100によって心房中隔HAを把持するステップ(S2-4)と、を有する。ここで、S2-1~S2-2のステップは、第1の実施形態におけるS1-1~S1-2のステップと同様であるので、説明を省略する。
本実施形態の医療デバイス15を使用した処置方法について説明する。図15に示すように、本実施形態の処置方法は、心房中隔HAに貫通孔Hhを形成するステップ(S2-1)と、貫通孔Hhに拡張体100を配置するステップ(S2-2)と、拡張体100によって貫通孔Hhの径を拡張させるステップ(S2-3)と、拡張体100によって心房中隔HAを把持するステップ(S2-4)と、を有する。ここで、S2-1~S2-2のステップは、第1の実施形態におけるS1-1~S1-2のステップと同様であるので、説明を省略する。
また、本実施形態の処置方法は、S2-4の後、貫通孔Hh付近における血行動態を確認するステップ(S2-5)と、追加拡張の必要性を判断するステップ(S2-6)と、S2-6で追加拡張が必要と判断された場合に、貫通孔Hhの追加拡張を行うステップ(S2-7)と、貫通孔Hhの大きさを維持するための維持処置を行うステップ(S2-8)と、維持処置が施された後の貫通孔Hh付近における血行動態を確認するステップ(S2-9)と、を有している。ここで、S2-8~S2-9のステップは、第1の実施形態におけるS1-6~S1-7のステップと同様であるので、説明を省略する。
S2-2のステップまでは、拡張体100は収納シース30内に収納された状態である。S2-3のステップにおいて、術者は、収納シース30を基端側に移動させ、拡張体100を収納シース30から露出させる。これにより、図13に示すように拡張体100は、基端側拡張部106と先端側拡張部107及び可動部103の全てが径方向に拡張して、貫通孔Hhの径を拡張させる。
貫通孔Hhの径を拡張させたら、S2-4のステップにおいて、術者は、牽引シャフト33を基端側に移動させる。これにより、可動部103の曲がり部105が屈曲して把持部102が把持方向に閉じる。これにより、図14に示すように把持部102が心房中隔HAを両側から把持する。
S2-5で血行動態を確認し、貫通孔Hhをさらに拡張する必要があると判断された場合、S2-7のステップにおいて追加拡張が行われる。追加拡張は、牽引シャフト33を図14の状態によりさらに基端側に移動させることにより、拡張体100の径をより大きくすることで、行われる。また、その状態から牽引シャフト33を先端側に移動させ、拡張体100の径を一旦小さくした上で、再度、牽引シャフト33を基端側に移動させ、拡張体100の径を大きくすることもできる。さらに、牽引シャフト33の進退移動を繰り返し、貫通孔Hhを少しずつ押し広げていくこともできる。
また、S2-2とS2-3の間に、プレ拡張のステップを行うこともできる。プレ拡張は、拡張体100の拡張力によって十分に拡張できないほど生体組織が硬くなっている場合に行われる。プレ拡張では、S2-3と同様に拡張体100を拡張させて貫通孔の径を拡大し、S2-4と同様に牽引シャフト33を基端側に移動させることで、把持部102により心房中隔HAを両側から把持する。さらに、S2-7と同様に牽引シャフト33をさらに基端側に移動させることで、拡張体100をさらに拡張させ、貫通孔Hhを押し広げる。この動作を1回または複数回行うことで、貫通孔Hhの周辺の生体組織を裂けさせ、その後の拡張を可能な状態とする。
本実施形態では、心房中隔HAの貫通孔Hhを把持部102で拡張させてから、把持部102で把持するようにしたが、前述のように、把持部102と可動部103の曲げ強度のバランスを調整することで、拡張体100の拡張に伴い、貫通孔Hhの拡大と把持を同時に行わせるようにしてもよい。
[可動部の変形例]
把持部を開閉させる可動部は様々な形態とすることができる。曲がり部54における曲げ強度を小さくするための形状として、第1の実施形態では把持部51より細径状としている。この他にも、図16(a)に示すように、スリット部54aを設けることができる。また、図16(b)に示すように、切欠き部54bを設けることができる。また、図16(c)に示すように、複数の小孔部54cを設けることができる。また、図16(d)に示すように、スリット部54aと切欠き部54bを両方設けることができる。また、図16(e)に示すように、周囲より柔らかい材料で形成された軟質部54dを設けることができる。また、周囲よりも厚みを薄くした部分を設けることで、可動部を形成することができる。
把持部を開閉させる可動部は様々な形態とすることができる。曲がり部54における曲げ強度を小さくするための形状として、第1の実施形態では把持部51より細径状としている。この他にも、図16(a)に示すように、スリット部54aを設けることができる。また、図16(b)に示すように、切欠き部54bを設けることができる。また、図16(c)に示すように、複数の小孔部54cを設けることができる。また、図16(d)に示すように、スリット部54aと切欠き部54bを両方設けることができる。また、図16(e)に示すように、周囲より柔らかい材料で形成された軟質部54dを設けることができる。また、周囲よりも厚みを薄くした部分を設けることで、可動部を形成することができる。
図17(a)に示すように、可動部112は、基端側把持部111aと先端側把持部111bの長さ方向中間部同士を連結してもよい。また、図17(b)に示すように、可動部117は、基端側把持部116aと先端側把持部116bとが互いに回動可能となるヒンジ部118を有していてもよい。ヒンジ部118は、回転軸部118aを有している。回転軸部118aには、把持部116が閉じる方向に付勢力を与えてもよい。これにより、把持部116によって心房中隔HAを弾性的に把持することができる。
可動部は、把持部を扇状に開閉させるものに限られず、シャフト部20の軸方向に沿って平行移動させるものであってもよい。図18(a)に示すように、拡張体120には、平行移動部122を設けることができる。平行移動部122は、先端側把持部121bから基端側把持部121aに向かって延びる延出部123を有し、基端側把持部121aには延出部123を保持するスライド保持部124が設けられる。延出部123は、スライド保持部124によって、軸方向に沿って移動することができる。これにより、把持部121間の距離を変化させることができ、把持部121を把持方向に開閉させることができる。延出部123のスライドは、例えば、延出部123に連結され手元部まで延びるワイヤ(図示しない)等を用いて行うことができる。
図18(b)に示すように、平行移動部126は、一方の把持部127をバネ等の弾性体128で支持した構造とすることもできる。これにより、弾性体128で支持された把持部127は、軸方向に沿って移動可能となる。この場合、弾性体128は、伸長方向に把持部127を付勢しており、把持部127または弾性体128に連結され手元部まで延びるワイヤ(図示しない)等を用いて、付勢力を規制することができる。また、収縮状態の拡張体125においては、弾性体128の付勢力が規制されるロック部(図示しない)を設け、拡張体125が径方向に拡張するのに伴い、ロック部が解除されるようにしてもよい。
図18(c)に示すように、平行移動部131は、一方の把持部132を移動用バルーン133で支持した構造とすることもできる。移動用バルーン133が拡縮することにより、把持部132は軸方向に沿って移動することができる。この場合、移動用バルーン133に拡張用流体を供給する追加のルーメン(図示しない)が設けられる。
[拡張体の変形例]
拡張体は、基端側と先端側とで分離していてもよい。図19に示すように、この拡張体140は、基端側拡張部141と先端側拡張部143との2つの独立した部材を有している。基端側拡張部141は基端側把持部142を有し、先端側拡張部143は先端側把持部144を有している。基端側拡張部141と先端側拡張部143は、ワイヤ等の連結部145によって連結されている。この場合に、図中矢印で示すように、牽引シャフト33を基端側に移動させることで、先端側拡張部143は基端側に移動し、先端側把持部144と基端側把持部142との距離が小さくなる。これによって、先端側把持部144と基端側把持部142とで生体組織を把持することができる。
拡張体は、基端側と先端側とで分離していてもよい。図19に示すように、この拡張体140は、基端側拡張部141と先端側拡張部143との2つの独立した部材を有している。基端側拡張部141は基端側把持部142を有し、先端側拡張部143は先端側把持部144を有している。基端側拡張部141と先端側拡張部143は、ワイヤ等の連結部145によって連結されている。この場合に、図中矢印で示すように、牽引シャフト33を基端側に移動させることで、先端側拡張部143は基端側に移動し、先端側把持部144と基端側把持部142との距離が小さくなる。これによって、先端側把持部144と基端側把持部142とで生体組織を把持することができる。
拡張体は、バルーンによって形成することもできる。この場合、図20(a)に示すように、シャフト部20の先端部にはバルーン150が設けられる。バルーン150は、軸方向の先端側と基端側の領域が、柔軟で拡張しやすい高コンプライアンス部151である。また、バルーン150の軸方向中央部の領域は、硬く拡張しにくい低コンプライアンス部152である。このようなバルーン150を拡張させると、図20(b)に示すように、高コンプライアンス部151の部分が拡張し、低コンプライアンス部152の部分はあまり拡張しない。これにより、高コンプライアンス部151の中央側の各表面が、低コンプライアンス部152を挟んで対向する。この対向した各面が、生体組織を把持する把持部153,153となる。
拡張体は、バルーンの拡張力によって拡張するようにしてもよい。この場合、図21(a)に示すように、シャフト部20の先端部には、バルーン160と、基端側拡張部161と、先端側拡張部163とが設けられる。基端側拡張部161は、基端側把持部162を有し、その先端部はバルーン160に接している。先端側拡張部163は、先端側把持部164を有し、その先端部はバルーン160に接している。バルーン160を拡張させると、図21(b)に示すように、基端側拡張部161と先端側拡張部163は、それぞれ径方向に拡張する。
拡張体は、線材とバルーンとの組み合わせであってもよい。この場合、図22(a)に示すように、シャフト部20の先端部には、バルーン172と基端側拡張部170とが設けられる。基端側拡張部170には、基端側把持部171が設けられる。バルーン172は、基端側の領域が、硬く拡張しにくい低コンプライアンス部174であり、先端側の領域が、柔軟で拡張しやすい高コンプライアンス部173である。バルーン172を拡張させると、図22(b)に示すように、バルーン172のうち拡張しやすい高コンプライアンス部173の部分が大きく拡張し、その基端側の表面が基端側拡張部170の基端側把持部171と対向する。この部分が、生体組織を把持する先端側把持部175となる。
[拡張体配置方法の変形例]
上述の実施形態では、拡張体21を拡張させる際に、まず先端側を拡張させ、その後に基端側を拡張させて心房中隔HAを把持する。これに対し、拡張体21の基端側を先に拡張させ、その後に先端側を拡張させて心房中隔HAを把持することもできる。この場合、図23(a)に示すように、拡張体21のうち、凹状部53のやや先端側を貫通孔Hh内に位置させた状態で、収納シース30を基端側に移動させ、拡張体21の全体を露出させる。これにより、拡張体21の凹状部53より先端側の部分は、貫通孔Hhにより拡張が抑えられ、拡張体21の凹状部53より基端側の部分は、径方向に拡張した状態となる。
上述の実施形態では、拡張体21を拡張させる際に、まず先端側を拡張させ、その後に基端側を拡張させて心房中隔HAを把持する。これに対し、拡張体21の基端側を先に拡張させ、その後に先端側を拡張させて心房中隔HAを把持することもできる。この場合、図23(a)に示すように、拡張体21のうち、凹状部53のやや先端側を貫通孔Hh内に位置させた状態で、収納シース30を基端側に移動させ、拡張体21の全体を露出させる。これにより、拡張体21の凹状部53より先端側の部分は、貫通孔Hhにより拡張が抑えられ、拡張体21の凹状部53より基端側の部分は、径方向に拡張した状態となる。
次に、図23(b)に示すように、拡張体21を先端側に移動させ、拡張体21の基端側把持部51aを心房中隔HAに対して押し当てる。これにより、拡張体21の先端側の部分は、左心房HLa内に進入する。これにより、拡張体21の先端側の部分が径方向に拡張し、図23(c)に示すように、基端側把持部51aと先端側把持部51bとにより、心房中隔HAが把持された状態となる。
また、拡張体21の基端側を先に拡張させ、その後に先端側を拡張させて心房中隔HAを把持する手法において、先端側シース36を用いてもよい。この場合、図24(a)に示すように、シャフト部20は、収納シース30と先端側シース36とを有している。先端側シース36は、収納シース30とは独立して軸方向に沿って移動可能とされている。拡張体21は、基端側が収納シース30に収納され、先端側が先端側シース36に収納されている。シャフト部20は、先端側シース36の基端部が貫通孔Hh内に位置するまで挿入される。
次に、図24(b)に示すように、収納シース30を基端側に移動させ、拡張体21の凹状部53より基端側の部分を露出させる。これにより、拡張体21の凹状部53より基端側の部分は、径方向に拡張する。続いて、図24(c)に示すように、拡張体21を先端側に移動させ、拡張体21の基端側把持部51aを心房中隔HAに対して押し当てる。その後、先端側シース36を先端側に向かって移動させる。先端側シース36を拡張体21の先端より先端側まで移動させることで、図24(d)に示すように、拡張体21の先端側の部分も径方向に拡張する。これによって、基端側把持部51aと先端側把持部51bとが心房中隔HAを把持する。
以上のように、上述の実施形態に係る医療デバイス10は、長尺なシャフト部20と、シャフト部20の先端部に設けられ径方向に拡縮可能な拡張体21と、を有し、拡張体21は、生体組織を把持する基端側把持部51a及び先端側把持部51bを有する把持部51と、把持部51を把持方向に開閉させる可動部52と、を有する。これにより、本実施形態の医療デバイス10は、把持方向に開閉可能な把持部51によって、生体組織を両側から把持するので、拡張体21の位置ずれを抑制することができる。
また、可動部52は両側の把持部51a,51bの間に設けられ、把持部51を扇状に開閉させるようにすれば、簡単な機構で把持部51を開閉させることができる。
また、可動部52は、把持部51と曲げ強度が異なる曲がり部54であるようにすれば、1本の線材で把持部51と可動部52を形成でき、簡易な構造とすることができる。
また、可動部117は、把持部116を互いに回動可能とする回転軸部118aを有するようにすれば、把持部116の可動性を高くでき、把持部116により生体組織をより確実に把持することができる。
また、可動部123は、対向する把持部127の少なくとも一方を軸方向に沿って移動させる平行移動部126を有するようにすれば、把持部127を平行に移動させることで、生体組織を把持しやすくすることができる。
また、シャフト部20は、拡張体21に接続され、拡張体21の先端部を拡張体21の基端部に対して軸方向に移動させる牽引シャフト33を有するようにすれば、牽引シャフト33を手元部で操作することで、拡張体の状態を容易に操作できる。
また、牽引シャフト33により拡張体21の先端部が基端部側に移動することで、把持部51が拡張方向に移動するようにすれば、牽引シャフト33の操作によって、拡張体21を拡径させ、貫通孔Hhの径を大きくすることができる。
また、牽引シャフト33により拡張体21の先端部が基端部側に移動することで、把持部51が把持方向に閉じるようにすれば、牽引シャフト33の操作によって、把持部51により生体組織を把持することができる。
また、把持部51を開いた状態に維持する規制要素30をさらに有し、規制要素30を解除することで、把持部51が把持方向に閉じるようにすれば、規制要素30の操作によって、把持部51による生体組織の把持を容易に行うことができる。
また、把持部51は、生体組織に対して維持処置を行う維持処置要素22を有するようにすれば、生体組織を把持した把持部51に維持処置要素22が配置されているので、維持処置要素22の位置ずれを抑制することができる。
また、上述の実施形態に係る処置方法は、径方向に拡縮可能な拡張体21を有する医療デバイス10を用いて、生体組織の貫通孔Hhを拡張させる処置方法であって、拡張体21が有する把持部51を生体組織の貫通孔Hhに位置決めするステップと、把持部51により生体組織を貫通孔Hhの両側から把持するステップと、拡張体21を拡張させて貫通孔Hhの径を大きくするステップと、把持部51が有する維持処置要素22により維持処置を行うステップと、を有する。これにより、把持部51で生体組織を把持した状態で維持処置が行われるので、維持処置要素の位置ずれを抑制しつつ、目標部位に対して正確にエネルギーを付与することができる。
また、把持部51により生体組織を把持するステップは、拡張体21を拡張させて貫通孔Hhの径を大きくするステップの前、または後、あるいは同時に行われるようにすれば、把持部51による生体組織の把持を任意のタイミングで行い、維持処置時の拡張体21の位置ずれを抑制できる。
なお、本発明は、上述した実施形態のみに限定されるものではなく、本発明の技術的思想内において当業者により種々変更が可能である。
拡張体21の可動部52は、把持部51が閉じる方向に予め形状付けられていてもよい。この場合、収納シース30を基端側に移動させ、拡張体21の全体を収納シース30の外部に露出させると、可動部52は、その形状付けによって、把持部51を閉じる方向に向かって自動的に移動させることができる。加えて、牽引シャフト33により、基端側の把持部51と先端側の把持部51との軸方向における距離を調整することができる。
また、可動部52は、医療デバイス10の中心軸方向に向かう生体組織の弾性力によって変形させるようにしてもよく、これによって、把持部51を移動させて生体組織を把持させることもできる。

前記拡張体は、生体組織を把持する基端側把持部及び先端側把持部を有する把持部と、該把持部を把持方向に開閉させる可動部と、を有する医療デバイス。 - 前記可動部は両側の前記把持部の間に設けられ、前記把持部を扇状に開閉させる請求項1に記載の医療デバイス。
- 前記可動部は、前記把持部と曲げ強度が異なる曲がり部である請求項2に記載の医療デバイス。
- 前記可動部は、前記把持部を互いに回動可能とする回転軸部を有する請求項2に記載の医療デバイス。
- 前記可動部は、対向する前記把持部の少なくとも一方を軸方向に沿って移動させる平行移動部を有する請求項1に記載の医療デバイス。
- 前記シャフト部は、前記拡張体に接続され、前記拡張体の先端部を前記拡張体の基端部に対して軸方向に移動させる牽引シャフトを有する請求項1~5のいずれか1項に記載の医療デバイス。
- 前記牽引シャフトにより前記拡張体の先端部が基端部側に移動することで、前記把持部が拡張方向に移動する請求項6に記載の医療デバイス。
- 前記牽引シャフトにより前記拡張体の先端部が基端部側に移動することで、前記把持部が把持方向に閉じる請求項6に記載の医療デバイス。
- 前記把持部を開いた状態に維持する規制要素を有し、該規制要素を解除することで、前記把持部が把持方向に閉じる請求項1~8のいずれか1項に記載の医療デバイス。
- 前記把持部は、生体組織に対して維持処置を行う維持処置要素を有する請求項1~9のいずれか1項に記載の医療デバイス。
- 径方向に拡縮可能な拡張体を有する医療デバイスを用いて、生体組織の貫通孔を拡張させる処置方法であって、
前記拡張体が有する把持部を生体組織の貫通孔に位置決めするステップと、
前記把持部により生体組織を前記貫通孔の両側から把持するステップと、
前記拡張体を拡張させて前記貫通孔の径を大きくするステップと、
前記把持部が有する維持処置要素により維持処置を行うステップと、
を有する処置方法。 - 前記把持部により生体組織を把持するステップは、前記拡張体を拡張させて前記貫通孔の径を大きくするステップの前、または後、あるいは同時に行われる請求項11に記載の処置方法。
