Abstract
본 발명은 인 흡수 능력이 뛰어난 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P 균주를 이용한 고인산혈증 신장질환 예방 또는 치료용 약학조성물 및 고인산혈증 신장질환 개선용 건강기능성 식품에 관한 것이다. 본 발명에 따르면, 본 발명에서 선별한 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P는 인 흡수 능력이 뛰어나 장내에서 인체 내로의 인 흡수를 우수하게 저해할 수 있다. 또한, 종래의 칼슘을 기반으로한 인 결합제를 대체할 수 있어, 혈중 칼슘 농도 증가로 인한 부작용을 최소화할 수 있고, 이로 인하여 고인산혈증 및 신장질환 합병증을 효과적으로 예방 또는 치료할 수 있다. 또한, 본 발명에서 선별한 락토바실러스 sp. KCCM 11826P는 기존의 인 결합제에 비하여 매우 저렴한 가격으로 복용이 가능하고, 균주의 특성상 요구르트, 분말 또는 환자의 영양식 등으로 개발이 가능하므로, 기존의 인 결합제보다 복용이 매우 편리하다.
Description
본 발명은 인 흡수 능력이 뛰어난 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P 균주를 이용한 고인산혈증 예방, 만성 신장질환 치료용 약학조성물 및 건강기능성 식품에 관한 것이다.
전 세계적인 노인 인구의 증가로 인하여 성인병인 당뇨병 및 고혈압 환자가 증가하는 추세이다. 이러한 당뇨병 및 고혈압은 만성신부전을 일으키는 가장 흔한 원인으로, 이로 인한 만성신부전 환자의 수 또한 급격하게 증가하고 있다. 우리나라 역시, 만성신부전의 유병률이 매년 증가하고 있으며, 특히 만 65세 이상의 노인의 경우 더 높은 유병률 증가를 보이고 있는 실정이다.
만성신부전 환자들은 일반인에 비하여 사망률이 매우 높으며, 생존율 또한 남자 65%, 여자 68% 정도로 매우 낮다. 만성 신부전 환자의 경우, 일반인에 비하여 혈관 석회화가 훨씬 심하게 진행되는 것으로 알려져 있으며, 이는 심혈관계 질환을 유발하는 중요한 요인 중 하나이다.
한편, 혈액투석 환자는 혈중 인 농도가 목표 수치 이상으로 증가함에 따라 혈관 석회화 및 사망률이 비례적으로 증가한다고 보고되었다. 따라서, 혈중 인 농도를 조절하는 것은 심혈관계 질환의 감소, 더 나아가 만성신부전 환자의 사망률을 낮추는데 매우 중요하다.
따라서, 만성신부전 환자의 심혈관계 질환 발생을 예방하기 위해서는, 약물 및 식이 조절 외에 추가적으로 '혈중 칼슘 및 인의 농도 조절 (5.5 mg/dL 이하)'이 필수적이다. 대부분의 만성신부전 환자들은 인 함유량이 낮은 음식을 먹도록 교육 받으며, 추가적으로 음식 속의 인이 몸으로 흡수되는 것을 막기 위해 식사 중에 인결합제를 복용한다. 하지만, 전체 혈액 투석환자의 40~50%의 환자만이 목표한 혈중 인 농도를 달성했다고 보고되었을 정도로, 혈중 인 농도 조절에 어려움을 겪고 있는 실정이다.
한편, 혈중 인 증가를 치료하기 위한 약들은 칼슘을 기반으로 하거나, 칼슘 기반의 인 결합제를 대체하는 형태로 판매되고 있다. 이 인 결합제들은 장내에서 음식 속의 인과 결합하여 대변으로 배출됨으로써 혈중 인 농도의 상승을 억제한다. 이들 중 칼슘제는 인의 혈중 농도를 제어하나, 칼슘의 혈중 농도를 증가시키는 단점이 있다. 이를 대체하기 위하여 세벨라머 탄산염 (sevelamer carbonate) 및 탄산란탄 (lanthanum carbonate)과 같은 인 결합제가 사용되고 있으나, 비싼 약제비, 낮은 인 결합력, 중금속으로 인한 잠재적 위험성 및 복용의 불편함과 같은 단점이 존재한다.
따라서, 기존 '칼슘 기반의 인 결합제의 대체'를 위한 약학조성물 및 건강기능성 식품에 대한 개발욕구가 절실한 실정이다.
이에, 본 발명자들은 기존의 칼슘 기반의 인 결합제를 대체할 수 있는 소재를 개발하고자 예의 노력하던 중, 여러 유산균들 가운데 가장 인 흡수 능력이 뛰어난 유산균을 선별하고 이를 바탕으로 본 발명을 완성하였다.
본 발명은 인체에 이로운 유산균 중 인 흡수 능력이 뛰어난 균주를 선별하여 이를 기존 인 결합제의 대체제로 사용함으로써, 고인산혈증의 합병증을 보이는 만성 신장질환 예방 또는 치료용 약학조성물 및 건강기능성 식품으로 개발하여 제공하고자 한다.
상기의 목적을 달성하기 위해 본 발명은 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P를 유효성분으로 포함하는 것을 특징으로 하는 고인산혈증 개선용 식품조성물을 제공한다.
또한, 본 발명의 고인산혈증 개선용 식품조성물에 있어서, 상기 고인산혈증은 바람직하게 신부전증으로 말미암은 것일 수 있다.
만성 신부전 환자의 사망률의 주요 원인 중 하나로 심뇌혈관질환이 주목받고 있다. 신장질환으로 인하여 신장이 인을 배설하는 역할을 제대로 하지 못하게 되면, 혈중 인 농도가 높아지게 되고, 증가한 인이 칼슘과 결합하여 칼슘-인 결합체를 형성하게 된다. 이렇게 형성된 칼슘-인 결합체는 심장판막, 대동맥, 관상동맥 등에 침착되어 심혈관계질환 및 사망을 야기하게 되는 것이다. 현재 이와 같은 혈관 석회화를 예방하기 위하여, 환자들은 인 결합제를 복용하고 있으나, 기존의 인 결합제는 칼슘을 기반으로 하기 때문에 인의 농도를 감소시킴과 동시에 칼슘 농도도 함께 증가시키게 된다. 이로 인하여 인과 칼슘이 서로 결합하여 칼슘-인 결합체를 형성함으로써 혈관 석회화를 야기할 뿐만 아니라, 부작용으로 고칼슘혈증 및 복부 팽만감 등의 소화기장애가 유발될 수 있다.
본 발명은 인 흡수 능력이 뛰어난 유산균인 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P를 선별해냄으로써, 칼슘을 기반으로 하는 인 결합제의 단점을 극복하고, 인 농도를 효과적으로 제어할 수 있는 기술을 개발하기에 이르렀다.
또한, 본 발명의 고인산혈증 개선용 식품조성물에 있어서, 식품조성물은 바람직하게 환자영양식, 육류, 곡류, 카페인 음료, 일반음료, 유제품, 초콜렛, 빵류, 스넥류, 과자류, 피자, 젤리, 면류, 껌류, 아이스크림류, 알코올성 음료, 술, 비타민 복합제 및 그 밖의 건강보조식품류 중 선택되는 어느 하나인 것이 좋다. 상기의 형태로 제조될 경우, 혈중 인 농도 증가로 인한 질병을 앓고 있는 환자가 편하고 쉽게 복용할 수 있다.
유산균은 우리가 기존에 손쉽게 섭취하던 요구르트, 유산균 발효유 등의 식품 형태로 다양하게 활용되어 왔으며, 유산균 중 하나인 락토바실러스는 우리나라 김치 등의 전통식품 발효에 중요한 역할을 하는 유산균으로, 인간의 면역기능 증진에 도움되는 것으로 알려져 있다. 따라서, 본 발명에서 선별한 유산균인 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P는 뛰어난 인 흡수 능력을 보유하고 있을 뿐만 아니라, 인체에 유익한 균주이고 다양한 식품군에 사용되어 질 수 있기 때문에, 고인산혈증 개선용 건강식품조성물로서 아주 적합하다고 할 수 있다.
한편, 본 발명은 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P을 유효성분으로 포함하는 것을 특징으로 하는 고인산혈증 예방 또는 치료용 약학조성물을 제공한다.
본 발명에서 선별한 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P는 이러한 기존의 인 결합제와는 달리, 칼슘을 기반으로 하지 않기 때문에 고칼슘혈증에 따른 질병의 발생이 없을 뿐만 아니라, 대량생산이 가능해 가격 면에서도 아주 저렴하게 이용할 수 있다.
또한, 본 발명의 고인산혈증 예방 또는 치료용 약학조성물에 있어서, 상기 고인산혈증은 바람직하게 신부전증으로 말미암은 것일 수 있다.
또한, 본 발명의 고인산혈증 예방 또는 치료용 약학조성물에 있어서, 약학 조성물은 바람직하게 과립제(GRANULES), 리모나데제(LEMONADES), 산제(POWDERS), 시럽제(SYRUPS), 액제(LIQUIDS AND SOLUTIONS), 엑스제(EXTRACTS), 엘릭실제(ELIXIRS), 유동엑스제(FLUIDEXTRACTS), 현탁제(SUSPESIONS), 전제(DECOCTIONS), 침제(INFUSIONS), 정제(TABLETS), 주정제(SPIRITS), 캅셀제(CAPSULES), 트로키제(TROCHES), 환제(PILLS), 연질 또는 경질 젤라틴 캅셀 중 선택되는 어느 하나인 것이 좋다. 상기의 제형으로 제조된 약학조성물은 함유된 성분을 인체 내로 잘 전달할 수 있을 뿐만 아니라 복용이 용이하다.
한편, 본 발명의 고인산혈증 예방 또는 치료용 약학조성물의 투여량은 투여방법, 복용자의 연령, 성별, 체중 및 질환의 중증도 등을 고려하여 결정하는 것이 좋다. 일 예로, 본 발명의 고인산혈증 예방 또는 치료용 약학조성물은 유효성분을 기준으로 하였을 때 1일 0.00001 내지 100 mg/kg(체중)으로 1회 이상 투여 가능하다. 그러나 상기의 투여량은 예시하기 위한 일 예에 불과하며, 복용자의 상태에 따라 의사의 처방에 의해 변화될 수 있다.
본 발명에 따르면, 본 발명에서 선별한 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P는 인 흡수 능력이 뛰어나 장내에서 인체내로의 인 흡수를 우수하게 저해할 수 있다. 즉, 본 발명의 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P를 섭취할 경우, 소장 내에서 식품 중 함유되어 있는 인을 흡수하여 배변으로 배출됨으로써 인의 농도를 효과적으로 조절할 수 있다.
또한, 본 발명은 종래의 칼슘을 기반으로한 인 결합제를 대체할 수 있어, 혈중 칼슘 농도 증가로 인한 부작용을 최소화할 수 있고, 이로 인하여 고인산혈증과 연관된 신장질환 합병증을 효과적으로 예방 또는 치료할 수 있다.
또한, 본 발명에서 선별한 락토바실러스 sp. KCCM 11826P는 기존의 인 결합제에 비하여 매우 저렴한 가격으로 복용이 가능하고, 균주의 특성상 요구르트, 분말 또는 환자의 영양식 등으로 개발이 가능하므로, 기존의 인 결합제보다 복용이 매우 편리하다.
또한, 본 발명의 유산균인 락토바실러스 sp. KCCM 11826P는 장 내에 비교적 오랜 시간 동안 머무를 수 있기 때문에, 기존의 인 결합제에 비하여 복용 간격이 길고, 적은 복용 횟수로도 장내 인 농도를 효과적으로 조절할 수 있다.

이하, 본 발명의 내용을 하기 실시예를 통해 더욱 상세히 설명하고자 한다. 다만, 본 발명의 권리범위가 하기 실시예에만 한정되는 것은 아니고, 그와 등가의 기술적 사상의 변형까지를 포함한다.
[실시예 1 : 인 흡수 능력이 뛰어난 균주 선별]
(1) 전통 발효 식품에서의 인 흡수 능력이 뛰어난 균주 분리
본 실시예에서는 여러 가지 된장, 고추장, 간장, 젓갈, 누룩, 식해 등의 전통 발효 식품에서 인 흡수 능력이 뛰어난 균주를 분리하고자 하였다.
전통 발효 식품을 인산 완충 식염수 (Phosphate bufferd saline, PBS)에 녹인 후, 고체 TSA, MRS, LB 배양 배지에 도말하여 30~37℃의 호기 혹은 혐기 조건으로 12~36시간 동안 세포 배양기에서 배양하였다. 배양 후, 생성된 콜로니의 인 흡수 능력을 평가하기 위하여 5-브로모-4-클로로-3-인도릴 인산염 디소듐염 (5-bromo-4-chloro-3-indolyl phosphate disodium salt, sigma)를 이용하여 비색법으로 평가하였다.
실험 결과, 인을 흡수한 균주의 경우 청록색으로 나타나는 것을 확인할 수 있었다 (도 1). 도 1은 인을 흡수한 균주를 선별하기 위해, 5-브로모-4-클로로-3-인도릴 인산염 디소듐염 (5-bromo-4-chloro-3-indolyl phosphate disodium salt) 시약을 이용한 비색법으로 실험한 결과이다. 도 1의 A는 균이 인을 흡수하여 시약에 의해 청록색으로 변색된 콜로니 사진이고, 도 1의 B는 균주가 인을 흡수하지 않아 시약에 의해 변색되지 않은 콜로니 사진이다.
상기의 비색법을 통하여 균을 배양한 500개 이상의 페트리 접시 중 색의 강도가 큰 콜로니를 3개 이상 선별하였으며, 이 콜로니를 '총 인의 농도가 20mM이 되도록 제조된 96-웰의 세포 배양용 마이크로플레이트 MRS 액체배지'에 각각 피킹하여 37℃ 및 30℃에서 3시간 동안 세포배양기에서 배양하였다. 그 후, 인산염 분석 키트 (phosphate assajy kit, sigma)를 이용하여 인 농도를 측정하였다.
측정 결과, 인이 첨가된 MRS 액체 배양 배지의 인 감소가 큰 균주 150여 종을 확인하여 일차적으로 스크리닝하였다. 스크리닝한 150 여개 균주들을 37℃ 및 30℃에서 6~36시간 동안 230rpm으로 진탕배양기 (shaking incubator)에서 전배양하였다. 그 후, OD (Optical density) 값을 1로 맞추어 150 여종 균의 세포수를 표준화(normalization)하고, 총 인의 농도가 20mM이 되도록 제조한 MRS 액체 배양 배지에서 37℃ 및 30℃에서 3시간 동안 230rpm으로 진탕배양기 (shaking incubator)에서 본배양하였다. 배양 0시간의 인 농도에서 3시간 후, 인 농도의 차를 계산, 이를 3시간 배양 후의 OD값으로 나눠주어 표준화된 각 균주당 인 흡수율을 얻었다. 표준화된 균주당 인 흡수율의 수학식은 다음과 같다.

상기의 계산식으로 얻어진 각 균주당 인 흡수율을 비교 분석한 결과, 인 흡수율이 가장 높은 균주를 선별하였으며, 이 균주를 16S rRNA 시퀀스 분석하였다. 분석 결과, 락토바실러스 sp. (Lactobacillus sp.) 균주와 상동성이 있는 것을 확인하였으며, 이 선별된 락토바실러스 sp. 균주를 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P 균주로 명명하고, 하기 실험예 1 내지 3에서 사용하였다.

본 실험예에서는 선별된 락토바실러스 sp. KCCM 11826P 균주의 인 흡수 능력을 확인하기 위하여, 일반적으로 많이 쓰이고 있는 유산균들과 인 흡수 능력을 비교하였다.
비교 실험에 사용된 유산균은 락토바실러스속 유산균 6종 (L.
brevis JCM 1059, L.
casei IAM 1045, L. acidophilus ATCC 11506, L.
plantarum ATCC 10012, L.
delbrueckii ATCC 7830, L. subsp. lactis ATCC 21053), 엔테로코커스속 유산균 2종 (Enterococcus
faecium ATCC 6057, E.
faecalis IFO 3971) 및 비피도박테리움속 유산균 1종 (Bifidobacterium longum ATCC 15707)이었다.
각 균주들은 각각의 최적 배양조건으로 인 농도가 20mM인 MRS 액체 배지에서 실시예 1의 전배양 및 본배양과 동일한 조건으로 배양하였다. 그 후, 실시예 1의 인 흡수율 계산식에 따라 각 균주의 인 흡수율을 얻은 후, 비교하였다.
실험 결과, 본 발명의 실시예 1에서 선별한 락토바실러스 sp. KCCM 11826P 균주의 인 흡수 능력이 가장 우수한 것을 확인할 수 있었으며, OD1 및 시간당 0.2923 mM의 인을 흡수하는 것으로 나타났다. 이는 비교 균주 중 인 흡수 능력이 가장 높은 E.
faecalis IFO 3971의 인 흡수량인 0.1467 mM와 비교하여도 뛰어난 인 흡수 능력을 보유한 것으로 알 수 있었다 (도 2). 도 2는 선별된 락토바실러스 sp.(Lactobacillus sp.) KCCM 11826P와, 락토바실러스속 유산균 6종 (L.
brevis JCM 1059, L.
casei IAM 1045, L. acidophilus ATCC 11506, L.

따라서, 본 발명의 실시예 1에서 선별된 균주인 락토바실러스 sp. KCCM 11826P의 인 흡수능력이 탁월함을 확인하였다.

본 발명의 목적은 유산균을 통하여 인의 장내 흡수를 저해하는 것이다. 따라서 본 실험예에서는 대표적 장내 세균인 대장균 top10과 본 실시예에서 선별된 락토바실러스 sp. KCCM 11826P의 인 흡수 능력을 비교함으로써, 락토바실러스 sp. KCCM 11826P가 장내의 인 흡수 저해 역할에 적합한 유산균인지 확인하고자 하였다.
락토바실러스 sp. KCCM 11826P와 대장균 top10 균주의 인 흡수 능력을 비교하기 위하여, 두 균주를 각각의 20mM 인이 포함된 MRS 액체 배지에서 발효하였다.
실험 결과, 락토바실러스 sp. KCCM 11826P 균주의 경우 발효 후, 8시간만에 OD가 13.35까지 증가한 반면, 대장균 top10의 경우 발효 후, 8시간에서의 OD가 6.65에 그쳤다 (도 3의 A).
또한, 락토바실러스 sp. KCCM 11826P의 경우, 발효 후, 8시간까지 3.17mM의 인을 흡수하였으나, 대장균 top10은 발효 후, 8시간까지 0.56mM의 인을 흡수하는데 그쳤다.
따라서, 본 발명에서 선별한 락토바실러스 sp. KCCM 11826P가 기존의 장내 세균인 대장균 top10에 비해서 생장률 및 인 흡수 능력이 뛰어남을 확인할 수 있었다 (도 3의 B).
도 3은 락토바실러스 sp. KCCM 11826P와 대장균 top10 균주의 인 흡수 능력을 비교 측정한 결과이다. 도 3의 A는 락토바실러스 sp. KCCM 11826P와 대장균 top10 균주의 생장률을 측정한 그래프이고, 도 3의 B는 락토바실러스 sp. KCCM 11826P와 대장균 top10 균주의 인 흡수 능력을 측정하여 나타낸 그래프이다.
또한, 대장균 top10 균주는 6시간이 지난 후부터는 흡수한 인을 세포 밖으로 내보내는 것이 관찰되었으며, 24시간 이후에는 흡수했던 인을 모두 세포 밖으로 방출하는 것으로 나타났다.
따라서, 본 발명에서 선별한 락토바실러스 sp. KCCM 11826P는 장내의 인 흡수율을 저해하는 균으로서 매우 적합함을 확인할 수 있었다.

11826P의 인 흡수 능력과 인 결합제인 칼슘카보네이트 (Calcium carbonate)의 인 흡착 능력 비교]
본 실험예에서는, 인체 내 인 흡수를 방지하기 위하여 일반적으로 쓰이는 인 결합제 중 하나인 칼슘카보네이트의 인 흡착 능력과 본 발명에서 선별된 락토바실러스 sp. KCCM 11826P의 인 흡수 능력을 비교하고자 하였다.
비교실험을 위하여, 하기 실시예 2, 비교예 1 및 2를 준비하였다.
(1) 실시예 2 제조
1g/L 건조중량의 락토바실러스 sp. KCCM 11826P 균주를 인 농도가 20mM이 되도록 제조한 MRS 액체배지 5ml에 첨가함으로써 실시예 2를 제조하였다.
(2) 비교예 1 제조
칼슘카보네이트를 증류수에 녹여, 칼슘카보네이트의 농도가 1g/L가 되도록 용액을 제조한 후, 인 농도가 20mM이 되도록 제조한 MRS 액체배지 5ml에 첨가함으로써 비교예 1을 제조하였다.
(3) 비교예 2 제조
칼슘카보네이트 파우더를 인 농도가 20mM이 되도록 제조한 5ml의 MRS 액체배지에 1g/L의 농도가 되도록 첨가하고, 10초 동안 강하게 섞어 비교예 2를 제조하였다.
상기 실시예 2, 비교예 1 및 2를 37℃에서 230rpm의 조건으로 3시간 동안 세포배양기에서 반응시켰다.
실험 결과, 비교예 1은 1g 당 약 40.08mg의 인을 흡착하였고, 비교예 2는 인을 거의 흡착하지 못하는 것으로 나타났다. 이에 비해 본 발명에서 선별한 락토바실러스 sp. KCCM 11826P는 1g 당 약 58.69mg의 인을 흡착하여, 칼슘카보네이트에 비하여 우수한 인 흡수 능력을 보였다 (도 4). 도 4는 락토바실러스 sp. KCCM 11826P 균주와 칼슘카보네이트 용액 및 칼슘카보네이트 파우더 형태의 인 흡수력 및 흡착력을 비교 측정한 결과이다.

- 제1항에 있어서,상기 식품조성물은,환자영양식, 육류, 곡류, 카페인 음료, 일반음료, 유제품, 초콜렛, 빵류, 스넥류, 과자류, 피자, 젤리, 면류, 껌류, 아이스크림류, 알코올성 음료, 술, 비타민 복합제 및 그 밖의 건강보조식품류 중 선택되는 어느 하나인 것을 특징으로 하는 고인산혈증 개선용 식품 조성물
- 락토바실러스 sp. (Lactobacillus sp.) KCCM 11826P을 유효성분으로 포함하는 것을 특징으로 하는 고인산혈증 예방 또는 치료용 약학조성물
