Abstract
Description
本出願は、参照によりここに援用されるところの日本出願特願2016-078492号優先権を請求する。
メトホルミンは、血糖レベルを下げること以外にも、癌細胞の増殖を抑制し、癌発症率を低減し、さらに長寿にも関連する。よって、メトホルミンを摂取する患者数及びメトホルミン誘発乳酸アシドーシス(以下、MALAと称する場合がある)の発症率は、増加していくと考えられる。
メトホルミンは、全世界で1億2000万人以上に処方され(参照:非特許文献2)、耐糖能障害の患者数は、2025年までに4億2000万人まで増加すると予想されている(参照:非特許文献3)。しかしながら、メトホルミンが、重大な乳酸アシドーシスに関連し、全体的な発症率は、およそ3~10/100,000人年(person-years)であると報告されている(参照:非特許文献4)。MALAの発生率は低いが、50%もの高い死亡率を有する(参照:非特許文献5)。他のビグアナイドであるフェンホルミン又はブホルミンを服用した場合の乳酸アシドーシスの発症率は、メトホルミンよりも遥かに高く、多くの国々の市場において、フェンホルミン及びブホルミンが禁止されることにつながった(参照:非特許文献6)。
これらの事実は、メトホルミンが、糖尿病及び癌の治療、癌予防及びアンチエイジング目的に使用されるようになり、さらに、MALA患者数の増加につながるであろうことを示す。
乳酸アシドーシスに対しては、酸素吸入療法、末梢循環改善薬、輸血を行うことで全身の組織における乳酸の産生量を低下させることを目的とした対症療法が行われてきたが、血中乳酸値を直接低下させる治療法は限られた治療機関でしか実施できない血漿交換療法以外に現在までに存在しなかった。
〔1〕一般式(I)
R1は、ヒドロキシC1~C6アルキル基、C2~C7アルカノイル基、C2~C7アルカノイルC1~C6アルキル基、(C1~C6アルコキシ)カルボニル基、(C1~C6アルコキシ)カルボニルC1~C6アルキル基、カルボキシ基、又はカルボキシC1~C6アルキル基を示す]で表される化合物、又はその薬理上許容される塩を有効成分として含有する乳酸アシドーシスの予防又は治療のための医薬。
〔2〕R1が、カルボキシ基、ヒドロキシメチル基、1-ヒドロキシエチル基、2-ヒドロキシエチル基、2-ヒドロキシプロピル基、3-ヒドロキシプロピル基、2-ヒドロキシブチル基、アセチル基、メトキシカルボニル基、エトキシカルボニル基、2-オキソプロピル基、2-オキソブチル基、3-オキソブチル基、2-オキソペンチル基、メトキシカルボニルメチル基、又はカルボキシメチル基である前項1に記載の医薬。
〔3〕R1が、カルボキシ基、ヒドロキシメチル基、1-ヒドロキシエチル基、2-ヒドロキシエチル基、2-ヒドロキシプロピル基、3-ヒドロキシプロピル基、2-ヒドロキシブチル基、2-オキソプロピル基、2-オキソブチル基、3-オキソブチル基、又は2-オキソペンチル基である前項1に記載の医薬。
〔4〕一般式(I)を有する化合物が、
({[5-ヒドロキシ-2-({1-[4'-(ヒドロキシメチル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸、
({[5-ヒドロキシ-2-({1-[4'-(2-ヒドロキシプロピル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸、
[({5-ヒドロキシ-2-[(1-{4'-[(2S)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸、

〔5〕一般式(I)を有する化合物が、({[5-ヒドロキシ-2-({1-[4'-(ヒドロキシメチル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸である、前項1に記載の医薬。
〔6〕一般式(I)を有する化合物が、[({5-ヒドロキシ-2-[(1-{4'-[(2S)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸である、前項1に記載の医薬。
〔7〕乳酸アシドーシスが、薬剤誘発乳酸アシドーシス、腎機能不全誘発乳酸アシドーシス、ショック誘発乳酸アシドーシス、感染症誘発乳酸アシドーシス、心肺蘇生後誘発乳酸アシドーシス、心筋梗塞誘発乳酸アシドーシス、多臓器不全誘発乳酸アシドーシス、ミトコンドリア病誘発乳酸アシドーシス、癌誘発乳酸アシドーシス、肝切除誘発乳酸アシドーシス、重症呼吸器疾患誘発乳酸アシドーシス、播種性血管内凝固誘発乳酸アシドーシス、重症糖尿病誘発乳酸アシドーシス、又はアルコール誘発乳酸アシドーシスである、前項1乃至6のいずれか1項に記載の医薬。
〔8〕乳酸アシドーシスが、ビグアナイド誘発乳酸アシドーシスである、前項1乃至6のいずれか1項に記載の医薬。
〔9〕乳酸アシドーシスが、メトホルミン誘発乳酸アシドーシスである、前項1乃至6のいずれか1項に記載の医薬。
〔10〕経口投与用医薬である前項1乃至9のいずれか1項に記載の医薬。
〔11〕単回投与用医薬である前項1乃至10のいずれか1項に記載の医薬。
〔12〕前項1乃至11のいずれか1項に記載の医薬を投与することによる、乳酸アシドーシスを予防又は治療する方法。

本発明の乳酸アシドーシスの予防又は治療のための医薬は、乳酸アシドーシスに対し、経口による単回投与で治療成績を上げることが出来ることから、治療効果及び医療コストの両面から、医療の現場へ与える恩恵は大きい。

本発明の乳酸アシドーシスの予防又は治療における「乳酸アシドーシス」は、特に限定されないが、薬剤誘発乳酸アシドーシス{例えば、ビグアナイド誘発乳酸アシドーシス(より詳細には、メトホルミン誘発乳酸アシドーシス、ブホルミン誘発乳酸アシドーシス)}、腎機能不全誘発乳酸アシドーシス(例えば、慢性腎不全誘発乳酸アシドーシス)、ショック誘発乳酸アシドーシス(例えば、循環不全、出血等により誘発される乳酸アシドーシス、より詳しくは、拡張型心筋症の患者で心拍出が不十分で循環不全や出血等により誘発される乳酸アシドーシス)、感染症(敗血症)誘発乳酸アシドーシス(例えば、肺炎・腹膜炎誘発乳酸アシドーシス)、心肺蘇生後誘発乳酸アシドーシス、心筋梗塞誘発乳酸アシドーシス、多臓器不全誘発乳酸アシドーシス、ミトコンドリア病誘発乳酸アシドーシス(例えば、ミトコンドリア脳筋症誘発乳酸アシドーシス)、癌誘発乳酸アシドーシス、肝切除誘発乳酸アシドーシス、重症呼吸器疾患誘発乳酸アシドーシス、播種性血管内凝固誘発乳酸アシドーシス、重症糖尿病誘発乳酸アシドーシス、アルコール誘発乳酸アシドーシス等を挙げることができるが、特に好ましくはメトホルミン誘発乳酸アシドーシス、腎機能不全誘発乳酸アシドーシスである。
本発明の乳酸アシドーシスの予防又は治療のための医薬は、酸素吸入法、輸血、血漿交換療法などと併用することができる。
本発明の乳酸アシドーシスの予防又は治療のための医薬は、末梢循環改善薬を一緒に投与することができる。末梢循環改善薬としては、ニトログリセリン、プロスタグランジン製剤等が挙げられる。
R1は、ヒドロキシC1~C6アルキル基、C2~C7アルカノイル基、C2~C7アルカノイルC1~C6アルキル基、(C1~C6アルコキシ)カルボニル基、(C1~C6アルコキシ)カルボニルC1~C6アルキル基、カルボキシ基、又はカルボキシC1~C6アルキル基を示す]で表される化合物、又はその薬理上許容される塩も対象とする。
R1は、ヒドロキシC1~C6アルキル基、C2~C7アルカノイル基、C2~C7アルカノイルC1~C6アルキル基、(C1~C6アルコキシ)カルボニル基、(C1~C6アルコキシ)カルボニルC1~C6アルキル基、カルボキシ基、又はカルボキシC1~C6アルキル基を示す]で表される化合物、又はその薬理上許容される塩を乳酸アシドーシスの予防又は治療のための医薬の製造としての使用も対象とする。
本実施例で使用した全てのマウスは、日本クレア株式会社から入手した7~13週齢の野生型C57BL/6オスマウスであった。マウスは、特定病原体フリー施設で、12時間明期/12時間暗期サイクルで、自由摂餌で飼育した。
本実施例の統計学的分析において、サンプルサイズを決定するためのパワー計算は、行われていない。vehicle又は化合物A処理前に乳酸アシドーシスで死亡したマウスは、解析対象から除外した。グループ間の正規分布データの比較は、独立スチューデントのt検定(unpaired Student's t-test)を使用して分析した。生存率の差は、GraphPad Prismソフトウェアを使用して、ログランク(Mantel-Cox)検定により分析した。
({[5-ヒドロキシ-2-({1-[4'-(ヒドロキシメチル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸(化合物A)の製造
化合物Aは、WO2011/049126(特許文献1)の実施例1の方法に従って製造した。製造した化合物は、1H-NMR及び質量分析により、化合物Aとして同定した。
[({5-ヒドロキシ-2-[(1-{4'-[(2S)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸(化合物B)の製造
化合物Bは、WO2011/049126(特許文献1)の実施例45の方法に従って製造した。製造した化合物は、1H-NMR及び質量分析により、化合物Bとして同定した。
メトホルミン誘発乳酸アシドーシス(MALA)モデルへの化合物Aの投与の効果
メトホルミンは、乳酸脱水素酵素(LDH)による乳酸からピルビン酸への変換にとって望ましくないNADH(nicotinamide adenine dinucleotide)の蓄積を増進し、MALAを引き起こす。MALAは、特に、高齢の患者及び腎不全の患者において、高死亡率の重篤症状である。そこで、本試験例では、化合物AのMALAモデルの生存率の向上効果を確認した。
この結果から、本発明の一般式(I)で表される化合物(特に、化合物A)が、メトホルミン誘発乳酸アシドーシスの予防又は治療に効果があることを確認した。
MALA併発慢性腎臓病(CKD)モデルマウスへの化合物Aの投与の効果
メトホルミンは、CKD患者又は高血清中クレアチニン値を有する高齢者において、乳酸アシドーシスのリスクが認められるために、禁忌とみなされている(参考:非特許文献4)。そこで、本試験例では、化合物AのMALA併発CKDモデルの生存率の向上効果を確認した。
さらに、CKDモデルマウスに、1日目に0.5 mg/g体重メトホルミンを投与し、MALA併発CKDマウスモデルとした。該メトホルミン投与の4時間後に、全血サンプル(5μl)を、尾静脈から採取し、血中乳酸値をLactate Pro Test Meter及びLactate Pro Test Strip(Cycle Classic Imports社)を使用して測定した。8 mMを超える血中乳酸値のマウスについては、vehicle(0.5%メチルセルロース; 64625; Sigma-Aldrich社)又は10 mg/kg体重の化合物Aのいずれかを経口投与した。乳酸塩レベルが基準を満たさなかったマウスについては、翌日実験を繰り返した。これらのマウスについて、カプランマイヤー生存分析を行った。
この結果から、本発明の一般式(I)で表される化合物(特に、化合物A)が、慢性腎臓病の乳酸アシドーシスの予防剤又は治療に効果があることを確認した。
MALA併発慢性腎臓病(CKD)モデルマウスへの化合物Bの投与の効果
7週齢の野性型C57BL/6オスマウスを、標準又は0.2%アデニン飼料のいずれかを6週間自由摂餌で給餌した。0.2%アデニン飼料で給餌した個体群をCKDモデルマウスとした。
さらに、CKDモデルマウスに、1日目に0.5 mg/g体重メトホルミンを投与し、MALA併発CKDマウスモデルとした。該メトホルミン投与の4時間後に、全血サンプル(5μl)を、尾静脈から採取し、血中乳酸値をLactate Pro Test Meter及びLactate Pro TestStrip(Cycle Classic Imports社)を使用して測定した。8 mMを超える血中乳酸値のマウスについては、vehicle(0.5%メチルセルロース;64625; Sigma-Aldrich社)又は30 mg/kg体重の化合物Bのいずれかを経口投与した。乳酸塩レベルが基準を満たさなかったマウスについては、翌日実験を繰り返した。これらのマウスについて、カプランマイヤー生存分析を行った。その結果を図3に示した。
この結果から、本発明の一般式(I)で表される化合物(特に、化合物B)が、慢性腎臓病の乳酸アシドーシスの予防剤又は治療に効果があることを確認した。
製剤例1(錠剤)
100mgの粉末状の実施例化合物、124mgのラクトース、25mgのセルロース及び1mgのステアリン酸マグネシウムを混合し、打錠機により打錠して、1錠200mgの錠剤とする。この錠剤は必要に応じて糖衣を施すことができる。
これらの結果により、本発明の乳酸アシドーシスの予防又は治療のための医薬は、乳酸アシドーシスの新しい予防又は治療に利用することができる。
Claims (15)
- R1が、カルボキシ基、ヒドロキシメチル基、1-ヒドロキシエチル基、2-ヒドロキシエチル基、2-ヒドロキシプロピル基、3-ヒドロキシプロピル基、2-ヒドロキシブチル基、アセチル基、メトキシカルボニル基、エトキシカルボニル基、2-オキソプロピル基、2-オキソブチル基、3-オキソブチル基、2-オキソペンチル基、メトキシカルボニルメチル基、又はカルボキシメチル基である請求項1に記載の医薬。
- R1が、カルボキシ基、ヒドロキシメチル基、1-ヒドロキシエチル基、2-ヒドロキシエチル基、2-ヒドロキシプロピル基、3-ヒドロキシプロピル基、2-ヒドロキシブチル基、2-オキソプロピル基、2-オキソブチル基、3-オキソブチル基、又は2-オキソペンチル基である請求項1に記載の医薬。
- 一般式(I)を有する化合物が、
({[5-ヒドロキシ-2-({1-[4'-(ヒドロキシメチル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸、
({[5-ヒドロキシ-2-({1-[4'-(2-ヒドロキシプロピル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸、
[({5-ヒドロキシ-2-[(1-{4'-[(2S)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸、
[({5-ヒドロキシ-2-[(1-{4'-[(2R)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸、
({[5-ヒドロキシ-6-メチル-2-({1-[4'-(2-オキソプロピル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)ピリミジン-4-イル]カルボニル}アミノ)酢酸、及び
4'-[4-({4-[(カルボキシメチル)カルバモイル]-5-ヒドロキシ-6-メチルピリミジン-2-イル}メチル)ピペリジン-1-イル]ビフェニル-4-カルボン酸からなる群から選択される1つの化合物である、請求項1に記載の医薬。
- 一般式(I)を有する化合物が、({[5-ヒドロキシ-2-({1-[4'-(ヒドロキシメチル)ビフェニル-4-イル]ピペリジン-4-イル}メチル)-6-メチルピリミジン-4-イル]カルボニル}アミノ)酢酸である、請求項1に記載の医薬。
- 一般式(I)を有する化合物が、[({5-ヒドロキシ-2-[(1-{4'-[(2S)-2-ヒドロキシプロピル]ビフェニル-4-イル}ピペリジン-4-イル)メチル]-6-メチルピリミジン-4-イル}カルボニル)アミノ]酢酸である、請求項1に記載の医薬。
- 乳酸アシドーシスが、薬剤誘発乳酸アシドーシス、腎機能不全誘発乳酸アシドーシス、ショック誘発乳酸アシドーシス、感染症誘発乳酸アシドーシス、心肺蘇生後誘発乳酸アシドーシス、心筋梗塞誘発乳酸アシドーシス、多臓器不全誘発乳酸アシドーシス、ミトコンドリア病誘発乳酸アシドーシス、癌誘発乳酸アシドーシス、肝切除誘発乳酸アシドーシス、重症呼吸器疾患誘発乳酸アシドーシス、播種性血管内凝固誘発乳酸アシドーシス、重症糖尿病誘発乳酸アシドーシス、又はアルコール誘発乳酸アシドーシスである、請求項1乃至6のいずれか1項に記載の医薬。
- 乳酸アシドーシスが、ビグアナイド誘発乳酸アシドーシスである、請求項1乃至6のいずれか1項に記載の医薬。
- 乳酸アシドーシスが、メトホルミン誘発乳酸アシドーシスである、請求項1乃至6のいずれか1項に記載の医薬。
- 経口投与用医薬である請求項1乃至9のいずれか1項に記載の医薬。
- 単回投与用医薬である請求項1乃至10のいずれか1項に記載の医薬。
- 請求項1乃至11のいずれか1項に記載の医薬を投与することによる、乳酸アシドーシスを予防又は治療する方法。
- 乳酸アシドーシスが、薬剤誘発乳酸アシドーシス、腎機能不全誘発乳酸アシドーシス、ショック誘発乳酸アシドーシス、感染症誘発乳酸アシドーシス、心肺蘇生後誘発乳酸アシドーシス、心筋梗塞誘発乳酸アシドーシス、多臓器不全誘発乳酸アシドーシス、ミトコンドリア病誘発乳酸アシドーシス、癌誘発乳酸アシドーシス、肝切除誘発乳酸アシドーシス、重症呼吸器疾患誘発乳酸アシドーシス、播種性血管内凝固誘発乳酸アシドーシス、重症糖尿病誘発乳酸アシドーシス、又はアルコール誘発乳酸アシドーシスである、請求項12に記載の方法。
