Abstract
Description
臨床においては、シリンジ2筒分、即ち、合計10mLの製剤(フルベストラントとして500mg)が1回の投与量として適用されている。
例えば、特許第3713237号公報及び特表2004-534093号公報には、フルベストラント、医薬的に許容できるアルコール類(エタノール、ベンジルアルコール等)、安息香酸ベンジル、及びヒマシ油を含有する筋肉内注射に適する医薬製剤が開示されている。
特表2009-509942号公報には、フルベストラント、製薬上許容されるアルコール(エタノール、ベンジルアルコール等)、プロピレングリコール又はポリエチレングリコール、及びヒマシ油を含有する製剤が開示されている。
しかし、フルベストラントを高濃度で含有する医薬製剤を筋肉内注射すると、注射部位においてフルベストラントが析出し、析出した粒子状のフルベストラントによって、組織刺激(即ち、痛み)又は炎症が引き起こされるという問題が生じ得る。
特表2004-534093号公報では、特許第3713237号公報に記載された医薬製剤よりも、アルコール類及び安息香酸ベンジルの配合量を高めることで、フルベストラントを100mg/mL以上含有する医薬製剤を実現しているが、特表2004-534093号公報に記載された医薬製剤を筋肉内注射すると、注射部位において沈殿が生じ、特許第3713237号公報に記載された医薬製剤よりも強い組織刺激又は炎症が引き起こされることが記載されている。
特表2009-509942号公報に記載された製剤は、本発明者らが確認したところ、製剤の全質量に対して10質量%(即ち、100mg/mL)以上のフルベストラントを含有し得る。しかし、本発明者らが、特表2009-509942号公報に記載された製剤について、筋肉内注射後の環境を想定した評価試験(後述の実施例に記載の析出性の評価試験)を行ったところ、フルベストラントの析出が確認された。
(1)医薬組成物の全質量に対して8質量%以上のエタノールを含む。
(2)医薬組成物の全質量に対して3質量%以上の、プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコールを含む。
(3)ベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。
(4)水性溶媒の含有率が、医薬組成物の全質量に対して15質量%以上50質量%以下である。
(A)安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。
(B)医薬的に許容される非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上75質量%以下である。
[3] プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコールが、プロピレングリコールである[1]又は[2]に記載の医薬組成物。
[4] 医薬的に許容される非水性担体が、ヒマシ油、又は、ヒマシ油及びヒマシ油とは異なる植物油の混合物である[1]~[3]のいずれか1つに記載の医薬組成物。
[5] ヒマシ油とは異なる植物油が、ゴマ油、ラッカセイ油、ダイズ油、ツバキ油、トウモロコシ油、綿実油、オリーブ油、サフラワー油、ナタネ油、及び構成脂肪酸の平均炭素数が6以上12以下の脂肪酸トリグリセリドからなる群より選ばれる少なくとも1種の植物油である[4]に記載の医薬組成物。
[6] ヒマシ油とは異なる植物油が、ゴマ油及びラッカセイ油から選ばれる少なくとも1種の植物油である[4]に記載の医薬組成物。
(C)医薬組成物の全質量に対して40質量%以上のヒマシ油を含む。
(D)ヒマシ油とは異なる植物油の含有率が、医薬組成物の全質量に対して20質量%未満である。

本明細書に段階的に記載されている数値範囲において、ある数値範囲で記載された上限値又は下限値は、他の段階的な記載の数値範囲の上限値又は下限値に置き換えてもよい。また、本開示に記載されている数値範囲において、ある数値範囲で記載された上限値又は下限値は、実施例に示されている値に置き換えてもよい。
本明細書において、医薬組成物中の各成分の量は、各成分に該当する物質が医薬組成物中に複数存在する場合には、特に断らない限り、医薬組成物中に存在する複数の物質の合計量を意味する。
本実施形態の医薬組成物は、フルベストラントと、水性溶媒と、医薬的に許容される非水性担体(以下、単に「非水性担体」ともいう。)と、を含み、フルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上であり、水性溶媒が下記(1)~(4)の要件を満たし、かつ、医薬的に許容される非水性担体が下記(A)及び(B)の要件を満たす医薬組成物である。
(1)医薬組成物の全質量に対して8質量%以上のエタノールを含む。(2)医薬組成物の全質量に対して3質量%以上の、プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコール(以下、「特定多価アルコール」ともいう。)を含む。(3)ベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。(4)水性溶媒の含有率が、医薬組成物の全質量に対して15質量%以上50質量%以下である。
(A)安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。(B)医薬的に許容される非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上75質量%以下である。
これまで報告されているフルベストラントを含有する医薬製剤では、医薬製剤中にフルベストラントを高濃度で含有させることはできても、筋肉内注射後、体液と接触した際にフルベストラントが析出し得るという問題については、未だ解決できていない。
本実施形態の医薬組成物は、有効成分として、フルベストラントを含む。
フルベストラント(7α-[9-(4,4,5,5,5-ペンタフルオロペンチルスルフィニル)ノニル]エストラ-1,3,5(10)-トリエン-3,17β-ジオール)は、エストロゲン受容体アンタゴニストであり、乳癌を治療するための薬物として知られている。
なお、上限については、特に限定されず、例えば、30質量%以下であり、好ましくは20質量%以下であり、より好ましくは14質量%以下であり、更に好ましくは12質量%以下である。
本実施形態の医薬組成物中のフルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上であると、注射部位を従来の2箇所から1箇所に減らしたり、注射部位1箇所あたりの投与量を減らしたりすることが可能となり、患者の負担を低減し得る。
本実施形態の医薬組成物は、医薬組成物の全質量に対して15質量%以上50質量%以下の量の水性溶媒を含む。
また、本実施形態の医薬組成物に含まれる水性溶媒は、医薬組成物の全質量に対して8質量%以上のエタノールと、医薬組成物の全質量に対して3質量%以上の、プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコール(特定多価アルコール)と、を含み、アルコール中のベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。
本実施形態における水性溶媒としては、エタノール、グリセリン、プロピレングリコール、1,3-ブチレングリコール、ジプロピレングリコール、ベンジルアルコール等が挙げられる。
本実施形態の医薬組成物において、エタノールは、医薬組成物中におけるフルベストラントの溶解性の向上に寄与する。
本実施形態の医薬組成物中のエタノールの含有率が、医薬組成物の全質量に対して8質量%以上であると、フルベストラントの溶解性が良好となり、フルベストラントを高濃度で、即ち、医薬組成物の全質量に対して8質量%以上の濃度で含む医薬組成物を実現し得る。また、低温で保存した場合に濁りが生じ難くなる傾向がある。
エタノールは、薬局方(日本、米国、欧州、及びその他各国の薬局方に記載のもの)の基準を満たす品質のものであれば、特に限定されない。
本実施形態の医薬組成物におけるエタノールとしては、例えば、日本薬局法に記載されるエタノール(15℃において95.1体積%~96.9体積%のエタノールを含むもの)、及び無水エタノール(15℃において99.5体積%以上のエタノールを含むもの)が挙げられる。
本実施形態の医薬組成物において、特定多価アルコールは、医薬組成物中におけるフルベストラントの溶解性の向上に寄与する。
本実施形態の医薬組成物中の特定多価アルコールの含有率が、医薬組成物の全質量に対して3質量%以上であると、フルベストラントの溶解性が良好となり、フルベストラントを高濃度で、即ち、医薬組成物の全質量に対して8質量%以上の濃度で含む医薬組成物を実現し得る。
フルベストラントの溶解性をより高める観点からは、本実施形態の医薬組成物は、特定多価アルコールとして、少なくともプロピレングリコールを含むことが好ましく、特定多価アルコールがプロピレングリコールであることが特に好ましい。
本実施形態の医薬組成物中のベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満であると、医薬組成物がフルベストラントを高濃度で、即ち、医薬組成物の全質量に対して8質量%以上の濃度で含んでいても、筋肉内注射後、体液と接触した場合にフルベストラントの析出が生じ難くなる傾向がある。
ここでいう「水を意図的に配合しない」とは、本実施形態の医薬組成物に含まれる各成分が原材料の段階で含んでいる水分を持ち込むことによって必然的に含まれることになる水、及び大気からの吸湿によって必然的に含まれることになる水以外には、水を積極的に配合しないことを意味する。「大気からの吸湿によって含まれることになる水」には、医薬組成物の保管中に、医薬組成物中に保持される水が含まれる。
具体的には、本実施形態の医薬組成物中の水の含有率は、医薬組成物の全質量に対して、好ましくは5質量%以下であり、より好ましくは3質量%以下であり、更に好ましくは2質量%以下であり、特に好ましくは1質量%以下である。
本実施形態の医薬組成物中の水性溶媒の含有率が、医薬組成物の全質量に対して15質量%以上であると、フルベストラントの溶解性が良好となり、フルベストラントを高濃度で、即ち、医薬組成物の全質量に対して8質量%以上の濃度で含む医薬組成物を実現し得る。
本実施形態の医薬組成物中の水性溶媒の含有率が、医薬組成物の全質量に対して50質量%以下であると、治療上有効な血中フルベストラント濃度を一定期間保持させるために必要な非水性担体の含有率を十分に確保することができるため、筋肉内注射後、医薬組成物が体液と接触し、医薬組成物中におけるフルベストラントの溶解性の向上に寄与していた水性溶媒が体液中に拡散した場合に、フルベストラントの析出が生じ難くなる傾向がある。
本実施形態の医薬組成物は、医薬組成物の全質量に対して40質量%以上75質量%以下の量の医薬的に許容される非水性担体を含む。
また、本実施形態の医薬組成物に含まれる非水性担体は、安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。
本実施形態の医薬組成物において、非水性担体は、治療上有効な血中フルベストラント濃度を一定期間保持することに寄与する。
本実施形態において、「非水性担体」とは、医薬的に許容される非水性の液体物質を意味し、具体的には、25℃の水に対する溶解度が1質量%未満である液体物質をいう。
非水性担体としては、植物油、非水性エステル化合物等が挙げられる。
本実施形態において、「植物油」とは、植物の種子、果実等から抽出により得られる油脂を意味し、植物油としては、医薬品添加物として使用実績のある植物油が好ましい。
植物油としては、ヒマシ油、ゴマ油、ラッカセイ油、ダイズ油、ツバキ油、トウモロコシ油、綿実油、オリーブ油、サフラワー油、ナタネ油、中鎖脂肪酸トリグリセリド等が挙げられる。ここで、中鎖脂肪酸トリグリセリドとは、トリグリセリドを構成する脂肪酸(即ち、構成脂肪酸)の平均炭素数が6以上12以下の油脂を意味する。構成脂肪酸は、飽和脂肪酸であってもよく、不飽和脂肪酸であってもよい。中鎖脂肪酸トリグリセリドとしては、炭素数が6以上12以下の飽和脂肪酸のトリグリセリドが好ましい。
非水性エステル化合物としては、安息香酸ベンジル、オレイン酸エチル、ミリスチン酸イソプロピル、セバシン酸ジエチル等が挙げられる。
ヒマシ油とは異なる植物油としては、フルベストラントの溶解性及び筋肉注射後のフルベストラントの徐放性がより良好であるという観点から、ゴマ油、ラッカセイ油、ダイズ油、ツバキ油、トウモロコシ油、綿実油、オリーブ油、サフラワー油、ナタネ油、及び構成脂肪酸の平均炭素数が6以上12以下の脂肪酸トリグリセリド(即ち、中鎖脂肪酸トリグリセリド)からなる群より選ばれる少なくとも1種の植物油が好ましい。
また、ヒマシ油とは異なる植物油としては、日本及び外国における筋肉内に注射する注射液への使用実績の観点からは、ゴマ油、ラッカセイ油、ダイズ油、ツバキ油、及びトウモロコシ油からなる群より選ばれる少なくとも1種の植物油が好ましく、日本における筋肉内に注射する注射液への使用実績の観点からは、ゴマ油及びラッカセイ油から選ばれる少なくとも1種の植物油がより好ましい。
例えば、非水性担体がヒマシ油及びヒマシ油とは異なる植物油の混合物である場合、非水性担体に対するフルベストラントの溶解度は、ヒマシ油に対するフルベストラントの溶解度を1.0としたときに、0.70~1.0の範囲であることが好ましい。
本実施形態の医薬組成物では、非水性担体に対するフルベストラントの溶解度が上記の範囲であると、治療上有効な血中フルベストラント濃度を一定期間保持させることができる。
なお、ここでいう「フルベストラントの溶解度」とは、37℃における溶解度を意味する。
(C)医薬組成物の全質量に対して40質量%以上のヒマシ油を含む。
(D)ヒマシ油とは異なる植物油の含有率が、医薬組成物の全質量に対して20質量%未満である。
本実施形態の医薬組成物は、非水性担体が、上記(C)及び(D)の要件を更に満たすことで、体液と接触した場合にフルベストラントの析出がより生じ難くなる傾向がある。
本実施形態の医薬組成物中の安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満であると、医薬組成物がフルベストラントを高濃度で、即ち、医薬組成物の全質量に対して8質量%以上の濃度で含んでいても、筋肉内注射後、体液と接触した場合にフルベストラントの析出が生じ難くなる傾向がある。
本実施形態の医薬組成物中の非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上であると、治療上有効な血中フルベストラント濃度を一定期間保持させることができ、筋肉内注射後、医薬組成物が体液と接触し、医薬組成物中におけるフルベストラントの溶解性の向上に寄与していた水性溶媒が体液中に拡散した場合に、フルベストラントの析出が生じ難くなる傾向がある。
本実施形態の医薬組成物中の非水性担体の含有率が、医薬組成物の全質量に対して75質量%以下であると、医薬組成物中におけるフルベストラントの溶解性を高めるために必要な水性溶媒の含有率を確保することができる。
その他の添加剤としては、グリセリン、アスコルビン酸又はその塩、塩酸、グルコン酸又はその塩、酢酸又はその塩、乳酸又はその塩、ホウ酸又はその塩、リン酸又はその塩、硫酸又はその塩、酒石酸又はその塩、クエン酸又はその塩、水酸化カリウム、水酸化カルシウム、水酸化ナトリウム、水酸化マグネシウム、モノエタノールアミン、ジエタノールアミン、トリエタノールアミン、トロメタモール、メグルミン、グリシン、ヒスチジン又はその塩、ε-アミノカプロン酸、アルギニン又はその塩、システイン又はその塩、メチオニン、アラニン、ロイシン、アスパラギン酸又はその塩、グルタミン酸又はその塩、アルギニン又はその塩、チオグリセリン、チオグリコール酸又はその塩、タウリン、エデト酸ナトリウム、リドカイン又はその塩、ニコチン酸アミド、クロロブタノール、クレアチニン、ブチルヒドロキシトルエン、ブチルヒドロキシアニソール、ソルビタンセスキオレイン酸エステル、オレイン酸エチル、乳酸エチル、チロメサール、ポリオキシエチレン硬化ヒマシ油、ポリソルベート20、ポリソルベート80等が挙げられる。
但し、その他の添加剤は、これらに限定されない。
本実施形態の医薬組成物がその他の添加剤を含む場合、医薬組成物中のその他の添加剤の含有率は、医薬組成物の全質量に対して10質量%以下が好ましく、5質量%以下がより好ましく、3質量%以下が更に好ましい。
フルベストラントと、水性溶媒と、医薬的に許容される非水性担体と、を含み、フルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上(より好ましくは8質量%以上12質量%以下)であり、水性溶媒が下記(i)~(iv)の要件を満たし、かつ、医薬的に許容される非水性担体が下記(a)~(d)の要件を満たす組成。
(i)医薬組成物の全質量に対して10質量%以上20質量%以下のエタノールを含む。
(ii)医薬組成物の全質量に対して10質量%以上20質量%以下のプロピレングリコールを含む。
(iii)ベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。
(iv)水性溶媒の含有率が、医薬組成物の全質量に対して20質量%以上40質量%以下である。
(a)安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。
(b)医薬組成物の全質量に対して40質量%以上のヒマシ油を含む。
(c)ゴマ油及びラッカセイ油から選ばれる少なくとも1種の植物油の含有率が、医薬組成物の全質量に対して20質量%未満である。
(d)医薬的に許容される非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上75質量%以下である。
本実施形態の医薬組成物は、筋肉内注射用として好適に用いることができる。
本実施形態の医薬組成物によれば、フルベストラントを高濃度(例えば、医薬組成物の全質量に対して8質量%以上の濃度)で含み、かつ、筋肉内注射後、体液と接触した場合にフルベストラントの析出が生じ難いため、注射部位を従来の2箇所から1箇所に減らしたり、注射部位1箇所あたりの投与量を減らしたりすることが可能となり、患者の負担を低減し得る。また、本実施形態の医薬組成物は、フルベストラントの溶解性が良好で、かつ、低温で保存しても、医薬組成物に含まれる成分に相分離又は析出が生じ難く、フルベストラント等の成分が良好に混合されている状態が安定に保持されている点においても、筋肉内注射に適した医薬製剤である。
また、本実施形態の医薬組成物は、有効成分としてフルベストラントを含むため、乳癌、子宮筋腫、及び子宮内膜症の治療用として好適に用いることができる。
本実施形態の医薬組成物の製造方法は、既述の医薬組成物を製造できればよく、特に限定されるものではない。
本実施形態の医薬組成物を製造する方法としては、含まれる成分が均一に混合された医薬組成物を得やすいという観点から、以下で説明する、本実施形態の医薬組成物の製造方法が好ましい。
本実施形態の製造方法は、フルベストラントと、エタノールと、特定多価アルコールと、医薬的に許容される非水性担体と、を一括して混合する場合と比較して、含まれる成分が均一に混合された医薬組成物を得やすいという利点を有する。
以下、本実施形態の製造方法について説明するが、既述の医薬組成物と共通する事項、例えば、医薬組成物に含まれる成分及びその量については、説明を省略する。
第1の工程は、フルベストラントと、エタノールと、特定多価アルコールと、を混合し、フルベストラントの溶解液を得る工程である。
既述の医薬組成物が、その他の添加剤を含む場合には、フルベストラントと、エタノールと、特定多価アルコールと、その他の添加剤と、を混合し、フルベストラントの溶解液を得ることが好ましい。
混合の方法としては、特に限定されず、例えば、撹拌による混合が挙げられる。
第1の工程では、通常、雰囲気温度15℃~60℃の条件下で、フルベストラントと、エタノールと、特定多価アルコールと、を混合し、フルベストラントの溶解液を得る。
第2の工程は、第1の工程で得られたフルベストラントの溶解液と、医薬的に許容される非水性担体(例えば、ヒマシ油)と、を混合し、医薬組成物を得る工程である。
混合の方法としては、特に限定されず、例えば、撹拌による混合が挙げられる。
第2の工程では、通常、雰囲気温度15℃~60℃の条件下で、フルベストラントの溶解液と医薬的に許容される非水性担体とを混合し、医薬組成物を得る。
本実施形態の製造方法は、必要に応じて、第1の工程及び第2の工程以外のその他の工程を有していてもよい。また、第1の工程及び第2の工程は、複数の工程を含んで構成されていてもよい。
その他の工程としては、医薬組成物を滅菌する工程、医薬組成物を容器に充填する工程等が挙げられる。
滅菌する工程としては、滅菌フィルターを用いたろ過滅菌法が好ましい。
本実施形態の医薬組成物を充填する容器としては、バイアル、アンプル、シリンジ等が挙げられる。
これらの中でも、本実施形態の医薬組成物を充填する容器としては、医療現場における取り扱い性の観点から、シリンジが好ましく、ガラス製のシリンジがより好ましい。即ち、本実施形態の医薬組成物の剤形としては、医薬組成物を予めシリンジに充填したプレフィルドシリンジが好ましい。
本発明の他の実施形態は、有効成分としてフルベストラント含む既述の医薬組成物を、乳癌の治療の対象となる患者へ投与することを含む乳癌の治療方法も包含する。
また、本発明の他の実施形態は、有効成分としてフルベストラント含む既述の医薬組成物を、子宮筋腫の治療の対象となる患者へ投与することを含む子宮筋腫の治療方法も包含する。
さらに、本発明の他の実施形態は、有効成分としてフルベストラント含む既述の医薬組成物を、子宮内膜症の治療の対象となる患者へ投与することを含む子宮内膜症の治療方法も包含する。
<実施例1>
撹拌子を入れた清潔なガラス容器に、フルベストラント10質量部、エタノール11質量部、及び特定多価アルコールとしてプロピレングリコール(PG)14質量部を秤量した後、撹拌し、フルベストラントを溶解させ、フルベストラントの溶解液を得た。次いで、得られたフルベストラントの溶解液にヒマシ油を加え、全量を100質量部に調整した後、更に撹拌して均一化し、実施例1の医薬組成物を得た。
医薬組成物の組成を、下記の表1~10に示す組成に変更したこと以外は、実施例1と同様の操作を行って、実施例2~53、101及び102、並びに、比較例1~66及び101の医薬組成物を得た。
1.濁りの有無
上記にて得られた実施例1~53、101及び102、並びに、比較例1~66及び101の各医薬組成物について、少なくとも1mLを、それぞれ無色透明のガラス瓶(5mL容量)に量り取り、冷蔵庫(雰囲気温度:2℃~8℃)に保存した。保存開始から1時間以上経過後、冷蔵庫からガラス瓶を取り出した。取り出し直後に、ガラス瓶の表面に生じた曇りをキムワイプ(登録商標)で拭き取ってから、ガラス瓶に入った医薬組成物を目視にて観察し、濁りの有無を確認した。結果を表1~10に示す。
なお、表1~10では、ガラス瓶に入った医薬組成物に、濁りが確認されなかった場合を「なし」と表記し、濁りが確認された場合を「あり」と表記した。
一方、ガラス瓶に入った医薬組成物に濁りが確認される状態は、含まれる成分が均一に混合されていない状態を示し、低温(2℃~8℃)での保存中に、医薬組成物に含まれる成分のいずれかが相分離又は析出しており、筋肉内注射に適した製剤とはいえないことを意味する。
上記にて得られた実施例1~53、101及び102、並びに、比較例1~66及び101の各医薬組成物に、更に少なくとも飽和量のフルベストラントを加え、15℃~30℃の条件下で少なくとも1日以上撹拌した後、10000rpm(round per minute)以上、10分以上の条件にて遠心分離を行い、過剰のフルベストラントを除き、上清を得た。
得られた上清に溶解しているフルベストラントの量を、下記の条件の高速液体クロマトグラフィー(HPLC)にて測定し、フルベストラントの溶解度(質量%)を算出した。
そして、下記の式(I)に基づき、溶解比率(質量%)を算出し、下記の評価基準に従い、フルベストラントの溶解性を評価した。
溶解比率(質量%)=〔医薬組成物を製造する際に仕込んだフルベストラントの配合量(質量%)〕/〔フルベストラントの溶解度(質量%)〕×100・・・式(I)
評価結果が「A」、「B」、又は「C」であれば、許容内と判断した。
A:溶解比率が70質量%以下である。
B:溶解比率が70質量%より大きく80質量%以下である。
C:溶解比率が80質量%より大きく90質量%以下である。
D:溶解比率が90質量%より大きい。
カラム:XBridge C8(製品名、粒子径:3.5μm、カラムサイズ:4.6mm×150mm、Waters社)
移動相A:メタノール/水=70/30
移動相B:メタノール
グラジェント条件(移動相Bの比率):0%(開始)→0%(12min)→100%(12.1min)→100%(20min)→0%(20.1min)→0%(30min、停止)
検出波長:225nm
流量:1.0mL/min
カラム温度:40℃
上記にて得られた実施例1~53、101及び102、並びに、比較例1~66及び101の各医薬組成物のうち、上記の1.濁りの有無の評価試験において、濁りが確認されず、かつ、上記の2.フルベストラントの溶解性の評価試験において、フルベストラントの溶解度が10.0質量%以上であったものについて、析出性の評価を行った。
なお、析出性の評価試験は、医薬組成物が体液と接触したときに、医薬組成物中のフルベストラントが析出するか否かを確認することを目的とするものであり、人間の体液の代わりに、人間の体液の浸透圧とほぼ同等の浸透圧を示す生理食塩水を用いたモデル実験によって行った。
各医薬組成物について、それぞれフルベストラントを50mg含む量(例えば、実施例1の医薬組成物の場合には0.5mL)を量り取り、無色透明のガラス瓶(30mL容量)に入れた生理食塩水20mLに、静かに滴下した。次いで、ガラス瓶を密栓し、25℃の恒温室にて3日間静置した後、油相(即ち、医薬組成物の非水性担体からなる相)を回収し、油相の状態を目視にて観察し、析出の有無及び状態を確認した。結果を表1~10に示す。
なお、表1~10では、油相に、析出が確認されなかった場合を「A」と表記し、析出がわずかに確認された場合を「B」と表記し、析出が顕著に確認された場合を「C」と表記した。「A」又は「B」であれば、実用上問題がない。
油相溶解比率(質量%)=〔HPLC分析にて測定された油相に溶解したフルベストラントの濃度A(mg/g)〕/〔フルベストラントが全て溶解しているときの油相中のフルベストラントの濃度B(mg/g)〕×100・・・式(1)
油相中のフルベストラントの濃度B(mg/g)=〔医薬組成物中のフルベストラントの含有率(質量%)〕×1000/[〔医薬組成物中のフルベストラントの含有率(質量%)〕+〔医薬組成物中の非水性担体の合計含有率(質量%)〕]・・・式(2)
油相溶解比率が70質量%以上であれば、析出が十分に抑制されていると判断した。
なお、表1~10では、油相溶解比率が、70質量%以上の場合を「良好」と表記し、70質量%未満の場合を「不良」と表記した。
表1~10中、「NT」は、試験を行わなかったことを意味する。
表1~10中、「フルベストラント」を「FLV」、「エタノール」を「EtOH」、「プロピレングリコール」を「PG」、1,3-ブチレングリコールを「1,3-BG」、「ベンジルアルコール」を「BnOH」、「安息香酸ベンジル」を「BnBz」と表記した。
表10における「中鎖脂肪酸トリグリセリド」は、Sasol社製の中鎖脂肪酸トリグリセリド(商品名:ミグリオール(登録商標)812、構成脂肪酸:カプリル酸及びカプリン酸)である。
これらの結果から、実施例1~53、101、及び102の医薬組成物は、医薬組成物の全質量に対して8質量%以上という従来(5質量%)よりも高濃度のフルベストラントを含有しながらも、体液と接触した場合にフルベストラントの析出が生じ難く、筋肉内注射に適した医薬製剤であることが明らかとなった。
一方、比較例1~66及び101の医薬組成物では、医薬組成物の全質量に対して8質量%以上という従来よりも高濃度のフルベストラントを含有すると、濁りの有無、フルベストラントの溶解性、及び析出性の評価のうち、少なくとも1つ以上の評価において、良好な結果が得られなかった。
本明細書に記載された全ての文献、特許出願、及び技術規格は、個々の文献、特許出願、及び技術規格が参照により取り込まれることが具体的に、かつ、個々に記された場合と同程度に、本明細書中に参照により取り込まれる。
Claims (12)
- フルベストラントと、水性溶媒と、医薬的に許容される非水性担体と、を含み、
フルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上であり、水性溶媒が下記(1)~(4)の要件を満たし、かつ、医薬的に許容される非水性担体が下記(A)及び(B)の要件を満たす医薬組成物。
(1)医薬組成物の全質量に対して8質量%以上のエタノールを含む。
(2)医薬組成物の全質量に対して3質量%以上の、プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコールを含む。
(3)ベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。
(4)水性溶媒の含有率が、医薬組成物の全質量に対して15質量%以上50質量%以下である。
(A)安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。
(B)医薬的に許容される非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上75質量%以下である。 - フルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上14質量%以下である請求項1に記載の医薬組成物。
- プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコールが、プロピレングリコールである請求項1又は請求項2に記載の医薬組成物。
- 医薬的に許容される非水性担体が、ヒマシ油、又は、ヒマシ油及びヒマシ油とは異なる植物油の混合物である請求項1~請求項3のいずれか1項に記載の医薬組成物。
- ヒマシ油とは異なる植物油が、ゴマ油、ラッカセイ油、ダイズ油、ツバキ油、トウモロコシ油、綿実油、オリーブ油、サフラワー油、ナタネ油、及び構成脂肪酸の平均炭素数が6以上12以下の脂肪酸トリグリセリドからなる群より選ばれる少なくとも1種の植物油である請求項4に記載の医薬組成物。
- ヒマシ油とは異なる植物油が、ゴマ油及びラッカセイ油から選ばれる少なくとも1種の植物油である請求項4に記載の医薬組成物。
- 医薬的に許容される非水性担体が、下記(C)及び(D)の要件を更に満たす請求項1~請求項6のいずれか1項に記載の医薬組成物。
(C)医薬組成物の全質量に対して40質量%以上のヒマシ油を含む。
(D)ヒマシ油とは異なる植物油の含有率が、医薬組成物の全質量に対して20質量%未満である。 - エタノールの含有率が、医薬組成物の全質量に対して10質量%以上20質量%以下である請求項1~請求項7のいずれか1項に記載の医薬組成物。
- プロピレングリコール及び1,3-ブチレングリコールから選ばれる少なくとも1種の多価アルコールの含有率が、医薬組成物の全質量に対して10質量%以上20質量%以下である請求項1~請求項8のいずれか1項に記載の医薬組成物。
- フルベストラントと、水性溶媒と、医薬的に許容される非水性担体と、を含み、
フルベストラントの含有率が、医薬組成物の全質量に対して8質量%以上であり、水性溶媒が下記(i)~(iv)の要件を満たし、かつ、医薬的に許容される非水性担体が下記(a)~(d)の要件を満たす医薬組成物。
(i)医薬組成物の全質量に対して10質量%以上20質量%以下のエタノールを含む。
(ii)医薬組成物の全質量に対して10質量%以上20質量%以下のプロピレングリコールを含む。
(iii)ベンジルアルコールの含有率が、医薬組成物の全質量に対して5質量%未満である。
(iv)水性溶媒の含有率が、医薬組成物の全質量に対して20質量%以上40質量%以下である。
(a)安息香酸ベンジルの含有率が、医薬組成物の全質量に対して1質量%未満である。
(b)医薬組成物の全質量に対して40質量%以上のヒマシ油を含む。
(c)ゴマ油及びラッカセイ油から選ばれる少なくとも1種の植物油の含有率が、医薬組成物の全質量に対して20質量%未満である。
(d)医薬的に許容される非水性担体の含有率が、医薬組成物の全質量に対して40質量%以上75質量%以下である。 - ベンジルアルコールの含有率が、医薬組成物の全質量に対して2質量%以下である請求項1~請求項10のいずれか1項に記載の医薬組成物。
- 筋肉内注射用である請求項1~請求項11のいずれか1項に記載の医薬組成物。
