Abstract
Description
従来、化膿性炎症を伴うざ瘡や表在性皮膚感染症の主な治療法としては、軽度から中等度ではナジフロキサシン等の外用抗菌剤、中等度から重度ではミノサイクリン、ロキシスロマイシン等の経口抗菌剤が繁用されている。
外用抗菌剤に関して、新たな医薬品として、1-シクロプロピル-8-メチル-7-[5-メチル-6-(メチルアミノ)-3-ピリジル]-4-オキソ-1,4-ジヒドロ-3-キノリンカルボン酸を有効成分として含有する製剤の開発も行われている(例えば、特許文献1、特許文献2を参照)。
服薬コンプライアンスを向上させるための方法として、薬剤師による服薬指導等も挙げられるが、徐放性製剤にする等、服薬回数を減らす方法も積極的に研究されている。
(1)1-シクロプロピル-8-メチル-7-[5-メチル-6-(メチルアミノ)-3-ピリジル]-4-オキソ-1,4-ジヒドロ-3-キノリンカルボン酸(以下、「本発明に係る化合物A」という)及び/又はその医薬上許容される塩を有効成分として含有し、ヒト患者に1日1回投与することにより皮膚科領域感染症に対する治療及び/又は予防効果を発揮することを特徴とする外用剤(以下、「本発明外用剤」という)。
(2)皮膚科領域感染症が、単純性皮膚軟部組織感染症、複雑性皮膚軟部組織感染症又は化膿性炎症を伴うざ瘡である、上記(1)に記載の外用剤。
(3)単純性皮膚軟部組織感染症が表在性皮膚感染症又は深在性皮膚感染症である、上記(2)に記載の外用剤。
(4)表在性皮膚感染症が、毛包炎、毛瘡、化膿性汗孔周囲炎又は伝染性膿痂疹である、上記(3)に記載の外用剤。
(5)化膿性炎症を伴うざ瘡が、尋常性ざ瘡、新生児ざ瘡又は集簇性ざ瘡である、上記(2)に記載の外用剤。
(6)剤型が軟膏剤、ゲル剤、クリーム剤、乳剤、粘着テープ剤又はローション剤である、上記(1)に記載の外用剤。
(7)剤型がローション剤である、上記(1)に記載の外用剤。
(8)低級アルコール、水溶性高分子及び多価アルコールを含有し、pHが9~12の範囲内である、上記(7)に記載の外用剤。

これらを1種又は2種以上の組み合わせで用いることができ、その配合量は本発明に係るローション剤全量に対し、0.1~5重量%の範囲内が適当であり、0.7~2重量%の範囲内が好ましい。
これらを1種又は2種以上の組み合わせで用いることができ、その配合量は本発明に係るローション剤全量に対し、1~30重量%の範囲内が適当である。
これらを1種又は2種以上の組み合わせで用いることができ、その配合量は本発明に係るローション剤全量に対し、0.01~20重量%の範囲内が適当であり、0.1~2重量%の範囲内が好ましい。
かかる安定化剤としては、特に限定されないが、例えば、アスコルビン酸、エデト酸ナトリウム、チオ硫酸ナトリウム、亜硫酸ナトリウム、ピロ亜硫酸ナトリウム、亜硝酸ナトリウム、亜硫酸水素ナトリウム、感光素201号等を挙げることができる。
また、本発明に係るローション剤は、防腐剤を配合しなくても保存効果を有するが、必要に応じて防腐剤を添加することもできる。
(1)対象患者
全顔に10個以上30個未満の炎症性皮疹を伴う患者であって、年齢が13歳以上の患者を対象患者とした。
上記の条件を満たす患者であっても、下記のいずれかに該当する患者は対象患者から除外した。
1)尋常性ざ瘡以外のざ瘡(集簇性ざ瘡、ステロイドざ瘡、壊死性ざ瘡、職業性ざ瘡等)を顔面に伴う患者
2)試験薬の評価に影響を及ぼすと考えられる合併症(湿疹、アトピー性皮膚炎、酒さ等)を顔面に有する患者
3)月経周期に伴い、明らかにざ瘡の症状に変動が認められる患者
4)治療開始前2週間以内に、顔面に局所投与医薬品(一般用医薬品を含む)を使用した患者
5)治療開始前4週間以内に、以下の全身投与医薬品(内服及び注射等全身作用を目的としたもの)を使用した患者
・ざ瘡治療薬(一般用医薬品を含む。但し、ビタミンB2及びB6は使用可とする)
・ステロイド
・抗菌剤
・その他、海外で使用されているざ瘡治療薬等、試験薬の有効性評価に影響を及ぼすと考えられる薬剤
6)治療開始前4週間以内に、顔面へ以下の療法を受けた患者
・ケミカルピーリング
・レーザー治療・光線療法
7)治療開始前2週間以内に、スクラブ入り洗顔料を使用した患者
8)キノロン系合成抗菌剤に対し、過敏症の既往のある患者
9)外用剤に対し、皮膚過敏症の既往のある患者
10)問診にて重篤な合併症が疑われる患者
11)妊娠中又は治療期間中に妊娠を希望する患者及び授乳中の患者
12)治療開始前6カ月(180日)以内に、他の試験に参加していた患者
13)その他、試験に従事する医師が不適切と判断した患者
(2)試験方法
対象患者を被験薬投与群(1日1回投与群、1日2回投与群)、比較薬投与群と対照薬投与群に分けた。その後、被験薬、比較薬又は対照薬を朝と夜に、4週間投与した。
被験薬は製剤例3で調製した製剤を用い、比較薬は1%ナジフロキサシンローション(市販品、1日2回)を用い、対照薬は化合物Aを含まないローション剤をそれぞれ用いた。
被験薬1日1回投与群は、朝は対照薬を投与し、夜は被験薬を投与した。
治療開始後、1週間毎に顔面を観察し、炎症性皮疹数を計測した。
炎症性皮疹とは、紅色丘疹(直径1cmまでの紅色の隆起性の毛孔一致性丘疹)や膿疱(紅色丘疹の頂点に黄色の膿を有するものから、全体が黄白色を呈するものまでみられ、時に圧痛を伴うもの)を伴うものを意味する。
(3)評価方法
最終評価時の炎症性皮疹数の減少率に対して、投与群別に要約統計量及び95%信頼区間を算出した。
(4)評価結果
表5に示す通り、最終評価時の炎症性皮疹数の減少率(平均±標準偏差)は、対照薬投与群と比べて、全ての群で高くなっていた。
(1)対象患者
顔面に11個以上40個以下の炎症性皮疹を伴う患者であって、年齢が13歳以上50歳未満の患者を対象患者とした。
上記の条件を満たす患者であっても、下記のいずれかに該当する患者は対象患者から除外した。
1)試験薬の評価に影響を及ぼすと考えられる合併症(尋常性ざ瘡以外のざ瘡及び酒さ等)を顔面に伴う患者
2)試験期間中に顔面に皮疹が生じる可能性のある皮膚疾患(アトピー性皮膚炎等)を合併する患者
3)月経周期に伴い有効性評価に影響を及ぼす程のざ瘡の症状の変動が認められる患者
4)治療開始日前12週間以内に、レチノイド又はレチノイド様作用を有する薬剤を内服又は顔面へ外用した患者
5)治療開始日12週間前から治療開始日4週間前までに、抗菌剤の全身投与医薬品を合計14日以上使用した患者
6)治療開始日前4週間以内に、以下の治療を受けた患者
・ざ瘡治療薬(ビタミンB2及びB6を除く)、ステロイド、抗菌剤及びその他試験薬の有効性評価に影響を及ぼすと考えられる全身投与医薬品
・顔面にケミカルピーリング、レーザー治療・光線療法、面皰吸引・圧出、その他、ざ瘡の治療を目的とした療法
7)治療開始日前2週間以内に、以下の治療・施術を受けた患者
・顔面への局所投与医薬品の使用、顔面へのエステティック施術
・顔面へのざ瘡のケアを目的とした医薬部外品及び化粧品(スクラブ入り洗顔料等)の使用
8)治療開始日前4カ月(120日)以内に、他の試験に参加していた患者
9)本試験の観察期間に組み入れられたことがある患者
10)キノロン系合成抗菌剤に対する過敏症、外用剤に対する皮膚過敏症、光線過敏症の既往がある患者
11)問診にて重篤な合併症が疑われる患者
12)妊娠中又は試験期間中に妊娠を希望する患者及び授乳中の患者
13)観察期間に併用禁止薬又は併用禁止療法を併用した患者
14)観察期間の朝又は夜いずれかの試験薬の投与遵守率が70%未満の患者
15)観察期間の炎症性皮疹数の減少率が30%以上の患者
16)その他、試験に従事する医師が不適切と判断した患者
(2)試験方法
観察期間として、対照薬を朝と夜に、2週間投与した。観察期間終了後、対象患者を被験薬投与群(1日1回投与群、1日2回投与群)と対照薬投与群に分けた。その後、治療期間として、被験薬又は対照薬を朝と夜に、12週間投与した。
被験薬は製剤例4で調製した製剤を用い、対照薬は化合物Aを含まないローション剤を用いた。
被験薬1日1回投与群は、朝は対照薬を投与し、夜は被験薬を投与した。

投与群別に要約統計量及び95%信頼区間を算出した。対照薬投与群と被験薬1日2回投与群及び被験薬1日1回投与群間の対比較を行った。盲検下レビューで、最終評価時の炎症性皮疹数の減少率に外れ値が認められたため、Separate-ranking型のSteel検定を用いた。
また、事後的に対照薬投与群と被験薬1日2回投与群及び被験薬1日1回投与群の中央値の差のホッジス・レーマン推定値とその95%信頼区間を算出した。
2)副次評価項目
治療期間中の炎症性皮疹数の継時変化を、中央値のデータから算出した。
(4)評価結果
表6に示す通り、最終評価時の炎症性皮疹数の減少率は、被験薬1日2回投与群及び被験薬1日1回投与群は対照薬投与群との間で統計学的に有意な差を認めた(Separate-ranking型のSteel検定、被験薬1日2回投与群:p=0.0003、被験薬1日1回投与群:p=0.0004)。
更に、図1に示す通り、炎症性皮疹数は、全ての投与群で2週後から12週後まで経時的に減少した。4週以降の被験薬1日2回投与群及び被験薬1日1回投与群の炎症性皮疹数は、対照薬投与群に比べて少なかった。
(1)対象患者

9)治療開始日前4カ月(120日)以内に、他の試験に参加していた患者
10)化合物Aを含有する外用剤を用いた試験に参加したことがある患者
11)本試験に参加したことがある患者
12)キノロン系合成抗菌剤に対する過敏症、外用剤に対する皮膚過敏症、光線過敏症の既往がある患者
13)本試験への参加が適切ではない重篤な合併症(全身疾患を含む)が疑われる患者
14)妊娠中又は試験期間中に妊娠を希望する患者及び授乳中の患者
15)観察期間に併用禁止薬又は併用禁止療法を併用した患者
16)観察期間の朝又は夜いずれかの投与遵守率が70%未満の患者
17)観察期間の炎症性皮疹数の減少率が30%以上の患者
18)その他、試験に従事する医師が不適切と判断した患者

投与群別に要約統計量を算出した。被験薬投与群と対照薬投与群を2標本Wilcoxon検定で対比較(有意水準両側5%)し、優越性を検証した。優越性が検証できた場合は、被験薬投与群と比較薬投与群の中央値の差に対するホッジス・レーマン推定値の95%信頼区間下限が非劣性限界値-10.1%上回っていることにより非劣性を検証した。
非劣性限界値は、試験例1の結果より、比較薬投与群と対照薬投与群の最終評価時の炎症性皮疹数の減少率(中央値、それぞれ50.00%と29.71%)の差20.29%の約1/2の10.1%と設定した。
2)副次評価項目
治療期間中の炎症性皮疹数の継時変化を、中央値のデータから算出した。
(4)評価結果
表7に示す通り、最終評価時の炎症性皮疹数の減少率(中央値)は、被験薬投与群が54.77%、比較薬投与群が53.59%、対照薬投与群が41.67%であった。
被験薬投与群と対照薬投与群を対比較した結果、有意な差が認められ(p<0.001、2標本Wilcoxon検定)、被験薬投与群の対照薬投与群に対する優越性が検証できた。
被験薬投与群と比較薬投与群の減少率の中央値の差に対するホッジス・レーマンの推定値(95%信頼区間)は0.00%(-7.14%~6.35%)で、95%信頼区間下限が非劣性限界値-10.1%を上回り、被験薬投与群の比較薬投与群に対する非劣性が検証できた。
更に、図2に示す通り、各評価時期での被験薬投与群と比較薬投与群の炎症性皮疹数の減少率は、ともに治療開始2週後以降12週後まで経時的に大きくなり、同様の推移を示した。
対照薬投与群との比較では、被験薬投与群と対照薬投与群の炎症性皮疹数の減少率の中央値の差の推定値(95%信頼区間)から、治療開始2週後以降で対照薬投与群との有意な差がみられた。
(1)対象患者
年齢が13歳以上であって、発赤(新鮮なもので暗紅色の残存紅斑を除く)、腫脹(急性炎症時のもので炎症後の線維化を除く)及び紅色丘疹・膿疱の症状が、全て中等度以上の毛包炎又は毛瘡を伴い、かつ評価部位に複数の皮疹を有する患者を対象患者とした。
上記の条件を満たす患者であっても、下記のいずれかに該当する患者は対象患者から除外した。
1)細菌学的検査(紅色丘疹又は膿疱の内容物採取)の実施が困難な患者
2)試験薬の評価に影響を及ぼすと考えられる他の皮膚疾患及び全身疾患を有する患者
3)治療開始日前4週間以内に、抗菌剤の全身投与医薬品を使用した患者
4)治療開始日前2週間以内に有効性評価部位の皮疹へ抗菌剤の局所投与医薬品を使用した患者
5)キノロン系合成抗菌剤に対する過敏症、外用剤に対する皮膚過敏症、光線過敏症の既往がある患者
6)問診にて重篤な合併症が疑われる患者
7)妊娠中及び授乳中の患者又は試験期間中に避妊する意思のない患者
8)治療開始日前4カ月(120日)以内に試験に参加していた患者
9)これまでに化合物Aを含有する外用剤の試験に参加したことのある患者
10)本試験に参加したことがある患者
11)その他、試験に従事する医師が不適切と判断した患者
(2)試験方法
治療開始日に、発赤、腫脹及び紅色丘疹・膿疱が、全て中等度以上の部位の中から評価部位を1カ所特定した。
製剤例4で調製した製剤を1日1回(夜)、7日間、全ての皮疹(評価部位以外の皮疹を含む)に清潔な指先で適量塗布した。
治療開始後、3日、7日に評価部位を観察し、皮膚所見(発赤、腫脹、紅色丘疹、膿疱)を項目毎にスコア化し、有効性を判定した。
なお、皮膚所見のスコアは表8に示す基準を、有効性の判定は表9に示す基準に従った。
(3)評価方法
最終評価時の評価部位の皮膚所見のスコアから有効率とその95%信頼区間を算出した。
(4)評価結果
表10に示す通り、有効率は70.0%であった。
Claims (14)
- 1-シクロプロピル-8-メチル-7-[5-メチル-6-(メチルアミノ)-3-ピリジル]-4-オキソ-1,4-ジヒドロ-3-キノリンカルボン酸及び/又はその医薬上許容される塩を有効成分として含有し、ヒト患者に1日1回投与することにより皮膚科領域感染症に対する治療及び/又は予防効果を発揮することを特徴とする外用剤。
- 皮膚科領域感染症が、単純性皮膚軟部組織感染症、複雑性皮膚軟部組織感染症又は化膿性炎症を伴うざ瘡である、請求項1に記載の外用剤。
- 単純性皮膚軟部組織感染症が表在性皮膚感染症又は深在性皮膚感染症である、請求項2に記載の外用剤。
- 表在性皮膚感染症が、毛包炎、毛瘡、化膿性汗孔周囲炎又は伝染性膿痂疹である、請求項3に記載の外用剤。
- 化膿性炎症を伴うざ瘡が、尋常性ざ瘡、新生児ざ瘡又は集簇性ざ瘡である、請求項2に記載の外用剤。
- 剤型が軟膏剤、ゲル剤、クリーム剤、乳剤、粘着テープ剤又はローション剤である、請求項1~5のいずれか1項に記載の外用剤。
- 剤型がローション剤である、請求項1~5のいずれか1項に記載の外用剤。
- 低級アルコール、水溶性高分子及び多価アルコールを含有し、pHが9~12の範囲内である、請求項7に記載の外用剤。
- 低級アルコールが、エタノール又はイソプロパノールである、請求項8に記載の外用剤。
- 水溶性高分子が水溶性セルロース誘導体である、請求項8に記載の外用剤。
- 水溶性セルロース誘導体がヒドロキシエチルセルロースである、請求項10に記載の外用剤。
- 多価アルコールが、1,3-ブチレングリコールである、請求項8に記載の外用剤。
- 低級アルコールがエタノールであって、水溶性高分子が水溶性セルロース誘導体であって、多価アルコールが1,3-ブチレングリコールである、請求項8に記載の外用剤。
- エタノールの含有量が1~20重量%の範囲内である、請求項13に記載の外用剤。
