また、卵性と膜性との関係としては、二卵性双胎であればほぼ確実に2絨毛膜双胎となり、一卵性双胎であれば、受精卵が2つに分かれた時期によって、2絨毛膜双胎となるか、または1絨毛膜双胎となる。二卵性双胎であって1絨毛膜双胎となる場合は極めて稀である。また、1絨毛膜双胎であればほぼ確実に一卵性双胎であり、2絨毛膜双胎であれば、一卵性双胎である場合と、二卵性双胎である場合とがある。1絨毛膜双胎であっても二卵性であることがわずかにではあるが存在する。さらに、胎児の性別が異なるときは膜性情報によらず二卵性双胎であり、胎児の性別が同じであるときは一卵性双胎または二卵性双胎のいずれかであるが、胎児の性別および膜性情報のみによって区別することはできない。
しかしながら、一卵性双胎である場合、1絨毛膜2羊膜双胎が約75%であり、2絨毛膜2羊膜双胎が約25%であり、1絨毛膜1羊膜双胎が約1%であることが知られている。そのため、超音波検査によって、1絨毛膜2羊膜双胎であると診断された場合には、1卵性双胎と推定してもよく、1絨毛膜2羊膜双胎であると診断された場合には、1卵性双胎とみなしてもよい。なお、胎児の性別が異なる場合には、膜性情報によらず、二卵性双胎として取り扱ってよい。
一卵性双胎では、2つの胎児は同一の受精卵に由来するので、2つの胎児の遺伝子型は一致し、遺伝子型の数は1つである。これに対して、二卵性双胎では、2つの胎児はそれぞれ異なる受精卵に由来するので、2つの胎児の遺伝子型は一致せず、遺伝子型の数は2つである。
母体血サンプルから単離する有核赤血球候補細胞の個数は、胎児の場合分け情報に基づいて決定する。
遺伝子型解析(本発明において「遺伝子多型解析」および「Y染色体存否確認」を総称していう。)によって検出される遺伝子型の数は、上述した膜性と卵性との関係および卵性と遺伝子型との関係から、2絨毛膜2羊膜双胎では2つ、1絨毛膜2羊膜双胎、1絨毛膜1羊膜双胎および単胎では1つと推定される。ただし、2絨毛膜2羊膜双胎であっても一卵性双胎である可能性が存在するとともに、1絨毛膜2羊膜双胎または1絨毛膜1羊膜双胎であっても二卵性双胎である可能性が少ないながらも存在し、その可能性は1絨毛膜2羊膜双胎の方が1絨毛膜1羊膜双胎よりも大きく、1絨毛膜1羊膜双胎は単胎と同じか僅かに大きい程度である。そのため、単離する有核赤血球候補細胞の個数の大小関係は、下記不等式に示すとおりである。

(母体血サンプル)
本発明において、母体血は超音波検査を行った、またはこれから行う妊婦の末梢血であり、母体血サンプルは妊婦から採血した末梢血そのもの、または妊婦から採血した末梢血に抗凝固剤を添加、生理食塩水で希釈などの処理を施したものをいう。超音波検査を行って膜性診断を行う妊婦と母体血を採血する妊婦は同一個体である。
母体血には、母体由来の好酸球、好中球、好塩基球、単核球、リンパ球等の白血球や、核のない成熟した赤血球に加えて、母体由来の有核赤血球、そして胎児由来の有核赤血球が含まれる。胎児由来の有核赤血球は、妊娠後、6週程度から母体血中に存在するといわれており、そのため本発明では、妊娠後6週程度以降に採血した妊婦の末梢血を検査する。
超音波検査の結果に基づく胎児の場合分けの結果を取得し、単離する有核赤血球候補細胞の個数を決定してから、母体血サンプルから有核赤血球候補細胞を単離するため、超音波検査による膜性診断を採血よりも後に行う場合には、少なくとも膜性診断が行われるまでは、母体血サンプルを冷蔵保存しておくことが好ましい。
母体血サンプルから有核赤血球細胞を単離するためには、まず、母体血サンプル中の有核赤血球細胞を不連続密度勾配遠心分離法によって濃縮することが望ましい。
不連続密度勾配遠心分離法は、密度が相違する第一の媒体および第二の媒体を用いて、不連続な密度勾配を形成させ、その中間の密度を有する成分を分画、濃縮する方法である。
本発明で用いる、第一の媒体および第二の媒体としては、ポリビニルピロリドンでコートされた直径15~30nmのケイ酸コロイド粒子分散液であるパーコール(登録商標、Percoll、シグマアルドリッチ社製)、ショ糖から作られた側鎖に富んだ中性の親水性ポリマーであるフィコール-パック(登録商標、Ficoll-Paque、GEヘルスケアライフサイエンス社製)、ポリスクロースとジアトリゾ酸ナトリウムによる溶液であるヒストパック(登録商標、Histopaque、シグマアルドリッチ社製)等の媒体を使用することができる。本発明では、パーコールまたはヒストパックを使用することが好ましい。パーコールは、密度(比重)1.130の製品が市販されており、希釈することで目的とする密度(比重)の媒体を調製することが可能である。ヒストパックは、市販されている密度1.077g/mLの媒体と密度1.119g/mLの媒体を用いて、所望の密度である、第一の媒体および第二の媒体を調製することが可能である。
本発明では、母体血サンプルから分離した有核赤血球細胞を含む画分を、透明基板、好ましくは、スライドガラス上に塗布を行い、分析用の標本を作製する。この標本を用いて、細胞の形態情報を用いて胎児由来の有核赤血球候補細胞を選別する。本発明では、細胞の、細胞質の面積に対する核領域の面積の割合、および核の円形度合い、核領域の面積などを利用して、胎児由来の有核赤血球候補細胞を選別することが可能であるが、細胞質の面積に対する核領域の面積の割合、または核の円形度合いを満たす条件を有する細胞を、胎児由来の有核赤血球候補細胞として選別することが好ましい。

(全ゲノム増幅)
全ゲノム増幅は、例えば単一細胞から回収した微量なDNAを106倍程度に増幅することが可能であり、その増幅産物を用いて、遺伝子変異等の遺伝子解析を容易にする増幅法である。
本発明で用いる全ゲノム増幅法としては、母体血サンプルから単離した有核赤血球候補細胞のそれぞれについて、一般的な方法である界面活性剤を用いた細胞溶解、プロテアーゼK等を用いたタンパク質分解工程を経ることで、細胞から分離することにより得られたゲノムDNAを用いる。
また、細胞からゲノムDNAを分離することなく、細胞1個をそのまま、PCR反応溶液に添加してもよい。
また、全ゲノム増幅産物の濃度について、NanoDrop(Thermo Fisher Scientific社製)、BioAnalyzer(Agilent社製)、Quantus Fluorometer(Promega社製)等のDNA濃度を測定することができる装置を用いて測定することが好ましい。
《遺伝子多型解析》
全ゲノム増幅した有核赤血球候補細胞は、形態情報に基づいて胎児由来の有核赤血球候補細胞を選別し、単離したものを全ゲノム増幅したとはいえ、胎児由来の有核赤血球細胞が含まれていない場合や、母親由来の有核赤血球細胞が混入していることが考えられる。
そのため、実施態様としては、遺伝子多型解析および/またはY染色体存否確認によって、全ゲノム増幅した有核赤血球候補細胞から胎児由来の有核赤血球細胞を同定することが好ましい。
PCR(ポリメラーゼ連鎖反応)は、DNAポリメラーゼを使用して鋳型DNAを繰り返し複製させる。増幅させるDNA塩基配列の始めと終わりの部分で鋳型DNAとハイブリダイズする短いDNAプライマーを加えてポリメラーゼにより複製を開始する。複製を繰り返すたびに鋳型2本鎖DNAの2本の鎖が解離し別々に複製される。
マルチプレックスPCRは、一般的なPCRに用いられる耐熱性DNAポリメラーゼ、反応バッファーを用いることが可能であるが、夫々のプライマー対が鋳型DNAにアニーリングする温度が異なる為、反応条件の検討が必要な場合がある。そのため、マルチプレックスPCRに最適化された耐熱性DNAポリメラーゼ、反応バッファーを用いることが好ましく、本発明では、Multiplex PCR Assay kit(タカラバイオ(株)社製)を用いて反応することがより好ましい。
マルチプレックスPCRで増幅された産物は、アガロースゲル電気泳動により増幅有無を確認することが好ましい。さらに、マルチプレックスPCR増幅産物をQIAquick PCR Purification Kit(QIAGEN社製)を用いて精製することが好ましい。また、常磁性微小ビーズを利用した精製法であるAMPure XP Kit(BECKMAN COULTER社製)を用いることも可能である。
また、マルチプレックスPCR増幅産物の濃度について、NanoDrop(Thermo Fisher Scientific社製)、BioAnalyzer(Agilent社製)、Quantus Fluorometer(Promega社製)等のDNA濃度を測定することができる装置を用いて測定することが好ましい。
増幅産物の配列解析には、Miseq(Illumina社製)を用いることが可能である。複数のマルチプレックスPCR増幅産物をMiseqを用いて測定する場合、夫々のマルチプレックスPCR増幅産物に6~8塩基で構成されるサンプル識別配列(インデックス配列)とMiseqフローセル上のオリゴヌクレオチド配列にハイブリダイゼーションさせる為のP5、P7配列を付加する必要がある。これらの配列の付加により、最大96種類のマルチプレックスPCR増幅産物を1回で測定することが可能である。
インデックス配列、P5、P7配列のマルチプレックスPCR増幅産物の両末端へ付加する方法としては、アダプターライゲーション法、PCR法を用いることが可能である。
また、複数のマルチプレックスPCR増幅産物を混合してMiseqで測定する場合には、夫々のPCR産物を正確に定量することが望ましい。定量する方法としては、BioAnalyzer(Agilent社製)、Quantus Fluorometer(Promega社製)を用いることも可能であるが、マルチプレックスPCR増幅産物を定量PCR法で測定する方法がさらに好ましい。本発明での定量方法としては、日本ジェネティクス(株)社製のKAPA Library Quantification Kitsを用いて定量することが好ましい。
母親由来の有核赤血球細胞にはY染色体は存在しないため、Y染色体の検出は胎児由来であることを示す有力な証拠である。
Y染色体の存否を確認する方法としては、例えば、Y染色体特異的塩基配列を含み、Y染色体特異的にPCR増幅することが可能なY染色体上の領域を標的としてPCR反応を行い、正しい増幅産物が生成したか否かを確認する方法が挙げられる。正しい増幅産物が生成したか否かは、例えば、アガロースゲル電気泳動で所望のサイズのバンドが得られるか否かを確認することによって、またはPCR増幅産物の塩基配列を解読し、所定の領域を増幅したものであるか否かを確認することによって、確認することができる。
標的領域として、遺伝子多型部位を含む領域を選択すれば、遺伝子多型解析にも利用することができ、双胎児の性別が両方とも男であった場合に有用である場合がある。
Y染色体の存否を確認する別の方法としては、例えば、Y染色体特異的な蛍光プローブを用いるFISH法が挙げられる。具体的には、例えば、Abbott社製CEP X/Y DNA Probe Kitを用いた方法が挙げられる。
場合分け情報から推定される胎児の遺伝子型の数は、単胎、1絨毛膜1羊膜双胎および1絨毛膜2羊膜双胎である場合については1つ、2絨毛膜2羊膜双胎である場合については2つである。
遺伝子解析(本発明において「遺伝子多型解析」および「Y染色体存否確認」を総称していう。)によって検出される有核赤血球細胞の遺伝子型の数は、0以上であり、場合分け情報から推定される胎児の遺伝子型の数+1を上限とする。
遺伝子解析によって検出された胎児由来の有核赤血球細胞の遺伝子型の数が場合分け情報から推定される胎児の遺伝子型の数と一致しない(不一致である)ときは、工程3に戻り、工程3以降の工程を繰り返す。工程7から工程3に戻る回数は、特に限定されないが、1~5回が好ましく、1~3回がより好ましい。
遺伝子解析によって検出された胎児由来の有核赤血球細胞の遺伝子型の数が場合分け情報から推定される胎児の遺伝子型の数と一致するときは、工程7に進み、胎児由来の有核赤血球細胞の染色体異数性を判別する。
なお、遺伝子解析によって検出された有核赤血球細胞(胎児由来のものと母親由来のものとを含む)の遺伝子型の数が場合分け情報から推定される胎児の遺伝子型の数+1であるときは、胎児由来の有核赤血球細胞の遺伝子型の数と胎児の遺伝子型の数とは一致する。
例えば、単胎、1絨毛膜1羊膜双胎および1絨毛膜2羊膜双胎のいずれかの場合に胎児が場合分けされたときの有核赤血球細胞について検出された遺伝子型が2種類である場合は、そのうち1種類が胎児由来の有核赤血球細胞の遺伝子型であり、2絨毛膜2羊膜双胎である場合に胎児が場合分けされたときの有核赤血球細胞について検出された遺伝子型が3種類である場合は、そのうち2種類が胎児由来の有核赤血球細胞の遺伝子型であると判断することができる。これらの場合、有核赤血球細胞の遺伝子型を母親の遺伝子型と対比しなくても、得られた2種類または3種類の遺伝子型のすべてについて染色体異数性の判別を行えば、胎児由来の有核赤血球細胞の染色体異数性を判別することができる。母親由来の有核赤血球細胞には染色体異数性がないため、染色体異数性が検出できた場合には胎児由来の有核赤血球細胞に染色体異数性が存在することとなる。
(増幅DNA断片の配列リード数の計測)
ポリメラーゼ連鎖反応により増幅され、胎児有核赤血球の細胞が同定された核酸断片に対して、あらかじめ決定された40bp以上200bp以下の領域の配列を有する増幅産物の増幅量をシークエンサーで求める。基準(あるいは参照)として、母親由来の有核赤血球の細胞と同定された細胞の、あらかじめ決定された40bp以上200bp以下の領域の配列を有する増幅産物の増幅量をシークエンサーで求める。胎児が正常な状態であれば、胎児由来の増幅産物の増幅量と、母親由来の増幅産物の増幅量とは、ほぼ、1:1の量比となると予想される。胎児が増幅した染色体由来のトリソミーである疾患を有する場合には、その比は、1:1.5(あるいは2:3)の比となると予想される。
本発明の胎児の染色体異数性の非侵襲的判別システムは、
[1´]超音波検査の結果に基づく胎児の場合分け情報を取得するための胎児情報取得手段と、胎児の場合分け情報に基づいて母体血サンプルに含まれる細胞の形態情報を取得する形態情報取得手段と、母体血サンプルに含まれる細胞を単離する細胞単離手段と、単離した細胞を全ゲノム増幅するPCR装置と、全ゲノム増幅産物をマルチプレックスPCRするマルチプレックスPCR装置と、PCR増幅産物の塩基配列を解読し、配列リード数を計測するDNAシークエンシング装置とを含む。
[2´]上記胎児情報取得手段によって取得した情報に基づいて、単離する有核赤血球候補細胞の個数が決定され、
上記形態情報取得手段によって取得した情報に基づいて、単離する有核赤血球候補細胞が選択され、
上記単離手段によって、有核赤血球候補細胞が単離され、
上記PCR装置を用いて単離された有核赤血球候補細胞の全ゲノム増幅が行われ、
上記マルチプレックスPCR装置を用いてマルチプレックスPCRが行われ、
上記DNAシークエンシング装置を用いてマルチプレックスPCRにより得られたDNAフラグメントの塩基配列解読および配列リード数の計測が行われ、遺伝子多型解析および/またはY染色体存否確認によって検出された有核赤血球候補細胞の遺伝子型の数と胎児の場合分けから推定される胎児の遺伝子型の数とを比較し、一致しているときは次工程に進み、一致していないときは有核赤血球候補細胞を単離する工程に戻り、以降の工程を繰り返す。
(超音波検査)
超音波検査装置を用いて、妊娠10週目以前の妊婦を検査し、双胎児妊娠が確認された被験者から末梢血サンプルを入手した。超音波検査装置としては、GEヘルスケア社製のVscan、東芝メディカルシステムズ社製のAplioシリーズを用いることが可能であるが、本発明では、富士フイルム社製のFC1、FAZONE CB、FAZONE Mを用いることが好ましい。
超音波検査により、被験者の膜性は、2絨毛膜2羊膜双胎であることが確認された。胎児の性別は両方男児であり、この時点では、一卵性双胎か二卵性双胎の判別は困難であった。しかしながら、2絨毛膜2羊膜双胎は、確率的には二卵性双胎の可能性が高いと判断した為、後述の有核赤血球候補細胞採取工程において、20個以上の採取を目安とした。
7mL採血管に抗凝固剤として、EDTAのナトリウム塩を10.5mg添加した後、上記の超音波検査を行った妊婦のボランティアから、インフォームドコンセントを行った後にボランティア血として末梢血7mLを採血管内に得た。その後、生理食塩水を用いて、血液を希釈した。
パーコール液(登録商標、シグマアルドリッチ社製)を使用して、密度1.070g/mLと、密度1.095g/mLの液を調製し、遠沈管に、遠沈管の底部に密度1.095g/mLの液2mLを添加し、冷蔵庫で4℃に30分冷却した。その後、冷蔵庫から遠沈管を取り出し、密度1.095g/mLの液の上に、界面が乱れないようにゆっくり、密度1.070g/mLの液2mLを重層した。その後、密度1.070g/mLの媒体の上に、上記で採血した血液の希釈液11mLをゆっくり、遠沈管に添加した。その後、遠心分離を2000rpmで20分間行った。
遠沈管を取り出し、密度1.070g/mLと、密度1.095g/mLの液の間に沈積した画分をピペットを用いて採取した。このように採取した血液の画分を、片手でスライドガラス基板(以下「スライドガラス基板1」という。)を保持し、その1端に1滴採取した血液の画分を点着した。もう一方の手で別のスライドガラス基板(以下「スライドガラス基板2」という。)を持ち、1端をスライドガラス基板1に30°の角度で接触させ、スライドガラス基板2の接触下面を血液の画分に触れることで、毛管現象により2枚のスライドガラス基板に囲まれた空間に血液が広がった。次に角度を保ったまま、スライドガラス基板2をスライドガラス基板1の血液を置いた領域と反対の領域の方向に滑らせて、スライドガラス基板1上に血液を均一に塗布した。塗布終了後、送風で1時間以上十分に乾燥させた。このスライドガラス基板をメイ・ギュルンワルド染色液に3分間浸漬し、リン酸緩衝液に浸漬して洗浄後、ギムザ染色液(リン酸緩衝液で希釈して濃度3%とした。)に10分間浸漬した。その後、純水で洗浄後乾燥させることにより、染色済みのスライドガラス基板を複数枚作製した。
スライドガラス基板上に塗布した細胞から、胎児由来の有核赤血球候補細胞を選別するため、電動XYステージと、対物レンズ、CCDカメラを備えた光学顕微の測定系と、XYステージ制御部、Z方向制御部とを備えた制御部と、画像入力部と画像処理部、およびXY位置記録部とを備えた制御ユニット部を準備した。
上記のように準備したスライドガラス基板上に塗布した血液細胞をXYステージに乗せて、スライドガラス上に焦点を合わせてスキャンし、光学顕微鏡より得られた画像を取り込み、画像解析により標的細胞である有核赤血球を探索した。

スライドガラス基板上に存在する細胞から、式(I)および(II)を満たすものを20個、胎児由来の有核赤血球候補細胞として選択した。
上記のように選択された胎児由来の有核赤血球候補細胞20個を、マイクロマニュピュレータを使用して回収した。
マイクロマニュピュレータを用いて単離した20個の細胞それぞれについて、New England Biolabs社製PicoPLEX WGA kitを用いて、説明書記載に則り細胞中の微量なDNAを約100万倍に増幅し、全ゲノム増幅を行った。
得られた全ゲノム増幅産物は、QIAquick PCR Purification Kit(QIAGEN社製)を用いて不純物を除去し、Quantus Fluorometer dsDNA System(Promega社製)を用いて増幅産物の濃度を測定した。
胎児細胞の分析、染色体の数的異常を分析する目的で、表1および表2に示す23箇所の染色体位置からマルチプレックスPCRに用いるプライマーを作製した。各検出領域のPCR増幅塩基長としては、100~220塩基対となるようにプライマーを作製、また、各検出領域内は遺伝的多型を含有するようにプライマーの位置を設計した。設計した46種類のプライマーは、各プライマーの終濃度が25nmol/Lとなるように混合した。
反応溶液は、鋳型として各細胞から得られた増幅産物10ngと、46種類混合プライマー8μLと、Multiplex PCR Mix1 0.125μLと、Multiplex PCR Mix2 12.5μLと、水とで最終液量25μLに調製した。
PCR条件は、94℃ 60秒で変性した後、94℃ 30秒、60℃ 90秒、72℃ 30秒を30サイクルとして反応を行った。
得られたPCR産物は、QIAquick PCR Purification Kit(QIAGEN社製)を用いて精製した。
プライマーとしては、表3に示すD501-F(配列番号47)、D701-R(配列番号48)、D702-R(配列番号49)、D703-R(配列番号50)、D704-R(配列番号51)、D705-R(配列番号52)およびD706-R(配列番号53)を各1.25μM用いて、Multiplex PCR Assay kitを用いてPCRをおこなった。
PCR条件としては、94℃ 3分で変性した後、94℃ 45秒、50℃ 60秒、72℃ 30秒を5サイクルで反応した後、94℃ 45秒、55℃ 60秒、72℃ 30秒を11サイクルで反応をおこなった。
得られたPCR産物は、AMPure XP Kit(BECKMAN COULTER社製)を用いて精製し、BioAnalyzerを用いて濃度を測定した。
さらに正確な増幅産物の定量として、日本ジェネティクス(株)社製のKAPA Library Quantification Kitsを用いて定量を行った。
23種類の混合マルチプレックスPCR増幅産物を、Miseq Reagent Kit v2 300 Cycle(ILLUMINA社製)を用いてシークエンスを行い、得られたFastQファイルをBWAを用いてヒトゲノム配列(hg19)へマッピングを行い、SAMtoolsにより遺伝子多型情報を抽出、BEDtoolsにより各検出領域のシークエンスリード数を算出することで解析を行った。
各細胞から増幅した全ゲノム増幅産物を用いて、ILLUMINA社製ゲノムアナライザーMiseqを用いて、13番、18番、21番染色体の表1および表2に示す遺伝子領域のSNPsを比較することで、20個の細胞のうち、3個の細胞が異なるSNP情報を有することが確認できた。また、さらに2個の細胞が異なるSNP情報を有することが確認できた。別途、核の形状より白血球と予想される細胞をマイクロマニュピュレータを用いて、血液を塗布したスライド上から回収し、20個の細胞と同様にしてDNAを増幅し、SNPを調べたところ、15個の細胞のSNPと一致することが確認された。以上より、5個の細胞が胎児由来の有核赤血球細胞であり、その上3個と2個で違うSNP情報が取得されたことから二卵性双胎と確定し、15個の細胞が、母親由来の有核細胞であることが確認された。
マルチプレックスPCR増幅産物の配列解析で判別した胎児由来と同定された有核赤血球の21番染色体の検出領域の増幅産物の量をMiseqを用いて、確定させた。別途、母親由来と同定された有核細胞のうちの1個の細胞の21番染色体の検出領域の増幅産物の量を、ILLUMINA社製ゲノムアナライザーMiseqを用いて、確定させた。この2つの量比を計算したところ、1:1に近い値であり、双胎児は正常染色体数であることが推定された。
実施例1で採血した妊婦とは別の妊婦から、超音波検査により被験者の膜性は、2絨毛膜2羊膜双胎であることが確認された。胎児の性別は男児、女児であり、この時点で二卵性双胎であることが確認された。その性別結果に基づき、有核赤血球の遺伝型としては、母体由来、胎児男児由来、胎児女児由来の3種類の遺伝型を持つ有核赤血球を取得する必要があるため、有核赤血球候補細胞の単離としては20個以上の採取を目安とした。
実施例1と同様にして、胎児有核赤血球の選別工程において20個の有核赤血球候補細胞を回収し、DNA増幅工程まで同様におこなった。この後のマルチプレックスPCR工程で、実施例1と同様にして、SNPsの確認により胎児由来の有核赤血球が4個存在することを確認した。また、胎児が男児、女児であることから性染色体有無の確認により胎児由来の有核赤血球を判別した結果、男児由来が2個、女児由来が2個であることを確認した。
胎児の染色体数的異常としては、双胎児は正常染色体数であることが推定された。
実施例1、2で採血した妊婦とはさらに別の妊婦から末梢血を採取し、超音波検査により被験者の膜性は、2絨毛膜2羊膜双胎であることが確認された。胎児の性別は男児、女児であり、この時点で二卵性双胎であることが確認された。その性別結果に基づき、有核赤血球の遺伝型としては、母体由来、胎児男児由来、胎児女児由来の3種類の遺伝型を持つ有核赤血球を取得する必要があるが、本発明の有効性を確認する為、有核赤血球の単離としては10個の採取を実施した。
実施例2と同様にして、胎児有核赤血球の選別工程において10個の細胞を回収し、DNA増幅工程まで同様におこなった。この後のマルチプレックスPCR工程で、実施例2と同様にして、SNPsを確認したところ胎児由来の有核赤血球を4個の存在を確認した。しかし、胎児が男児由来の有核赤血球のみしか取得することができず、女児由来の胎児有核赤血球を取得することができていないことが明らかとなった。
Claims (5)
- 母体血中から単離した有核赤血球候補細胞のDNAを解析する工程を有する胎児の染色体異数性の非侵襲的判別方法であって、
前記胎児が、超音波検査の結果に基づいて、単胎、1絨毛膜1羊膜双胎、1絨毛膜2羊膜双胎および2絨毛膜2羊膜双胎から選択されるいずれか1つの場合に場合分けがされた胎児であり、かつ、前記DNAを解析する有核赤血球候補細胞の個数を前記場合分けに基づいて最適化する、胎児の染色体異数性の非侵襲的判別方法。 - 前記最適化することが、前記DNAを解析する有核赤血球候補細胞の個数を、
単胎である場合の個数≦1絨毛膜1羊膜双胎である場合の個数<1絨毛膜2羊膜双胎である場合の個数<2絨毛膜2羊膜双胎である場合の個数
と設定することを含む、請求項1に記載の胎児の染色体異数性の非侵襲的判別方法。 - 前記母体血中から単離した有核赤血球候補細胞のDNAを解析する工程が以下の工程を含む、請求項1または2に記載の胎児の染色体異数性の非侵襲的判別方法:
〈工程1〉下記a)~d)に示す、超音波検査によって得られた膜性情報に基づく胎児の場合分け情報を取得する工程
a)単胎
b)1絨毛膜1羊膜双胎
c)1絨毛膜2羊膜双胎
d)2絨毛膜2羊膜双胎
〈工程2〉母体血サンプルから単離する有核赤血球候補細胞の個数を前記場合分け情報に基づいて決定する工程、
〈工程3〉母体血サンプルから前記個数の有核赤血球候補細胞を単離する工程、
〈工程4〉単離した有核赤血球候補細胞を、それぞれ、全ゲノム増幅する工程、
〈工程5〉全ゲノム増幅産物を用いて、遺伝子多型解析および/またはY染色体存否確認を行う工程、
〈工程6〉前記遺伝子多型解析および/または前記Y染色体存否確認によって検出された胎児由来の有核赤血球細胞の遺伝子型の数と前記胎児の場合分け情報から推定される胎児の遺伝子型の数とを比較し、一致しているときは次工程に進み、一致していないときは工程3に戻ることを選択する工程、ならびに
〈工程7〉前記胎児由来の有核赤血球細胞の染色体異数性を判別する工程。 - 超音波検査の結果に基づく胎児の場合分け情報を取得するための胎児情報取得手段と、胎児の場合分け情報に基づいて母体血サンプルに含まれる細胞の形態情報を取得する形態情報取得手段と、母体血サンプルに含まれる細胞を単離する細胞単離手段と、単離した細胞を全ゲノム増幅するPCR装置と、全ゲノム増幅産物をマルチプレックスPCRするマルチプレックスPCR装置と、PCR増幅産物の塩基配列を解読し、配列リード数を計測するDNAシークエンシング装置とを含む、胎児の染色体異数性の非侵襲的判別システム。
- 前記胎児情報取得手段によって取得した情報に基づいて、単離する有核赤血球候補細胞の個数が決定され、
前記形態情報取得手段によって取得した情報に基づいて、単離する有核赤血球候補細胞が選択され、
前記単離手段によって、有核赤血球候補細胞が単離され、
前記PCR装置を用いて単離された有核赤血球候補細胞の全ゲノム増幅が行われ、
前記マルチプレックスPCR装置を用いてマルチプレックスPCRが行われ、
前記DNAシークエンシング装置を用いてマルチプレックスPCRにより得られたDNAフラグメントの塩基配列解読および配列リード数の計測が行われ、遺伝子多型解析および/またはY染色体存否確認によって検出された有核赤血球候補細胞の遺伝子型の数と胎児の場合分けから推定される胎児の遺伝子型の数とを比較し、一致しているときは次工程に進み、一致していないときは有核赤血球候補細胞を単離する工程に戻り、以降の工程を繰り返す、
請求項4に記載の胎児の染色体異数性の非侵襲的判別システム。
