Abstract
Description
本願は、2014年09月05日に、日本に出願された特願2014-181012号に基づき優先権を主張し、その内容をここに援用する。
挿入部は、可撓性を有さない硬質の筒状の部材である。挿入部の内部には、2つのチャンネルを有する。挿入部の先端には、観察部(観察装置)が取付けられている。
先端部は、体腔内の組織に対して各種手技を行うためのもので、第一アーム(アーム部)及び第二アーム(アーム部)と、第一アームの先端側と第二アームの先端側とを離間させ、手技を行いやすい位置関係にするためのリンク部とを備えている。すなわち、両アーム及び観察部は、挿入部の先端に取付けられている。
そこで、アーム部を、挿入部の先端よりも基端側の挿入部の側面に取付け、チャンネルから突出する処置具の処置部の位置を観察部に近づける検討が行われている。しかし、挿入部の側面から突出するアーム部の向きによっては、観察部で処置具の処置部が観察しにくくなる。具体的には、挿入部の側方(挿入部の長手方向に直交する方向)にアーム部が突出していると、アーム部は観察部よりも基端側に配置されることになる。この場合には、チャンネルから突出する処置具を観察部で観察するのは難しい。
本発明の第1の態様の内視鏡システムは、体内に挿入可能な挿入部と、前記挿入部に設けられ、前方に規定される視野範囲内を観察可能な観察部と、湾曲操作可能であって基端部が前記挿入部に設けられ、処置具を挿通可能なチャンネルの先端側が開口するアーム部と、前記チャンネルの前記開口における中心軸線が前記観察部の前記視野範囲を通らない視野範囲外状態を検出する第一の検知部と、前記チャンネルに前記処置具が挿通されたことを検出する第二の検知部と、前記第二の検知部により前記チャンネルに前記処置具が挿通されたことを検出したときに、前記第一の検知部が前記視野範囲外状態を検出すると、使用者に警告する警告部と、を備えることを特徴としている。
本発明の第2の態様に係る内視鏡システムによれば、上記第1の態様において、前記第二の検知部により前記チャンネルに前記処置具が挿通されたことを検出したときに、前記第一の検知部が前記視野範囲外状態を検出すると、前記チャンネルの少なくとも一部を変形させて前記処置具を挿通不能にする封鎖部をさらに備えてもよい。
本発明の第4の態様に係る内視鏡システムによれば、上記第1の態様において、前記警告部は、警告を表示する、あるいは、音を発生してもよい。
以下、本発明に係る内視鏡システムの第1実施形態を、図1から図5を参照しながら説明する。
図1及び2に示すように、本実施形態の内視鏡システム1は処置具200とともに用いられる。以下では、まず処置具200について説明する。
図示はしないが、処置具挿入部201の管路には処置具操作ワイヤが処置具挿入部201に対して進退可能に挿通されている。処置具操作ワイヤの先端部は、図示しないリンク機構を介して一対の把持片202aに接続されている。リンク機構は、一対の把持片202aの先端部を互いに離間させたり接近させたりする開閉操作を可能にする。
一対の把持片202aやリンク機構は、ステンレス鋼やチタンなどの金属で形成されている。
このように構成された処置具200は、操作部本体203aに対してスライダ203bを長手方向にスライドさせると、処置具挿入部201に対して処置具操作ワイヤが進退する。これにより、リンク機構が動作して一対の把持片202aを開閉操作させることができる。
以下では、後述する内視鏡操作部40に対する挿入部10側を先端側、挿入部10に対する内視鏡操作部40側を基端側とそれぞれ称する。
先端硬質部11は、術者等の使用者が工具を用いずに撓ませることができない程度の比較的硬い材料で形成されている。先端硬質部11は、例えばステンレス鋼やチタン等の金属で形成することができる。
先端硬質部11の先端面11aには、前述の観察ユニット20と、前方に照明光を照射する照明ユニット21が設けられている。
観察ユニット20は、レンズ群やCMOS(Complementary Metal Oxide Semiconductor)等の撮像素子20aを有している。外部からレンズ群に入射した光は、レンズ群により撮像素子20a上の受光面で結像される。撮像素子20aは、受光面で結像した像の画像を取得する。撮像素子20aは、取得した画像を信号に変換し、制御部65に送信する。観察ユニット20の撮像素子20aは、先端硬質部11の先端面11aの前方に規定される視野範囲R1内を観察可能である。視野範囲R1は、例えば四角錐形である。
複数の節輪のうちの最も先端側の節輪には、内視鏡操作ワイヤ14、15(図2参照)の先端部が取付けられている。内視鏡操作ワイヤ14、15は、軸線C1周りに等角度ごとに4本取付けられている。各内視鏡操作ワイヤ14、15は、複数の節輪及び可撓管部13を通して内視鏡操作部40まで延びている。可撓管部13に対して内視鏡操作ワイヤ14、15を挿入部10の長手方向に進退させることで、内視鏡湾曲部12を挿入部10の軸線C1に対して所望の方向に湾曲させることができる。
可撓管部13は、軟性の樹脂やコイルで形成されている。
アーム硬質部31、33は、先端硬質部11と同一の材料で形成することができる。
アーム湾曲部32は、内視鏡湾曲部12と同様に構成されている。アーム湾曲部32内に配置された図示しない節輪には、2本のアーム操作ワイヤ34の先端部が取付けられている。アーム操作ワイヤ34は、複数の節輪、アーム硬質部33、及び挿入部10を通して内視鏡操作部40まで延びている。アーム硬質部33の基端部は先端硬質部11の側面に取付けられている。
アーム硬質部33に対してアーム操作ワイヤ34をアーム部30の長手方向に進退させることで、アーム湾曲部32をアーム部30の長手方向に直交する方向Xに湾曲させることができる。方向Xは、アーム硬質部31が視野範囲R1に接近したり視野範囲R1から離間したりする方向である。
アーム湾曲部32は真っ直ぐな形状であるときに、アーム部30も全体として真っ直ぐな形状になる。
内視鏡湾曲部駆動モータ42の回転軸42aには、回転軸42aと同軸にプーリ46が取付けられている。プーリ46には、2本の内視鏡操作ワイヤ14の基端部がそれぞれ取付けられている。
アーム湾曲部駆動モータ44の回転軸44aには、回転軸44aと同軸にプーリ50が取付けられている。プーリ50には、2本のアーム操作ワイヤ34の基端部がそれぞれ取付けられている。アーム湾曲部駆動モータ44では、回転軸44aにエンコーダ51が接続されている。エンコーダ51で検出された回転軸44aの回転量、及び回転軸44aが回転する向きは、制御部65に送信される。
一方で、回転軸44aが基準位置から回転軸44a周りの第二の側に回転すると、アーム湾曲部32は方向Xのうち視野範囲R1から離間する第二の側X2に湾曲する。
一方で、図3に示すように、チャンネル35の中心軸線C2が観察ユニット20の視野範囲R1を通らない視野範囲外状態であるときには、チャンネル35に挿通された処置具200は視野範囲R1を通らない。このため、チャンネル35に処置具200を挿通しても、チャンネル35の先端から突出した処置具200を観察ユニット20で観察することはできない。
チャンネル35の基端部には、図1及び2に示すフォトインタラプタ(第二の検知部)55が取付けられている。フォトインタラプタ55は、図示はしないが発光素子及び受光素子を内蔵する公知の構成の装置である。発光素子及び受光素子は、チャンネル35を形成するチャンネルチューブを挟んで対向するように配置されている。通常時には、発光素子が発する検出光を受光素子が受けることができる。チャンネル35に挿通された処置具200の把持部202や処置具挿入部201が、発光素子が発する検出光を遮ると、受光素子が検出光を受けられなくなる。このようにして、フォトインタラプタ55は、チャンネル35に処置具200が挿通されたことを検出する。フォトインタラプタ55は、検出結果を制御部65に送信する。
アングルノブ56を操作することで、内視鏡湾曲部駆動モータ42、43が駆動して、回転軸42a、43aが回転する。これにより、内視鏡操作ワイヤ14、15が挿入部10の長手方向に進退し、内視鏡湾曲部12が湾曲する。
ボタン57を操作することで、後述する光源85から光を照射させることができる。
バス66には、挿入部10の撮像素子20a、内視鏡操作部40の内視鏡湾曲部駆動モータ42、43、アーム湾曲部駆動モータ44、エンコーダ51、フォトインタラプタ55、アングルノブ56、ボタン57、表示部80、及び光源85が接続されている。
第一の検知部72を構成する視野範囲外状態演算部67のメモリーには、アーム湾曲部駆動モータ44の回転軸44aが基準位置から回転した向き及び回転量と、そのときのアーム湾曲部32が前述の視野範囲内状態及び視野範囲外状態のいずれの状態かを表すテーブル(表)が記憶されている。
視野範囲外状態演算部67の演算素子は、エンコーダ51から送信される回転軸44aの回転量、及び回転軸44aが回転する向き、及びメモリーに記憶されたテーブルに基づいて、アーム湾曲部32が視野範囲内状態及び視野範囲外状態のいずれの状態かを検出する。視野範囲外状態演算部67は、検出結果を警告演算部68に送信する。
主制御部69は、撮像素子20aから送信された信号が表示部80で適切に表示されるように各種処理を行う。また、アングルノブ56やボタン57による指示に基づいて、光源85等を制御する。
視野範囲外状態演算部67、警告演算部68、及び主制御部69は、下部に車輪74aが取付けられたラック74に配置されている。
補助操作部70では、操作部本体70a上に湾曲操作ボタン70b、70cが設けられている。
補助操作部70の湾曲操作ボタン70b、70cを操作することで、アーム湾曲部駆動モータ44が駆動して回転軸44aが回転する。これにより、アーム操作ワイヤ34がアーム部30の長手方向に進退し、アーム湾曲部32が第一の側X1、あるいは、第二の側X2に湾曲する。
表示部80は、例えばラック74の上部に取付けられている。
光源85は、所定の電力を供給されることで、挿入部10の光ファイバに光を照射する。
術者等の使用者は、例えば図示しないオーバーチューブに内視鏡システム1の挿入部10を挿通させ、アーム部30をオーバーチューブで覆う。内視鏡システム1を起動し、ボタン57を操作して、光源85から光を照射させる。光源85から照射された光は光ファイバに案内され、挿入部10の先端面11aの先端側を照明する。
観察ユニット20が取得した視野範囲R1内の画像は、制御部65の主制御部69で処理され、液晶パネル81の画像表示領域R6に表示される。
図3に示すように、挿入部10の先端が処置対象部位P1に対向する位置まで挿入部10を挿入する。挿入部10に対してオーバーチューブを基端側に移動させて、オーバーチューブの先端から挿入部10の先端硬質部11やアーム部30を先端側に突出させる。このとき、アーム部30が全体として真っ直ぐな形状になっている場合について説明する。
使用者は、内視鏡システム1のチャンネル35に処置具200を把持部202側から挿通させる。フォトインタラプタ55はチャンネル35に処置具200が挿通されたことを検出し、検出結果を制御部65の警告演算部68に送信する。
警告部73の警告演算部68は、フォトインタラプタ55によりチャンネル35に処置具200が挿通されたことを検出したときに、視野範囲外状態演算部67が視野範囲外状態を検出したことで、図4に示すように表示部80の液晶パネル81の情報表示領域R7に文字による表示(警告)R8を表示することで使用者に警告する。
情報表示領域R7への表示は、チャンネル35の先端側の開口35aから処置具200が突出する前に行うことが好ましい。このために、フォトインタラプタ55はチャンネル35のできるだけ基端側に取付けられていることが望ましい。
警告演算部68は、液晶パネル81の情報表示領域R7の表示R8を止める。
使用者は再びチャンネル35に処置具200を挿通させ、チャンネル35の先端から把持部202を突出させる。チャンネル35の先端から突出した処置具200の把持部202は観察ユニット20で観察され、図5に示すように表示部80の画像表示領域R6に把持部202が表示される。
処置具200の処置具操作部203を操作して一対の把持片202aを開閉操作させ、処置対象部位P1に適切な処置を行う。
警告部73は、液晶パネル81の情報表示領域R7に表示R8を表示することで警告する。これにより、使用者に警告の内容を確実に視認させることができる。
警告部が、警告部73の表示部80に代えてスピーカー等の発音部を備えてもよい。この場合、発音部から音を発生することで使用者に警告する。
次に、本発明の第2実施形態について図6及び図7を参照しながら説明するが、前記実施形態と同一の部位には同一の符号を付してその説明は省略し、異なる点についてのみ説明する。
図6に示すように、本実施形態の内視鏡システム2は、第1実施形態の内視鏡システム1の各構成に加えて、内視鏡操作部40の操作部本体41に封鎖部90が内蔵されている。封鎖部90は、有底筒状のソレノイド91内に棒状のプランジャ(鉄芯)92が挿通されて構成されている。
図示はしないが、ソレノイド91に内蔵されるコイルの両端部は電力供給源に接続されている。電力供給源は、バス66に接続されている。
コイルに電力を供給していないときには、プランジャ92はソレノイド91側に引き戻されている。このとき、チャンネル35を形成するチャンネルチューブ35bは潰れていなく、チャンネル35の内径はチャンネル35の長手方向の位置によらずほぼ一定である。プランジャ92の先端部(ソレノイド91とは反対側の端部)は、チャンネルチューブ35bの外面に接触している。
すなわち、主制御部69は電力供給源によりソレノイド91のコイルに電力を供給させる。コイルが発生する磁界により、図7に示すようにプランジャ92がチャンネルチューブ35b側に押込まれ、チャンネル35が変形し、プランジャ92で押込まれた部分のチャンネル35が潰れる。チャンネル35が潰れることでチャンネル35の内部が塞がり、チャンネル35に処置具200が挿通することができなくなる。
なお、電力供給源による電力の供給を止めると、プランジャ92はソレノイド91側に引き戻される。チャンネルチューブ35bの弾性力により、プランジャ92で内径が小さくなっていたチャンネルチューブ35bが元の形状に戻りチャンネル35の内径が大きくなる。
さらに、封鎖部90を備えることで、チャンネル35に処置具200が挿通されるのを確実に防止することができる。
なお、本実施形態では、チャンネル35に処置具200を挿通させる前に予め封鎖部90でチャンネル35を潰しておいてもよい。この場合、第一の検知部72が視野範囲内状態を検出したときに封鎖部90はチャンネル35を潰すのをやめて元の内径に戻す。
また、封鎖部90として、ソレノイド91とプランジャ92とを備えた構成にしたが、封鎖部は、チャンネル35の少なくとも一部を変形させて処置具200を挿通不能にする構成であればこれに限らない。
次に、本発明の第3実施形態について図8から図12を参照しながら説明するが、前記実施形態と同一の部位には同一の符号を付してその説明は省略し、異なる点についてのみ説明する。
図8及び9に示すように、本実施形態の内視鏡システム3は、第1実施形態の内視鏡システム1の観察ユニット20、補助操作部70に代えて観察ユニット100、視野移動演算部130、及び補助操作部131を備えている。
観察ユニット100は、先端側に設けられた硬性の硬質部101と、硬質部101の基端側に設けられ湾曲操作可能な湾曲部102と、湾曲部102の基端側に設けられた硬性の硬質部103とを有している。
湾曲部102は、アーム湾曲部32と同様に構成されている。湾曲部102内に配置された図示しない節輪には、2本の操作ワイヤ110の先端部、及び2本の操作ワイヤ111の先端部が取付けられている(操作ワイヤ111は図9のみに示す)。操作ワイヤ110、111は、複数の節輪、硬質部103、及び挿入部10を通して内視鏡操作部40まで延びている。
硬質部103の基端部は先端硬質部11の側面に取付けられている。
アーム湾曲部駆動モータ115、湾曲部駆動モータ116、117は、アーム湾曲部駆動モータ44と同様に構成されている。すなわち、アーム湾曲部駆動モータ115の回転軸115aには、回転軸115aと同軸にプーリ120が取付けられている。プーリ120には、2本のアーム操作ワイヤ36の基端部がそれぞれ取付けられている。
湾曲部駆動モータ117の回転軸117aには、回転軸117aと同軸にプーリ124が取付けられている。プーリ124には、2本の操作ワイヤ111の基端部がそれぞれ取付けられている。
湾曲部102が真っ直ぐな形状であるときに、湾曲部駆動モータ116の回転軸116aは基準位置にある。
なお、アーム湾曲部32を方向Yに湾曲させたときにチャンネル35の開口35aにおける中心軸線C2が観察ユニット100の視野範囲R2を通りにくくならないように、アーム湾曲部32が方向Yに湾曲する範囲を制限してもよい。
湾曲部駆動モータ116を駆動することで、湾曲部102は観察ユニット100の長手方向に直交する方向Eに湾曲する。観察ユニット100の湾曲部102が方向Eに湾曲したときに硬質部101が移動する基準面と、アーム部30のアーム湾曲部32が方向Xに湾曲したときにアーム硬質部31が移動する基準面とは平行である。
一方で、湾曲部駆動モータ117を駆動することで、湾曲部102は観察ユニット100の長手方向及び方向Eにそれぞれ直交する方向Fに湾曲する。
視野移動演算部130は、演算素子、メモリー、および制御プログラム等で構成されている。
視野移動演算部130のメモリーには、アーム湾曲部駆動モータ44の回転軸44aが基準位置から回転した向き及び回転量と、そのときのアーム湾曲部32の湾曲形状に対するアーム部30の中心軸線C2を観察ユニット100の視野範囲R2が通り視野範囲内状態となるための湾曲部駆動モータ116の回転軸116aが基準位置から回転させる向き及び回転量の範囲とを表すテーブルが記憶されている。
本実施形態では、観察ユニット100の湾曲部102を方向Fに湾曲させても視野範囲内状態と視野範囲外状態とが切替わらない。このため、エンコーダは湾曲部102を方向Fに湾曲させるための湾曲部駆動モータ117の回転軸117aに取付けられず、湾曲部102を方向Eに湾曲させるための湾曲部駆動モータ116の回転軸116aに取付けられている。
湾曲操作ボタン135a、135bを操作すると、アーム湾曲部駆動モータ44が駆動してアーム湾曲部32が方向Xに湾曲する。湾曲操作ボタン135c、135dを操作すると、アーム湾曲部駆動モータ115が駆動してアーム湾曲部32が方向Yに湾曲する。
湾曲操作ボタン136a、136bを操作すると、湾曲部駆動モータ116が駆動して湾曲部102が方向Eに湾曲する。そして、湾曲操作ボタン136c、136dを操作すると、湾曲部駆動モータ117が駆動して湾曲部102が方向Fに湾曲する。
本実施形態においても、フォトインタラプタ55によりチャンネル35に処置具200が挿通されたことを検出したときに、第一の検知部72が視野範囲外状態を検出すると、警告部73が使用者に警告する。
さらに、視野移動部132を備えることで、アーム部30のアーム湾曲部32を湾曲させることなく、観察ユニット100を移動させることで視野範囲内状態にすることができる。
観察ユニット140は先端硬質部11の凹部11b内に配置され、観察ユニット140の基端部は、先端硬質部11にピン145で回転可能に支持されている。観察ユニット140の基端部には2本の操作ワイヤ110の先端部が取付けられている。湾曲部駆動モータ116を駆動することで、観察ユニット140をピン145周りに回転させることができる。
例えば、前記第1実施形態から第3実施形態では、アーム部30の基端部は先端硬質部11の側面に取付けられている構成とした。しかし、アーム部30の基端部は先端硬質部11の先端面11aに取付けられているとしてもよい。観察ユニットは視野範囲が比較的狭い場合には視野範囲外状態である場合が多くなるので、本実施形態の内視鏡システムを好適に用いることができる。
内視鏡システムが内視鏡湾曲部駆動モータ42、43を備えず、内視鏡操作ワイヤ14、15がアングルノブ56に直接接続されているように構成してもよい。この場合でも、アングルノブ56を操作することで、内視鏡操作ワイヤ14、15が進退して内視鏡湾曲部12が湾曲する。
内視鏡システムがオーバーチューブと内視鏡とを備えていて、挿入部10が、オーバーチューブと、オーバーチューブに挿通される内視鏡の内視鏡挿入部とで構成されてもよい。この場合、アーム部はオーバーチューブに取付けられることになる。

Claims (4)
- 体内に挿入可能な挿入部と、
前記挿入部に設けられ、前方に規定される視野範囲内を観察可能な観察部と、
湾曲操作可能であって基端部が前記挿入部に設けられ、処置具を挿通可能なチャンネルの先端側が開口するアーム部と、
前記チャンネルの前記開口における中心軸線が前記観察部の前記視野範囲を通らない視野範囲外状態を検出する第一の検知部と、
前記チャンネルに前記処置具が挿通されたことを検出する第二の検知部と、
前記第二の検知部により前記チャンネルに前記処置具が挿通されたことを検出したときに、前記第一の検知部が前記視野範囲外状態を検出すると、使用者に警告する警告部と、
を備える内視鏡システム。 - 前記第二の検知部により前記チャンネルに前記処置具が挿通されたことを検出したときに、前記第一の検知部が前記視野範囲外状態を検出すると、前記チャンネルの少なくとも一部を変形させて前記処置具を挿通不能にする封鎖部をさらに備える
請求項1に記載の内視鏡システム。 - 前記第二の検知部により前記チャンネルに前記処置具が挿通されたことを検出したときに、前記第一の検知部が前記視野範囲外状態を検出すると、
前記視野範囲を前記チャンネルの前記開口における中心軸線が通るように前記観察部を移動させる視野移動部をさらに備える
請求項1に記載の内視鏡システム。 - 前記警告部は、警告を表示する、あるいは、音を発生する
