Measurement of creatinine, albumin, calcium and inorganic phosphorus in various samples and eGFR were calculated. Abstract
Description
[1] 生体試料中のFGF-23および25(OH)Dを測定することを特徴とする、腎不全の予後判定方法。
[2] FGF-23の測定が、免疫学的測定方法により行われる[1]記載の方法。
[3] 25(OH)Dの測定が、免疫学的測定方法により行われる[1]または[2]記載の方法。
[5] FGF-23測定試薬が、FGF-23に結合する抗体を含む試薬である[4]記載のキット。
[6] 25(OH)D測定試薬が、25(OH)Dに結合する抗体を含む試薬である[4]または[5]記載のキット。
本発明の腎不全の予後判定方法は、生体試料中のFGF-23および25(OH)Dを測定することを特徴とする方法である。本発明の腎不全の予後判定方法は、例えば以下の工程を含む方法である。
[1]生体試料中のFGF-23を測定する工程;
[2]工程[1]で得られた測定値を、予め作成した、FGF-23濃度と測定値との間の関係を示す検量線に照らし合わせて、生体試料中のFGF-23濃度を決定する工程;
[3]生体試料中の25(OH)Dを測定する工程;
[4]工程[3]で得られた測定値を、予め作成した、25(OH)D濃度と測定値との間の関係を示す検量線に照らし合わせて、生体試料中の25(OH)D濃度を決定する工程;
[5]工程[2]で決定されたFGF-23濃度と工程[4]で決定された25(OH)D濃度とから、FGF-23濃度が、測定に供される全ての生体試料に対するFGF-23濃度を基に決定した中央値以上であり、かつ、25(OH)D濃度が、測定に供される全ての生体試料に対する25(OH)D濃度を基に決定した中央値未満である場合、腎不全の予後不良と判定し、FGF-23濃度がFGF-23中央値未満であり、かつ、25(OH)D濃度が25(OH)D中央値以上である場合、腎不全の予後良好と判定する工程。
[1]生体試料中のFGF-23を、担体上に固定化され、かつ、FGF-23に結合する第1抗体もしくはそのフラグメント、および、FGF-23に結合する第2抗体もしくはそのフラグメントに標識が結合した標識化第2抗体と反応させて、担体上に第1抗体、FGF-23、および、標識化第2抗体からなる免疫複合体を形成させる工程;
[2]形成した免疫複合体中の標識を測定する工程;および、
[3]工程[2]で得られた測定値を、予め作成したFGF-23濃度と測定値との間の関係を示す検量線に照らし合わせることにより、生体試料中のFGF-23濃度を決定する工程。
標識は、抗FGF-23抗体に結合し、FGF-23の測定を可能とする標識であれば特に制限はなく、例えばペルオキシダーゼ、アルカリホスファターゼ、β-ガラクトシダーゼ、蛍光物質、発光物質等が挙げられる。
イムノアッセイ法としては、例えば前述の測定法等が挙げられる。
[1]生体試料中の25(OH)Dを、担体上に固定化され、かつ、25(OH)Dに結合する第1抗体もしくはそのフラグメント、および、25(OH)Dに結合する第2抗体もしくはそのフラグメントに標識が結合した標識化第2抗体と反応させて、担体上に第1抗体、25(OH)D、および、標識化第2抗体からなる免疫複合体を形成させる工程;
[2]形成した免疫複合体中の標識を測定する工程;および、
[3]工程[2]で得られた測定値を、予め作成した25(OH)D濃度と測定値との間の関係を示す検量線に照らし合わせることにより、生体試料中の25(OH)D濃度を決定する工程。
上記工程[1]と工程[2]との間に洗浄工程を挿入することもできる。
本発明の腎不全の予後判定用キットは、本発明の腎不全の予後判定方法に使用されるキットであり、FGF-23測定試薬および25(OH)D測定試薬を含む。本発明のキットには、さらに、生体試料の希釈液、反応緩衝液、洗浄液、標識体の検出用試薬等が含まれていてもよい。また本発明のキットに、測定に適した機器を組み合わせて、キットとしてもよい。
FGF-23測定試薬としては、生体試料中のFGF-23を測定し得る試薬であれば特に制限はなく、例えば(1)担体に固定化され、かつ、FGF-23に結合する第1抗体、および、FGF-23に結合する第2抗体に標識が結合した標識化第2抗体を含む試薬、(2)担体に固定化され、かつ、FGF-23に結合する抗体、および、FGF-23に競合する競合物質に標識が結合した標識化競合物質を含む試薬、(3)担体に固定化され、かつ、FGF-23に競合する競合物質、および、FGF-23に結合する抗体に標識が結合した標識化抗体を含む試薬等が挙げられる。さらに、FGF-23測定試薬には、必要に応じて、生体試料の希釈液、反応緩衝液、洗浄液、標識検出用試薬、FGF-23の標準物質等が含まれていてもよい。
25(OH)D測定試薬としては、生体試料中の25(OH)Dを測定し得る試薬であれば特に制限はなく、例えば(1)担体に固定化され、かつ、25(OH)Dに結合する第1抗体、および、25(OH)Dに結合する第2抗体に標識が結合した標識化第2抗体を含む試薬、(2)担体に固定化され、かつ、25(OH)Dに結合する抗体、および、25(OH)Dに競合する競合物質に標識が結合した標識化競合物質を含む試薬、(3)担体に固定化され、かつ、25(OH)Dに競合する競合物質、および、25(OH)Dに結合する抗体に標識が結合した標識化抗体を含む試薬等が挙げられる。さらに、25(OH)D測定試薬には、必要に応じて、生体試料の希釈液、反応緩衝液、洗浄液、標識検出用試薬、25(OH)Dの標準物質等が含まれていてもよい。
慢性腎臓病を有する大阪ビタミンDスタディーに登録されている患者から保存期の慢性腎臓病患者738名を選定し、本検討におけるインフォームドコンセントを得た。大阪大学医学部附属病院の倫理委員会にて本検討が承認された。2005年5月から2007年7月まで患者登録し2010年7月まで経過観察を実施した。
全血、血清または血漿からなる血液サンプルおよび尿サンプルは患者登録時点で採取し、30分以内に血液サンプルは遠心分離し、得られた血清は測定まで-80℃で凍結保存した。各種サンプルにおけるクレアチニン、アルブミン、カルシウム及び無機リンの測定およびeGFRを算出した。eGFRはイヌリンクレアランスに基づいた日本における標準法に従い算定した。
患者基本情報ならびに各種測定値を用い腎イベント発症に関するリスクについて多変量解析を実施した。その結果を図1に示す。図1は各種危険因子とハザード比との関係を示す図であり、各種危険因子に係る3つの数値は、左からハザード比、95%信頼区域の下限、95%信頼区域の上限を意味する。ここで、ハザード比とは、尿蛋白がない群[Proteinuria (-)]を対照群とした場合の、特定の危険因子の群の腎イベント発生が何倍高いかを示す値である。また、95%信頼区域とは、95%の確率で母平均が含まれる範囲である。
25(OH)D [25D]濃度に関して、10 ng/mL未満、10 ng/mL以上20 ng/mL未満、20 ng/mL以上30 ng/mL未満、30 ng/mL以上の4群に分けた。またFGF-23 [FGF23]濃度に関しても、31.7~80.5 pg/mLの濃度範囲を基に、FGF-23が低濃度から高濃度の順にQ1~Q5の5群に分けそれぞれハザード比(HR)および95%信頼区域(95%C.I.)を検討した。その結果を図2に示す。図2は各種危険因子とハザード比との関係を示す図であり、各種危険因子に係る括弧内の3つの数値は、左からハザード比、95%信頼区域の下限、95%信頼区域の上限を意味する。
全対象における25(OH)D濃度の中央値が23.0 ng/mL、FGF-23濃度の中央値が49.4 pg/mLであったため25(OH)D濃度に関して23.0 ng/mL未満を低値群、23.0 ng/mL以上を高値群とした。またFGF-23濃度に関して49.4 pg/mL未満を低値群、49.4 pg/mL以上を高値群とし、それぞれの組み合わせで25(OH)D濃度低値かつFGF-23濃度低値群(df群)、25(OH)D濃度高値かつFGF-23濃度低値群(Df群)、25(OH)D濃度低値かつFGF-23濃度高値群(dF群)、25(OH)D濃度高値かつFGF-23濃度高値群(DF群)の4群に分類し、約5年間における腎イベント(血清クレアチニン倍増もしくは透析導入)のカプランマイヤー生存曲線を作成した。ここでのカプランマイヤー生存曲線とは、4群のそれぞれの群について、腎イベントを発症せずに生存する確率を日数との関係で示したものである。
前項の25(OH)D [25D]とFGF-23 [FGF23]の組み合わせ4群における腎イベントのハザード比および95%信頼区域を検討した。その結果を図5に示す。

- 生体試料中の線維芽細胞増殖因子-23および25水酸化ビタミンDを測定することを特徴とする、腎不全の予後判定方法。
- 線維芽細胞増殖因子-23の測定が、免疫学的測定方法により行われる請求項1記載の方法。
- 25水酸化ビタミンDの測定が、免疫学的測定方法により行われる請求項1または2記載の方法。
- 線維芽細胞増殖因子-23測定試薬および25水酸化ビタミンD測定試薬を含むことを特徴とする、腎不全の予後判定用キット。
- 線維芽細胞増殖因子-23測定試薬が、線維芽細胞増殖因子-23に結合する抗体を含む試薬である請求項4記載のキット。
- 25水酸化ビタミンD測定試薬が、25水酸化ビタミンDに結合する抗体を含む試薬である請求項4または5記載のキット。
